Objective Hypertriglyceridemic waist(HW),hypertriglyceridemic waist-to-height ratio(HWHtR),and waist-to-hip ratio(WHR)have been shown to be indicators of cardiometabolic risk factors.However,it is not clear which indicator is more suitable for children and adolescents.We aimed to investigate the relationship between HW,HWHtR,WHR,and cardiovascular risk factors clustering to determine the best screening tools for cardiometabolic risk in children and adolescents.Methods This was a national cross-sectional study.Anthropometric and biochemical variables were assessed in approximately 70,000 participants aged 6–18 years from seven provinces in China.Demographics,physical activity,dietary intake,and family history of chronic diseases were obtained through questionnaires.ANOVA,χ2 and logistic regression analysis was conducted.Results A significant sex difference was observed for HWHtR and WHR,but not for HW phenotype.The risk of cardiometabolic health risk factor clustering with HW phenotype or the HWHtR phenotype was significantly higher than that with the non-HW or non-HWHtR phenotypes among children and adolescents(HW:OR=12.22,95%CI:9.54-15.67;HWHtR:OR=9.70,95%CI:6.93-13.58).Compared with the HW and HWHtR phenotypes,the association between risk of cardiometabolic health risk factors(CHRF)clustering and high WHR was much weaker and not significant(WHR:OR=1.14,95%CI:0.97-1.34).Conclusion Compared with HWHtR and WHR,the HW phenotype is a more convenient indicator with higher applicability to screen children and adolescents for cardiovascular risk factors. 展开更多

Type 1 diabetes(T1D)is associated with general-and diabetes-specific stress which has multiple adverse effects.Hence measuring stress is of great importance.An algometer measuring pressure pain sensitivity(PPS)has been shown to correlate to certain stress measures in adults.However,it has never been investigated in children and adolescents.The aim of our study was to examine associations between PPS and glycated hemoglobin(HbA1c),salivary cortisol and two questionnaires as well as to identify whether the algometer can be used as a clinical tool among children and adolescents with T1D.Eighty-three participants aged 6-18 years and diagnosed with T1D were included in this study with data from two study visits.Salivary cortisol,PPS and questionnaires were collected,measured,and answered on site.HbA1c was collected from medical files.We found correlations between PPS and HbA1c(rho=0.35,P=0.046),cortisol(rho=-0.25,P=0.02)and Perceived Stress Scale(rho=-0.44,P=0.02)in different subgroups based on age.Males scored higher in PPS than females(P<0.001).We found PPS to be correlated to HbA1c but otherwise inconsistent in results.High PPS values indicated either measurement difficulties or hypersensibility towards pain. 展开更多

Objective:To comprehend the clinical characteristics and treatment approaches for children and adolescents in Qinghai Province with two types of echinococcosis,cystic echinococcosis(CE)and alveolar echinococcosis(AE).Methods:A total of 128 pediatric inpatients with echinococcosis at the People’s Hospital of Qinghai Province and the Clinical Research Institute of Echinococcosis of Qinghai Province between January 2016 and December 2021 were chosen as subjects.Demographic and clinical data were collected,and double data entry was executed using EpiData 3.02.Factors influencing the cure of echinococcosis were analyzed with echinococcosis cure as the dependent variable,employing statistical analysis via SPSS 19.0.Results:Of the cases,35.9%had CE,and 64.1%had AE.Both types were observed in patients of all ages,with the majority aged 13-18.The number of cysts and their sizes varied between CE and AE.Complications were prevalent,including liver,gallbladder,lung,and nutritional complications.Univariate analyses revealed significant differences in outcomes based on factors such as cyst size(for CE),liver function grade(for AE),hydatid hypersensitivity test,operation,and length of hospital stay(P<0.05).Conclusion:This comprehensive analysis of hospitalized cases sheds light on the clinical data of echinococcosis in children and adolescents in Qinghai Province.The findings contribute to a scientific foundation for formulating effective prevention and control measures tailored to this demographic,facilitating an improved understanding of echinococcosis in Qinghai province. 展开更多

Methods: The clinical data of 7 children and adolescents with differentiated thyroid carcinoma were retrospectively analyzed, and the clinicopathologic features of differentiated thyroid carcinoma were analyzed by gender, tumor size and BRAF mutation. Results: There were 7 cases of thyroid papillary carcinoma. The mean age of patients was (18.71 ± 2.75), and the mean tumor diameter was (2.4 ± 1.04) cm. Lymph node metastasis rate was 100% (7/7). In children and adolescents, the lesion volume was larger, membrane invasion and vascular cancer thrombus were more likely to occur, BRAF mutation was less common, and the difference was statistically significant. Conclusion: Children and adolescents with differentiated thyroid carcinoma are more aggressive and prone to membrane invasion and lymph node metastasis;BRAF mutation is less common than in adults. 展开更多

