Objective To investigate the distribution and antimicrobial susceptibility of causative microorganisms recovered from patients with intra-abdominal infections(IAIs).Methods A total of 2,926 bacterial and fungal strain...Objective To investigate the distribution and antimicrobial susceptibility of causative microorganisms recovered from patients with intra-abdominal infections(IAIs).Methods A total of 2,926 bacterial and fungal strains were identified in samples collected from 1,679 patients with IAIs at the Peking Union Medical College Hospital between 2011 and 2021.Pathogenic bacteria and fungi were identified using matrix-assisted laser desorption/ionization time-of-flight mass spectrometry.Antimicrobial susceptibility testing(AST)was performed using the VITEK 2 compact system and the Kirby–Bauer method.AST results were interpreted based on the M100-Ed31 clinical breakpoints of the Clinical and Laboratory Standards Institute.Results Of the 2,926 strains identified,49.2%,40.8%,and 9.5%were gram-negative bacteria,gram-positive bacteria,and fungi,respectively.Escherichia coli was the most prevalent pathogen in intensive care unit(ICU)and non-ICU patients;however,a significant decrease was observed in the isolation of E.coli between 2011 and 2021.Specifically,significant decreases were observed between 2011 and 2021 in the levels of extended-spectrumβ-lactamase(ESBL)-producing E.coli(from 76.9%to 14.3%)and Klebsiella pneumoniae(from 45.8%to 4.8%).Polymicrobial infections,particularly those involving co-infection with gram-positive and gram-negative bacteria,were commonly observed in IAI patients.Moreover,Candida albicans was more commonly isolated from hospital-associated IAI samples,while Staphylococcus epidermidis had a higher ratio in community-associated IAIs.Additionally,AST results revealed that most antimicrobial agents performed better in non-ESBL-producers than in ESBL-producers,while the overall resistance rates(56.9%–76.8%)of Acinetobacter baumanmii were higher against all antimicrobial agents than those of other common gram-negative bacteria.Indeed,Enterococcus faecium,Enterococcus faecalis,S.epidermidis,and S.aureus were consistently found to be susceptible to vancomycin,teicoplanin,and linezolid.Similarly,C.albicans exhibited high susceptibility to all the tested antifungal drugs.Conclusion The distribution and antimicrobial susceptibility of the causative microorganisms from patients with IAls were altered between 2011 and 2021.This finding is valuable for the implementation of evidence-based antimicrobial therapy and provides guidance for the control of hospital infections.展开更多

This study aims to optimize causative factors using logistic regression(LR) and an artificial neural network(AN...Landslide susceptibility maps(LSMs) play a vital role in assisting land use planning and risk mitigation. This study aims to optimize causative factors using logistic regression(LR) and an artificial neural network(ANN) to produce a LSM. The LSM is produced with 11 causative factors and then optimized using forward-stepwise LR(FSLR), ANN, and their combination(FSLR-ANN) until eight causative factors were found for each method. The ANN method produced superior validation results compared with LR. The ROC values for the training data set ranges between 0.8 and 0.9. On the other hand, validation with the percentage of landslide fall into LSM class high and very high, ANN method was higher(92.59%) than LR(82.12%). FSLR-ANN with nine causative factors gave the best validation results with respect to area under curve(AUC) values, and validation with the percentage of landslide fall into LSM class high and very high. In conclusion, ANN was found to be better than LR when producing LSMs. The best Optimization was combination of FSLR-ANN with nine causative factors and AUC success rate 0.847, predictive rate 0.844 and validation with landslide fall into high and very high class with 91.30%. It is an encouraging preliminary model towards a systematic introduction of FSLR-ANN model for optimization causative factors in landslide susceptibility assessment in the mountainous area of Ujung Loe Watershed.展开更多

Causative verbs in MA are characterized by the systematic gemination of their medial consonants. However, it is not easy to determine the morphological nature of the causative morpheme involved in this derivation. Also, it is not clearly known why the causative morpheme gets realized exactly on the second segment of the base form.Therefore, we seek to achieve the following goals. First, we intend to determine the nature of the causative morpheme. Second, we aim to explain why the causative morpheme is realized on the second segment of the base form. In this respect, we show that the causative morpheme is represented by a featureless consonantal mora that targets the second segment of the base root, turning it into a geminate. We also show that the causative morpheme gets infixed thanks to the privileged status of root-initial segments.展开更多
