Airblasts,as one common phenomenon accompanied by rapid movements of landslides or rock/snow avalanches,commonly result in catastrophic damages and are attracting more and more scientific attention.To quantitatively a...Airblasts,as one common phenomenon accompanied by rapid movements of landslides or rock/snow avalanches,commonly result in catastrophic damages and are attracting more and more scientific attention.To quantitatively analyze the intensity of airblast initiated by landslides,the Wangjiayan landslide,occurred in the Wenchuan earthquake,is selected here with the landslide propagation and airblast evolution being studied using FLUENT by introducing the Voellmy rheological law.The results reveal that:(1)For the Wangjiayan landslide,its whole travelling duration is only 12 s with its maximum velocity reaching 36 m/s at t=10 s;(2)corresponding to the landslide propagation,the maximum velocity,28 m/s,of the airblast initiated by the landslide also appears at t=10 s with its maximum pressure reaching594.8 Pa,which is equivalent to violent storm;(3)under the attack of airblast,the load suffered by buildings in the airblast zone increases to 1300 Pa at t=9.4 s and sharply decreased to-7000 Pa as the rapid decrease of the velocity of the sliding mass at t=10 s,which is seriously unfavorable for buildings and might be the key reason for the destructive collapse of buildings in the airblast zone of the Wangjiayan landslide.展开更多

The microstructures and corrosion behaviors of as-cast,T4-treated,and T6-treated Mg-6Gd-3Y-0.5Zr alloys were systematically investigated by SEM,TEM,immersion test,and electrochemical corrosion test.The results show th...The microstructures and corrosion behaviors of as-cast,T4-treated,and T6-treated Mg-6Gd-3Y-0.5Zr alloys were systematically investigated by SEM,TEM,immersion test,and electrochemical corrosion test.The results show that the microstructure of the as-cast alloy is composed ofα-Mg and Mg_(24)(Gd,Y)_(5) eutectic phase,and in T4-treated alloy,Mg_(24)(Gd,Y)_(5) phase dissolves into theα-Mg matrix,leading to an increase in the(Y,Gd)H_(2) phase.After T6 treatment,nanoscale Mg_(24)(Gd,Y)_(5) phase dispersedly precipitates from theα-Mg matrix,and exhibits a specific orientation relationship with the α-Mg:(332)Mg_((24)(Gd,Y)_(5))//(1011)_(α-Mg),[136]Mg_((24)(Gd,Y)_(5))//[1210]_(α-Mg).The corrosion resistance of the Mg-6Gd-3Y-0.5Zr alloys can be ranked in the following order:T6-treated alloy exhibits the highest corrosion resistance,followed by the T4-treated alloy,and finally,the as-cast alloy.The corrosion products of the alloys are all composed of MgO,Mg(OH)_(2),Gd_(2)O_(3),Y_(2)O_(3),and MgCl_(2).The corrosion behavior of Mg-6Gd-3Y-0.5Zr alloy is closely related to the precipitated phase.By establishing the relationship between corrosion rate,hydrogen evolution rate,and corrosion potential,it is further demonstrated that during the micro galvanic corrosion process,the coarse Mg_(24)(Gd,Y)_(5)phase in the as-cast alloy undergoes extensive dissolution,and(Y,Gd)H_(2) phase promotes the dissolution of theα-Mg matrix in the T4-treated alloy,intensifying the hydrogen evolution reaction.The T6-treated alloy,with dispersive precipitation of nanoscale Mg_(24)(Gd,Y)_(5) phase,exhibits better corrosion resistance performance.展开更多
BACKGROUND Transarterial chemoembolization(TACE)combined with microwave ablation(MWA)is an effective treatment strategy for patients with advanced gastric cancer and liver metastasis.However,it may cause severe postop...BACKGROUND Transarterial chemoembolization(TACE)combined with microwave ablation(MWA)is an effective treatment strategy for patients with advanced gastric cancer and liver metastasis.However,it may cause severe postoperative pain and inflammatory responses.The paravertebral block(PVB)is a regional anesthetic technique that provides analgesia to the thoracic and abdominal regions.AIM To evaluate the effect of PVB on postoperative analgesia and inflammatory response in patients undergoing TACE combined with MWA for advanced gastric cancer and liver metastasis.METHODS Sixty patients were randomly divided into PVB and control groups.The PVB group received ultrasound-guided PVB with 0.375%ropivacaine preoperatively,whereas the control group received intravenous analgesia with sufentanil.The primary outcome was the visual analog scale(VAS)score for pain at 6 h,12 h,24 h,and 48 h after the procedure.Secondary outcomes were the dose of sufentanil used,incidence of adverse events,and levels of inflammatory markers(white blood cell count,neutrophil percentage,C-reactive protein,and procalcitonin)before and after the procedure.RESULTS The PVB group had significantly lower VAS scores at 6 h,12 h,24 h,and 48 h after the procedure compared with the control group(P<0.05).The PVB group also had a significantly lower consumption of sufentanil and a lower incidence of nausea,vomiting,and respiratory depression than did the control group(P<0.05).Compared with the control group,the PVB group had significantly lower levels of inflammatory markers 24 h and 48 h after the procedure(P<0.05).CONCLUSION PVB can effectively reduce postoperative pain and inflammatory responses and improve postoperative comfort and recovery in patients with advanced gastric cancer and liver metastasis treated with TACE combined with MWA.展开更多
Background:To investigate the effects of acupuncture on post-stroke limb spasm model rats and the underlying mechanism.Methods:A total of 50 Sprague-Dawley rats were randomly divided into three groups,Control group(10...Background:To investigate the effects of acupuncture on post-stroke limb spasm model rats and the underlying mechanism.Methods:A total of 50 Sprague-Dawley rats were randomly divided into three groups,Control group(10),Model group(20)and Zhishen Tiaoxing(ZSTX)acupuncture group(20).Middle cerebral artery occlusion was conducted in SD rats to establish post-stroke limb spasm rats,which were treated with ZSTX acupuncture.Behavioral assays were determined by the Narrow ally test,the limb muscle tension was detected by the BL-420S test system,and infarct volume was assessed after the cerebral infarction by 2,3,5-triphenyltetrazolium chloride staining.Heterogeneous neurotransmitterγ-aminobutyric acid(GABA)and its receptors GABAA and GABAB in the cerebral cortex of the infarct area were determined by immunofluorescence assay.The release of Trkb and K-Cl cotransporter isoform 2 was detected by an immunofluorescence double labeling study.Western Blot was utilized to measure the expression of BDNF and Trkb.Results:The results showed that the behavioral assays in post-stroke limb spasm rats were significantly improved by the treatment of ZSTX acupuncture.14 days of ZSTX acupuncture can effectively inhibit muscle tone and decrease Infarct volume,which was measured with BL-420S biological function experiment system and triphenyltetrazolium chloride.Meanwhile,the results of Double-Label Immunofluorescence Assays showed that ZSTX acupuncture improved the expression of GABA,GABAA,GABAB,BDNF,and K-Cl cotransporter isoform 2.Double-Label Immunofluorescence Assays and WB results showed that 14 days ZSTX acupuncture declined the expression of Trkb.Conclusions:Our results suggest that 14 days of ZSTX acupuncture can significantly improve post-stroke limb spasm.Meanwhile,the pathogenesis of post-stroke limb spasm and the efficacy of ZSTX acupuncture involve metabolic pathways of neurotransmitters,and electro-acupuncture can treat post-stroke limb spasm by regulating BDNF/Trkb-KCC2 signaling pathway.展开更多
BACKGROUND Patients with atrial fibrillation(AF)and prior stroke history have a high risk of cardiovascular events despite anticoagulation therapy.It is unclear whether catheter ablation(CA)has further benefits in the...BACKGROUND Patients with atrial fibrillation(AF)and prior stroke history have a high risk of cardiovascular events despite anticoagulation therapy.It is unclear whether catheter ablation(CA)has further benefits in these patients.METHODS AF patients with a previous history of stroke or systemic embolism(SE)from the prospective Chinese Atrial Fibrillation Registry study between August 2011 and December 2020 were included in the analysis.Patients were matched in a 1:1 ratio to CA or medical treatment(MT)based on propensity score.The primary outcome was a composite of all-cause death or ischemic stroke(IS)/SE.RESULTS During a total of 4.1±2.3 years of follow-up,the primary outcome occurred in 111 patients in the CA group(3.3 per 100 person-years)and in 229 patients in the MT group(5.7 per 100 person-years).The CA group had a lower risk of the primary outcome compared to the MT group[hazard ratio(HR)=0.59,95%CI:0.47–0.74,P<0.001].There was a significant decreasing risk of all-cause mortality(HR=0.43,95%CI:0.31–0.61,P<0.001),IS/SE(HR=0.73,95%CI:0.54–0.97,P=0.033),cardiovascular mortality(HR=0.32,95%CI:0.19–0.54,P<0.001)and AF recurrence(HR=0.33,95%CI:0.30–0.37,P<0.001)in the CA group compared to that in the MT group.Sensitivity analysis generated consistent results when adjusting for time-dependent usage of anticoagulants.CONCLUSIONS In AF patients with a prior stroke history,CA was associated with a lower combined risk of all-cause death or IS/SE.Further clinical trials are warranted to confirm the benefits of CA in these patients.展开更多

