Effect of scalp acupuncture combined with language rehabilitation training in treating post-stroke motor aphasia:A systematic review and meta-analysis

Objective:To evaluate the efficacy of scalp acupuncture combined with language rehabilitation training in the treatment of motor aphasia.Methods:CNKI,VIP,Wan Fang Database,MEDLINE,Embase,Web of Science and Cochrane Li... Objective:To evaluate the efficacy of scalp acupuncture combined with language rehabilitation training in the treatment of motor aphasia.Methods:CNKI,VIP,Wan Fang Database,MEDLINE,Embase,Web of Science and Cochrane Library were searched for published researches up to March,2021.Randomized controlled trials RCTs that focused on scalp acupuncture combined with language rehabilitation training in the treatment of motor aphasia were included.We managed the data analysis with RevMan 5.3 software.Results:A total of 16 RCTs with 1323 patients were involved.The results of meta-analysis showed that:①The effective rate of scalp acupuncture combined with language rehabilitation training in the treatment of motor aphasia after stroke was significantly better than that of simple language rehabilitation training[OR=3.94,95%CI(2.73,5.68),P<0.00001];②In the evaluation of language function,compared with the language rehabilitation training,the scalp acupuncture combined with language rehabilitation training can significantly improve the reading ability of the patients with motor aphasia after stroke[MD=7.22,95%CI(3.55,10.89),P=0.0001],writing ability[MD=6.51,95%CI(3.61,9.41),P<0.0001],expressive ability[MD=4.13,95%CI(2.37,5.89),P<0.0001],retelling ability[MD=5.00,95%CI(2.38,7.63),P=0.0002],listening comprehension ability[MD=5.36,95%CI(3.12,7.61),P<0.00001]and naming ability[MD=5.60,95%CI(4.20,7.00),P<0.00001];③Compared with simple language rehabilitation training,scalp acupuncture combined with language rehabilitation can significantly improve the daily life language communication ability of patients with motor aphasia,and the difference was statistically significant[MD=30.01,95%CI(11.30,48.72),P=0.002].Conclusion:Scalp acupuncture combined with language rehabilitation training has a significant effect on motor aphasia.However,due to the small sample size,more RCTs are needed to confirm that. 展开更多

Objective; To observe clinical therapeutic effect of scalp acupuncture in the treatment of facial spasm. Methods: Sixty-five cases of facial spasm were randomly divided into control group (n = 31 ) and treatment group(n = 34). Patients of the treatment group were treated with penetrative acupuncture from Qienshencong(前神聪 Extra) to Xuanli(悬厘 GB 6), Baihui(百会 GV 20) to Qubin(曲鬓 GB 7), etc., and those of control group treated with acupuncture of Taiyang(太阳 EX-HN 5), Yengbai(阳白 GB 14), Quanliao(颧髎 SI 18) and Xiaguan(下关 ST 7), etc. once daily, 20 sessions altogether. Results. After treatment, of the 31 cases and 34 cases in control and treatment groups, 5 and 12 were cured, 8 and 13 had remarkable improvement, 11 and 8 had improvement, 7 and 1 failed, with the total effective rates being 77.42% and 97.06% respectively; and the therapeutic elfact of treatment group was significantly superior to that of control group(P〈0.01). Conclusion; Scalp acupurcture has an obvious therapeutic effect for facial spasm. 展开更多

