Associated with a significant morbidity, this ectoparasite is present in all tropical regions, such as Gabon. Unfortunately, in this country, no study has ever been undertaken on this disease. However, in both urban and rural areas of this country, it causes disfigurement and mutilation of the feet or hands in infested persons, and is sometimes responsible for the school dropout of many children. It is in this context that this timely and relevant research was conducted to assess the prevalence and risk factors associated with Tungiasis in school-aged children in the city of Port Gentil, Ogooué-Maritime province, Gabon in Central Africa. Materials and Methods: This prospective and cross-sectional study took place from May 22 to August 18, 2022, in two popular and underprivileged neighborhoods of the city of Port Gentil. It consisted of a physical diagnosis of the participants, and with the help of a questionnaire submitted to their parents/guardians, numerous data were collected. Entered into an Excel 2016 spreadsheet, the data were exported to the R software in its R Commander interface, for two types of analyses. A univariate and a multivariate analysis, using logistic regression, first on socio-demographic characteristics and then on habitual and environmental behavioral characteristics, was performed and the variables in this analysis reported odds ratios and their 95% confidence intervals. The results were statistically significant at p Results: A total of 637 school-aged children with a mean age of 12.73 years and a standard deviation of 5.44, were registered and examined for this study. Among them, Tunga penetrans infestation was found in 242 children, indicating an overall prevalence of 37.99% (95% CI [0.34 - 0.41]). Univariate and multivariate analyses, using logistic regression, first according to socio-demographic characteristics and then according to risk factors indicated that, the age groups of 5 to 9 years (Adjusted Odds Ratio = 9.27;95% CI: [6.66 - 12. 91]) and 10 - 14 years (Adjusted Odds Ratio = 0.16;95% CI: [0.10 - 0.25]), Eshira ethnicity (Adjusted Odds Ratio = 7.46;95% CI: [2.97 - 18.76]), child’s primary education level (Adjusted Odds Ratio = 0.008;95% CI: [0.006 - 0.011]), Father’s/Guardian’s education level (Adjusted Odds Ratio = 0.22;95% CI: [0.13 - 0.35]), homemaker status (Adjusted Odds Ratio = 4.89;95% CI: [3.51 - 6. 79]) and risk factors such as: promiscuity with dogs, cats or other animals around the house (Adjusted Odds Ratio = 38.30;95% CI: [22.13 - 66.26]), wearing shoes a few times (Adjusted Odds Ratio = 0.02;95% CI [0.01 - 0.03]), walking sometimes barefoot, on land or sand (Adjusted Odds Ratio = 0.28;95% CI: [0.18 - 0.42]), having other sources of water consumed than the tap (Adjusted Odds Ratio = 0.25;95% CI: [0.16 - 037]), living in a dwelling with soil or sand (Adjusted Odds Ratio = 6.8;95% CI: [4.58 - 10.09]), having average housing conditions (Adjusted Odds Ratio = 46.8;95% CI [24.29 - 90.16]), and the number of people living in the family that was greater than or equal to 6 (Adjusted Odds Ratio = 0.1;95% CI [0.07 - 0.13]), were significantly associated with the prevalence of Tungiasis. Conclusion: The results of this study will inform control programs and water and sanitation interventions for the Tunga penetrans epidemic in Port Gentil, Gabon.展开更多

A cohort study among 6 - 13 years old children was conducted in the Lake Zone region of Tanzania to assess S. mansoni infection using a circulating cathodic antigen (CCA), Kato Kartz while urine dipstick to screen for urine protein levels and red blood cells. A blood sample was taken for every child to determine creatinine levels and later a status of estimated glomerular filtration rate (eGFR). The prevalence of S. mansoni infection was 64%, 46%, and 24% at baseline, 6 months, and 1 year respectively using CCA test. There was a 62.5% reduction in S. mansoni infection from baseline (p S. mansoni using CCA test, reductions in proteinuria, and hematuria in school children in Tanzania. There was complete remission in eGFR < 90 ml/min/1.73 m2 from 3.9% to 0% at 6 months. This suggests that praziquantel is effective, but there is still a need for integrated strategies to minimize reinfections.展开更多

