Macromolecular inhibitors of malarial cysteine proteases —An invited review

There are evidences indicating that cysteine proteases play an essential role in malaria parasites;therefore, an obvious area of investigation is the inhibition of these enzymes to treat malaria. Small cysteine protease inhibitors of malaria are well studied, but macromolecular nature of inhibitor is a new field to explore. In malarial cysteine proteases, there are macromolecular endogenous inhibitors playing important roles in regulation of the cysteine protease activity of parasite and host. Recent studies suggested that there are known and characterized endogenous inhibitors like falstatin present in P. falciparum, PbICP (inhibitor of cysteine protease in P. berghei), PyICP (inhibitor of cysteine protease in P. yoelli), and other macromolecular inhibitors which are the prodomain of enzyme itself regulating the activity of the mature enzyme. All the known macromolecular endogenous inhibitors are using specific loop-like structure to interact with malarial cysteine proteases. The majority of macromolecular inhibitors are competitive in nature, and block access to the active site of their target protease, but do not bind in a strictly substrate-like manner. They rather interact with the protease subsites and catalytic residues in a non-catalytically competent manner. In future, designing inhibitors based on these protein-protein interactions will be a new approach in the field of malaria. Since macromolecular inhibitors can gain potency through the burial of a large surface area and specificity through contacts with secondary binding sites critical for inhibition, and could be less prone to drug resistant mutation. 展开更多

[Objective] This study aimed to clone and analyze the cysteine proteinase inhibitor gene from seedless litchi. [Method] According to the EST sequence of cysteine proteinase inhibitor in constructed SSH subtractive library of seedless litchi abortion, nucleotide sequence of the cysteine proteinase inhibitor gene was obtained by using RACE technology and analyzed by using bioinformatics software. [Result] A cysteine protease inhibitor gene was obtained with the sequence of 635 bp containing a 321 bp open reading frame. It was predicted that the encoded protein contained 106 amino acids with conserved domain of cysteine proteinase inhibitor and had relatively high homology with the cysteine proteinase inhibitor gene of several species. [Conclusion] This study laid the foundation for further exploring the physiological functions of this cysteine proteinase inhibitor gene in plants. 展开更多

老年高危心血管病患者应用依洛尤单抗的有效性及安全性研究

目的探讨老年高危心血管病患者应用依洛尤单抗治疗的有效性和安全性。方法纳入2019年11月至2022年11月在解放军总医院第一、二、六、八医学中心心内科住院的经常规他汀类药物治疗后血脂控制不达标的心血管病高危患者153例,根据患者年龄... 目的探讨老年高危心血管病患者应用依洛尤单抗治疗的有效性和安全性。方法纳入2019年11月至2022年11月在解放军总医院第一、二、六、八医学中心心内科住院的经常规他汀类药物治疗后血脂控制不达标的心血管病高危患者153例,根据患者年龄分为非老年组(<60岁)46例,60~74岁老年组66例,≥75岁老年组41例,均按指南应用依洛尤单抗治疗;选取同期在解放军总医院第一、二、六、八医学中心心内科住院血脂控制不达标的年龄≥75岁心血管病高危未应用依洛尤单抗患者50例为常规治疗组,应用一种他汀类药物联合依折麦布治疗。比较各组患者临床基线资料及用药第4、12周的血液指标和12周内药物不良反应及主要不良心血管事件(MACE)发生情况。结果非老年组、60~74岁老年组和≥75岁老年组用药第4、12周低密度脂蛋白胆固醇(LDL-C)和总胆固醇(TC)水平较基线降低,差异有统计学意义(P<0.05,P<0.01);3组用药第12周LDL-C、TC水平比较无显著差异(P>0.05)。非老年组、60~74岁老年组、≥75岁老年组用药12周内不良事件发生率比较无显著差异(2.2%vs 3.0%vs 2.4%,P>0.05)。≥75岁老年组和常规治疗组用药第12周LDL-C、TC水平显著低于基线(P<0.05,P>0.01);≥75岁老年组用药第12周LDL-C水平显著低于常规治疗组[(1.36±0.44)mmol/L vs(1.87±0.56)mmol/L,P<0.01]。≥75岁老年组和常规治疗组用药12周内MACE发生率比较无显著差异(12.2%vs 16.0%,P>0.05),2组生存率比较无显著差异(P=0.576)。结论各年龄组患者应用依洛尤单抗治疗均可在短期内取得良好疗效,75岁以上老年患者应用依洛尤单抗治疗同样具有良好的有效性和安全性。 展开更多