Objective:Cluster needling at scalp acupoints has showed satisfying effects with acute cerebral ischemia in clinic whereas the mechanisms are not yet clear completely.This study investigated the influence of cluster needling at scalp acupoints on neurological function,as well as on neurofilament protein 200(NF200)and signal transducer and activator of transcription 3(STAT3)expression,in rats with acute cerebral ischemia.Methods:Fifty-four Sprague-Dawley rats were randomly assigned in equal numbers to the false operation(group F),model(group M),or cluster needling scalp acupuncture(group C)groups.Each group was divided into three subgroups,of six rats each,by acupuncture treatment time(24 h,7 days,and 14 days).The rat local cerebral ischemia model was prepared using a modified suture occlusion method.Group C rats were treated by cluster needling scalp acupuncture,while groups F and M did not receive acupuncture treatment.Neurological effects were evaluated using the Longa score.NF200 and STAT3 expression were measured by western blotting.Results:At 24 h,there were no statistical difference between group C and group M in nerve function(P>.05).On days 7 and 14,nerve function scores in group C were significantly lower than that in group M(respectively were P<.05 and P<.01).In addition,on days 14,expression of NF200 was significantly higher in group C compared with group M(P<.05).Compared with group M,STAT3 expression was also higher in group C on days 7 and 14,although these differences were not statistically significant(both P>.05).Conclusion:Cluster needling scalp acupuncture were efficient in improving nerve function scores in rats with cerebral ischemia,and promoting the recovery of motor function.These improvements were associated with increases in NF200 and STAT3 expression. 展开更多

The instant therapeutic effects: the headache in 163 cases disappeared; in 9 cases the effect was quite obvious; and in 7 cases it was effective; the total effective rate amounted to 98.9%. Objective:To observe the clinical efficacy of body acupuncture combined with FANG’s scalp acupuncture in treating limb dysfunction in the remission stage of stroke and explore the mechanism.Methods:Sixty patients in the remission stage of stroke with limb dysfunction were divided into a control group and a treatment group using the random number table method,with 30 cases in each group.The control group was treated with ordinary acupuncture based on the conventional rehabilitation treatment,and the treatment group was additionally offered FANG’s scalp acupuncture.The interventions were conducted once daily with 10 sessions as 1 course.The Fugl-Meyer assessment(FMA)and Barthel index(BI)scores and serum malondialdehyde(MDA)and superoxide dismutase(SOD)levels were compared after 2 treatment courses,and the clinical efficacy was evaluated.Results:After treatment,the FMA and BI scores increased in both groups(P<0.05)and were higher in the treatment group than in the control group(P<0.05).Both groups showed a decreased MDA level and an increased SOD level after the intervention,all showing statistical significance(P<0.05);there were significant differences between the two groups(P<0.05).Conclusion:Based on the conventional rehabilitation treatment,ordinary acupuncture used alone or in combination with FANG’s scalp acupuncture can lower the oxidative stress level and improve limb function in treating limb dysfunction in the remission stage of stroke;body acupuncture plus FANG’s scalp acupuncture can produce better results. 展开更多

Objective: To observe the therapeutic effect of scalp acupuncture on cerebral blood flow in pseudobulbar paralysis patients for analyzing mechanisms of scalp acupuncture in the treatment of wind stroke. Methods: A total of 38 inpatients (26 males and 12 females) were treated with electroacupuncture (EA) of scalp point Dingzhongxian (MS 5), Dingnie Houxiexian (MS 7), Dingpangxian II (MS 9) and Dingnie Qianxiexian (MS 6). Before and after acupuncture treatment, clinical symptoms of dysphagia and dysphonia were compared, and the mean blood flow speed (MBFS) values of the anterior cerebral artery (ACA), middle cerebral artery (MCA) and posterior cerebral artery (PCA) detected by using Doppler blood flow meter. Results: Following two courses (4 weeks) of scalp acupuncture treatment, of the 38 cases, 23 had their dysphagia and dysphonia cured (60.5%), 10 (25.3%) had remarkable improvement, 3(7.9%) experienced improvement and 2 (5.3%) had no apparent changes. Simultaneously, MBFS of ACA, MCA and PCA increased significantly in comparison with that of pre treatment (P<0.01). Additionally, results also showed that scalp acupuncture could stabilize the blood circulation between both hemispheres of the brain. Conclusion: Scalp acupuncture has a fairly good therapeutic effect in improving stroke caused dysphagia and dysphonia and in facilitating cerebral blood flow. 展开更多

