Application of Clinical Nursing Pathway in the Peri-Treatment Period of Immunoadsorption Therapy for Rheumatic Immune Diseases

Objective: The paper aims to investigate the clinical nursing pathway (CNP) in the application of immunosorption therapy in patients with rheumatic immune disease. Methods: Convenience sampling method was used to select inpatients who received immunoadsorption therapy from January 2020 to December 2022 in the rheumatology and Immunology department of a 3A hospital in Jingzhou City. 30 patients from January 2020 to June 2021 were selected as control group, and 30 patients from July 2021 to December 2022 were selected as observation group. The control group was given routine nursing. On the basis of the control group, the observation group used a clinical nursing pathway for intervention during the perioperative period of immunosorbent therapy. The incidence of adverse reactions, patient satisfaction, and nurse satisfaction during immunosorbent therapy between the control group and the observation group were compared. Results: After intervention, the incidence of adverse reactions in the observation group was significantly lower than that in the control group, while patient satisfaction and nurse satisfaction in the observation group were significantly higher than those in the control group. The results are all statistically significant (P Conclusion: Clinical nursing pathway is beneficial to reduce the incidence of adverse reactions in patients with immunoadsorption during peri-treatment and improve the satisfaction of patients and nurses. 展开更多

Objective:To explore the safety and effectiveness nursing of 12 h discharged daytime laparoscopic cholecystectomy guided by enhanced recovery after surgery(ERAS)concept.Methods:Included 180 cases from the 12 h discharged daytime laparoscopic cholecystectomy patients which assessmended and guided by ERAS,with the 180 case of routine cholecystectomy patients as control group at the same time.To quantitatively analyze the related indexes of perioperative period and ERAS concept,and evaluate the clinical safety,and effectiveness.Results:It displayed no significant differences in average age,gender distribution,duration of gallbladder related disease and distribution of clinical symptoms between the two groups(P>0.05).There were also no significant differences in preoperative smoking history,hypertension history,cardio-cerebrovascular history,abdominal operation history and disease composition ratio between the two groups(P>0.05).None of the patients had conversion to laparotomy.Compared with the conventional surgery group,there were no significant differences in the average amount of intraoperative blood loss,operation time and muscle strength before returning to the ward in patients with ERAS guided day surgery(P>0.05).When compared with the conventional surgery group,ERAS guided day surgery group had no significant difference in muscle strength between the two groups when going under the ground(t=1.64,P=0.082).Also,the level of peripheral white blood cells in patients with ERAS guided day surgery group was not significantly increased at 6 h after surgery.Meanwhile,liver function related indexes,transaminase,total bilirubin,indirect bilirubin were not significantly abnormal(P>0.05).Conclusion:12 h daytime laparoscopic cholecystectomy guided by ERAS concept can improve the clinical efficacy of patients with cholecystolithiasis,which is safe and feasible. 展开更多

Objective:To explore the application effect of microteaching combined with problem-based learning(PBL)teaching mode in teaching clinical nursing interns in otorhinolaryngology department.Methods:A total of 72 nursing students who interned in our hospital from June 2022 to February 2023 were selected,and all of them were comprehensively trained in basic theoretical knowledge as well as practical skills before the beginning of their learning tasks.The students were randomly divided into the control group and the experimental group,with 36 students in each group.The control group was taught using the traditional clinical nursing teaching mode,and the experimental group was taught using microteaching combined with the PBL teaching mode,subsequently comparing the differences between the two groups of interns in the degree of mastery of theoretical knowledge,hands-on skills,teamwork ability,patient satisfaction,and other aspects.Results:In terms of mastery of theoretical knowledge,the interns in the experimental group(97.22%)were significantly better than that of the control group(75.00%)(P<0.05);the interns in the experimental group had significantly better practical skills(77.78%)than that of the control group(55.56%)(P<0.05);the interns in the experimental group had significantly better teamwork ability than the control group(P<0.05);through the questionnaire survey,it was found that students’satisfaction with teaching in the experimental group(97.22%)was also significantly higher than that in the control group(75.00%)(P<0.05).Conclusion:The application of microteaching combined with PBL teaching mode in the teaching of clinical nursing interns in otorhinolaryngology department achieved significant results.It can not only improve the professional knowledge and application ability of nursing students,but also cultivate their independent thinking,problem-solving skill,as well as teamwork ability.It can also improve the teaching quality and patient satisfaction,and contribute positively to the development of medical education. 展开更多

