Objective:To analyze the effectiveness of applying ShuGan JieYu Capsules(SGJYC)combined with trazodone in patients with post-stroke depression(PSD)with insomnia.Methods:60 cases of PSD with insomnia patients admitted to the hospital from May 2022 to May 2023 were selected and randomly divided into a reference group(trazodone)and a research group(SGJYC combined with trazodone)of 30 cases each.Statistics were analyzed using the Hamilton Depression Rating Scale(HAM-D),Pittsburgh Sleep Quality Index(PSQI),and Activities of Daily Living(ADL)scale before treatment and 4,8,and 12 weeks after treatment.Results:Before treatment,The results of HAM-D,PSQI,and ADL scale studies in the two groups before treatment were not statistically significant(P>0.05);4,8,and 12 weeks after treatment,the results of HAM-D and PSQI studies in the research group were lower than that of the reference group,and the results of ADL scale studies were higher than that of the reference group.There was a significant difference between the groups(P<0.05).The total adverse reaction rate of the research group was lower than that of the reference group,and there was a significant difference between the groups(P<0.05).Conclusion:The combination of SGJYC and trazodone reduced depression in post-stroke patients,corrected insomnia,improved sleep quality,was safe,and had a low rate of adverse reactions. 展开更多

The Moderating Effect of Organizational Identification on Occupational Stress and Insomnia Among Nurses

Objective:To explore the relationship between organizational identification,occupational stress,and insomnia among nurses and to analyze the moderating role of organizational identification between occupational stress and insomnia.Methods:Through stratified sampling,720 nurses working in a tertiary A-level hospital in Shandong Province from March to May 2021 were surveyed using an effort-reward imbalance questionnaire,an organizational identification questionnaire,and the Athens Insomnia scale.Results:Organizational identification was negatively correlated with occupational stress and insomnia(r_(1)=-0.121,P_(1)<0.01;r_(2)=-0.144,P_(2)<0.01).Occupational stress was positively correlated with insomnia(r=0.379,P<0.01).Organizational identity significantly moderated nurses’occupational stress and insomnia(β=-0.192,t=-2.558,P<0.05).Conclusion:Occupational stress and organizational identification can predict insomnia.Organizational identification could negatively mediate the relationship between occupational stress and insomnia.Improving organizational identification may help alleviate insomnia symptoms by relieving occupational stress. 展开更多

Development and validation of tongue imaging-based radiomics tool for the diagnosis of insomnia degree:a two-center study

Background:Traditional Chinese medicine(TCM)is commonly used for the diagnosis and treatment of insomnia,with tongue diagnosis being particularly important.The aim of our study was to develop and validate a novel tongue imaging-based radiomics(TIR)method for accurately diagnosing insomnia severity.Methods:This two-center analysis prospectively enrolled 399 patients who underwent tongue imaging between July and October 2021 and divided them into primary and validation cohorts by study center.Here,we referred to the Insomnia Severity Index(ISI)standard and the degree of insomnia was evaluated as absent,subthreshold,moderate,or severe.For developed the TIR diagnostic tool,a U-Net algorithm was used to segment tongue images.Subsequently,seven imaging features were selected from the extracted high-throughout radiomics features using the least absolute shrinkage and selection operator algorithm.Then,the final radiomics model was developed in the primary cohort and tested in the independent validation cohort.Finally,we assessed and compared the diagnostic performance differences between TCM tongue diagnosis and our TIR diagnostic tool with the ISI gold standard.The confusion matrix was calculated to evaluate the diagnostic performance.Results:Seven tongue imaging features were selected to build the TIR tool,with showing good correlations with the insomnia degree.The TIR method had an accuracy of 0.798,a macro-average sensitivity of 0.78,a macro-average specificity of 0.906,a weighted-average sensitivity of 0.798,and a weighted specificity of 0.916,showing a significantly better performance compared to the average performance of three experienced TCM physicians(mean accuracy of 0.458,P<0.01).Conclusions:The preliminary study demonstrates the potential application of TIR in the diagnosis of insomnia degree and measurement of sleep health.The integration of quantitative imaging analysis and machine learning algorithms holds promise for advancing both of TCM and precision sleep medicine. 展开更多