The main objective of this study is to give descriptions of formal and functional aspects of causative constructions in Afaan Oromoo.To achieve the objective,written texts,native speaker informants and introspections ...The main objective of this study is to give descriptions of formal and functional aspects of causative constructions in Afaan Oromoo.To achieve the objective,written texts,native speaker informants and introspections are predominantly used as sources of data.The findings reveal that the three structural aspects of causatives-morphological,lexical and syntactic-are used in the language.Morphological causatives are highly productive,and affixes with-s and-i in several combinations as well as-eess are employed for such purposes.Causative Morphemes are detected to derive causatives of basic verb stems of different semantic categories and to involve causations in word-class changing.There are also simple and complex causations in which there are several causatives suffixes,causers and micro-events indicated morphologically and syntactically.Semantically,direct,indirect,and assistive/cooperative causatives are identified.Pseudo-causatives are uncovered as peculiar futures of the language too.There are,even,structures with explicit causative affixes which are called subjectless causatives,but they do not show any causal relations between the participants in the structure.展开更多
This paper explores the nature of the post-syntactic operations responsible for the representations of the linear order of terminal nodes.In particular,it argues in favor of a unified model of the morphosyntax and mor...This paper explores the nature of the post-syntactic operations responsible for the representations of the linear order of terminal nodes.In particular,it argues in favor of a unified model of the morphosyntax and morphophonology,wherein the theory of Distributed Morphology and Optimality Theory operate in a single module.The testing ground is an investigation of the formation of morphological causatives in Moroccan Arabic.Herein,the process of realizing causatives is morphological gemination,whereby the second consonant of the root is doubled.Investigating the question of what triggers the infixal process,I argue against the linearization algorithm suggested in Embick&Noyer(2001),Embick&Marantz(2008),and Embick(2006,2010).Instead,the claim I defend here is that the onus of the linearization process falls on the prosody in Arabic,the central assumption being that the morphosyntactic structure,the output of the syntactic derivation,is the input to OT morphophonological constraints.These constraints are responsible for the linearization of the terminal nodes of the syntactic derivation.I show that adopting one theory over others misses important generalizations about the language.展开更多
BACKGROUND Glucose imbalance is common in total parenteral nutrition(TPN).Hypoglycemia seems to be less frequent than hyperglycemia,but it influences the clinical outcome to a greater extent.Therefore,it should be eff...BACKGROUND Glucose imbalance is common in total parenteral nutrition(TPN).Hypoglycemia seems to be less frequent than hyperglycemia,but it influences the clinical outcome to a greater extent.Therefore,it should be effectively prevented and treated.However,there is no relevant report on how to treat hypoglycemia caused by TPN in patients with liver cell injury.CASE SUMMARY We present three patients with liver cell injury who developed severe hypo-glycemia during or after TPN infusion.The causes of severe hypoglycemia and glucose-raising strategies were discussed.According to the physiological charac-teristics of the hepatocellular injury,the ratio of nutrition components prescribed in TPN was appropriately adjusted for the three cases.We simultaneously reduced the dose of insulin and fat emulsion,and increased the dose of glucose in TPN.The blood glucose level was restored to normal range and clinical symptoms were eliminated.CONCLUSION When hypoglycemia occurs during or after TPN in patients with hepatocellular injury,physicians need to simultaneously reduce insulin and fat emulsion,and increase glucose,and correct severe hypoglycemia in time to reduce its adverse consequences.展开更多

The etiology of upper gastrointestinal bleeding(UGIB)varies by age,from newborns to adolescents,with some of the causes overlapping between age groups.While particular causes such as vitamin K deficiency and cow’s mi...The etiology of upper gastrointestinal bleeding(UGIB)varies by age,from newborns to adolescents,with some of the causes overlapping between age groups.While particular causes such as vitamin K deficiency and cow’s milk protein allergy are limited to specific age groups,occurring only in neonates and infants,others such as erosive esophagitis and gastritis may be identified at all ages.Furthermore,the incidence of UGIB is variable throughout the world and in different hospital settings.In North America and Europe,most UGIBs are nonvariceal,associated with erosive esophagitis,gastritis,and gastric and duodenal ulcers.In recent years,the most common causes in some Middle Eastern and Far Eastern countries are becoming similar to those in Western countries.However,variceal bleeding still predominates in certain parts of the world,especially in South Asia.The most severe hemorrhage arises from variceal bleeding,peptic ulceration,and disseminated intravascular coagulation.Hematemesis is a credible indicator of a UGI source of bleeding in the majority of patients.Being familiar with the most likely UGIB causes in