Optimization of Technical Parameters for Detecting Mycobacteria in Hospital Wastewater in Tropical Urban Areas: The Case of the City of Abidjan (Côte d’Ivoire)

The loads of organic matter, microorganisms, detergents and antibiotics in liquid hospital effluents make them complex environments, raising numerous health and ecological questions. Investigations of mycobacteria in water lack adequate techniques. This study is the first part of a pilot project aimed at developing an optimized protocol for the isolation of mycobacteria from hospital effluents, as a prelude to more in-depth investigation in this matrix. The aim was to compare the performance of two decontamination methods, three culture media and two incubation temperatures generally proposed in the literature, in order to identify the most effective methods in each case, as well as possible areas for improvement in the isolation of these germs from this environmental matrix. The results show that liquid hospital effluent can be decontaminated using both the NaOH method (4%;for 30 min.) and the CPC method (0.05%;for 30 min.), with the same mycobacteria recovery efficiency. Despite the low concentration, decontamination with CPC killed more mycobacteria and sufficiently eliminated contaminating germs. In contrast, decontamination with NaOH was less harmful to mycobacteria, but did not remove many contaminating germs. On the other hand, LJG medium performed better than LJGF medium and LJGP medium for the growth of mycobacteria in hospital waters. Finally, there was no difference in performance between the two incubation temperatures of 30℃ and 37℃. The results of this study show that further evaluation of existing protocols is required in order to optimize methods for the pre-treatment of hospital effluent for the isolation of mycobacteria. 展开更多

Most hospitals in developing countries, including Benin, do not have a wastewater treatment plant. In this study, the wastewater from two hospitals in northern Benin was characterized and then treated with Azadirachta indica leaves, Moringa oleifera and Luffa cylindrica seeds by coagulation/flocculation process. The wastewater characteristics showed that the collected samples are greatly polluted by organic matter and fecal bacteria such as Escherichia coli, Enterococcus fecal and Total coliforms. Jar-test results revealed that 95.74%, 78%, 49.19% of turbidity, 51.35%, 38.32%, 22.19% of COD, 93.16%, 85.26%, 83.30% against Escherichia coli, 92.11%, 90.93%, 94.60% against total Coliforms and 99.37%, 91%, 99%, 55.07% against Enterococcus were removed from hospital wastewater using Moringa oleifera, Luffa cylindrica seed and Azadirachta indica leaves respectively at dose of 100 mg/L. The results highlighted that the natural coagulants could be successfully used for the removal of turbidity and fecal bacteria from hospital wastewater. 展开更多

These water needs to be checked by a biological purification before leaving to nature. Hospital wastewater has differences than domestic waste because of especially blood, body waste, drugs, chemicals, medical device waste and radioactive materials. We aimed to determine genotoxic effects of total pollution in hospital wastewater on alive by Salmonella microsome test method. In this study, we decided on three hospitals which weren’t checked as biological purification of waste. The samples were taken for six 1-week periods between March 2009 and June 2009. Mutagenite studies of samples taken from hospitals were made with ,Salmonella typhimurium TA 98 and TA 100. Wastewater samples were evaporated. 27 different test materials were prepared using DMSO, ethanol and acetone solvents, two different MGA plaques were used for each test material. Each experiment was made for 3 times with known results of mutagens and we made it ready for “Ames” test method. We had genotoxicity 50% in Istanbul University Medical Faculty Hospital, 56% in Haseki Hospital and 61% in Vak?f Gureba Hospital. According to three hospitals result there are 9 positives, 9 negatives in DMSO;9 positives, 9 negatives in ethanol;12 positives, 6 negatives in acetone. These values are totally 56%. Our results give important information about mutagenic effect of total pollution in hospital wastewater. It is first time researched in Turkey that effect on DNA of pollution is from hospital wastewaters. In prospective studies, it is necessary to use this system as a method to monitor mutagenic genotoxic pollution in hospital wastewaters. These kinds of studies present applicability and importance of our method because of placing in the literature. The isolated bacteria from the wastewater soils were found to be mainly of Morganella morganii and Bacillus cereus and these isolated bacteria were found to be very resistant to a wide range of antibiotics and heavy metals. Antibiotics that were used for treating infections caused by these bacteria like chloramphenicol and ciprofloxacin were also found to be insensitive. The degree of resistance was also very high when compared to earlier reports of antibiotic resistance observed in the corresponding bacteria. Our results suggested that the high degree of resistance is probably conferred by the continued exposure to antibiotics from hospital waste leading to a selected population of highly antibiotics-resistant bacteria. 展开更多

