BACKGROUND Digital intraoral scanning,although developing rapidly,is rarely used in occlusal reconstruction.To compensate for the technical drawbacks of current occlusal reconstruction techniques,such as time consumption and high technical requirements,digital intraoral scanning can be used in clinics.This report aims to provide a way of selecting the most suitable maxillo-mandibular relationship(MMR)during recovery.CASE SUMMARY A 68-year-old man with severely worn posterior teeth underwent occlusal reconstruction with fixed prosthesis using digital intraoral scanning.A series of digital models in different stages of treatment were obtained,subsequently compared,and selected using digital intraoral scanning together with traditional measurements,such as cone beam computed tomography,joint imaging,and clinical examination.Using digital intraoral scanning,the MMR in different stages of treatment was accurately recorded,which provided feasibility for deciding the best occlusal reconstruction treatment,made the treatment process easier,and improved patient satisfaction.CONCLUSION This case report highlights the clarity,recordability,repeatability,and selectivity of digital intraoral scanning to replicate and transfer the MMR during occlusal reconstruction,expanding new perspectives for its design,fabrication,and postoperative evaluation. 展开更多

A new method for intraoral tissue defect repair:diepithelialized tissue flap with vascular anastomosis

Diepithelialized tissue flap(DTF)with vascular anastomosis was de-signed in August 1987,and was used for the repair of 12 cases of large intraoraltissue defect.All the operations were successful.The clinical data were reportedand the procedure to incise and transplant a DTF described.When the healingprocess and changes of DTF after transplantation were observed,it was foundthat a layer of new smooth epidermis was formed on the surface of the DTF.Theorigin of this new epidermis was discussed.Ten out of the 12 cases have been fol-lowed up for from half a year to 2 and a half years,and the results weresatisfactory.The new epidermis showed no hair growth and the patients did nothave any rough feeling,which usually occurred after ordinary skin grafting.It isbelieved that DTF transplantation is likely an ideal method for the repair andreconstruction of intraoral soft tissue defect.Its disadvantage was that the DTFcontracted severely after healing.Finally the authors put forward the problemsconcerning the transplantation of DTF,which should be investigated further. 展开更多

Several capture techniques are used in intraoral optical scanners in the dental market,such as Triangulation(Cerec Omnicam,Dentsply Sirona),Activewave front sampling(3M ESPE)and confocal technology(iTero,Align).The accuracy of intraoral scanners is the most significant focal point for developers to research.This in-vitro study studied the accuracy of confocal scanners launched from 2015-2020(Trios 3,Trios 4,iTero Element;3Shape Trios A/S,Copenhagen,Denmark,and iTero Element2,and iTero Element5D;Align Technologies,San Jose,CA,USA).A 3D printing model modified from the American National Standard No.132 was scanned five times each scanner.Both Trios3 and Trios4 were scanned using regular scan mode(N)and high-resolution mode(HR).All scanning methods followed the recommendations from the manufacturers.Then the digital models were exported and saved as STL files.Various measurements were determined in the digital model from each scan using Rhinoceros 3D Software(Rhino,Robert McNeel&Associates for Windows,Washington DC,USA).Measurements from the 3D printed model were used as control.All data were recorded in Microsoft Excel and then transferred to SPSS.Descriptive statistics were recorded.Multiple comparisons of various measurements were made among the different scanners and with the control using One-way ANOVA and post hoc using Sheffe(p<0.01).The surface area in the X and Y axis ranged from 2–60 mm,while the depth(Z-axis)ranged from 2–8 mm.The Trios and iTero families showed similar accuracy.However,for the diagonal,the Trios series showed better results compared with the iTero series.Within the same brand,different versions showed no significant change regarding accuracy. 展开更多

Dose Distributions in Simulated Electron Radiotherapy with Intraoral Cones Using Treatment Planning System

