Objective:To investigate the effect of Yiqi Huoxue decoction combined with neuromuscular electrical stimulation on improving intensive care unit(ICU)acquired debility in mechanically ventilated patients.Methods:50 pat...Objective:To investigate the effect of Yiqi Huoxue decoction combined with neuromuscular electrical stimulation on improving intensive care unit(ICU)acquired debility in mechanically ventilated patients.Methods:50 patients who were admitted to the ICU and received mechanical ventilation treatment in our hospital from June 2022 to June 2023 and were complicated with ICU-acquired neurasthenia were selected,and randomly grouped using the randomized envelope method into two groups:control group with 25 patients who received neuromuscular electrical stimulation alone;observation group with 25 patients who received the traditional Chinese medicine Yiqi Huoxue decoction.Comparison indexes:treatment efficiency,degree of emotional recovery(APACHEⅡscore),muscle strength status(MRC score),motor status(FAC rating),and self-care ability(BI index score).Results:The treatment efficiency of patients in the observation group patients was higher as compared to those in the control group(P<0.05).There was no significant difference in the comparison of the results of the scores(ratings)of each index between the two groups before treatment(P>0.05).After the treatment,the APACHEⅡscores of patients in the observation group were significantly lower as compared to those in the control group,while the MRC scores,FAC ratings,and BI index scores were higher in the observation group than those of the control group patients(P<0.05).Conclusion:The combined application of Yiqi Huoxue decoction and neuromuscular electrical stimulation in the treatment of patients with ICU-acquired neurasthenia complicated by mechanical ventilation significantly enhanced the clinical efficacy,the patient’s muscle strength,motor status,and ability of self-care.Hence,it has high application value and is worthy to be popularized.展开更多
BACKGROUND Early diabetic nephropathy(DN)is a complication of diabetes mellitus.It mainly affects kidney microvessels and glomerular function,and its timely and effective treatment is critical for early DN.However,the...BACKGROUND Early diabetic nephropathy(DN)is a complication of diabetes mellitus.It mainly affects kidney microvessels and glomerular function,and its timely and effective treatment is critical for early DN.However,the effects of treatments comprising simple Western medicine are not optimal.With the promotion and implementation of integrated Chinese and western medicine treatments,remarkable results have been achieved for many diseases.To this end,we explored the clinical efficacy of integrated traditional Chinese and western medicines for the treatment of early DN.AIM To investigate the effect of sitagliptin tablets combined with Yiqi yangyin huoxue decoction on clinical efficacy and hemorheology in patients with early DN.METHODS Through a retrospective analysis,123 patients with early DN were admitted to the endocrinology clinic of the Changzhou NO.7 People’s Hospital from January 2021 to October 2022 and were selected as study subjects.After rigorous screening,100 patients with early DN were enrolled.The control group(CG,n=50)and the observation group(OG,n=50)were divided according to the treatment method.The CG were treated with sitagliptin,and the OG were treated with sitagliptin plus the Yiqi yangyin huoxue decoction.Both groups were treated for 3 mo.For both groups,the baseline data and clinical efficacy were compared,and changes in blood glucose levels,lipid levels,renal function,and hematological indicators before(T0)and after(T1)treatment were assessed.RESULTS The total effective rate for the OG was 94.00%and that of the CG was 80.00%(P<0.05).After treatment(T1),the levels of fasting blood glucose,2 h postprandial glucose,total cholesterol,triacylglycerol,and low-density lipoprotein cholesterol in OG patients were obviously lower than those in the CG(P<0.05),and cystatin C,homocysteine,urinary microalbumin,and blood creatinine values in OG patients were also obviously lower than those in the CG(P<0.05);erythrocyte deposition,plasma viscosity,whole blood high shear viscosity,and whole blood low shear viscosity were markedly lower in OG patients than in the CG(P<0.05).CONCLUSION Sitagliptin combined with Yiqi yangyin huoxue decoction has a remarkable effect when used to treat patients with early DN.Further,it is helpful in improving hemorheological indices and controlling disease progression.展开更多

