Many patients referred for surgery already have a poor clinical condition and an advanced New York Heart Association functional class. There is no consensus on the optimal surgical technique for this condition, including on whether to perform the procedure on a beating or an arrested heart and whether to perform valve repair or replacement. Methods: We analyzed four case series between 2015 and 2022 in which patients with secondary tricuspid regurgitation (TR) underwent valve repair on a beating heart and right atrial plication for a dilated right atrium. The TRI-SCORE was calculated for each patient. Results: All patients experienced a favorable postoperative course with significant improvements in heart failure symptoms. TR was markedly reduced;however, in one patient with concomitant mitral regurgitation (MR) and a high TRI-SCORE, MR worsened postoperatively. This patient later died from unknown causes due to multiple comorbidities in the late phase. Conclusions: Tricuspid valve repair on a beating heart was effective for improving the cardiac function, and the TRI-SCORE proved useful as a preoperative risk assessment tool. The underlying mechanism by which TR exacerbates MR requires further investigation.展开更多

Method: Files of 54 patients (35 males, 19 females;mean age: 39.1...Objective: This study seeks to evaluate the results of surgery performed according to the etiological causes of diaphragmatic eventration or paralysis. Method: Files of 54 patients (35 males, 19 females;mean age: 39.1 ± 3.01 years) who had undergone diaphragm plication between January 2005 and June 2011 in two clinics located in Istanbul were studied in terms of pulmonary spirometry, applied surgical procedure, duration of hospitalization, morbidity, and mortality. Results: In diaphragm plications, observed etiologies were isolated diaphragmatic eventration without phrenic nerve injury (Group 1) in 20 (37%) patients due to blunt thoracic trauma;Group 2 (thymoma in 19 (35.2%) patients;lung cancer in 7 (12.9%) patients;mediastinal tumor in 3 (5.6%) patients) and congenital heart surgery (Group 3) in 5 (9.3%) patients. The mean time to plication was 7.4 months in Group 1. The mean duration of hospitalization was 4 days in Group 1, 6.2 days in Group 2, 11.8 days in Group 3. Mean forced vital capacity, forced expiratory volume at 1 second improved by 17%, 20.3%, respectively (p In Group 1, mean dyspnea-related hospital admission before plication was 2.4 times. At the end of 6 months after plication mean dyspnea-related hospital admission was 0 time. All of them returned to daily activities or their job within 6 months in Group 1. Postoperative mortality was observed in 2 (3.7%) patients in Group 3 while the overall complication rates were 24%. The morbidity rates were 8%, 27.5%, 60%, respectively;and the mortality rates were 0%, 0%, 40%, respectively. Conclusion: As a result, it could be suggested that early and timely performed plications for diaphragmatic eventration improves functional status and assesses a shorter lenght of stay. On the other hand,simultaneously performed plications in intraoperative phrenic nerve resections can be performed with acceptable morbidity and no mortality. The most common symptom is dyspnea. In symptomatic patients with unilateral diaphragm paralysis and dyspnea disproportionate to the degree of physical activity, diaphragm plication is the treatment of choice to relieve dyspnea. We report on our experience with laparoscopic plication of a paralysed hemidiaphragm. The laparoscopic approach is an attractive surgical alternative for the treatment of phrenic nerve palsy. This technique combines the advantage of an excellent field of vision during surgery with a fast postoperative recovery and early discharge from hospital. In accordance to recent literature, we have successfully applied the laparoscopic approach to treat our patients who suffered from a unilateral paralysis of the diaphragm. Preoperative symptoms of exertional dyspnea and orthopnea were significantly reduced.展开更多