METHODS: Seventy patients with ad...AIM:To evaluate the effects and safety of combination chemotherapy with oxaliplatin (L-OHP) and S-1 (SOX regimen) in older patients with advanced gastric cardiac adenocarcinoma (GCA). METHODS: Seventy patients with advanced GCA were classified according to age into an older group (≥ 75 years) and a control group (< 75 years). The SOX regimen was administered to the two groups as follows: S-1 (40 mg/m2 po bid) on days 1 to 14 followed by a 7-d off period, plus L-OHP (65 mg/m2 iv) for 2 h on days 1 and 8 of a 21-d cycle. This regimen was repeat-ed for four to six cycles. Response and swallow statuses were evaluated after two cycles (6 wk). Effects and toxicity were evaluated four weeks after chemotherapy was completed. RESULTS: The response rate was 65.6% (21/32) in the older group and 68.4% (26/38) in the control group (χ2 = 0.062 and P = 0.804). Improvement in swallowing was 78.1% (25/32) in the older group and 76.3% (29/38) in the control group (χ2 = 0.032 and P = 0.857). Effi cacy was 68.8% (22/32) in the older group and 65.8% (25/38) in the control group (χ2 = 0.069 and P = 0.793). Toxicities were reversible and similar in both groups (P > 0.05). CONCLUSION: The SOX regimen is an effective, safe and well-tolerated regimen for older patients with advanced GCA.展开更多

