Analysis on the Effect of Yiqi Huoxue Decoction Combined with Neuromuscular Electrical Stimulation in Improving ICU-Acquired Debility in Mechanically Ventilated Patients

Objective:To investigate the effect of Yiqi Huoxue decoction combined with neuromuscular electrical stimulation on improving intensive care unit(ICU)acquired debility in mechanically ventilated patients.Methods:50 patients who were admitted to the ICU and received mechanical ventilation treatment in our hospital from June 2022 to June 2023 and were complicated with ICU-acquired neurasthenia were selected,and randomly grouped using the randomized envelope method into two groups:control group with 25 patients who received neuromuscular electrical stimulation alone;observation group with 25 patients who received the traditional Chinese medicine Yiqi Huoxue decoction.Comparison indexes:treatment efficiency,degree of emotional recovery(APACHEⅡscore),muscle strength status(MRC score),motor status(FAC rating),and self-care ability(BI index score).Results:The treatment efficiency of patients in the observation group patients was higher as compared to those in the control group(P<0.05).There was no significant difference in the comparison of the results of the scores(ratings)of each index between the two groups before treatment(P>0.05).After the treatment,the APACHEⅡscores of patients in the observation group were significantly lower as compared to those in the control group,while the MRC scores,FAC ratings,and BI index scores were higher in the observation group than those of the control group patients(P<0.05).Conclusion:The combined application of Yiqi Huoxue decoction and neuromuscular electrical stimulation in the treatment of patients with ICU-acquired neurasthenia complicated by mechanical ventilation significantly enhanced the clinical efficacy,the patient’s muscle strength,motor status,and ability of self-care.Hence,it has high application value and is worthy to be popularized. 展开更多

BACKGROUND Early diabetic nephropathy(DN)is a complication of diabetes mellitus.It mainly affects kidney microvessels and glomerular function,and its timely and effective treatment is critical for early DN.However,the effects of treatments comprising simple Western medicine are not optimal.With the promotion and implementation of integrated Chinese and western medicine treatments,remarkable results have been achieved for many diseases.To this end,we explored the clinical efficacy of integrated traditional Chinese and western medicines for the treatment of early DN.AIM To investigate the effect of sitagliptin tablets combined with Yiqi yangyin huoxue decoction on clinical efficacy and hemorheology in patients with early DN.METHODS Through a retrospective analysis,123 patients with early DN were admitted to the endocrinology clinic of the Changzhou NO.7 People’s Hospital from January 2021 to October 2022 and were selected as study subjects.After rigorous screening,100 patients with early DN were enrolled.The control group(CG,n=50)and the observation group(OG,n=50)were divided according to the treatment method.The CG were treated with sitagliptin,and the OG were treated with sitagliptin plus the Yiqi yangyin huoxue decoction.Both groups were treated for 3 mo.For both groups,the baseline data and clinical efficacy were compared,and changes in blood glucose levels,lipid levels,renal function,and hematological indicators before(T0)and after(T1)treatment were assessed.RESULTS The total effective rate for the OG was 94.00%and that of the CG was 80.00%(P<0.05).After treatment(T1),the levels of fasting blood glucose,2 h postprandial glucose,total cholesterol,triacylglycerol,and low-density lipoprotein cholesterol in OG patients were obviously lower than those in the CG(P<0.05),and cystatin C,homocysteine,urinary microalbumin,and blood creatinine values in OG patients were also obviously lower than those in the CG(P<0.05);erythrocyte deposition,plasma viscosity,whole blood high shear viscosity,and whole blood low shear viscosity were markedly lower in OG patients than in the CG(P<0.05).CONCLUSION Sitagliptin combined with Yiqi yangyin huoxue decoction has a remarkable effect when used to treat patients with early DN.Further,it is helpful in improving hemorheological indices and controlling disease progression. 展开更多