In hospital, a variety of substances are in use for medical purposes as diagnostics and researches. Among them, heavy metals which are not biodegradable by chemical reactions have bioaccumulation properties. Cadmium is a toxic pollutant that enters hospital wastewaters by different ways. However, isolated bacteria from hospital effluents have high resistance to antimicrobial agents and therefore the ability to uptake cadmium and other metals. So, their use is recommended for wastewater bioremediation. In this study, twelve cadmium-resistant bacteria were isolated from hospital wastewaters and identified. The cadmium-resistant isolates characterized include both Gram-negative [75%] and Gram-positive [25%]. The Minimum Inhibitory Concentration of hospital wastewaters isolates was determined in solid media and was ranged from 250 to 950 micrograms/milliliters. All the isolates showed co-resistance to zinc and mercury. They were also antibiotic resistant and tolerated up a significant concentration of NaCl. The cadmium-resistant species Klebsiella pneumoniae and Pseudomonas aeruginosa were the most resistant to cadmium. The species Klebsiella pneumonia also resists to the highest concentrations of zinc and mercury. The results of toxicity tests on Vibrio fischeri, showed that the DI50 [15 min] as low as 0.07 carried away 50% luminescence inhibition. 展开更多

The widespread emergence of antibiotic resistance among bacterial pathogens has become one of the most serious challenges in Ethiopia. This study determined the prevalence and drug resistance patterns of bacterial pathogens isolated from treated and untreated wastewater released from Ayder Referral Hospital in Northern Ethiopia. A cross sectional study design was conducted from September-December, 2015 in wastewater released from Ayder referral hospital. A total of 40 composite samples were aseptically collected, transported and processed for enumeration of indicator organisms, bacteriological identification and susceptibility testing following standard procedure. Data obtained were entered and analyzed using SPSS version 20. Mean heterotrophic plate count, total coliform count, fecal coliform count and E. coli count were found to be 1.6 × 106 CFU/mL, 2.2 × 106 CFU/100 mL, 2.0 × 105 CFU/100 mL and 1.1 × 104 CFU/100 mL from treated wastewater respectively. Among the total samples 134 bacterial isolates were detected and [84 (62.7%)] were from untreated wastewater and [50 (37.3%)] were from treated wastewater. The most frequently isolated bacteria from untreated wastewater samples was Klebsiella spp [14 (16.7%)] followed by S. aureus [13 (15.5%)] and P. aeruginosa [12 (14.3%)], similarly in treated wastewater samples Klebsiella spp [10 (20%)], P. aeruginosa [8 (16%)] and S. aureus [8 (16%)] were frequently detected. The overall multi-drug resistance (MDR) in this study was [79/134 (79.1%)]. MDR from untreated wastewater sample was [64/84 (76.2%)] while from treated wastewater sample was [42/50 (84%)] and shows significant difference with (COR: 1.64, 95% CI: 1.15 - 3.29, P: 0.001). It is concluded that treated hospital wastewater contains large numbers of antibiotic resistant bacteria. Therefore, there should be continuous monitoring and evaluation of the effluent quality of the ponds and chlorination of the final effluent should be developed. 展开更多

Ecological Wastewater Treatment System in a School Environment Using a Horizontal Flow Biological Reactor: The Case of Typha