目的探究肺癌患者血清乳酸脱氢酶(LDH)、半胱氨酸蛋白酶抑制剂C(CysC)、血小板/白细胞比值(PWR)水平检测意义。方法2023年1月至2023年6月选取常州市第四人民医院72例肺癌患者设为观察组,另选取同期肺部良性病变患者72例设为对照组,比较2... 目的探究肺癌患者血清乳酸脱氢酶(LDH)、半胱氨酸蛋白酶抑制剂C(CysC)、血小板/白细胞比值(PWR)水平检测意义。方法2023年1月至2023年6月选取常州市第四人民医院72例肺癌患者设为观察组,另选取同期肺部良性病变患者72例设为对照组,比较2组LDH、CysC、PWR及各项肿瘤标志物[癌胚抗原(CEA)、神经元特异性烯醇化酶(NSE)、细胞角蛋白片段19(CYFRA21-1)],分析LDH、PWR、CysC与各项肿瘤标志物的相关性;比较不同病理特征肺癌患者LDH、CysC、PWR水平,分析肺癌患者LDH、PWR、CysC水平与病理特征的相关性,评价LDH、CysC、PWR结合CEA、CYFRA21-1、NSE对肺癌的诊断价值。结果观察组PWR低于对照组,LDH、CysC、CEA、CYFRA21-1及NSE高于对照组(P<0.05)。肺癌患者PWR与肿瘤标志物CEA、CYFRA21-1、NSE呈负相关(P<0.05),LDH、CysC与肿瘤标志物CEA、CYFRA21-1、NSE呈正相关(P<0.05)。病理类型方面:肺腺癌与肺鳞癌患者LDH、CysC、PWR比较,差异无统计学意义(P>0.05);临床分期方面:Ⅰ~Ⅱ期患者PWR高于Ⅲ~Ⅳ期患者,LDH、CysC低于Ⅲ~Ⅳ期患者(P<0.05);有无淋巴结转移方面:无淋巴结转移患者PWR高于有淋巴结转移患者,LDH、CysC低于有淋巴结转移患者(P<0.05)。肺癌患者LDH、PWR、CysC与病理类型无关(P>0.05),PWR与临床分期、淋巴结转移呈负相关(P<0.05),LDH、CysC与临床分期、淋巴结转移呈正相关(P<0.05)。(ROC)曲线显示,LDH、CysC、PWR联合诊断肺癌的曲线下面积(AUC)值为0.910,CEA、CYFRA21-1及NSE联合诊断AUC值为0.923,进一步采用LDH、CysC、PWR结合CEA、CYFRA21-1、NSE诊断AUC值最高,为0.940,敏感度、特异度分别为93.06%、87.33%。结论肺癌患者PWR下降,LDH、CysC水平升高,且其水平与肿瘤标志物、临床分期及淋巴结转移密切相关,可辅助肿瘤标志物对肺癌进行诊断,以期提高早期肺癌诊断率。 展开更多

AIM: Cysteine peptidase (CP) and its inhibitor (CPI) are a matrix protease that may be associated with colorectal carcinoma invasion and progression, and vitamin E is also a stimulator of the immunological system. Our purpose was to determine the correlation between the expression of cysteine peptidases and their endogenous inhibitors,and the level of vitamin E in sera of patients with colorectal cancer in comparison with healthy individuals.METHODS: The levels of cysteine peptidases and their inhibitors were determined in the sera of patients with primary and metastatic colorectal carcinoma and healthy individuals using fluorogenic substrate, and the level of vitamin E was determined by HPLC.RESULTS: The levels of cysteine peptidases and their inhibitors were significantly higher in the metastatic colorectal cancer patients than that in the healthy controls (P<0.05).The activity of CP increased 2.2-fold, CPI 2.8-fold and vitamin E decreased 3.4-fold in sera of patients with metastasis in comparison with controls. The level of vitamin E in healthy individuals was higher, whereas the activity of cysteine peptidases and their inhibitors associated with complexes was lower than that in patients with cancer of the digestive tract.CONCLUSION: These results suggest that the serum levels of CP and their inhibitors could be an indicator of the prognosis for patients with metastatic colorectal cancer. Vitamin E can be administered prophylactically to prevent digestive tract neoplasmas. 展开更多