METHODS A total of 335 subjects who underwent checkups in the Digestive and Liver Disease Department of our hospital were randomly selected. RT-PCR was used to detect the level of miR-106b in serum samples. Laboratory and imaging examinations were carried out to confirm the HCC diagnosis in patients who had a > 2-fold change in miR-106b levels. Ultrasound-guided biopsy was also used for HCC diagnosis when necessary. On this basis, the clinical data of these subjects, including history of hepatitis virus infection, obesity, long-term history of alcohol use and stage of HCC, were collected. Then, the impact of these factors on the level of miR1-06b in serum was analyzed. Furthermore, receiver operating characteristic (ROC) curve was drawn to evaluate the diagnostic efficacy of miR-106b for HCC. RESULTS A total of 35 subjects had abnormal serum miR-106b levels, of which 20 subjects were diagnosed with HCC. t-test revealed that the difference in serum miR-106b level in terms of sex, age, history of hepatitis virus infection, obesity and long-term history of alcohol use was not statistically significant. However, serum miR-106b levels in patients with advanced HCC (stage. /.) was higher than in patients with early HCC (stage./.), and the difference was statistically significant (P = 0.000). Moreover, the ROC curve revealed that the area under the curve value for miR-106b was 0.885, which shows that serum miR-106b level has a certain clinical value for HCC diagnosis. CONCLUSION The random sampling survey shows that serum miR-106b level is a valuable diagnostic marker for HCC. However, the diagnostic threshold value needs to be further researched.展开更多
BACKGROUND There are few effective tools to predict survival in patients with invasive intraductal papillary mucinous neoplasms of the pancreas.AIM To develop comprehensive nomograms to individually estimate the survi...BACKGROUND There are few effective tools to predict survival in patients with invasive intraductal papillary mucinous neoplasms of the pancreas.AIM To develop comprehensive nomograms to individually estimate the survival outcome of patients with invasive intraductal papillary mucinous neoplasms of the pancreas.METHODS Data of 1219 patients with invasive intraductal papillary mucinous neoplasms after resection were extracted from the Surveillance,Epidemiology,and End Results database,and randomly divided into the training(n=853)and the validation(n=366)cohorts.Based on the Cox regression model,nomograms were constructed to predict overall survival and cancer-specific survival for an individual patient.The performance of the nomograms was measured according to discrimination,calibration,and clinical utility.Moreover,we compared the predictive accuracy of the nomograms with that of the traditional staging system.RESULTS In the training cohort,age,marital status,histological type,T stage,N stage,M stage,and chemotherapy were selected to construct nomograms.Compared with the American Joint Committee on Cancer 7th staging system,the nomograms were generally more discriminative.The nomograms passed the calibration steps by showing high consistency between actual probability and nomogram prediction.Categorial net classification improvements and integrated discrimination improvements suggested that the predictive accuracy of the nomograms exceeded that of the American Joint Committee on Cancer staging system.With respect to decision curve analyses,the nomograms exhibited more preferable net benefit gains than the staging system across a wide range of threshold probabilities.CONCLUSION The nomograms show improved predictive accuracy,discrimination capability,and clinical utility,which can be used as reliable tools for risk classification and treatment recommendations.展开更多
BACKGROUND Lumbar facet joint syndrome(LFJS)is a pain condition arising from lumbar facet joint diseases.Treatments of LFJS includes patient education,oral medication,bed rest,physical therapy,and procedural intervent...BACKGROUND Lumbar facet joint syndrome(LFJS)is a pain condition arising from lumbar facet joint diseases.Treatments of LFJS includes patient education,oral medication,bed rest,physical therapy,and procedural interventions.For some refractory cases that fail conservative therapies,dorsal ramus medial brunch radiofrequency ablation is warranted.However,as nerve fibers can regenerate,their efficacy is impermanent,and the recurrence rate is relatively high.Considering synovial impingement is a paramount pathogenesis of LFJS,in this case,we removed the culprit hyperplastic articular capsule and the articular process partially through a spinal endoscope.As the culprit hyperplastic joint capsule was excised,it is supposed to generate more prolonged efficacy and a lower recurrence rate than radiofrequency treatment.CASE SUMMARY A 40-year-old female patient was diagnosed with LFJS.She complained of low back pain and right buttock pain for half a year.The patient was placed in the prone position.After disinfection and draping,a 25-cm 18-gauge needle was inserted into the dorsal surface of the right L5 articular process.Subsequently,a guidewire,dilating tubes,and a working cannula was inserted successively.The spinal endoscope was positioned in the working cannula.Under the endoscope,the microvascular tissue,muscle tissue attached on the L5 inferior articular process and S1 superior articular process,as well as the capsule and minor portion of the inferior articular process were removed.After the joint space was clear and no bleeding points existed,the endoscope and working cannula were shifted,and the incision was sutured.After treatment,the symptoms were completely relieved.The patient was pain-free during the follow-up period of 6 mo.CONCLUSION The endoscopic partial joint capsule and articular process excision is an effective procedure for LFJS,especially for cases caused by synovial impingement.展开更多
In this work,a new(Y,Gd)H_(2) precipitate was identified and systematically investigated in the as-cast Mg-6Gd-3Y-0.5Zr alloy by XRD,SEM with EDS,TEM with EDS techniques and thermodynamics analysis.Results show that t...In this work,a new(Y,Gd)H_(2) precipitate was identified and systematically investigated in the as-cast Mg-6Gd-3Y-0.5Zr alloy by XRD,SEM with EDS,TEM with EDS techniques and thermodynamics analysis.Results show that the as-cast alloy contains α-Mg,Mg_(24)(Gd,Y)_(5),and(Y,Gd)H_(2) phase.The(Y,Gd)H_(2) phase usually forms near the eutectic phase Mg_(24)(Gd,Y)_(5) or in the α-Mg grains,displaying a rectangle-shape.The Mg_(24)(Gd,Y)_(5) and(Y,Gd)H_(2) phases crystalize in bcc and fcc structure,respectively,and the(Y,Gd)H_(2) phase has a semi-coherent relationship with α-Mg matrix.The thermodynamics calculation results reveal that the hydrogen dissolved in the melt leads to the formation of hydrides.It is also found that the(Y,Gd)H_(2) hydride can form directly from the liquid phase during solidification.Additionally,it can precipitate by the decomposition of Mg_(24)(Gd,Y)_(5) phase due to absorbing hydrogen from the remaining melt.展开更多

The reaction of chlorine atom Cl(2P)(Cl(2P3/2)and Cl^*(2P1/2))with D2 was investigated at collision energy from 4.5 kcal/mol to 6.5 kcal/mol with a high-resolution crossed molecular beam apparatus using the technique ...The reaction of chlorine atom Cl(2P)(Cl(2P3/2)and Cl^*(2P1/2))with D2 was investigated at collision energy from 4.5 kcal/mol to 6.5 kcal/mol with a high-resolution crossed molecular beam apparatus using the technique of D-atom Rydberg tagging detection.The contribution from the spin-orbit excited reaction Cl^*(2P1/2)+D2,which is prohibited by Born-Oppenheimer(BO)approximation,was observed.Collision-energy dependence of differential cross sections(DCSs)near the backward scattering direction was measured.The BOforbidden reaction Cl^*+D2 was found to be dominant at lower collision energy.As collision energy increases,reactivity of BO-allowed reaction Cl+D2 increases much faster than that of BO-forbidden reaction and becomes dominant at higher collision energy.Our experiment indicates that the additional energy of spin-orbit excitation in Cl^*facilitates BO-forbidden reaction to pass through the barrier at lower collision energy,while BO approximation is still valid at collision energy near and above the reaction barrier.This tendency of reactivity of Cl/Cl^*+D2 is similar to the isotopic reaction of Cl/Cl^*+H2.展开更多

Dark photons have been well motivated as strong candidates for dark force carriers and light dark matter in the sub-GeV mass range.Compared with collider experiments,fixed-target experiments provide a complementary ap...Dark photons have been well motivated as strong candidates for dark force carriers and light dark matter in the sub-GeV mass range.Compared with collider experiments,fixed-target experiments provide a complementary approach to searching for dark photons,particularly in the lower mass range.We have studied the physics potential of the electron-on-target experiment based on the Shanghai SHINE facility,which provides 10 MHz single electron beam at 8 GeV energy.This analysis focuses on dark photons being produced via electron and nucleon interaction and then decays to dark matter candidates,which escape detection as missing momentum in the detector.This experiment takes advantage of using missing momentum to enhance signal versus background separation power.In this study,signal samples as a function of dark photon mass and an inclusive background sample with 2.5 billion events are simulated with GEANT4.For better background estimates,major rare background processes have also been simulated.This paper presents the experiment and detector design,signal and background simulations,analysis strategy,and the prospective study of the experiment sensitivity.With 9×10^(14) electron-on-target events(about three years running),this experiment is expected to rule out most of the sensitive regions predicted by popular dark photon models.展开更多

