However, the optimal architectural construction methods for adequate thermal comfort for occupants in hot and arid environments are not mastered. This article evaluates the influence of architectural and constructive modes of buildings made of CEB walls and concrete block walls, to optimize and compare their thermal comfort in the hot and dry tropical climate of Ouagadougou, Burkina Faso. Two identical pilot buildings whose envelopes are made of CEB and concrete blocks were monitored for this study. The thermal models of the pilot buildings were implemented in the SketchUp software using an extension of EnergyPlus. The models were empirically validated after calibration against measured thermal data from the buildings. The models were used to do a parametric analysis for optimization of the thermal performances by simulating plaster coatings on the exterior of walls, airtight openings and natural ventilation depending on external weather conditions. The results show that the CEB building displays 7016 hours of discomfort, equivalent to 80.1% of the time, and the concrete building displays 6948 hours of discomfort, equivalent to 79.3% of the time. The optimization by modifications reduced the discomfort to 2918 and 3125 hours respectively;i.e. equivalent to only 33.3% for the CEB building and 35.7% for the concrete building. More study should evaluate thermal optimizations in buildings in real time of usage such as residential buildings commonly used by the local middle class. The use of CEB as a construction material and passive means of improving thermal comfort is a suitable ecological and economical option to replace cementitious material.展开更多
In order to better meet the objective requirements of the use safety of the high-rise glulam building floor structure and the living comfort of the residents,the transient excitation,environmental excitation and frequ...In order to better meet the objective requirements of the use safety of the high-rise glulam building floor structure and the living comfort of the residents,the transient excitation,environmental excitation and frequency spectrum identification methods were used to carry out experimental modal test in-site on the three rooms numbered A,B and C of the same glulam structural building.The three rooms have different functions,different floor sizes and different floor supporting structures.The research results have shown that the first-order bending frequency of the floor structure of Room A is 27.50 Hz,the transverse second-order bending frequency is 34.75 Hz,the longitudinal second-order bending frequency is 53.25 Hz,and the first-order torsional frequency is 56.25 Hz.The reinforced wooden beam at the bottom of the floor of Room A increases the transverse stiffness of the floor structure,but does not offset the anisotropy caused by the longitudinally installed glulam floors.The fundamental frequency values of the floor structures of the three rooms numbered A,B,and C are 27.5,13 and 18 Hz,respectively.This has a relatively high innovation and reference significance for integrating the theory of structural dynamic characteristics with the dynamic testing technology,improving the design level of high-rise glulam structure buildings,and improving the living comfort of residents.展开更多
BACKGROUND Running is a hugely popular sport.Unfortunately,running-related injury(RRI)rates are high,particularly amongst amateur and recreational runners.Finding ways to reduce RRI rates and maximise comfort and perf...BACKGROUND Running is a hugely popular sport.Unfortunately,running-related injury(RRI)rates are high,particularly amongst amateur and recreational runners.Finding ways to reduce RRI rates and maximise comfort and performance for runners is important.Evidence regarding whether orthotics can successfully improve these parameters is limited and contradicting.Further research is required to provide runners with clearer guidance on the usefulness of orthotics.AIM To investigate the effect of Aetrex Orthotics on comfort,speed and RRI rates during recreational running.METHODS One hundred and six recreational runners were recruited on a voluntary basis via running clubs and social media pages and randomised into either the intervention or control group.Participants in the intervention group ran with Aetrex L700 Speed Orthotics inserted in their usual running shoes,whilst participants in the control group ran in their usual running shoes with no orthotics.The study ran for an 8-wk period.Participants provided data relating to running comfort,distance,and time during weeks 3-6.Participants provided data relating to any RRIs they sustained during all 8 wks.Running distance and time were used to calculate running speed in miles per hour(mph).For each outcome variable,95%confidence intervals and P values were calculated to assess the statistical significance between the groups.For comfort and speed data,univariate multi-level analysis was performed,and for outcome variables with significant between group differences,multi-level multivariate analysis was performed to evaluate any confounding effects of gender and age.RESULTS Ninety-four participants were included in the final analysis(drop-out rate=11%).Comfort and speed from 940 runs and 978 injury data reports were analysed.Participants who ran with orthotics reported,on average,speeds 0.30 mph faster(P=0.20)and comfort scores 1.27 points higher(P≤0.001)than participants who ran with no orthotics.They were also 2.22 times less likely to sustain an injury(P=0.08)than participants who ran with no orthotics.However,findings were only significant for comfort and not for speed or injury rates.Age and gender were found to be significant predictors of comfort.However,the improvements in comfort reported by participants who ran with orthotics were still significant after adjusting for age and gender.CONCLUSION This study found orthotics to improve comfort and speed and prevent RRIs whilst running.However,these findings were only statistically significant for comfort.展开更多
BACKGROUND Patients with colorectal cancer may need postoperative nursing to improve prognosis,and conventional nursing is not effective.Clinical research is needed to explore nursing methods that can more effectively...BACKGROUND Patients with colorectal cancer may need postoperative nursing to improve prognosis,and conventional nursing is not effective.Clinical research is needed to explore nursing methods that can more effectively improve postoperative conditions on colorectal cancer patients undergoing colostomy.AIM To explore the effect of internet multiple linkage mode-based extended care combined with in-hospital comfort care on colorectal cancer patients undergoing colostomy.METHODS Data from 187 patients with colostomy treated in our hospital from May 2019 to March 2022 were collected and divided into three groups,A(n=62),B(n=62)and C(n=63),according to different intervention methods.Group A received internet multiple linkage mode-based extended care combined with in-hospital comfort care.Group B received internet multiple linkage mode-based extended care.Group C received usual care intervention.Complications were compared among the three groups.The stoma self-efficacy scale,Hamilton Anxiety Scale,RESULTS The complication rate of group A,B and C(16.13%,20.97%and 60.32%,respectively)was significantly different(all P<0.05).The incidence of complications in groups A and B was lower than that in group C,and there was no significant difference between groups A and B(P>0.05).After intervention,the scores of ostomy care,social contact,diet choice,confidence in maintaining vitality,confidence in self-care of ostomy,confidence in sexual life,confidence in sexual satisfaction and confidence in physical labor in the three groups were all higher than before intervention,and the scores of groups A and B were higher than those of group C,with statistical significance(P<0.05).The Hamilton Anxiety Scale and Hamilton Depression Scale scores of the three groups after intervention were lower than those before intervention.The scores of groups A and B were lower than those of group C,and the score of group A was lower than that of group B,all with statistical significance(all P<0.05).There was a statist-ically significant difference in cancer-induced fatigue among the three groups(P<0.05).After intervention,the scores of physical health,psychological health,social health and mental health of the three groups were lower than before the intervention.The scores of group A and B were lower than that of group C;and the score of group A was lower than that of group B,all with statistical significance(all P<0.05).CONCLUSION Internet multiple linkage mode-based extended care combined with in-hospital comfort care can effectively improve self-efficacy,bad mood,cancer-related fatigue and life quality of colorectal cancer patients undergoing colostomy.展开更多