specific ages and geographic areas is especially important for adequate orientation in clinical settings,the use of proper diagnostic tests, and rapid initiation of the therapy. The fundamental approach to the management of UGIBincludes an immediate assessment of severity, detecting possible causes, and providing hemodynamic stability,followed by early endoscopy. Unusual UGIB causes must always be considered when establishing a diagnosis inthe pediatric population because some of them are unique to children. Endoscopic techniques are of significantdiagnostic value, and combined with medicaments, may be used for the management of acute bleeding. The taxonomic status of Bufo species in East Asia,particularly those closely related to B.gargarizans,has been controversial for over half a century,and recent molecular studies further fueled the dispute.Whether B.an...The taxonomic status of Bufo species in East Asia,particularly those closely related to B.gargarizans,has been controversial for over half a century,and recent molecular studies further fueled the dispute.Whether B.andrewsi,B.bankorensis,B.minshanicus,B.sachalinensis,B.tibetanus,and B.tuberculatus constitute valid species or are geographic or ecological variants of B.gargarizans is at the centre of the discussion.Most of these forms possess morphological diagnostic traits,but those differences are minimal and often inconsistent.Molecular data produced many contradictory patterns.Lack of clear diagnostic characters,different usage or definition of various names,mis-identified specimens,limited amount of data or samples,and frequent inter-specific hybridization may all have contributed to the current controversies.Some contradiction may also result from differences between similarity-based clustering and gene tree-based branching patterns.Nevertheless,a dominant allopatric differentiation pattern within this species complex is apparent,and several regional population groups have been identified.Some of the groups resemble old names(e.g.B.minshanicus,B.tibetanus),while others are new.These groups may represent geographic or ecological variants,incipient species,or valid species,and the current data do not warrant a convincing taxonomic revision.To move forward,a single study with comprehensive sampling and genomic data is needed to establish a solid framework,which will define naturally existing population clusters/lineages,and detect their levels of divergence and approximate geographic boundaries.Morphological and ecological examination will follow to understand their biology,determine their taxonomic status,and identify consistent diagnostic characters if necessary.展开更多
Emotion cause extraction(ECE)task that aims at extracting potential trigger events of certain emotions has attracted extensive attention recently.However,current work neglects the implicit emotion expressed without an...Emotion cause extraction(ECE)task that aims at extracting potential trigger events of certain emotions has attracted extensive attention recently.However,current work neglects the implicit emotion expressed without any explicit emotional keywords,which appears more frequently in application scenarios.The lack of explicit emotion information makes it extremely hard to extract emotion causes only with the local context.Moreover,an entire event is usually across multiple clauses,while existing work merely extracts cause events at clause level and cannot effectively capture complete cause event information.To address these issues,the events are first redefined at the tuple level and a span-based tuple-level algorithm is proposed to extract events from different clauses.Based on it,a corpus for implicit emotion cause extraction that tries to extract causes of implicit emotions is constructed.The authors propose a knowledge-enriched jointlearning model of implicit emotion recognition and implicit emotion cause extraction tasks(KJ-IECE),which leverages commonsense knowledge from ConceptNet and NRC_VAD to better capture connections between emotion and corresponding cause events.Experiments on both implicit and explicit emotion cause extraction datasets demonstrate the effectiveness of the proposed model.展开更多
BACKGROUND Platelet transfusion is of great significance in the treatment of thrombocytopenia caused by myelosuppression during intensive chemotherapy in patients with acute leukemia.In recent years,with platelet tran...BACKGROUND Platelet transfusion is of great significance in the treatment of thrombocytopenia caused by myelosuppression during intensive chemotherapy in patients with acute leukemia.In recent years,with platelet transfusion increasing,ineffective platelet transfusion has become increasingly prominent.Generally speaking,platelet antibodies can be produced after repeated transfusion,thus rendering subsequent platelet transfusion ineffective.We report a case of first platelet transfusion refractoriness(PTR)in a patient with acute myelocytic leukemia(AML).Due to the rarity of such cases in clinical practice,there have been no relevant case reports so far.CASE SUMMARY A 51-year-old female patient attended the hospital due to throat pain and abnormal blood cells for 4 d.Her diagnosis was acute myelocytic leukemia[M2 type Fms related receptor tyrosine kinase 3,Isocitrate