Aim: This study aims to evaluate the difference between depth data from an intraoral cone and a conventional irradiation tube calculated using a treatment planning system (TPS), and that measured using an intraoral cone for electron radiotherapy. Background: A TPS is only compatible with conventional irradiation tubes. However, such systems are not suitable for determining dose distributions when a special cone is employed. Materials and Methods: Dose distributions were calculated using the beam data for mounted intraoral cones using a TPS. Then, the dose distribution by field size was calculated for a low-melting-point lead alloy using the beam data for a mounted conventional tube. The calculated data were evaluated against the measured intraoral-cone depth data based on the dose and depth differences. Results: The calculated data for the intraoral cone case did not match the measured data. However, the depth data obtained considering the field size determined for the lead alloy using the conventional tube were close to the measured values for the intraoral cone case. The difference in the depth at which the absorbed dose was 50% of the maximum value of the percentage depth dose was less than ±4 mm for the generalized Gaussian pencil beam convolution algorithm and less than ±1 mm for the electron Monte Carlo algorithm. Conclusion: It was found that the measured and calculated dose distributions were in agreement, especially when then electron Monte Carlo algorithm was used. Thus, the TPS can be employed to determine dose distributions for intraoral cone applications. 展开更多

Aim: This study was performed to evaluate the movement of the proximal segment following different methods of ramus osteotomy that is one of the side effects of orthognathic surgery. Theoretically, with intraoral verticosagittal ramus osteotomy, it can minimize the movement of the proximal segment. The changes in the intergonal distance of mandible and the angle of the ramus flaring in two methods of osteotomy have been compared in this study. Materials and Methods: In this randomized clinical trial, included 60 patients (32 males) with mandibular prognathism and without any asymmetry were selected and divided into two groups (n = 30). One group underwent bilateral sagittal split ramus osteotomy technique to achieve mandibular setback and the other by the intraoral verticosagittal ramus osteotomy technique. Intergonial width and inner-ramal angle in the transverse plane were measured on radiographs preoperatively and 1 and 12 weeks postoperatively. Data were analyzed using covariance test with the significance level set at P 0.1). Conclusion: Considering our findings, there was no significant difference between two ramus osteotomy techniques regarding changes in mandibular width and inter-ramal flaring angle. 展开更多

Victim identification through dental features is one of the main objectives of forensic dentistry.In circumstances where information regarding antemortem dental records is missing,reconstruction of a biological profile can be useful as a first step toward personal identification.This reconstructive method provides valuable information,namely regarding the individual’s ancestry,through the detection and degree of expression of dental morphological traits,which may help to restrict the number of candidates for identification.Technological advances allowed the development of alternative methods for dental evaluation,that complement or substitute those already in use in clinical practice.Among these,intraoral three‑dimensional(3D)images are increasingly used in dentistry,as they have a high level of accuracy and are easy to obtain and store.However,a fundamental question regarding forensic dentistry is whether they allow recognition and analysis of dental morphological traits in detail,namely those related to ancestry.In this study,we evaluated 20 teeth morphological features using intraoral 3D imaging from 77 individuals from Northern Portugal.Our results showed that it was possible to identify and classify a large part of the main morphological traits used in the estimation of ancestry.As these 3D images present sufficient morphological detail to be classified,we believe that future applications of this technique can be expected in forensic dentistry. 展开更多

Many new types of toothpaste are commercially manufactured with diverse fragrances, colors, probiotics, and pharmaceutical ingredients to enhance the functionalities of toothpaste. Our study attempted to create a toothpaste formulation that might facilitate the intraoral delivery of vitamin D3 into the bloodstream. Simply brushing our teeth with toothpaste should be easy to take the essential vitamin regularly. In this study, an emulsion-based toothpaste mixed with an azone compound and sodium dodecyl sulfate as penetration enhancers blended thoroughly with other ingredients and then with vitamin D. Multiple toothpaste characteristic tests were performed, such as abrasiveness, scratchiness, spreadability, pH, foaming, cleaning, and antibacterial strength with our vitamin D toothpaste, and compared with those of other commercial brand toothpaste. To confirm the intraoral delivery of vitamin D through the oral cavity, an earthworm transport study and TEER value test were conducted using L. terrestris skin. Our data demonstrated the high feasibility of intraoral delivery of vitamin D based on those two skin studies with various experimental support;our vitamin D toothpaste had comparable characteristics with other commercial toothpaste for cleaning functionality. 展开更多