Methods: The procedures guided by the Joanna Briggs Institute will be applied. The searches will be carried out in the Medical Literature Analysis and Retrieval System Online, Excerpta Medica Database, Cumulative Index to Nursing and Allied Health Literature, Cochrane Library, Scopus, Web of Science, Latin American and Caribbean Health Sciences Literature, Nursing Database, Scientific Electronic Library databases. Studies which are available in full and published in English, Spanish or Portuguese will be selected. There will be no restrictions to the study design or time frame. Two reviewers will independently screen all citations with the aid of software. The degree of agreement between the researchers will be verified by statistics that measure reliability. Through narrative descriptions, charts, and tables, we will present the results obtained. Data analysis will involve descriptive statistics, and qualitative evaluation. We will use the Preferred Reporting Items for Systematic Reviews and Meta-Analyses extension for Scoping Reviews checklist to review and write this review. Out of the three approaches to assessing thermal comfort namely, rational, empirical and direct, the direct approach is being used in the present study because of easy availability of all inputs and reasonable comprehension of the assessments. Assessment and ranking of cities using this approach based on the percentage of comfortable hours alone may however be erroneous and misleading as this approach does not consider the percentages of uncomfortable classes which could often be substantially high. The modified approach for thermal comfort classification demonstrates cumulative representation of all classes of thermal comfort including uncomfortablity and provides relative ranking of cities. Analysis of the results is presented here for five megacities (Delhi, Mumbai, Chennai, Kolkata and Hyderabad) representing varying geographical and climatic locations of India. These cities are ranked based on the routine and modified approaches and results are discussed in detail on monthly, seasonal and annual average basis. When the cities are compared only on the basis of comfortable hours, the decreasing order of comfortability is Hyderabad, Kolkata, Delhi, Chennai and Mumbai. However, considering the second methodology, it is revealed that the contribution of uncomfortable hours is greater in Kolkata and Chennai in comparison to Mumbai. The proposed methodology could be an improvement over the current practices and provides a more rational method for relative ranking of cities that could be used for tourism and energy demands.展开更多