Objective: To investigate the effect of the nursing path on postoperative complications after endoscopic submucosal dissection(ESD).Method: Patients' nursing paths for ESD were established based on a systematic analysis of relevant literature, previous clinical procedures and consultation with chief physicians from a gastroenterology department. Forty patients were divided into the observation or control group and were assigned to follow the nursing path or the conventional path after an ESD, respectively. Next, the incidence of postoperative complications of these two groups was measured, primarily assessing the occurrences of bleeding, perforation, infection, and anxiety.Results: The incidence of complications after ESD in the observation group was remarkably lower than that of the control group(P < 0.05).Conclusion: The nursing path for ESD has a positive role in reducing certain frequent complications that occur after ESD, particularly infection and anxiety; in addition, the nursing path optimizes nursing care in patients with gastrointestinal stromal tumor. 展开更多

Objective:To evaluate the effect of the clinical nursing pathway applied to functional exercise in patients with hip replacements before and after the operation.Methods:The China National Knowledge Infrastructure(CNKI),WanFang Data,Chinese science and technology journal database(VIP),PubMed,Web of Science,EMBASE,CBM and the Cochrane Library(2015-5) were searched for randomized controlled trials(RCTs) on clinical nursing pathways for functional exercise in patients with hip replacements before and after surgery from June 2015 to January2010.The references included in the literature were also retrieved.To meet the literature standard,2reviewers independently selected and extracted data according to the inclusion criteria and assessed the risks of bias.RevMan 5.3 software was used in this meta-analysis.The quality of evidence was evaluated using grade profiler3.6 software,the level recommended for grading.Results:A total of 15 RCTs and 1248 patients were included.The meta-analysis showed that,in the clinical nursing path group,the Harris score of hip function[SMD = 3.35,95%CI(2.53.4.16),P < 0.00001]and incidence of thrombosis embolism[RR = 0.28,95%CI(0.15,0.53),P < 0.0001],pulmonary infection[RR = 0.33,95%CI(0.14,0.82),P = 0.02],urinary retention[RR = 0.22,95%CI(0.09.0.52),P = 0.0005],constipation[RR = 0.20,95%CI(0.10,0.40),P < 0.00001],patients' satisfaction for nursing care[RR = 1.26,95%CI(1.17,1.36),P< 0.000011 and shortened hospitalization times[SMD =-1.91,95%CI(-2.39.-1.43),P < 0.0001[were statistically significantly better than those in the control group.However,in reducing joint dislocations[RR- 0.25,95%CI(0.05,1.15),P = 0.08],pressure ulcers[RR = 0.25,95%CI(0.03,2.19),P=0.21],and incidence of complications[RR = 0.42,95%CI(0.15.1.12),P = 0.08],there was no statistically significant difference between the two groups.Funnel plot analysis of the average length of stay showed that there might be some publication bias in the literature.The GRADE evaluation results showed that the level of Harris scores for hip function was moderate and the incidence of thrombosis,urinary retention and satisfaction of patients regarding nursing were low,and the rest of the factors analyzed were very low.Conclusions:The effect of the clinical nursing pathway applied to functional exercises in patients with hip replacements before and after surgery was significantly better than that of routine nursing.However,it was restricted by the evaluation grade of the research results and the standardization and uniformity of the research.The results of the above study need to be verified by more high-quality RCTs. 展开更多

Objective: The aims of this study were to understand the status quo of authors, collaborations between institutions, research hotspots,and research frontiers of fund-sponsored clinical nursing papers and to provide a reference for Chinese scholars to conduct clinical nursing studies in the future.Methods: The visualization software CiteSpace was used to analyze fund-sponsored clinical nursing papers published between 2012 and 2016 in 12 core journals of nursing.Results: In the clinical nursing field, there are many cooperative author groups; however, the collaborations between institutions are not close and exist mainly within the same province or city. High-frequency keywords have revealed the four clinical nursing research hotspots of population, diseases, nursing intervention, and others. Chinese medicine nursing, prevention, treatment and nursing, and new technology-aided nursing of ventilator-associated pneumonia are the study frontiers of clinical nursing.Conclusions: Clinical nursing studies are rich in content and cover a wide range of areas, and their hotspots and frontiers are closely related to the requirements of clinical nursing. Collaborations across regions, provinces, and cities are not adequate; there is an urgent need to strengthen the cross-regional exchanges and collaborations to promote the further development of clinical nursing. 展开更多