The present study aimed to explore the efficacy, feasibility, and acceptability of a cognitive training program (targeting child's multiple EF components and involving parent support in daily life), as a nonpharmacological intervention for children with ADHD. Methods: Forty-four school-age children with ADHD and their parents participated in 12 sessions of EF training (last for 12 weeks) and 88 health controls (HC) were also recruited. Training effects were explored using both neuropsychological tests (Stroop color-word test, Rey-Osterrieth complex figure test, trail making test, tower of Hanoi, and false-belief task) and reports of daily life (ADHD rating scale-IV, Conners' parent rating scale, and behavior rating inventory of executive function [BRIEF]) by analysis of paired sample t-test and Wilcoxon signed-rank test. The differences on EF performances between children with ADHD after training and HC were explored using multivariate analysis. Results: The results (before vs. after EF training) showed that after intervention, the children with ADHD presented better performances of EF both in neuropsychological tests (word interference of Stroop: 36.1 ± 14.6 vs. 27.1 ± 11.1, t = 4.731, P 〈 0.001 ; shift time of TMT: 194.9 ± 115.4 vs. 124.8 ± 72.4, Z = -4.639, P 〈 0.001 ; false-belief task: χ^2 = 6.932, P = 0.008) and reports of daily life (global executive composite of BRIEF: 148.9 ± 17.5 vs. 127.8 ± 17.5, t =6.433, P 〈 0.001). The performances on EF tasks for children with ADHD after EF training could match with the level of HC children. The ADHD symptoms (ADHD rating scale total score: 32.4 ± 8.9 vs. 22.9 ± 8.2, t = 6.331, P 〈 0.001) and behavioral problems of the children as reported by parents also reduced significantly after the intervention. Participants reported that the EF training program was feasible to administer and acceptable. Conclusions: The EF training program was feasible and acceptable to children with ADH D and parents. Although replication with a larger sample and an active control group are needed, EF training program with multiple EF focus and parent involving in real-life activities could be a potentially promising intervention associated with significant EF (near transfer) and ADHD symptoms improvement (far transfer).展开更多

However, little is known about the cognitive function of children with comorbid ADHD and LD. The present study aimed to explore the cognitive function of children and adolescents with ADH D and learning difficulties in comparison with children with ADHD and healthy controls in different age groups in a large Chinese sample. Methods: Totally, 1043 participants with ADHD and learning difficulties (the ADHD + learning difficulties group), 870 with pure ADHD (the pure ADHD group), and 496 healthy controls were recruited. To investigate the difference in cognitive impairment using a developmental approach, all participants were divided into three age groups (6-8, 9-11, and 12-14 years old). Measurements were the Chinese-Wechsler Intelligence Scale for Children, the Stroop Color-Word Test, the Trail-Making Test, and the Behavior Rating Inventory of Executive Function-Parents (BRIEF). Multivariate analysis of variance was used. Results: The results showed that after controlling for the effect of ADHD symptoms, the A DHD + learning difficulties group was still significantly worse than the pureADHD group, which was, in turn, worse than the control group on full intelligence quotient (98.66 ± 13.87 vs. 105.17 ± 14.36 vs. 112.93 ± 13.87, P 〈 0.001 ). The same relationship was also evident for shift function (shifting time of the Trail-Making Test, 122.50 [62.00, 194.25] s vs. 122.00 [73.00, 201.50] s vs. 66.00 [45.00, 108.00] s, P 〈 0.001) and everyday life executive function (BRIEF total score, 145.71 ± 19.35 vs. 138.96± 18.00 vs. 122.71 ± 20.45, P 〈 0.001 ) after controlling for the effect of the severity of ADHD symptoms, intelligence quotient, age, and gender. As for the age groups, the differences among groups became nonsignificant in the 12-14 years old group for inhibition (meaning interference of the Stroop Color-Word Test, 18.00 [13.00, 25.00] s vs. 17.00 [15.00, 26.00] s vs. 17.00 [10.50, 20.00] s, P =0.704) and shift function (shifting time of the Trail-Making Test, 62.00 [43.00, 97.00] s vs. 53.00 [38.00, 81.00] s vs. 101.00 [88.00, 114.00] s, P = 0.778). Conclusions: Children and adolescents with ADHD and learning difficulties have more severe cognitive impairment than pure ADHD patients even after controlling for the effect of ADHD symptoms. However, the differences in impairment in inhibition and shift function are no longer significant when these individuals were 12-14 years old.展开更多

This randomized controlled clinical trial was designed to evaluate the effects of a comprehensive executive skill training program for school-aged children with ADHD in a relatively large sample. Methods: The children (aged 6-12 years) with ADHD were randomized to the intervention or waitlist groups. A healthy control group was composed of gender- and age-matched healthy children. The intervention group received a 12-session training program lbr multiple executive skills. Executive function (EF), ADHD symptoms, and social functioning in the intervention and waitlist groups were evaluated at baseline and the end of the final training session. The healthy controls (HCs) were only assessed once at baseline. Repeated measures analyses of variance were used to compare EF, ADHD symptoms, and social function between intervention and waitlist groups. Results: Thirty-eight children with ADH D in intervention group, 30 in waitlist group, and 23 healthy children in healthy control group were included in final analysis. At posttreatment, intervention group showed significantly lower Behavior Rating Inventory of Executive Function (BRIEF) total score (135.89 ±16.80 vs. 146.09 ± 23.92, P = 0.04) and monitoring score (18.05 ± 2.67 vs. 19.77 ± 3.10, P = 0.02), ADHD-IV overall score (41.11 ± 7.48 vs. 47.20 ± 8.47, P 〈 0.01 ), hyperactivity-impulsivity (HI) subscale score ( 18.92 ± 5.09 vs. 21.93 ± 4.93, P = 0.02), and inattentive subscale score (22.18 ±3.56 vs. 25.27± 5.06, P 〈 0.01 ), compared with the waitlist group. Repeated measures analyses of variance revealed significant interactions between time and group on the BRIEF inhibition subscale (F = 5.06, P 0.03), working memory (F- 4.48, P = 0.04), A DH D-IV overall score (F = 21.72, P 〈 0.01), HI subscale score (F = 19.08, P 〈 0.01), and inattentive subscale score (F- 12,40, P 〈 0.01 ). Multiple-way analysis of variance showed significant differences on all variables of BRIEF, ADHD-rating scale-IV, and WEISS Functional Impairment Scale-Parent form (WFIRS-P) among the intervention and waitlist groups at posttreatment and HCs at baseline. Conclusions: This randomized controlled study on executive skill training in a relatively large sample provided some evidences that the training could improve EF deficits, reduce problematic symptoms, and potentially enhance the social thnctioning in school-aged children with ADHD.展开更多

文摘Airblasts,as one common phenomenon accompanied by rapid movements of landslides or rock/snow avalanches,commonly result in catastrophic damages and are attracting more and more scientific attention.To quantitatively analyze the intensity of airblast initiated by landslides,the Wangjiayan landslide,occurred in the Wenchuan earthquake,is selected here with the landslide propagation and airblast evolution being studied using FLUENT by introducing the Voellmy rheological law.The results reveal that:(1)For the Wangjiayan landslide,its whole travelling duration is only 12 s with its maximum velocity reaching 36 m/s at t=10 s;(2)corresponding to the landslide propagation,the maximum velocity,28 m/s,of the airblast initiated by the landslide also appears at t=10 s with its maximum pressure reaching594.8 Pa,which is equivalent to violent storm;(3)under the attack of airblast,the load suffered by buildings in the airblast zone increases to 1300 Pa at t=9.4 s and sharply decreased to-7000 Pa as the rapid decrease of the velocity of the sliding mass at t=10 s,which is seriously unfavorable for buildings and might be the key reason for the destructive collapse of buildings in the airblast zone of the Wangjiayan landslide.

