Analysis of the Effectiveness of Modified Non- Traumatic Filling Technology in the Treatment of Pediatric Dental Caries

Objective:To analyze the effectiveness of modified non-traumatic filling technology in the treatment of pediatric dental caries.Methods:Ninety-seven children with dental caries who were treated in our hospital(Panyu Maternal and Child Care Service Center of Guangzhou)from January 2022 to December 2023 were selected and randomly divided into two groups,with 48 cases in the experimental group and 49 in the control group.The experimental group was treated with modified non-traumatic filling techniques,while the control group was treated with conventional filling techniques.Observation indicators such as the total effectiveness of the treatment,incidence of adverse events,treatment compliance,and pain scores were analyzed after the intervention.Result:After intervention,the total effectiveness of the treatment in the experimental group was higher than that in the control group(P<0.05).The incidence of adverse events in the experimental group was lower than that of the control group(P<0.05).The treatment compliance of patients in the experimental group was higher than that in the control group(P<0.05).The pain score of the experimental group was lower than that of the control group(P<0.05).Conclusion:The application of modified non-traumatic filling technology in the treatment of pediatric dental caries showed good therapeutic outcomes.After the intervention,the child’s symptoms were significantly alleviated,the incidence of adverse events such as filling material falling off was reduced,their compliance was improved,and the pain was relieved.This procedure is worth to be promoted for clinical application. 展开更多

Introduction: A scanning acoustic microscope (SAM) is an apparatus for imaging acoustic properties. This apparatus can non-invasively and rapidly evaluate the hardness of materials in the elastic region. This device shows great potential for the diagnosis of dental caries in the clinical setting. However, since the tissue elastic modulus measured using a SAM is a property of the elastic region and the Knoop hardness is a property of the plastic region, the hardness properties differ completely. Therefore, we investigated whether the acoustic impedance measured using a SAM is related to the Knoop hardness, which is used as the standard for removal of carious dentin. Method: Polished sections were prepared from 20 extracted carious wisdom teeth. The acoustic impedance and Knoop hardness were measured for each section. In addition to comparing carious and healthy dentin in SAM images, we evaluated the difference between the carious and healthy dentin in terms of the acoustic impedance and Knoop hardness. We also evaluated the correlation between the Knoop hardness and acoustic impedance. Results: The SAM images were visualized as two-dimensional color images based on the acoustic impedance values. The mean acoustic impedance of carious dentin was significantly lower than that of healthy dentin, showing a similar trend as Knoop hardness. A strong correlation was observed between the two. Discussion: The acoustic impedance values obtained through acoustic microscopy differed significantly between carious and sound dentin. Both types of dentins were visualized using two-dimensional color images. A strong correlation was observed between the acoustic impedance value, which indicates the hardness of the elastic region, and the Knoop hardness, which indicates the hardness of the plastic region. The results of the present study indicate that acoustic impedance accurately reflects the hardness of dentin. 展开更多

Objective:To explore the curative effect of Carisolv,a minimally invasive caries removal technique and traditional mechanical caries removal treatment on children’s dental caries.Methods:A total of 97 children with dental caries who were treated in the Department of Stomatology in Affiliated Hospital of Chifeng University,Chifeng from September 2017 to May 2019 were selected and recruited as the research subjects.They were divided into two groups by random number table method.Forty-nine individuals were assigned in the control group while the remaining 48 individuals in the observation group.The control group was treated with traditional mechanical caries removal method,and the observation group was treated with minimally invasive caries removal technique,i.e.Carisolv.Both groups were followed up for six months.The degree of pain,recovery time of dental function and complications after six months of treatment were observed in the two groups of children.Results:During the treatment,compared with the control group,the children in the observation group experienced lower degree of pain and had shorter recovery time of dental function.After six months of treatment,the incidence of complications in the observation group was lower than that in the control group.The difference was statistically significant(P<0.05).Conclusion:Compared with traditional mechanical caries removal method,Carisolv,a minimally invasive caries removal technique could reduce the pain of children during the treatment process,shorten the time to restore dental function,reduce the occurrence of complications,and had a better therapeutic effect in treating children’s dental caries. 展开更多

