METHODS: We retrospectively reviewed vascular injuries in 200 consecutive patients who underwent videolaparoscopic cholecistectomy from 2003 to 2005. One hundred and one patients with placement of radial expanding trocars were assigned into group A and 99 patients with placement of pyramidal tipped trocars into group B. All the patients were submitted to open access according to Hasson for the first trocar. RESULTS: Bleeding did not occur at the intraoperative cannula-site in group A. However, it occurred at the intraoperative cannula-site of 7 patients (7.1%) in group B, with a statistically significant difference (P < 0.01). No mortality was registered. More vascular lesions were found in group B. CONCLUSION: The advantage of Hasson technique is that peritoneal cavity access is gained under direct vision, preventing most severe injuries. The open technique with radial expanding trocars is recommended for secure access to the abdominal cavity in videolaparoscopy. Great care should be taken to avoid major complications and understanding the abdominal wall anatomy is important for reducing bleeding during or after s placement of trocars.展开更多

Methods Eighty cases of facial spasm were randomly divided into treatment group and control group, 40 cases in each group,treated respectively by abdominal acupuncture combined with local selected points and local points simply. After 30 sessions of treatment, the therapeutic effect was evaluated. Results In the treatment group, the total effective rate reached 92.5%, the curative rate 75.0%; while in the control group, the total effective rate was 80.0%, the curative rate 55.0% with a significant difference between the two groups ( P<0.05 ). Conclusion The therapeutic effect of abdominal acupuncture combined with local points is superior to that of simple local selection of points in the treatment of facial spasm.展开更多

Each patient underwent a puncture of the lesion with both standard 22-gauge(G) AN(Echo Tip Ultra; Cook Medical, Bloomington, Indiana, United States) and the novel 22 G PC(Echo Tip Pro Core; Cook Medical, Bloomington, Indiana, United States) in a randomized fashion; histology was attempted in the PC group only. The main study endpoint was the overall diagnostic accuracy, including the contribution of histology to the final diagnosis. Secondary outcome measures included material adequacy, number of needle passes, and complications.RESULTS Fifty six consecutive patients(29 men; mean age 68 years) with pancreatic lesions(n = 38), lymphadenopathy(n = 13), submucosal tumors(n = 4), or others lesions(n = 1) underwent EUS-FNA using both of the needles in a randomized order. AN and PC reached similar overall results for diagnostic accuracy(AN: 88.9 vs PC: 96.1, P = 0.25), specimen adequacy(AN: 96.4% vs PC: 91.1%, P = 0.38), mean number of passes(AN: 1.5 vs PC: 1.7, P = 0.14), mean cellularity score(AN: 1.7 vs PC: 1.1, P = 0.058), and complications(none). A diagnosis on the basis of histology was achieved in the PC group in 36(64.3%) patients, and in 2 of those as the sole modality. In patients with available histology the mean cellularity score was higher for AN(AN: 1.7 vs PC: 1.0, P = 0.034); no other differences were of statistical significance.CONCLUSION Both needles achieved high overall diagnostic yields and similar performance characteristics for cytological diagnosis; histological analysis was only possible in 2/3 of cases with the new needle.展开更多

Objective To observe the clinical effect of acupoint catgut embedding combined with abdominal acupuncture for treatment of abdominal obesity under the theoretical guidance of regulating the three energizer.Methods One...Objective To observe the clinical effect of acupoint catgut embedding combined with abdominal acupuncture for treatment of abdominal obesity under the theoretical guidance of regulating the three energizer.Methods One hundred and twenty patients were randomly divided into acupoint catgut embedding group(group A),abdominal acupuncture group(group B),acupoint catgut embedding combined with abdominal acupuncture group(group C) and blank control group(group D) based on blocked random method,with 30 cases each;the first three groups based on the principle of regulating three energizer were respectively given acupoint catgut embedding at Zhongwan(中脘 CV12),Tianshu(天枢 ST 25),Yinjiao(阴交 CV 7),Zhigou(支沟 TE 6),Guanyuan(关元 CV 4),Zusanli(足三里 ST 36) with catgut embedding once every 7 days,three times for each course of treatment,abdominal acupuncture once every3 days,seven times for each course of treatment.Results The therapeutic effect of each group after treatment was respectively 46.7%(14/30),43.3%(13/30),76.7%(23/30) and 26.7%(8/30),with statistically significant difference.For comparison of waist circumference difference,there was no statistical significance between acupoint catgut embedding combined with abdominal acupuncture and acupoint catgut embedding group(P=0.54),and for comparison of weight difference,there was statistical significance in terms of therapeutic effect between acupoint catgut embedding combined with abdominal acupuncture and A,B,D groups(4.49±0.59 vs 2.64±0.53,2.87±0.44,1.45±0.26,all P〈0.05).Conclusion Acupoint catgut embedding combined with abdominal acupuncture had the best effect in terms of total effective rate for treatment of abdominal obesity,equivalent therapeutic effect to simple acupoint catgut embedding in terms of waist circumference reduction,and the best therapeutic effect in terms of weight reduction.展开更多

