Comprehensive Rehabilitation Therapy of Traditional Chinese Medicine Combined with Modern Rehabilitation Training Improves the Spasticity and Motor Function of Hemiplegia after Stroke

Objective:To analyze the impact of comprehensive rehabilitation therapy of traditional Chinese medicine(TCM)(based on modern rehabilitation training)on the spasticity and motor function in stroke patients with hemiplegia.Methods:Seventy-nine stroke and hemiplegia patients admitted to the hospital from June 2021 to June 2023 were selected and randomly divided into a control group(39 cases)using modern rehabilitation training,and an observation group combined with comprehensive TCM rehabilitation therapy(40 cases),over 1 month.The clinical index data of the two groups were compared.Results:There were differences in the clinical index data between the two groups.The total effective rate after 2 treatment in the observation group(92.50%)was higher than that of the control group(74.36%)(χ^(2)=4.727,P<0.05).All central sensitization inventory(CSI)and stroke quality of life(PRO)scores in both groups were lower after treatment,with the observation group having lower scores as compared to the control group(P<0.05).The scores of FMA(upper limbs,lower limbs),Barthel index scores,and Functional Ambulation Categories(FAC)scores of both groups increased after treatment,with the observation group having higher scores as compared to the control group(P<0.05).Conclusion:Comprehensive TCM rehabilitation therapy had a significant therapeutic effect on patients with hemiplegia after stroke.It improved the patient’s spasticity,limb movement,and walking function.Their daily living abilities and quality of life were also enhanced. 展开更多

Objective:To explore the effects of acupuncture and tuina combined with rehabilitation therapy on oxidative stress response and motor function in stroke hemiplegia patients.Methods:Sixty stroke hemiplegia patients were randomly divided into observation and control groups,with 30 cases each.The control group received conventional rehabilitation therapy,while the observation group also underwent acupuncture and tuina.After 8 weeks of treatment,oxidative stress indicators[malondialdehyde(MDA),superoxide dismutase(SOD),glutathione peroxidase(GSH-Px)],neurological deficit scores(NIHSS),Fugl-Meyer motor function scores(FMA),and activities of daily living(Barthel Index)were compared between the two groups.Results:Before treatment,there were no significant differences in MDA,NIHSS scores,FMA scores,and Barthel Index between the two groups(P>0.05).After 8 weeks,the observation group had lower MDA and NIHSS scores,but higher SOD,GSH-Px,FMA scores,and Barthel Index compared to the control group,with statistically significant differences(P<0.05).Conclusion:Acupuncture and tuina combined with rehabilitation therapy can effectively improve oxidative stress levels,promote neurological recovery,enhance motor function,and improve daily living activities in stroke hemiplegia patients.This combined therapy is recommended for clinical application. 展开更多

Effect of N-Acetylcysteine Combined with Lung Rehabilitation Therapy on Exercise Endurance and Quality of Life of Patients with Rheumatoid Arthritis-Related Interstitial Lung Disease

Objective:To explore the effect of N-acetylcysteine combined with lung rehabilitation therapy on exercise endurance and quality of life in patients with rheumatoid arthritis-related interstitial lung disease(RA-ILD).Methods:Fifty-six patients with RA-ILD admitted to Xijing Hospital from May 2022 to January 2024 were randomly divided into two groups:a non-rehabilitation group and a pulmonary rehabilitation group,with 28 patients in each group.Both groups received routine treatment.Additionally,the non-rehabilitation group received N-acetylcysteine treatment,while the lung rehabilitation group received lung rehabilitation treatment in addition to N-acetylcysteine.The improvement in exercise endurance and dyspnea between the two groups after treatment was compared and the quality of life of the patients was observed.Results:After treatment,the exercise endurance score in the lung rehabilitation group(335.67±45.29)was higher than that in the non-rehabilitation group(P<0.05).The dyspnea score in the lung rehabilitation group(0.72±0.16)was lower than that in the non-rehabilitation group(P<0.05).Additionally,FVC(3.18±0.58 L),FEV1(2.28±0.56 L),FEV1/FVC(69.69±5.56),and DLCO(60.53±5.92 mL/mmHg/min)were higher in the lung rehabilitation group compared to the non-rehabilitation group after treatment(P<0.05).Conclusion:Lung rehabilitation therapy combined with N-acetylcysteine treatment can effectively improve dyspnea symptoms,lung function,and exercise endurance in patients with RA-ILD.This approach helps to improve patient’s quality of life and is beneficial for their prognosis. 展开更多