Early childhood caries(ECC) is a term used to describe dental caries in children aged 6 years or younger. Oral streptococci, such as Streptococcus mutans and Streptococcus sorbrinus, are considered to be the main etiological agents of tooth decay in children. Other bacteria, such as Prevotella spp. and Lactobacillus spp., and fungus, that is, Candida albicans, are related to the development and progression of ECC. Biomolecules in saliva, mainly proteins, affect the survival of oral microorganisms by multiple innate defensive mechanisms, thus modulating the oral microflora. Therefore, the protein composition of saliva can be a sensitive indicator for dental health. Resistance or susceptibility to caries may be significantly correlated with alterations in salivary protein components. Some oral microorganisms and saliva proteins may serve as useful biomarkers in predicting the risk and prognosis of caries. Current research has generated abundant information that contributes to a better understanding of the roles of microorganisms and salivary proteins in ECC occurrence and prevention. This review summarizes the microorganisms that cause caries and tooth-protective salivary proteins with their potential as functional biomarkers for ECC risk assessment. The identification of biomarkers for children at high risk of ECC is not only critical for early diagnosis but also important for preventing and treating the disease. 展开更多

The epidemic tendency of dental caries of school students over the past 15 years in China was analyzed in order to provide bases for prevention of dental caries.Data sets of boys and girls at the age of 7,9,12 years(deciduous caries)and 7,9,12,14,17 years(permanent caries)were collected from the series of Chinese National Surveillance on Students' Constitution and Health(CNSSCH)between 1991 to 2005,a survey that covers 30 provinces of and autonomous regions of China,with Tibet Autonomous Region and Taiwan Province excluded owing to unavailability of related data.According to their socioeconomic status,three urban regions and three rural regions(province capital must be included)were selected in each province,where its socioeconomic status and sports exercise can represent the general level of the locality.All subjects were Han students.Prevalence of deciduous and permanent caries was measured.In 2005,the prevalence of deciduous caries at 7,9,12 years was 52.87%,52.04% and 9.80% in urban boys,59.37%,58.68% and 12.18% in rural boys,and that of permanent caries in boys at 7,9,12,14,and 17 years was 2.35%,5.63%,13.94%,19.05% and 21.29% in urban boys,3.42%,7.26%,13.35%,16.16%,18.55% in rural boys,while in girls at the same age groups,the prevalence of deciduous caries was 53.53%,49.48%,7.92% in urban girls,59.95%,56.13%,8.85% in rural girls;the prevalence of permanent caries was 3.61%,8.99%,19.32%,24.75%,28.62% in urban girls,and 3.47%,10.65%,17.66%,22.52%,24.27% in rural girls.The epidemic manifested a gradient distribution in age groups,which was closely related to region of the study population.A dramatic decreasing trend was witnessed among all sex-age subgroups in the rural groups between 1990 and 2005,and such a trend was stronger in deciduous caries than in permanent caries,however,this kind of decreasing trend was not steady,because there was bounce during some year.Although the morbidity of dental caries in China is lower than the standard set by the World Health Organization,and the prevalence of dental caries decreased from 1991 to 2005,the prospect of epidemic dental caries in China is in no way optimistic.The total population with dental caries is still very large and the prevalence decreased unstably.Therefore,financial support on school oral health care should be increased,and a national school oral health service network should be set up. 展开更多

AIM: To systematically review the literature on the efficacy of noninvasive methods of arresting the progression of non-cavitated occlusal carious lesions in dentin.METHODS: The Medline/Pub Med,LILACS,Sci ELO and Scopus databases were searched to identify relevant publications through to November 2013.Only clinical trials evaluating the ability of noninvasive methods to arrest the progression of occlusal non-cavitated carious lesions in dentin were included.Screening,data extraction and quality assessment were conducted independently and in duplicate.RESULTS: Of 167 citations identified,nine full text articles were screened and five were included in the analysis.All papers reported on occlusal fissure sealing using a self-curing glass ionomer(n = 1) or resin-based(n = 4) sealant.Only the use of resin-based sealant to obliterate occlusal fissures arrested the progression of non-cavitated occlusal carious lesions in dentin.CONCLUSION: Occlusal fissure sealing with a resin-based sealant may arrest the progression of non-cavitated occlusal dentinal caries.Further clinical trials with longer follow-up times should be performed to increase scientific evidence. 展开更多