This study first carried out a questionnaire survey on female consumers to know about their preference and demand on pantyhose. Then thermal manikin test and subject wearing trials were performed to evaluate the commonly used pantyhose. It was found that female consumers felt cold on the legs when wearing pantyhose which puts forward more requirements on pantyhose for its ability to keep warm. The thinner and more permeable the pantyhose,the thermal insulation was smaller. And the thicker the pantyhose,the pressure it caused to the body was higher. Sample 1# brought the highest prickle sensation, and prickle sensation was increased with the movement of the body. Samples 2# and 3# caused the highest and lowest thermal sensation,which was in line with their thermal insulation.展开更多

The aim of this study was to compare the tec...Increased interest of clinicians for using 3.0 T magnetic resonance imaging (MRI), as imaging modality of choice for their patients, has been evident in the past few years. The aim of this study was to compare the technical quality of the obtained tomography using 3.0 T and 1.5 T MRI, and to compare the subjective feeling of discomfort of patients and subjective acoustic noise experience during imaging using MRI at 3.0 T and 1.5 T. Brain MRI (1.5 and 3.0 T) was performed in 58 patients, according to a standardized protocol. All studies have been randomly described by independent two radiologists. The reference standard for the existence of technical artifact is established on the basis of both radiologists’ consensus. We also compared the subjective feelings of the discomfort and acoustic noise during the both MRI (1.5 T and 3.0 T) exams. Artifacts were significantly more common during 3.0 T MRI in comparison with the 1.5 T MRI (χ2 = 5.286, P < 0.05), as well as in male patients (χ2 = 8.841, P < 0.05), and sub-jective assessments of discomfort and acoustic noise were higher in patients who underwent imaging using 3.0 T MRI, (χ2 = 125.959, df = 1, P < 0.001) and in females (χ2 = 195.449, df = 1, P < 0.001). Additional research is needed to prove that appropriate information for patients about the discomfort during 3.0 T MRI and their psychological preparation is very important element for optimal use of 3.0 T MRI in daily clinical practice.展开更多

A hybrid approach which integrates all kinds of investigation methods in real environment and virtual environment is described. The real experimental environment includes the WBV(whole body vibration) test, questionnaires for human subjective sensation and motion capture. The virtual experimental environment includes the theoretical calculation on simplified 5-DOF human body vibration model, the vibration simulation and analysis within ADAMS/VibrationTMmodule, and the digital human biomechanics and occupational health analysis in Jack software. While the real experimental environment provides realistic and accurate test results, it also serves as core and validation for the virtual experimental environment. The virtual experimental environment takes full advantages of current available vibration simulation and digital human modelling software, and makes it possible to evaluate the sitting posture comfort in a human-vehicle system with various human anthropometric parameters. How this digital evaluation system for car seat comfort design is fitted in the Industry 4.0 framework is also proposed.展开更多

METHODS: All colonoscopies performed in our four endoscopy centres are recorded in two report...AIM: To explore the relationship of patient comfort and experience to commonly used performance indicators for colonoscopy. METHODS: All colonoscopies performed in our four endoscopy centres are recorded in two reporting systems that log key performance indicators. From 2008 to 2011, all procedures performed by qualified endoscopists were evaluated; procedures performed by trainees were excluded. The following variables were measured: Caecal intubation rate (CIR), nurse-reported comfort levels (NRCL) on a scale from 1 to 5, polyp detection rate (PDR), patient experience of the procedure (worse than expected, as expected, better than expected), and use of sedation and analgesia. Pearson' s correlation coefficient was used to identify relationships between performance indicators.RESULTS: A total of 17027 colonoscopies were performed by 23 independent endoscopists between 2008 and 2011. Caecal intubation rate varied from 79.0% to 97.8%, with 18 out of 23 endoscopists achieving a CIR of > 90%. The percentage of patients experiencing significant discomfort during their procedure (defined as NRCL of 4 or 5) ranged from 3.9% to 19.2% with an average of 7.7%. CIR was negatively correlated with NRCL-45 (r=-0.61, P < 0.005), and with poor patient experience (r=-0.54, P < 0.01). The average dose of midazolam (mean 1.9 mg, with a range of 1.1 to 3.5 mg) given by the endoscopist was negatively correlated with CIR (r=-0.59, P < 0.01). CIR was positively correlated with PDR (r=0.44, P < 0.05), and with the numbers of procedures performed by the endoscopists (r=0.64, P < 0.01). CONCLUSION: The best colonoscopists have a higher CIR, use less sedation, cause less discomfort and find more polyps. Measuring patient comfort is valuable in monitoring performance.展开更多