The purpose of this study is to comprehensively evaluate the modern training model of rehabilitation therapy technology talents.Selecting the third-year students of the rehabilitation therapy technology program in School Y as the research subject,300 questionnaires were collected and the effective response rate was 92%.The strengths and weaknesses of the modern training model were analyzed through a mixed qualitative and quantitative research method.It was found that 68%of the students thought that the modern model had obvious advantages in practical teaching,but 42%of the students thought that it still needed to be improved in personalized teaching.This study provides an empirical basis and specific suggestions for optimizing the cultivation of rehabilitation therapy technology talents. 展开更多

Effects of early rehabilitation therapy on patients with mechanical ventilation 被引量:13

BACKGROUND: For patients in intensive care unit(ICU), mechanical ventilation is an effective treatment to survive from acute illness and improve survival rates. However, long periods of bed rest and restricted physical activity can result in side effects. This study aimed to investigate the feasibility of early rehabilitation therapy in patients with mechanical ventilation.METHODS: A randomized controlled trial was carried out. Sixty patients, with tracheal intubation or tracheostomy more than 48 hours and less than 72 hours, were admitted to the ICU of the Affiliated Hospital of Medical College, Qingdao University, from May 2010 to May 2012. These patients were randomly divided into a rehabilitation group and a control group. In the rehabilitation group, rehabilitation therapy was performed twice daily, and the training time and intensity were adjusted according to the condition of the patients. Early rehabilitation therapy included heading up actively, transferring from the supine position to sitting position, sitting at the edge of the bed, sitting in chair, transferring from sitting to standing, and ambulating bedside. The patient's body mass index, days to first out of bed, duration of mechanical ventilation, length of ICU stay, APACHE II score, highest FiO2, lowest PaO2/FiO2 and hospital mortality of patients were all compared between the rehabilitation group and the control group. The differences between the two groups were compared using Student's t test.RESULTS: There was no significant difference in body mass index, APACHE II score, highest FiO2, lowest PaO2/FiO2 and hospital mortality between the rehabilitation group and the control group(P>0.05). Patients in the rehabilitation group had shorter days to first out of bed(3.8±1.2 d vs. 7.3±2.8 d; P=0.00), duration of mechanical ventilation(5.6±2.1 d vs. 12.7±4.1 d; P=0.005) and length of ICU stay(12.7±4.1 d vs. 15.2±4.5 d; P=0.01) compared with the control group.CONCLUSION: Early rehabilitation therapy was feasible and effective in improving the outcomes of patients with mechanical ventilation. 展开更多

This study aimed to determine the effectiveness of comprehensive rehabilitation for patients with aneurysmal subarachnoid hemorrhage (aSAH) and to explore the factors influencing the prognosis of rehabilitation. This was a retrospective study. Twenty-five patients with aSAH were treated with physical therapy, occupational therapy, speech therapy, cognitive therapy, music therapy, Chinese acupuncture, hyperbaric oxygen, and transcranial magnetic stimulation. The general data of all patients were collected, and the functional scores at admission were compared with those at discharge. The Mini Mental State Examination, Fugl-Meyer Assessment Scale (FMAS) for motor and balance assessment, Holden Functional Ambulation Classification (FAC), modified Rankin Scale, National Institute of Health Stroke Scale, Modified Barthel Index for activities of daily living (ADL), and Glasgow Outcome Scale were significantly improved among 25 patients with aSAH after 1 month of comprehensive rehabilitation training. Hydrocephalus was an independent factor of the ability to perform ADLs (odds ratio, 0.29;95% confidence interval, 2.03. 3.15;p = 0.000). The improvement of ADLs in aSAH patients was not related to sex, surgical method, aneurysm location, age, or smoking status. Comprehensive and professional rehabilitation is effective for the cognition, movement, walking, ADLs, and functional prognosis of patients with aSAH, while early hydrocephalus may be a risk factor for poor ADLs. 展开更多

