Application Value of Evidence-Based Nursing in the Perioperative Care of Ankle Fracture Patients

Objective:To analyze the value of applying evidence-based nursing in the perioperative care of patients with ankle fractures and propose corresponding measures to improve the quality of patient care.Methods:The study started in May 2022 and was terminated in March 2023.During this period,50 patients with ankle fractures treated by our hospital were selected as research subjects.They were divided into a control group and a research group.The control group had 25 patients who received routine care,and the research group had 25 who received evidence-based care.The nursing effects of the two groups were observed and compared.Results:The pain scores,quality of life,incidence of adverse reactions,and nursing satisfaction of patients in the research group were all significantly better than those in the control group(P<0.05).Conclusion:Evidence-based nursing can effectively improve the quality of care,reduce the risk of adverse reactions,and promote patient recovery,so it is suitable for clinical implementation. 展开更多

The evidence-based Traditional Chinese Medicine(TCM) nursing in standard plan is to be constructed by a combination of evidence-based thinking and expert consensus.(1) The symptoms of diseases with better curative effect in TCM and the nursing techniques of TCM based on scientific evidence can be preliminarily presented through bibliometric analysis.(2) Expert consultation letters can be developed based on bibliometric results, and symptoms of dominant diseases in TCM and TCM nursing techniques can be selected through those letters.(3) Based on the selected symptoms and TCM nursing techniques, the PICO can be determined, and then evidence search, evidence extraction, evidence analysis can be conducted and synthesized, and recommendations would be chosen.(4) The recommendation grade of each recommendation can be determined by applying the Delphi method and referring to GRADE.(5) The plan will be determined through discussions of experts. The construction of TCM nursing plan would make sure that TCM nursing is scientific, and internationalize TCM nursing. 展开更多

Objective:The study was to analyze the application effect of informationized teaching method based on evidence-based nursing in surgical nursing teaching.Methods:From December 2019 to December 2020,60 students were selected as the research objects and randomly divided into two groups,each with 30 students in the teaching group.The observation group applied informationized teaching based on evidence-based nursing method,and the control group used the traditional teaching model.The teaching effect was evaluated.Results:The test scores of subjective theoretical knowledge and objective theoretical knowledge of the observation group were significantly higher than those of the control group,and the comprehensive ability evaluation of the observation group was also higher(P<0.05).The majority of students accepted the informationized teaching method based on evidence-based nursing,and a few held a neutral or disapproval attitude.Conclusion:Informationized teaching method based on evidencebased nursing can improve students'theoretical and practical levels in surgical nursing teaching,and most students also accept this teaching method,which has application value. 展开更多

Objective:To understand the evidence-based practice readiness of developing evidence application projects in a 3A-level general hospital in Beijing,to identify barriers to evidence-based practice and to promote the successful application of evidence.Methods:All the clinical nurses who were going to participate in the application units of the evidence-based practice project were selected as the survey objects,and the self-made“Basic Information Questionnaire”and“Clinical Readiness to Evidence-based Nursing Assessment(CREBNA)”were used to conduct the survey.The total amounts and subscales were calculated.The factors that influence the score of the total scale were scored and analyzed.Results:The CREBNA total score was(119.87±19.18),the evidence subscale score was(47.94±8.54),the organizational environmental subscale score was(36.09±6.11),and the facilitator subscale score was(35.83±7.56).Univariate analysis showed that the total score of the scale was related to years of work,scientific research experience,knowledge of evidence-based care,and participation in evidence-based practice.Conclusion:The current evidence-based practice preparation situation is good.It is feasible to carry out evidence-based practice activities in the hospital.The follow-up development plan should be made based on the corresponding obstacle factors. 展开更多