Using urine test strips and filtration techniques, the study found prevalences ranging from 11.88% to 34.53% across the municipalities. Method: Epidemiological data (urine) were collected and examined with test strips to assess the presence of terminal haematuria: the prevalence of infestation and parasite density was quantified using the filtration technique. Socio-demographic and economic factors were recorded using a questionnaire to assess the correlation with disease. Results: Prevalences of 34.53% (N = 48 out of 139), 13.53% (N = 18 out of 133) and 11.88% (N = 17 out of 143) respectively for the municipalities of Aguégués, N’dali and Sô Ava were calculated. The study showed that the variables “age”, “sex”, “religion” and “socio-professional” activity were not significantly correlated with bilharziasis (p > 0.05) and it appears that these factors are not related to bilharziasis in the surveyed households. While age, sex, religion, and socio-professional activity showed no significant correlation with bilharziasis, factors like agriculture, fishing, and place of residence were statistically significant in relation to the disease. Conclusion: The findings suggest that these socio-demographic and economic factors impede the elimination of schistosomiasis in the examined areas.展开更多

文摘School-age children are in a specific development stage corresponding to juvenility, when the white matter of the brain experiences ongoing maturation.Dffusion-weighted magnetic resonance imaging(DWI),especially diffusion tensor imaging(DTI),is extensively used to characterize the maturation by assessing white matter properties in vivo.In the analysis of DWI data, spatial normalization is crucial for conducting inter-subject analyses or linking the individual space with the reference space.Using tensor-based registration with an appropriate diffusion tensor template presents high accuracy regarding spatial normalization. However, there is a lack of a standardized diffusion tensor template dedicated to school-age children with ongoing brain development. Here,we established the school-age children diffusion tensor(SACT)template by optimizing tensor reorientation on high-quality DTI data from a large sample of cognitively normal participants aged 6-12 years. With an age-balanced design, the SACT template represented the entire age range well by showing high similarity to the age-specific templates.Compared with the tensor template of adults, the SACT template revealed significantly higher spatial normalization accuracy and inter-subject coherence upon evaluation of subjects in two different datasets of schoolage children.A practical application regarding the age associations with the normalized DTI-derived data was conducted to further compare the SACT template and the adult template.Although similar spatial patterns were found, the SACT template showed significant effects on the distributions of the statistical results, which may be related to the performance of spatial normalization.Looking forward, the SACT template could contribute to future studies of white matter development in both healthy and clinical populations. The SACT template is publicly available now(tp://igshare com/aricles/dataseu'SACT_.template/14071283).
文摘BACKGROUND Gender consciousness directly affects the development of gender identity, which is a continuous and lifelong process.Meanwhile, hospitalization is a part of many children's lives and has an impact on their gender development.AIM To investigate the current situation of gender identity in lower primary school children by conducting a survey of 202 hospitalized children in the lower grades and to provide a theoretical basis and foundation for the cultivation of gender identity and medical treatment of children based on the results. This study aims to inspire clinical medical staff to scientifically and reasonably arrange hospital wards for lower primary school children and pay attention to gender protection during the medical treatment process and to help children shape a unified and clear gender identity, which will enable them to better integrate into society and promote their personality development.METHODS The gender consciousness scale for elementary and middle school students was RESULTS Gender identity was already present in lower primary school children. The children's gender roles and gender equality consciousness were strong, exceeding the critical value, but their gender characteristics, gender identity, and gender ideal consciousness were weak.Children aged 6 had the weakest gender identity, and girls had significantly stronger gender identity than boys.CONCLUSION Gender identity is already present in lower primary school children, providing a basis and inspiration for the cultivation of gender identity and medical treatment of lower primary school children.Clinical medical staff should be aware of and understand these results and should scientifically and reasonably arrange hospital wards for lower primary school children.