文摘BACKGROUND Patients with colorectal cancer may need postoperative nursing to improve prognosis,and conventional nursing is not effective.Clinical research is needed to explore nursing methods that can more effectively improve postoperative conditions on colorectal cancer patients undergoing colostomy.AIM To explore the effect of internet multiple linkage mode-based extended care combined with in-hospital comfort care on colorectal cancer patients undergoing colostomy.METHODS Data from 187 patients with colostomy treated in our hospital from May 2019 to March 2022 were collected and divided into three groups,A(n=62),B(n=62)and C(n=63),according to different intervention methods.Group A received internet multiple linkage mode-based extended care combined with in-hospital comfort care.Group B received internet multiple linkage mode-based extended care.Group C received usual care intervention.Complications were compared among the three groups.The stoma self-efficacy scale,Hamilton Anxiety Scale,RESULTS The complication rate of group A,B and C(16.13%,20.97%and 60.32%,respectively)was significantly different(all P<0.05).The incidence of complications in groups A and B was lower than that in group C,and there was no significant difference between groups A and B(P>0.05).After intervention,the scores of ostomy care,social contact,diet choice,confidence in maintaining vitality,confidence in self-care of ostomy,confidence in sexual life,confidence in sexual satisfaction and confidence in physical labor in the three groups were all higher than before intervention,and the scores of groups A and B were higher than those of group C,with statistical significance(P<0.05).The Hamilton Anxiety Scale and Hamilton Depression Scale scores of the three groups after intervention were lower than those before intervention.The scores of groups A and B were lower than those of group C,and the score of group A was lower than that of group B,all with statistical significance(all P<0.05).There was a statist-ically significant difference in cancer-induced fatigue among the three groups(P<0.05).After intervention,the scores of physical health,psychological health,social health and mental health of the three groups were lower than before the intervention.The scores of group A and B were lower than that of group C;and the score of group A was lower than that of group B,all with statistical significance(all P<0.05).CONCLUSION Internet multiple linkage mode-based extended care combined with in-hospital comfort care can effectively improve self-efficacy,bad mood,cancer-related fatigue and life quality of colorectal cancer patients undergoing colostomy.

文摘With the continuous intensification of the global greenhouse effect,thermal comfort has become a highly concerned issue in the living environment.The study explores the influencing factors and improvement strategies of urban street thermal comfort.The urban heat island effect,environmental parameters,building layout,and green planting all have a significant impact on the thermal comfort of streets.Improvement strategies include optimizing cultivating pattern of plants,adjusting street layout,and improving paving materials of road.The multi-layer vegetation structure provides green shade,reduces local temperature,and humidifies the surrounding environment.It should adjust the street orientation and aspect ratio to provide sunlight and ventilation,and reduce ambient temperature.Paving materials with low reflectivity and light color and permeable underlying surface should be chosen to reduce heat absorption,increase shading and greenery,and improve thermal comfort.
文摘Purpose: Carrying out a scoping review to fill gaps in current knowledge regarding comfort needs in the care provided by nurses to girls/women who experience puerperium. Methods: The procedures guided by the Joanna Briggs Institute will be applied. The searches will be carried out in the Medical Literature Analysis and Retrieval System Online, Excerpta Medica Database, Cumulative Index to Nursing and Allied Health Literature, Cochrane Library, Scopus, Web of Science, Latin American and Caribbean Health Sciences Literature, Nursing Database, Scientific Electronic Library databases. Studies which are available in full and published in English, Spanish or Portuguese will be selected. There will be no restrictions to the study design or time frame. Two reviewers will independently screen all citations with the aid of software. The degree of agreement between the researchers will be verified by statistics that measure reliability. Through narrative descriptions, charts, and tables, we will present the results obtained. Data analysis will involve descriptive statistics, and qualitative evaluation. We will use the Preferred Reporting Items for Systematic Reviews and Meta-Analyses extension for Scoping Reviews checklist to review and write this review. Conclusion: We will summarize the main information available in the literature on the subject, aiming to obtain an overview of the practices employed, and the gaps present in knowledge that require greater attention from the scientific community.