BACKGROUND Elderly patients tend to have poor self-efficacy and poor confidence in postoperative rehabilitation for hip fractures,and are prone to negative emotions,which affect treatment compliance.AIM To evaluate the effects of evidence-based intervention on postoperative fear,compliance,and self-efficacy in elderly patients with hip fractures.METHODS A total of 120 patients with hip fracture surgically treated from June 2018 to June 2020 at the orthopedic department of our hospital were selected and divided into intervention and routine groups(n=60 each)according to different nursing methods.The basic rehabilitation methods of the two groups were consistent,but patients in the intervention group received evidence-based nursing interventions at the same time.Differences between groups in the scores of motion phobia,pain fear,rehabilitation training compliance,self-efficacy,nursing satisfaction,and hip joint function were compared before and after the intervention.RESULTS Before the intervention,there were no statistically significant differences in motion phobia and pain fear scores between the groups(all P>0.05).However,motion phobia scores at 1 wk after intervention initiation(P<0.05),and pain fear scores at 1 wk and 2 wk after intervention initiation(all P<0.05),were significantly lower in the intervention group than in the routine group.On the first day of intervention,there was no significant difference in rehabilitation treatment compliance between the groups(P>0.05);however,at 2 wk after intervention initiation,rehabilitation compliance was significantly better in the intervention group than in the routine group(P<0.05).Before the intervention,there were no statistically significant differences in the scores for the two self-efficacy dimensions(overcoming difficulties and rehabilitation exercise self-efficacy)and the total self-efficacy score between the groups(all P>0.05).After 2 wk of intervention,the scores for these two dimensions of self-efficacy and the total self-efficacy score were significantly higher in the intervention group than in the routine group(all P<0.05).At 3 and 6 mo after surgery,hip function as evaluated by the Harris hip score,was significantly better in the intervention group than in the routine group(P<0.05).Additionally,overall nursing satisfaction was significantly higher in the intervention group than in the routine group(P<0.05).CONCLUSION Evidence-based nursing intervention can alleviate fear of postoperative rehabilitation in elderly patients who underwent hip fracture surgery,and improve rehabilitation treatment compliance and patient self-efficacy,which promote hip function recovery. 展开更多

Background:Evidence-based interventions(EBIs)are frequently adapted or modified during the implementation process to meet the needs of the target context,public health professionals,or health practitioners.However,little is known about how the EBIs of the same clinical problem at a different time(sustainability)and a different setting(scalability)should be adapted to facilitate implementation.To address this gap,this research aims to analyze the adaptations process of EBIs by taking post-stroke dysphagia screening as an example based on a series of empirical studies.Methods:We reviewed three best practice implementation projects of post-stroke dysphagia identification and management in China's Mainland,and conducted a comparative analysis of three projects of dysphagia screening practical decision-making according to core elements of evidence-based nursing(EBN).Results:We identified the core elements of EBN that influence decision-making in each best practice implementation project.The decision-making of screening time and tool for dysphagia in the three projects varied.Project 1:Post-stroke dysphagia was screened using Water Swallow Test(WST)before the first intake of liquid or food.Project 2:A sustainability study based on project 1,post-stroke dysphagia was screened using Standardized Swallowing Assessment(SSA)before the first intake of liquid or food,within 24 hours of admission.Project 3:A community scalability study based on project 2,post-stroke dysphagia was screened using Water Swallow Test(WST)before the first intake of liquid or food,within 24 hours after referral.Conclusions:Adaption of EBIs needs to fully consider the best available external evidence from systematic research,clinical expertise,patient preferences,as well as context. 展开更多

A quality evaluation of nursing intervention studies in China's Mainland: From 1979 to 2012

Background:The number of papers published in the field of nursing practice has greatly increased in recent years in China's Mainland,yet the quality of these papers is highly variable.There has been no attempt to comprehensively estimate the overall quantity and quality of these papers.Objectives:To systematically retrieve nursing intervention studies published in simplified Chinese from 1979 to 2012,critically appraise their quality,and provide suggestions for future development.Methods:All of the papers were retrieved from China Biology Medicine disc database.The preliminary screening of retrieved publications was performed prior to conducting a rigorous quality evaluation of the remaining publications.10 characteristics would be included in consideration in quality evaluations.Results:69,150 papers were retrieved according to searching strategy.7391 of them were included after preliminary screening and appraised critically.Among the 10 characteristics considered in quality evaluations,the lowest ratings were observed for the factors of“utilisation of blind method”(13 articles),“description of loss of follow-up”(499 articles),“appropriate calculation of sample size”(511 articles)and“randomised assignment of patients to treatments”(652 articles).Conclusions:Chinese papers published in the field of nursing practice have increased over time,but improvements remain needed to ensure that thorough studies with high-quality research methodologies are being performed.Future nursing researchers should not only improve the design of their intervention studies but also clearly describe the methodology they used,especially in group randomisation,blinded research designs,and estimations of required sample sizes. 展开更多

Objective: Based on evidence-based thinking, a qualitative research method was used to analyze the clinical application status of the traditional Chinese medicine nursing program to provide scientific references for its optimization. Methods: Based on the three elements of the PARIHS framework, combined with clinical investigation, the interview outline was determined to interview the users and promoters of the program. After transcribing and coding the collected data with Nvivo software, referring to Colaizzi ’ s seven-step analysis and generic analysis, all codes are classified and integrated with the core elements and specific connotations of the PARIHS framework to form a theme. Results: The interviewees thought that although the program promoted the development of traditional Chinese medicine nursing to a certain extent, the content lacked scientific research evidence support, and some contents were inconsistent with clinical scenarios and past experience, and the implementation method was rigid, and the effect evaluation was unreasonable. There were 11 themes in three categories. Conclusion: Specific operation processes and scientific research evidence should be added to the scheme, and it should be improved according to clinical practice. 展开更多

