Methods A total of 119 infertile patients undergoing the first IVF/ICSI-ET cycle were recruited. Three groups were divided according to a color Doppler ultrasound examination performed on the day of hCG injection. Group A, endometrial and subendometrial blood flows were 2 branches and below; group B, endometrial and subendometrial blood flows were between 3 and 4 branches; group C, endometrial and subendometrial blood flows were 5 branches and above. Patients were transferred 1-3 embryos each. Demographic data, ovarian responses, endometrial thickness, PI, RI, development of embryo and IVF result among groups were compared. Results Demographic data, ovarian responses, endometrial thickness, PI, RI and development of embryo among groups have no significant difference. The pregnancy rate of group A was significantly lower than that of group B (P〈0.05) and group C (P〈0.01). The implantation rate of group A was significantly lower than than of group C (P〈0.01). There was no significant difference of the rate of pregnancy and implantation between group B and group C (P〉0.05). Conclusion Endometrial and subendometrial blood flows measured by vaginal color Doppler ultrasound is a good predicator of pregnancy during IVF treatment. A good endometrial and subendometrial blood flows is benefit for the result of IVF.展开更多

Method...Objective: To evaluate the effect of manual acupuncture on endometrial blood flow parameters by three-dimensional(3D) power Doppler ultrasound in women undergoing in vitro fertilization embryo transfer(IVF-ET). Methods: Seventy patients undergoing IVF-ET were equally randomized into traditional or sham acupuncture treatment group for totally 4 days(from the day of oocyte aspiration to the day of embryo transfer) of treatment by random envelope method at the Reproductive Medicine Center and Outpatient Department of Integrated Traditional Chinese and Western Medicine, Tongji Hospital, Tongji Medicine College, Huazhong University of Science and Technology from January 2013 to December 2015. Patients in the traditional acupuncture group accepted traditional acupuncture methods with manual acupuncture, and Zhongji(CV3), Qihai(CV 6), Sanyinjiao(SP6), Taichong(LR 3), Tianshu(ST 25), Guilai(ST 29) and Zusanli(ST 36) were chosen. Patients at the sham acupuncture group accepted shallow acupuncture methods at 4 non-meridian points at each shoulder and upper arm. Outcome measures included endometrial ultrasonic indices such as vascularization index(VI), flow index(FI) and vascularization flow index(VFI), endometrial thickness and volume, subendometrial VI(sVI), subendometrial FI(sFI), subendometrial VFI(sVFI), implantation rate, clinical pregnancy rate, abortion rate, live birth rate and number of live births. Results: Finally, 34 patients in the traditional acupuncture group and 35 in the sham acupuncture group completed this trial. VI, FI and VFI of the traditional acupuncture group were significantly higher than those in the sham acupuncture group(P<0.05). No significant differences were found in endometrial thickness, endometrial volume, sVI, sFI, sVFI, implantation rate, clinical pregnancy rate, abortion rate, live birth rate and number of live births(P>0.05). Conclusions: Manual acupuncture performed after oocyte aspiration and before transplantation improved the endometrial blood flow parametersVI, RI and VFI in women who underwent IVF-ET, instead of sVI, sFI and sVFI. Successful embryo implantation depends on a well-functioning endometrium and high-grade quality embryos.An increasing body of evidence suggests that sildenafil citrate plays an important role in endometrial receptivit...Successful embryo implantation depends on a well-functioning endometrium and high-grade quality embryos.An increasing body of evidence suggests that sildenafil citrate plays an important role in endometrial receptivity and implantation.However,few studies have investigated the effects of oral sildenafil citrate on the clinical outcomes of patients with recurrent implantation failure(RIF).In the present study,we aimed to determine the impact of sildenafil citrate on improving the endometrial blood flow and pregnancy rate in RIF patients.A total of 346 frozen-thawed transfer cycles from 305 RIF patients undergoing IVF at the Center for Reproductive Medicine of Peking University Third Hospital from June 2013 to December 2017 were enrolled.Oral sildenafil citrate was given to the patients,and the related effects on endometrial blood flow,endometrial thickness,and clinical outcomes were investigated.In the 346 transferred cases,the proportion of three groups(A:undetectable blood flow;B:sub-endometrial blood flow;C:endometrial and sub-endometrial blood flow)before and after oral administration of sildenafil citrate was as follows:group A:36.7%(n=127)vs.7.5%(n=26);group B:42.8%(n=148)vs.24.0%(n=83);group C:20.5%(n=71)vs.68.5%(n=237).Moreover,the endometrial blood flow in 137 FET cycles was improved(61.8%).Besides,the implantation rate was 19.47%(132/678),and the clinical pregnancy rate reached 32.66%(113/346),while the endometrial blood flow of the pregnancy group was better compared with the non-pregnancy group.This study explored the improving effects of sildenafil citrate on the endometrial receptivity and clinical outcomes of RIF patients,confirming the crucial role of good endometrial blood flow in a successful pregnancy.Meanwhile,the low incidence of adverse events also supported the security and feasibility of sildenafil citrate in the clinical therapy of RIF patients.展开更多