Objective:To investigate the effect of a clinical nursing pathway and visual health education on patients with perianal abscesses was the objective of this study.Methods:Eighty patients with perianal abscesses undergoing in-patient surgical treatment(blinded for review)between December 2019 and November 2020 were divided randomly and evenly into a control group and an experimental group.The control group received routine nursing care,and the experimental group received visual health education nursing care.Digital pain scores,patient satisfaction scores,nursing quality scores,and scores measuring knowledge of perianal abscess disease for the two groups before and after nursing care were compared and analyzed.Results:The experimental group had visual analog scale(VAS)scores significantly lower than those of the control group(P<0.05).For total nursing satisfaction,the experimental group demonstrated a higher satisfaction rate(P<0.05).Clinical nursing quality scores were significantly higher in the experimental group than those in the control group(P<0.05).The perianal abscess disease-related knowledge scores after intervention were significantly higher in the experimental group than those in the control group(P<0.01).Conclusions:Visual health education nursing can help patients better understand and deal with perianal abscess disease. 展开更多

The patients have been quarantined and treated in special shelter hospitals,and mild illness accounts for the majority of the total number of patients. This article reports one case of the first batches of infected medicalstaff, who got infected with 2019 novel coronavirus and later discharged 2 weeks after receiving treatment and nursing care in Wuhan UnionHospital. Considering all aspects of the patient’s hospitalization, the patient was performed ward environment management, medical nursing,symptomatic treatment, nutrition management, vital signs monitoring, observation and nursing, psychological assessment and support, andinfection protection and management. As the earliest successful case of cure, the patient’s manifestations are classical and common. Moreover,we hope to share it with colleagues to fight against COVID‑19 for reference. The treatment and nursing guidelines can be applied to treat a largenumber of mild cases, thus improving the discharge rate of patients and reducing the pressure of epidemic situation. 展开更多

The aim of this systematic review was to illuminate intuition in clinical nursing. Frequently described as a defining characteristic of professional expertise, intuition is gaining acceptance as a legitimate form of knowledge in clinical nursing. A total of 352 abstracts were read and eight quantitative studies included. A thematic analysis was performed to one main theme, two themes, and four sub-themes emerged. The main theme was: Sensing an unconscious and conscious state of mind, and the two themes were: A sudden emotional awareness and reflection, and arousal of conscious thought processes. The first theme included two sub-themes: Sensing spiritual connections with patients and experiencing physical sensations;worrying and reassuring feelings. The second theme comprised two sub-themes: Willingness to act on personal, interpersonal, and clinical experiences;the influence of maturity and social support in clinical decision-making. An implication for clinical nursing was the need to develop sensitivity as a key to understanding the patient’s illness. In conclusion, leadership and management could facilitate discussions about intuition as a legitimate method of processing information and making decisions about patient care. 展开更多

Investigation on Psychological Health and Resilience of Clinical Nursing Students in “Post-Pandemic Era”

The study aims to learn about the psychological health and counseling situation of nursing students in the post-pandemic era through investigation on the psychological health, resilience and counseling demand of nursing students by randomly selecting clinical nursing students interned in a Grade III Level A hospital in Guangzhou from June 2020 to January 2021 as the objects of study. The internship can be divided into three stages: initial (1st - 3rd month), middle (4th - 7th month) and final (8th - 12th month). The investigator educated the nursing students about psychological health in the initial stage to improve their adaptability to clinical practice. Then they were given questionnaires including Symptom Checklist 90 (SCL-90), Connor-Davidson Resilience Scale (CD-RISC) and general questionnaires. And this study further completed difference analysis on psychological health and resilience in gender, educational background, internship stages and non-only/only children. It turned out that all factors scored above the national norm except anxiety in nine symptom clusters and three dimensions of CD-RISC. As for the scores of four factors of somatization, interpersonal relationship, hostility and bigotry and three dimensions of tenacity, optimism and strength in the CD-RISC, the difference was considered to indicate statistical significance (P < 0.001). Among factors affecting the psychological health of clinical nursing students, score differences involving somatization and compulsion (P < 0.001, P < 0.05) under different educational backgrounds showed statistically significant. With regard to different clinical stages, there were significant differences in somatization, compulsion, tenacity and strength (P < 0.001, P < 0.05). Finally, the conclusion is drawn that, during the period of pandemic prevention and control, the overall psychological status of clinical nursing students was good, but there remain part of cases with mild psychological problems that cannot be ignored. In the post-pandemic era, it is necessary to strengthen the awareness of teaching groups of clinical hospitals to psychological counseling for nursing students. 展开更多

