Based on the MTLL lightning return stroke model,a two-dimensional cylindrical finite-difference time-domain( FDTD) model for underground horizontal electric field was established,and the effects of distance from lightning channel,depth,soil electrical conductivity and its distribution,and return stroke current on the characteristics of underground horizontal electric field were analyzed. The results showed that as long as the electrical conductivity of soil existed,the peak of underground horizontal electric field decreased with the increase in the horizontal distance from lightning channel. When the electrical conductivity of the upper soil was much smaller than that of the lower soil,the peak of corresponding horizontal electric field was larger than the electric field peak as the electrical conductivity of the upper soil was larger than that of the lower soil. When the electrical conductivity of the upper soil was less than that of the lower soil,the growth and decay of the horizontal electric field were faster than the growth and decay as the electrical conductivity of the upper soil was larger than that of the lower soil. 展开更多

Using the characteristics of symmetric current wave-form induced by lightning stroke without causing fault and that of asymmetric current waveform generated by fault within a very short time interval, the waveform of transient current above and below time-axes are integrated respec-tively. First, through comparing the relative ratio of them with threshold value, the primary criterion identifying fault and lightning stroke is constructed;Secondly, to improve the reliability of discrimination between lightning stroke with and without causing fault, according to the difference of them, the secondary criterion is also defined. The simulation results and analysis demonstrate that the proposed integral criterions are valid and correct. 展开更多

<b>Objective:</b> The objective is to investigate whether acupuncture can protect brain by regulating the expression of ICAM-5 mRNA in brain tissue. <b>Methods:</b> Male SD rats were used to co... <b>Objective:</b> The objective is to investigate whether acupuncture can protect brain by regulating the expression of ICAM-5 mRNA in brain tissue. <b>Methods:</b> Male SD rats were used to construct a middle cerebral artery occlusion reperfusion injury (Middle cerebral artery occlusion, MCAO) model, and the rats were randomly divided into 4 groups, namely the normal group (group A) and the sham operation group (group B), MCAO group (group C), MCAO + acupuncture group (group D);real-time fluorescent quantitative PCR (RT-qPCR) was used to detect the expression of ICAM-5 mRNA in the brain tissue of the ischemic area, and HE staining and TUNEL were used to detect the level of neuronal apoptosis to evaluate the therapeutic effect. <b>Results:</b> Acupuncture can improve neuronal morphology and reduce neuronal apoptosis in rats with ischemic stroke. RT-qPCR: compared with group A and group B, the expression level of ICAM-5 mRNA in the brain tissues of group C and group D decreased, while compared with group C, The expression of ICAM-5 mRNA in the brain tissue of group D was significantly up-regulated (P < 0.05). There was no statistical difference between group A and group B (P > 0.05). <b>Conclusion:</b> The effectiveness of acupuncture in the treatment of ischemic stroke may achieve neuroprotection by up-regulating the expression level of ICAM-5 mRNA. 展开更多

Effect of safflower yellow pigment combined with cerebellin on neurological function in patients with ischemic stroke and its mechanism

Objective: To investigate the effects of safflower yellow pigment combined with cerebellin on neurological function in patients with ischemic stroke and analyze its possible mechanism. Methods: A total of 130 patients with ischemic stroke admitted to my hospital from May 2017 to August 2018 were enrolled. The patients were divided into the control group and the study group according to the random number table method, 65 cases in each group. The control group was treated with cerebroside carnosine. And the study group was treated with safflower yellow pigment on the basis of the control group. The changes of neurological function index, oxidative stress index, vascular endothelial function index and inflammatory factor were compared before and after treatment. Results: There were no significant differences in preoperative neurological parameters, oxidative stress, vascular endothelial function and inflammatory factors between the two groups (P>0.05). After treatment, the neurological function indexes S-100β and NSE were significantly lower in both groups, and NGF levels were significantly increased. The S-100β and NSE levels in the study group were (0.91±0.10) ng/L and (12.91±1.33) ng/L, respectively, which were significantly lower than control group. While the NGF level of the study group was (79.52±8.07) μg/L significantly higher than that of the control group (both P<0.05). The levels of Ox-LDL were significantly reduced and GSH-Px levels were significantly elevated in both groups after treatment. The level of oxidative stress index Ox-LDL was (563.51±57.10) μg/dL, which was significantly lower than that of the control group, and the GSH-Px level was (154.55±16.07) U/L, which was significantly higher than the control group (both P<0.05). The levels of vascular endothelial function NT-proBNP, TXB2 and ET-1 were significantly lower in the two groups after treatment. The NT-proBNP, TXB2 and ET-1 in the study group were (95.91±9.77) pg/mL, (245.69±25.06) pg/mL and (64.26±6.65) ng/L, respectively, which were significantly lower than the control group (both P<0.05). The levels of inflammatory factors MMP-9 and TNF-α were significantly lower in the two groups after treatment. The levels of MMP-9 and TNF-α in the study group were (60.64±6.12) ng/mL and (0.33±0.04) ng/mL, respectively, which were significantly lower than those in the control group (both P<0.05). Conclusion: Safflor yellow combined with cerebellin in the treatment of ischemic stroke has higher clinical efficacy and can significantly improve the neurological function of patients. The possible mechanism is related to the improvement of vascular endothelial function and stress response. 展开更多