We report on the application of Laser Induced Breakdown Spectroscopy(LIBS)technique to the study of Ca,P,Zn and Sr evolutions for adult caries-affected teeth using nanosecond laser pulses.The aim of this work is to better understand the behaviors of Zn and Sr as trace-elements in the caries eruption with respect to the behaviors of one of the main compounds of the hydroxyapatite crystal which is the calcium.The study was focused on the investigation of these elements’evolutions from three parts of the enamel surfaces of twenty two adult caries-affected teeth;the healthy part,the dental plaque part and the caries-affected part.The decrease rates of Zn,Sr and P was,also,compared to the Ca one.Comparison concerned normalized emission line intensities.For every species,normalization was done relatively to the emission line intensities of the healthy part.Results showed that abundances of these elements decrease similarly from the healthy parts to the affected-caries parts.The higher decrease rate was noticed for the calcium.The evolutions of Zn and Sr for the three parts of the teeth surfaces cannot inform about the substitution of the calcium by these trace-elements,however the comparison of their decrease rates to the calcium one can be considered as a valuable index of this substitution. 展开更多

Preceding studies have focused primarily on bacterial diversity at the taxonomic level. Although these studies have provided significant information regarding the connection between dental caries and oral microbiomes, further comprehension of this microbial community's ecological relevance is limited. This study identified the carbon source metabolic differences in dental plaque between children with and without ECC.We compared the microbial community functional diversity in 18 caries-free subjects with 18 severe ECC patients based on sole carbon source usage using a Biolog assay. The anaerobic microbial community in the ECC patients displayed greater metabolic activity than that of the control group. Specific carbon source metabolism differed significantly between the two groups. Subjects from the two groups were well distinguished by cluster and principal component analyses based on discriminative carbon sources.Our results implied that the microbial functional diversity between the ECC patients and healthy subjects differed significantly. In addition, the Biolog assay furthered our understanding of oral microbiomes as a composite of functional abilities, thus enabling us to identify the ecologically relevant functional differences among oral microbial communities. 展开更多

The influence of exogenic hormones (indole-3-acetic acid (IAA), abscisic acid (ABA) and kinetin) on defense reaction of wheat (Triticum aestivum L.) calli to the bunt agent Tilletia caries (D.C.) Tul. & C. Tul. was studied. ABA and kinetin induced the oxalate oxidase activity, increased the Н2О2 level, decreased germination of fungi teliospores and induced on calli the occurrence of dense sites non-infected by pathogen. On the contrary, IAA led to the decrease of oxalate oxidase activity, loosening of calli and increase germination of bunt agent teliospores and growth of fungi mycelium, besides stimulated rhizoids formation of wheat calli. Probably, the accumulation of Н2О2 in wheat calli under the influence of kinetin and ABA connected with activity of oxalate oxidase is one of the factors increasing defense reaction of wheat to bunt agent. 展开更多

Isolate 1 of wheat callus had a high growth rate in vitro even in the presence of 0.1 mg/l fungicide Baitan. Isolate 2 of wheat callus had a low growth rate whereas 0.1 mg/l Baitan significantly inhibited its growth. Comparison of nucleotide sequences of 18S RNA gene of the two isolates showed high level of homology between them, but a large number of nucleotide substitutions have been found. The most characteristic excision was five nucleotides at position 461 of the isolate 2, compared with the isolate 1. Our results allow to assume that the environmental stress including high pesticide concentration may cause the resistance of T. caries pathogen to fungicides. 展开更多