Methods: A computer search was conducted for all evidence on chemotherapy-associated nausea and vomiting interventions, including guidelines, expert consensus, best clinical practice information booklet, recommended practice, evidence summary, and systematic review. The search period is from April 30, 2022. After the literature quality evaluation, the evidence extraction and summary of the literature meeting the quality standards. Results: A total of 16 literatures were included, including 9 guidelines, 5 systematic reviews and 2 expert consensus papers. 46 pieces of best evidence on the assessment and management of chemotherapy related nausea and vomiting in cancer patients were summarized, including 6 aspects including risk assessment and management, non-drug management, drug management, multidisciplinary cooperation, education and training, and health education. Conclusion: This study summarized the current best evidence on the assessment and management of chemotherapy-related nausea and vomiting in cancer patients. Clinical staff should apply evidence according to specific clinical scenarios, professional skills and patients’ wishes in order to reduce the degree and incidence of chemotherapy-related nausea and vomiting in cancer patients and improve the quality of care. 展开更多

Objective:The overarching objective of this study was to examine the effectiveness of HIV symptom management guidelines in China in reducing the incidence and severity of symptoms and improving patients'quality of life.Methods:We conducted a controlled,pre-and post-implementation design in the HIV/AIDS inpatient unit in Shanghai.Patients recruited from November 2014 to February 2015 were in the intervention group and those from October 2013 to February 2014 were in the control group.There were 74 patients in each group.Participants in the intervention group received interventions based on the HIV symptom management guidelines.Overall symptom severity,depression,and quality of life were measured in two groups at baseline,week 4,and week 8.Results:Totally 126 patients completed the research,65 in the intervention group and 61 in the control group.The total symptom severity scores showed a statistically significant difference between groups across time(P<0.05).It showed that frequencies of fatigue(36.9%vs.44.3%),fever(6.2%vs.11.5%),loss in weight(9.2%vs.16.4%),mouth ulcers(12.3%vs.16.4%),headaches(9.2%vs.19.7%)and depression(F=1.09,P>0.05)in the intervention group were lower than those in the control group in week 8 without statistical significance.The multilevel growth mixture model indicated a greater increase in the total score of quality of life for the group treated according to the symptom management guidelines(P=0.04).Conclusion:The evidence-based HIV symptom management guidelines can improve a patient's quality of life and relieve negative symptoms.The guidelines can be applied in a similar context to other HIV/AIDS units or clinics. 展开更多

The effect of a urinary incontinence selfmanagement program for older women in South Korea:A pilot study 被引量:3

Background:Although self-management approaches have shown strong evidence of positive outcomes for urinary incontinence prevention and management,few programs have been developed for Korean rural communities.Objectives:This pilot study aimed to develop,implement,and evaluate a urinary incontinence self-management program for community-dwelling women aged 55 and older with urinary incontinence in rural South Korea.Methods:This study used a one-group pre-post-test design to measure the effects of the intervention using standardized urinary incontinence symptom,knowledge,and attitude measures.Seventeen community-dwelling older women completed weekly 90-min group sessions for 5 weeks.Descriptive statistics and paired t-tests and were used to analyze data.Results:The mean of the overall interference on daily life from urine leakage(pre-test:M=5.76±2.68,post-test:M=2.29±1.93,t=4.609,p<0.001)and the sum of International Consultation on Incontinence Questionnaire scores(pre-test:M=11.59±3.00,post-test:M=5.29±3.02,t=-5.881,p<0.001)indicated significant improvement after the intervention.Improvement was also noted on the mean knowledge(pre-test:M=19.07±3.34,post-test:M=23.15±2.60,t=7.550,p<0.001)and attitude scores(pre-test:M=2.64±0.19,post-test:M=3.08±0.41,t=5.150,p<0.001).Weekly assignments were completed 82.4%of the time.Participants showed a high satisfaction level(M=26.82±1.74,range 22e28)with the group program.Conclusions:Implementation of a urinary incontinence self-management program was accompanied by improved outcomes for Korean older women living in rural communities who have scarce resources for urinary incontinence management and treatment.Urinary incontinence self-management education approaches have potential for widespread implementation in nursing practice. 展开更多