In this editorial,we comment on an article by Alhammad et al that was published in a recent issue of the World Journal of Clinical Cases(Manuscript No.:91134).We specifically focus on the mental health problems caused by coronavirus disease 2019(COVID-19),their mechanisms,and targeted rehabilitation strategies.Severe acute respiratory syndrome coronavirus 2,via its spike protein,binds to angiotensin-converting enzyme 2 and other receptors prior to infiltrating diverse cells within the central nervous system,including endothelial cells,neurons,astrocytes,and oligodendrocytes,thereby contributing to the development of mental illnesses.Epidemiological data from 2020 underscored the global upsurge in major depressive and anxiety disorders by 27.6%and 25.6%,respectively,during the pandemic.The commented research show that 30%of post-intensive care unit discharge patients with COVID-19 in the Arabic region exhibited Hospital Anxiety and Depression Scale scores that were indicative of anxiety and depression.While acknowledging psychosocial factors,such as grief and loss,it is crucial to recognize the potential neurological impact of the virus through various mechanisms.Accordingly,interventions that encompass dietary measures,health supplements,and traditional Chinese medicine with neuroprotective properties are necessary.This editorial underscores the urgency to implement comprehensive rehabilitation approaches to address the intricate interplay between COVID-19 and mental well-being. 展开更多

Depression,a common mental illness,seriously affects the health of individuals and has deleterious effects on society.The prevention and treatment of depression has drawn the attention of many researchers and has become an important social issue.The treatment strategies for depression include drugs,psychotherapy,and physiotherapy.Drug therapy is ineffective in some patients and psychotherapy has treatment limitations.As a reliable adjuvant therapy,physiotherapy compensates for the shortcomings of drug and psychotherapy and effectively reduces the disease recurrence rate.Physiotherapy is more scientific and rigorous,its methods are diverse,and to a certain extent,provides more choices for the treatment of depression.Physiotherapy can relieve symptoms in many ways,such as by improving the levels of neurobiochemical molecules,inhibiting the inflammatory response,regulating the neuroendocrine system,and increasing neuroplasticity.Physiotherapy has biological effects similar to those of antidepressants and may produce a superimposed impact when combined with other treatments.This article summarizes the findings on the use of physiotherapy to treat patients with depression over the past five years.It also discusses several methods of physiotherapy for treating depression from the aspects of clinical effect,mechanism of action,and disadvantages,thereby serving as a reference for the in-depth development of physiotherapy research. 展开更多

Efficacy observation on the treatment of knee osteoarthritis with warm needling and rehabilitation therapy 被引量:2

Objective To compare the clinical efficacies among the combined therapy of warm needling and rehabilitation, simple warm needling and rehabilitation therapy for knee osteoarthritis (OA). Methods Ninty cases of OA were divided randomly into three groups, 30 cases in each one. In an acupuncture and rehabilitation group, warm needling was applied at the acupoints such as Xuehai (血海SP 10), Neixiyan (内膝眼 EX-LE 4), Dubi (犊鼻 ST 35) and Zusanli(足三里ST 36) etc.. Additionally, rehabilitation therapy was combined such as joint gymnastics, isometric exercise, stretching exercise and endurance training. In the warm needling group, warm needling was given simply. In the rehabilitation group, rehabilitation therapy was only adopted. The joint pain scale and severity index scale of knee OA were taken as the efficacy assessment indices among the groups. Results The clinical cured rate was 36.7% (11/30) in acupuncture and rehabilitation group, which was superior to 13.3% (4/30) in warm needling group and 13.3% (4/30) in rehabilitation group (both P〈0.01) separately. The results of the joint pain scale and severity index scale of knee joint OA in three groups after 1 session and 2 sessions of treatment were reduced obviously as compared with those before treatment (P〈0.01, P〈0.05). After 2 sessions of treatment, every index in acupuncture and rehabilitation group was reduced much more obviously as compared with that in the other two groups (all P〈0,01). Conclusion Warm needling in combination with rehabilitation therapy has good efficacy on knee OA and relieves remarkably the symptoms of it, which is superior to those in either simple warm needling or rehabilitation training therapy. 展开更多