文摘The sensitivity of the dark photon search through invisible decay final states in low-background experiments relies sig-nificantly on the neutron and muon veto efficiencies,which depend on the amount of material used and the design of the detector geometry.This paper presents the optimized design of the hadronic calorimeter(HCAL)used in the DarkSHINE experiment,which is studied using a GEANT4-based simulation framework.The geometry is optimized by comparing a traditional design with uniform absorbers to one that uses different thicknesses at different locations on the detector,which enhances the efficiency of vetoing low-energy neutrons at the sub-GeV level.The overall size and total amount of material used in the HCAL are optimized to be lower,owing to the load and budget requirements,whereas the overall performance is studied to satisfy the physical objectives.

文摘The DarkSHINE experiment proposes a novel approach to single-electron-on-fixed-target exploration that focuses on the search for dark photons through their invisible decay into dark matter particles.Central to this initiative is an advanced tracking detector designed to achieve exceptional sensitivity in the detection of light dark matter candidates.This study evaluates the performance of several prototype AC-coupled low-gain avalanche diode(AC-LGAD)strip sensors specifically developed for the DarkSHINE tracking detector.The electrical properties of the sensors from two batches of wafers with different n^(+)doses are thoroughly evaluated.Spatial and temporal resolutions are measured using an infrared laser source.The spatial resolutions range from 6.5 to 8.2μm and from 8.8 to 12.3μm for the sensors from two distinct dose batches,each with a 100μm pitch size.Furthermore,the sensors demonstrate time resolutions of 8.3 and 11.4 ps,underscoring the potential of AC-LGAD technology in enhancing the performance of the DarkSHINE tracking detector.

文摘The microstructures and corrosion behaviors of as-cast,T4-treated,and T6-treated Mg-6Gd-3Y-0.5Zr alloys were systematically investigated by SEM,TEM,immersion test,and electrochemical corrosion test.The results show that the microstructure of the as-cast alloy is composed ofα-Mg and Mg_(24)(Gd,Y)_(5) eutectic phase,and in T4-treated alloy,Mg_(24)(Gd,Y)_(5) phase dissolves into theα-Mg matrix,leading to an increase in the(Y,Gd)H_(2) phase.After T6 treatment,nanoscale Mg_(24)(Gd,Y)_(5) phase dispersedly precipitates from theα-Mg matrix,and exhibits a specific orientation relationship with the α-Mg:(332)Mg_((24)(Gd,Y)_(5))//(1011)_(α-Mg),[136]Mg_((24)(Gd,Y)_(5))//[1210]_(α-Mg).The corrosion resistance of the Mg-6Gd-3Y-0.5Zr alloys can be ranked in the following order:T6-treated alloy exhibits the highest corrosion resistance,followed by the T4-treated alloy,and finally,the as-cast alloy.The corrosion products of the alloys are all composed of MgO,Mg(OH)_(2),Gd_(2)O_(3),Y_(2)O_(3),and MgCl_(2).The corrosion behavior of Mg-6Gd-3Y-0.5Zr alloy is closely related to the precipitated phase.By establishing the relationship between corrosion rate,hydrogen evolution rate,and corrosion potential,it is further demonstrated that during the micro galvanic corrosion process,the coarse Mg_(24)(Gd,Y)_(5)phase in the as-cast alloy undergoes extensive dissolution,and(Y,Gd)H_(2) phase promotes the dissolution of theα-Mg matrix in the T4-treated alloy,intensifying the hydrogen evolution reaction.The T6-treated alloy,with dispersive precipitation of nanoscale Mg_(24)(Gd,Y)_(5) phase,exhibits better corrosion resistance performance.
文摘BACKGROUND Transarterial chemoembolization(TACE)combined with microwave ablation(MWA)is an effective treatment strategy for patients with advanced gastric cancer and liver metastasis.However,it may cause severe postoperative pain and inflammatory responses.The paravertebral block(PVB)is a regional anesthetic technique that provides analgesia to the thoracic and abdominal regions.AIM To evaluate the effect of PVB on postoperative analgesia and inflammatory response in patients undergoing TACE combined with MWA for advanced gastric cancer and liver metastasis.METHODS Sixty patients were randomly divided into PVB and control groups.The PVB group received ultrasound-guided PVB with 0.375%ropivacaine preoperatively,whereas the control group received intravenous analgesia with sufentanil.The primary outcome was the visual analog scale(VAS)score for pain at 6 h,12 h,24 h,and 48 h after the procedure.Secondary outcomes were the dose of sufentanil used,incidence of adverse events,and levels of inflammatory markers(white blood cell count,neutrophil percentage,C-reactive protein,and procalcitonin)before and after the procedure.RESULTS The PVB group had significantly lower VAS scores at 6 h,12 h,24 h,and 48 h after the procedure compared with the control group(P<0.05).The PVB group also had a significantly lower consumption of sufentanil and a lower incidence of nausea,vomiting,and respiratory depression than did the control group(P<0.05).Compared with the control group,the PVB group had significantly lower levels of inflammatory markers 24 h and 48 h after the procedure(P<0.05).CONCLUSION PVB can effectively reduce postoperative pain and inflammatory responses and improve postoperative comfort and recovery in patients with advanced gastric cancer and liver metastasis treated with TACE combined with MWA.