Replacement of Unesthetic Posterior Metal Crowns with Monolithic Zirconia Crowns: A Case Report

Advances in metal-free materials and the popularization of Computer-Aided Design and Manufacturing (CAD/CAM) have led to the wide clinical use of all-ceramic crowns for esthetic restorations. A 72-year-old woman presented to our hospital with unesthetic restorations on the right upper and lower posterior teeth. Intraoral examination revealed poorly fitting metal crown margins. Defective prostheses were removed, and provisional restorations were provided to stabilize the mandibular position. Optical impressions and the maxillomandibular relationship were recorded using an intraoral scanner, and monolithic zirconia crowns were fabricated using CAD/CAM technology for complete veneer crown restorative treatment. Occlusal examination revealed an improvement in occlusal force distribution at initial examination (right side: 33.5%, left side: 66.5%) after placement of the zirconia crowns (right side: 54.9%, left side: 45.1%). Occlusal force and occlusal force distribution area also showed an increasing trend. The Oral Health Impact Profile short form (OHIP-14) score decreased from 7 points at initial examination to 0 points after prosthodontic treatment. Appropriate diagnosis and treatment planning contributed to the increased occlusal force and balanced occlusal force distribution. Therefore, the present case indicates the potential of monolithic zirconia crowns to achieve both esthetic and stable functional outcomes. 展开更多

Changing from analogue to digital radiography has not only made the process simpler and faster but also made image storage, manipulation(brightness/contrast, image cropping, etc.) and retrieval easier. The three-dimensional imaging has made the complex cranio-facial structures more accessible for examination and early and accurate diagnosis of deep seated lesions. Recently, the interest in oral malodor has been grown and the number of people having trouble with oral malodor is increasing year by year. Generally, as the branch of medicine which deals with patients complaining about oral malodor, dentistry is mostly chosen. On the current situation, however, the dental office side typically finds it difficult to deal with several kinds of oral malodor which show no improvement even though cavities or periodontal disease is completely cured. Main components to cause oral malodor are volatile sulfur compounds (methyl mercaptan, dimethyl sulfide, hydrogen sulfide);in the present study, therefore, the effect of mouthwash solution containing chlorine dioxide to relieve oral malodor and increase saliva production was tested. The subjects were 92 patients (Control group: 2 males, 3 females, age;30.8 ± 4 years old, Mouthwash group: 28 males, 59 females, age: 36.9 ± 1.3 years old) who visited the hospital complaining about oral malodor. Intraoral gas, exhaled gas, the saliva production at rest, the ability for salivation and the buffering ability of saliva were examined twice, before the subjects used the mouthwash solution every day for one month. The simple chromatography oral chroma, which was insulated from the influence of temperature and humidity and also was able to measure with a high degree of accuracy, was used to measure odor as the device analysis method. The control group did not change in VSCs, saliva production and the buffering ability of saliva. After using mouthwash solution, the concentration of the 3 major components gas of intraoral gas and exhaled gas, i.e. methyl mercaptan, dimethyl sulfide, hydrogen sulfide, were all decreased. As for hydrogen sulfide, intraoral gas (from 225.8 ± 28.1 ppb to 41.1 ± 8.8 ppb) and exhaled gas (from 212.0 ± 50.4 ppb to 34.6 ± 16.6 ppb) was significantly decreased. Also, saliva production at rest (from 1.3 ± 0.1 ml to 1.7 ± 0.1 ml) and the ability to salivate (from 4.5 ± 0.3 ml to 5.1 ± 0.3 ml) were both significantly increased. As for the buffering ability of saliva, significant changes were not detected on the change of salivary pH after using mouthwash solution, and it did not have any effects on the buffering ability of saliva. In conclusion, these results suggest that the mouthwash solution containing chlorine dioxide has significant effects on inhibiting malodor. 展开更多

