Objective: To systematically evaluate the safety and efficacy of traditional Chinese medicine (TCM) in the treatment of chronic cough. Methods: Databases CNKI, WanFang Data, VIP, CBM, PubMed, Web of Science, Cochrane ...Objective: To systematically evaluate the safety and efficacy of traditional Chinese medicine (TCM) in the treatment of chronic cough. Methods: Databases CNKI, WanFang Data, VIP, CBM, PubMed, Web of Science, Cochrane Library from inception to February 2021 were electronically searched by two researchers, to collect randomized controlled trials (RCTs) of traditional Chinese medicine in the treatment of chronic cough, UACS/PNDS, CVA, EB, AC, GERD and other types of cough. Revman 5.3 software was used to analyze and summarize the collected literature, and the methodological quality of the included studies was evaluated by the bias risk assessment scale of Cochrance Collaboration Network. Results: A total of 26 articles involving 2 820 patients, with 1 420 in the experimental group and 1 400 in the control group were included. The results of meta-analysis showed that compared to the control group which was treated with western medicine alone, TCM significantly improved the clinical effective rate [RR=1.20, 95% CI(1.16,1.25), P<0.000 01], alleviated cough syndrome score [SMD=-0.83, 95% CI(-1.21,-0.44)], and enhenced the score of quality of life [MD=-2.18, 95%CI(1.63,2.74), P<0.000 01];at the same time, the experimental group was better than the control group in improving lung function and reducing the time of cough relief. In terms of safety, a total of 24 patients in 5 literatures had adverse reactions, but the symptoms were mild and could be relieved quickly. In addition, there was no significant difference in adverse reactions between the control group and the experimental group, showing good safety. Conclusion: TCM treatment can significantly improve the cure rate of cough, reduce cough score, improve systemic symptoms, relieve psychological anxiety, improve lung function, and improve cough symptoms rapidly, with good safety.展开更多

METHODS:Effects of ASHMI and its components aqueous extracts of Lingzhi(Ganoderma lucidum),Kushen(Sophora flavescens) and Gancao(Glycyrrhiza uralensis),on Th2 cytokine secretion by murine memory Th2 cells(D10.G4.1) and eotaxin-1 secretion by human lung fibroblast(HLF-1) cells were determined by measuring levels in culture supernatants by enzymelinked immunosorbent assay.Potential synergistic effects were determined by computing interaction indices from concentration-effect curve parameters. RESULTS:Individual Lingzhi,Kushen and Gancao extracts and ASHMI(the combination of individual extracts) inhibited production of interleukin(IL)-4 and IL-5 by murine memory Th2 cells and eotaxin-1 production by HLF-1 cells.The mean 25%-inhibitory-concentration(IC25) values(mg/mL) for ASHMI,Lingzhi,Kushen and Gancao for IL-4 production were 30.9,79.4, 123,and 64.6,respectively;for IL-5 production were 30.2,263,123.2 and 100,respectively; for eotaxin-1 were 13.2,16.2,30.2,and 25.1,respectively.The IC50 values(mg/mL) for ASHMI,Lingzhi,Kushen and Gancao for IL-4 production were 158.5,239.9,446.7,and 281.8, respectively;for eotaxin-1 were 38.1,33.1,100,and 158.5,respectively.The interaction indices of ASHMI constituents at IC25 were 0.35 for IL-4,0.21 for IL-5 and 0.59 for eotaxin-1.The interaction indices at IC50 values were 0.50 for IL-4 and 0.62 for eotaxin-1 inhibition.Inhibition of IL-5 did not reach IC50 values.All interaction indices were below 1 which indicated synergy. A total of 60 participants, aged 2 to 5, were randomized into either the treatment group(n = 40) or the placebo group(n = 20). The treatment group was treated with granules of TCM herbs for tonifying Qi and kidney, and replenishing spleen, and the placebo group was given placebo granules for 3 months.The number of asthma attacks was counted and TCM syndrome scores were measured at 1, 2, 3, 6, 9,and 12 months. The airway resistance and levels of eosinophil cationic protein for the two groups were observed before and after 3 months. The results were statistically analyzed.RESULTS: Compared with the placebo group, the number of asthma attacks significantly decreased in the treatment group(P < 0.05). For the treatment group, the TCM syndrome scores decreased after 1 and 2 months; there was also a significant difference in scores between the two groups(P < 0.05).The difference remained after the medicine was stopped for 9 months(P < 0.05). After the 3-month treatment, compared with the placebo group, the airway resistance decreased in the treatment group(P < 0.05). No adverse events were reported in the treatment group.CONCLUSION: The TCM herbs for tonifying Qi and kidney, and replenishing spleen reduced the number of intermittent asthma attacks, decreased the TCM syndrome scores, and reduced the airway resistance in the children aged 2 to 5.展开更多