Objective This study aimed to reveal the insomnia burden and relevant influencing factors among informal caregivers(ICs)of hospitalized patients with lung cancer.Methods A cross-sectional study on ICs of hospitalized patients with lung cancer was conducted from December 31,2020 to December 31,2021.ICs’burden was assessed using the Caregiver Reaction Assessment(CRA),Hospital Anxiety and Depression Scale(HADS),and Insomnia Severity Index(ISI).Linear and logistic regression models were used to identify the influencing factors.Results Among 289 ICs of hospitalized patients with lung cancer,83(28.72%),53(18.34%),and 14(4.84%)ICs experienced mild,moderate,and severe insomnia,respectively.The scores concerning self-esteem,lack of family support,financial problems,disturbed schedule,and health problems were 4.32±0.53,2.24±0.79,2.84±1.14,3.63±0.77,and 2.44±0.95,respectively.ICs with higher Activities of Daily Living Scale(ADLS)scores were associated with a lower risk of insomnia,with an odd ratio(OR)and 95%confidence interval(CI)of 0.940(0.898–0.983).Among the ICs,female gender(OR=2.597),alcohol consumption(OR=3.745),underlying medical conditions(OR=11.765),long-term caregiving experience(OR=37.037),and higher monthly expenses(OR=5.714)were associated with a high risk of insomnia.Conclusion Of the hospitalized patients with lung cancer,51.9%experienced insomnia.Patients’ADL,ICs gender,alcohol consumption,underlying medical conditions,caregiving duration,and monthly expenses were influencing factors.Therefore,prompt screening and early intervention for ICs of patients with lung cancer is necessary. 展开更多

Background:The Tibetan medicine Sug-Mel-sum-thang(SMST)is often used to treat insomnia disorder(ID);however,the underlying mechanism remains unclear.In this study,network pharmacology prediction was conducted to analyze the molecular mechanisms of SMST in treating ID.Methods:The active ingredients of the three herbs used in SMST were obtained from the Shanghai Institute of Organic Chemistry,Chinese Academy of Sciences.Chemistry Database[DB/OL].The chemical formulae of the active ingredients were downloaded from PubChem in canonical SMILES format.These were then imported into the SwissTargetPrediction platform to identify drug targets.ID targets were obtained from databases such as DrugBank,GeneCards,and DisGeNET.Potential targets of SMST for the treatment of ID were identified using the online Venny mapping platform.The potential target protein-protein interaction was constructed using the STRING platform,and the obtained protein-protein interaction information was imported into Cytoscape 3.7.1 for image optimization and core gene extraction and ClueGO in Cytoscape 3.7.1 was used for the enrichment analysis of potential targets.Cytoscape 3.7.1 was also used to construct the regulatory network of the Tibetan medicine compound target pathway and conduct a topological analysis.Results:The mechanism of action of SMST in the treatment of ID involved 72 compounds,including nerolidol,apigenin,luteolin,and piperine.The key targets identified were COMT,CNR1,AKT1,SLC6A4,TNF,CTNNB1,and CHRNA4.The enrichment analysis obtained 69 KEGG pathways,mainly related to Alzheimer’s disease,cAMP signaling pathway,serotonergic synapse,and other pathways.The core active ingredient molecule docked with the target to a higher degree.The results showed that the active ingredient exhibited good binding activity with the related targets.Some parts with good docking fractions were selected for surface plasmon resonance analysis.Conclusion:Various active components in SMST play a role in the treatment of ID by acting on key targets,such as COMT and CNR1,to regulate multiple signaling pathways. 展开更多