文摘Background:To investigate the effects of acupuncture on post-stroke limb spasm model rats and the underlying mechanism.Methods:A total of 50 Sprague-Dawley rats were randomly divided into three groups,Control group(10),Model group(20)and Zhishen Tiaoxing(ZSTX)acupuncture group(20).Middle cerebral artery occlusion was conducted in SD rats to establish post-stroke limb spasm rats,which were treated with ZSTX acupuncture.Behavioral assays were determined by the Narrow ally test,the limb muscle tension was detected by the BL-420S test system,and infarct volume was assessed after the cerebral infarction by 2,3,5-triphenyltetrazolium chloride staining.Heterogeneous neurotransmitterγ-aminobutyric acid(GABA)and its receptors GABAA and GABAB in the cerebral cortex of the infarct area were determined by immunofluorescence assay.The release of Trkb and K-Cl cotransporter isoform 2 was detected by an immunofluorescence double labeling study.Western Blot was utilized to measure the expression of BDNF and Trkb.Results:The results showed that the behavioral assays in post-stroke limb spasm rats were significantly improved by the treatment of ZSTX acupuncture.14 days of ZSTX acupuncture can effectively inhibit muscle tone and decrease Infarct volume,which was measured with BL-420S biological function experiment system and triphenyltetrazolium chloride.Meanwhile,the results of Double-Label Immunofluorescence Assays showed that ZSTX acupuncture improved the expression of GABA,GABAA,GABAB,BDNF,and K-Cl cotransporter isoform 2.Double-Label Immunofluorescence Assays and WB results showed that 14 days ZSTX acupuncture declined the expression of Trkb.Conclusions:Our results suggest that 14 days of ZSTX acupuncture can significantly improve post-stroke limb spasm.Meanwhile,the pathogenesis of post-stroke limb spasm and the efficacy of ZSTX acupuncture involve metabolic pathways of neurotransmitters,and electro-acupuncture can treat post-stroke limb spasm by regulating BDNF/Trkb-KCC2 signaling pathway.
文摘BACKGROUND Patients with atrial fibrillation(AF)and prior stroke history have a high risk of cardiovascular events despite anticoagulation therapy.It is unclear whether catheter ablation(CA)has further benefits in these patients.METHODS AF patients with a previous history of stroke or systemic embolism(SE)from the prospective Chinese Atrial Fibrillation Registry study between August 2011 and December 2020 were included in the analysis.Patients were matched in a 1:1 ratio to CA or medical treatment(MT)based on propensity score.The primary outcome was a composite of all-cause death or ischemic stroke(IS)/SE.RESULTS During a total of 4.1±2.3 years of follow-up,the primary outcome occurred in 111 patients in the CA group(3.3 per 100 person-years)and in 229 patients in the MT group(5.7 per 100 person-years).The CA group had a lower risk of the primary outcome compared to the MT group[hazard ratio(HR)=0.59,95%CI:0.47–0.74,P<0.001].There was a significant decreasing risk of all-cause mortality(HR=0.43,95%CI:0.31–0.61,P<0.001),IS/SE(HR=0.73,95%CI:0.54–0.97,P=0.033),cardiovascular mortality(HR=0.32,95%CI:0.19–0.54,P<0.001)and AF recurrence(HR=0.33,95%CI:0.30–0.37,P<0.001)in the CA group compared to that in the MT group.Sensitivity analysis generated consistent results when adjusting for time-dependent usage of anticoagulants.CONCLUSIONS In AF patients with a prior stroke history,CA was associated with a lower combined risk of all-cause death or IS/SE.Further clinical trials are warranted to confirm the benefits of CA in these patients.

文摘AIM:To evaluate the effects and safety of combination chemotherapy with oxaliplatin (L-OHP) and S-1 (SOX regimen) in older patients with advanced gastric cardiac adenocarcinoma (GCA). METHODS: Seventy patients with advanced GCA were classified according to age into an older group (≥ 75 years) and a control group (< 75 years). The SOX regimen was administered to the two groups as follows: S-1 (40 mg/m2 po bid) on days 1 to 14 followed by a 7-d off period, plus L-OHP (65 mg/m2 iv) for 2 h on days 1 and 8 of a 21-d cycle. This regimen was repeat-ed for four to six cycles. Response and swallow statuses were evaluated after two cycles (6 wk). Effects and toxicity were evaluated four weeks after chemotherapy was completed. RESULTS: The response rate was 65.6% (21/32) in the older group and 68.4% (26/38) in the control group (χ2 = 0.062 and P = 0.804). Improvement in swallowing was 78.1% (25/32) in the older group and 76.3% (29/38) in the control group (χ2 = 0.032 and P = 0.857). Effi cacy was 68.8% (22/32) in the older group and 65.8% (25/38) in the control group (χ2 = 0.069 and P = 0.793). Toxicities were reversible and similar in both groups (P > 0.05). CONCLUSION: The SOX regimen is an effective, safe and well-tolerated regimen for older patients with advanced GCA.
文摘AIM To analyze the incidence of hepatocellular carcinoma (HCC) in a population that underwent health checkups and had high serum miR-106b levels. METHODS A total of 335 subjects who underwent checkups in the Digestive and Liver Disease Department of our hospital were randomly selected. RT-PCR was used to detect the level of miR-106b in serum samples. Laboratory and imaging examinations were carried out to confirm the HCC diagnosis in patients who had a > 2-fold change in miR-106b levels. Ultrasound-guided biopsy was also used for HCC diagnosis when necessary. On this basis, the clinical data of these subjects, including history of hepatitis virus infection, obesity, long-term history of alcohol use and stage of HCC, were collected. Then, the impact of these factors on the level of miR1-06b in serum was analyzed. Furthermore, receiver operating characteristic (ROC) curve was drawn to evaluate the diagnostic efficacy of miR-106b for HCC. RESULTS A total of 35 subjects had abnormal serum miR-106b levels, of which 20 subjects were diagnosed with HCC. t-test revealed that the difference in serum miR-106b level in terms of sex, age, history of hepatitis virus infection, obesity and long-term history of alcohol use was not statistically significant. However, serum miR-106b levels in patients with advanced HCC (stage. /.) was higher than in patients with early HCC (stage./.), and the difference was statistically significant (P = 0.000). Moreover, the ROC curve revealed that the area under the curve value for miR-106b was 0.885, which shows that serum miR-106b level has a certain clinical value for HCC diagnosis. CONCLUSION The random sampling survey shows that serum miR-106b level is a valuable diagnostic marker for HCC. However, the diagnostic threshold value needs to be further researched.

文摘BACKGROUND There are few effective tools to predict survival in patients with invasive intraductal papillary mucinous neoplasms of the pancreas.AIM To develop comprehensive nomograms to individually estimate the survival outcome of patients with invasive intraductal papillary mucinous neoplasms of the pancreas.METHODS Data of 1219 patients with invasive intraductal papillary mucinous neoplasms after resection were extracted from the Surveillance,Epidemiology,and End Results database,and randomly divided into the training(n=853)and the validation(n=366)cohorts.Based on the Cox regression model,nomograms were constructed to predict overall survival and cancer-specific survival for an individual patient.The performance of the nomograms was measured according to discrimination,calibration,and clinical utility.Moreover,we compared the predictive accuracy of the nomograms with that of the traditional staging system.RESULTS In the training cohort,age,marital status,histological type,T stage,N stage,M stage,and chemotherapy were selected to construct nomograms.Compared with the American Joint Committee on Cancer 7th staging system,the nomograms were generally more discriminative.The nomograms passed the calibration steps by showing high consistency between actual probability and nomogram prediction.Categorial net classification improvements and integrated discrimination improvements suggested that the predictive accuracy of the nomograms exceeded that of the American Joint Committee on Cancer staging system.With respect to decision curve analyses,the nomograms exhibited more preferable net benefit gains than the staging system across a wide range of threshold probabilities.CONCLUSION The nomograms show improved predictive accuracy,discrimination capability,and clinical utility,which can be used as reliable tools for risk classification and treatment recommendations.
文摘BACKGROUND Lumbar facet joint syndrome(LFJS)is a pain condition arising from lumbar facet joint diseases.Treatments of LFJS includes patient education,oral medication,bed rest,physical therapy,and procedural interventions.For some refractory cases that fail conservative therapies,dorsal ramus medial brunch radiofrequency ablation is warranted.However,as nerve fibers can regenerate,their efficacy is impermanent,and the recurrence rate is relatively high.Considering synovial impingement is a paramount pathogenesis of LFJS,in this case,we removed the culprit hyperplastic articular capsule and the articular process partially through a spinal endoscope.As the culprit hyperplastic joint capsule was excised,it is supposed to generate more prolonged efficacy and a lower recurrence rate than radiofrequency treatment.CASE SUMMARY A 40-year-old female patient was diagnosed with LFJS.She complained of low back pain and right buttock pain for half a year.The patient was placed in the prone position.After disinfection and draping,a 25-cm 18-gauge needle was inserted into the dorsal surface of the right L5 articular process.Subsequently,a guidewire,dilating tubes,and a working cannula was inserted successively.The spinal endoscope was positioned in the working cannula.Under the endoscope,the microvascular tissue,muscle tissue attached on the L5 inferior articular process and S1 superior articular process,as well as the capsule and minor portion of the inferior articular process were removed.After the joint space was clear and no bleeding points existed,the endoscope and working cannula were shifted,and the incision was sutured.After treatment,the symptoms were completely relieved.The patient was pain-free during the follow-up period of 6 mo.CONCLUSION The endoscopic partial joint capsule and articular process excision is an effective procedure for LFJS,especially for cases caused by synovial impingement.