Background: The COVID-19 epidemic is a public health security event affecting the whole country, which has a certain impact on the development and current situation of the health field. As a special group, children and adolescents need to receive attention and special care. Purpose: To study the health status of children and adolescents under the influence of COVID-19. Method: To collect and analyze the physical, mental and mental health data of 448 children aged 3 - 18 years in the Pediatric Medical Center of the Second Hospital of Jilin University since the COVID-19 outbreak, so as to grasp the physical, mental and mental health status and existing problems of children and adolescents under the impact of COVID-19 outbreak. Results: Food (seafood, mangoes, dairy products, animal protein, soy, peanuts, etc.) is the main allergen in the children and adolescents who participated in the survey, followed by inhalation (pollen, cat and dog hair, etc.). The diet was relatively normal, but there were still unhealthy eating behaviors, the detection rate was 5.36%. High calorie diet, high cholesterol diet, high salt diet, low protein diet, liquid diet in order from high to low, 85.27% of the normal water intake, drinking behavior was greatly affected by the family environment and school environment. The main problem affecting children’s digestion is constipation. Functional constipation accounts for more than 90% of children’s constipation, and its causes are mainly genetic factors, metabolic factors, mental factors, diet factors, etc. Genetic factors and unreasonable diet are the causes of children’s research results show that among 448 children and adolescents, 425 are in normal mental conditions. 94.87%;13, or 2.9%, were in the doldrums;There were 10 people in the state of hyperactivity, accounting for 2.23%, the main cause of functional. Conclusion: The health cloud platform was used to manage and analyze the data, in order to lay the foundation for the healthy growth and development of children and adolescents as well as the whole life cycle management of individual health. 展开更多

Objective To examine increases in average height among Chinese children and adolescents.Methods The data were obtained from the China Health and Nutrition Survey conducted during the period 1989–2015. A stratified multistage cluster sampling method was utilized to select participants aged 2–22 years in each province. Linear regression was used to examine the effects of age, birth cohort, and survey period on height.Results A total of 15,227 males and 13,737 females were included in the final analysis. Age(A) showed a continuous effect on height. The average heights of the investigated groups increased continuously during the investigation period. By 2015, the average height of the overall group increased by 7.87 cm compared to the average height during the 1989 survey. Moreover, birth year(cohort, C) also had a stable effect on height. Using the height of individuals born in or before 1975 as a reference, the average height of each birth cohort increased in comparison to the previous birth cohort.Conclusions The height of Chinese children and adolescents was affected by age, period, and cohort effects, and this effect is governed by certain rules. The age-period-cohort model can be used to analyze the trends of children’s and adolescent’s heights. The findings provide a scientific basis for the formulation of children’s and adolescents’ growth and development policies in China. 展开更多

BACKGROUND Irritable bowel syndrome(IBS)is a highly prevalent gastrointestinal disorder in children and adults,which increased over the past twenty years.The Mediterranean diet is a well-known diet full of antioxidants and anti-inflammatory ingredients.AIM To evaluate the safety,tolerability,and effects of adherence to the Mediterranean diet on disease patterns in children and adolescents with IBS.METHODS This prospective,cross-sectional case-controlled study included 100 consecutive IBS patients diagnosed according to Rome IV criteria,aged 12-18 years.Patients Group II on their regular diet for six months.Besides IBS scores(IBS-SSS,IBS-QoL,and total score),different clinical and laboratory parameters were evaluated at the start and end of the study.RESULTS The Mediterranean diet was safe and well-tolerated in IBS patients.IBS children and adolescents with good adherence to the Mediterranean diet(KIDMED Score≥8 points);group I showed significant improvement in IBS scores.IBS-SSS in the Mediterranean diet group was 237.2±65 at the beginning of the study and decreased to 163.2±33.8 at the end of the study(P<0.001).It did not show a significant improvement in the group with a regular diet(248.3±71.1 at the beginning of the study compared to 228.5±54.3 at the study end with P<0.05).The mean IBS-SSS in the Mediterranean diet group significantly improved compared with the group with a regular diet.Mean IBS-QoL in group I improved from 57.3±12.9 at the start of the study to 72.4±11.2 at the study end(P<0.001)and significantly improved when compared to its level in group II at the study end(59.2±12.7 with P<0.001),while group II showed no significant improvement in IBSQoL at the study end when compared to the beginning of the study(59.2±11.7 with P>0.05).The mean total IBS score in group I became 28.8±11.2 at the end of our study compared to 24.1±10.4 at the start(P<0.05)and significantly improved when compared to its level in group II at the end of the study(22.1±12.5 with P<0.05),while in group II,non-significant improvement in the total score at the end of our study compared to its mean level at the start of the study(22.8±13.5 with P>0.05).CONCLUSION The Mediterranean diet was safe and associated with significant improvement in IBS scores in children and adolescent patients with IBS. 展开更多