BACKGROUND Sleep breathing,one of the basic human needs,is a physiological need that affects cardiac functions,body temperature,daily vitality,muscle tone,hormone secretion,blood pressure,and many more.In the international literature,studies reported that patients have had sleep problems in the hospital since the 1990s,but no measurement tool has been developed to determine the causes of hospitalacquired insomnia in individuals.These findings suggest that sleep remains in the background compared to activities such as nutrition and breathing.Although patients generally experience hospital-acquired sleep problems,there is no measurement tool to determine hospital-acquired sleep problems.These features show the originality of the research.AIM To develop a measurement tool to determine the sleep problems experienced by patients in the hospital.METHODS A personal information form,hospital-acquired insomnia scale(HAIS),and insomnia severity index(ISI)were used to collect research data.The study population consisted of patients hospitalized in the internal and surgical clinics of a research hospital in Turkey between December 2021 and March 2022.The sample consisted of 64 patients in the pilot application stage and 223 patients in the main application stage.Exploratory factor analysis and confirmatory factor analysis(CFA)analyses were performed using the SPSS 20 package program and the analysis of moment structure(AMOS)package program.Equivalent forms method used.RESULTS The HAIS consisted of 18 items and 5 subscales.The Cronbach alpha values of the subscales ranged between 0.672 and 0.842 and the Cronbach alpha value of the overall scale was 0.783.The scale explained 58.269%of the total variance.The items that constitute the factors were examined in terms of content integrity and named as physical environmental,psychological,safety,socioeconomic,and nutritional factors.CFA analysis of the 5-factor structure was performed in the AMOS package program.The fit indices of the obtained structure were examined.It was determined that the values obtained from the fit indices were sufficient.A significant correlation was determined between the HAIS and the ISI,which was used for the equivalent form method.CONCLUSION The HAIS is a valid and reliable measurement tool for determining patients’level of hospitalacquired insomnia.It is recommended to use this measurement tool to determine the insomnia problems of patients and to adapt it in other countries. 展开更多

Objective:To systematically evaluate the efficacy and safety of the spirit turtle eight method acupuncture therapy in the treatment of insomnia,to better serve the clinic and provide reference ideas for the treatment of insomnia.Methods:Search CNKI,CBM,Wanfang,Weipu,China Clinical Trial Registry databases on the use of the spirit turtle eight method compared with others interventions to treat insomnia.The search period is from the library until August 1,2021,two researchers will conduct independent quality evaluation and effect size extraction on the literature,and use STATA16.0 and Review5.3 to conduct Meta analysis and quality evaluation on the literature.Through screening,a total of 18 eligible documents participated in the meta-analysis,with a total of 1583 cases.Results:①The effective rate of the spirit turtle eight method in the treatment of insomnia is higher than the control group[RR=1.213,95%CI(1.155,1.274),Z=7.71,P<0.01];②The Pittsburgh Sleep Quality Index(PSQI)score is lower than other acupuncture treatments[WMD=-2.33,95%CI(-2.75,-1.91),Z=10.97,P<0.01];③The one-month follow-up PSQI score after the treatment of insomnia by the spirit turtle eight method is lower than the control group[WMD=-3.263,95%CI(-3.446,-3.080),Z=34.96,P<0.01];④No studies have reported serious adverse reactions in the treatment group and control group.Conclusion:Based on the current research,the spirit turtle eight method are better than the control group in the treatment of insomnia,and are suitable for clinical application.However,more high-quality clinical studies are still needed to prove its efficacy. 展开更多

