Objective: Lower urinary tract symptoms(LUTS) caused by benign prostatic hyperplasia(BPH) affect the quality of life of elderly individuals. Acupuncture and moxibustion are used in the clinic in China for improving LU...Objective: Lower urinary tract symptoms(LUTS) caused by benign prostatic hyperplasia(BPH) affect the quality of life of elderly individuals. Acupuncture and moxibustion are used in the clinic in China for improving LUTS symptoms due to BPH. However,there is no evidence to suggest which is the best option. We compared the efficacy of acupuncture and moxibustion to provide evidence for clinical decision-making.Methods: PubMed, Embase, Cochrane Library, Web of Science, China National Knowledge Infrastructure, Wan Fang Data, and VIP databases were searched from inception to July 2020 to identify the randomized controlled trials(RCTs) of acupuncture and moxibustion for LUTS due to BPH. Two researchers filtered studies and extracted the information independently. This study conducted a network meta-analysis using the Bayesian random method. The interventions ranking was evaluated using the surface under the cumulative ranking curve(SUCRA).Results: We finally included 40 studies comprising 10 treating therapies and 3,655 patients with LUTS caused by BPH. In terms of the International Prostate Symptom Score, maximum urinary flow rate, and quality of life, electroacupuncture(EA) [MD =-3.6,95% credible interval(CrI)(-5.5,-1.8), very low certainty of evidence;MD = 2.2, 95% CrI(1.1, 3.3), low certainty of evidence;MD =-1.3, 95% CrI(-2.2,-0.43), very low certainty of the evidence] may be consistently the optimal treatment compared with other interventions, with SUCRA values of 84%, 81%, and 89%, respectively.Conclusions: Of all treatments, EA may have the best efficacy with fewer adverse events for LUTS due to BPH. The quality of evidence supporting this result is low to very low certainty of the evidence due to the limitations of primary studies;thus, more highquality RCTs are needed for further evidence.展开更多

Currently BPH is associated with unregulated proliferation of connective tissue and glandular epithelium within the prostatic transition zone, and it has been described as relevant characteristic of BPH—the increase of the total number of cells, and not only an increase in cell size. To date, there are few studies on the quantitative morphology of glandular tree of BPH compared with normal prostate. The scarce investigations about this particular suggest that the glandular tree branches and expands as the hyperplastic transformation occurs in the prostate. Methods: To verify if this gland expansion and branching was similar to that occurs in the normal prostate, this study deals with the estimation of several stereological parameters as: labeling index for the proliferating cell nuclear antigen to quantify the rate of proliferation of prostate epithelium, average thickness of glandular epithelium, fraction of the volume occupied by the epithelium relative to the total prostate volume, connectivity density of prostate glands, to quantify the branching of prostate glands, and the average volume and the volume-weighted mean glandular volume of prostate acini to assess the mean size of the prostate acini and its variability. Results: All these estimates have been performed in prostate specific antigen immunostained sections from prostates of young men (controls) and in adenomectomy specimens from the adenofibromiomatous variety of BPH. Conclusion: We conclude that the epithelial proliferation is not the only factor intervening in the development of BPH. In addition, a more prolonged survival of epithelial population, together with some degree of hypertrophy of acini expressed by the increase of volume fraction and thickness of acinar epithelium, is relevant in order to the growth and expansion of the BPH glandular tree that shows more abundant and heterogeneous acinar sprouts than in normal prostate.展开更多

Acupoints used were Qimen (期门 LR 14), Wuyi (屋翳 ST 15), Weishu (胃俞 BL 21), etc.. Acupuncture treatment was conducted once daily and 30 treatments constituted one therapeutic course. Results: After 3 courses of treatment, of the 53, 30 and 37 cases of the liver-qi-stagnation, phlegm-coagulation and Thoroughfare-Conception vessel maladjustment types, 43 (81.1%), 24 (80.0%) and 29 (78.4%) were cured, 7 (13.2%), 5 (16.7%)and 5 (13.5%) markedly improved, 3 (5.7%), 1 (3.3%) and 2 (5.4%) improved respectively, and 1 (2.7%) of Thoroughfare-Conception vessel maladjustment type failed. A two-years’ follow-up showed that 3 of the cured 92 cases had a relapse, and after 2 more courses of treatment, they were cured again. Conclusion: Acupuncture therapy is effective for the treatment of hyperplasia of mammary glands.展开更多
Clinical observations on 300 cases of hyperplasia of mammary glands (group A) treated by red hot needle plus filiform needle are reported in the present study. "Ashi" points were used as the main acupoints. ...Clinical observations on 300 cases of hyperplasia of mammary glands (group A) treated by red hot needle plus filiform needle are reported in the present study. "Ashi" points were used as the main acupoints. For comparison, other 300 cases (group B) were treated with routine acupuncture method, and Rugen (ST 18) was used as the main acupoint. In addition, other auxiliary acupoints were supplemented in the light of the concrete syndromes in these two groups. The treatment was given once daily, with 10 sessions being a therapeutic course. Of the 300 cases in group A, 240 were clinically cured, amounting to 80%; 58 were effective, amounting to 19.3%; 2 were failed, with a total effective rate of 99.3%. Of the 300 cases in group B, 113 were clinically cured, 165 effective, 22 were failed, with a total effective rate of 92.7%. The therapeutic effect of group A is significantly superior to that of group B (P<0.05).展开更多