A 54-year-old man underwent single-lung transplantation due to interstitial pneumonia. The patient required circulatory support with venoarterial extracorporeal membrane oxygenation and was unable to be weaned from ventilatory support with nitric oxide. Although enhanced CT scanning showed stenotic anastomosis of the right pulmonary artery (PA), pulmonary angiograph findings revealed that PA flow was normal under sedation and considerably decreased with spontaneous breathing. Fluoroscopy showed that the right diaphragm moved inversely to the position of the left diaphragm, indicating that the right phrenic nerve was paralytic. We performed diaphragmatic plication 7 days after lung transplantation and weaning from ventilator support was accomplished soon thereafter. Phrenic nerve dysfunction is an important clinical problem following lung transplantation. In the present case, diaphragmatic plication was effective for treatment of circulatory failure due to phrenic nerve paralysis even in acute phase after lung transplantation.展开更多
General recommendations regarding surgical techniques are not always appropriate for all Peyronie’s disease(PD)patients.Therefore,the purpose of this study was to investigate the effects of plication procedures in PD...General recommendations regarding surgical techniques are not always appropriate for all Peyronie’s disease(PD)patients.Therefore,the purpose of this study was to investigate the effects of plication procedures in PD patients with severe penile curvature and the effects of early surgical correction in patients who no longer have progressive deformities.The clinical data from 72 patients who underwent plication procedures were analyzed in this study.Patients were divided into Groups A and B according to the curvature severity(≤60°or>60°)and Groups 1 and 2 according to the duration of disease stabilization(≥3 months or<3 months).At the 1-year follow-up,90.0%(36/40)and 90.6%(29/32)patients reported complete penile straightening,and 60.0%(24/40)and 100.0%(32/32)patients reported penile shortening in Groups A and B,respectively.No curvature recurrence occurred in any patient,and no significant differences were observed in postoperative International Index of Erectile Function–Erectile Function domain(IIEF-EF),erectile pain,sensitivity,or suture knots on the penis whether such outcomes were grouped according to the curvature severity or the duration of stabilization.However,the duration from symptom onset to surgical management in Group 1 was significantly longer than that in Group 2(mean±standard deviation[s.d.]:20.9±2.0 months and 14.3±1.2 months,respectively,P<0.001).The present study showed that the plication procedures seemed to be an effective choice for the surgical treatment of PD patients with severe penile curvature.In addition,the early surgical treatment seemed to benefit those patients who already had no erectile pain and no longer exhibited progressive deformity.展开更多

The symptomatology suddenly worsened with the onset of hemorrhagic shock, which prompted an emergency hysterotomy. The diagnosis of placenta abruptio grade III b of SHER was evoked. Uterine atony was objectified intraoperatively and uterine compression by the B-Lynch technique was performed. The evolution of the patient was favorable after a stay of 5 days in intensive care.展开更多

Patients and Methods: Thi...Objective: This study aims at assessing the incidence of haematoma formation in abdominoplasty cases who had previous bariatric surgery and in those who did not have bariatric surgery before. Patients and Methods: This is a 4 years retrospective study involving patients who underwent abdominoplasty surgery between the period of July 2015 and June 2019. The incidence of haematoma formation was assessed in relation to history of previous bariatric surgery in these patients. Results: 164 patients met the inclusion criteria of the study;46 of them were males and 118 were females. The mean age of the patients included in the study was 38.6 (21 - 60 years). The mean BMI was 27 kg/m<sup>2</sup> (21.4 - 34.7 Kg/m<sup>2</sup>). The mean hospital stay was 6.6 days (4 - 19 days). 107 patients had history of previous bariatric surgery performed and 57 had not. 21 patients developed haematoma (12.8%). 16 of the patients who had haematoma had previous bariatric surgery done (15% of post bariatric surgery cases) while 5 patients developed haematoma with no history of previous bariatric surgery (8.8%). Sixteen male patients out of the 46 male patients in the study developed postoperative haematoma (34.8% of the male patients) while only 5 of the 118 female patients developed postoperative haematoma (4.2% of the female patients) which shows a significant difference in haematoma formation when comparing males to females in general (p Conclusion: Our study shows that the incidence of post abdominoplasty haematoma formation is significantly higher in male patients who had bariatric surgery before when compared to those who had not (p < 0.05). The incidence was higher in males when compared to females (p < 0.001) indicating that males with previous history of bariatric surgery have the highest chance of developing haematoma formation. Combining liposuction of the lower flanks, plication of the recti muscles or both to the abdominoplasty procedure did not result significantly in an increased risk of haematoma formation.展开更多