Background:Professor Yan-Xin Wang has been committed to the use of traditional Chinese medicine formulas to treat insomnia from the liver for many years,and has achieved excellent clinical results.In order to better inherit Yan-Xin Wang’s academic thoughts.The purpose of this study is to use clinical data to explore the clinical experience of Prof.Yan-Xin Wang in the application of Chinese medicine to treat insomnia patients from the liver,explore the compatibility and medication rules of traditional Chinese medicine,and give more clinical treatment ideas for insomnia.Methods:The general data and prescription information of insomnia patients treated with Chinese herbal medicine by Prof.Yan-Xin Wang from January 1,2021,to December 31,2021,were summarized according to the inclusion and exclusion criteria,and the data were subjected to frequency statistics and drug association rules,complex network diagram analysis and cluster analysis.Results:A total of 159 patients and prescriptions were included in the study,of which 81.1%were women and 18.9%were men,containing 128 herbs;the highest frequency of use was 91.8%for Bupleuri Radix.Six Chinese herbs were used more than 70%of the time,namely Bupleuri Radix,Scutellariae Radix,oyster shell,Glycyrrhizae Radix,Os Draconis,and Ziziphi Spinosae Semen.The top 20 herbs in terms of frequency of use were analyzed in terms of the four Qi,five flavours,and their attributions.The four Qi were mainly calm and warm,the five flavours were mainly bitter and acrid,followed by sweet,and the attributions were mainly to the liver,spleen,and heart meridians.The Chinese medicine association rules set the confidence level>80%and the support level>10%,resulting in 10 two-herb and three-herb associations with the highest confidence level,such as Os Draconis is associated with oyster shell,Platycodonis Radix is associated with Achyranthis Bidentatae Radix,Scutellariae Radix is associated with Bupleuri Radix,Os Draconis,Bupleuri Radix is associated with oyster shell,Os Draconis,Scutellariae Radix is associated with oyster shell,etc.Cluster analysis yielded 3 classes of drug formulas.The complex network diagram shows that the core prescription drugs are composed of Bupleuri Radix,Chuanxiong Rhizoma,Pseudostellariae Radix,Jujubae Fructus,Os Draconis,Coptidis Rhizoma,Scutellariae Radix,Ziziphi Spinosae Semen,Smilacis Glabrae Rhizoma,Cinnamomi Cortex,White Moutan Cortex,Atractylodis Rhizoma,Glycyrrhizae Radix,oyster shell,Rehmanniae Radix Praeparata,Tritici Levis Fructus,Pinelliae Rhizoma Praeparatum,and Cinnamomi Ramulus.Conclusion:Prof.Yan-Xin Wang believes that the main treatment for patients with insomnia is based on the liver,by tonifying the deficiency and supporting the righteousness,mutually regulating the liver and spleen,and calming the mind and nourishing the heart,while adding and subtracting appropriate herbs according to the patient’s co-morbidities,which can significantly improve the patient’s insomnia symptoms. 展开更多

Objectives:The present study aims to assess the prevalence and predictors of smartphone addiction and insomnia among nurses working in the outpatient department(OPD)after the second wave of the coronavirus disease 2019(COVID-19)pandemic.Materials and Methods:A descriptive,cross-sectional study was carried out among 117 OPD nurses between October and December 2021 using a purposive sampling technique.Two self-reported standardized scales,the Smartphone Addiction Scale-ShortVersion and Insomnia Severity Index were used.Kolmogorov–Smirnov test,Mann–Whitney U,and Kruskal–Wallis Htest were used.Pearson’s correlation and Scatter plot were used to determine the relationship between the study variables.A stepwise multiple linear regression analysis was also performed.Results:The majority of participants had slight smartphone addiction(78.6%)and suffered from sub-threshold to severe forms of insomnia(73.5%).A significant mild positive correlation was found between smartphone addiction and insomnia(r=0.195,P<0.05).Stepwise multivariate logistic regression analysis predicted factors such as female gender and exposure to smartphones for more than 5 years influencing smartphone addiction.A strong influence of exposure to the smartphone for more than 5 years was found on insomnia severity.Conclusion:Smartphone addiction and insomnia were identified problems among nurses working in the OPD after the second wave of the COVID-19 pandemic,requiring an urgent need to identify and manage various factors responsible for smartphone addiction and insomnia such as female gender and years of exposure to smartphones. 展开更多