AIM: To examine the effectiveness of human placental inhibitors, by injecting vitamin E to rats with transplanted Norris-5123 hepatoma, on the expression of cathepsins B and L in tumor, liver, lung and blood sera after transplantation of Norris 5123 hepatoma.METHODS: Animals were divided into 10 groups receiving three different concentrations of vitamin E and inhibitors along or in combination and compared with negative control (healthy rats) and positive control (tumor rats). Effectiveness of treatment was evaluated with regard to survival time,tumor response and determination of the activities of proteolytic enzymes and their inhibitors using flurogenic substrates.RESULTS: Cathepsins B and L activities were elevated by 16-fold in comparison with negative control tissues, and their endogenous inhibitor activity decreased by 1.2-fold before treatment. In several cases, tumors completely disappeared following vitamin E plus human placental cyteine protease inhibitor (CPI) compared with controls.The number of complete tumor responses was higher when 20 m/kg vitamin E plus 400 μg of CPI was used, i.e.7/10 rats survived more than two mo. Cathepsins B and L were expressed significantly in tumor, liver, lung tissues and sera in parallel to the increasing of the endogenous inhibitor activity compared with the controls after treatment (P<0.0001).CONCLUSION: The data indicate formation of metastasis significantly reduced in treated rats, which might provide a therapeutic basis for anti-cancer therapy. 展开更多

The study was aimed at the evaluation of the effects of breed, age, different digestion stimulators, and dietary crude protein (CP) level on the activities of proteolytic enzymes in pancreatic tissue and duodenal chymus (in vivo), serum trypsin and α1-proteinase inhibitor (A1PI) concentrations in meat-type chicks. The study of age dynamics of trypsin and A1PI concentrations was performed on the chicks of hybrid cross “Smena-8”and two parental lines (Plymouth Rock and Cornish) fed standard commercial corn-wheat-SBM diets. Twenty birds per breed were euthanized at 1, 7, 14, 21, 28 and 35 days of age to obtain blood samples and pancreatic homogenate. Experiments on the effects of different digestion promotors (probiotic, acidifier, phytobiotic, enzymatic preparation) and different CP levels (finisher diet, CP 20%, vs. ground corn, CP 8.5%) were performed on 12 hybrid chicks with fistulated duodenum from 14 to 50 days of age. The following conclusions were made: 1) At 1 day of age high proteolytic activity in pancreatic tissue and maximal serum concentrations of trypsin and A1PI were found in both hybrid and parental lines. Since 7 to 35 days of age A1PI concentration was nearly constant, serum trypsin concentration decreased while proteolytic activity in pancreatic tissue exhibited undulate increase;2) Proteolytic activity in pancreatic tissue was higher in hybrids compared to the parental lines from 7 to 35 days of age (p 0.05);3) Supplementation of diet with exogenous enzymes stimulated the digestion due to the increase in protease activity in duodenal chymus by 9.1% compared to unsupplemented control (p 0.05);4) Proteolytic activity in duodenal chymus significantly responded to the substitution of ground corn for the complete diet by 2-fold decrease while serum trypsin concentration responded by 2.5-fold increase (p 0.001). 目的:探讨人血清半胱氨酸蛋白酶抑制剂S(CST4)、鳞状上皮细胞癌抗原(SCC)、糖类抗原19-9(CA19-9)、可溶性细胞角蛋白组分(CYFRA21-1)在口腔颌面部恶性肿瘤中的诊断价值。方法:选取2022年1—11月于河北医科大学第四医院治疗的232例口腔... 目的:探讨人血清半胱氨酸蛋白酶抑制剂S(CST4)、鳞状上皮细胞癌抗原(SCC)、糖类抗原19-9(CA19-9)、可溶性细胞角蛋白组分(CYFRA21-1)在口腔颌面部恶性肿瘤中的诊断价值。方法:选取2022年1—11月于河北医科大学第四医院治疗的232例口腔颌面部恶性肿瘤患者、49例口腔颌面部良性肿瘤患者以及同时期的40例健康体检者为研究对象,分析上述3组人群血清CST4、SCC、CA19-9和CYFRA21-1表达水平,制作ROC曲线评价不同指标对口腔颌面部恶性肿瘤的诊断效能。结果:口腔颌面部恶性肿瘤组的血清CST4、SCC、CYFRA21-1中位数水平明显高于良性肿瘤组和健康对照组,差异均具有统计学意义(P<0.05)。不同部位及不同病理类型口腔颌面部恶性肿瘤患者血清CST4的表达水平无明显差异(P>0.05)。ROC曲线显示血清CST4检测对口腔颌面部恶性肿瘤的诊断价值较高(AUC=0.954,敏感度为97.5%,特异度为78.45%,截断值为74.68 ng/mL),且CST4、SCC和CYFRA21-1三者联合检测对口腔颌面部恶性肿瘤的临床诊断效能更高。结论:血清CST4、SCC和CYFRA21-1在口腔颌面部恶性肿瘤中均高表达,与单独检测血清肿瘤标志物相比,三者联合检测对口腔颌面部恶性肿瘤的诊断具有较高的参考价值,可推荐作为口腔颌面部恶性肿瘤辅助诊断的肿瘤标志物。 展开更多