OBJECTIVE:To investigate the efficacy of scalp acupuncture Yikang therapy on Baihui(GV20),Sishencong(EX-HN1),Zhisanzhen,Niesanzhen,on neurobehavior in young rats with cerebral palsy based on Notch signaling pathway.METHODS:Thirty 7-day-old rats were randomly divided into sham,model and acupuncture,10 rats in each group.The cerebral palsy model was established by the accepted modeling method,the acupuncture group selected"Baihui(GV20)","Sishencong(EX-HN1)","Zhisanzhen"and"Niesanzhen"for intervention 24 h after the model was made.The body masses were recorded before and after the treatment,respectively.After the intervention,the rats were subjected to suspension experiment,slope experiment,tactile stimulation experiment and Morris water maze experiment.After the end of the experiment,the morphological changes of hippocampal histology were observed by hematoxylineosin(HE)staining under light microscope,and the expression of Notch1,Notch3 and Hes5 were detected by Western blot and quantitative real-time polymerase chain reaction(PCR).RESULTS:The changes in body mass of the rats in each group were different;in behavioral experiments,compared with the sham,the suspension time of the model was shortened,the slope experiment,tactile stimulation experiment,and escape latency time were prolonged,and the number of platform crossing was reduced in the model,compared with the model,the suspension time of the acupuncture was prolonged,the slope experiment,tactile stimulation experiment,and escape latency time were shortened,and the number of platform crossing times was increased;HE staining showed severe hippocampal damage in the model and reduced hippocampal damage in the acupuncture.Western Blot and real-time fluorescence quantitative PCR showed that the expression of Notch1,Notch3 and Hes5 were increased in the model and the expression of Notch1,Notch3,Hes5 in acupuncture were decreased.CONCLUSIONS:Scalp acupuncture Yikang therapy may improve neurobehavior and reduce brain injury in rats with cerebral palsy by downregulating the expression of Notch1,Notch3,and Hes5. 展开更多

Background:To investigate the effects of acupuncture on post-stroke limb spasm model rats and the underlying mechanism.Methods:A total of 50 Sprague-Dawley rats were randomly divided into three groups,Control group(10),Model group(20)and Zhishen Tiaoxing(ZSTX)acupuncture group(20).Middle cerebral artery occlusion was conducted in SD rats to establish post-stroke limb spasm rats,which were treated with ZSTX acupuncture.Behavioral assays were determined by the Narrow ally test,the limb muscle tension was detected by the BL-420S test system,and infarct volume was assessed after the cerebral infarction by 2,3,5-triphenyltetrazolium chloride staining.Heterogeneous neurotransmitterγ-aminobutyric acid(GABA)and its receptors GABAA and GABAB in the cerebral cortex of the infarct area were determined by immunofluorescence assay.The release of Trkb and K-Cl cotransporter isoform 2 was detected by an immunofluorescence double labeling study.Western Blot was utilized to measure the expression of BDNF and Trkb.Results:The results showed that the behavioral assays in post-stroke limb spasm rats were significantly improved by the treatment of ZSTX acupuncture.14 days of ZSTX acupuncture can effectively inhibit muscle tone and decrease Infarct volume,which was measured with BL-420S biological function experiment system and triphenyltetrazolium chloride.Meanwhile,the results of Double-Label Immunofluorescence Assays showed that ZSTX acupuncture improved the expression of GABA,GABAA,GABAB,BDNF,and K-Cl cotransporter isoform 2.Double-Label Immunofluorescence Assays and WB results showed that 14 days ZSTX acupuncture declined the expression of Trkb.Conclusions:Our results suggest that 14 days of ZSTX acupuncture can significantly improve post-stroke limb spasm.Meanwhile,the pathogenesis of post-stroke limb spasm and the efficacy of ZSTX acupuncture involve metabolic pathways of neurotransmitters,and electro-acupuncture can treat post-stroke limb spasm by regulating BDNF/Trkb-KCC2 signaling pathway. 展开更多

