Two and a half years ago, it was foun... The patient was found to have 4+urine sugar by physical examination 14 years ago and was treated with oral hypoglycemic drugs. Insulin was injected intramuscularly nine years ago. Two and a half years ago, it was found that the color of the thumb, index and middle toe of the left foot became black. He went to a third-class hospital in Beijing and was diagnosed as “diabetes foot”. He was treated with “balloon dilation of lower limb blood vessels of diabetes foot”. Half a year ago, the third toe on the right side was broken and treated in the hospital again. “Popliteal artery stent implantation” was given for the diagnosis of “double kidney insufficiency, diabetes foot, left heart failure, combined heart valve disease”, “Hemofiltration therapy” and anti-inflammatory, amino acid supplementation, kidney function protection, anticoagulation, anemia correction and other treatments. Later, he went to our hospital and was diagnosed by the TCM diagnosis: category of consumptive disease, toe or finger gangrene (syndrome/pattern of qi and yin deficiency). Western medicine diagnosed: stage V of diabetes nephropathy, type II diabetes foot gangrene, combined with heart valve disease, hypoalbuminemia, double kidney cyst, moderate anemia, pleural effusion, hyperkalemia, pulmonary infection, and total heart failure. The patient was treated by the Qi-acupuncture therapy of TCM in combination with Chinese and Western medicine Medical treatment made the patient significantly better and discharged. 展开更多

Morphological evolution of a large sand bar in the Qiantang River Estuary of China since the 1960s

A large sand bar develops in the inner Qiantang River Estuary,China.It is a unique sedimentary system,elongating landwards by about 130 km.Based on long-term series of bathymetric data in each April,July,and November since the 1960s,this study investigated the morphological behavior of this bar under natural conditions and the influence of a large-scale river narrowing project(LRNP)implemented in the last decades.The results show that three timescales,namely the seasonal,interannual and decadal timescales,can be distinguished for the sand bar evolution.The first two are related to the seasonal and interannual variations of river discharge.During high discharge seasons or years,erosion took place at the upper reach and sedimentation at the lower reach.Consequently,the bar apex shifted seaward.The opposite development took place during low discharge seasons or years.The decadal timescale is related to LRNP.Due to the implementation of LRNP,the upper reach has experienced apparent erosion and currently a new equilibrium state has been reached;whereas the lower reach has been accumulated seriously and the accumulation still continues.Nonlinear relationships for how the bar apex location and elevation depend on the river discharge over various stages of LRNP have been established.Compared with the earlier stage of LRNP,the bar apex at present has shifted seaward by about 12 km and lowered by about 1 m.The sand bar movement has significant feedback on the hydrographic conditions along the estuary and has practical implications for coastal management. 展开更多

Short-term offshore extension of Brahmaputra-Ganges and Irrawaddy freshwater plumes to the central northern Bay of Bengal based on in situ and satellite observations

In this study, the short-term offshore extension of Brahmaputra-Ganges(BG) and Irrawaddy freshwater plumes to the central northern Bay of Bengal(BoB) was investigated based on in situ and satellite observations. In the summer and winter of 2015, two significant freshening events with periods of weeks were observed from a moored buoy at 15°N, 90°E in the BoB. Soil Moisture Active Passive(SMAP) satellite sea surface salinity compares well with the in situ data and shows that these freshening events are directly related to the short-term offshore extension of the BG and Irrawaddy freshwater, respectively. These data combined with the altimeter sea level anomaly data show that the offshore extending plumes result from freshwater modulated by eddies. During summer, the BG freshwater is modulated by a combination of three closely located eddies: a large anticyclonic eddy(ACE) off the northwestern BoB coast and two cyclonic eddies in the northern BoB. Consequently, the freshwater extends offshore from the river mouth and forms a long and narrow tongue-shaped plume extending southwestward to the central BoB. During winter, the Irrawaddy freshwater is modulated by two continuous ACEs evolved from Rossby wave propagating westward from the Irrawaddy Delta off Myanmar, forming a tongueshaped plume extending to the central BoB. Strong salinity fronts are formed along the boundaries of these tongue-shaped plumes. These findings confirm good capability of the SMAP data to investigate the short-term offshore extension of the BG and Irrawaddy freshwater. This study provides direct evidences of the pathways of the offshore extension of the BG and Irrawaddy freshwater and highlights the role of eddies in the northern BoB freshwater plume variability. 展开更多