Background:In myocardial ischemia,hypoxia leads to destruction of the cytoskeleton,and especially the imbalance of microtubule polymerization-depolymerization,which seriously affects the structure and function of card...Background:In myocardial ischemia,hypoxia leads to destruction of the cytoskeleton,and especially the imbalance of microtubule polymerization-depolymerization,which seriously affects the structure and function of cardiomyocytes.We previously showed that a Yiqi Huoxue Decoction (YQHX) improves mitochondrial function and decreases anti-oxidative effects in hypoxia-induced H9c2 cell injury.Therefore,in this study we investigated whether YQHX protects against hypoxia-induced damage by decreasing damage to the cardiac cytoskeleton.Methods:After reaching 70%-80% confluence,H9c2 cells were synchronized in serum-free Dulbecco's Modified Eagle Medium for 6 hours,then divided into control,model,and YQHX (100,200,400 μg/mL) groups,which were then grown in a hypoxic atmosphere for 12 hours.Cardiac cell viability was assessed using an xCELLigence system.The levels of lactate dehydrogenase,maleic dialdehyde,and superoxide dismutase in H9c2 cell supernatants were measured.Hoechst 33258 staining was employed to observe cardiac cell apoptosis.Confocal microscopy,immunofluorescence,and western blot analysis were performed to evaluate the protective effects of the YQHX against hypoxia-induced injury in the H9c2 cell line.Results:Cells that were pretreated with YQHX were more able to maintain their microtubule structure in the early stages of hypoxia and had better myocardial fitness in response to hypoxia compared with cells that were not pretreated.However,hypoxia-induced upregulation of α-tubulin and β-tubulin expression antagonized the protective effect of YQHX (100 μg/mL).In addition,YQHX (100 μg/mL) treatment significantly upregulated MAP4 protein expression (P =.003) and downregulated p-AMPKα protein expression (P <.001) compared with the model group.Conclusion:The results indicate that YQHX plays a role in protecting against oxidative stress injury and apoptosis in H9c2 cells.Notably,our results suggested that the YQHX could mitigate the damage to the cardiac cytoskeleton and the dysregulation of AMPK-related protein signaling pathways that are induced by hypoxia.展开更多

文摘目的探讨活血通络汤在急性缺血性脑卒中(acute ischemic stroke,AIS)患者治疗中的效果及对其对中医证候、神经功能的影响。方法选取82例六安市中医院2021年1月—2022年1月收治的AIS患者作为研究对象,将其分为观察组(41例)和对照组(41例)(随机数字表法)。对照组接受常规西医治疗,观察组在对照组基础上接受活血通络汤治疗。两组治疗时间均为1个月。对两组治疗前后的美国国立卫生研究院卒中量表(National Institute of Health stroke scale,NIHSS)、蒙特利尔认知评估(Montreal Cognitive Assessment,MoCA)评分、中医证候积分、生活质量评分、全血高切黏度、全血低切黏度、血浆纤维蛋白原(fibrinogen,FIB)及D-二聚体(D-dimer,D-D)水平、血管内皮功能及治疗后的临床疗效进行比较。结果与观察组比较,对照组治疗后临床总有效率较低;治疗后两组中医证候各项评分、NIHSS评分、全血高切黏度、全血低切黏度、血浆FIB、D-D水平、血清内皮素-1(endothelin-1,ET-1)、血管性假血友病因子(von Willebrand factor,vWF)水平与治疗前比均降低,其中观察组低于对照组;治疗后两组生活质量评分、MoCA评分、血清一氧化氮(nitric oxide,NO)、血管舒张功能(flow-mediated dilation,FMD)水平与治疗前比均升高,组间进行比较,对照组较低(P<0.05)。结论活血通络汤应用于AIS患者可有效改善中医证候,利于神经功能和认知功能恢复,提高生活质量,调节血液状态和血管内皮功能,疗效显著。
文摘目的 探讨益气养阴消癥通络汤治疗早期糖尿病肾病(DN)的临床效果。方法 64例早期糖尿病肾病患者,随机分为对照组和观察组,每组32例。对照组使用常规西药治疗,在此基础上,观察组使用益气养阴消癥通络汤治疗。比较两组临床疗效,血糖及肾功能指标[空腹血糖(FBG)、餐后2 h血糖(2 h PG)、糖化血红蛋白(HbA1c)、血肌酐(SCr)、尿素氮(BUN)、尿白蛋白排泄率(UAER)],中医证候积分,血液流变学指标[血浆粘度、全血高切粘度、全血低切粘度、纤维蛋白原(FIB)、D-二聚体(D-D)]。结果 观察组治疗总有效率为93.75%,明显高于对照组的75.00%(P<0.05)。观察组治疗后FBG、2 h PG、HbA1c、BUN、Scr、UAER分别为(6.35±0.90)mmol/L、(8.97±1.25)mmol/L、(6.71±0.86)%、(5.95±1.04)mmol/L、(120.73±17.68)μmol/L、(75.70±10.67)μg/min,均低于对照组的(7.84±1.21)mmol/L、(10.96±1.78)mmol/L、(8.10±0.93)%、(8.67±1.39)mmol/L、(135.06±19.28)μmol/L、(109.31±12.75)μg/min(P<0.05)。观察组治疗后咽干口渴、手足心热、神疲乏力、腰痛膝软、肢体水肿等中医证候积分分别为(0.98±0.16)、(0.92±0.15)、(1.01±0.21)、(0.96±0.18)、(0.95±0.20)分,均明显低于对照组的(1.53±0.21)、(1.49±0.20)、(1.75±0.24)、(1.50±0.23)、(1.48±0.23)分(P<0.05)。观察组治疗后血浆粘度、全血高切粘度、全血低切粘度、FIB、D-D分别为(1.23±0.21)mPa·s、(3.67±0.68)mPa·s、(8.74±1.36)mPa·s、(2.62±0.37)g/L、(1.90±0.35)μg/ml,均低于对照组的(1.88±0.25)mPa·s、(5.14±0.79)mPa·s、(11.08±1.93)mPa·s、(3.29±0.45)g/L、(2.68±0.40)μg/ml(P<0.05)。结论 益气养阴消癥通络汤治疗早期糖尿病肾病的临床效果显著,能提高血糖控制效果,改善肾功能,降低血液粘稠度,缓解证候。
