Most endoscopic patients require some forms of sedation and/or anesthesia. The goals of this sedation are to guard the patient's safety, minimize physical discomfort, to control behavior and to diminish psychological responses. Generally, moderate sedation for these procedures has been offered by the non-anesthesiologist by using benzodiazepines and/or opioids. Anesthesiologists and non-anesthesiologist personnel will need to work together for these challenges and for safety of the patients. The sedation training courses including clinical skills and knowledge are necessary for the registered nurses to facilitate the patient safety and the successful procedure. However, appropriate patient selection and preparation, adequate monitoring and regular training will ensure that the use of nurse-administered sedation is a feasible and safe technique for gastrointestinal endoscopic procedures.展开更多

A priority to develop pre and post registration nurse education in India has increasingly been highlighted in nursing and health policy imperatives in recent years. Nurses are often the only health care professionals in primary and secondary care within rural and remote healthcare settings in India. They are confronted with the dual challenge of resource constraints and rapidly changing disease profile with little or no access to continuing professional development. Objectives: 1) To identify key continuing professional development priorities of registered nurses working in remote and rural health care settings in India. 2) To identify barriers and facilitators to continuing professional development as perceived by registered nurses working in these settings. 3) To identify preferred modes of continuing professional development by registered nurses working in remote and rural health care settings in India. Design: Quantitative Design. Setting: Two large health care facilities in remote and rural parts of India. Participants: Registered Nurses working in two large not for profit health care organisations participated in the study. Nursing assistants and student nurses were excluded from the study. 368 participants consented to participate in the survey and 271 (73.6%) participants completed the survey. Methods: A questionnaire based cross sectional survey was undertaken as part of the Continuing Professional Development needs assessment among registered nurses working in rural and remote settings. Results: Continuing Professional Development priorities included training on clinical competencies focussed on managing emergency situations related to non-communicable diseases [50%], managerial competencies related to ethical dilemmas [60%] such as support for families with financial difficulties accessing health care [17.8%], women undergoing abortions [14.6%], or those with HIV infection [12.9%]. Preferred modes for Continuing Professional Development included conference attendance [54%], skills training [48%] and in-house training [32%]. Key facilitators for Continuing Professional Development included, professional development [77%], personal interest [42%], opportunities for professional engagement with colleagues [39%], the need to reduce knowledge and skill gap [36%] and career progression [28%]. Geographic distance [59%], low staffing levels [51%], cost [43%], domestic responsibilities [40%], and work commitments [39%] were reported as key barriers to Continuing Professional Development. Conclusions: The findings from this survey, the first of its kind in India, provides evidence on priorities, barriers and facilitators for continuing professional development of registered nurses working in rural and remote settings in India.展开更多

Background: There is evidence RN’s do not consider NGRN’s as adequately prepared for practice. However, there does not appear to be consensus on what characterizes work-readiness. Methods: An adapted online questionnaire was sent to RNs in a range of clinical settings. They rated NGRNs’ skills and the applicability of those skills to their clinical setting using a 5-point Likert scale. Results: Perceptions of transferability of skills across settings varied. In the skills grouping of routine assessment, basic clinical skills, advanced clinical skills the percentage rated as “not applicable” was over 40% in 75% of the skills. The low or zero non-applicable responses to communication skills indicate this skill group contains transferable skills regardless of the setting. The zero non-applicable responses for preparedness for practice for all but two skills indicate these groups contain transferable skills across settings. Conclusion: There appears to be some alignment between expectations from RNs and current NGRN preparation. The lack of transferability of more advanced technical skills raises questions about the role of education in preparing the NGRN versus that of industry-based graduate programmes. The increase in nursing roles and the increased emphasis on aged care and primary health care dictates that generalist preparation of NGRNs emphasising the attributes of an RN should be the focus rather competency in advanced technical skills.展开更多

Study Eligibility Criteria, Participants and Interventions: Published articles addressing the research objectives, written in English, spaning between September 2010 and March 2019 directly answer the PSCOT question with results applicable to the population specific-nursing students, simulation was utilized. Study Appraisal and Synthesis Methods: Extracted data from sorted studies were analysed and assessed for quality using the John Hopkins Evidence-Based Appraisal Tools. Results: The two-database yielded 520 articles, 10 met the eligibility criteria. Findings documented three important themes: 1) Clinical competence among nurses;2) Medical residents’ clinical competence and 3) Was about the competence of anaesthesiology residents. Limitations: The identified studies used small sample sizes ranging from 20 to 98 participants in the single research studies. Conclusions: The review of the literature found that the active teaching strategy of simulation was more effective in promoting clinical competency in nursing students compared with the lecture method. The evidence-based teaching project proposes the use of simulation as the primary teaching method to improve the clinical competency abilities of registered general nursing (RGN) diploma students.展开更多