The aim of this study was to describe the performance on a decision-making task of children and adolescents (aged 7 - 17 years) from a school in the city of Sogamoso. This non-experimental descriptive, cross-sectional study was conducted by means of a non probability sampling of type cases. The sample consisted of 325 subjects, to whom the Hungry Donkey Task in its version adapted for Colombia was applied. Through descriptive statistics, the test performance trend was established, finding that participants aged 7 to 11 years had better performance than adolescents 12 to 14 years old, which partially supports the hypothesis of decision-making development in J shape. Subjects aged 15 to 17 years had the most disadvantageous performance, which may indicate immaturity and slow development of prefrontal areas and the influence of the trend towards risk-taking, characteristic of this age range. 展开更多

Pediatric and adult spinal cord injuries (SCI) are distinct entities.Children and adolescents with SCI must suffer from lifelong disabilities,which is a heavy burden on patients,their families and the society.There are differences in Chinese and foreign literature reports on the incidence,injury mechanism and prognosis of SCI in children and adolescents.In addition to traumatic injuries such as car accidents and falls,the proportion of sports injuries is increasing.The most common sports injury is the backbend during dance practice.Compared with adults,children and adolescents are considered to have a greater potential for neurological improvement.The pathogenesis and treatment of pediatric SCI remains unclear.The mainstream view is that the mechanism of nerve damage in pediatric SCI include flexion,hyperextension,longitudinal distraction and ischemia.We also discuss the advantages and disadvantages of drugs such as methylprednisolone in the treatment of pediatric SCI and the indications and timing of surgery.In addition,the complications of pediatric SCI are also worthy of attention.New imaging techniques such as diffusion tensor imaging and diffusion tensor tractography may be used for diagnosis and assessment of prognosis.This article reviews the epidemiology,pathogenesis,imaging,clinical characteristics,treatment and complications of SCI in children and adolescents.Although current treatment cannot completely restore neurological function,patient quality of life can be enhanced.Continued developments and advances in the research of SCI may eventually provide a cure for children and adolescents with this kind of injury. 展开更多

Tuberculosis screening among children and adolescents in China:insights from a mathematical model

Background Tuberculosis(TB)continues to be prevalent in China also among children and adolescents in China.We built a dynamic mathematical model for TB transmission in China,and applied it to compare the epidemic trends 2021–2030 under a range of screening interventions focusing on children and adolescents.Methods We developed a dynamic mathematical model with a flexible structure.The model can be applied either stochastically or deterministically,and can encompass arbitrary age structure and resistance levels.In the present version,we used the deterministic version excluding resistance but including age structure with six groups:0–5,6–11,12–14,15–17,18–64,and 65 years and above.We parameterized the model by literature data and fitting it to case and death estimates provided by the World Health Organization.We compared the new TB cases and TB-related deaths in each age group over the period 2021–2030 in 10 scenarios that involved intensified screening of particular age groups of children,adolescents,or young adults,or decreased or increased diagnostic accuracy of the screening.Results Screening the entire age class of 18-year-old persons would prevent 517,000 TB cases and 14,600 TB-related deaths between years 2021 and 2030,corresponding to 6.6%and 5.5%decrease from the standard of care projection,respectively.Annual screening of children aged 6–11 and,to a lesser extent,0–5 years,also reduced TB incidence and mortality,particularly among children of the respective ages but also in other age groups.In contrast,intensified screening of adolescents did not have a major impact.Screening with a simpler and less accurate method resulted in worsened outcomes,which could not be offset by more intensive screening.More accurate screening and better sensitivity to detect latent TB could prevent 2.3 million TB cases and 68,500 TB deaths in the coming 10 years.Conclusion Routine screening in schools can efficiently reduce the burden of TB in China.Screening should be intensified particularly among children in primary school age. 展开更多

