Objectives: In ensuring public welfare with primary medical and health services,the primary medical staff faces new tasks.Increasing workload,and therefore degrees of stress and burnout,can influence job satisfaction ...Objectives: In ensuring public welfare with primary medical and health services,the primary medical staff faces new tasks.Increasing workload,and therefore degrees of stress and burnout,can influence job satisfaction and lead to presenteeism,which is defined as the appearance to be on the job but not actually working.The purpose of this study is to investigate the current worling situation and the relationship between presenteeism and mindfulness of primary medical staff and determine the mediating effect of self-efficacy on this relationship.Method: A cross-sectional survey was performed with 580 primary medical staff from 9 hospitals in Shaanxi province,northwest China.Presenteeism,mindfulness,and self-efficacy were measured by using a general information questionnaire,the Five-Facet Mindfulness Questionnaire,the General Self-Efficacy Scale,and the Stanford Presenteeism Scale.Mediating effect was analyzed by a series of hierarchical multiple regressions.Results: A high level of presenteeism was found among 47.4% of the study participants.Presenteeism was negatively correlated with mindfulness(r--0.409,P < 0.001) and self-efficacy(r--0.678,P < 0.001).A positive correlation was found between mindfulness and self-efficacy(r-0.584,P< 0.001).When controlling for self-efficacy (β =-0.018,P> 0.05),the association was insignificant between presenteeism and mindfulness.Conclusion: The results identified the effect of mindfulness on presenteeism of primary medical staff is realized through self-efficacy,which also suggested to enhance self-efficacy on center location when developing management strategies for mental health education or training among primary medical staff.展开更多

There has been no study examining the state of presenteeism with objective measures. We compared cerebral hemodynamic changes, measured by near-infrared spectroscopy (NIRS), during neuropsychological tests conducted by university students with presenteeism and healthy controls. Twenty-two university students participated in the study;11 of them with impaired performance caused by mental health problem were allocated to the presenteeism group and 11 without health problems to the control group. Presenteeism was assessed by the Presenteeism Scale for Students. To evoke hemodynamics changes, the participants completed a Word Fluency Test (WFT) and a Trail Making Test (TMT). The NIRS probes were located over the bilateral prefrontal area. Students with presenteeism had significantly higher incidences of depression than controls. However, there was no significant difference in behavioral performance examinations between the two groups. With regard to hemodynamics changes, the repeated measures analysis of covariance of the NIRS signals revealed significant interactions between group and task activation. Although we observed a significant increase in oxygenated hemoglobin concentration during the WFT among controls (simple main effect;left channel, F(1, 19) = 27.34, P F(1, 19) = 22.05, P < 0.001), no changes were found in students with presenteeism during either the WFT (simple main effect;left channel, F(1, 19) = 0.12, P F(1, 19) = 0.08, P t = ﹣0.94, P with Bonferroni correction = 0.745;right channel, t = ﹣2.19, P with Bonferroni correction < 0.113). This is the first study to reveal differences in activity in the cerebral cortex associated with presenteeism. The fact that students with presenteeism have prefrontal dysfunction might reinforce the concept of presenteeism.展开更多
Although work factors have been associated with both presenteeism and exhaustion among hospital physicians, we lack knowledge on the dynamic relationship between demands in the work context and presenteeism and how th...Although work factors have been associated with both presenteeism and exhaustion among hospital physicians, we lack knowledge on the dynamic relationship between demands in the work context and presenteeism and how this can be mediated by symptoms of exhaustion when controlling for job resources. The objective of this study is to examine a health impairment process of presenteeism among university hospital physicians. A cross-sectional survey of 545 university hospital physicians in Norway was conducted. Variables included in the model were presenteeism, exhaustion, work-family conflict, role conflict, social support and control over work pace. Findings from structural equation modeling indicated that exhaustion mediates the relationship between job demands and presenteeism. Job resources had no direct effect on presenteeism in the hypothesized model. The variables in the study explained 17% of the variance in presenteeism. The study is one of the first to demonstrate that the relationship between job demands and presenteeism is mediated by exhaustion when controlling for job resources. The results highlight the importance of considering the link between health symptoms and job demands to reduce the negative effects of presenteeism.展开更多
