Efficacy of Mometasone Furoate Nasal Spray in Delayed Antibiotic Prescriptions for Acute Rhinosinusitis: A Multicenter, Randomized, Open-Label, Comparative, Prospective, Parallel Group Study

While their clinical presentation is similar, post-viral rhinosinusitis does not indicate the possible development of bacterial rhinosinusitis which occurs in only 0.5% to 5% of all cases of ARS. However, unreasonable antibacterial therapy is prescribed in 54% to 77% of cases. The perspective is to use delayed antibacterial therapy management. The purpose of the study was to investigate the efficacy of mometasone furoate nasal spray within the delayed prescription approach. Methods: A multicenter, randomized, open-label, comparative, prospective, parallel group study enrolled a total of 156 patients with ARS aged 18 to 60 years, of them 155 patients completed the study. The patients were randomized into the main group of Mometasone Furoate (Flix) Nasal Spray or into the control group of amoxicillin clavulanate tablets while both groups were administered a standard ARS therapy. Findings: The use of MF nasal spray within the delayed antibiotics prescription approach has a significantly higher efficacy compared to the immediate antibiotics prescription in the first days of treatment (p < 0.05). Antibacterial therapy was prescribed to 42.3% of patients in the main group compared to 100.0% of patients in the control group (p < 0.05). It was shown that there were statistically significant differences in the dynamics of changes in the total symptom score between the patients administered antibiotics and the antibiotic-naive patients. Conclusion: The use of MF within the delayed antibiotics prescription approach reliably improves the pattern of symptoms over time at the disease onset and significantly reduces the need for NSAIDs administration and antibiotics prescription frequency. 展开更多

Methyl 2-furoate(FAME2)is a model for the potential renewable biofuel of dimethyl furan-2,5-dicarboxylate,with the development of its new synthesis method.The potential energy surfaces of H-abstractions and OHadditions between FAME2 and hydroxyl radical(OH)were studied using CCSD(T)/CBS//M062X/ccpVTZ.The subsequent isomerization and decomposition reactions were also determined for the primary radicals produced.The results showed that H-abstraction on the branched methyl group was the dominant channel and that the OH-addition reactions on the furan ring had a significant pressure dependency.The rate coefficients presented here provide important kinetic data to support future improvement of the combustion mechanism of FAME2,and present a sound basis for further research into practical fuels. 展开更多

Influence of tacrolimus combined mometasone furoate on serum inflammatory factors and immune function in children with eczema

Objective:To study the influence of tacrolimus combined mometasone furoate on serum inflammatory factors and immune function in children with eczema.Methods: A total of 122 children with eczema based on random data table method were divided into control group (n=58) and observation group (n=64) The control group were given 0.1% mometasone furoate ointment treatment and the observation group of patients were given 0.03% tacrolimus ointment treatment on the basis of the control group. After 3 weeks, the serum levels of inflammatory factors and index of immune function before and after treatment were compared. Results:Before the treatment, the levels of IL-2, IL-4, IL-5, INF-γ, CD3+, CD4+, CD8+, CD4+/CD8+ were similar in two groups, there was no statistically significant difference. Comparing the levels of inflammatory after treatment, IL-2, INF-γ levels of two groups were significantly higher than those of the same group before treatment, and IL-2, INF-γ levels of the observation group were significantly higher than those of the control group. After treatment, IL-4 IL-5 levels of the two groups were significantly declined and IL-4 IL-5 levels of the observation group were significantly lower than those of the control group. Immune function indicators level detection indicated that the levels of CD3+, CD4+, CD8+, CD4+/CD8+of control group before and after treatment had no significant change. After treatment, the levels of CD3+, CD4+ and CD4+/CD8+ of observation group were significantly increased and were significantly higher than those of the control group, whereas the levels of CD8+ were significantly reduced and significantly lower than those of the control group.Conclusions:Tacrolimus combined moetasone furoate is an effective therapy to adjust the serum levels of inflammatory cytokines and improve immune function of children eczema Indeed, it has a certain clinical value. 展开更多

Objective:To retrospectively evaluate efficacy and safety of 0.05%halometasone/1%triclosan cream(Hal/TCS)in treating truncal vitiligo of children and adolescents.Methods:We retrospectively reviewed the records of children and adolescents with truncal vitiligo treated only with topical Hal/TCS twice daily or only with topical 0.1%mometasone furoate cream(mometasone)twice daily for at least 3 months,and collected relevant data.Efficacy outcomes included patients’mean repigmentation score after 1-and 3-month treatment calculated based on patients’repigmentation percentage,and effective rate(percentage of patients with≥50%repigmentation).Subgroup analyses of patients’mean repigmentation score based on their age,vitiligo stage,and duration were conducted.Rate of adverse events were calculated.Results:One hundred and twenty-two eligible patients were included,among them,63 and 59 patients were in the Hal/TCS group and the mometasone group,respectively.After 3 months of treatment,the Hal/TCS group had significantly higher mean repigmentation score and effective rate than the mometasone group(53.49±28.33vs.41.46±27.16,P=0.02;53.97%vs.35.59%,P=0.04,respectively).Patients with shorter disease duration(≤12 months)and patients with progressive vitiligo responded significantly better to the Hal/TCS treatment than patients with longer disease duration and patients with stable vitiligo,respectively.Both treatments were well tolerated and the two treatment groups had comparable rates of adverse events.Conclusions:Short-term Hal/TCS treatment was effective,well-tolerated,and safe in treating vitiligo in the trunk of children and adolescents. 展开更多