The American Association of Orthodontists recently issued a consumer alert, warning of "a substantial risk for irreparable damage" from a new trend called "do-it-yourself" orthodontics, consisting of patients autonomously using elastics to correct tooth position. The elastics can work their way below the gums and around the roots of the teeth, causing damage to the periodontium and even resulting in tooth loss. The cost of implants to replace these teeth would well exceed the cost of proper orthodontic care. This damage could also occur in a dental office, when a general dentist tries to perform a simplified orthodontic correction of a minor tooth malposition. The present case report describes a case of tooth loss caused by a displaced intraoral elastic, which occurred during a simple preprosthetic orthodontic treatment. 展开更多

BACKGROUND Digital radiography has recently been used in dentistry as a substitute for conventional film radiography worldwide.Digital imaging has many advantages and provides new possibilities for recording and interpreting radiographic data.This system uses different types of digital receptors.AIM To detect the frequency,type,and reasons behind the appearance of intraoral image artifacts acquired by photostimulable phosphor plates(PSP).METHODS This retrospective descriptive study was conducted in the oral and maxillofacial radiology unit of the dental clinics of the College of Dentistry,Princess Nourah University(PNU).All intraoral digital radiographs were acquired using(Gendex Expert DC.,United States)an intraoral X-ray machine with 7-mA,65-kVP using a PSP system(Soredex DIGORA Optime imaging plate)and laser scanners(Soredex DIGORA Optime),which can house all sizes of reusable intraoral PSP sensor plates with image acquisition software(MIPACS Dental Enterprise viewer 3.2.2).A total of 50000 intraoral radiographs were retrieved from the clinical database from April 2018 to April 2020 to evaluate the reason,type,and solutions to these image artifacts.RESULTS Overall,50000 intraoral digital radiographs were acquired in a two-year-period;that is,from April 2018 to April 2020.Of these,3550(7.1%)retakes were performed due to the presence of image artifacts.Periapical radiographs were the most common image type of intraoral retakes(80.8%).Imaging artifacts were divided into three categories:operator,plate and scanning errors.Out of 3550 retakes,5%,1.37%,and 0.73%were related to operator,plate,and scanning errors,respectively.The cone cut was the most common operator error(988 images),Bite marks were the most common plate error(276 images),and delayed scanning artifacts were the most common scanning errors(145 images).The calculated kappa value for interobserver reliability was 0.99,indicating almost perfect interobserver agreement.CONCLUSION Our study discussed intraoral image artifacts that were characteristic of PSP,where the most common artifacts were bitemarks,image size reduction,scratches,and delayed scanning. 展开更多

On examination a mass was seen on the floor of the mouth displacing the tongue superiorly. Diffuse fullness was noted in the submental region. The lesion was excised under general anaesthesia with nasotracheal intubation. The entire mass was removed intra orally. The histological features were suggestive of epidermoid cyst. Salivary gland tumors represent 2% - 6.5% of all head and neck tumors. Since salivary gland tumors have various clinical features and histological types, it is often difficult to diagnose and treat them. The aim of this study was to conduct a retrospective clinical-statistical analysis of 37 minor salivary gland tumors (MSGTs) treated in the Department of Oral Surgery at Nihon University School of Dentistry at Matsudo over a 16-year period. The frequencies and distributions of sex, age, occurrence site, preoperative examination (fine needle cytology and/or biopsy), treatment, and prognosis of the tumors were analyzed and compared with previous reports. The average age at diagnosis was 58.1 years (range 22 - 91 years). The peak occurrence of tumors was in the sixties (10 cases, 27.0%), followed by the forties (8 cases, 21.6%) and the seventies (7 cases, 18.9%). The average age of patients with benign tumors was 56.8 years (range 22 - 91 years). For malignant tumors, the average age was 61.7 years (range 42 - 81 years). The male-female ratio was 1:2.1 for all minor salivary gland tumors, 1:2.4 for benign tumors, and 1.5:1 for malignant tumors. There were 27 (73.0%) benign and 10 (27.0%) malignant tumors. Pleomorphic adenoma (PA) was the most common tumor (24 (64.9%) cases). Most salivary gland tumors originated from the palate (21 cases, 56.8%);the second most common site was the buccal mucosa (14 cases, 37.8%). For most patients, an intraoral mass was the primary presentation, and the second most common symptom was swelling. Surgical treatment was performed for all cases, both benign and malignant tumors. There was one case of local recurrence of PA, as well as one of multiple lung metastases after surgery for adenoid cystic carcinoma. Diagnosis and treatment of MSGTs are often delayed because the patients have few symptoms, and it is difficult to distinguish benign from malignant tumors. More cases need to be examined in the future, and the diagnostic accuracy of imaging and histopathological diagnosis needs to be improved. 展开更多