Dehydrogenase 1,Nucleophosmin 1,Neuroblastoma RAS viral oncogene homolog(+)high-risk group].She was treated with"IA"(IDA 10 mg day 1-3 and Ara-C 0.2 g day 1-5)chemotherapy.When her condition improved,the patient was discharged from the hospital,instructed to take medicine as prescribed by the doctor after discharge,and returned to the hospital for further chemotherapy on time.CONCLUSION We report a rare case of first platelet transfusion failure in a patient with AML during induction chemotherapy,which may be related to the production of platelet antibodies induced by antibiotics and excessive tumor load.This also suggests that we should consider the influence of antibiotics when the rare situation of first platelet transfusion failure occurs in patients with AML.When platelet antibodies are produced,immunoglobulins can be used to block antibodies,thereby reducing platelet destruction.For patients with PTR,both immune and non-immune factors need to be considered and combined in clinical practice along with individualized treatment to effectively solve the problem.展开更多

Using regional meteorological stations and ERA5 reanalysis data, the causes...An extreme monsoonal heavy rainfall event lasted for nine days and recurred in the interior of northern south China from June 13 to 21, 2022. Using regional meteorological stations and ERA5 reanalysis data, the causes of this extreme monsoonal rainfall event in south China were analyzed and diagnosed. The results are shown as follows. A dominant South Asian high tended to be stable near the Qinghai-Tibet Plateau, providing favorable upper-level dispersion conditions for the occurrence of heavy rainfall in south China. A western Pacific subtropical high dominated the eastern part of the South China Sea, favoring stronger and more northward transport of water vapor to the northern part of south China at lower latitudes than normal. The continuous heavy precipitation event can be divided into two stages. The first stage(June 13-15) was the frontal heavy rainfall caused by cold air(brought by an East Asian trough)from the mid-latitudes that converged with a monsoonal airflow. The heavy rains occurred mostly in the area near a shear in front of the center of a synoptic-system-related low-level jet(SLLJ), and the jet stream and precipitation were strongest in the daytime. The second stage(June 16-21) was the warm-sector heavy rainfall caused by a South China Sea monsoonal low-level jet penetrating inland. The heavy rainfall occurred on the windward slope of the Nanling Mountains and in the northern part of a boundary layer jet(BLJ). The BLJ experienced five nighttime enhancements, corresponding well with the enhancement of the rainfall center, showing significant nighttime heavy rainfall characteristics. Finally, a conceptual diagram of inland-type warm-sector heavy rainfall in south China is summarized.展开更多

文摘Objective To investigate the distribution and antimicrobial susceptibility of causative microorganisms recovered from patients with intra-abdominal infections(IAIs).Methods A total of 2,926 bacterial and fungal strains were identified in samples collected from 1,679 patients with IAIs at the Peking Union Medical College Hospital between 2011 and 2021.Pathogenic bacteria and fungi were identified using matrix-assisted laser desorption/ionization time-of-flight mass spectrometry.Antimicrobial susceptibility testing(AST)was performed using the VITEK 2 compact system and the Kirby–Bauer method.AST results were interpreted based on the M100-Ed31 clinical breakpoints of the Clinical and Laboratory Standards Institute.Results Of the 2,926 strains identified,49.2%,40.8%,and 9.5%were gram-negative bacteria,gram-positive bacteria,and fungi,respectively.Escherichia coli was the most prevalent pathogen in intensive care unit(ICU)and non-ICU patients;however,a significant decrease was observed in the isolation of E.coli between 2011 and 2021.Specifically,significant decreases were observed between 2011 and 2021 in the levels of extended-spectrumβ-lactamase(ESBL)-producing E.coli(from 76.9%to 14.3%)and Klebsiella pneumoniae(from 45.8%to 4.8%).Polymicrobial infections,particularly those involving co-infection with gram-positive and gram-negative bacteria,were commonly observed in IAI patients.Moreover,Candida albicans was more commonly isolated from hospital-associated IAI samples,while Staphylococcus epidermidis had a higher ratio in community-associated IAIs.Additionally,AST results revealed that most antimicrobial agents performed better in non-ESBL-producers than in ESBL-producers,while the overall resistance rates(56.9%–76.8%)of Acinetobacter baumanmii were higher against all antimicrobial agents than those of other common gram-negative bacteria.Indeed,Enterococcus faecium,Enterococcus faecalis,S.epidermidis,and S.aureus were consistently found to be susceptible to vancomycin,teicoplanin,and linezolid.Similarly,C.albicans exhibited high susceptibility to all the tested antifungal drugs.Conclusion The distribution and antimicrobial susceptibility of the causative microorganisms from patients with IAls were altered between 2011 and 2021.This finding is valuable for the implementation of evidence-based antimicrobial therapy and provides guidance for the control of hospital infections.