Objective:To determine the effects of Chinese medicine(CM)involving triple rehabilitation therapy on the progression of knee osteoarthritis(KOA).Methods:A total of 722 patients recruited from 38 community health service centers located in China from March 2013 to March 2017 were randomly divided into treatment and control groups equally,using a cluster randomization design.Health education combined with CM involving triple rehabilitation therapy for KOA(electro-acupuncture,Chinese medicinal herb fumigating-washing,and traditional exercises)was administered in the treatment group while conventional rehabilitation therapy(physical factor therapy,joint movement training,and muscle strength training)was administered in the control group.Patients with a visual analog scale(VAS)scores>A were treated with dispersible meloxicam tablets(7.5 mg,once daily).The Lequesne index scores,VAS scores,range of motion(ROM),lower limb muscle strength,knee joint circumference,quantitative scores of KOA symptoms,and the short-form 36 item health survey questionnaire(SF-36)scores were measured for each patient at 5 checkpoints(before treatment,at the 2nd week and the 4th week during the 4-week treatment period,at 1 month and 3 months after end of treatment),and adverse reactions were observed also.Results:A total of 696 patients completed the entire process,with 351 in the treatment group and 345 in the control group.At all treatment checkpoints,the treatment group demonstrated better outcomes than the control group with regard to the total Lequesne index scores,effective rate and improvement rate of the total Lequesne index scores,VAS scores,lower limb muscle strength,knee circumference,quantitative scores of KOA symptoms,and SF-36 scores as well(P<0.05 or P<0.01).No adverse reactions were encountered in this study.Conclusions:CM involving triple rehabilitation therapy can alleviate KOA-related pain and swelling,improve lower limb muscle strength,promote flexion and activity of the knee and improve the quality of life in patients undergoing KOA.It is suitable for patients with early or mid-stage KOA.(Registration No.ChiCTR-TRC-12002538) 展开更多

Objective:We describe the first-year implementation experience of an Instrumented Sensory Integration Therapy Program in Audiological&Balance Center patients.Design:This is a retrospective descriptive study.Participants included Seventy-three adults with diagnoses of acute,episodic,or chronic vestibular syndromes.They were classified into the following two groups:group 1 included 46 individuals treated with ISIT plus VRT,and group 2 included 27 individuals treated only with ISIT.Results:The Sensory Organization Test(SOT)for both groups showed a statistical significance for all three sensory inputs;visual systems(G1:p=0.0003;G2:p=0.0337),vestibular system(G1:p<0.0001;G2:p=0.0003),and balance as demonstrated by compound balance score(G1:p<0.0001;G2:p=0.0035),and balance percentage deficit(G1:p<0.0001;G2:p=0.0078).Conclusions:The severity and complexity of functional neurological disorders in the context of vestibular syndromes seem to require between 10 and 20 therapy sessions,and combined ISIT plus VRT appears to be more effective than ISIT as a monotherapy. 展开更多

Curative effect of hydrogen peroxide combined with silver ion disinfection on pelvic floor dysfunction