The overarching goal of this study is to offer an effective and sustainable solution to the challenges of sanitation in rural and school settings in the northern region of Senegal. The study explores a wastewater treatment approach based on phytoremediation, with a particular focus on the use of horizontally-flowing reed bed filters. Furthermore, it aims to adapt and optimize these systems for the specific needs of Senegal, focusing on wastewater in school environments. Thus, we constructed a horizontally-flowing reed bed filter, planted with Typha, at the Ndiébène Gandiol school in Senegal. We will investigate the efficiency of wastewater treatment by this horizontally-flowing reed bed filter, emphasizing the role of the plant used: Typha. The filter is described in detail, specifying its dimensions, its composition of flint gravel, and the choice of plants, namely Typha. The experimental protocol is detailed, describing the sampling at the entrance and exit of the filter to evaluate water quality. The parameters analyzed include Chemical Oxygen Demand (COD), Biochemical Oxygen Demand over 5 days (BOD5), suspended solids, ammonium, nitrates, phosphates, pH, conductivity, and fecal coliforms. The results indicate a significant improvement in water quality after treatment. COD, BOD5, suspended solids, and fecal coliforms are greatly reduced, thus demonstrating the efficacy of the Typha filter. However, nitrate concentrations remain relatively stable, suggesting room for improvement in their elimination. A perspective of reuse of the treated water is considered, showing that the effluents from the planted filter meet Senegalese and international standards for irrigation. The findings suggest that these waters could be used for a variety of crops, thereby reducing the pressure on freshwater resources. In conclusion, the Typha-based filtration system shows promising results for improving water quality in this region of Senegal. However, adjustments are necessary for more effective nitrate removal. This study paves the way for sustainable use of treated wastewater for irrigation, thus contributing to food security and the preservation of water resources. 展开更多

The objective of our study was to investigate the echocardiographic aspects of type 2 diabetics. Patients and Method: Descriptive and cross-sectional study of 12 months from June 2020 to June 2021. We included hospitalized type 2 diabetics who underwent transthoracic cardiac ultrasound in the Department of Medicine and Endocrinology at the Mali Hospital. Results: We collected 128 type 2 diabetics. The predominance was male with a sex ratio of 1.2. The mean age of patients was 60.06 ± 11.54 years with extremes of 28 and 84 years. Echocardiographic abnormalities were dominated by abnormal relaxation of left ventricle in 62.5%, increased of left ventricle mass in 54.7% and left atrium dilation in 28.1%. Patients with type 2 diabetes mellitus and hypertension had more left atrium dilation with a p of 0.02. Disorders of global kinetics and systolic dysfunction were more prevalent in smoking patients with statistically significant associations, respectively, p = 0.02;p = 0.03. Dyslipidemia had a statistically significant association with segmental kinetic disorders with a p of 0.008. Duration of diabetes greater than 5 years was associated with left atrium dilation and p-value was 0.04. Conclusion: Diabetes is responsible for cardiovascular manifestations that can be identified with transthoracic echocardiography. Its performance in diabetic patients makes it possible to refine the patient’s management. 展开更多

Ecological Wastewater Treatment System Using a Horizontal Flow Biological Reactor: The Case of Vetiver

Confronted with the challenge of wastewater management, particularly in the school environment of Senegal, our study set out to achieve multiple objectives. Following field surveys, laboratory analyses of wastewater samples were carried out, revealing a significant pollutant load. In the community of Gandiol, near Saint-Louis (Senegal), the school of Ndiebene Gandiol 1 faces significant sanitation challenges. Our study aimed to address this issue by using a constructed filter composed of two filtering bed cells measuring 12 × 8.5 m, preceded by a septic tank. We particularly focused on the influence of Vetiver;a plant chosen for its purification potential. Our analyses showed remarkable efficiency of the filter. Elimination rates reached 95% for 5-Day Biochemical Oxygen Demand (BOD5), 91% for Chemical Oxygen Demand (COD), and 92% for SS, far exceeding the Senegalese standards set at 50 mg/L, 200 mg/L, and 40 mg/L, respectively. Furthermore, the concentration of fecal coliforms was reduced to 176 FCU/100mL, well below the Senegalese threshold of 2000 FCU/100mL and close to the World Health Organization’s (WHO) recommendation of 1000 FCU/100mL. However, despite these promising results, some parameters, particularly the concentration of certain pollutants, approached the thresholds defined by European legislation. For example, for Suspended Solids (SS), the post-treatment level of 3 mg/L was well below the Senegalese standard but edged close to the European minimum of 10 mg/L. In conclusion, the Vetiver filter demonstrated a remarkable ability to treat school wastewater, offering high pollutant elimination percentages. These results suggest significant opportunities for the reuse of treated water, potentially in areas such as irrigation, though some adjustments may be necessary to meet the strictest standards such as those of the European union (EU). 展开更多