Lactobacilli have been consistently associated with dental caries for decades;however, knowledge of this group of bacteria in the etiology of the disease is limited to quantitative elucidation. Nowadays, explicit identification of oral Lactobacillus species is possible, despite their taxonomic complexity. Here we describe a combined approach involving both cultivation and genetic methods to ascertain and characterize the diversity and abundance of the Lactobacillus population in the oral cavities of children with severe early childhood caries (S-ECC). Eighty 3- to 6-year-old children (40 S-ECC and 40 caries free) who were seeking dental care at the Pediatric Dental Clinic of Bellevue Hospital in New York City were invited to participate in this study. Clinical data on sociodemographic information and oral health behavior were obtained from the primary caregiver. The data included a detailed dental examination, children’s medical history, and a questionnaire survey. Combined non-stimulated saliva and supragingival plaque samples were collected from each child and cultivated on selective media for quantitative measures of lactobacilli levels. The procedure for Lactobacillus species screening will include the random selection of 50 colonies per plate, ex- traction of DNA from each colony, and genotyping by arbitrarily primed polymerase chain reaction (AP-PCR). Each unique Lactobacillus AP-PCR genotype will be selected for taxonomic assessment by 16S rRNA gene sequencing analysis. Lactobacillus species will be identified by comparing the 16S rRNA sequences with the Ribosomal Database and the Human Oral Microbiome Database. Meanwhile, the same set of clinical samples will be independently subjected to genomic DNA isolation, 16S rRNA amplification with Lactobacillus genus-specific primers, sequencing, and taxonomic identification, both at genus and species levels with a customized pipeline. The distribution and phylogenetic differences of these Lactobacillus species will be compared between children with or without S-ECC. One of the main objectives of this study is to establish a study protocol for the identification and characterization of lactobacilli in the oral cavity. Future caries risk assessments can include lactobacilli counts (quantitative) and the presence/absence of specific cario- genic genetic signatures of a Lactobacillus species (qualitative) associated with S-ECC. 展开更多

Recently,the diagnoses of dental caries and other dental issues are in a queue as only X-ray-based techniques are available in most hospitals around the world.Terahertz(THz)parametric imaging(TPI)is the latest technology that can be applied for medical applications,especially dental caries.This technology is harmless and thus suitable for biological samples owing to the low energy of THz emission.In this paper,a developed TPI system is used to investigate the two-dimensional(2 D)and three-dimensional(3D)images of different samples from human teeth.After analyzing the measured images of human teeth,the results suggest that the THz parametric technology is capable of investigating the inner side structure of the teeth.This technique can be useful in detecting the defects in all types of human and animal teeth.The measurement and analytical calculations have been performed by using the TPI system and MATLAB,respectively,and both are in good agreement.The characteristics of THz waves and their interactions with the tooth samples are summarized.And the available THz-based technologies,such as TPI,and their potential applications of diagnoses are also presented. 展开更多

Background: Nowadays, though more attention is being paid to oral and dental hygiene all over the world, dental caries and periodontal diseases are increasing daily. We decided to study the effectiveness of natural toothbrush in prevention of dental caries and plaque formation. Methods: This analytic, semi-experimental clinical trial included 390 individuals. Subjects were divided into 3 groups: using natural toothbrush, using artificial toothbrush and using both natural and artificial toothbrush. The plaque index and DMFT was assessed at the beginning of the study until 1 year. ANOVA, Kruskal Wallis, LSD, Paired t-test, chi square, Fisher exact and Mc-Nemar tests were used for data analysis. Results: The plaque index in all three groups showed a mean reduction of 0.71. The mean DMFT in the samples before intervention was 8.38 ± 4.14 and increased to 9.1 ± 4.56. At the end of the study, the lowest plaque index was observed in the group using both the natural and artificial methods. During one year follow-up, the plaque index decreased 1.16, 0.83, and 0.21 degrees in the first, second and third groups. Conclusion: The increase in DMFT in the group that used artificial toothbrush was more than other groups. Thus, use of natural toothbrush leads to a decrease in growth rate of DMFT. Therefore natural toothbrush can be used for the cleaning of teeth and prevention of dental caries. 展开更多

