New technologies as a strategy to decrease medication errors: how do they affect adults and children differently?

Background:Medication error can occur throughout the drug treatment process,with special relevance in children given the risk of adverse effects resulting from a medication error is more prevalent than in adults.The significance of medication error in children is also greater because small error that would be tolerated in adults can cause significant damage in children.Moreover,the likelihood of injury is higher than in adults.Data sources:Based on the data published,most medication errors take place in prescribing and administration stages in both populations.Taking in account that child's risk factors are different from those of adults,with some specific causes to pediatrics,we have reviewed available data about new technologies as a strategy to reduce pediatric medication errors.Results:Even though there is a lack of standardized definitions and terminology that makes studies difficult to compare,we checked that new technologies have proven to be effectives in reducing medication errors,mainly computerized physician order entry(CPOE)and platforms to aid decision-making.However,we also observed that the use of these informatic tools can also generate new errors.Conclusions:Implementation of CPOE programs for pediatrics,communication improvement between healthcare professionals taking care of admitted children and the knowledge of these programs should be the mayor priorities for the safety of hospitalized children. 展开更多

The introduction of new surgical techniques and technologies has traditionally been unregulated.In many settings surgeons frequently adopt novel procedures without following a structured program of implementation or supervision.The appearance of innovative technology played a pivotal role in the advancement of new surgical techniques during the industrial revolution.Innovation has been an essential component of surgical development,which led to contemporary surgical techniques such as minimally invasive surgery.Different initiatives have been developed to guide the safe introduction of new surgical techniques and other procedures.Those include comprehensive concepts such as the Idea,Development,Exploration,Assessment,Long-term study framework,which could be particularly relevant when reflecting on the novel transanal total mesorectal excision(taTME),introduced a decade ago.This relatively novel and complex procedure promised to overcome some of the major limitations of traditional surgical approaches for rectal cancer.According to the Idea,Development,Exploration,Assessment,Long-term study framework,taTME is in the phase of exploration,where there is an existing and increasing number of reports being published as the experience grows.The current management of rectal cancer is in a state of radical evolution,with multiple options that were not previously available.Ta TME is only one technique amongst many which could be part of a rectal cancer surgeon’s armamentarium;however,it requires further rigorous study and evaluation. 展开更多

A new process was used for producing FeAl alloy pow de rs with double consumable rotating electrodes and the powders made in this appar atus were analyzed. In this new technology, tungsten rod serves as a cathode ele ctrode, while the alloy rod as an anode electrode. The conventional rotating ele ctrode process must have an anode with pre-melting alloys; however, in this new process, using pure iron as cathode electrode and pure aluminum as anode electr ode can eliminate the step of pre-melting. The effects of process variables, which include electrode rotational speed, a nd electrode diameter of the mean particle diameter were determined. Results showed that both the rotational speed and diameter of electrodes would a ffect the mean diameter of particles. There are three kinds of powders with diff erent composition produced in this study and the possible mechanisms are discuss ed. The process parameters and volume mean diameter of the powders have been cor related to find an experimental equation. The results show that when the rotational speed and the diameter of the anode el ectrode are increased, the powders size will decrease. However, the powders size will increase with cathode electrode. 展开更多