目的探讨抗菌肽Cathelicidin-PY对脂多糖(LPS)/D-氨基半乳糖(D-GalN)诱导的急性肝衰竭(ALF)小鼠的保护作用及其可能的作用机制。方法取60只C57BL/6小鼠,随机分为对照组、模型组、小剂量Cathelicidin-PY干预组(1.0 mg.kg^(-1))和大剂量Ca... 目的探讨抗菌肽Cathelicidin-PY对脂多糖(LPS)/D-氨基半乳糖(D-GalN)诱导的急性肝衰竭(ALF)小鼠的保护作用及其可能的作用机制。方法取60只C57BL/6小鼠,随机分为对照组、模型组、小剂量Cathelicidin-PY干预组(1.0 mg.kg^(-1))和大剂量Cathelicidin-PY干预组(3.0 mg.kg^(-1)),每组15只。采用LPS/D-GalN腹腔联合注射构建小鼠ALF模型,其中干预组在注射LPS/D-GalN前2 h经尾静脉注射Cathelicidin-PY。在造模后5 h,每组取5只小鼠采集血标本和肝组织,记录剩余小鼠24 h存活情况。采用ELISA法检测血清肿瘤坏死因子α(TNF-α)和白细胞介素1β(IL-1β)水平,采用TUENL染色法检测肝细胞凋亡,采用Western blot法检测肝组织半胱氨酸天冬氨酸蛋白酶3剪切体(cleaved caspase-3)蛋白表达。结果小剂量和大剂量Cathelicidin-PY干预组小鼠24 h存活率为40%(4/10)和60%(6/10),均显著高于模型组的10.0%(1/10,P<0.05);模型组小鼠血清TNF-α和IL-1β水平分别为(1069.4±178.0)pg/mL和(1354.1±162.9)pg/mL,显著高于对照组【分别为(140.7±33.6)pg/mL和(436.2±46.8)pg/mL,P<0.05】,而小剂量和大剂量Cathelicidin-PY干预组血清TNF-α和IL-1β水平均显著低于模型组(P<0.05);模型组小鼠肝细胞凋亡数量和Cleaved caspase-3蛋白表达均较对照组显著增多或增强(P<0.05),而小剂量和大剂量Cathelicidin-PY干预组肝细胞凋亡数量和Cleaved caspase-3蛋白表达均较模型组显著减少或减弱(P<0.05)。结论Cathelicidin-PY能通过减轻炎症反应和抑制肝细胞凋亡,改善LPS/D-GalN诱导的ALF小鼠肝损伤,降低动物死亡率。 展开更多