Methods: A mail survey using an anonymous self-administered questionnaire was conducted on clinical nursing competence regarding communication, care and prediction of worsening of symptoms with the authors’ previous research as a framework. The subjects were nurses working in hospice/palliative care units in Japan, and the period of survey was January to February, 2014. Results: In this study, the relationship between the clinical nursing competence and nursing occupational experience of HPNs in hospice/palliative care which was clarified in the authors’ previous research was examined. The results showed that the amount of clinical nursing experience as a nurse was related to clinical nursing competence in the care of end-of-life patients and worsening of symptoms. The abundance of specialized nursing experience was also related to clinical nursing competence in communication in addition to care and prediction of worsening of symptoms. Training experience was also shown to be effective for hospice/palliative care. Conclusion: From these results, clinical nursing competence of HPNs in care of end-of-life patients and prediction of worsening of symptoms are possibly learned through the accumulation of clinical nursing experience as a nurse. However, HPN’s clinical nursing competence in communication was suggested to be developed through nursing practice in hospice/palliative care. 展开更多

Objective: This study was conducted to understand the trends and research hotspots of clinical nursing in China and abroad, thereby to provide guidance for nursing practice and studies.Methods: We retrieved clinical nursing literature(excluding literature related to psychological nursing and traditional Chinese medicine nursing) cited in Wanfang Data(www.wanfangdata.com.cn) and PubMed between 2007 and 2015 and subsequently performed bibliometric analyses on article volume,journal, clustering of high-frequency keywords, and co-occurrence of keywords.Results: A total of 10374 publications, excluding those related to psychological nursing, were retrieved via a PubMed search. The literature displayed a rising trend in terms of annual publication volume. The journal distribution of clinical nursing publications overall was consistent with Bradford's Law. The main contents of clinical nursing literature between 2007 and 2009 included:(1) studies on effects of the midwife and midwifery methods for natural childbirth;(2) studies on emergency nursing care;(3)studies on dementia care;(4) studies on bandage dressing in trauma nursing; and(5) studies on quality of life in cancer patients nursing. The main contents of clinical nursing literature between 2010 and 2012 included:(1) studies on obstetric nurse-patient relationships;(2) studies on patients' medical team in stroke nursing;(3) studies on cancer nursing methods;(4) studies on nursing patients with trauma related to urinary incontinence; and(5) studies on the quality of life of caregivers of dementia patients.The main contents of clinical nursing literature between 2013 and 2015 included(1) studies on skin care and bandaging in trauma nursing;(2) studies on evidence-based nursing in AIDS care;(3) studies on nurses' role in obstetric nurse-patient relationships; studies on cancer nursing methods;(4) studies on nursing for Alzheimer's disease and dementia; and(5) studies on caregivers and nursing of stroke patients. A Wanfang Data search retrieved a total of 85570 publications, whose volume also exhibited an annual rising trend, in particular with regard to those in core journals. However, percentage of total publication volume, mean cited frequency per article, and total ranking of clinical nursing publications overall displayed a decreasing trend. The main contents of clinical nursing literature between 2007 and 2009 included:(1) diabetes nursing;(2) nursing for cancer;(3) nursing for senility-related diseases;(4)nursing in the event of complications;(5) nursing for hypertension;(6) studies on quality of life after nursing interventions;(7) chemotherapy nursing;(8) studies on compliance after nursing interventions;(9) constipation nursing; and(10) nursing for stroke patients. The main contents of clinical nursing literature between 2010 and 2012 included:(1) diabetes nursing;(2) nursing for cancer;(3) nursing for senility-related diseases;(4) studies on quality of life after nursing interventions;(5) nursing in the event of complications;(6) nursing for hypertension;(7) prevention nursing;(8) perioperative nursing;(9)pain nursing; and(10) studies on compliance after nursing interventions. The main contents of clinical nursing literature between 2013 and 2015 included:(1) diabetes nursing;(2) nursing in the event of complications;(3) cancer nursing;(4) studies on quality of life after nursing interventions;(5) nursing for stroke patients;(6) nursing for the elderly;(7) studies on compliance after nursing interventions;(8)nursing for hypertension;(9) constipation nursing; and(10) prevention nursing.Conclusions: Clinical nursing research in China is in a rapid development stage as defined by Price's law whereby there is a rapid growth in publication volume. By comparison, clinical nursing research in developed countries is in the stable development stage or maturation phase, which is characterized by a flat curve of publication volume and stable development rate. There is a gap of 25-30 years between China and these countries in terms of the growth phases of clinical nursing literature. 展开更多