文摘AIM:To compare the consistency of two autorefractors(Tianle RM-9000 and Topcon KR-800)for school-age myopia children, and to provide a basis for largescale data analysis and comparison.METHODS:The refractive error in 909 subjects(age 4–18y)were measured using both autorefractors without cycloplegia. The data were analyzed using Fourier decomposition and the correlation coefficients, intraclass correlation coefficients(ICC),and Bland-Altman limits of agreement(LoA)for each parameter were calculated.RESULTS:There was a strong correlation between the spherical equivalent(SE),sphere diopter(DS),and cylinder diopter(DC)readings of the Tianle RM-9000 and those of the Topcon KR-800,with correlation coefficient values of 0.98,0.98 and 0.83 and ICC values of 0.99,0.99 and 0.93,respectively.However,the correlation coefficients and ICC values of J0 and J45 were unreliable(R=-0.004,-0.034;both ICC<0.10).Bland-Altman analysis revealed that SE,DS,and DC measured by the Tianle RM-9000 were significantly biased toward myopia compared with the Topcon KR-800,and the mean differences were-0.072,-0.026,-0.091 D,respectively(all P<0.01).The minimum absolute value of the difference within the 95%LoA for SE,DS,and DC was 0.63 D,0.50 D,0.62 D,respectively;all these values were in the clinically acceptable range. For J0 and J45,the mean differences were close to zero(P=0.43,0.84);however,the 95%LoA were relatively wide(J0 SD:0.53;95%CI:-1.00,1.10;J45 SD:0.52;95%CI:-1.00,1.00).CONCLUSION:The two autorefractors are consistent with each other,as the differences in SE,DS,and DC were within the clinically acceptable range.Readers can compare the data measured by either device in different studies and use the two devices in the same study to generate a dataset that can be analyzed together. However, the J0 and J45 vectors are unreliable and should not be used to assess astigmatism.
文摘Background: Tungiasis is a skin infestation caused by the female sand flea called Tunga penetrans. Conclusion: The results of this study will inform control programs and water and sanitation interventions for the Tunga penetrans epidemic in Port Gentil, Gabon.

文摘Background:Effective communication between nurse and patient is paramount in establishing the relationship on which patient care is based and influences the quality of nursing care leading to improved health outcomes for patients. It is necessary for nurses to understand children’s perspectives of nurseepatient communication in order to choose appropriate communication skills to promote effective communication and a sweet relationship.Purpose:To explore the communication status of children who were in the early stages of living with leukemia and their perspectives of nurseepatient communication.Method:In-depth interviews were conducted with Chinese children with leukemia using a descriptive qualitative research method, and the data were analyzed using Krippendorf’s content analysis.Results:Twenty-nine children with leukemia were interviewed.Three communication states were identified among children in the first three months of living with leukemia:totally unwilling to communicate, reduced communication with others and increased communication with parents.Nurseepatient communication for children with leukemia formed three themes:content, form and occasion of communication. Conclusions:With a better understanding of children’s communication status and their expectations of communication, nurses can promote effective nurseepatient communication strategies to meet children’s psychological needs and build harmonious relationship.
文摘The aim of the study was to assess the effect of praziquantel (PZQ) treatment on hematuria, proteinuria and the status of eGFR following treatment in school children infected with S. mansoni. This suggests that praziquantel is effective, but there is still a need for integrated strategies to minimize reinfections.
文摘Background:Despite the ubiquity of polyparasitism, its health impacts have been inadequately studied. The aim of this study was to determine the prevalence and determinants of polyparasitism with Schistosoma haematobium,Plasmodium and soil-transmitted helminths(STH)following sustained control measures,as well as evaluate the outcomes and clinical correlates of infection in school-aged children(SAC)living in the schistosomiasis endemic focus of Muyuka-Cameroon.Methods:In a cross-sectional study, urine, blood and stool samples were each collected from SAC(4-14 years)selected at random between March and June 2015.Microhaematuria in urine was detected using reagent strip and 5.haematobium ova by filtration/microscopy methods.Plasmodium was detected using Giemsa-stained blood films and complete blood count was obtained using an auto-haematology analyser.STH in stool was detected by the Kato-Katz method.Categorical and continuous variables were compared as required,Kappa value estimated and the adjusted odds ratio(aOR)in the multivariate analysis was used to evaluate association of the risk factors with infection.
文摘Introduction: This research, conducted in the municipalities of Aguégués, Sô-Ava, and N’dali in Benin, focused on identifying socio-demographic and economic factors associated with urinary bilharzia in 415 school-aged children from 8 to 14 years. Conclusion: The findings suggest that these socio-demographic and economic factors impede the elimination of schistosomiasis in the examined areas.
文摘Background Few studies have investigated which factors were related to adolescents'social media use. This study aimed to evaluate which factors were associated with heavy social media use on weekdays and weekends among 13-year-old adolescents. Methods We analyzed data from 3727 children from the Generation R Study,a population-based cohort study in the Neth-erlands.Associations of demographic factors(child age, sex, ethnic background, and family situation),socioeconomic posi-tion(parental educational level, parental employment status, and net household income),screen-based behaviors(computer playing and TV viewing),and the home environment(communication, supervision, and restriction)with adolescents'heavy social media use(≥2 hours/day)were assessed separately on weekdays and weekends.Multivariate logistic regression analysis was applied. Results The prevalence of heavy social media use was 37.7%on a weekday and 59.6%on a weekend day.Being a girl,liv-ing in a one-parent family, and more time spent playing on the computer were associated with heavy social media use on weekdays and weekends(all P<0.05).Low socioeconomic position adolescents(low parental educational level and low household income)were more likely to show heavy social media use only on weekends(all P<0.05).Children whose social media use was restricted by parents on weekdays or children whose social media use was supervised by parents on weekends had lower odds of heavy social media use(all P<0.05).Conclusions Being a girl, living in a one-parent family,or having a longer computer playing time were associated with heavy social media use on weekdays and weekends.More studies are needed to understand the factors associated with heavy social media use and the impact of heavy social media use on child health.