Objective: To review, categorise, and synthesise findings from literature on health information technology (HIT) functionalities, HIT use, and the impact of HIT on hospital performance. Materials and Methods: We conducted a systematic integrative literature review based on a compre-hensive database search. To organise, categorise and synthesise the ex-isting literature, we adopted the affordance actualization theory. To align the literature with our research framework, we used four categories: 1) the functionalities of HIT and how these functionalities are measured;2) use and immediate outcomes of HIT functionalities;3) different perfor-mance indicators and how HIT functionalities affect them;and 4) what hospital characteristics influence the outcome of hospital performance. Results: Fifty-two studies were included. We identified four types of HIT. Only ten studies (19.2%) define the use of HIT by explicitly meas-uring the use rate of HIT. We identified five dimensions of hospital per-formance indicators. Every dimension showed mixed results;however, in general, HIT has a positive impact on mortality and patient readmis-sions. We found several hospital characteristics that may affect the rela-tionship between HIT and hospital-level outcomes. Discussion: Further efforts should focus on embedded research on HIT functionalities, use and effects of HIT implementations with more performance indicators and adjusted for hospital characteristics. Conclusion: The proposed framework could help hospitals and researchers make decisions regard-ing the functionalities, use and effects of HIT implementation in hospi-tals. Background: Hospital Acquired Infections (HAIs) remain a common cause of death, functional disability, emotional suffering and economic burden among hospitalized patients. Knowledge of HAIs is important in its prevention and control. This study seeks to assess the knowledge of Hospital Acquired Infections (HAIs) among medical students in a Tertiary Hospital in Jos North Local Government Area, Plateau State, Nigeria. Methods: This was a descriptive cross-sectional study done in October 2019 among clinical medical students using a Multistage sampling technique. Data was collected using a self-administered structured questionnaire and analyzed using the IBM SPSS 20 (Statistical Package for the Social Sciences). Ethical approval was granted by Bingham University Teaching Hospital, Ethics Committee, Jos, Plateau State. Results: A total of 219 students in the clinical arm of the College of Medicine and Health Sciences were selected. A higher proportion (97.7%) of respondents knew about Hospital Acquired Infections and 85.4% knew that Hospital Acquired infections occur in the hospital, and (86.3%) considered patients contagious with half (58.9%) considered patients as the most important source of HAIs, followed by care givers (13.2%), then doctors including medical students and interns (10.0%) and lastly nurses (8.7%). The majority of respondents (70.8%) considered Surgical Wound Infections to be the most commonly occurring HAI, followed by UTIs (69.9%), RTIs (61.2%), BSIs (37.0%) and others (0.9%). The clinical thermometer was the instrument that most commonly transmits HAIs (82.6%), then followed by stethoscope (62.1%), white coats (53.9%), and blood pressure cuff (51.1%). Most respondents knew the infectious substances, like blood (96.3%), nasal discharge (82.6%), saliva (85.3%), and faeces (79.4%) transmitted HAIs, 72.6% of the respondents said that they were aware of the recommended hand washing techniques by WHO. Conclusion: The majority of students 91.3% had good knowledge while 8.7% had poor knowledge of HAIs. Lower classes had more respondents with poor knowledge. This finding was statistically significant (p = 0.002, Chi-square 12.819). Students are encouraged to keep up the level of knowledge they have about HAIs. These students can help improve the knowledge of those whose knowledge level is low. Government and NGOs should support sponsorship for capacity-building events targeted at HAIs for healthcare workers and medical students. 展开更多

