BACKGROUND The midpoint transverse process to pleura(MTP)block,a novel technique for thoracic paravertebral block(TPVB),was first employed in laparoscopic renal cyst decortication.CASE SUMMARY Thoracic paravertebral nerve block is frequently employed for perioperative analgesia during laparoscopic cyst decortication.To address safety concerns associated with TPVBs,we administered MTP blocks in two patients prior to administering general anesthesia for laparoscopic cyst decortication.The MTP block was performed at the T9 level under ultrasound guidance,with 20 mL of 0.5%ropivacaine injected.Reduced sensation to cold and pinprick was observed from the T8 to T11 dermatome levels.Immediately postoperative Numeric Pain Rating Scale scores were 0/10 at rest and on movement,with none exceeding a mean 24 h numeric rating scale>3.CONCLUSION MTP block was effective technique for providing postoperative analgesia for patients undergoing laparoscopic renal cyst decortication. 展开更多

Objective: To compare the clinical therapeutic effects on the syndrome of L3 transverse process between thick silver warming needling and electric acupuncture. Objective: To compare the clinical therapeutic effects on the syndrome of L3 transverse process between thick silver warming needling and electric acupuncture. Methods: One hundred and twenty cases of the syndrome of L3 transverse process were randomized into observation group and the control, 60 cases for each one. Thick silver warming needling was applied in observation group and electric acupuncture in the control. Results: The curative rate was 81.7% in observation group and 58.3% in the control, indicating significant difference (P<0.01). The significant difference in the therapeutic effects between two groups had not been discovered yet for the cases within 3 months of sickness (P>0.05), but, the therapeutic effect in observation group was superior to that in the control for the cases over 3 months of sickness (P<0.01). Conclusion: The therapeutic effects of thick silver warming needling and electric acupuncture were quite advantageous on the syndrome of L3 transverse process, but that of thick silver warming needling was superior to electric acupuncture. 展开更多

OBJECTIVE: To explore the long-term effects and pain relief mechanism of acupotomy by observing changes in nitric oxide synthase (NOS) and beta-en- dorphin (~3-EP) in the hypothalamus, spinal cord, and peripheral blood of rats with third lumbar ver- tebrae (L3) transverse process syndrome. METHODS: Twenty-eight SD rats were randomly as- signed to normal, model, electroacupuncture (EA), and acupotomy group. The last three groups were put through an operation to emulate L3 transverse process syndrome. Fourteen days after the simulation operation, EA and acupotomy treatments were applied to the respective groups. Fifty-six days afterthe simulation operation, biochemistry tests and enzyme-linked immunosorbent assay were used to measure NOS and 13-EP in the hypothalamus, spinal cord, and peripheral blood. RESULTS: Rats with the simulation operation showed significantly higher levels of NOS and II3-EP in the hypothalamus, spinal cord, and peripheral blood than those in the normal group. The EA and acupotomy groups had significantly lower levels of NOS and β-EP than those in the model group. There was no statistical difference between the EA and acupotomy groups. CONCLUSION: EA and acupotomy treatments significantly lowered NOS and β-EP levels in the hypothalamus, spinal cord, and peripheral blood and alleviated L3 transverse process syndrome. 展开更多

As a continuation of a recent linear analysis by Mao et al.(Acta Mech Sin,2010,26:355),in this paper we propose a general theoretical formulation for the compressing process in complex Newtonian fluid flows,which covers gas dynamics,aeroacoustics,nonlinear thermoviscous acoustics,viscous shock layer,etc.,as its special branches.The principle on which our formulation is based is the maximally natural and dynamic Helmholtz decomposition of the Navier-Stokes equation,along with the kinematic Helmholtz decomposition of the velocity field.The central results are the new dilatation equation and velocity-potential equation,which are the counterparts of vorticity transport equation and vector stream-function equation for the shearing process,respectively.Various couplings of the compressing process with shearing and thermal processes,including its physical sources,are carefully identified.While the possible applications and influences of the new formulation are yet to be explored,our preliminary discussion on the pros and cons of previous formulations pertain to acoustic analogy and that on the process splitting and coupling in highly compressible turbulence indicates that at least the formulation can serve as a new frame of reference by which one may gain some additional insight and thereby develop new approaches to the multi-process complex flow problems. 展开更多