The first one (A: Uterus Isthmic Plication) was a new uterine compression suture, which compresses the hysterotomy site. In a severe case, we further added vessel ligation suture after </span><span style="font-family:Verdana;">performing A-suture (B-combination suture: B-suture: A + Wide Lateral Uterine </span><span style="font-family:Verdana;">Vascular Ligation). </span><b><span style="font-family:Verdana;">Methods:</span></b><span style="font-family:Verdana;"> Of 140 PPH cases, 90 were with mild PPH and 40 were with severe PPH. Our policy was: perform A-suture to mild PPH and perform B (combination) to severe PPH. Study was performed during 2018-2019. The primary endpoint to evaluate the efficacy of the</span><span><span style="font-family:Verdana;"> procedures was preserving the uterus (no hysterectomy). </span><b><span style="font-family:Verdana;">Results:</span></b><span style="font-family:Verdana;"> A-procedure,</span></span><span style="font-family:Verdana;"> performed to 90 patients with mild PPH, was effective to all 90 patients, with all preserving the uterus. B-procedure, performed 38 patients with severe PPH (excluding two patients with uterine rupture), was effective in 37 patients, with one having undergone hysterectomy due to coagulopathy. </span><b><span style="font-family:Verdana;">Conclusion:</span></b><span style="font-family:Verdana;"> Uterus Isthmic Plication (A-procedure) and addition of Wide Lateral Uterine Vascular Ligation to procedure-A (B-procedure) achieved hemostasis in patients with PPH. 文摘General recommendations regarding surgical techniques are not always appropriate for all Peyronie’s disease(PD)patients.Therefore,the purpose of this study was to investigate the effects of plication procedures in PD patients with severe penile curvature and the effects of early surgical correction in patients who no longer have progressive deformities.The clinical data from 72 patients who underwent plication procedures were analyzed in this study.Patients were divided into Groups A and B according to the curvature severity(≤60°or>60°)and Groups 1 and 2 according to the duration of disease stabilization(≥3 months or<3 months).At the 1-year follow-up,90.0%(36/40)and 90.6%(29/32)patients reported complete penile straightening,and 60.0%(24/40)and 100.0%(32/32)patients reported penile shortening in Groups A and B,respectively.No curvature recurrence occurred in any patient,and no significant differences were observed in postoperative International Index of Erectile Function–Erectile Function domain(IIEF-EF),erectile pain,sensitivity,or suture knots on the penis whether such outcomes were grouped according to the curvature severity or the duration of stabilization.However,the duration from symptom onset to surgical management in Group 1 was significantly longer than that in Group 2(mean±standard deviation[s.d.]:20.9±2.0 months and 14.3±1.2 months,respectively,P<0.001).The present study showed that the plication procedures seemed to be an effective choice for the surgical treatment of PD patients with severe penile curvature.In addition,the early surgical treatment seemed to benefit those patients who already had no erectile pain and no longer exhibited progressive deformity.