BACKGROUND Insomnia is a disease where individuals cannot maintain a steady and stable sleep state or fail to fall asleep.Western medicine mainly uses sedatives and hypnotic drugs to treat insomnia,and long-term use is prone to drug resistance and other adverse reactions.Acupuncture has a good curative effect and unique advantages in the treatment of insomnia.AIM To explore the molecular mechanism of acupuncture at Back-Shu point for the treatment of insomnia.METHODS We first prepared a rat model of insomnia,and then carried out acupuncture for 7 consecutive days.After treatment,the sleep time and general behavior of the rats were determined.The Morris water maze test was used to assess the learning ability and spatial memory ability of the rats.The expression levels of inflammatory cytokines in serum and the hippocampus were detected by ELISA.qRTPCR was used to detect the mRNA expression changes in the ERK/NF-κB signaling pathway.Western blot and immunohistochemistry were carried out to evaluate the protein expression levels of RAF-1,MEK-2,ERK1/2 and NF-κB.RESULTS Acupuncture can prolong sleep duration,and improve mental state,activity,diet volume,learning ability and spatial memory.In addition,acupuncture increased the release of 1L-1β,1L-6 and TNF-αin serum and the hippocampus and inhibited the mRNA and protein expression of the ERK/NF-κB signaling pathway.CONCLUSION These findings suggest that acupuncture at Back-Shu point can inhibit the ERK/NF-κB signaling pathway and treat insomnia by increasing the release of inflammatory cytokines in the hippocampus. 展开更多

For the conventional nursing group, routine standard nursing mode was adopted after enterostomy, while extra systematic “3H” nursing service on the basis of conventional nursing management was applied for its counterpart. SAS and SDS scores, Pittsburgh sleep quality index, Barthel index and nursing satisfaction were compared between the two groups. Results: SAS and SDS scores, Pittsburgh sleep quality index, Barthel index and nursing satisfaction of “3H” nursing mode group were evidently better than those of conventional nursing management group, and the difference was significant (P Conclusion: The “3H” nursing mode can effectively improve negative emotions of patients with enterostomy, such as anxiety and depression, improve their sleep quality and self-care ability in daily life, and play an important role in building a harmonious relationship between doctors and patients, nurses and patients, which is worthy of clinical application. 展开更多

Mobile phone addiction is a growing concern among college students, with significant implications for their mental health. This study examines the complex relationship between mobile phone addiction and its impact on college students’ insomnia, symptoms of depression, and anxiety. A sample of 399 college students from traditional Chinese medicine colleges in Guangxi, China, completed the Mobile Phone Addiction Tendency Scale questionnaire. Among them, 155 students with high Mobile Phone Addiction Tendency scores were selected for the correlation study. The analysis revealed a strong association between mobile phone addiction and insomnia. Furthermore, mobile phone addiction was significantly associated with symptoms of depression, including feelings of sadness, worthlessness, and hopelessness, as well as heightened anxiety symptoms, such as excessive worry and restlessness. These findings emphasized the importance of targeted interventions and preventive measures to address the negative impact of mobile phone addiction on college students’ mental health. 展开更多