Objective To evaluate the efficacy and safety of scalp acupuncture in the treatment of ischemic stroke. Objective To evaluate the efficacy and safety of scalp acupuncture in the treatment of ischemic stroke. Methods The method recommended in the Cochrane Collaboration was used for the systematic evaluation of the randomized controlled trials (RCTs) of scalp acupuncture in the treatment of ischemic stroke. Results One thousand three hundred and ninety-five cases from 13 papers in Chinese version were in compliance with the inclusive criteria and all of them were the low-quality trials. The results of the included 5 trials in Meta analysis indicated that the efficacy of scalp acupuncture was superior to medication in the treatment of ischemic stroke (RR=1.27, 95% CI 1.06, 1.51). The results of 3 trials in Meta analysis suggested that the recovery in the nerve function defect of the patients with ischemic stroke treated with scalp acupuncture was apparent as compared with medication (WMD = -2.96, 95% C1-4.00, -1.92). The results of 5 trials in Meta analysis showed that the efficacy of scalp acupuncture on ischemic stroke was potentially superior to body acupuncture (RR=I.10, 95% CI 0.93, 1.28). Conclusion It is seen in the Meta analysis results of the included 13 RCTs that scalp acupuncture achieves the definite efficacy on ischemic stroke. Due to the limitation of the inclusive numbers and quality of the research, especially the lack of long-term indices, the conclusion reliability of this research is relatively low. Hence, it is required much more evidences for a further confirmation from the large scale and multiple central RCTs with high quality and intrinsic authenticity. 展开更多

Objective To observe the effect of scalp acupuncture on the treatment of children with cerebral palsy(CP).Methods Thirty-six children in the USA with cerebral palsy were treated with Chinese scalp acupuncture at the Neuroacupuncture Institute in Santa Fe,New Mexico,USA.Foot motor and sensory area,motor area,speech I area,speech II area,balance area,vision area,apraxia area,and I vision area were selected base on patient's clinical manifestation.Rotating manual stimulation was performed during each session of acupuncture.The retention of needles was 30 minutes.Results These 36 patients received between 3–20 treatments,5 had complete resolution of their symptom,14 cases a marked effect,14 had some effect,and 3 patients experienced no improvement.No adverse events were reported.Conclusion Chinese scalp acupuncture in these 33 cases appears to be an effective treatment for patients with CP and should be considered as an adjunctive therapy for more CP patients.Chinese scalp acupuncture relieved many of these patients' symptoms,increased their quality of life,and reversed many physical disabilities.Some of the patients returned to a normal life.Those treatments have laid a foundation for research into the clinical application of Chinese scalp acupuncture in the treatment of CP. 展开更多

Methods on benign prostate hyperplasia Two hundred and forty patients diagnosed clinically with BPH were randomly divided into 3 groups. In scalp-acupuncture group (80 cases), foot-motor-sensory area and reproductive area were selected. In body-acupuncture group (82 cases), the acupoints were selected, such as Zhengji(中极 CV 3), Guanyau(关元 CV 4) and Sanyfnjiao (三阴交 SP 6). In combined therapy group of scalp acupuncture and body acupuncture (78 cases), scalp acupuncture was applied together with body acupuncture. It was to observe the changes in prostate size, Qmax, IPSS (international prostatic symptom score) and QOL (quality of life) before and after treatment of 3 groups. Results The total effective rate was 72.5% in scalp-acupuncture group, 72.0% in bodyacupuncture group and 85.9% in combined therapy group of scalp acupuncture and body acupuncture. The therapeutic effect in combined therapy group was superior to that in either scalp-acupuncture group or body-acupuncture group (both P〈0. 05). The improvements had been obtained in Qmax, post-void residual volume (PRV), IPSS and QOL after treatment in 3 groups, in which, the improvement in combined therapy group was apparent. Conclusion Either scalp acupuncture or body acupuncture relieves clinical symptoms and improves the quality of life in BPH patients. But, the best therapeutic effect has been obtained specially in the treatment with combined therapy of scalp acupuncture and body acupuncture. 展开更多