文摘Objective:To explore and analyze the effect of comfort nursing in patients with chronic obstructive pulmonary disease(COPD)complicated with respiratory failure.Methods:60 patients with COPD and respiratory failure who were admitted to the Department of Respiratory Medicine of our hospital from May 2020 to May 2023 were selected as subjects of this study,and they were divided into comfort group and reference group by odd and even number draw method,with 30 cases in each group.The comfort group received comfort nursing,and the reference group received general nursing.The lung function performance and living conditions were compared between the groups.Results:Before the intervention,there was no statistically significant difference(P>0.05)in terms of lung functions such as forced expiratory volume in the first second(FEV1),forced vital capacity(FVC),and the ratio FEV1/FVC between the groups;after the intervention,the lung functions of the comfort group were significantly better than those in the reference group(P<0.05).Before the intervention,there was no statistically significant difference(P>0.05)between the groups in terms of mental state,physical function,social situation,and spiritual aspects;after the intervention,the mental state,physical function,social situation,and spiritual aspects of the comfort group were significantly better than those of the reference group(P<0.05).Conclusion:Comfort nursing care for COPD patients with respiratory failure can improve their lung function and quality of life,and achieve ideal nursing effects.
文摘Objective:To explore and analyze the clinical effect of comfort nursing on children with acute otitis media.Methods:62 children with acute otitis media caused by upper respiratory tract infection who visited the outpatient clinic of our hospital from June 2022 to June 2023 were selected for this study.They were divided into a study group(n=31)and a control group(n=31).Children in the control group received basic care,while children in the study group received comfort nursing along with basic care.The hearing thresholds,body temperature,treatment compliance,and nursing satisfaction of the two groups of children were compared.Results:After the nursing intervention,the hearing threshold and average body temperature of the children in the study group were lower than those in the control group(P<0.05);the treatment compliance of the children in the study group was higher than that in the control group(P<0.05);the nursing satisfaction of the study group was higher than that of the control group(P<0.05).Conclusion:Comfort nursing can improve the treatment effect,treatment compliance,and nursing satisfaction of children with children with acute otitis media caused by upper respiratory tract infection.

文摘This paper focuses on climate comfort degree evaluated from relationships between spatial characteristics and behaviors of a winter city.It is demonstrated that the influences of climatic factors on human comfort are remarkably different for different spatial characteristics and functions of the sites.An evaluation method of climatic comfort is propsed,in which attaining comfort is an adaptation process,and the dynamic changes in the process are according to the user’s subjective tendency to the variations of the spatial characteristics and functions of the space.The evaluation criteria are based on both physical and psychological hierarchy needs of a person,which include the spatial form for microclimatic conditions as well as the spatial social-economic characteristics of a site.To improve the local environment of a winter city by planning measurements,the Harbin Central Pedestrian Street is taken as an example in the application of the evaluation method,and different planning optimizing strategies are proposed accordingly.The results show that the pattern of climate comfort is characterized by reflecting the socialeconomic value and individual behavior characteristics.
文摘A thermal stress index of a geographic location over a period of time can provide knowledge of overall climate perceptible to the general public. Out of the three approaches to assessing thermal comfort namely, rational, empirical and direct, the direct approach is being used in the present study because of easy availability of all inputs and reasonable comprehension of the assessments. Assessment and ranking of cities using this approach based on the percentage of comfortable hours alone may however be erroneous and misleading as this approach does not consider the percentages of uncomfortable classes which could often be substantially high. The modified approach for thermal comfort classification demonstrates cumulative representation of all classes of thermal comfort including uncomfortablity and provides relative ranking of cities. Analysis of the results is presented here for five megacities (Delhi, Mumbai, Chennai, Kolkata and Hyderabad) representing varying geographical and climatic locations of India. These cities are ranked based on the routine and modified approaches and results are discussed in detail on monthly, seasonal and annual average basis. When the cities are compared only on the basis of comfortable hours, the decreasing order of comfortability is Hyderabad, Kolkata, Delhi, Chennai and Mumbai. However, considering the second methodology, it is revealed that the contribution of uncomfortable hours is greater in Kolkata and Chennai in comparison to Mumbai. The proposed methodology could be an improvement over the current practices and provides a more rational method for relative ranking of cities that could be used for tourism and energy demands.