CST4在胃癌人群检测中的临床意义

目的探讨人半胱氨酸蛋白酶抑制剂S(CST4)在胃癌人群检测中的临床意义。方法选取青岛大学附属医院2019年1月至2019年12月入院就诊的104例胃癌患者为胃癌组,44例胃良性病变患者(包括胃息肉和胃炎患者)设为胃良性病变组,另选取在门诊就诊... 目的探讨人半胱氨酸蛋白酶抑制剂S(CST4)在胃癌人群检测中的临床意义。方法选取青岛大学附属医院2019年1月至2019年12月入院就诊的104例胃癌患者为胃癌组,44例胃良性病变患者(包括胃息肉和胃炎患者)设为胃良性病变组,另选取在门诊就诊的56名健康体检者为对照组。通过酶联免疫吸附试验比较各实验组的CST4检测水平,分析胃癌组患者CST4与临床病理参数关系以及CST4与癌胚抗原(CEA)、糖类抗原19-9(CA19-9)、糖类抗原242(CA242)、糖类抗原125(CA125)、糖类抗原72-4(CA72-4)检测水平的差异。结果胃癌组中CST4的中位数水平明显高于胃良性病变组和对照组,差异有统计学意义(P<0.05);胃癌组中CST4的检测水平,男性与女性比较,差异无统计学意义(P>0.05);年龄<61岁与≥61岁的患者比较,肿瘤分化程度低与肿瘤分化程度中/高的患者比较,TNM分期为Ⅰ/Ⅱ与TNM分期为Ⅲ/Ⅳ的患者比较,差异均有统计学意义(P<0.05)。CST4的阳性检测率明显高于CEA、CA19-9、CA242、CA125、CA72-4单项检测和联合检测,CST4与上述相关肿瘤标志物联合检测阳性率最高,差异有统计学意义(P<0.05)。CST4分别与CEA、CA19-9、CA242、CA125、CA72-4比较,差异有统计学意义(P<0.05)。CST4、CA125+CA19-9+CA242+CA72-4+CEA、CST4+CA125+CA19-9+CA242+CA72-4+CEA三组比较,差异有统计学意义(P<0.05)。CST4诊断胃癌最优的阈值浓度为104.02U/mL,对应的灵敏度和特异度分别为0.529和0.733。结论血清CST4对青岛地区胃癌患者早期诊断具有较好的预测作用;血清CST4可作为胃癌潜在的肿瘤标志物,CST4联合CEA、CA19-9、CA242、CA125、CA72-4对胃癌的血清学诊断效果更高,具有较高的临床价值。 展开更多

