Patientstreated with TA had less prevalence of multivessel disease(39.7% vs 54.7%, P = 0.003) and higher prevalence of initial thrombolysis in myocardial infarction flow < 3(P < 0.001) than NTA group. There was a higher rate of direct stenting(58.7% vs 45.5%, P = 0.009), with shorter(24.1 ± 11.8 mm vs 26.9 ± 15.7 mm, P = 0.038) and larger stents(3.17 ± 0.43 mm vs 2.93 ± 0.44 mm, P < 0.001) in the TA group as compared to NTA group. The number of implanted stents(1.3 ± 0.67 vs 1.5 ± 0.84, P = 0.009) was also lower in TA group. CONCLUSION: In an "all-comers" STEMI population, the use of TA resulted in more efficient procedure leading to the implantation of less number of stents per lesion of shorter lengths and larger sizes.展开更多

Main measurements included postoperative uncorrected-visual acuity(UCVA), corrected distance visual acuity(CDVA), refractive sphere and cylinder, manifest refraction spherical equivalent(MRSE), flat and steep corneal keratometry(K1 and K2), endothelial cell density(ECD), and time of epithelium healing and suture removal.RESULTS: Groups were comparable for diagnosis and preoperative visual acuity. Follow-up averaged 23 mo(range, 12-36 mo). At 12 mo, the mean UCVA was better in the manual-DALK group(P=0.039), and the refractive sphere was lower in the FS-DALK group(P=0.040). MRSE between groups differed at 1, 6, and 12 mo postoperatively(P=0.047, 0.025, 0.042, respectively). Mean CDVA, cylinder, K1, K2,corneal astigmatism, ECD, and time of epithelium healing were similar between groups. Stability of MRSE, ECD, and K1 returned sooner after FS-DALK. Initial loosened suture removal time was earlier in the manual-DALK group(P=0.042) while complete suture removal time was similar(P=0.122).CONCLUSION: Manual and femtosecond assisted corneal trephination in DALK are options for advanced keratoconus. FS-DALK do not result in improved visual acuity but it is more stable during the follow-up period. FSDALK in the present form show limited benefit, so surgical design and parameters still need to be optimized and explored.展开更多

These neurons could he driven by electrical stimull activiting innocuous and noxious afferent fibres in the ipsilateral plantar nerve. Traditional manual acupuncture delivered at the local acupoints Zusanli, Chengshan, Kunlun and Yongquan induced a strong inhibition or the C-fiber response. in 19 of 42 neurons obtained but did not after the A-fibre response of the neurons. The inhibition of the fibre response outlasted the period of acupuncture for more than 30 min. Neither Anor C-fibre responses in the remaining 23 neurous could be affected by manual acupuncture. These results suggest that the acupuncture stimulation specifically influences nociceptive nociceptive transmission, maybe through a presynaptic action,Furthermore, the fact that the inhibitory effect outlasts the stimulation by more than 30 min indicates that either a neuromodulatory ,presumably peptidergic action is at hand or that a temporary synaptic modification occurs in the spinal dorsal horn.展开更多

文摘AIM: To evaluate the impact of thrombus aspiration(TA) on procedural outcomes in a real-world ST-segment elevation myocardial infarction(STEMI) registry.METHODS: From May 2006 to August 2008, 542 consecutive STEMI patients referred for primary or rescue percutaneous coronary intervention were enrolled and the angiographic results and stent implantation characteristics were compared according to the performance of manual TA.RESULTS: A total of 456 patients were analyzable and categorized in TA group(156 patients; 34.2%) and non-TA(NTA) group(300 patients; 65.8%). CONCLUSION: In an "all-comers" STEMI population, the use of TA resulted in more efficient procedure leading to the implantation of less number of stents per lesion of shorter lengths and larger sizes.