Results: Before treatment IPSS and QOL of the patients were 22.1±6.7 and 4.5±0.6 respectively, and after treatment were 9.1±4.3 and 1.9±1.1 respectively, there were very significant differences in IPSS and QOL between pre-treatment and post-treatment(P〈 0.01). Methods on benign prostate hyperplasia Two hundred and forty patients diagnosed clinically with BPH were randomly divided into 3 groups. In scalp-acupuncture group (80 cases), foot-motor-sensory area and reproductive area were selected. In body-acupuncture group (82 cases), the acupoints were selected, such as Zhengji(中极 CV 3), Guanyau(关元 CV 4) and Sanyfnjiao (三阴交 SP 6). In combined therapy group of scalp acupuncture and body acupuncture (78 cases), scalp acupuncture was applied together with body acupuncture. It was to observe the changes in prostate size, Qmax, IPSS (international prostatic symptom score) and QOL (quality of life) before and after treatment of 3 groups. Results The total effective rate was 72.5% in scalp-acupuncture group, 72.0% in bodyacupuncture group and 85.9% in combined therapy group of scalp acupuncture and body acupuncture. The therapeutic effect in combined therapy group was superior to that in either scalp-acupuncture group or body-acupuncture group (both P〈0. 05). The improvements had been obtained in Qmax, post-void residual volume (PRV), IPSS and QOL after treatment in 3 groups, in which, the improvement in combined therapy group was apparent. Conclusion Either scalp acupuncture or body acupuncture relieves clinical symptoms and improves the quality of life in BPH patients. After treatment, the two groups were compared with each other in terms of treatment result, international prostate symptom score (I-PSS), quality of life index (QLI), maximum flow rate (Qmax), and residual urine volume (RUV). Results: The total effective rate was 96.7% in the treatment group versus 70.0% in the control, and the difference was significant (P0.05). Both groups had marked improvement in I-PSS, QLI score, Qmax and RUV (P0.05). Methods: Clinical literatures about the treatment of MGH with acupuncture published in the recent 16 years were retrieved from Chinese Journal Full-text Database (CJFD) and established into a database by Excel. The SPSS 20 version software and Clementine 12.0 version software were adopted to analyze the frequency and association rules of points-selection in the treatment of MGH with acupuncture. Results: The top 3 points used most frequently in acupuncture treatment of MGH were Danzhon8 (CV 17), Taichong (LR 3) and Zusanli (ST 36); points from the Stomach Meridian of Foot Yangming and Liver Meridian of Foot Jueyin were most commonly used; the commonly selected points were predominantly distributed in thoracic and abdominal regions and lower limbs; emphasis on the combination use of local and distal points; of the specific points, the five Shu-Transmitting points were mostly used; association analysis showed that the associations among Taichong (LR 3), Danzhong (CV 27) and Zusanli (ST 36) were the most significant. Conclusion: The data mining results substantially accord with the general rules of acupuncture-moxibustion theories in traditional Chinese medicine, able to reflect the points-selection principles and features in acupuncture treatment of MGH and provide evidence for the points selection in the treatment of MGH in acupuncture clinic.展开更多

文摘Objective: To explore and analyze the points-selection rules in acupuncture treatment of mammary gland hyperplasia (MGH) by data mining and statistical method. Methods: Clinical literatures about the treatment of MGH with acupuncture published in the recent 16 years were retrieved from Chinese Journal Full-text Database (CJFD) and established into a database by Excel. The SPSS 20 version software and Clementine 12.0 version software were adopted to analyze the frequency and association rules of points-selection in the treatment of MGH with acupuncture. Results: The top 3 points used most frequently in acupuncture treatment of MGH were Danzhon8 (CV 17), Taichong (LR 3) and Zusanli (ST 36); points from the Stomach Meridian of Foot Yangming and Liver Meridian of Foot Jueyin were most commonly used; the commonly selected points were predominantly distributed in thoracic and abdominal regions and lower limbs; emphasis on the combination use of local and distal points; of the specific points, the five Shu-Transmitting points were mostly used; association analysis showed that the associations among Taichong (LR 3), Danzhong (CV 27) and Zusanli (ST 36) were the most significant. Conclusion: The data mining results substantially accord with the general rules of acupuncture-moxibustion theories in traditional Chinese medicine, able to reflect the points-selection principles and features in acupuncture treatment of MGH and provide evidence for the points selection in the treatment of MGH in acupuncture clinic.