Prognostic analysis of recurrence in children and adolescents with differentiated thyroid cancer 被引量:3

Background:The standard treatment for pediatric differentiated thyroid carcinoma(DTC)still requires consideration because of a lack of clinical evidence.The purpose of this study was to summarize the clinical experiences and explore the risk factors for postoperative recurrence through a retrospective analysis to develop better clinical strategies for pediatric DTC.Methods:This study retrospectively analyzed children and adolescents with DTC who were treated between January 1999 and December 2014 at the Cancer Hospital,Chinese Academy of Medical Sciences.Clinicopathological results and outcomes were collected.A log-rank test of Kaplan-Meier curves and the Cox regression model were used to determine the factors associated with recurrence.Results:Data of 150 patients were collected in this study.During the follow-up,there was only one disease-related death.The recurrence rates at 3,5,and 10 years were 13.6%,18.7%,and 28.6%,respectively.There was a significant difference in the rate of recurrence according to age(P<0.001),extrathyroidal extension(P<0.001),lymph node metastasis(P=0.023),and invasion of the trachea and esophageal wall(P=0.004).Cox regression analysis demonstrated that age(P=0.006)and extrathyroidal extension(P=0.013)were significant dependent factors of post-operative recurrence.Conclusions:The prognosis of DTC in children and adolescents is favorable.A close follow-up is recommended because of the high recurrence rate.A comparatively higher recurrence rate was observed in the younger age group,and new age-based divisions may be needed to conveniently evaluate the possibility of recurrence. 展开更多

Objective:To investigate the spectrum of diseases in female children and adolescents hospitalized for obstetric and gynecological conditions.Methods:This retrospective study included patients(age≤19 years)admitted to the Obstetrics and Gynecology Hospital of Fudan University,Shanghai,between January 2003 and December 2017.Their medical records-age,principal diagnosis,and treatment-were reviewed and analyzed.Diagnoses were divided into eight disease classifications.Comparison between groups was analyzed with Student’s t-test.Results:This study enrolled 4,628 inpatients.The number of inpatients increased each year,especially in the groups of gynecologic tumors,pregnancy-related conditions,and disorders of the breast.The mean age was 16.33±2.42 years(range:2-19 years).Of all the inpatients,41.9%(n=1,939)were hospitalized for gynecologic tumors,and 85.5%(n=1,657)of the tumors were benign.Altogether,ovarian cysts(28.66%,n=1,325)were the most common disease observed in this group.In addition,32.9%(n=1,524)of the inpatients were hospitalized for pregnancy,childbirth,and puerperium;within this group,21.9%(n=1,014)were hospitalized for artificial abortions.Conclusions:The efforts of all stakeholders toward the implementation of government policies to promote sexual and reproductive health among children and adolescents in China have been effective. 展开更多

Background::Non-transport unintentional injuries(NTUIs)are major public concerns,especially among children and adolescents in low-and middle-income countries.With environmental and cognitive changes,a recent systematic description of global trends and regional differences concerning NTUIs is urgently needed for the global agenda of relevant policy-making and intervention target findings.Methods::We used mortality,population,and socio-demographic-index(SDI)data from Global Burden of Disease 2019 to analyze the trends of NTUIs mortality.We applied the slope index of inequality(SII)and relative index of inequality(RII)to measure the absolute and relative inequality between countries and territories.The concentration curve and concentration index(CI)were also used to measure the inequality.We conducted a sensitivity analysis to make our findings credible.Results::In 2019,there were 205,000 deaths due to NTUIs among children and adolescents aged 5 to 24 years,which decreased from 375,000 in 1990.In 2019,the age-standardized mortality rate(ASMR)was 8.13 per 100,000,ranging from the lowest in the Netherlands(0.90 per 100,000)to the highest in the Solomon Islands(29.34 per 100,000).The low-middle SDI group had the highest ASMR of NTUIs,while the low SDI group had the slowest decrease.After excluding the death caused by"exposure to forces of nature"and"other unintentional injuries",drowning accounted for the most deaths in almost every SDI group,gender,and age group,but the major causes of death varied in different subgroups.For example,animal contact was a major cause in low and low-middle SDI groups but less in high SDI groups,while high and high-middle SDI groups had a higher proportion of deaths for foreign body and poisonings.The SII showed a declining trend,but the RII and CI did not,which might indicate that inequality was persistent.Similar results were found in the sensitivity analysis.Conclusions::Despite the declining trend of the mortality rate and the narrowing gap between countries,there were still a large number of children and adolescents dying from NTUIs,and those experiencing social-economic disadvantages remained at high mortality.Embedding the prevention of NTUIs into sustainable development goals might contribute to the progress of reducing death and inequalities,which ensures that no one is left behind. 展开更多