文摘BACKGROUND:Acupuncture and moxibustion against visceral noxious stimulation present different mechanisms in the peripheral and central nervous systems, involving release of neurotransmitter substance P,acetylcholine esterase,leucine-enkephalin,and c-Fos protein expression. However, there are few reports addressing changes in neurotransmitter expression following manual acupuncture and electroacupuncture against visceral traction pain.OBJECTIVE:To explore changes in neurotransmitter expression in the ileum and protein expression in the medullary visceral zone of visceral traction pain rats undergoing pretreatment of emulational manual acupuncture, and to investigate the differences between emulational manual acupuncture and electroacupuncture.DESIGN,TIME AND SETTING:The randomized, controlled study was performed at the Biomedical Engineering Laboratory,Shanghai University of Traditional Chinese Medicine,Shanghai,China from August 2008 to July 2009.MATERIALS:G6805 electroacupuncture apparatus(Shanghai Medical Electronic Machine Factory,China) and ZSF-I acupuncture manipulation simulation therapeutic system(Chinese Medical Engineering Room,Shanghai University of Traditional Chinese Medicine,Shanghai China) were used in the present study.METHODS:A total of 40 male Sprague Dawley rats were equally and randomly assigned to sham surgery, model, emulational manual acupuncture and electroacupuncture groups. In the emulational manual acupuncture and electroacupuncture groups, emulational manual acupuncture and electroacupuncture were applied at bilateral Zusanli(ST 36) acupoints for 30 minutes, and models of visceral traction pain were established immediately.MAIN OUTCOME MEASURES:Substance P expression,c-Fos and glial fibrillary acidic protein expression were measured using immunohistochemistry.Acetylcholine esterase activity was examined utilizing a colorimetric method.Leucine-enkephalin content was detected using a radioimmune assay.Degree of pain in rats was assessed by pain score.RESULTS:Pain score, substance P expression in the ileum, acetylcholine esterase activity, expression of c-Fos protein and glial fibrillary acidic protein in the medullary visceral zone were significantly decreased following pretreatment of emulational manual acupuncture and electroacupuncture in rats with visceral traction pain(P < 0.05).Compared with the electroacupuncture group, the leucine-enkephalin content was significantly increased, and pain score was significantly diminished in the emulational manual acupuncture group(P < 0.05).CONCLUSION:Emulational manual acupuncture pretreatment decreases acetylcholine esterase activity, increases leucine-enkephalin release, downregulates expression of c-Fos protein and glial fibrillary acidic protein and ultimately inhibits visceral traction pain by reducing substance P release. The effectiveness in inhibiting visceral traction pain is greater when using emulational manual acupuncture compared with electroacupuncture. This is because emulational manual acupuncture effectively increases leucine-enkephalin release.

FSDALK in the present form show limited benefit, so surgical design and parameters still need to be optimized and explored.

文摘Purpose:To assess the effectiveness of automated external defibrillator(AED)and manual external defibrillator(MED)training for third-year nurse students. Methods:We conducted post-demonstration and post-practice evaluation for MED defibrillation, and pre-training,post-demonstration,and post-practice evaluation for AED defibrillation. Results:Following MED training, time and confidence to defibrillate were improved significantly post-practice(p<0.001,p<0.001,respectively).In post-demonstration and postpractice evaluation, most students placed electrodes correctly(84.21%vs.80.70%),cleared before defibrillation(75.44%vs.89.47%),and performed cardiopulmonary resuscitation immediately after defibrillation(81.81%vs.94.44%);the evaluations were not statistically different(p=0.806,p=0.094,p=0.198,respectively).For AED training, time and confidence to defibrillate post-demonstration and post-practice were significantly improved(p<0.001 vs.p<0.001;p<0.001 vs.p<0.001,respectively)compared to that of pre-training;there was no obvious difference between the post-demonstration and postpractice evaluation(p=0.235,=0.346,respectively).Post-AED demonstration, most students could place electrodes correctly(85.96%),clear(91.23%),and perform CPR immediately after defibrillation(85.96%),which remained at a high level post-practice(94.74%,85.96%,82.46%,respectively);there was no significant difference between the two evaluations(p=0.203,p=0.557,p=0.776,respectively).Conclusion:Combining MED and AED defibrillation training is effective and feasible for third-year nurse students.Minimal training is effective for AED,while MED requires additional practice.
文摘Physiotherapists usually ignore the electric polarization of human bodies that occurs under the influence of the electromagnetic forces of the Earth. This is irrational, since the positive or negative electrification of human tissues has the opposite effect on both their properties and functional activity. How physiotherapists must take into account the polarizing effect of the electromagnetic forces of the Earth when analysing the functional states of the tissues of the human body is shown here.It also shows how these electromagnetic forces can be used by manual and physiotherapists.