Pigmentation of the oral mucosa is relatively common and has a wide variety of etiologies. Although most pigmented lesions of the oral mucosa are associated with deposition of melanin and accidental displacement of a dental alloy, accurate differential diagnosis of a pigmented lesion is important, especially in the case of malignant melanoma. We report two cases of oral mucosal pigmentation associated with accidental displacement of a dental alloy in which malignant melanoma was suspected. Excisional biopsy was carried out in these cases with the incision line set at approximately 5 mm from the lesions. Histopathologically, brownish foreign substances were observed in the lamina propria. Metal quantitative analyses of the extracted specimens were carried out by electron probe microanalysis (EPMA). The metal components and the mass concentration revealed that the metals were derived from a dental casting silver alloy in Case 1 and from a gold-silver palladium alloy in Case 2. Although exogenous pigmentation originating from a dental alloy is not rare, differential diagnosis of oral pigmented lesions is sometimes very difficult. In such cases, histopathological examination may be necessary for the diagnosis to exclude melanocytic lesions and EPMA may be effective to identify the causative dental alloy. 展开更多

Effect of Right Supine Tracheal Extubation on Respiratory Complications and Airway Pressure in the Waking Period of General Anesthesia in Pediatric Stomatology

<b>Objective:</b> The purpose of this study was to investigate the effect of right supine endotracheal intubation on respiratory complications and airway pressure of general anesthesia, and to provide guid... <b>Objective:</b> The purpose of this study was to investigate the effect of right supine endotracheal intubation on respiratory complications and airway pressure of general anesthesia, and to provide guidance for clinical application. <b>Methods:</b> Seventy-two children who received oral treatment under general anesthesia from November 2020 to November 2021 in Yantai Stomatological Hospital were randomly divided into three groups, 24 cases in each group. All three groups of children entered the PACU after the surgery. The children in Group I were extubated in the supine position, the children in Group II were immediately changed to the right decubitus after extubation in the horizontal position, and the children in Group III were extubated in the right decubitus. HR, MAP and SpO<sub>2</sub> of T1 (the time point at the beginning of surgery), T2 (the time point at 1 hour after surgery), T3 (the time point after extubation), T4 (the time point at 1 minute after extubation), T5 (the time point at 3 minutes after extubation) in the three groups were observed, t1 (operation time) and t2 (the time of leaving the PACU) were recorded. The airway pressure (P1) in the recumbent position and the airway pressure (P2) in the right decubitus position before extubation were recorded in Group III. The number of sputum suction and complications after extubation were counted. <b>Results:</b> The t2 in Group III was shorter than that in Groups I and II, and the number of sputum suction in Group III was less than that in Groups I and II (P < 0.05). The HR at T3, T4 and T5 in Group III was lower than that in Group I, and the HR at T4 and T5 was lower than that in Group II (P < 0.05). There were significant differences in the incidence of respiratory complications among the three groups (P < 0.05). The incidence of asphyxia, bucking and glossocoma postoperative agitation in Group III was significantly lower than that in Group I, and the incidence of asphyxia and choking was lower than that in Group II (P < 0.05). The incidence of glossoptosis in Group II was significantly lower than that in Group I (P < 0.05). In Group III, the airway pressure P2 in the right decubitus position during endotracheal intubation was higher than that of P1 in the supine position during endotracheal intubation (P < 0.05). The 95% Confidence Interval (CI) of airway pressure difference was 1.416 - 1.834 cmH<sub>2</sub>O. <b>Conclusion:</b> For children undergoing intraoral therapy under general anesthesia, tracheal extubation in the right decubitus position can improve the circulation fluctuation before and after extubation, reducing the number of sputum suction and respiratory tract-related complications, and can shorten the departure time. The body position change during the tracheal intubation will slightly increase the airway pressure, but the supine position after extubation can better ensure the smooth spontaneous breathing of children, which can provide the reference for clinical application. 展开更多

