Prostate cancer(PCa)is one of the most common malignant tumors in the male genitourinary system,ranking second in incidence worldwide.Traditional Chinese medicine(TCM),as an important component of complementary and alternative medicine,shows unique advantages in cancer treatment.Chinese herbal medicine is usually composed of multiple ingredients and involves multiple signaling pathways,which showed function of inducing apoptosis of cancer cells,arresting the cell cycle,inhibiting invasion and metastasis,reducing drug resistance,and regulating immune function.Physical therapy is also an important treatment of TCM.Currently,Physical therapy such as acupuncture or Tai Chi and Qigong are gaining increased recognition in the management of PCa,particularly in addressing issues like urinary incontinence and bone metastasis-related pain.This article reviews the TCM treatment and therapy of PCa,in order to provide new research avenues and treatment options for the treatment of PCa with TCM and improve the quality of life of patients. 展开更多

Literature study on traditional Chinese medicine syndromes after renal transplantation

BACKGROUND Kidney transplantation is the most effective means to treat patients with renal failure,but its postoperative problems such as rejection reactions,immunosuppressant poisoning,chronic transplant kidney nephropathy,etc.still have not been effectively solved.This study searched for literature on traditional Chinese medicine(TCM)syndromes after kidney transplantation in China,conducted statistical analysis of the results,and sought to identify the underlying patterns.AIM To understand the TCM syndromes after renal transplantation and associated rules and provide a theoretical basis for further clinical research.METHODS The literature pertaining to TCM syndromes in renal transplantation,published in the China National Knowledge Infrastructure,Wanfang database,and WIP database from 1970 to 2021,was meticulously searched and comprehensively and statistically analyzed.RESULTS Following the established inclusion and exclusion criteria,13 studies were selected for analysis.Post-renal transplantation,no significant discrepancy was noted among the groups based on the location of TCM viscera.However,when categorized according to TCM pathogenic factors,the groups with spleen and kidney yang deficiency,as well as liver and kidney yin deficiency,exhibited a statistically significant difference in the frequency.CONCLUSION Currently,the research on TCM syndromes pertaining to renal transplantation is in its nascent phase.It is imperative to conduct a multicentric,large-scale survey of TCM syndromes subsequent to renal transplantation in the ensuing years. 展开更多

Objective To improve the accuracy and professionalism of question-answering(QA)model in traditional Chinese medicine(TCM)lung cancer by integrating large language models with structured knowledge graphs using the knowledge graph(KG)to text-enhanced retrievalaugmented generation(KG2TRAG)method.Methods The TCM lung cancer model(TCMLCM)was constructed by fine-tuning Chat-GLM2-6B on the specialized datasets Tianchi TCM,HuangDi,and ShenNong-TCM-Dataset,as well as a TCM lung cancer KG.The KG2TRAG method was applied to enhance the knowledge retrieval,which can convert KG triples into natural language text via ChatGPT-aided linearization,leveraging large language models(LLMs)for context-aware reasoning.For a comprehensive comparison,MedicalGPT,HuatuoGPT,and BenTsao were selected as the baseline models.Performance was evaluated using bilingual evaluation understudy(BLEU),recall-oriented understudy for gisting evaluation(ROUGE),accuracy,and the domain-specific TCM-LCEval metrics,with validation from TCM oncology experts assessing answer accuracy,professionalism,and usability.Results The TCMLCM model achieved the optimal performance across all metrics,including a BLEU score of 32.15%,ROUGE-L of 59.08%,and an accuracy rate of 79.68%.Notably,in the TCM-LCEval assessment specific to the field of TCM,its performance was 3%−12%higher than that of the baseline model.Expert evaluations highlighted superior performance in accuracy and professionalism.Conclusion TCMLCM can provide an innovative solution for TCM lung cancer QA,demonstrating the feasibility of integrating structured KGs with LLMs.This work advances intelligent TCM healthcare tools and lays a foundation for future AI-driven applications in traditional medicine. 展开更多

In this editorial,we comment on the article by Micucci et al published in the recent issue.We focus on the heterogenous nature of gastric cancer(GC)and the potential benefits of integrating traditional Chinese medicine(TCM)with the modern technology of network pharmacology(NP)and omics sequencing.GC is a heterogenous disease,as it incorporates several biochemical pathways that contribute to pathogenesis.TCM acknowledges the multifactorial,heterogenous nature of disease and utilizes an integrative approach to medicine.NP,a modern philosophy within drug development,integrates traditional knowledge of nutraceuticals and modern technologies to address the complex interactions of pathways within the body.Omics technologies,which is at the core of precision medicine,has allowed for this newfound principle of drug development.Metabolic pathways are better distinguished,leading to more targeted drug development.However,the use of omics technology needs to be employed to better characterize the subtypes of GC.This will allow TCM’s use of nutraceuticals in the application of NP to better target metabolic pathways that may aid in the prevention of GC as well as enhance treatment. 展开更多