文摘Landslide susceptibility maps(LSMs) play a vital role in assisting land use planning and risk mitigation. This study aims to optimize causative factors using logistic regression(LR) and an artificial neural network(ANN) to produce a LSM. The LSM is produced with 11 causative factors and then optimized using forward-stepwise LR(FSLR), ANN, and their combination(FSLR-ANN) until eight causative factors were found for each method. The ANN method produced superior validation results compared with LR. The ROC values for the training data set ranges between 0.8 and 0.9. On the other hand, validation with the percentage of landslide fall into LSM class high and very high, ANN method was higher(92.59%) than LR(82.12%). FSLR-ANN with nine causative factors gave the best validation results with respect to area under curve(AUC) values, and validation with the percentage of landslide fall into LSM class high and very high. In conclusion, ANN was found to be better than LR when producing LSMs. The best Optimization was combination of FSLR-ANN with nine causative factors and AUC success rate 0.847, predictive rate 0.844 and validation with landslide fall into high and very high class with 91.30%. It is an encouraging preliminary model towards a systematic introduction of FSLR-ANN model for optimization causative factors in landslide susceptibility assessment in the mountainous area of Ujung Loe Watershed.
文摘The traditional Japanese grammar teaching often only pays attention to the interpretation of syntax and the integrity of grammar structure. This violates the cultivation of communicative competence, and is not in conformity with the society’s requirements of applied foreign language talents. Cognitive linguistics theory, which links language form with semantic concept, reveals the internal relation of man’s thinking and language. If we can subtly apply cognitive linguistic theory into Japanese grammar teaching to explore the cognitive process in the speakers’ brain while expressing, we can get a good understanding of diffi cult points and “special case”. This paper explores the introductory methods and efficacy of the cognitive linguistics theory applied in Japanese grammar teaching method, by lecturing causative sentences an example.
文摘This work interrogates why certain real-world events that meet criteria for being considered causative events cannot be expressed using Hupa’s morphological or syntactic causative constructions,but must be encoded using one of two periphrastic constructions in which the Causer or cause is not marked as an argument.Based on fieldwork with a native speaker,I probe into these two periphrastic constructions in depth,accounting for their distributions through an appeal to Næss’s(2007)account of semantic transitivity.
文摘This paper aims at investigating the morphophonological make-up of derived causatives in Moroccan Arabic within the framework of Optimality Theory (Prince &Smolensky, 2004). Causative verbs in MA are characterized by the systematic gemination of their medial consonants. However, it is not easy to determine the morphological nature of the causative morpheme involved in this derivation. Also, it is not clearly known why the causative morpheme gets realized exactly on the second segment of the base form.Therefore, we seek to achieve the following goals. First, we intend to determine the nature of the causative morpheme. Second, we aim to explain why the causative morpheme is realized on the second segment of the base form. In this respect, we show that the causative morpheme is represented by a featureless consonantal mora that targets the second segment of the base root, turning it into a geminate. We also show that the causative morpheme gets infixed thanks to the privileged status of root-initial segments.
文摘The main objective of this study is to give descriptions of formal and functional aspects of causative constructions in Afaan Oromoo.To achieve the objective,written texts,native speaker informants and introspections are predominantly used as sources of data.The findings reveal that the three structural aspects of causatives-morphological,lexical and syntactic-are used in the language.Morphological causatives are highly productive,and affixes with-s and-i in several combinations as well as-eess are employed for such purposes.Causative Morphemes are detected to derive causatives of basic verb stems of different semantic categories and to involve causations in word-class changing.There are also simple and complex causations in which there are several causatives suffixes,causers and micro-events indicated morphologically and syntactically.Semantically,direct,indirect,and assistive/cooperative causatives are identified.Pseudo-causatives are uncovered as peculiar futures of the language too.There are,even,structures with explicit causative affixes which are called subjectless causatives,but they do not show any causal relations between the participants in the structure.
