Atherosclerotic involvements are an essential causal element of prospect in diabetes mellitus(DM), with carotid atherosclerosis(CA) being a common risk-factor for prospective crisis of coronary artery diseases(CAD) and/or cerebral infarction(CI) in DM subjects. From another point of view, several reports have supplied augmenting proof that hepatocyte growth factor(HGF) has a physiopathological part in DM involvements. HGF has been a mesenchymal-derived polyphenic factor which modulates development, motion, and morphosis of diverse cells, and has been regarded as a humor intermediator of epithelial-mesenchymal interplays. The serum concentrations of HGF have been elevated in subjects with CAD and CI, especially during the acute phase of both disturbances. In our study with 89 type 2 DM patients, the association between serum concentrationsof HGF and risk-factors for macrovascular complicationsinclusive of CA were examined. The average of serumHGF levels in the subjects was more elevated than thereference interval. The serum HGF concentrations associated positively with both intimal-media thickness(IMT)(r = 0.24, P = 0.0248) and plaque score(r = 0.27, P =0.0126), indicating a relationship between the elevatedHGF concentrations and advancement of CA involvements. Multivariate statistical analysis accentuated thatserum concentrations of HGF would be associated inde-pendently with IMT(standardized = 0.28, P = 0.0499).The review indicates what is presently known regardingserum HGF might be a new and meaningful biomarkerof macroangiopathy in DM subjects. 展开更多

BACKGROUND The accumulation of advanced glycation end products(AGEs)have been implicated in the development and progression of diabetic vasculopathy.However,the role of profilin-1 as a multifunctional actin-binding protein in AGEs-induced atherosclerosis(AS)is largely unknown.AIM To explore the potential role of profilin-1 in the pathogenesis of AS induced by AGEs,particularly in relation to the Janus kinase 2(JAK2)and signal transducer and activator of transcription 3(STAT3)signaling pathway.METHODS Eighty-nine individuals undergoing coronary angiography were enrolled in the study.Plasma cytokine levels were detected using ELISA kits.Rat aortic vascular smooth muscle cells(RASMCs)were incubated with different compounds for different times.Cell proliferation was determined by performing the MTT assay and EdU staining.An AGEs-induced vascular remodeling model was established in rats and histological and immunohistochemical analyses were performed.The mRNA and protein levels were detected using real-time PCR and Western blot analysis,respectively.In vivo,shRNA transfection was performed to verify the role of profilin-1 in AGEs-induced proatherogenic mediator release and aortic remodeling.Statistical analyses were performed using SPSS 22.0 software.RESULTS Compared with the control group,plasma levels of profilin-1 and receptor for AGEs(RAGE)were significantly increased in patients with coronary artery disease,especially in those complicated with diabetes mellitus(P<0.01).The levels of profilin-1 were positively correlated with the levels of RAGE(P<0.01);additionally,the levels of both molecules were positively associated with the degree of coronary artery stenosis(P<0.01).In vivo,tail vein injections of AGEs induced the release of proatherogenic mediators,such as asymmetric dimethylarginine,intercellular adhesion molecule-1,and the N-terminus of procollagen III peptide,concomitant with apparent aortic morphological changes and significantly upregulated expression of the profilin-1 mRNA and protein in the thoracic aorta(P<0.05 or P<0.01).Downregulation of profilin-1 expression with an shRNA significantly attenuated AGEs-induced proatherogenic mediator release(P<0.05)and aortic remodeling.In vitro,incubation of vascular smooth muscle cells(VSMCs)with AGEs significantly promoted cell proliferation and upregulated the expression of the profilin-1 mRNA and protein(P<0.05).AGEs(200μg/mL,24 h)significantly upregulated the expression of the STAT3 mRNA and protein and JAK2 protein,which was blocked by a JAK2 inhibitor(T3042-1)and/or STAT3 inhibitor(T6308-1)(P<0.05).In addition,pretreatment with T3042-1 or T6308-1 significantly inhibited AGEs-induced RASMC proliferation(P<0.05).CONCLUSION AGEs induce proatherogenic events such as VSMC proliferation,proatherogenic mediator release,and vascular remodeling,changes that can be attenuated by silencing profilin-1 expression.These results suggest a crucial role for profilin-1 in AGEs-induced vasculopathy. 展开更多