The therapeu c effi cacy was observed a er treatments. Results Sixty-two cases were cured,markedly effective in 8 cases,effective in 7 cases and failed in1 case; the total eff ec ve rate was 98.72%. Objective To compare clinical therapeutic effects of scalp acupuncture, plum-blossom needles tapping the head area combined with speech training and simple speech training on aphasia of ischemic apoplexy. Methods Sixty cases of aphasia of ischemic apoplexy were randomly divided into an observation group and a control group, 30 cases in each group. The patients in the control group received speech training, and in the observation group, on the basis of speech training, acupuncture was given at the anterior oblique line of vertex temporal, the posterior oblique line of vertex temporal and the middle line of vertex, and after withdrawal of the needles, the area enclosed by the 4 lines of the anterior oblique line of vertex temporal, the middle line of vertex, the posterior oblique line of vertex temporal, and connective line of Qǔbìn (曲鬓 GB 7) with Xuánlí(悬厘 GB 6) was tapped 2-3 times with plum-blossom needles. The assessment criteria in Chinese Aphasia Set Tests were used for comparison of scores of all the sub-items between the two groups. Results After treatment, scores of information amount, fluency, retelling, naming of words, naming of colors, naming of response, answer to yes or no question, hearing and recognizing, executing an order, etc. were signif icantly increased in both the two groups (all P0.05), and the scores of all the above items in the observation group were more significantly increased as compared with those in the control group (all P0.05). Conclusion Scalp acupuncture, plum-blossom needle tapping combined with speech training has a significant therapeutic effect on aphasia of ischemic apoplexy, which is better than that of simple speech training. 展开更多

Methods:Ninety-one eligible patients with Broca&#39;s aphasia after cerebral stroke were randomized into an observation group and a control group. Forty-six cases in the observation group were intervened by scalp acupuncture plus speech rehabilitation, while 45 cases in the control group were treated by speech rehabilitation alone. The aphasia battery of Chinese (ABC) and Boston diagnostic aphasia examination (BDAE) were adopted to evaluate the clinical efficacy. Results:After the treatment, the scores of oral expression, reading and writing and global score in the observation group were significantly higher than those in the control group (allP〈0.05). There was a significant difference in comparing the BDAE grading between the two groups after the treatment (P〈0.05). After intervention, the basically-recovered plus markedly-effective rate was 45.7% in the observation group versus 24.4% in the control group, and the between-group difference was statistically significant (P〈0.05). Conclusion:Scalp acupuncture plus speech rehabilitation is effective in treating Broca&#39;s aphasia after cerebral stroke, and worth promoting. 展开更多

Objective: To evaluate the effectiveness of scalp acupuncture (SA), a modern acupuncture technique specialized to neurological disorders, in managing motor function and symptoms for Parkinson's disease (PD) patients. Methods: Two independent reviewers extracted data from all of the randomized clinical trials (RCTs) that assessed the efficacy of SA for PD compared with conventional therapies (CTs). Sixteen electronic databases were searched. The risk of bias was appraised with the Cochrane Collaboration tool, and the reporting of the included studies was evaluated by the Consolidated Standards of Reporting Trials (CONSORT) 2010 checklist and the revised Standards for Reporting Interventions in Clinical Trials of Acupuncture (STRICTA) guidelines. Results: In total, 4 RCTs met the inclusion criteria. As assessed by the Unified PD Rating Scale (UPDRS), 2 RCTs showed that SA combined with CTs proved superior to CTs alone [60 cases; weighted mean difference, -3.94; 95% confidence interval (CI), -6.05 to -1.64, P=0.01; 12=0%]. Based on the Webster scale, however, 3 RCTs showed no superior effect of SA when combined with CTs with high heterogeneity (154 cases; risk ratio, 1.29; 95% CI, 0.79 to 2.12, P=0.30; 12=84%). The Cochrane risk of bias, adherence to the CONSORT and the STRICTA checklist showed that the quality of all the included RCTs was generally low. Conclusions: The result of our systematic review and meta-analysis suggested that the effectiveness of SA for PD is promising, however, the evidence is not convincing. A sham-controlled RCT design that adheres to the CONSORT and STRICTA guidelines to overcome methodological weakness and that includes a large sample size is strongly recommended to confirm the precise effect of SA on PD. 展开更多

