Purpose We compared running economy(RE)and 3-km time-trial(TT)variables of runners wearing Nike Vaporfly 4%(VP4),Saucony Endorphin lightweight racing flats(FLAT),and their habitual running(OWN)footwear.Methods Eightee...Purpose We compared running economy(RE)and 3-km time-trial(TT)variables of runners wearing Nike Vaporfly 4%(VP4),Saucony Endorphin lightweight racing flats(FLAT),and their habitual running(OWN)footwear.Methods Eighteen male recreational runners(age=33.5±11.9 year(mean±SD),peak oxygen uptake(VO_(2peak))=55.8±4.4 mL/kg·min)attended 4 sessions approximately 7 days apart.The first session consisted of a VO_(2peak) test to inform subsequent RE speeds set at 60%,70%,and 80%of the speed eliciting VO2peak.In subsequent sessions,treadmill RE and 3-km TTs were assessed in the 3 footwear conditions in a randomized,counterbalanced crossover design.Results Oxygen consumption(mL/kg·min)was less in VP4(from 4.3%to 4.4%,p≤0.002)and FLAT(from 2.7%to 3.4%,p≤0.092)vs.OWN across intensities,with a non-significant difference between VP4 and FLAT(1.0%-1.7%,p≥0.292).Findings related to energy cost(W/kg)and energetics cost of transport(J/kg·m)were comparable.VP43-km TT performance(11:07.6±0:56.6 mm:ss)was enhanced vs.OWN by 16.6 s(2.4%,p=0.005)and vs.FLAT by 13.0 s(1.8%,p=0.032).The 3-km times between OWN and FLAT(0.5%,p=0.747)were similar.Most runners(n=11,61%)ran their fastest TT in VP4.Conclusion Overall,VP4 improved laboratory-based RE measures in male recreational runners at relative speeds compared to OWN,but the RE improvements in VP4 were not significant vs.FLAT.More runners exhibited better treadmill TT performances in VP4(61%)vs.FLAT(22%)and OWN(17%).The variability in RE(-10.3%to 13.3%)and TT(-4.7%to 9.3%)improvements suggests that responses to different types of shoes are individualized and warrant further investigation.展开更多

The types of footwear investigated in this study were open back shoes, commonly worn by hospitalized patients, and closed back shoes. Previous studies have shown that open back shoes, or slippers, increase the risk of falling (among elderly). We hypothesized that our findings would suggest that open back shoes negatively affect gait mechanics in healthy individuals. Methods: Healthy individuals (n = 12) participated in a walking test while wearing closed back shoes and open back shoes. The explanatory variables in this study were the analysis of gait, posture, and balance before and after walking. The objective variable was footwear (closed back shoes vs. open back shoes). A paired t-test was performed to detect significant differences between the two conditions. Results: Among the test items measured, we found a significant difference in minimum wide tilt angle and left-right differences in step length and intensity while walking between the conditions of closed back shoes and open back shoes. These results suggest that open back shoes could negatively impact posture and balance while walking, even in healthy subjects. Conclusion: It is imperative to improve patient awareness of the risk of falling. We believe that the inclusion of our findings in educational pamphlets and in-house notices could help improve patient awareness and more effectively prevent falls among patients.展开更多
AIM:To define footwear outcomes following hallux valgus surgery,focusing on patient return to comfortable and heeled footwear and patterns of post-operative footwear selection.METHODS:Surgical intervention is indicate...AIM:To define footwear outcomes following hallux valgus surgery,focusing on patient return to comfortable and heeled footwear and patterns of post-operative footwear selection.METHODS:Surgical intervention is indicated for symptomatic cases of hallux valgus unresponsive to conservative methods,with favourable reported outcomes.The return to various types of footwear postoperatively is reflective of the degree of correction achieved,and corresponds to patient satisfaction.Patients are expected to return to comfortable footwear post-operatively without significant residual symptoms.Many female patients will additionally attempt to return to high-heeled,narrow toe box shoes.However,minimal evidence exists to guide their expectations.Sixtyfive female hallux valgus patients that had undergone primary surgery bet ween 2011 and 2013 were retrospectively identified using our hospital surgical database.Patients were reviewed using a footwearspecific outcome questionnaire at a mean 18.5 mo follow-up.RESULTS:Eighty-six percent of patients were able to return to comfortable footwear post-operatively with minimal discomfort.Of those intending to resume wearing heeled footwear,62%were able to do so,with 77%of these patients wearing these as or more frequently than pre-operatively.No significant difference was observed between pre-and post-operative heel size.Mean time to return to heeled footwear was 21.4wk post-operation.Cosmetic outcomes were very high and did not adversely impact footwear selection.CONCLUSION:We report high rates of return to both comfortable and heeled shoes in female patients following primary hallux valgus surgery.We observed an"all-ornone phenomenon"where patients rejected a return to heeled footwear unless able to tolerate them at the same frequency and heel size as pre-operatively.A minority of patients were unable to return to comfortable footwear post-operatively,which had adverse ramifications on their quality-of-life.We recommend that the importance of managing patient expectations through appropriate pre-operative counselling be emphasized in forefoot surgery.展开更多