BACKGROUND Mechanical thrombectomy is the most effective treatment for great cerebral artery embolization within a set time window.Typically,an arteriogram does not show the localization of the stent after release and whether a thrombus is captured or not.Thus,improving the visualization of a stent in interventional therapy will be helpful for clinicians.AIM To analyze stent imaging findings to enhance clinicians’understanding of a special circumstance,wherein a Solitaire AB retrievable stent was visible during the imaging of a thrombus capture that improved the success rate of stent-based mechanical thrombectomy.METHODS This was a retrospective study with four acute ischemic stroke(AIS)patients who underwent stent-based mechanical thrombectomy.RESULTS Patient 1 was a 64-year-old man admitted after 5 h of confusion;angiography revealed basilar artery occlusion.We inserted a stent into the left posterior cerebral artery-P2 segment and visualized the expanded stent that successfully captured a thrombus.Patient 2 was a 74-year-old man admitted with confusion,which lasted approximately 3 h.Angiography revealed a left middle cerebral artery(MCA)-M1 segment occlusion.A stent was deployed in the distal M2 segment,and we could visualize the stent by capturing the thrombus.Patient 3 was a 74-year-old woman admitted after experiencing left hemiplegia for 3 h.We deployed a stent at the distal right MCAM2 segment,and the developing stent captured a large thrombus.Patient 4 was an 82-year-old man who presented with confusion for 3 h.A developing stent was placed in the distal left MCA-M1 segment,which captured a large thrombus and several fragmented thrombi.CONCLUSION To the best of our knowledge,this is the first report of stent imaging in patients with AIS.We demonstrated the usefulness and substantial potential of stent imaging in stent-based mechanical thrombectomy for AIS. 展开更多

2019年夏季在广州开展了火箭引雷试验,成功地触发了14次闪电,总回击(RS)数达74次,平均回击数为5.3次,单次闪电的最大回击数为14次。该文对火箭引雷的回击电流及不同距离电场波形特征进行了分析。回击的间隔时间、峰值电流、10%~90%上升... 2019年夏季在广州开展了火箭引雷试验,成功地触发了14次闪电,总回击(RS)数达74次,平均回击数为5.3次,单次闪电的最大回击数为14次。该文对火箭引雷的回击电流及不同距离电场波形特征进行了分析。回击的间隔时间、峰值电流、10%~90%上升时间、半峰值宽度、1 ms转移电荷和1 ms作用积分的几何平均值分别为38.45 ms、12.38 kA、0.25μs、8.31μs、0.68C和2.12×10^(3)A^(2)·s,回击电流的转移电荷量和作用积分量均与回击峰值电流呈幂函数相关性。回击的先导电场强度峰值和回击电场强度峰值会随距离的增大而减小,而电场的10%~90%上升时间和半峰值宽度会随距离的增大而增大。回击电场强度峰值会随距离的增大而呈现出幂函数减小,且峰值电流与不同距离的先导电场强度峰值和回击电场峰值均呈现一定线性相关,距离越大,其线性拟合关系越好。 展开更多

基于网络药理学和分子对接技术探究脑络欣通治疗缺血性脑卒中的机制

目的采用网络药理学方法探究益气活血方脑络欣通治疗缺血性脑卒中(ischemic stroke,IS)的作用机制,并通过分子对接技术进行初步验证。方法通过TCMSP、ETCM、化学专业数据库及文献筛选脑络欣通活性成分及作用靶点,通过OMIM、DisGeNET、Ge... 目的采用网络药理学方法探究益气活血方脑络欣通治疗缺血性脑卒中(ischemic stroke,IS)的作用机制,并通过分子对接技术进行初步验证。方法通过TCMSP、ETCM、化学专业数据库及文献筛选脑络欣通活性成分及作用靶点,通过OMIM、DisGeNET、GeneCards以及Drugbank数据库挖掘IS相关靶点;将交集靶点导入STRING平台和Cytoscape 3.8.2软件构建PPI网络并通过拓扑分析获得核心靶点,同时构建“中药—活性成分—核心靶点—疾病”可视化网络图;运用R软件进行GO功能及KEGG通路富集分析。最后借助Auto Dock Tools软件以及PyMOL软件进行分子对接和可视化。结果脑络欣通的潜在活性成分78个,治疗IS的潜在作用靶点200个,核心靶点包括STAT3、JUN、MAPK1、TP53等;作用较突出的活性成分包括槲皮素、β-谷甾醇、山柰酚、木犀草素等。进一步的生物功能分析获得150条信号通路,包括PI3K-AKT信号通路、HIF-1信号通路、FoxO信号通路等关键通路。分子对接结果表明关键活性成分与核心靶点之间存在分子结合位点,并有较强的结合活性。结论益气活血方脑络欣通可能是通过多成分、多靶点协同作用于氧化应激、炎症反应、细胞凋亡、自噬等生理病理环节,从而发挥治疗IS的作用。 展开更多

