It has over 9,000 positive recommendations from official government and clinical guidelines. However, it still remains relatively inaccessible in the United States, Europe and elsewhere, especially compared to the strength of evidence-based recommendations for its use. Acupuncture would benefit from robust implementation strategies, utilizing insights and approaches from implementation science. The clinical use of Botox for migraine suffered from weaker evidence of effectiveness and greater evidence of harm, but using a streamlined and robust implementation strategy, Allergan was able to achieve widespread implementation from when it began its efforts around 2010. Such a systematic approach that identifies and overcomes barriers to implementation for acupuncture would benefit millions of people who currently are offered less effective and more invasive treatments, contrary to the recommendations of clinical guidelines.展开更多

Methods:A total of 36 SD rats were randomly divided into normal group, sham operation group, model group, EA pericardium meridian group, model+D-Ser group, and EA pericardium meridian+D-Ser group. The MCAO model was prepared by Zea Longa method. The model+D-Ser group and the EA pericardium meridian+D-Ser group were given exogenous D-Ser reagent(160 mg/kg i.p.) 30 minutes after the success of the insertion of the suture. EA pericardium meridian group and EA pericardium meridian+D-Ser group were treated with electroacupuncture for 30 min on the next day. The remaining groups were only bound for 30 min.Behavioral evaluation was performed at 6 h after successful modeling and 2 d after intervention, and then the rats were sacrificed. The content of D-Ser in ischemic brain tissue was measured by liquid chromatography tandem mass spectrometry, and the protein expression of NR1, NR2A and NR2B was detected by Western Blot. Results : Before treatment, compared with the normal group and the sham operation group, the behavioral scores of each group increased after modeling (P<0.05 );Compared with the model group, the behavioral scores of the model+D-Ser group and the EA pericardium meridian+D-Ser group were increased (P<0.05).After treatment, compared with the model group, there was no significant difference in the EA pericardium meridian group (P>0.05),The behavioral scores of the model+D-Ser group and the EA pericardium meridian+D-Ser group were still increased (P<0.05);Compared with the model+D-Ser group, the behavioral score of the EA pericardium meridian+D-Ser group was not statistically significant (P>0.05).Comparison before and after treatment in the group, except for the normal group and the sham operation group, the behavioral scores of each group were slightly increased, and the difference was not statistically significant (P>0.05).Compared with the model group, the content of D-Ser and the protein expression of NR1, NR2A and NR2B in the brain tissue of the model+D-Ser group were significantly increased (P<0.05);The protein expression of NR1, NR2A and NR2B decreased in EA pericardium meridian group (P<0.05);Compared with the model+ D-Ser group, the D-Ser content and the protein expression of NR1, NR2A and NR2B in the EA pericardium meridian+D-Ser group were decreased (P<0.05). Conclusion : EA at pericardium meridian points can reduce the D-Ser content in brain tissue of rats with acute MCAO, regulate the decrease of NR1, NR2A and NR2B protein expression, inhibit the NMDA receptor activity, and play a neuroprotective role.展开更多

文摘Acupuncture enjoys a robust evidence base for dozens of clinical conditions and decades of research exploring its mechanisms of action. Such a systematic approach that identifies and overcomes barriers to implementation for acupuncture would benefit millions of people who currently are offered less effective and more invasive treatments, contrary to the recommendations of clinical guidelines.