Background:As a disease,insomnia is often ignored by the general public.Insomnia,which not directly fatal,may be equivalent to chronic suicide if it is not paid attention to.However,there are many theories on how to treat insomnia,and researchers have been searching for a cure.Chinese herbal medicine(CHM)and exercise therapy(ET)are relatively effective,and have been used since ancient times to treat insomnia.This study aimed to examine the effect of CHM combined with ET(CHM-ET)on patients with insomnia.Methods:We searched eight electronic databases including PubMed,Web of Science,EMBASE,the Cochrane Library,the China National Knowledge Infrastructure(CNKI),the China Biology Medicine disc(CBMdisc),the China Science and Technology Journal Database(VIP),the Wanfang Database to find randomized controlled trials(RCTs)evaluating CHM-ET in the treatment of insomnia patients up to September 13,2022.Two researchers read and screened the publications to extract data.We used the pittsburgh sleep quality index(PSQI)as the evaluation indicators for each study,and the other was the total effective rate(TER).The Cochrane risk-of-bias assessment tool was used to assess the risk of bias of the included literature.The level of evidence for this result was assessed by GARDE method.The meta-analysis was performed using STATA 14 software and RevMan 5.3.The research method was registered with PROSPERO(CRD42022350926).Results:We included fourteen randomized controlled trials,which including a total of 1,126 participants.Compared with the control group,the experimental group showed significantly improvements in sleep effect as reflected by the reduced PSQI score[mean difference(MD)=-2.47,95%confidence interval(CI)(-3.15,-1.78),I^(2)=92%]with low quality of evidence,and increased TER[risk ratio(RR)=1.23,95%CI(1.14,1.33),I^(2)=40%]with moderate quality of evidence.Compared with hypnotic drugs,CHM-ET significantly reduced the PSQI score[mean difference(MD)=-3.18,95%CI(-5.48,-0.89),I^(2)=73%]with low quality of evidence.The PSQI of CHM-ET significantly decreased compared with single CHM[mean difference(MD)=-3.04,95%CI(-5.84,-0.25),I^(2)=98%]with low quality of evidence,and ET[mean difference(MD)=-2.44,95%CI(-2.87,-2.02),I^(2)=0%]with moderate quality of evidence.No serious adverse reactions were observed.Conclusion:This review suggested that CHM-ET may be an effective treatment for insomnia.However,given the limited quality of the studies and methodologies included in the trials,further rigorous randomized controlled trials are needed for more accurate results. 展开更多

Objective: To observe the effect of insomnia patients with deficiency of heart and spleen in Germany by the therapy of ear points pressing with bean. Methods: A total of 80 cases of insomnia with deficiency of heart and spleen in Germany were divided into the observation group and the control group, with 40 cases in each group. The patients in the control group were treated with oral modified Guipi Decoction(归脾汤), while the patients in the observation group were treated with oral traditional Chinese medicine(TCM) and the therapy of ear points pressing with bean. The efficacy, sleep quality, the TCM syndromes score and the incidence of adverse reactions were compared between the two groups. Results: The effective rate in the observation group was higher than that in the control group, and there was statistical difference between the two group(P<0.05). After treatment, the Pittsburgh sleep quality index scale(PSQI) score, insomnia severity index(ISI) score and the TCM syndromes score in the two groups were decreased sharply(P<0.01), and those scores in the observation group were lower than that in the control group(P<0.05). There was no adverse reaction between the two groups. Conclusion: The therapy of ear points pressing with bean is effective on insomnia in Germany, which is worth further study and promotion. 展开更多

Meta-analysis of randomized controlled trials of acupuncture in the treatment of menopausal insomnia

Objective:We aimed to investigate the safety and efficacy of acupuncture in the treatment of climacteric insomnia compared with western medicine through a comprehensive metaanalysis.Methods:Literature retrieval,research selection and data extraction were done independently and in duplicate.Effect-size estimates are expressed as risk ratio(RR)and 95%confidence interval(CI).Results:Summary data from 27 articles,involving a total of 2349 menopausal women,were meta-analyzed.The overall analysis showed that the clinical cure rate(RR=1.85,95%CI:1.57~2.18,P<0.001)of acupuncture was 1.85 times higher than western medicine,and the markedly effective rate was 1.3 times higher than that of westernmedicine(RR=1.30,95%CI:1.09~1.54,P=0.003).In terms of Pittsburgh Sleep quality Index((PSQI)),simple acupuncture is superior to western medicine in sleep quality,falling asleep time,sleep time,sleep efficiency and daytime dysfunction.In the evaluation of adverse reactions,the incidence of adverse events in the acupuncture-related therapy group(3.5%)was lower than that in the western medicine group(12.5%).Conclusion:The results of metaanalysis show that the clinical efficacy of acupuncture in the treatment of climacteric insomnia is better than that of western medicine which has better efficacy and safety.The acupuncture therapy is suitable for clinical. 展开更多