Microcrystalline structure changes of teeth enamel crystals during the artificial caries and synthetic saliva reminerlization were reported. Enamel samples were thinned by argon-ion-milling and examined by high-resolution transmitting electron microscope. The results revealed that the lattice dissolution of enamel hydroxyapatite started from central parts and grain boundaries of crystals, the former were often parallel to (1010) plane and advancing along the [0001] zone axis of crystals. The results also shown that synthetic saliva may reduce the quantity of bright contrast spots in lattice image for artificial carious crystals. In addition, some defects such as dislocations were found in grains, and they are believed to be related to the dissolution of lattice. Three kinds of structural weak sites revealed in crystals, i. e. (1)dislocations, (2) vacancies or impurities, and (3) grain boundaries. 展开更多

Is Caries in the Early Mixed Dentition Associated with Caries Development during Orthodontic Treatment?

Objective: To investigate if caries in the pre-treatment early mixed dentition is associated with caries development in the permanent dentition during orthodontic treatment. Material and Methods: We included 41 consecutive patients (29 girls, 12 boys) with a pre-treatment documentation in the mixed (mean age 9.4 years) and a post treatment documentation in the permanent dentition (age 14.9 years) (two-phased treatment time 4.5 (±1.6) years). The DMFT/S indices were calculated. High-risk and low-risk groups were defined according to dmfs + DMFS score before treatment. Results: Initial dmft/s + DMFT/S (SD) was 5.15 (3.60) and 8.32 (6.64);final DMFT/S was 2.76 (2.84) and 3.01 (3.20). The missing (because of decay) second deciduous molars were most powerfully associated with caries increment during treatment, showing significant correlations to second premolars (r = 0.47, p = 0.003), while fillings on second deciduous molars seem to influence the prevalence of fillings on permanent molars (r = 0.44, p = 0.001). The increment at surface level was 2.01 (2.61) in the whole sample and 2.60 (3.81) in the high-risk group (4 boys, 6 girls). Compared to the low-risk group (10 girls), post-treatment caries experience was significantly higher in the high-risk group (p = 0.029). Boys were more at risk than girls (p = 0.005). Conclusions: Children with elevated caries experience in the early mixed dentition are exposed to higher caries risk during orthodontic treatment. Thus, in prevision of treatment, caries should already be assessed in the mixed dentition, so that an extended prophylaxis program can be initiated. 展开更多

Effect of socio-demographic status on dental caries in pupils by using a multilevel hurdle model

Introduction: Dental caries is the most prevalent chronic disease among schoolchildren. We aimed to investigate the association between dental caries index and socio-demographic status in schoolchildren by using a multilevel hurdle model. Materials and Methods: A cross-sectional epidemiological study was carried out on 906 primary school pupils in Kerman, Iran in 2012. The subjects were selected through a stratified cluster random sampling. At first, the whole target area was stratified according to two geographic areas, the north and south area. Then each area was stratified according to gender. In the next step, several schools were considered as clusters, which were selected randomly in each geographic area and gender strata. All the schoolchildren in these schools were included in this study as samples. Twelve-year-old pupils were examined for dental caries. The dependent variable in this study was the dmft/DMFT index. Eight variables, including socio-demographic information, were collected. Multilevel negative binomial hurdle model was employed for data analysis. Results: The prevalence of caries-free pupils was 30.1% and the mean dmft/DMFT was 3.6 ± 2.2. Negative binomial part of the adjusted model showed that the dmft/DMFT adjusted rate for females was 1.36 (95% CI: 1.10-1.79) times higher than males. Also, the dmft/DMFT adjusted rate for overweight pupils was 0.86 (95% CI: 0.74-0.98) times lower than those with normal weight. Logistic part of the adjusted model showed that the posibility of caries-free state in overweight pupils was 1.95 (95% CI: 1.22-3.11) times higher than those with normal weight. In addition, pupils whose fathers and mothers were workers and housewives, respectively, and those with a high maternal age were at a greater risk for caries experience than others (p < 0.05). Conclusion: BMI, gender, parent’s job and mother’s age at delivery were factors effecting on dental caries in schoolchildren. These pupils need more attention to dental care. 展开更多