Introduction: Acute coronary syndromes (ACS) are a diagnostic and therapeutic emergency. Objective: Studying the feasibility, difficulties and results of coronary angioplasty in acute coronary syndromes at the Luxembourg Mother-Child University Hospital in Bamako. Patients and Methods: Cross-sectional, descriptive study with prospective recruitment over 18 months from September 2020 to February 2022. All patients aged at least 18 years old admitted for SCA and having undergone PCI during the study period were included. Result: We collected 249 patients for SCA, of whom 160 underwent angioplasty, either an angioplasty/SCA ratio of 0.64. The average age of the patients was 59.54 ± 11.62 with extremes of 32 and 92 years. The age group of 45 to 65 years was the most representative. The predominance was male, sex ratio of 2.4. The main cardiovascular risk factors were high blood pressure (58.23%) and diabetes (45.78%). Persistent ST-segment elevation on the electrocardiogram was present in 71.48%. The treatment time was more than 12 hours after the first medical contact in 95.5%. The approach was radial in 96.5% of cases. Coronarography was pathological in 91.16% of our patients. The lesions were tri-truncular in 34.13% with the anterior inter ventricular as culprit artery in 72% of cases. The majority of patients (64%) had undergone angioplasty with implantation of an active stent. Angioplasty was performed successfully in 98% and per procedural mortality was 1.87%. Only 6.45% of ACS with ST elevation benefited from primary angioplasty. Conclusion: Percutaneous coronary intervention is performed routinely in our center with satisfactory results. Difficulties exist, related to the diagnostic delay of ACS and the high cost of angioplasty. 展开更多

The coagulation process is a widely applied technology in water and wastewater treatment.Novel composite polyferric mag-nesium-silicate-sulfate(PFMS)coagulants were synthesized using Na_(2)SiO_(3)·9H_(2)O,Fe_(2)(SO_(4))_(3),and MgSO_(4) as raw materials in this paper.The effects of aging time,Fe:Si:Mg,and OH:M molar ratios(M represents the metal ions)on the coagulation performance of the as-pre-pared PFMS were systematically investigated to obtain optimum coagulants.The results showed that PFMS coagulant exhibited good co-agulation properties in the treatment of simulated humic acid-kaolin surface water and reactive dye wastewater.When the molar ratio was controlled at Fe:Si:Mg=2:2:1 and OH:M=0.32,the obtained PFMS presented excellent stability and a high coagulation efficiency.The removal efficiency of ultraviolet UV254 was 99.81%,and the residual turbidity of the surface water reached 0.56 NTU at a dosage of 30 mg·L^(-1).After standing the coagulant for 120 d in the laboratory,the removal efficiency of UV254 and residual turbidity of the surface wa-ter were 88.12%and 0.68 NTU,respectively,which accord with the surface water treatment requirements.In addition,the coagulation performance in the treatment of reactive dye wastewater was greatly improved by combining the advantages of magnesium and iron salts.Compared with polyferric silicate-sulfate(PFS)and polymagnesium silicate-sulfate(PMS),the PFMS coagulant played a better decolor-ization role within the pH range of 7-13. 展开更多

However, the diagnosis is often unknown at birth, and therefore late. In Ivory Coast, there are few studies on cryptorchidism and micropenis. We conducted this study to identify the epidemiological, clinical, etiological and therapeutic characteristics of the micropenis associated or not with cryptorchidism at Yopougon University Hospital. Methods: We conducted a retrospective descriptive study in the Department of Endocrinology-Diabetology of the Yopougon University Hospital carried out over 13 years from January 2005 to December 2018. All patients were included regardless of the presence of a micropenis associated or not with cryptorchidism in the clinical or paraclinical examination. Results: A total of 14 micropenis were reported associated in 6 cases with cryptorchidism. It was unilateral cryptorchidism in 4 patients or 66.66% of cases. The average age of affected patients was 21.32 years with extremes ranging from 10 months to 48 years. The reason for consultation was micropenis in 12 out of 14 cases or 85.75%. The associated signs apart from cryptorchidism were obesity in 42.86% of cases, gynecomastia in 35.71% of cases. The aetiology was dominated by idiopathic causes. Treatment with testosterone enanthate was more effective in pre-pubertal children than in adults. Conclusion: The diagnosis of micropenis associated or not with cryptorchidism is late because of the frustrations, taboos and psycho-social states that it causes. However, drug treatment is effective when treatment is started early. 展开更多