Purpose This study aimed to investigate whether there is a systematic change of leg muscle activity,as quantified by surface electromyography(EMG),throughout a standard running footwear assessment protocol at a predet...Purpose This study aimed to investigate whether there is a systematic change of leg muscle activity,as quantified by surface electromyography(EMG),throughout a standard running footwear assessment protocol at a predetermined running speed.Methods Thirty-one physically active adults(15 females and 16 males)completed 5 testing rounds consisting of overground running trials at a speed of 3.5 m/s.The level of muscle activity from 6 major leg muscles was recorded using surface EMG.The variables assessed were the EMG total intensity as a function of time and the cumulative EMG overall intensity.Systematic effects of the chronological testing round(independent variable)on the normalized EMG overall intensity(dependent variable)were examined using Friedman analysis of variates and post hoc pairwise Wilcoxon signed-rank tests(α=0.05).Results There was a systematic reduction in overall EMG intensity for all 6 muscles over the time course of the running protocol(p<0.001)until the fourth testing round when EMG intensities reached a steady state.The one exception was the biceps femoris muscle,which showed a significant reduction of EMG intensity during the stance phase(p<0.001)but not the swing phase(p=0.16).Conclusion While running at a predetermined speed,the neuromuscular system undergoes an adaptation process characterized by a progressive reduction in the activity level of major leg muscles.This process may represent an optimization strategy of the neuromuscular system towards a more energetically efficient running style.Future running protocols should include a familiarization period of at least 7 min or 600 strides of running at the predetermined speed.展开更多

In a Public Healthcare Service, footwear approval is under the respons...Adherence of patients to treatment and appropriateness of prescribed footwear are mandatory for successful prevention of Diabetic foot ulceration. In a Public Healthcare Service, footwear approval is under the responsibility of the clinician. In some cases kinetic measurements are taken inside the shoes to support the clinical decision;however, an overall agreement is still missing with respect to a simple, reliable and effective test protocol based on proper threshold values. Authors’ past experiences, specific needs of the Diabetic outpatient service, and suggestions from valuable literature led to an instrumental test protocol based on reference peak pressure (PP) thresholds and gait line (GL) specific for each level of risk. Permission was obtained for one preliminary validation session at the Authors’ outpatient service, during which 11 patients at high/very high risk of ulceration were examined—9 for testing new footwear, 2 for monitoring 12-month-old footwear. The protocol was well accepted by patients and operators, fast and easy to be used. Based on the instrumental, clinical and visual inspection data;4 new footwear did not pass the test;5 passed the test but integration or minor changes were requested;the 2 old footwear were found no more effective. As a first positive feedback, after 4 months of wearing the modified prescriptions none of the patients had developed plantar ulcers or signs of tissue damage. Successive data processing proved that in all patients altered PPs and deviated GLs did correlate with alterations of other kinetic parameters. The protocol, which is proved to be sensitive and valuable for the Diabetic foot care, may have a general validity;the delivered specific PP thresholds and reference data, instead, can be reliably used if the test is performed with a calibrated Pedar Insole System and with the wide Insoles in the range UW-YW (EU size 36 - 45).展开更多

The author conducted descriptive survey and quatitative study to determine the level of relationship of different variables on population and to measure the contribution of free variables to restricted variables. The result shows that the resources capabilities, including leather raw materials, synthetic raw materials, funding, human resources, science management, and information and technology influence cross functional coordination in footwear micro industry in West Jawa. This study also reveals that significant resources capabilities elements include leather raw materials, funding, and human resources supplies, while other elements provided insignificant impact.展开更多

Diabetic foot ulcers(DFU)have been connected to rising global mortality rates.An increased plantar tissue stress beneath the numb foot of neuropathy patients is associated with DFU.This article summarises essential th...Diabetic foot ulcers(DFU)have been connected to rising global mortality rates.An increased plantar tissue stress beneath the numb foot of neuropathy patients is associated with DFU.This article summarises essential therapeutic footwear concepts for the treatment of DFUs and shows how new sensor technologies have made this possible.People with neuropathic DFUs can address their foot problems with therapeutic footwear without being constrained in their daily activities.This intervention in maintaining DFUs must be discrete,power-efficient,accurate,and user-friendly in order to offer a trustworthy and acceptable way for effective health monitoring.The device can be inserted into a person's shoes from the plantar aspect of the foot to gather more information about their health.The findings showed that weight-bearing activities and sensor offloading devices can be employed as effective offloading therapies to heal DFU.This article examines the therapeutic footwear-based technological sensor system,which keeps track of many elements that are helpful in tracking foot ulcers,available treatments,and potential research areas in the future.展开更多