In order to clarify the potential role of calcium sensing receptor(Ca SR), a typical G protein coupled receptor(GPCR), in hyperglacemia-induced macroangiopathy, experimental hyperglycemia models in vivo and in vitro were prepared. Firstly, SD rats were divided into control group(n=10) and diabetes group(n=10), and diabetic model was induced via high-fat diet feeding and streptozotocin(STZ, 30 mg/kg) injection. Hydroxyproline level, determined via Choramnie T oxidation method, in vessel wall in diabetic rats was 30% more than that in control group. The gene transcription and expression levels were detected by real-time PCR and Western blotting, respectively. Both of collagen Ⅰ and Ⅲ mR NA levels in diabetic aorta were nearly twice those in normal aorta. The cleaved caspase-3 and-9 were elevated 1.5 and 2.5 times respectively in diabetic vascular cells. As compared with controls, m RNA and protein levels of CaS R in aorta were increased by 3 and 1.5 times in diabetes group. The expression levels of Bax as well as pro-apoptotic kinases(phospho-p38 and phosphor-JNK) were also increased 2, 0.5 and 0.5 times respectively in diabetic rats. To further validate the involvement of Ca SR in cell apoptosis and explore the potential mechanism, the endothelial cell line(human umbilical vascular endothelial cells, HUVECs) was stimulated with high concentration of glucose(33 mmol/L) to mimic hyperglycemia in vitro. Cell-based assays also showed that the Ca SR level and key apoptotic proteins(cleaved caspase-3 and-9, Bax, phospho-p38 and phosphor-JNK) were elevated in response to stimulation, and inhibition of Ca SR by using specific inhibitor(NPS-2143, 10 μmol/L) could protect cells against apoptosis. Our results demonstrated that Ca SR might take important part in the development of diabetic macroangiopathy through promoting cell apoptosis induced by hyperglycemia. 展开更多

Objective:To study the relationship of interleukin-12(IL-12)and IL-4 levels with macroangiopathy in type 2 diabetes mellitus by comparing the serum IL-12 and IL-4 levels and intima-media thickness(IMT)between the healthy controls and the type 2 diabetic patients with or without macroangiopathy.Methods:30 healthy controls and 120 cases with type 2 diabetes mellitus from November 2011 to November 2012 were selected.In accordance with the carotid intima-media thickness,the type 2 diabetes mellitus patients were divided into group A with IMT below 1.0 mm,group B with IMT between 1.0 mm and 1.2 mm,group C with IMT between 1.2 mm and 1.4 mm,group D with IMT over 1.4 mm.ELISA was used to measure the levels of serum IL-12 and IL-4 in all groups.And the measurement results of serum IL-12 and IL-4 levels in the groups were compared.Results:Type 2 diabetes mellitus groups had significantly higher serum IL-12 levels and much lower IL-4 levels than the healthy control group.In the type 2 diabetes mellitus groups,the thicker the IMT was,the higher the incidence of complications became.IL-12 level was positively correlated with IMT while the IL-4 level was negatively correlated with IMT.Conclusions:IL-12 levels in patients with type 2 diabetes were significantly higher and IL-4 levels were significantly lower than those in the healthy people.In patients with type 2 diabetes,the IL-12 levels increased while the IL-4 levels gradually declined with the advance of macroangiopathy. 展开更多