文摘Taking a heavy-duty truck as a research platform, the changing characteristics of shifting force, shift time, and slipping work are obtained through theoretical analysis and manual shift test of a real vehicle. Based on the analysis of the test results,a gear-shifting control strategy of the hydraulic automated shift control system is designed and experimentally verified on the bench. By optimizing the control parameters of high-speed switching valves,a control strategy and parameters are obtained, which can meet the requirements of dynamic performance and reliability.
文摘●AIM:To describe a modified technique of donor lenticule dissection for thin manual Descemet stripping endothelial keratoplasty(TM-DSEK).●METHODS:Donor material was soaked in balanced salt solution(BSS)for 30 min, before being mounted on an artificial anterior chamber(AAC).Rather than BSS,the AAC was filled with filtered air, resulting in a visible reflection at the corneal endothelium-air interface. This reflection served as a landmark for the depth of the dissection, facilitating the creation of a thin lenticule with low risk of perforation.Dissection was commenced at a standardized depth of 500 microns, with no initial pachymetry necessary.Totally 29 donor corneas were dissected by a novice TM-DSEK surgeon.Dissection time, central graft thickness at 2 mo and complications were analysed.●RESULTS:Results were similar to other endothelial keratoplasty techniques, despite the cases being performed by a novice DSEK surgeon.Mean dissection time was 7 min(range 6-10).One graft perforation occurred(3.45%),but the air tamponaded the break and enabled dissection to be restarted and completed from a different location.Mean central graft thickness after at least two months follow-up was 106 microns(range 25-170).●CONCLUSION:A problem with manual DSEK is the risk of graft perforation by attempting to dissect too thin a lenticule,or creating a thick graft due to fear of perforating. This modified air-guided technique addresses this problem, and is recommended for surgeons either embarking on the learning curve,or who wish to achieve more consistently thin grafts while reducing perforation rates.
文摘Objective: To observe the therapeutic effect of acupuncture plus manual reposition for treatment of acute lumbar vertebral articular dyskinesia for choosing a better remedy. Methods: 66 cases of acute lumbar vertebral articular dyskinesia were randomly divided into acupuncture plus manual reposition group (treatment group, n=33) and routine manual reposition group (control group, n=33). Yaotong point was punctured, when, the patient was asked to move his or her waist simultaneously. Results: After one session of treatment, of the two 33 cases in treatment and control groups, 28 (84.85%) and 20 (60.61%) were cured, 4 (12.12%) and 9 (27.27%) were improved, and 1 (3.03%) and 4 (12.12%) failed in the treatment. The therapeutic effect of treatment group was significantly superior to that of control group (P<0.05). Conclusion: Acupuncture combined with manual reposition is apparently superior to simple routine manual reposition in relieving acute lumbar vertebral articular dyskinesia.
文摘The study of international law and regulation of cyber war in the Tallinn Manual 2.0 is questionable with regard to attribution, the use of force, the exercise of the right of self-defense, the application of the principle of distinction, and even the definition of‘cyber war'. The application of contemporary law of war to cyberspace, including the militarization of cyberspace itself, should be reviewed judiciously. The construction of a cyberspace community with a shared future may be a better way out.