文摘BACKGROUND:Acupuncture improves the prognosis of neonatal hypoxic-ischemic encephalopathy(HIE).However, the cytological mechanism of acupuncture therapy remains poorly under-stood.In situ neural stem cell(NSC) proliferation theory proposes that the proliferation and differentiation of NSCs plays an important role in HIE treatment with acupuncture.OBJECTIVE:To investigate NSC proliferation and differentiation in the brain of a rat model of HIE during acupuncture at Ren,Du,and urinary bladder meridians.DESIGN,TIME AND SETTING:A randomized, controlled animal experiment was performed at the Central Laboratory of Shantou University Medical College from July 2005 to June 2009.MATERIALS:A 32# 1-cun stainless steel acupuncture needle was purchased from Suzhou Acupuncture Supplies Co.Ltd.,China.METHODS:A total of 90 Sprague-Dawley rats, aged 7 days, were randomly assigned to acupunc-ture,model and normal groups, with 30 animals in each group.Animals in acupuncture and model groups were subjected to left common carotid artery ligation followed by hypoxia for 2 hours to establish neonatal HIE models.Acupuncture group rats underwent acupuncture at Ren,Du,and uri-nary bladder meridians, once a day.MAIN OUTCOME MEASURES:The number, appearance, and distribution of bromodeoxyuridine(BrdU)-positive cells in the cerebral cortex and hippocampus of each group were compared. In ad-dition,NSC differentiation in the occipital cortex and hippocampal dentate gyrus 40 days following model establishment was detected.RESULTS:BrdU-positive cells were dispersed in the cerebral cortex and hippocampus. The num-ber of BrdU-positive cells in occipital cortex and hippocampal dentate gyrus of HIE rats remained unchanged following 3 and 7 days of acupuncture, but a significant increase was detected on days 14 and 28(P<0.01 or P<0.05).At 40 days, immunofluorescence showed that a majority of BrdU-positive cells were co-lableled with the neuron marker, and neuron specific enolase, and a few were co-labeled with the astrocyte marker, and glial fibrillary acidic protein.CONCLUSION:Acupuncture at Ren,Du,and urinary bladder meridians promoted NSC proliferation in the occipital cortex and hippocampal dentate gyrus of HIE rats.Moreover,acupuncture-induced neoformative NSCs mostly differentiated into neurons.
文摘OBJECTIVE To systematically assess the efficacy and safety of acupuncture in treating acute tonsillitis, providing a reference for clinical decision-making.METHODS We searched PubMed,CENTRAL,Embase,Web of Science,China National Knowledge Infrastruc⁃ture,China Biomedical, clinical research registra⁃tion platforms, grey literature, and the reference lists of the selected studies from inception to Oct.30,2022.The risk of bias assessment used Rev⁃Man.Meta-analysis was performed using STATA with Hedges′g value. In addition,we performed subgroup analysis,meta-regression,and publica⁃tion bias detection with Harbord′s and Egger′s tests.RESULTS We included 19 randomized controlled trials comprising 1701 patients,of which 1 study was assessed high risk of bias. The primary outcome, the response rate, revealed that acupuncture was more effective than antibi⁃otics. The secondary outcomes showed that the difference in the reduction of the VAS scores, sore throat time, and white blood cell counts was statistically significant with acupuncture compared to antibiotics. However, the difference in modulat⁃ing neutrophil percentage and C-reactive protein was not statistically significant.Besides, acupuncture treatments showed a lower incidence of adverse events than antibiotics.CONCLUSION Acupuncture therapy on acute tonsillitis is safe, and the response rate is superior to antibiotics. For alleviating sore throat symptoms, shortening sore throat time, and improving immune in fl am mation index, acupuncture has positive signi fi cance.Nevertheless, because of the limitations of this study, our conclusions should be inter⁃preted with caution.Future efforts still need more high-quality trials to improve the methodology and reporting quality.

文摘Depression and insomnia have become a frequent disease in the clinic.Clinically, depression and insomnia occur synchronously. Most researchers believe acupuncture treats insomnia and depression comorbidity through mechanisms such as inflammation theory,HPA axis theory, neurotransmitter theory, nervous system structure theory, immunoregulation theory, etc.Acupuncture has a clear effect on depression and insomnia comorbidity, and is two-way,specific,and integral. Therefore,a summary on the mechanism of using acupuncture to treat depression and insomnia comorbidity is made,to provide reference for future clinical application and laboratory research.
文摘BACKGROUND Insomnia is a disease where individuals cannot maintain a steady and stable sleep state or fail to fall asleep.Western medicine mainly uses sedatives and hypnotic drugs to treat insomnia, and long-term use is prone to drug resistance and other adverse reactions.Acupuncture has a good curative effect and unique advantages in the treatment of insomnia.AIM To explore the molecular mechanism of acupuncture at Back-Shu point for the treatment of insomnia.METHODS We first prepared a rat model of insomnia, and then carried out acupuncture for 7 consecutive days. After treatment, the sleep time and general behavior of the rats were determined. The Morris water maze test was used to assess the learning ability and spatial memory ability of the rats. The expression levels of inflammatory cytokines in serum and the hippocampus were detected by ELISA.qRTPCR was used to detect the mRNA expression changes in the ERK/NF-κB signaling pathway.Western blot and immunohistochemistry were carried out to evaluate the protein expression levels of RAF-1,MEK-2,ERK1/2 and NF-κB.RESULTS Acupuncture can prolong sleep duration, and improve mental state, activity, diet volume, learning ability and spatial memory. In addition, acupuncture increased the release of 1L-1β,1L-6 and TNF-αin serum and the hippocampus and inhibited the mRNA and protein expression of the ERK/NF-κB signaling pathway.CONCLUSION These findings suggest that acupuncture at Back-Shu point can inhibit the ERK/NF-κB signaling pathway and treat insomnia by increasing the release of inflammatory cytokines in the hippocampus.