Infertility is of male origin in 40% of cases. Our objective was to evaluate the epidemiological and clinical aspects of male infertility in our setting. Methods: This was a cross-sectional and descriptive study, with retrospective data collection from the files of patients received for male infertility, over 5 years and 6 months (from January 1<sup>st</sup>, 2016 to April 30<sup>th</sup>, 2021) at the Urology Department of the Yaounde Central Hospital. Phone calls were made to patients with incomplete records for completion following a verbal consent. Data reported on data collection forms were entered into a data entry form designed on CSPro software version 7.2. The database was later exported for descriptive statistics using the IBM SPSS software version 23. Qualitative variables were summarized in frequencies and percentages while quantitative variables were summarized in means with their standard deviations for normally distributed data, or medians and interquartile ranges otherwise. Results: Out of 110 patients whose records were analyzed, the median age was 37.0 years, with an interquartile range of 8.3, with 55.5% in the age group 30 to 39 years. Of these 95 (86.4%) were married, 58 (52.7%) were from the West region, 76 (69.1%) were employed. Past history was dominated by mumps 40 (36.4%), varicocele surgery 25 (22.7%), alcoholism 87 (79.1%) and smoking 15 (13.6%). A disorder of spermatogenesis was found in all our patients: 37 (33.6%) azoospermia, 27 (24.6%) asthenospermia and 26 (23.7%) oligospermia patients. Paraclinical evidence of chlamydia and gonococcal infections were found respectively in 38 (34.5%) and 14 (12.7%) patients, varicocele and testicular dystrophy respectively in 95 (86.4%) and 38 (34.5%) patients. Infertility was primary in 67 (60.9%) patients. Conclusion: Most patients with infertility were in their thirties, had risky social habits and comorbidities. An early start of health promotion activities, comorbidities prevention and early detection of spermogram abnormalities would make it possible to reduce the frequency of male infertility. 展开更多

Introduction: Anemia in people living with human immunodeficiency virus (PLHIV) is a major health problem. Although anemia often responds to combination antiretroviral therapy, many patients remain anemic despite treatment, and such persistent anemia continues to adversely affect prognosis, regardless of drug response. Scientists have identified some of the factors involved. However, the mechanisms put in place have not been effective in overcoming them. Examples include the withdrawal of zidovudine from antiretroviral treatment lines, iron and folate supplementation, etc. Anemia is still a major concern in HIV-positive patients. The aim of this study is to assess the prevalence of anemia and its associated factors among PLHIV followed up at the outpatient treatment centre (CTA) of the Panzi General Reference Hospital (HGR) in South Kivu, Democratic Republic of Congo (DRC). Method: We conducted a cross-sectional, comparative study of 276 HIV-infected adults on antiretroviral therapy (ART) followed up at the CTA of Panzi HGR. Socio-demographic and nutritional parameters were collected using a survey questionnaire, and clinical assessment and nutritional status were performed at the centre. Hemoglobin, seric albumin and viral load determinations were performed at the HGRP laboratory. We constructed univariate and multivariate logistic regression models to assess factors associated with anemia in people living with HIV/AIDS. Results: We found a prevalence of anemia of 39.4%, including 4.1% severe anemia, 17.7% moderate anemia and 17.5% mild anemia. After multivariate adjustment, the factors associated with anemia in our PLHIV were: moderate undernutrition (aOR = 1.26;95% CI: 1.50 - 4.20;p = 0.001), severe undernutrition (aOR = 115.4;95% CI: 2.04 - 164.52;p = 0.021), hypoalbuminemia (aOR = 2.11;95% CI: 1.87 - 5.10;p = 0.004) and the lower degree of dietary diversity (aOR = 1.56;95% CI: 1.10 - 4.32;p = 0.034). Conclusion: The prevalence of anemia in PLHIV on ART is high. This greatly affects quality of life and increases the need for care. Early detection tools and management algorithms are essential in the follow-up of PLWHIV. 展开更多