急性缺血性脑卒中机械取栓术后恶性脑水肿的危险因素

目的:探讨急性缺血性脑卒中(AIS)患者机械取栓术后恶性脑水肿(MCE)发生的危险因素。方法:回顾性收集2018年8月至2022年6月在温州医科大学附属第一医院接受机械取栓术并实现血管完全再通的AIS患者400例,根据术后是否发生MCE将患者分为MCE... 目的:探讨急性缺血性脑卒中(AIS)患者机械取栓术后恶性脑水肿(MCE)发生的危险因素。方法:回顾性收集2018年8月至2022年6月在温州医科大学附属第一医院接受机械取栓术并实现血管完全再通的AIS患者400例,根据术后是否发生MCE将患者分为MCE组(n=89)和非MCE组(n=311)。采用多因素Logistic回归分析确定AIS患者机械取栓术后MCE发生的独立危险因素。结果:400例行机械取栓术后血管完全再通的AIS患者中,男277例,女123例,年龄(68.09±12.17)岁。AIS患者机械取栓术后MCE组的舒张压高于非MCE组,差异有统计学意义(P<0.05)。而MCE组的ASPECT评分低于非MCE组,更可能位于前循环位置。多因素Logistic回归结果显示,AIS患者机械取栓术后舒张压(OR=1.022,95%CI=1.005~1.039,P=0.010)、术前ASPECT评分(OR=0.742,95%CI=0.655~0.840,P<0.001)、NIHSS评分(OR=1.043,95%CI=1.004~1.083,P=0.031)以及前循环血栓(OR=8.942,95%CI=2.595~30.816,P=0.001)是术后MCE发生的独立影响因素。结论:术前低ASPECT评分、高NIHSS评分以及前循环血栓是AIS患者机械取栓术后MCE的独立危险因素,且术后舒张压升高会增加术后发生MCE风险。术后血压管理尤其是降低舒张压可能减少患者术后发生MCE的风险。 展开更多

目的探讨丹参-当归治疗缺血性脑卒中(CIS)的潜在作用机制。方法通过中药系统药理学数据库与分析平台(TCMSP)、Swiss TargetPrediction数据库获取丹参和当归的活性成分及作用靶点,并通过检索PubMed、中国知网相关文献进行补充;通过GeneCa... 目的探讨丹参-当归治疗缺血性脑卒中(CIS)的潜在作用机制。方法通过中药系统药理学数据库与分析平台(TCMSP)、Swiss TargetPrediction数据库获取丹参和当归的活性成分及作用靶点,并通过检索PubMed、中国知网相关文献进行补充;通过GeneCards、人类孟德尔遗传数据库(OMIM)、DisGeNET数据库获取CIS的潜在靶点;将丹参-当归治疗CIS的共有靶点导入String 11.0平台,构建活性成分与疾病靶点蛋白的蛋白相互作用(PPI)网络,利用Cytoscape 3.8.2软件构建疾病-药物-活性成分-靶点可视化网络;将共有靶点导入DAVID数据库进行基因本体论(GO)功能富集和京都基因与基因组百科全书(KEGG)通路富集分析,并利用Cytoscape 3.8.2软件构建活性成分-核心靶点-通路网络;利用AutoDock软件对疾病-药物-活性成分-靶点网络中度值排名前6的核心靶点和活性成分进行分子对接验证。结果共检索到82种活性成分,其中丹参65个、当归17个,潜在作用靶点787个,疾病靶点671个,共有靶点76个。度值排名前6的活性成分为丹参醇B、丹参新醌D、木犀草素、阿魏酸、藁本内酯、丹参酮ⅡA,核心靶点为MAPK14,MAPK1,AKT1,PTGS2,EGFR,JAK2。共获得GO功能条目2545个,其中生物学过程2244个,细胞组成128个,分子功能173个,分别涉及对脂多糖的反应、膜筏、内肽酶活性等;KEGG信号通路152条,主要涉及PI3K-Akt信号通路、脂质和动脉粥样硬化、钙信号通路等。分子对接结果证明了6种活性成分与其相应核心靶点均有较好的结合活性。结论丹参-当归治疗CIS具有多成分-多靶点-多通路的调控特点,其作用机制可能与抗炎、抗氧化应激、抗神经细胞凋亡、调节自噬功能等有关。 展开更多