The association of metabolic syndrome(MS)with arterial compliance in children and adolescents was explored.337 subjects(188 men and 149 women)aged 6–18(10.95±3.01)years,out of“Beijing Child Metabolic Syndrome Study”,were divided into three case groups(one component,two components,three&more components of MS)and one control group based on the Cook’s MS definition in children and adolescents.Measurements including anthropometry,blood pressure,fasting plasma glucose and insulin,serum lipid profile were done.Homeostasis model assessment for insulin resistance(HOMA-IR)index was calculated for estimating individual insulin resistance.Arterial compliance was measured using digital pulse wave analyzing method from the pulse trace machine(Micro medical,London),and then the stiffness index(SI)was determined.The mean value of SI in MS group was significant higher than that in control group[(7.69±1.63)vs(6.25±0.86)m/s,P<0.01].With the increase of the clustering of MS components,SI and HOMA-IR were gradually increased.After taking account of gender,age and pubertal development,the partial correlation analysis showed that the amount of components of MS and HOMA-IR were positively correlated with SI(both P values were less than 0.05).The arterial compliance of MS group was significantly lowered in children and adolescents,and with the increase of the clustering of MS components,arterial compliance was gradually decreased.It was suggested that arterial compliance assessment in children and adolescents was important for early prevention of cardiovascular diseases. 展开更多

Purpose: This study was to present national estimates of participating in moderate-to-vigorous physical activity(MVPA) and adherence to the recommendations of 60 min/day of MVPA among Chinese school-aged children and to assess demographic differences in MVPA.Methods: Cross-sectional analyses of data from the 2016 Physical Activity and Fitness in China—The Youth Study. Participants were 90,712 primary, junior middle, and junior high school children(boy: 47%; girl: 53%), recruited from 1204 rural and urban schools across 32 administrative provinces and regions in the Mainland of China. Main outcomes were(a) average MVPA minutes per day in the previous 7 days by self-reports and(b) percentage meeting MVPA recommendations.Results: Average MVPA time was 45.4 min/day, with boys having more MVPA(47.2 min/day) than girls(43.7 min/day) overall and across the 3 school grade categories. About 30% of participants met MVPA recommendations, with a higher percentage of boys(32%) than girls(28%) overall and across the 3 grades categories. Urban school children outperformed rural children in terms of MVPA time. Overall, boys were more likely to meet MVPA recommendations(adjusted odds ratio(a OR) = 1.19, 95% confidence interval(CI): 1.16–1.22) compared with girls; children in higher grades(junior middle(a OR = 0.92, 95%CI: 0.87–0.98) and junior high(a OR = 0.59, 95%CI: 0.53–0.66)) were less likely to meet recommendations compared with primary school children. The odds of meeting recommendations did not differ between urban and rural children(p = 0.07),but urban boys were found to be more likely to meet recommendations compared with rural boys(a OR = 1.14, 95%CI: 1.06–1.19).Conclusion: Overall, the average MVPA minutes per day among Chinese school-aged children is low, and less than one-third of them meet MVPA recommendations. These results were most evident among junior middle and junior high school children and those living rural areas. 展开更多

Severe acute respiratory syndrome coronavirus 2 infection was declared a pandemic in January 2020.Since then,several measures to limit virus transmission have been imposed;among them,home confinement has been the most severe,with drastic changes in the daily routines of the general population.The“stay at home”rule has impaired healthcare service access,and patients with chronic conditions were the most exposed to the negative effects of this limitation.There is strong evidence of the worsening of obesity and diabetes mellitus in children during this period.To overcome these issues,healthcare providers have changed their clinical practice to ensure follow-up visits and medical consultation though the use of telemedicine.Telemedicine,including telephone calls,videocalls,data platforms of shared telemedicine data platforms mitigated the negative effect of pandemic restrictions.Published evidence has documented good metabolic control and weight management outcomes in centers that performed extensive telemedicine services last year during the pandemic.This review discusses studies that investigated the use of telemedicine tools for the management of pediatric obesity and diabetes. 展开更多