Botulinum toxin A (BTX-A) has been proposed as alternatives to traditional treatments. Objective: To summarize the clinical experience of nursing care for the treatment of the Botulinum toxin A (BTX-A) injections due to neurological detrusor overactivity (NDO). Methods: We reviewed the data of 32 inpatients who received BTX-A injections between August 2014 to July 2016. Their mean age was 27.4 years old (range: 19 - 38). They were all on clean intermittent catheterization. All patients presented detrusor overactivity resistant to oral anticholinergic treatment. Results: There were significant difference between outcomes at baseline and week12 for Pdetmax (60.08 cmH2O vs. 28.04 cmH2O, P Conclusions: Patients were satisfied with the treatment and nursing by targeted perioperative nursing. 展开更多

Development and Practice of Integrated Nursing Model NNN Link for Breastfeeding Clinical Nursing Model as a Whole

Objective: To establish breastfeeding clinical holistic nursing model used in clinical nursing and teaching, it is possible to improve nurses and nursing students to master knowledge of breastfeeding, analysis, judgment, decision-making and the ability to care-related issues, promoting pure improve breastfeeding rates. Method: 1) The North American Nursing Diagnosis Nursing Association (NANDA), Nursing Outcomes Classification (NOC) and Nursing Classification (NIC) link (NNN link) clinical clues reasoning to determine the contents of the questionnaire were used. Then, the Delphi method to care was used. 2) The survey questionnaire was designed. The content included the clinical holistic nursing care model in breastfeeding and the model’s use situation. 3) Questionnaire survey: in the national midwifery care industries, volunteers were collected who were willing to use the breastfeeding clinical holistic nursing care model in the nursing work. We issued 98 questionnaires and took back 76 valid questionnaires. Results: The construction of the breastfeeding clinical holistic nursing model includes 9 nursing diagnoses which contain the breastfeeding effective, the lack of knowledge, the decision conflicts, the risk of breast tenderness, the risk of cracked nipple, the invalid hazard occurring in breastfeeding, the risk of caregiver role strain, the risk of breast feeding jaundice and the ineffective community coping, 18 nursing outcomes and 64 nursing measures. In the composition of breastfeeding after clinical holistic nursing model in clinical practice, 100% of obstetric nurses think that the breastfeeding nursing model can promote the nursing staff to care patients according to the nursing process, can help nurses to analyze, evaluate, make decision, and care about breastfeeding related problems, as well as can promote the mother’s milk feeding rate. Conclusion: The establishment of the breastfeeding nursing model provides a learning material for obstetric breast-feeding. Moreover, nursing diagnosis, nursing outcomes and nursing measures correspond according to the form of chart, which are easy to use and find. The breastfeeding clinical holistic nursing model is practicing in clinical nursing, which can help nursing staff to improve the ability of nursing personnel according to evidence-based nursing patients, to improve the ability of nursing personnel analysis, evaluation, decision-making and nursing in breastfeeding problems and to promote the rate of breastfeeding. 展开更多

Objective:To ensure that only competent graduates are licensed to practice nursing,councils conduct licensing examinations,which may include among others clinical competency assessment.This review explored current practices in clinical competency assess­ment of nursing students as part of a larger study aimed at developing an evidence-based,context-specific framework for clinical competency assessment in a sub-Saharan African(SSA)country.Methods:A scoping guided by the Preferred Reporting Items for Systematic reviews and Meta-Analyses extension for Scoping Reviews(PRISMA-ScR)was conducted.Results:Findings from 28 out of 1151 studies identified from Scopus,PubMed,CINAHL,Wiley Online Library,and ProQuest were included and synthesized.Results show that a good assessment system must be valid,reliable,transparent,feasible,fair,objective,and must provide feedback and continually improve to have an educational impact.Clinical competency assessment systems must be developed on sound empirical evidence,pilot tested,and involve thorough training and evaluation of the examiners.Continuous evaluation of the assessment system is also essential to ensure the quality and relevance of the assessment system.Only one of the included studies was conducted in Africa.Conclusions:The paucity of clinical competency assessment research in sub-Saharan Africa may lead to benchmarking assessment systems on research conducted outside the context.Sub-Saharan Africa has a set of circumstances that demand a context-specific clinical competency assessment framework to guide clinical competency assessment. 展开更多