Harmonizing the mind and body:The interrelationship between traditional Chinese medicine body constitution,mental health and sleep quality

BACKGROUND Traditional Chinese medicine(TCM)body constitution is a key factor in determining and influencing the occurrence,development,and variation of diseases.As the mind interact with the body,TCM body constitution is useful in preventing and treating psychiatric disorders and insomnia.AIM To investigate the interrelationship between TCM body constitution,psychiatric disorders and sleep quality.METHODS A total of 2441 participants were enrolled in our study.TCM body constitution was measured using the Constitution in Chinese Medicine Questionnaire,sleep quality was assessed using the Chinese version of the Pittsburgh Sleep Quality Index,and psychopathological symptoms were evaluated using Symptom Checklist-90.Logistic regression was used to estimate the interrelationship between TCM body constitution,psychiatric disorders and sleep quality.RESULTS The Pittsburgh Sleep Quality Index score and the proportions of participants with Qi-deficiency constitution,Qi-stagnation constitution,and blood-stasis constitution were significantly higher in the psychiatric symptoms group.Additionally,psychiatric symptoms were considerably associated with sleep quality,Qi-deficiency constitution,and Qi-stagnation constitution,but were inversely associated with male sex.Obsessive–compulsive symptoms were considerably associated with sleep quality,Qi-deficiency constitution and Qi-stagnation constitution.Depression was considerably associated with sleep quality,hyperuricemia,and Qi-stagnation constitution.Anxiety was considerably associated with sleep quality,diabetes,and Qi-stagnation constitution.Finally,poor sleep quality was considerably associated with Qi-deficiency constitution,Qi-stagnation constitution,dampness-heat constitution,phlegm-dampness constitution,and blood-stasis constitution but was inversely associated with male sex and body mass index>23.9.CONCLUSION TCM body constitutions are strongly associated with psychiatric symptoms and sleep quality.Further research is needed to verify whether TCM body constitution can be used to treat and prevent psychiatric conditions as well as enhance sleep quality. 展开更多

Traditional Chinese medicine(TCM)has demonstrated unique advantages in the prevention and treatment of chronic diseases such as glycolipid metabolism disorder.However,its widespread application has been hindered by the unclear biological essence of TCM syndromes and therapeutic mechanisms.As an emerging interdisciplinary field,phenomics integrates multi-dimensional data including genome,transcriptome,proteome,metabolome,and microbiome.When combined with TCM's holistic philosophy,it forms TCM phenomics,providing novel approaches to reveal the biological connotation of TCM syndromes and the mechanisms of herbal medicine.Taking glycolipid metabolism disorder as an example,this paper explores the application of TCM phenomics in glycolipid metabolism disorder.By analyzing molecular characteristics of related syndromes,TCM phenomics identifies differentially expressed genes,metabolites,and gut microbiota biomarkers to elucidate the dynamic evolution patterns of syndromes.Simultaneously,it deciphers the multi-target regulatory networks of herbal formulas,demonstrating their therapeutic effects through mechanisms including modulation of insulin signaling pathways,improvement of gut microbiota imbalance,and suppression of inflammatory responses.Current challenges include the subjective nature of syndrome diagnosis,insufficient standardization of animal models,and lack of integrated multi-omics analysis.Future research should employ machine learning,multimodal data integration,and cross-omics longitudinal studies to establish quantitative diagnostic systems for syndromes,promote the integration of precision medicine in TCM and western medicine,and accelerate the modernization of TCM. 展开更多

Objective:Generative artificial intelligence(AI)technology,represented by large language models(LLMs),has gradually been developed for traditional Chinese medicine(TCM);however,challenges remain in effectively enhancing AI applications for TCM.Therefore,this study is the first systematic review to analyze LLMs in TCM retrospectively,focusing on and summarizing the evidence of their performance in generative tasks.Methods:We extensively searched electronic databases for articles published until June 2024 to identify publicly available studies on LLMs in TCM.Two investigators independently selected and extracted the related information and evaluation metrics.Based on the available data,this study used descriptive analysis for a comprehensive systematic review of LLM technology related to TCM.Results:Ten studies published between 2023 and 2024 met our eligibility criteria and were included in this review,including 40%LLMs in the TCM vertical domain,40%containing TCM data,and 20%honoring the TCM contribution,with a foundational model parameter range from 1.8 to 33 billion.All included studies used manual or automatic evaluation metrics to evaluate model performance and fully discussed the challenges and contributions through an overview of LLMs in TCM.Conclusions:LLMs have achieved significant advantages in TCM applications and can effectively address intelligent TCM tasks.Further in-depth development of LLMs is needed in various vertical TCM fields,including clinical and fundamental research.Focusing on the functional segmentation development direction of generative AI technologies in TCM application scenarios to meet the practical needs-oriented demands of TCM digitalization is essential. 展开更多