文摘This paper explores the nature of the post-syntactic operations responsible for the representations of the linear order of terminal nodes.In particular,it argues in favor of a unified model of the morphosyntax and morphophonology,wherein the theory of Distributed Morphology and Optimality Theory operate in a single module.The testing ground is an investigation of the formation of morphological causatives in Moroccan Arabic.Herein,the process of realizing causatives is morphological gemination,whereby the second consonant of the root is doubled.Investigating the question of what triggers the infixal process,I argue against the linearization algorithm suggested in Embick&Noyer(2001),Embick&Marantz(2008),and Embick(2006,2010).Instead,the claim I defend here is that the onus of the linearization process falls on the prosody in Arabic,the central assumption being that the morphosyntactic structure,the output of the syntactic derivation,is the input to OT morphophonological constraints.These constraints are responsible for the linearization of the terminal nodes of the syntactic derivation.I show that adopting one theory over others misses important generalizations about the language.
文摘BACKGROUND Glucose imbalance is common in total parenteral nutrition(TPN).Hypoglycemia seems to be less frequent than hyperglycemia,but it influences the clinical outcome to a greater extent.Therefore,it should be effectively prevented and treated.However,there is no relevant report on how to treat hypoglycemia caused by TPN in patients with liver cell injury.CASE SUMMARY We present three patients with liver cell injury who developed severe hypo-glycemia during or after TPN infusion.The causes of severe hypoglycemia and glucose-raising strategies were discussed.According to the physiological charac-teristics of the hepatocellular injury,the ratio of nutrition components prescribed in TPN was appropriately adjusted for the three cases.We simultaneously reduced the dose of insulin and fat emulsion,and increased the dose of glucose in TPN.The blood glucose level was restored to normal range and clinical symptoms were eliminated.CONCLUSION When hypoglycemia occurs during or after TPN in patients with hepatocellular injury,physicians need to simultaneously reduce insulin and fat emulsion,and increase glucose,and correct severe hypoglycemia in time to reduce its adverse consequences.

文摘Climate change is the phrase used to describe long-term changes in temperatures and weather patterns. Changes in the atmosphere and their interactions with diverse geologic, chemical, biological, and geographic variables are the main contributors to this cyclical adjustment of the Earth’s climate. Such changes may be induced purposefully, because of burning fossil fuels, clearing forests, and raising animals, or they may be natural, brought on by significant volcanic eruptions or variations in the sun’s activity. By significantly increasing the amount of greenhouse gases already in the atmosphere, this heightens the greenhouse effect and contributes to global warming. This work includes several additional theoretical and practical explanations of sustainable development. The theoretical work encompasses hundreds of researches that identify requirements for how development routes might satisfy sustainable development (SD) criteria using economic theory, complex systems approach, ecological science, and other techniques. The agreements made by the Parties in various nations across the world will consider a wide range of perspectives about what would be considered undesirable effects on the environment, the climate system, sustainability, economic growth, or food production.

文摘The paper examines the correlation between the degree of agentivity in Russian causatives and their semantic structure.It supports the view that the distinction between agentivity vs.non-agentivity in causatives is an important feature with various ramifications,as demonstrated in Comrie(1976),Talmy(2000).It also motivates the role of the"control"feature in the event structure of the causatives.

文摘Dural defects are common in spinal and cranial neurosurgery.A series of complications,such as cerebrospinal fluid leakage,occur after rupture of the dura.Therefore,treatment strategies are necessary to reduce or avoid complications.This review comprehensively summarizes the common causes,risk factors,clinical complications,and repair methods of dural defects.The latest research progress on dural repair methods and materials is summarized,including direct sutures,grafts,biomaterials,non-biomaterial materials,and composites formed by different materials.The characteristics and efficacy of these dural substitutes are reviewed,and these materials and methods are systematically evaluated.Finally,the best methods for dural repair and the challenges and future prospects of new dural repair materials are discussed.
文摘Non-variceal upper gastrointestinal bleeding(NVUGIB)is a common gastroenterological emergency associated with significant morbidity and mortality.Gastroenterologists and other involved clinicians are generally assisted by international guidelines in its management.However,NVUGIB due to peptic ulcer disease only is mainly addressed by current guidelines,with upper gastrointestinal endoscopy being recommended as the gold standard modality for both diagnosis and treatment.Conversely,the management of rare and extraordinary rare causes of NVUGIB is not covered by current guidelines.Given they are frequently lifethreatening conditions,all the involved clinicians,that is emergency physicians,diagnostic and interventional radiologists,surgeons,in addition obviously to gastroenterologists,should be aware of and familiar with their management.Indeed,they typically require a prompt diagnosis and treatment,engaging a dedicated,patient-tailored,multidisciplinary team approach.The aim of our review was to extensively summarize the current evidence with regard to the management of rare and extraordinary rare causes of NVUGIB.