Objective:To evaluate the effects of interactive dynamic scalp acupuncture(IDSA),simple combination therapy(SCT),and traditional scalp acupuncture(TSA)on motor function and gait of the lower limbs in post-stroke hemiplegia patients.Methods:A total of 231 patients with post-stroke hemiplegia was randomly divided into IDSA(78 cases),SCT(78 cases),and TSA(75 cases)groups by a random number table.Scalp acupuncture(SA)and lower-limb robot training(LLRT)were both performed in the IDSA and SCT groups.The patients in the TSA group underwent SA and did not receive LLRT.The treatment was administered once daily and 6 times weekly for 8 continuous weeks,each session lasted for 30 min.The primary outcome measures included Fugl-Meyer Assessment of the Lower Extremity(FMA-LE),Berg Balance Scale(BBS),Modified Barthel Index(MBI),and 6-min walking test(6 MWT).The secondary outcome measures included stride frequency(SF),stride length(SL),stride width(SW),affected side foot angle(ASFA),passive range of motion(PROM)of the affected hip(PROM-H),knee(PROM-K)and ankle(PROM-A)joints.The patients were evaluated before treatment,at 1-and 2-month treatment,and 1-,and 2-month follow-up visits,respectively.Adverse events during 2-month treatment were observed.Results:Nineteen patients withdrew from the trial,with 8 in the IDSA group,5 in the SCT group,and 6 in the TSA group.The FMA-LE,BBS,6 MWT and MBI scores in the IDSA group were significantly increased after 8-week treatment and2 follow-up visits compared with the SCT and TSA groups(P<0.05 or P<0.01).Compared with pre-treatment,the grade distribution of BBS and MBI scores in the 3 groups were significantly improved at 1,2-month treatment and 2 follow-up visits(P<0.05 or P<0.01).The SF,PROM-H,PFOM-K and PROM-A in the IDSA group was significantly increased compared with the SCT and TSA groups after 8-week of treatment(P<0.05 or P<0.01).Compared with the SCT group,ASFA of the IDSA group was significantly reduced after 8-week of treatment(P<0.05).SF,SL,PROM-K and PROM-A were significantly increased at the 2 nd follow-up visit whereas the ASFA was significantly reduced in the IDSA group compared with the SCT groups at 1 st follow-up visit(P<0.05 or P<0.01).The SF was significantly increased in the SCT group compared with the TSA group after 8-week treatment(P<0.05).Compared with the TSA group,PROM-K,PROM-A were significantly increased at the 2 nd follow-up visit(P<0.05).Conclusions:The effects of IDSA on lower-limb motor function and walking ability of post-stroke patients were superior to SCT and TSA.The SCT was comparable to TSA treatment,and appeared to be superior in improving the motion range of the lower extremities.(Registration No.ChiCTR1900027206) 展开更多

Methods:One hundred and thirty-six cases with hemiplegia after stroke who met the inclusion criteria were randomly divided into three groups according to visiting sequence. Forty-eight cases in the observation group were treated by scalp acupuncture combined with rehabilitation training, 35 cases in the medicine group were treated by Chinese and Western medicines, and 53 cases in the medicine plus rehabilitation group were treated by Western medicine and rehabilitation training. Patients' consciousness, speech and limb functions were scored before and after treatment, and the results were compared. Results:After treatment, the scores of consciousness, speech and limb functions after treatment were lower than those before treatment. And their decrease in the observation group were more statically significant than that in the medicine group and the medicine plus rehabilitation group (P<0.01 or P<0.05). The total effective rates of the three groups were significantly different (P<0.01 or P<0.05). The total effective rate of the observation group was better than that of the other two groups (both P<0.01). Conclusion:Scalp acupuncture combined with rehabilitation therapy has better effect for stroke hemiplegia. 展开更多