Objective The study of medicine formulas is a core component of traditional Chinese medicine(TCM),yet traditional learning methods often lack interactivity and contextual understanding,making it challenging for beginners to grasp the intricate composition rules of formulas.To address this gap,we introduce Formula-S,a situated visualization method for TCM formula learning in augmented reality(AR)and evaluate its performance.This study aims to evaluate the effectiveness of Formula-S in enhancing TCM formula learning for beginners by comparing it with traditional text-based formula learning and web-based visualization.Methods Formula-S is an interactive AR tool designed for TCM formula learning,featuring three modes(3D,Web,and Table).The dataset included TCM formulas and herb properties extracted from authoritative references,including textbook and the SymMap database.In Formula-S,the hierarchical visualization of the formulas as herbal medicine compositions,is linked to the multidimensional herb attribute visualization and embedded in the real world,where real herb samples are presented.To evaluate its effectiveness,a controlled study(n=30)was conducted.Participants who had no formal TCM knowledge were tasked with herbal medicine identification,formula composition,and recognition.In the study,participants interacted with the AR tool through HoloLens 2.Data were collected on both task performance(accuracy and response time)and user experience,with a focus on task efficiency,accuracy,and user preference across the different learning modes.Results The situated visualization method of Formula-S had comparable accuracy to other methods but shorter response time for herbal formula learning tasks.Regarding user experience,our new approach demonstrated the highest system usability and lowest task load,effectively reducing cognitive load and allowing users to complete tasks with greater ease and efficiency.Participants reported that Formula-S enhanced their learning experience through its intuitive interface and immersive AR environment,suggesting this approach offers usability advantages for TCM education.Conclusions The situated visualization method in Formula-S offers more efficient and accurate searching capabilities compared to traditional and web-based methods.Additionally,it provides superior contextual understanding of TCM formulas,making it a promising new solution for TCM learning. 展开更多

[Objectives]This study was conducted to analyze the medication laws of traditional Chinese medicine prescriptions for the treatment of spleen-deficiency irritable bowel syndrome in Guanling Autonomous County Hospital of Traditional Chinese Medicine.[Methods]Prescriptions for the treatment of spleen-deficiency irritable bowel syndrome(IBS)were retrieved from the TCM family of the hospital,traditional Chinese medical doctor Wu Zhongli,in the period from November 2023 to April 2024.Microsoft Excel 2007 was employed to set up an information table of TCM prescriptions,and the age,gender,herbal properties,efficacy categories and the frequency of use were analyzed to explore the medication laws of TCM in the hospital for the treatment of spleen-deficiency irritable bowel syndrome.[Results]Among the 259 TCM prescriptions included,152 kinds of TCM decoction pieces were used.The decoction pieces were mainly warm in nature,and decoction pieces cold in nature took the second place.The flavors of the herbs were mostly sweet,bitter and pungent.Most of them were attributive to the spleen,stomach meridian and lung meridians,and the herbs were mainly used for tonifying deficiency and regulating qi.The herbs with higher frequency of use included Radix Glycyrrhiza,Pericarpium Citri Reticulatae,poria,and Angelicae Sinensis Radix,the main effects of which are replenishing qi to invigorate the spleen,activating qi and eliminating phlegm,clearing damp and promoting diuresis,and relaxing bowel.[Conclusions]Chinese medicine treatment of IBS with spleen deficiency in hospitals is mainly based on replenishing qi to invigorate the spleen,activating qi and eliminating phlegm,clearing damp and promoting diuresis,and relaxing bowel,and Xiangsha Liujunzi Decoction is commonly used in clinical treatment based on syndrome differentiation with modifications. 展开更多

Digitization is the inevitable path for the natural development of traditional Chinese medicine(TCM)in the context of the Fourth Industrial Revolution.The goal of TCM digitization is to generate intelligence from numbers.Originating from the reasoning paradigm of Xiangshu(象数,image-number)or phenotype-numerology thinking,TCM came with a deep correlation of clinical observations with digits and laid a strong theoretical basis for digitization.The digitization of TCM should start from the clinical aspect,solve the problem of electronic medical records,achieve standardization and informatization,and on this basis,form a TCM knowledge base through knowledge-building.This process depends on the combined efforts of multiple disciplines such as medicine,mathematics,and engineering to achieve the digitization and intelligent transformation of TCM.This era calls for TCM to break down barriers,embrace opportunities,and move towards digitization.However,during the transformation,it should maintain its essence,avoid simplistic conversions,be guided by scientific value,leverage cutting-edge technologies,and enhance the depth and breadth of the interpretation of TCM connotations.The digitization of TCM will also improve its service capabilities,create an innovative digitally-intelligent TCM service platform,and contribute to the development of“Healthy China”initiatives with wisdom and solutions. 展开更多