文摘The etiology of upper gastrointestinal bleeding(UGIB)varies by age,from newborns to adolescents,with some of the causes overlapping between age groups.While particular causes such as vitamin K deficiency and cow’s milk protein allergy are limited to specific age groups,occurring only in neonates and infants,others such as erosive esophagitis and gastritis may be identified at all ages.Furthermore,the incidence of UGIB is variable throughout the world and in different hospital settings.In North America and Europe,most UGIBs are nonvariceal,associated with erosive esophagitis,gastritis,and gastric and duodenal ulcers.In recent years,the most common causes in some Middle Eastern and Far Eastern countries are becoming similar to those in Western countries.However,variceal bleeding still predominates in certain parts of the world,especially in South Asia.The most severe hemorrhage arises from variceal bleeding,peptic ulceration,and disseminated intravascular coagulation.Hematemesis is a credible indicator of a UGI source of bleeding in the majority of patients.Being familiar with the most likely UGIB causes in specific ages and geographic areas is especially important for adequate orientation in clinical settings,the use of proper diagnostic tests, and rapid initiation of the therapy. The fundamental approach to the management of UGIBincludes an immediate assessment of severity, detecting possible causes, and providing hemodynamic stability,followed by early endoscopy. Unusual UGIB causes must always be considered when establishing a diagnosis inthe pediatric population because some of them are unique to children. Endoscopic techniques are of significantdiagnostic value, and combined with medicaments, may be used for the management of acute bleeding. Finally,surgical treatment is reserved for the most severe bleeding.
文摘Prepandemic time trends in mortality from chronic liver disease(CLD)differed according to specific cause of death(decreasing for liver cirrhosis,stable or increasing for liver cancer),etiology(increasing for nonalcoholic fatty liver disease,generally decreasing for other etiologies),and world region(decreasing in areas with the highest burden of hepatitis B virus,increasing in Eastern Europe and other countries).The coronavirus disease 2019(COVID-19)pandemic affected mortality of patients with CLD both directly,with a higher risk for severe illness and death depending on age,stage and etiology of the disease,and indirectly,through social isolation and loss of support,harmful drinking,and difficulties in access to care.Nevertheless,only sparse data are available on variations in CLD as a cause of death during the pandemic.In the USA,in 2020-2021 a growth in mortality was registered for all liver diseases,more marked for alcoholic liver disease,especially among young people aged 25-44 years and in selected ethnic groups.COVID-19 related deaths accounted only for a minor part of the excess.Further data from mortality registers of other countries are warranted,preferably adopting the so-called multiple cause-of-death approach,and extended to deaths attributed to viral hepatitis and liver cancer.
文摘The taxonomic status of Bufo species in East Asia,particularly those closely related to B.gargarizans,has been controversial for over half a century,and recent molecular studies further fueled the dispute.Whether B.andrewsi,B.bankorensis,B.minshanicus,B.sachalinensis,B.tibetanus,and B.tuberculatus constitute valid species or are geographic or ecological variants of B.gargarizans is at the centre of the discussion.Most of these forms possess morphological diagnostic traits,but those differences are minimal and often inconsistent.Molecular data produced many contradictory patterns.Lack of clear diagnostic characters,different usage or definition of various names,mis-identified specimens,limited amount of data or samples,and frequent inter-specific hybridization may all have contributed to the current controversies.Some contradiction may also result from differences between similarity-based clustering and gene tree-based branching patterns.Nevertheless,a dominant allopatric differentiation pattern within this species complex is apparent,and several regional population groups have been identified.Some of the groups resemble old names(e.g.B.minshanicus,B.tibetanus),while others are new.These groups may represent geographic or ecological variants,incipient species,or valid species,and the current data do not warrant a convincing taxonomic revision.To move forward,a single study with comprehensive sampling and genomic data is needed to establish a solid framework,which will define naturally existing population clusters/lineages,and detect their levels of divergence and approximate geographic boundaries.Morphological and ecological examination will follow to understand their biology,determine their taxonomic status,and identify consistent diagnostic characters if necessary.

