Background:miRNAs are closely related to bone metabolism.Studies have shown that Erxian decoction can improve bone metabolism,possibly achieving this regulatory effect through miRNA targets.Netinfer was used to predic...Background:miRNAs are closely related to bone metabolism.Studies have shown that Erxian decoction can improve bone metabolism,possibly achieving this regulatory effect through miRNA targets.Netinfer was used to predict the miRNA targets of Erxian decoction for the treatment of postmenopausal osteoporosis,and the results were validated by clinical trials.Methods:In this study,we identified possible targets of Erxian decoction in osteoporosis by means of network pharmacological analysis and bioinformatic prediction.Fifteen cases of postmenopausal osteoporosis with kidney Yin and Yang deficiency(In traditional Chinese medicine,kidney Yin nourishes and moistens the tissues of the internal organs of the body,while kidney Yang promotes and warms the tissues of the internal organs of the body.)were treated with Erxian decoction for four weeks,and serum bone metabolism indices(P1NP,osteocalcin,andβ-CTX)and miRNA-335-5p expression were measured before and after treatment.Results:The constructed miRNA postmenopausal osteoporosis related gene network of the effective compound of the Erxian decoction has 296 points and 981 edges.The 39 postmenopausal osteoporosis related genes regulated by miRNA-335-5p were enriched in ossification,while the signaling pathways were enriched in rheumatoid arthritis,the Toll signaling pathway,the HIF-1 signaling pathway,and the MAPK signaling pathway.After taking Erxian decoction,the expression of the serum bone formation index(P1NP,osteocalcin)and miRNA-335-5p gene expression levels increased significantly.The alterations in P1NP and osteocalcin were correlated with the changes in miRNA-335-5p.Conclusion:Circulating miRNA-335-5p may serve as an important target of Erxian decoction in the treatment of postmenopausal women.The effect of Erxian decoction on bone formation is significant,but the underlying mechanism requires further investigation.展开更多

Methods Two hundred and fifty-one patients with soft tissue sarcoma (STS) treated at Shanghai Cancer Hospital during 1986 ...Objective To explore the therapeutic principles and prognostic factors of soft tissue sarcoma. Methods Two hundred and fifty-one patients with soft tissue sarcoma (STS) treated at Shanghai Cancer Hospital during 1986 -1990 were reviewed retrospectively. Results The 1-, 3 - , 5 - , 10 - year tumor-free survival rates were 67. 74% , 57. 16% , 52, 41% , 38. 60% , respectively.The overall survival rates for 1,3,5 and 10 years were 81. 01 % ,67.75% ,60.79% ,and 49.23% respectively. Log-rank test showed that the patients with different pathological findings, histological grades,mass location and size, anatomical depth, and surgical margin showed different outcomes. Whether the sarcomas invaded the vessels or metastasized would influence the survival rates. The patients who underwent different interventions or operations also had different outcomes. The prognosis of STS was associated with age,histological type,histological grade, tumor size, surgical margin and metastasis according to the Cox regression展开更多

Methods A total of 2 086 cases of kidneys for transplantation from Angust 1980 to December ...Objective To summarize the experience of 20 years in using hypertonic citrate adenine(HC-A) for clinical kidney preservation. Methods A total of 2 086 cases of kidneys for transplantation from Angust 1980 to December 2000 were reported. The donors’ age ranged from 18 to 49 years. The cadaveric kidneys were harvested with en-bloc method, and were perfused via renal arteries with 250 ml HC- A at 2 - 4 ℃ and 10 cm H2O, and then stored in HC- A at 2 - 4 ℃ throughout preservation. Serum creatinine was measured at 1 week and 1 month after transplantation. Graft survival rates at 1 year and 3,5,10 years and mean graft survival periods were followed up. According to the cold ischemic time, the cases were divided into 3 groups; Group 1, n = 573, with cold ischemic time less than 12 h; Group 2, n = 779, with 12 to 24 h; and Group 3, n = 734, with more than 24 h. BACKGROUND The pontic design of fixed dental prostheses(FDPs)is strongly associated with the phonetic function,and the phonetic function of anterior FDPs with different pontic designs remains understudied.AIM To inves...BACKGROUND The pontic design of fixed dental prostheses(FDPs)is strongly associated with the phonetic function,and the phonetic function of anterior FDPs with different pontic designs remains understudied.AIM To investigate the immediate and short-term influence of pontic design of anterior FDPs on Chinese speech in a clinical case using objective acoustic analysis.METHODS Two FDPs with two types of pontic design(saddle pontic and modified ridge lap pontic)were fabricated for one patient with maxillary anterior teeth missing.The acoustic analysis of patient’s articulation was conducted immediately after wearing the FDPs and 1 wk after wearing these FDPs.RESULTS The effect of FDP on Chinese vowels(/a/,/o/,/e/,/i/,/u/,and/ü/)was insignificant,because the recovery of vowel distortion occurred within 1 wk for both FDPs.Three(/f/,/s/,and/sh/)of eight Chinese fricative consonants were found to have obvious distortions,and the/s/sound distortion last for more than 1 wk for the patient wearing FDP with modified ridge lap pontic design.CONCLUSION The influence of anterior FDP on articulation of Chinese vowels is insignificant,while the articulation of Chinese fricative consonants is more susceptible.When fabricating anterior FDPs for patients with speech related professions,saddle pontic design can be an alternative option compared with modified ridge lap pontic design.展开更多
Introduction: Myasthenia gravis is a chronic autoimmune neuromuscular disease, presents with weakness and fatigability of striated skeletal muscles. It is a rare disease in Cameroon. We report an uncommon case of myas...Introduction: Myasthenia gravis is a chronic autoimmune neuromuscular disease, presents with weakness and fatigability of striated skeletal muscles. It is a rare disease in Cameroon. We report an uncommon case of myasthenia gravis in a patient with feeding difficulties, notion of oronasal reflux and swallowing disorders as first complaints. Observation: We report the case of a 29-year-old woman consulted at our department of Otolaryngology and Cervico-Facial Surgery for dysphagia and swallowing disorders. She also presented with facial diplegia, oculomotor paralysis, nasal voice, and dysarthria which has been evolving for several years now. The clinical examination revealed multiple cranial nerve palsies. The complementary workup showed a decrement of more than 50% in the electroneuromyography and the presence of anti-acetylcholine receptor autoantibodies in the blood workup. A diagnosis of myasthenia confirms clinical presumption. We initially observed a worsening of neuromuscular disorders despite the pyridostigmine treatment and subsequently a clear improvement of the clinical features concerning swallowing and speech disorders, oculomotricity and facial diplegia under a treatment combining prednisone and azathioprine. Conclusion: Myasthenia gravis is a rare and potentially fatal autoimmune neuromuscular disease. We thus highlight the atypical clinical presentation and therapeutic itinerary of our patient and the importance to think about this clinical diagnosis in front of any multiple cranial nerve paralysis in otolaryngology consultation.展开更多