Methods Treatments were carried out on 52 patients with chronic diarrhea by using the left ascending and right descending method. Three acupoints at the left side, namely Xiàng (陷谷 ST 43), Zúsānl (足三 里 ST 36) and Qūchí (曲池 LI 11), and Lièqūe (列缺 LU 7) at the right side were selected, moxibustion was also carried out according to syndrome differentiation. The operation of left ascending and right descending method were firstly left and then right, first down and then up, firstly four limbs and then abdomen; the thumb moved backward and the index finger moved forward during needle twisting for the acupoints at the left side, and it was on the contrary for the right side. The needles were retained for 1 h for deficiency, and the pinholes were pressed when the needles were withdrawn; the needles were retained for 0.5 h for excess syndrome and newly-contracted disease, and the pinholes were enlarged by shaking the needles when they were withdrawn. Except that the needling was carried out twice a day for the patients with desertion following chronic diarrhea, the needling was carried out once a day, five days as a treatment course, the patients were given an interval for two days after every two treatment courses, and the therapeutic effect was observed after five treatment courses. Results totally 36 patients were cured, among which 11 patients were cured after one treatment course, 19 patients were cured after two to three treatment courses and 6 patients were cured after four to five treatment courses; eight patients were improved; six patients were not cured; the total effective rate was 88.0%. Conclusion the acupuncture with left ascending and right descending method has satisfactory therapeutic effect in treating chronic diarrhea.展开更多

Methods Thirty-six cases of postsurgical gastroparesis of abdominal cancer were treated with (1) acupuncture at Weishiu (胃俞 BL 21), Shangwan (上脘 CV 13), Zhangwan (中脘CV 12), Xiawan (下脘 CV 10) and Tianshu (天枢 ST 25) to dredging the meridians, (2) quick cupping on local area to promote gastrointestinal motility and (3) mild moxibustion at CV 12, Shanque (神阙 CV 8) and Zusanli (足三里 ST 36). The treatment was given once a day, 5 times made a course. The clinical efficacy was assessed after two courses of treatment. Results After the treatment, 27 cases were cured, 6 cases were effective and 3 cases were failed, and the total effective rate was 91.7% (33/36). Conclusion The acupuncture-moxibustion combined with cupping has remarkable clinical efficacy on postsurgical gastroparesis of abdominal cancer.展开更多

Me...Objective To compare the difference in the therapeutic effects on diarrhea- type irritable bowel syndrome (D-IBS) between acupuncture based on soothing the liver and strengthening the spleen and western medicine. Methods Seventy cases were randomized into an acupuncture group and a western medicine group, 35 cases in each one. In the acupuncture group, the conventional acupuncture was applied at Tianshu {天枢 ST 25), Zusanli (足三里 ST 36), Shangjuxu {上巨虚 ST 37), Sanyinjiao (三阴交 SP 6), Taichong (太冲 LR 3) and so on. Electric stimulation was supplemented at bilateral ST 25. The treatment was given once a day, 3-4 treatments in a week. In the western medicine group, pinaverium bromide (dicetel) was prescribed for oral administration, 50 mg each time, three times a day. Four weeks made one session in the two groups. Separately, before treatment and after 1 session treatment, the integral of clinical symptom and the score in the scale of the quality of life measure for irritable bowel syndrome (IBS-Q OL) were observed for the patients in the two groups. The efficacy and recurrence rate were assessed in the two groups. Results The symptom integral and IBS-QOL score in the two groups were improved significantly after treatment Call P〈0.01), and the improvements in the acupuncture group were superior to that in the western medicine group (P〈0.01). The effective rate was 94.3% {33/35) in the acupuncture group, which was superior to 77.1% (27/35) in the western medicine group (P〈0.01). In 3 months, the recurrence rate was 36.4% (12/33) in the acupuncture group, which was lower apparently than 72.0% (18/25) in the western medicine group (P〈O.01). Conclusion Acupuncture therapy based on soothing the liver and strengthening the spleen achieves the superior efficacy on D-IBS as compared with western medication, pinaverium bromide. This therapy improves apparently the quality of life of the patients and is lower in recurrence rate.展开更多