性别对急性缺血性卒中患者机械取栓术预后的影响

目的探讨性别差异对前循环急性大血管闭塞性卒中(AIS-LVO)患者机械取栓术预后的影响。方法回顾性连续纳入2019年1月至2021年12月在首都医科大学宣武医院神经外科接受机械取栓治疗的前循环AIS-LVO患者,于机械取栓术后90 d将改良Rankin量... 目的探讨性别差异对前循环急性大血管闭塞性卒中(AIS-LVO)患者机械取栓术预后的影响。方法回顾性连续纳入2019年1月至2021年12月在首都医科大学宣武医院神经外科接受机械取栓治疗的前循环AIS-LVO患者,于机械取栓术后90 d将改良Rankin量表(mRS)评分0~2分定义为预后良好,mRS评分3~6分定义为预后不良。收集患者基线资料(年龄、性别)、临床资料;临床资料包括血管疾病危险因素[高血压病、糖尿病、高脂血症、冠状动脉粥样硬化性心脏病(简称“冠心病”)、心房颤动、吸烟史、饮酒史、卒中史]、抗血小板聚集和抗凝药物使用史、基线美国国立卫生研究院卒中量表(NIHSS)评分、脑梗死分型、静脉溶栓、麻醉类型、发病至急诊时间、穿刺至血管再通时间(PTR)、发病至血管再通时间、基线Alberta卒中项目早期CT评分(ASPECTS)、闭塞部位、侧支循环状态、术后即刻再通情况。分别以性别、预后进行分组(男性组与女性组、预后不良组与预后良好组),比较两组患者基线和临床资料的差异。采用单因素Logistic回归分析性别在不同预后组间的差异,并采用Logistic多因素回归分析评估性别是否对机械取栓术患者的预后产生影响。结果共纳入了347例前循环AIS-LVO患者,其中女性组126例,男性组221例;预后良好组169例,预后不良组178例。(1)女性组患者年龄、心房颤动比例、基线NIHSS评分均高于男性组[70.5(59.0,80.0)岁比64.0(56.0,71.0)岁,Z=19.720;41.3%(52/126)比21.7%(48/221),χ^(2)=14.950;16(13,19)分比14(11,18)分,Z=9.670],吸烟、饮酒比例均低于男性组[5.6%(7/126)比54.8%(121/221),χ^(2)=83.420;3.2%(4/126)比49.8%(110/221),χ^(2)=78.990],组间差异均有统计学意义(均P<0.01);女性组和男性组患者脑梗死分型分布由高至低均为心源性栓塞[79.4%(100/126)、52.9%(117/221)]、大动脉粥样硬性[19.0%(24/126)、44.3%(98/221)]、其他型[1.6%(2/126)、2.7%(6/221)],组间差异有统计学意义(χ^(2)=24.010,P<0.01)。其余临床资料的不同性别组间差异均无统计学意义(均P>0.05)。(2)单因素分析显示,预后不良组女性比例、年龄、冠心病比例、卒中史比例、NIHSS评分、PTR均高于预后良好组[41.6%(74/178)比30.8%(52/169),70(61,77)岁比64(54,70)岁,31.5%(56/178)比16.0%(27/169),23.0%(41/178)比13.6%(23/169),17(13,20)分比13(11,17)分,39(29,69)min比36(25,50)min],而ASPECTS、良好侧支比例、血管成功再通比例均低于预后良好组[8(7,9)分比9(8,10)分,15.7%(28/178)比55.0%(93/169),86.0%(153/178)比98.2%(166/169)],组间差异均有统计学意义(均P<0.05);多因素Logistic回归分析结果显示,年龄增加(OR=1.025,95%CI:1.003~1.048)、冠心病(OR=2.467,95%CI:1.295~4.699)、NIHSS评分增加(OR=1.134,95%CI:1.073~1.198)、PTR延长(OR=1.011,95%CI:1.001~1.021)均为预后不良的危险因素(均P<0.05),ASPECTS增加(OR=0.722,95%CI:0.581~0.897)、良好侧支(OR=0.171,95%CI:0.096~0.303)、血管成功再通(OR=0.068,95%CI:0.015~0.313)均为预后良好的保护因素(均P<0.05),而性别非前循环AIS-LVO患者接受机械取栓治疗预后的影响因素(P>0.05)。结论接受取栓治疗的前循环AIS-LVO预后良好患者中女性比例低于预后不良者,但性别并非前循环AIS-LVO患者接受机械取栓术预后的影响因素。本研究结果尚需多中心、大样本数据进一步验证。 展开更多