文摘The wearing comfort of clothing has been frequently investigated,but there are few researches on the wearing comfort of pantyhose. This study first carried out a questionnaire survey on female consumers to know about their preference and demand on pantyhose. Then thermal manikin test and subject wearing trials were performed to evaluate the commonly used pantyhose. It was found that female consumers felt cold on the legs when wearing pantyhose which puts forward more requirements on pantyhose for its ability to keep warm. The thinner and more permeable the pantyhose,the thermal insulation was smaller. And the thicker the pantyhose,the pressure it caused to the body was higher. Sample 1# brought the highest prickle sensation, and prickle sensation was increased with the movement of the body. Samples 2# and 3# caused the highest and lowest thermal sensation,which was in line with their thermal insulation.
文摘Increased interest of clinicians for using 3.0 T magnetic resonance imaging (MRI), as imaging modality of choice for their patients, has been evident in the past few years. The aim of this study was to compare the technical quality of the obtained tomography using 3.0 T and 1.5 T MRI, and to compare the subjective feeling of discomfort of patients and subjective acoustic noise experience during imaging using MRI at 3.0 T and 1.5 T. Brain MRI (1.5 and 3.0 T) was performed in 58 patients, according to a standardized protocol. All studies have been randomly described by independent two radiologists. The reference standard for the existence of technical artifact is established on the basis of both radiologists’ consensus. We also compared the subjective feelings of the discomfort and acoustic noise during the both MRI (1.5 T and 3.0 T) exams. Artifacts were significantly more common during 3.0 T MRI in comparison with the 1.5 T MRI (χ2 = 5.286, P < 0.05), as well as in male patients (χ2 = 8.841, P < 0.05), and sub-jective assessments of discomfort and acoustic noise were higher in patients who underwent imaging using 3.0 T MRI, (χ2 = 125.959, df = 1, P < 0.001) and in females (χ2 = 195.449, df = 1, P < 0.001). Additional research is needed to prove that appropriate information for patients about the discomfort during 3.0 T MRI and their psychological preparation is very important element for optimal use of 3.0 T MRI in daily clinical practice.

文摘Most of the previous studies on the vibration ride comfort of the human-vehicle system were focused only on one or two aspects of the investigation. A hybrid approach which integrates all kinds of investigation methods in real environment and virtual environment is described. The real experimental environment includes the WBV(whole body vibration) test, questionnaires for human subjective sensation and motion capture. The virtual experimental environment includes the theoretical calculation on simplified 5-DOF human body vibration model, the vibration simulation and analysis within ADAMS/VibrationTMmodule, and the digital human biomechanics and occupational health analysis in Jack software. While the real experimental environment provides realistic and accurate test results, it also serves as core and validation for the virtual experimental environment. The virtual experimental environment takes full advantages of current available vibration simulation and digital human modelling software, and makes it possible to evaluate the sitting posture comfort in a human-vehicle system with various human anthropometric parameters. How this digital evaluation system for car seat comfort design is fitted in the Industry 4.0 framework is also proposed.
文摘AIM: To explore the relationship of patient comfort and experience to commonly used performance indicators for colonoscopy. METHODS: All colonoscopies performed in our four endoscopy centres are recorded in two reporting systems that log key performance indicators. From 2008 to 2011, all procedures performed by qualified endoscopists were evaluated; procedures performed by trainees were excluded. The following variables were measured: Caecal intubation rate (CIR), nurse-reported comfort levels (NRCL) on a scale from 1 to 5, polyp detection rate (PDR), patient experience of the procedure (worse than expected, as expected, better than expected), and use of sedation and analgesia. Pearson' s correlation coefficient was used to identify relationships between performance indicators.RESULTS: A total of 17027 colonoscopies were performed by 23 independent endoscopists between 2008 and 2011. Caecal intubation rate varied from 79.0% to 97.8%, with 18 out of 23 endoscopists achieving a CIR of > 90%. The percentage of patients experiencing significant discomfort during their procedure (defined as NRCL of 4 or 5) ranged from 3.9% to 19.2% with an average of 7.7%. CIR was negatively correlated with NRCL-45 (r=-0.61, P < 0.005), and with poor patient experience (r=-0.54, P < 0.01). The average dose of midazolam (mean 1.9 mg, with a range of 1.1 to 3.5 mg) given by the endoscopist was negatively correlated with CIR (r=-0.59, P < 0.01). CIR was positively correlated with PDR (r=0.44, P < 0.05), and with the numbers of procedures performed by the endoscopists (r=0.64, P < 0.01). CONCLUSION: The best colonoscopists have a higher CIR, use less sedation, cause less discomfort and find more polyps. Measuring patient comfort is valuable in monitoring performance.