文摘In this work,a new(Y,Gd)H_(2) precipitate was identified and systematically investigated in the as-cast Mg-6Gd-3Y-0.5Zr alloy by XRD,SEM with EDS,TEM with EDS techniques and thermodynamics analysis.Results show that the as-cast alloy contains α-Mg,Mg_(24)(Gd,Y)_(5),and(Y,Gd)H_(2) phase.The(Y,Gd)H_(2) phase usually forms near the eutectic phase Mg_(24)(Gd,Y)_(5) or in the α-Mg grains,displaying a rectangle-shape.The Mg_(24)(Gd,Y)_(5) and(Y,Gd)H_(2) phases crystalize in bcc and fcc structure,respectively,and the(Y,Gd)H_(2) phase has a semi-coherent relationship with α-Mg matrix.The thermodynamics calculation results reveal that the hydrogen dissolved in the melt leads to the formation of hydrides.It is also found that the(Y,Gd)H_(2) hydride can form directly from the liquid phase during solidification.Additionally,it can precipitate by the decomposition of Mg_(24)(Gd,Y)_(5) phase due to absorbing hydrogen from the remaining melt.

文摘A slow and clean uorine atom beam source is one of the essential components for the low-collision energy scattering experiment involving uorine atom.In this work,we describe a simple but ef-fective photolysis uorine atom beam source based on ultraviolet laser photolysis,the performance of which was demonstrated by high-resolution time-of-ight spectra from the reactive scattering of F+HD.This beam source paved the way for stud-ies of low energy collisions with uorine atoms.

文摘The reaction of chlorine atom Cl(2P)(Cl(2P3/2)and Cl^*(2P1/2))with D2 was investigated at collision energy from 4.5 kcal/mol to 6.5 kcal/mol with a high-resolution crossed molecular beam apparatus using the technique of D-atom Rydberg tagging detection.The contribution from the spin-orbit excited reaction Cl^*(2P1/2)+D2,which is prohibited by Born-Oppenheimer(BO)approximation,was observed.Collision-energy dependence of differential cross sections(DCSs)near the backward scattering direction was measured.The BOforbidden reaction Cl^*+D2 was found to be dominant at lower collision energy.As collision energy increases,reactivity of BO-allowed reaction Cl+D2 increases much faster than that of BO-forbidden reaction and becomes dominant at higher collision energy.Our experiment indicates that the additional energy of spin-orbit excitation in Cl^*facilitates BO-forbidden reaction to pass through the barrier at lower collision energy,while BO approximation is still valid at collision energy near and above the reaction barrier.This tendency of reactivity of Cl/Cl^*+D2 is similar to the isotopic reaction of Cl/Cl^*+H2.

文摘We operated a p-type point contact high purity germanium(PPCGe)detector(CDEX-1B,1.008 kg)in the China Jinping Underground Laboratory(CJPL)for 500.3 days to search for neutrinoless double beta(0νββ)decay of ^(76)Ge.A total of 504.3 kg⋅day effective exposure data was accumulated.The anti-coincidence and the multi/single-site event(MSE/SSE)discrimination methods were used to suppress the background in the energy region of interest(ROI,1989–2089 keV for this work)with a factor of 23.A background level of 0.33 counts/(keV⋅kg⋅yr)was realized.The lower limit on the half life of^(76)Ge 0νββdecay was constrained as T_(1/2)^(0ν)>1.0×10^(23)yr(90%C.L.),corresponding to the upper limits on the effective Majorana neutrino mass:<mββ><3.2–7.5 eV.

文摘Dark photons have been well motivated as strong candidates for dark force carriers and light dark matter in the sub-GeV mass range.Compared with collider experiments,fixed-target experiments provide a complementary approach to searching for dark photons,particularly in the lower mass range.We have studied the physics potential of the electron-on-target experiment based on the Shanghai SHINE facility,which provides 10 MHz single electron beam at 8 GeV energy.This analysis focuses on dark photons being produced via electron and nucleon interaction and then decays to dark matter candidates,which escape detection as missing momentum in the detector.This experiment takes advantage of using missing momentum to enhance signal versus background separation power.In this study,signal samples as a function of dark photon mass and an inclusive background sample with 2.5 billion events are simulated with GEANT4.For better background estimates,major rare background processes have also been simulated.This paper presents the experiment and detector design,signal and background simulations,analysis strategy,and the prospective study of the experiment sensitivity.With 9×10^(14) electron-on-target events(about three years running),this experiment is expected to rule out most of the sensitive regions predicted by popular dark photon models.