文摘Background Tungiasis,a neglected tropical parasitosis, disproportionately afects children.Few empirical studies have reported neurocognitive and mental health outcomes of children with ectoparasitic skin diseases like tungiasis.Pathophysiology of tungiasis suggests it could detrimentally afect cognition and behaviour. This study pioneered the investigation of neurocognitive and mental health outcomes in children with tungiasis. Methods This was a multi-site cross-sectional study including 454 quasi-randomly sampled school-children aged 8-14 from 48 randomly selected schools in two counties in Kenya and a district in Uganda. The participants were stratifed into infected and uninfected based on the presence of tungiasis. The infected were further classifed into mild and severe infection groups based on the intensity of the infection.Adapted, validated, and standardized measures of cognition and mental health such as Raven Matrices and Child Behaviour Checklist were used to collect data.Statistical tests including a multilevel, generalized mixed-efects linear models with family link set to identity were used to compare the scores of uninfected and infected children and to identify other potential risk factors for neurocognitive and behavioural outcomes. Results When adjusted for covariates, mild infection was associated with lower scores in literacy[adjustedβ(aβ)=−8.9;95%confdence interval(CI)−17.2,−0.6],language(aβ=−1.7;95%CI−3.2,−0.3),cognitive fexibility(aβ=−6.1;95%CI−10.4,−1.7)and working memory(aβ=−0.3;95%CI−0.6,−0.1).Severe infection was associated with lower scores in literacy(aβ=−11.0;95%CI−19.3,−2.8),response inhibition,(aβ=−2.2;95%CI−4.2,−0.2),fne motor control(aβ=−0.7;95%CI−1.1,−0.4)and numeracy(aβ=−3;95%CI−5.5,−0.4).Conclusions This study provides frst evidence that tungiasis is associated with poor neurocognitive functioning in children.Since tungiasis is a chronic disease with frequent reinfections, such negative efects may potentially impair their development and life achievements.

文摘Background:Schistosomiasis is a water-based disease transmitted by trematodes belonging to the genus Schistosoma. The aim of this study was to assess the relationship between the prevalence of schistosomiasis and access to water, sanitation and hygiene(WASH)and environmental and socioeconomic factors in the city of Korhogo, northern Côte d'Ivoire. Methods:A cross-sectional study including 728 randomly selected households was conducted in Korhogo in March 2015.The heads of the households were interviewed about access to WASH and environmental and socioeconomic factors. All children abed between 5 and 15 years living in the households were selected to provide stool and urine samples for parasitological diagnosis of Schistosoma mansoni and Schistosoma haematobium infection. The relationship between infection with S.mansoni and potential risk factors was analysed by a mixed logistic regression model with‘household’as a random factor.Likelihood ratio tests were used to identify factors that were significantly associated with a Schistosoma spp.infection.Results:The overall prevalence of schistosomiasis among school-aged children in Korhogo was 1.9%(45/2341)composed of 0.3%(3/1248)S.haematobium and 3.5%(42/1202)S.mansoni.Due to the low prevalence of S.haematobium infection, risk factor analysis was limited to S.mansoni.Boys were 7.8 times more likely to be infected with S.mansoni than girls.Children between 10 and 15 years of age were 3.8 times more likely to be infected than their younger counterparts aged 5-10 years. Moreover, living in a house further away from a water access point(odds ratio[OR]=0.29,95%confidence interval[CI]:0.13-0.70)and abstaining from swimming in open freshwater bodies(OR=0.16,95%CI:0.04-0.56)were significantly associated with decreased odds of S.mansoni infection. The socioeconomic status did not appear to influence the prevalence of S.mansoni.Conclusions:A strategy to reduce the incidence of schistosomiasis should focus on health education to change the behaviour of populations at risk and encourage communities to improve sanitation and infrastructure in order to reduce contact with surface water.