目的 观察人重组骨形态发生蛋白-7(recombinant human bone morphogenetic protein-7,rhBMP-7)对高糖诱导的人肾皮质近曲小管上皮细胞(human kidney-2,HK-2)的焦亡和炎症反应的作用。方法 体外培养高糖(high glucose,HG)刺激的HK-2细胞... 目的 观察人重组骨形态发生蛋白-7(recombinant human bone morphogenetic protein-7,rhBMP-7)对高糖诱导的人肾皮质近曲小管上皮细胞(human kidney-2,HK-2)的焦亡和炎症反应的作用。方法 体外培养高糖(high glucose,HG)刺激的HK-2细胞模拟糖尿病肾病模型。分为正常浓度葡萄糖对照组(Control)、高浓度葡萄糖组(HG)、Control+rhBMP-7组、HG+rhBMP-7组。采用免疫荧光方法检测HK-2细胞中E-钙黏蛋白(E-cadherin)、α平滑肌肌动蛋白(α-smooth muscle actin, α-SAM)、焦孔素D(gasderminD, GSDMD)蛋白和焦孔素E(gasderminE, GSDME)蛋白;TUNEL染色检测细胞死亡情况;蛋白质印迹法检测各组HK-2细胞中焦亡相关蛋白半胱氨酸蛋白酶-1(caspase-1)、半胱氨酸蛋白酶-4(caspase-4)、半胱氨酸蛋白酶-5(caspase-5)、 N-GSDMD和GAPDH的表达;酶联免疫吸附试验(enzyme linked immunosorbent assay,ELISA)检测各组HK-2细胞培养上清液中炎症因子白细胞介素-18(interleukin-18,IL-18)和白细胞介素-1β(interleukin-1β,IL-1β)的表达。结果 与Control组相比,高糖组HK-2细胞Tunel染色的阳性率明显增加,差异具有统计学意义(P<0.000 1),α-SMA蛋白和α-SMA mRNA表达量也显著增加(P<0.000 1),E-cadherin蛋白和Ecadherin mRNA表达量显著下降(P=0.000 6,P<0.000 1);HG组添加rhBMP-7后α-SMA蛋白和m RNA水平显著下降(P=0.011 7, P=0.002), E-cadherin蛋白和m RNA水平明显升高(P=0.001 5,P<0.000 1)。与对照组比较,HG组HK-2细胞GSDME/GSDMD蛋白的荧光信号明显增强。进而,HG组添加rhBMP-7后GSDME/GSDMD蛋白荧光信号显著减弱。同样,HG组HK-2细胞中焦亡相关蛋白caspase-1、caspase-4、caspase-5以及GSDMD的表达量显著高于对照组,差异明显(均P<0.001)。HG组添加rhBMP-7后焦亡相关蛋白的表达明显下降(分别P<0.001,P=0.002,P<0.001)。与对照组比较,HG组HK-2细胞培养上清液中IL-1β和IL-18蛋白浓度显著增加(均P<0.001)。进而导致rhBMP-7干预后HG组细胞上清液中IL-10和IL-1β蛋白浓度明显减少(P<0.001)。结论 rhBMP-7具有抑制HK-2细胞焦亡的作用,其通过调控caspase-1/GSDMD信号通路减缓高糖诱导的HK-2细胞焦亡。 展开更多

血清CASP2表达与缺氧缺血性脑病患儿NBNA评分及预后的关系

目的探讨缺氧缺血性脑病(HIE)患儿血清半胱氨酸天冬氨酸蛋白酶2(CASP2)水平与新生儿神经行为(NBNA)评分的相关性,以及CASP2在预后中的评估价值。方法选取2018年1月至2022年4月该院的114例HIE患儿为HIE组,根据患儿病情严重程度分为轻度... 目的探讨缺氧缺血性脑病(HIE)患儿血清半胱氨酸天冬氨酸蛋白酶2(CASP2)水平与新生儿神经行为(NBNA)评分的相关性,以及CASP2在预后中的评估价值。方法选取2018年1月至2022年4月该院的114例HIE患儿为HIE组,根据患儿病情严重程度分为轻度组40例,中度组52例,重度组22例;根据治疗6个月后的预后情况将114例HIE患儿分为预后不良组26例和预后良好组88例。同期选择于该院出生的健康新生儿106例作为对照组。记录NBNA评分,采用酶联免疫吸附试验检测血清CASP2水平;采用Pearson相关分析HIE患儿血清CASP2水平与NBNA评分的相关性;采用受试者工作特征(ROC)曲线分析NBNA评分及血清CASP2水平对HIE患儿预后的预测价值。结果HIE组血清CASP2水平高于对照组,NBNA评分低于对照组(P<0.05)。重度组血清CASP2水平高于中度组、轻度组,NBNA评分低于中度组、轻度组(P<0.05);中度组血清CASP2水平高于轻度组,NBNA评分低于轻度组(P<0.05)。HIE患儿NBNA评分与血清CASP2水平呈负相关(r=-0.608,P<0.05)。预后不良组血清CASP2水平高于预后良好组,NBNA评分低于预后良好组(P<0.05)。血清CASP2水平及NBNA评分单独及联合预测HIE患儿预后不良的曲线下面积分别为0.875(95%CI 0.797~0.952)、0.748(95%CI 0.639~0.857)、0.927(95%CI 0.873~0.981)。结论血清CASP2水平随着HIE患儿病情严重程度增加而升高,且与患儿NBNA评分密切相关,有重要的预后预测价值。 展开更多