Aim: Study the macroangiopathic complications in type 2 diabetics. Method: It is a descriptive cross-sectional study with an analytical aim covering the period from January 2019 to December 31, 2021 in the Endocrinology, Metabolism and Nutrition clinic of the National Teaching Hospital Hubert Koutoukou Maga (CNHU-HKM) We thus identified 150 type 2 diabetic patients. Results: The prevalence of macroangiopathy was 60% with 11.3% for stroke, 28.6% for acute coronary syndrom, 4% for cervical macroangiopathy, and 46.97% ± 25.36% for obliterating arteriopathy of the lower limbs (OALL). The mean age of the patients was 57.69 ± 1.77 years with a sex ratio 1. The duration of diabetes progression was greater than 10 years for more than half of the patients 52.6%. The main associated cardiovascular risk factors were arterial high blood pressure (64.7%), family history of diabetes (33.7%), obesity with 20.0%. The death rate was 7.3%. Conclusion: Macroangiopathy’s mortality rate of in type 2 diabetics is high. Prevention remains the best treatment and involves screening for factors associated with macroangiopathy. 展开更多

Introduction: Macroangiopathy plays an important role, with a high prevalence of morbidity and mortality in diabetic patients. The aim was to study the epidemiological, clinical, paraclinical, therapeutic and evolutionary profile of macroangiopathy in diabetic patients in the internal medicine department of the Abass Ndao hospital. Patients and methods: This was a descriptive and analytical cross-sectional study. Our investigations were recruited over a 7-year period (January 1, 2016 to December 31, 2022). Results: Three hundred and fifty-nine (359) patients (10.22%) were enrolled. The mean age was 62.83 years, with extremes ranging from 17 to 98 years. The [60 - 69] age group was more representative (37.32%). Women accounted for 180 cases (50.1%), with a sex ratio (m/f) of 0.99. The average duration of diabetes was 11.86 years. Average consultation time was 38.07 days, with extremes ranging from 1 to 368 days. Average hospital stay was 7.65 days. Inaugural diabetes was noted in 12 cases (3.34%). Type 2 diabetes accounted for 95.82% (n = 344) of patients. Hypertension was present in 150 patients (41.8%). Patients with 2 risk factors accounted for 173 cases (48.18%). Nineteen patients had already had a stroke (5.29% of cases). Fourteen (14) patients (4.2%) were amputees. Obliterative arteriopathy of the lower limbs (AOMI) was noted in 193 patients (54%). Stroke was noted in 101 patients (28%). Ischemic heart disease (IHD) was noted in 38 patients (11%). AOMI was more common in males (110 patients, 57%) than in females (43%). Seventy-three (73) patients (20.3%) died. Predictors of death were age over 60 and the existence of more than two cardiovascular risk factors. Conclusion: Diabetic macroangiopathy is a major cause of morbidity and mortality. The development and implementation of a prevention and management program is essential. 展开更多

Background A five-year follow-up study of intensive multifactorial intervention was undertaken to assess the changes of circulating serum amyloid A (SAA) levels and the incidence of atherosclerosis (AS) in patients with short-duration type 2 diabetes mellitus (T2DM) without macroangiopathy, and whether intensive multifactorial intervention could prevent or at least postpone the occurence of macroangiopathy. Methods Among 150 patients with short-duration T2DM, 75 were assigned to receive conventional outpatient treatment (conventional group) and the others underwent intensive multifactorial integrated therapy targeting hyperglycemia, hypertension, dyslipidemia and received aspirin simultaneously (intensive group). Results Plasma SAA levels were higher in diabetic patients than those in healthy control subjects, and decreased obviously after intensive multifactorial intervention. The levels of SAA were positively correlated with body mass index (BMI), waist hip ratio (WHR), triglyceride (TG), high sensitive C-reactive protein (hs-CRP) and common carotid intima-media thickness (CC-IMT). The standard-reaching rates of glycemia, blood pressure and lipidemia were significantly higher in intensive group than those of conventional group. The incidence of macroangiopathy decreased by 58.96% in intensive group compared with conventional group. Conclusions Intensive multifactorial intervention may significantly reduce the SAA levels and prevent the occurrence of AS in short-duration patients with T2DM. SAA might be one of the risk factors of T2DM combined with AS. 展开更多