文摘Emotion cause extraction(ECE)task that aims at extracting potential trigger events of certain emotions has attracted extensive attention recently.However,current work neglects the implicit emotion expressed without any explicit emotional keywords,which appears more frequently in application scenarios.The lack of explicit emotion information makes it extremely hard to extract emotion causes only with the local context.Moreover,an entire event is usually across multiple clauses,while existing work merely extracts cause events at clause level and cannot effectively capture complete cause event information.To address these issues,the events are first redefined at the tuple level and a span-based tuple-level algorithm is proposed to extract events from different clauses.Based on it,a corpus for implicit emotion cause extraction that tries to extract causes of implicit emotions is constructed.The authors propose a knowledge-enriched jointlearning model of implicit emotion recognition and implicit emotion cause extraction tasks(KJ-IECE),which leverages commonsense knowledge from ConceptNet and NRC_VAD to better capture connections between emotion and corresponding cause events.Experiments on both implicit and explicit emotion cause extraction datasets demonstrate the effectiveness of the proposed model.

文摘BACKGROUND Platelet transfusion is of great significance in the treatment of thrombocytopenia caused by myelosuppression during intensive chemotherapy in patients with acute leukemia.In recent years,with platelet transfusion increasing,ineffective platelet transfusion has become increasingly prominent.Generally speaking,platelet antibodies can be produced after repeated transfusion,thus rendering subsequent platelet transfusion ineffective.We report a case of first platelet transfusion refractoriness(PTR)in a patient with acute myelocytic leukemia(AML).Due to the rarity of such cases in clinical practice,there have been no relevant case reports so far.CASE SUMMARY A 51-year-old female patient attended the hospital due to throat pain and abnormal blood cells for 4 d.Her diagnosis was acute myelocytic leukemia[M2 type Fms related receptor tyrosine kinase 3,Isocitrate Dehydrogenase 1,Nucleophosmin 1,Neuroblastoma RAS viral oncogene homolog(+)high-risk group].She was treated with"IA"(IDA 10 mg day 1-3 and Ara-C 0.2 g day 1-5)chemotherapy.When her condition improved,the patient was discharged from the hospital,instructed to take medicine as prescribed by the doctor after discharge,and returned to the hospital for further chemotherapy on time.CONCLUSION We report a rare case of first platelet transfusion failure in a patient with AML during induction chemotherapy,which may be related to the production of platelet antibodies induced by antibiotics and excessive tumor load.This also suggests that we should consider the influence of antibiotics when the rare situation of first platelet transfusion failure occurs in patients with AML.When platelet antibodies are produced,immunoglobulins can be used to block antibodies,thereby reducing platelet destruction.For patients with PTR,both immune and non-immune factors need to be considered and combined in clinical practice along with individualized treatment to effectively solve the problem.

文摘An extreme monsoonal heavy rainfall event lasted for nine days and recurred in the interior of northern south China from June 13 to 21, 2022. Using regional meteorological stations and ERA5 reanalysis data, the causes of this extreme monsoonal rainfall event in south China were analyzed and diagnosed. The results are shown as follows. A dominant South Asian high tended to be stable near the Qinghai-Tibet Plateau, providing favorable upper-level dispersion conditions for the occurrence of heavy rainfall in south China. A western Pacific subtropical high dominated the eastern part of the South China Sea, favoring stronger and more northward transport of water vapor to the northern part of south China at lower latitudes than normal. The continuous heavy precipitation event can be divided into two stages. The first stage(June 13-15) was the frontal heavy rainfall caused by cold air(brought by an East Asian trough)from the mid-latitudes that converged with a monsoonal airflow. The heavy rains occurred mostly in the area near a shear in front of the center of a synoptic-system-related low-level jet(SLLJ), and the jet stream and precipitation were strongest in the daytime. The second stage(June 16-21) was the warm-sector heavy rainfall caused by a South China Sea monsoonal low-level jet penetrating inland. The heavy rainfall occurred on the windward slope of the Nanling Mountains and in the northern part of a boundary layer jet(BLJ). The BLJ experienced five nighttime enhancements, corresponding well with the enhancement of the rainfall center, showing significant nighttime heavy rainfall characteristics. Finally, a conceptual diagram of inland-type warm-sector heavy rainfall in south China is summarized.