BACKGROUND Pelvic floor dysfunction(PFD)is related to muscle fiber tearing during childbirth,negatively impacting postpartum quality of life of parturient.Appropriate and effective intervention is necessary to promote PFD recovery.AIM To analyze the use of hydrogen peroxide and silver ion disinfection for vaginal electrodes in conjunction with comprehensive rehabilitation therapy for postpartum women with PFD.METHODS A total of 59 women with PFD who were admitted to the hospital from May 2019 to July 2022 were divided into two groups:Control group(n=27)received comprehensive rehabilitation therapy and observation group(n=32)received intervention with pelvic floor biostimulation feedback instrument in addition to comprehensive rehabilitation therapy.The vaginal electrodes were disinfected with hydrogen peroxide and silver ion before treatment.Intervention for both groups was started 6 weeks postpartum,and rehabilitation lasted for 3 months.Pelvic floor muscle voltage,pelvic floor muscle strength,vaginal muscle voltage,vaginal muscle tone,pelvic floor function,quality of life,and incidence of postpartum PFD were compared between the two groups.RESULTS Before comprehensive rehabilitation treatment,basic data and pelvic floor function were not significantly different between the two groups.After treatment,the observation group showed significant improvements in the maximum voltage and average voltage of pelvic floor muscles,contraction time of type I and type II fibers,pelvic floor muscle strength,vaginal muscle tone,vaginal muscle voltage,and quality of life(GQOLI-74 reports),compared with the control group.The observation group had lower scores on the pelvic floor distress inventory(PFDI-20)and a lower incidence of postpartum PFD,indicating the effectiveness of the pelvic floor biostimulation feedback instrument in promoting the recovery of maternal pelvic floor function.CONCLUSION The combination of the pelvic floor biostimulation feedback instrument and comprehensive rehabilitation nursing can improve pelvic floor muscle strength,promote the recovery of vaginal muscle tone,and improve pelvic floor function and quality of life.The use of hydrogen peroxide and silver ion disinfectant demonstrated favorable antibacterial efficacy and is worthy of clinical application. 展开更多

Study results in the last decades show that amount and quality of physical exercises,then the active participation,and now the cognitive involvement of patient in rehabilitation training are crucial to enhance recovery outcome of motor dysfunction patients after stroke.Rehabilitation robots mainly have been developed along this direction to satisfy requirements of recovery therapy,or focused on one or more of the above three points.Therefore,rehabilitation robot based on neuro-machine interaction has been proposed for the paralyzed limb training of post-stroke patient,which utilizes motor related EEG,UCSDI(Ultrasound Current Source Density Imaging),EMG for the robot control and feeds back the multi-sensory interaction information such as visual,auditory,force,haptic sensation to the patient simultaneously.This neuro-controlled and perceptual rehabilitation robot will bring great benefits to post-stroke patients.In order to develop such a kind of rehabilitation robot,some key technologies,such as non-invasive precise measurement and decoding of neural signals,realistic sensation feedback,coordinated control for both the rehabilitation robot and the patient,need to be solved.In this paper,some fundamental problems in developing and optimizing such a kind of rehabilitation robot based on neuro-machine interaction are proposed and discussed. 展开更多

To investigate the efficacy of a combination therapy on gluteal muscle contracture, 286 definitely diagnosed patients were subjected to surgical treatment, and then functional exercises and physical therapy. The patients with severe symptoms were asked to have a set of specially-designed functional exercises. All the patients were followed up for 3 to 24 months by hospital visit, correspondence or telephone interview. The effective rate was 100%, and the curative rate was up to 94.6%. Few patients developed complications and relapse was rare. It is concluded that the combination therapy, including surgical removal of diseased tissues, functional exercises and physical therapy, is an effective approach for the treatment of severe juvenile gluteal muscle contracture. 展开更多

Objective:The aim of this study was to observe the physical training influence on the limbs of postoperative breast cancer patients with adjuvant chemotherapy and do dual-response quality of life(QOL) according to clinical characteristics of patients.Methods:from February 2007 to December 2008,169 patients with postoperative breast cancer were included.We put all the patients into two groups randomly,including 80 patients in the treatment group and 89 patients in the control group.When the cycle of chemotherapy has been completed or on the 28th day postoperatively,all the patients need to be measured the function of upper limbs.The patients in the treatment group perform the rehabilitative training by our rehabilitation gymnastics,and the patients in the control group perform the rehabilitative training by themselves.When 6 cycles of adjuvant chemotherapy have been completed,we evaluated the differences of the function of upper limbs in patients.At the same time,we hand out the QOL questionnaire to patients and their doctors for doing the two-way assessment.Results:Compared with patients in control group,patients in treatment group have better in the abductive angle of shoulders and myodynamia(P = 0.000).By analysis on 156(92.3%) questionnaires,we found that the match rate of doctors and patients with physiological functions and physical symptoms were better than mentation and relationships.The better match rate between doctors and patients in QOL questionnaires suggested that the QOL of patients would be improved in the higher rate.Conclusion:Exercising the function of limbs in early could be beneficial to the patients with postoperative breast cancer and effectively improve patient's QOL.Meanwhile,we put doctors' questionnaires in the assessment of patients' QOL,and the results show that the assessment of patients' QOL is more objective and complete. 展开更多