Methods One hundred and twenty patients with diarrhea irritable bowel syndrome were randomly divided into a warm needling group and a western medicine group by random digit table method, 60 patients were included in each group. Tiānshū (天枢 ST 25), Zúsānl (足三里 ST 36) and Sānyīnjiāo (三阴交 SP 6) were selected as the main acupoints for the warm needling group, the needles were retained for 30 min, once a day, an interval of 1 day was set up after treatment for six days, and the treatment were carried out for continuous four weeks. The patients in the western medicine group were orally administered with 2 mg loperamide hydrochloride capsules, 3 times a day, and the treatment were carried out for continuous four weeks. The treatment effects were compared between the two groups, and the recurrence rates were counted after for six-month followup. Results The total effective rate in the warm needling group was 86.7% (52/60) and the total effective rate in the western medicine group was 71.7% (43/60), and the difference between the two groups was statistically significant (P0.05); after six-month follow-up, the recurrence rate in the warm needling group was 21.4% (6/28), while the recurrence rate in the western medicine group was 54.5% (12/22), and the difference between the two groups was statistically significant (P0.05). Conclusion The warm needling therapy shows relatively satisfactory treatment effects in the treatment of diarrhea irritable bowel syndrome and the recurrence rate is relatively low.展开更多

Methods Fasting 7:30 and 8:00 a.m. on one day, two and three days before acupuncture, diabetes. Acupuncture was applied at Yishu (胰俞 Extra), Feishu (肺俞BL 13), Pisha which was (脾俞 BL 20), Shenshu (肾俞 BL 23), Sanyinjiao (三阴交 SP 6), Taxixi (太溪 KI 3) and auxiliary points, followed by needle remaining for 30 minutes, for continuous 3 days. Blood glucose values measured immediately after acupuncture were compared with those obtained before acupuncture. Results Before acupuncture application, no significant differences were found between the values measured at 7:30 and 8:00 a.m. (P〉0.05) in fasting blood glucose values. Fasting blood glucose values measured at 8:00 were lower than those at 7:30 on the three days during acupuncture treatment (P〈O.05), and they were also lower than those measured at 8:00 before acupuncture (P〈0.05). Conclusion Acupuncture has immediate effect of lowering blood sugar.展开更多

This therapy improves apparently the quality of life of the patients and is lower in recurrence rate.
文摘Objective To observe the clinical effect of warm needling in the treatment of diarrhea irritable bowel syndrome. Methods One hundred and twenty patients with diarrhea irritable bowel syndrome were randomly divided into a warm needling group and a western medicine group by random digit table method, 60 patients were included in each group. Tiānshū (天枢 ST 25), Zúsānl (足三里 ST 36) and Sānyīnjiāo (三阴交 SP 6) were selected as the main acupoints for the warm needling group, the needles were retained for 30 min, once a day, an interval of 1 day was set up after treatment for six days, and the treatment were carried out for continuous four weeks. The patients in the western medicine group were orally administered with 2 mg loperamide hydrochloride capsules, 3 times a day, and the treatment were carried out for continuous four weeks. The treatment effects were compared between the two groups, and the recurrence rates were counted after for six-month followup. Results The total effective rate in the warm needling group was 86.7% (52/60) and the total effective rate in the western medicine group was 71.7% (43/60), and the difference between the two groups was statistically significant (P0.05); after six-month follow-up, the recurrence rate in the warm needling group was 21.4% (6/28), while the recurrence rate in the western medicine group was 54.5% (12/22), and the difference between the two groups was statistically significant (P0.05). Conclusion The warm needling therapy shows relatively satisfactory treatment effects in the treatment of diarrhea irritable bowel syndrome and the recurrence rate is relatively low.
文摘Objective To observe blood sugar values were measured at respectively, in 40 patients with type lI instant effect of acupuncture on lowering blood glucose. Methods Fasting 7:30 and 8:00 a.m. on one day, two and three days before acupuncture, diabetes. Acupuncture was applied at Yishu (胰俞 Extra), Feishu (肺俞BL 13), Pisha which was (脾俞 BL 20), Shenshu (肾俞 BL 23), Sanyinjiao (三阴交 SP 6), Taxixi (太溪 KI 3) and auxiliary points, followed by needle remaining for 30 minutes, for continuous 3 days. Blood glucose values measured immediately after acupuncture were compared with those obtained before acupuncture. Results Before acupuncture application, no significant differences were found between the values measured at 7:30 and 8:00 a.m. (P〉0.05) in fasting blood glucose values. Fasting blood glucose values measured at 8:00 were lower than those at 7:30 on the three days during acupuncture treatment (P〈O.05), and they were also lower than those measured at 8:00 before acupuncture (P〈0.05). Conclusion Acupuncture has immediate effect of lowering blood sugar.
