METHODS: Electronic pathology database establis...AIM: To investigate the clinicopathologic features which predict surgical overall survival in patients with proxima gastric carcinoma involving the esophagus (PGCE). METHODS: Electronic pathology database established in the Department of Pathology of the Nanjing Drum Tower Hospital was searched for consecutive resection cases of proximal gastric carcinoma over the period from May 2004 through July 2009. Each retrieved pa- thology report was reviewed and the cases with tumors crossing the gastroesophageal junction line were se- lected as PGCE. Each tumor was re-staged, following the guidelines on esophageal adenocarcinoma, accord- ing to the 7th edition of the American Joint Commission on Cancer Staging Manual. All histology slides were studied along with the pathology report for a retrospec- tive analysis of 13 clinicopathologic features, i.e., age, gender, Helicobacter pylori (H. pylon} infection, surgical modality, Siewert type, tumor Bormann's type, size, dif- ferentiation, histology type, surgical margin, lympho- vascular and perineural invasion, and pathologic stage in relation to survival after surgical resection. Prognos- tic factors for overall survival were assessed with uni- and multi-variate analyses. RESULTS: Patients' mean age was 65 years (range: 47-90 years). The male: female ratio was 3.3. The 1-, 3- and 5-year overall survival rates were 87%, 61% and 32%, respectively. By univariate analysis, age, male gender, H. pylori, tumor Bormann's type, size, histology type, surgical modality, positive surgical margin, lym- phovascular invasion, and pT stage were not predictive for overall survival; in contrast, perineural invasion (P = 0.003), poor differentiation (P = 0.0003), 〉 15 to- tal lymph nodes retrieved (P = 0.008), positive lymph nodes (P = 0.001), and distant metastasis (P = 0.005) predicted poor post-operative overall survival. Celiac axis nodal metastasis was associated with significantly worse overall survival (P = 0.007). By multivariate analysis, ≥ 16 positive nodes (P = 0.018), lymph node ratio 〉 0.2 (P = 0.003), and overall pathologic stage (P= 0.002) were independent predictors for poor overa survival after resection. CONCLUSION: Patients with PGCE showed worse over- all survival in elderly, high nodal burden and advanced pathologic stage. This cancer may be more accurately staged as gastric, than esophageal, cancer.展开更多

Simple and common every- day equipment is used in the measurement; in particu...The global wall shear stress measurement tech- nique using shear-sensitive liquid crystal (SSLC) is extended to wind tunnel measurements. Simple and common every- day equipment is used in the measurement; in particular a tungsten-halogen light bulb provides illumination and a saturation of SSLC coating color change with time is found. Spatial wall shear stress distributions of several typical flows are obtained using this technique, including wall-jet flow, vortex flow generated by a delta wing and junction flow behind a thin cylinder, although the magnitudes are not fully calibrated. The results demonstrate that SSLC technique can be extended to wind tunnel measurements with no complicated facilities used.展开更多

Degree of leak was tested for correlation with pres- ence of Barrett's, use of PPIs, and length of Barrett's segment and duration of PPI use. RESULTS: Barrett's patients manifested a near 3-fold greater, upper gastrointestinal sucrose leak than healthy controls. A decrease of sucrose leak was ob- served in Barrett's patients who ceased PPI use for 7 d.Although initial introduction of PPI use (in a PPI-na'ive population) results in dramatic increase in sucrose leak, long-term, continuous PPI use manifested a slow spon- taneous decline in leak. The sucrose leak observed in Barrett's patients showed no correlation to the amount of Barrett's tissue present in the esophagus. CONCLUSION: Although future research is needed to determine the degree of paracellular leak in actual Barrett's mucosa, the relatively high degree of leak ob- served with in vivo sucrose permeability measurement of Barrett's patients reflects their PPI use and not their Barrett's tissue perse.展开更多

A shear force is applied on the crack surface. The analysis is based on the supposition that the stresses in each plate can be approximated by a plane stress condition. Elliptic interface problems with multi-domains and triple junction points have wide applications in engineering and science. However, the corner singularity makes it a chal- lenging problem for most existing methods. ...Elliptic interface problems with multi-domains and triple junction points have wide applications in engineering and science. However, the corner singularity makes it a chal- lenging problem for most existing methods. An accurate and efficient method is desired. In this paper, an efficient non-traditional finite element method with non-body-fitting grids is proposed to solve the elliptic interface problems with multi-domains and triple junctions. The resulting linear system of equations is positive definite if the matrix coefficients for the elliptic equations in the domains are positive definite. Numerical experiments show that this method is about second order accurate in the L~ norm for piecewise smooth solutions. Corner singularity can be handled in a way such that the accuracy does not degenerate. The triple junction is carefully resolved and it does not need to be placed on the grid, giving our method the potential to treat moving interface problems without regenerating mesh.展开更多