文摘The placenta abruptio is an extremely serious pathology which involves the maternal-fetal prognosis. We report the case of a 19-year-old patient who consulted for abdominopelvic pain associated with heavy vaginal bleeding on 19SA in a context of arterial hypertension. The symptomatology suddenly worsened with the onset of hemorrhagic shock, which prompted an emergency hysterotomy. The diagnosis of placenta abruptio grade III b of SHER was evoked. Uterine atony was objectified intraoperatively and uterine compression by the B-Lynch technique was performed. The evolution of the patient was favorable after a stay of 5 days in intensive care.
文摘Objective: This study aims at assessing the incidence of haematoma formation in abdominoplasty cases who had previous bariatric surgery and in those who did not have bariatric surgery before. Patients and Methods: This is a 4 years retrospective study involving patients who underwent abdominoplasty surgery between the period of July 2015 and June 2019. The incidence of haematoma formation was assessed in relation to history of previous bariatric surgery in these patients. Results: 164 patients met the inclusion criteria of the study;46 of them were males and 118 were females. The mean age of the patients included in the study was 38.6 (21 - 60 years). The mean BMI was 27 kg/m<sup>2</sup> (21.4 - 34.7 Kg/m<sup>2</sup>). The mean hospital stay was 6.6 days (4 - 19 days). 107 patients had history of previous bariatric surgery performed and 57 had not. 21 patients developed haematoma (12.8%). 16 of the patients who had haematoma had previous bariatric surgery done (15% of post bariatric surgery cases) while 5 patients developed haematoma with no history of previous bariatric surgery (8.8%). Sixteen male patients out of the 46 male patients in the study developed postoperative haematoma (34.8% of the male patients) while only 5 of the 118 female patients developed postoperative haematoma (4.2% of the female patients) which shows a significant difference in haematoma formation when comparing males to females in general (p Conclusion: Our study shows that the incidence of post abdominoplasty haematoma formation is significantly higher in male patients who had bariatric surgery before when compared to those who had not (p < 0.05). The incidence was higher in males when compared to females (p < 0.001) indicating that males with previous history of bariatric surgery have the highest chance of developing haematoma formation. Combining liposuction of the lower flanks, plication of the recti muscles or both to the abdominoplasty procedure did not result significantly in an increased risk of haematoma formation.

文摘<strong>Objective:</strong> <span style="font-family:""><span style="font-family:Verdana;">We introduced two novel hemostatic techniques to achieve hemostasis for postpartum hemorrhage (PPH). The first one (A: Uterus Isthmic Plication) was a new uterine compression suture, which compresses the hysterotomy site. In a severe case, we further added vessel ligation suture after </span><span style="font-family:Verdana;">performing A-suture (B-combination suture: B-suture: A + Wide Lateral Uterine </span><span style="font-family:Verdana;">Vascular Ligation). </span><b><span style="font-family:Verdana;">Methods:</span></b><span style="font-family:Verdana;"> Of 140 PPH cases, 90 were with mild PPH and 40 were with severe PPH. Our policy was: perform A-suture to mild PPH and perform B (combination) to severe PPH. Study was performed during 2018-2019. The primary endpoint to evaluate the efficacy of the</span><span><span style="font-family:Verdana;"> procedures was preserving the uterus (no hysterectomy). </span><b><span style="font-family:Verdana;">Results:</span></b><span style="font-family:Verdana;"> A-procedure,</span></span><span style="font-family:Verdana;"> performed to 90 patients with mild PPH, was effective to all 90 patients, with all preserving the uterus. B-procedure, performed 38 patients with severe PPH (excluding two patients with uterine rupture), was effective in 37 patients, with one having undergone hysterectomy due to coagulopathy. </span><b><span style="font-family:Verdana;">Conclusion:</span></b><span style="font-family:Verdana;"> Uterus Isthmic Plication (A-procedure) and addition of Wide Lateral Uterine Vascular Ligation to procedure-A (B-procedure) achieved hemostasis in patients with PPH. Prospective study is necessary to confirm the present data.</span></span>
文摘Apatites occurring in sedimentary rocks are carbonate fluorapatites （C-F apatites） with differentCO<sub>2</sub> contents. During diagenesis. the CO<sub>2</sub> in the C-F apatites gradually decreased with the increasing strengthof diagenesis. resulting in the systematic changes in refractive index. specific gravity, cell parameters and crystalchemistry of apatites. On the basis of X-ray diffraction and IR spectroscopic analyses, the author proposes twoparameters for determining diagenetic stages. i.e.. the crystallinity index （CR） and sharpness index （SH）. Thesetwo parameters serve as new criteria for the determination. which due to the common existence of phosphorousnodules and bands and argillo-crystalline cement in sedimentary rocks, most likely have wide prospects for ap-plication.