Objective To compare the efficacy difference between the acupuncture-moxibustion therapy for tonifying the primary and strengthening the shoulder and the rehabilitation therapy in the treatment of posthemiplegic omalgia (PO). Methods Fifty-six cases of PO were randomized into an acupuncture-moxibustion group (30 cases) and a rehabilitation group (26 cases). In the acupuncture-moxibustion group, the mild moxibustion with moxa stick was applied to Guānyuán (关元 CV 4), Qìhǎi (气海 CV 6) and Zúsānlǐ (足三里 ST 36) and acupuncture was applied to Jiānzhēn (肩贞 SI 9), Nàoshū (臑俞 SI 10), Qūchí (曲池 LI 11), etc. In the rehabilitation group, the conventional rehabilitation therapy was provided, including massaging local tissues, anti-cramping activity, etc. In both groups, the treatment was given once per day, 15 treatments made one session and two sessions were required totally. The visual analogue scale (VAS), Fugl-Meyer assessment (FMA) and clinical neurologic impairment scale were adopted as the observation indicators for the evaluation of the analgesic effect and functional effect before and 30 days after treatment separately. Results The total analgesic effective rate was 100.0% (30/30) in the acupuncture-moxibustion group and was 96.2% (25/26) in the rehabilitation group, without presenting the statistical significant difference between two groups (P0.05). VAS scores were all reduced significantly after treatment in both groups as compared with those before treatment (all P0.01), but the comparison of the D-values before and after treatment did not reach the statistical significance between two groups (P0.05). The scores of FMA and the clinical neurologic impairment scale were all improved significantly after treatment in both groups (all P0.01). The improvements in the acupuncture-moxibustion group were superior to the rehabilitation group (all P0.01). Conclusion Both therapies can achieve the analgesic effect on PO. The recovery of the motor and neurological functions after the impairment of the affected upper limb is superior in the treatment with the acupuncture and moxibustion therapy for tonifying the primary and strengthening the shoulder as compared with the rehabilitation therapy. 展开更多

Objective: To observe the difference in clinical efficacy between governor vessel-unblocking and mind- regulating acupuncture therapy combined with conventional rehabilitation therapy and simply conventional rehabilitation therapy for sensory and motor dysfunction of patients with spinal cord injury. Methods: Forty patients with spinal cord injury (SCI) were randomly assigned into rehabilitation combined with acupuncture group (group A) and rehabilitation group (group B), with 20 patients in each group. In group A, governor vessel-unblocking and mind-regulating acupuncture therapy combined with conventional rehabilitation therapy was adopted, and Baihui(百会 GV 20), Fengffu (风府GV 16), Dazhui (大椎GV 14), Zhiyang (至阳GV 9), Mingmen (命门 GV 4) and Yfioyangguan (腰阳关 GV 3) were adopted as the main acupoints. Conventional rehabilitation therapy was also applied, including the rehabilitation training of joint, motion, respiration, urinary bladder and intestinal tract. In group B, conventional rehabilitation therapy was adopted as same as the group A. Treatment in the two groups was conducted for once a day, 6 times a week, and 12 weeks in total. ASIA motor score (MS), ASIA sensory score (SS) and activity of daily living (ADL) score of patients in the two groups were observed before and after the treatment. Results: Before treatment, the differences in MS, SS and ADL score of patients in the two groups were not statistically significant (all P 〉 0.05), and the results were comparable. After treatment, MS, SS and ADL score of patients in the two groups were all higher than that before the treatment (all P 〈 0.05), and MS, SS and ADL score of the patients in group A were all higher than that in group B (all P 〈 0.05). Conclusion: The curative effect of governor vessel-unblocking and mind-regulating acupuncture therapy combined with conventional rehabilitation therapy was superior to that of simply conventional rehabilitation therapy in the treatment of sensory and motor dysfunction of the patients with SCI. 展开更多