Objective:Our aim was to investigate the correlation between free fatty acid(FFA) spectrum, blood stasis(BS) score, and macroangiopathy in type 2 diabetic patients with or without BS, as well as the possible relationship between BS and lipotoxicity. Methods: A total of 50 type 2 diabetes(T2D) patients with or without BS were enrolled from June to December 2014 in Shenzhen Traditional Chinese Medicine(TCM) Hospital, with 25 patients allocated to each of two groups. Basic information, BS score, blood glucose, blood lipids, etc., were measured for each patient. In addition, we tested the levels of interleukin(IL)?6, tumor necrosis factor α(TNF?α), and IL?18 with enzyme?linked immunosorbent assay. The macroangiopathy status of patients in the two groups was examined by color ultrasound and all factors related to BS scores were analyzed. Gas chromatography?mass spectrometry was used to explore the difference in the serum FFA spectra between the two different groups. In addition, the relationship between FFA spectra, BS scores, and macroangiopathy was analyzed. Results: BS scores, total cholesterol(TC), total triglyceride(TG), low?density lipoprotein cholesterol, IL?6, TNF?α, IL?18, carotid and femoral artery plaque, carotid intima?media thickness, carotid plaque area, and femoral artery plaque area were all significantly increased in T2D patients with BS syndrome(P < 0.05). A positive correlation was observed between age, duration of diabetes, carotid intima?media thickness, carotid plaque area, femoral artery plaque area, and BS score(P < 0.05). A total of 21 fatty acids were found in the serum, and total FFA(TFFA), saturated fatty acid(SFA), lauric acid(C12:0), palmitic acid(16:0), stearic acid(C18:0), arachidonic acid(C20:4n6), behenic acid(C22:0), and lignoceric acid(C24:0) scores were all found to contribute to the difference between FFA spectrums of the two groups; of the fatty acids, C12:0, C16:0, C18:0, C22:0, TFFA, and SFA positively correlated with BS scores as evaluated by Pearson's or Spearman's correlation analysis(P < 0.05). Only SFA entered the regression equation in the multiple linear regression analysis. C12:0, C16:0, C18:0, C20:4n6, TFFA, and SFA were positively correlated with carotid plaque area, whereas linoleic acid(C18:3n3), Cis?5, 8, 11, 14, and 17?eicosapentaenoic acids(C20:5n3) were negatively correlated(P < 0.05). C16:0 was positively correlated with the femoral artery plaque area and C18:3n3, cis?4, 7, 10, 13, 16, and 19?docosahexaenoic acids(C22:6) and nervonic acid were negatively correlated(P < 0.05). Conclusion: Serum FFA spectra were significantly different between T2D patients with BS and those without, and long?chain SFA made the greatest contribution. Serum FFA spectra were correlated with BS scores and diabetic macroangiopathy, which means that lipotoxicity and BS are correlated in T2D. 展开更多

Macroangiopathy is a complication of Type II Diabetes Mellitus(T2 DM),which is mainly caused by fibrosis of blood vessels.Using T2 DM rat models,we investigated whether the traditional Chinese medicine,Di-Dang Decoction(DDD),exhibited antifibrotic actions on great vessels.T2 DM rats were randomly divided into non-intervention group,early-,middle-,late-stage DDD intervention groups and control groups,including pioglitazone group and aminoguanidine group.After administration of DDD to T2 DM rats at different times,we detected the amount of extracellular matrix(ECM)deposition in the thoracic aorta.The results showed that early-stage intervention with DDD could effectively protect great vessels from ECM deposition.Considering that TGF-β1 is the master regulator of fibrosis,we further validated at the molecular level that,compared to middle-and late-stage intervention with DDD,early-stage intervention with DDD could significantly decrease the expression levels of factors related to the activated TGF-β1/Smad signalling pathway,as well as the expression levels of downstream effectors including CTGF,MMP and TIMP family proteins,which were directly involved in ECM remodelling.Therefore,early-stage intervention with DDD can reduce macrovascular fibrosis and prevent diabetic macroangiopathy. 展开更多