The palate appears to be the most commonly involved site. The most frequent benign tumor is the pleomorphic adenoma. Case report: We report a case of a pleomorphic adenoma, which occurred in the hard and soft palate of a 34-year-old female and presented as an asymptomatic slow growing, painless mass. The patient was successfully treated with surgical excision. Discussion: Minor salivary gland tumors should be considered in the differential diagnosis of oral cavity and especially palate tumors. Purpose:Odontogenic maxillary sinusitis(OMS)is frequently encountered in otorhinolaryngologists'clinical practice.Endoscopic sinus surgery(ESS)instead of surgeries in intraoral approach has been widely applied among OMS.However appropriate treatments due to the causes of the OMS as well as the outcome have been less investigated,meanwhile the inherent incidence of OMS may be still on the rise.This study was designed to conclude our systematic treatment within follow-up examination.Materials and methods:In this retrospective study patients confirmed diagnosis of OMS who had systematic follow-up examinations were analyzed.Medical histories of otorhinolaryngologists and dentists were reviewed as well as preoperative examination protocols.Result:Consecutive 29 Patients(10 women,19 men)were included.41.4%(12/29)patients experienced facial pain as the most frequent symptom.69.0%(20/29)patients admitted firstly to otorhinolaryngologists,13 patients were treated with surgical procedure while 7 patients lacking of surgical indication were transferred to dentists.31.0%(9/29)patients were advised by dentist for being suspected of maxillary sinusitis,including 7 patients suffered from sinonasal complications of dental treatment(SCDT).Conventional dental treatment(root planning,root end surgery,extraction)was the most common cause.Follow up for a mean of 15.1(ranges from 6 to 96)months showed 29 patients maintaining open maxillary ostium on endoscopic examination or improving on CT except recurrence in one patient with SCDT.Conclusion:Patients should be inspected by dentists carefully when the patient has symptoms rather than sinusitis-like symptoms,the possibility of OMS should always be considered.Dental examination can help to determine whether a maxillary sinusitis has a dental origin,periodontitis and odontogenic radicular cysts still are the most common causes comparing with iatrogenic factors.Patients treated with ESS showed better tolerance and fewer postsurgical complications.Not all patients with OMS including SDCT need definitely surgery whether ESS or intraoral approach,removing dental focus followed with antibiotics would be optimistic choice. 展开更多

Objective:To review our experience with infrahyoid myocutaneous flap in reconstruction after oral cancer resection.Methods:Chart reviews were completed for all patients who underwent oral reconstruction with an infrahyoid myocutaneous flap by a single surgeon in the Department of Otolaryngology at Chonburi Cancer Hospital from 2011 to 2017.Characteristics of the patients and postoperative complications were analyzed.Results:Of the 34 patients in the study,10(29.4%)developed partial flap loss and 1(2.9%)developed total flap loss.All cases of partial flap loss resolved with conservative treatment.Apparent cancer involvement of a cervical lymph node was significantly associated with flap failure(odds ratio:5.0,95%CI:1.03e24.28).Conclusions:The infrahyoid myocutaneous flap is a fairly reliable reconstruction method.The flap should be performed with caution in cases with gross lymph node involvement. 展开更多