Ziziphi Spinosae Semen(ZSS),a traditional Chinese medicine,is used in clinics for the treatment of insomnia in China and other Asian countries.Herein,we described for the first time a comparative pharmacokinetics study of the six major compounds of ZSS in normal control(NC)and para-chlorophenylalanine(PCPA)-induced insomnia model(IM)rats that were orally administered the aqueous extract of ZSS.An ultra-high-performance liquid chromatography coupled with quadrupole orbitrap mass(UHPLC-Q-Orbitrap-MS)method was developed and validated for the simultaneous determination of coclaurine,magnoflorine,spinosin,6000-feruloylspinosin,jujuboside A(JuA),and jujuboside B(JuB)in ZSS in rat plasma.The established approach was successfully applied to a comparative pharmacokinetic study.The systemic exposures of spinosin and 6000-feruloylspinosin were decreased in the IM group compared to the NC group,while plasma clearance(CL)was significantly increased.The Tmax values of JuA and JuB in IM rats were significantly lower than those in NC rats.The T1/2 of JuA in the IM group was significantly accelerated.The pharmacokinetic parameters of coclaurine and magnoflorine were not evidently affected between the two groups.These results indicate that the pathological state of insomnia altered the plasma pharmacokinetics of spinosin,6000-feruloylspinosin,JuA,and JuB in the ZSS aqueous extract,providing an experimental basis for the role of ZSS in insomnia treatment.The comparative pharmacokinetics-based UHPLC-Q-Orbitrap-MS using full-scan mode can therefore provide a reliable and suitable means for the screening of potentially effective substances applied as quality markers of ZSS. 展开更多

Objective To determine the factors influencing insomnia and construct early insomnia warning tools for rescuers to informbest practices for early screening and intervention.Methods Cluster sampling was used to conduct a cross-sectional survey of 1,133 rescuers from one unit in Beijing,China.Logistic regression modeling and R software were used to analyze insomniarelated factors and construct a PRISM model,respectively.Results The positive rate of insomnia among rescuers was 2.74%.Accounting for participants’age,education,systolic pressure,smoking,per capita family monthly income,psychological resilience,and cognitive emotion regulation,logistic regression analysis revealed that,compared with families with an average monthly income less than 3,000 yuan,the odds ratio(OR)values and the[95%confidence interval(CI)]for participants of the following categories were as follows:average monthly family income greater than 5,000 yuan:2.998(1.307–6.879),smoking:4.124(1.954–8.706),and psychological resilience:0.960(0.933–0.988).The ROC curve area of the PRISM model(AUC)=0.7650,specificity=0.7169,and sensitivity=0.7419.Conclusion Insomnia was related to the participants’per capita family monthly income,smoking habits,and psychological resilience on rescue workers.The PRISM model’s good diagnostic value advises its use to screen rescuer early sleep quality.Further,advisable interventions to optimize sleep quality and battle effectiveness include psychological resilience training and smoking cessation. 展开更多

The underlying factors for the disorder are physiological changes of the sleep-wake cycle, comorbid diseases and drug treatment. Cognitive behavioural therapy (CBT I) combined with light therapy and physical activity can be effective in the treatment of comorbid insomnia in older adults. Methods: Sixty-three (63) insomnia patients (47 female, 16 male) with an average age of 66.6 years participated in the program. Before and after the treatment, the participants completed questionnaires to assess their sleep quality (PSQI), day-time sleepiness (ESS), mood (BDI) and well-being (WHO-5-Index). In addition, they kept sleep diaries for six weeks. Results: Pre-post comparisons revealed a significant improvement in sleep latency and sleep quality, as well as a significant reduction of day-time sleepiness. In patients with both insomnia and depression, all depression scores improved slightly but significantly. Conclusions: Non-pharmacological combination treatment using CBT I, light therapy and physical activity seemed to be effective in older adults with comorbid insomnia and improve additionaly depressive symptoms. 展开更多