文摘As one of the most successful intracellular symbiotic bacteria,Wolbachia can infect many arthropods and nematodes.Wolbachia infection usually affects the reproduction of their hosts to promote their own proliferation and transmission.Currently,most of the studies focus on the mechanisms of Wolbachia interactions with host reproduction.However,in addition to distribution in the reproductive tissues,Wolbachia also infect various somatic tissues of their hosts,including the brain.This raises the potential that Wolbachia may influence some somatic processes,such as behaviors in their hosts.Sofar,information about the effects of Wolbachia infection on host behavior is still very limited.The present review presents the current literature on different aspects of the influence of Wolbachia on various behaviors,including sleep,learning and memory,mating,feeding and aggression in their insect hosts.We then highlight ongoing scientific efforts in the field that need addressing to advance this field,which can have significant implications for further developing Wolbachia as environmentally friendly biocontrol agents to control insect-borne diseases and agricultural pests.
文摘Background: Accumulating evidence indicates that attention deficit hyperactivity disorder (ADHD) is associated with core deficits in executive function (EF) which predicts poorer academic and occupational functioning. This makes early intervention targeting EF impairments important to prevent long-term negative outcomes. Cognitive training is a potential ADHD treatment target. The present study aimed to explore the efficacy, feasibility, and acceptability of a cognitive training program (targeting child's multiple EF components and involving parent support in daily life), as a nonpharmacological intervention for children with ADHD. Methods: Forty-four school-age children with ADHD and their parents participated in 12 sessions of EF training (last for 12 weeks) and 88 health controls (HC) were also recruited. Training effects were explored using both neuropsychological tests (Stroop color-word test, Rey-Osterrieth complex figure test, trail making test, tower of Hanoi, and false-belief task) and reports of daily life (ADHD rating scale-IV, Conners' parent rating scale, and behavior rating inventory of executive function [BRIEF]) by analysis of paired sample t-test and Wilcoxon signed-rank test. The differences on EF performances between children with ADHD after training and HC were explored using multivariate analysis. Results: The results (before vs. after EF training) showed that after intervention, the children with ADHD presented better performances of EF both in neuropsychological tests (word interference of Stroop: 36.1 ± 14.6 vs. 27.1 ± 11.1, t = 4.731, P 〈 0.001 ; shift time of TMT: 194.9 ± 115.4 vs. 124.8 ± 72.4, Z = -4.639, P 〈 0.001 ; false-belief task: χ^2 = 6.932, P = 0.008) and reports of daily life (global executive composite of BRIEF: 148.9 ± 17.5 vs. 127.8 ± 17.5, t =6.433, P 〈 0.001). The performances on EF tasks for children with ADHD after EF training could match with the level of HC children. The ADHD symptoms (ADHD rating scale total score: 32.4 ± 8.9 vs. 22.9 ± 8.2, t = 6.331, P 〈 0.001) and behavioral problems of the children as reported by parents also reduced significantly after the intervention. Participants reported that the EF training program was feasible to administer and acceptable. Conclusions: The EF training program was feasible and acceptable to children with ADH D and parents. Although replication with a larger sample and an active control group are needed, EF training program with multiple EF focus and parent involving in real-life activities could be a potentially promising intervention associated with significant EF (near transfer) and ADHD symptoms improvement (far transfer).
文摘Background: The cognitive function of children with either attention deficit hyperactivity disorder (ADHD) or learning disabilities (LDs) is known to be impaired. However, little is known about the cognitive function of children with comorbid ADHD and LD. The present study aimed to explore the cognitive function of children and adolescents with ADH D and learning difficulties in comparison with children with ADHD and healthy controls in different age groups in a large Chinese sample. Methods: Totally, 1043 participants with ADHD and learning difficulties (the ADHD + learning difficulties group), 870 with pure ADHD (the pure ADHD group), and 496 healthy controls were recruited. To investigate the difference in cognitive impairment using a developmental approach, all participants were divided into three age groups (6-8, 9-11, and 12-14 years old). Measurements were the Chinese-Wechsler Intelligence Scale for Children, the Stroop Color-Word Test, the Trail-Making Test, and the Behavior Rating Inventory of Executive Function-Parents (BRIEF). Multivariate analysis of variance was used. Results: The results showed that after controlling for the effect of ADHD symptoms, the A DHD + learning difficulties group was still significantly worse than the pureADHD group, which was, in turn, worse than the control group on full intelligence quotient (98.66 ± 13.87 vs. 105.17 ± 14.36 vs. 112.93 ± 13.87, P 〈 0.001 ). The same relationship was also evident for shift function (shifting time of the Trail-Making Test, 122.50 [62.00, 194.25] s vs. 122.00 [73.00, 201.50] s vs. 66.00 [45.00, 108.00] s, P 〈 0.001) and everyday life executive function (BRIEF total score, 145.71 ± 19.35 vs. 138.96± 18.00 vs. 122.71 ± 20.45, P 〈 0.001 ) after controlling for the effect of the severity of ADHD symptoms, intelligence quotient, age, and gender. As for the age groups, the differences among groups became nonsignificant in the 12-14 years old group for inhibition (meaning interference of the Stroop Color-Word Test, 18.00 [13.00, 25.00] s vs. 17.00 [15.00, 26.00] s vs. 17.00 [10.50, 20.00] s, P =0.704) and shift function (shifting time of the Trail-Making Test, 62.00 [43.00, 97.00] s vs. 53.00 [38.00, 81.00] s vs. 101.00 [88.00, 114.00] s, P = 0.778). Conclusions: Children and adolescents with ADHD and learning difficulties have more severe cognitive impairment than pure ADHD patients even after controlling for the effect of ADHD symptoms. However, the differences in impairment in inhibition and shift function are no longer significant when these individuals were 12-14 years old.
文摘Background: As medication does not normalize outcomes of children with attention deficit hyperactivity disorder (ADHD), especially in real-life functioning, nonpharmacological methods are important to target this field. This randomized controlled clinical trial was designed to evaluate the effects of a comprehensive executive skill training program for school-aged children with ADHD in a relatively large sample. Methods: The children (aged 6-12 years) with ADHD were randomized to the intervention or waitlist groups. A healthy control group was composed of gender- and age-matched healthy children. The intervention group received a 12-session training program lbr multiple executive skills. Executive function (EF), ADHD symptoms, and social functioning in the intervention and waitlist groups were evaluated at baseline and the end of the final training session. The healthy controls (HCs) were only assessed once at baseline. Repeated measures analyses of variance were used to compare EF, ADHD symptoms, and social function between intervention and waitlist groups. Results: Thirty-eight children with ADH D in intervention group, 30 in waitlist group, and 23 healthy children in healthy control group were included in final analysis. At posttreatment, intervention group showed significantly lower Behavior Rating Inventory of Executive Function (BRIEF) total score (135.89 ±16.80 vs. 146.09 ± 23.92, P = 0.04) and monitoring score (18.05 ± 2.67 vs. 19.77 ± 3.10, P = 0.02), ADHD-IV overall score (41.11 ± 7.48 vs. 47.20 ± 8.47, P 〈 0.01 ), hyperactivity-impulsivity (HI) subscale score ( 18.92 ± 5.09 vs. 21.93 ± 4.93, P = 0.02), and inattentive subscale score (22.18 ±3.56 vs. 25.27± 5.06, P 〈 0.01 ), compared with the waitlist group. Repeated measures analyses of variance revealed significant interactions between time and group on the BRIEF inhibition subscale (F = 5.06, P 0.03), working memory (F- 4.48, P = 0.04), A DH D-IV overall score (F = 21.72, P 〈 0.01), HI subscale score (F = 19.08, P 〈 0.01), and inattentive subscale score (F- 12,40, P 〈 0.01 ). Multiple-way analysis of variance showed significant differences on all variables of BRIEF, ADHD-rating scale-IV, and WEISS Functional Impairment Scale-Parent form (WFIRS-P) among the intervention and waitlist groups at posttreatment and HCs at baseline. Conclusions: This randomized controlled study on executive skill training in a relatively large sample provided some evidences that the training could improve EF deficits, reduce problematic symptoms, and potentially enhance the social thnctioning in school-aged children with ADHD.
