Methods A total of 2 086 cases of kidneys for transplantation from Angust 1980 to December ...Objective To summarize the experience of 20 years in using hypertonic citrate adenine(HC-A) for clinical kidney preservation. Methods A total of 2 086 cases of kidneys for transplantation from Angust 1980 to December 2000 were reported. The donors’ age ranged from 18 to 49 years. The cadaveric kidneys were harvested with en-bloc method, and were perfused via renal arteries with 250 ml HC- A at 2 - 4 ℃ and 10 cm H2O, and then stored in HC- A at 2 - 4 ℃ throughout preservation. Serum creatinine was measured at 1 week and 1 month after transplantation. Graft survival rates at 1 year and 3,5,10 years and mean graft survival periods were followed up. According to the cold ischemic time, the cases were divided into 3 groups; Group 1, n = 573, with cold ischemic time less than 12 h; Group 2, n = 779, with 12 to 24 h; and Group 3, n = 734, with more than 24 h. Nursing students have taken advantage of the flexible scheduling and variety of online nursing programs to pursue their education due to distance, family, or work demands;however, since the outbreak of COVID-19, nursing schools in the United States have adapted by implementing an online alternative to assess students’ clinical skills required to graduate. Nursing faculty have been forced into this new learning culture, navigating the online landscape, learning new technologies and teaching methods, and adapting to clinical social distance education. Nursing is a performance-based profession in which the clinical learning environment plays an important role in developing skills and professional abilities. The purpose of this evidence-based teaching project is to, based on National League for Nursing (NLN), not only contribute to improvement but ensure educational quality for undergraduate nursing students by examining how online simulation compared to clinical experience with an actual patient, affects nurses’ clinical competence. Therefore, after reviewing 14 peer-reviewed articles, evidence shows benefits for undergraduate nursing students to retain better clinical competence when virtual simulation and clinical sites are combined;however, further research is recommended. Articles were selected as the evidence base for this project, following specific inclusion criteria. Inclusion criteria for evidence were articles within six years, including undergraduate nurse students with online clinical education;the exclusion criteria were studies including graduate nurse students and articles older than six years. Based on the project’s outcome, it is suggested that a nurse residency should follow online simulations to complete clinical hours. .展开更多

The underlying anatomical and pathophysiological changes of ODS are complex. However, intra-rectal intussusception and rectocele are frequently found in patients with ODS and both are thought to play an important role in the pathogenesis of ODS. With the development of evaluation methods in anorectal physiology laboratories and radiology studies, a great variety of new operative procedures, especially transanal procedures, have been invented to treat ODS. However, no procedure has been proved to be superior to others at present. Each operation has its own merits and defects. Thus, choosing appropriate transanal surgical procedures for the treatment of ODS remains a challenge for all surgeons. This review provides an introduction of the current problems and options for treatment of ODS and a detailed summary of the essential assessments needed for patient evaluation before carrying out transanal surgery. Besides, an overview of the benefits and problems of current transanal surgical procedures for treatment of ODS is summarized in this review. However, intra-rectal intussusception and rectocele are frequently found in patients with ODS and both are thought to play an important role in the pathogenesis of ODS. With the development of evaluation methods in anorectal physiology laboratories and radiology studies, a great variety of new operative procedures, especially transanal procedures, have been invented to treat ODS. However, no procedure has been proved to be superior to others at present. Each operation has its own merits and defects. Thus, choosing appropriate transanal surgical procedures for the treatment of ODS remains a challenge for all surgeons. This review provides an introduction of the current problems and options for treatment of ODS and a detailed summary of the essential assessments needed for patient evaluation before carrying out transanal surgery. Besides, an overview of the benefits and problems of current transanal surgical procedures for treatment of ODS is summarized in this review. A report of clinical experience of some transanal surgical techniques used in the authors' center is also presented.

文摘In recent years,due to the increasingly accelerated pace of life,the incidence of hyperthyroidism is increasing year by year,which has a serious impact on the quality of life of patients.Traditional Chinese Medicine has a unique effect on hyperthyroidism,Professor Zhang Ning,my tutor,who has been devoting to the clinical study of traditional Chinese medicine in the prevention and treatment of thyroid diseases for a long time,has rich theoretical knowledge as well as clinical practice experience.She's also good at using classical prescriptions for syndrome differentiation treatment,and has achieved excellent clinical efficacy.Professor Zhang Ning summarized the diagnosis and treatment idea of"treating from the liver"and ran it throughout the whole treatment process.For the patients prone to abnormal liver function,it is advocated to focus on protecting the liver in terms of medication,increase the effect of anti-hyperthyroidism,and also obtain satisfactory efficacy.

文摘The current epidemic situation of the COVID-19 is still serious.As a designated unit for the diagnosis of COVID-19 in Hainan Province,We,The Second Affiliated Hospital of Hainan Medical University,established a multidisciplinary integrated individualized treatment team.We emphasized early improvement of relevant examinations,early antiviral treatment,dynamic monitoring of inflammatory markers and imaging changes.In addition,nutritional support,psychological intervention,and Chinese medicine treatment also play an important role.We summarized the experience in the diagnosis and treatment process for the reference of clinicians.