Objective:Yiqi Huoxue Decoction (YQHX) has been widely used for clinical treatment of ischemic heart disease.While oxidative stress plays a key role in the pathogenesis of ischemic heart disease,the function and molecular mechanism underlying antioxidative protective effects of YQHX on H9c2 cardiomyocytes against ischemia/hypoxia (I/H) have yet to be well clarified.Methods:H9c2 cells were subjected to 12 h of hypoxia with serum-free conditions and then treated with or without YQHX (100-400 μg/mL).Cell viability was examined using a CCK-8 assay.Maleic dialdehyde (MDA) and superoxide dismutase (SOD) activity were detected using commercial kits.Intracellular reactive oxygen species (ROS) levels and mitochondrial membrane potential were measured using fluorescence microscopy and confocal laser-scanning microscopy,respectively.Ultrastructural details of mitochondria in H9c2 cells were observed using transmission electron microscopy.The antioxidative protective pathway was assessed by measuring mRNA and protein expression of Nrf2 and HO-1,as well as AMPK activation.Results:I/H injury gradually induced oxidative stress.Treatment with YQHX significantly increased cell viability and reversed I/H-induced oxidative stress,including reducing the production of oxidative stress products (ROS and MDA),increasing SOD levels,improving mitochondrial morphology,and increasing mitochondrial membrane potential.YQHX was also observed to increase I/H-induced expression of Nrf2 and HO-1,and the activation effects of YQHX were blocked by an AMPK inhibitor.In addition,HPLC analysis showed that YQHX contained two active antioxidative constituents (calycosin and ferulic acid).Conclusion:The results suggest that anti-oxidative effects exerted by YQHX in H9c2 cardiomyocytes may be linked to upregulation of the AMPK-mediated Nrf2/HO-1 pathway. 展开更多

Background:In myocardial ischemia,hypoxia leads to destruction of the cytoskeleton,and especially the imbalance of microtubule polymerization-depolymerization,which seriously affects the structure and function of cardiomyocytes.We previously showed that a Yiqi Huoxue Decoction (YQHX) improves mitochondrial function and decreases anti-oxidative effects in hypoxia-induced H9c2 cell injury.Therefore,in this study we investigated whether YQHX protects against hypoxia-induced damage by decreasing damage to the cardiac cytoskeleton.Methods:After reaching 70%-80% confluence,H9c2 cells were synchronized in serum-free Dulbecco's Modified Eagle Medium for 6 hours,then divided into control,model,and YQHX (100,200,400 μg/mL) groups,which were then grown in a hypoxic atmosphere for 12 hours.Cardiac cell viability was assessed using an xCELLigence system.The levels of lactate dehydrogenase,maleic dialdehyde,and superoxide dismutase in H9c2 cell supernatants were measured.Hoechst 33258 staining was employed to observe cardiac cell apoptosis.Confocal microscopy,immunofluorescence,and western blot analysis were performed to evaluate the protective effects of the YQHX against hypoxia-induced injury in the H9c2 cell line.Results:Cells that were pretreated with YQHX were more able to maintain their microtubule structure in the early stages of hypoxia and had better myocardial fitness in response to hypoxia compared with cells that were not pretreated.However,hypoxia-induced upregulation of α-tubulin and β-tubulin expression antagonized the protective effect of YQHX (100 μg/mL).In addition,YQHX (100 μg/mL) treatment significantly upregulated MAP4 protein expression (P =.003) and downregulated p-AMPKα protein expression (P <.001) compared with the model group.Conclusion:The results indicate that YQHX plays a role in protecting against oxidative stress injury and apoptosis in H9c2 cells.Notably,our results suggested that the YQHX could mitigate the damage to the cardiac cytoskeleton and the dysregulation of AMPK-related protein signaling pathways that are induced by hypoxia. 展开更多

OBJECTIVE To explore the curative effect and mechanism of Yiqi Huoxue decoction in the treatment of coronary heart disease with Qi deficiency and blood stasis syndrome.METHODS The patients with coronary heart disease of Qi deficiency and blood stasis syndrome were treated with Yiqi Huoxue decoction for 3 months,and the changes of cardiac function were observed.61 serum samples(including 29 cases of disease group and 32 cases of Yiqi Huoxue expression group)were analyzed by non labeled proteomics.The disease group was used as the control group,and the protein with expression level difference of more than 1.2 folds(P<0.05)was screened.The molecular function,biological pathway and protein interaction of the different proteins were analyzed by bioinformatics,so as to identify the molecular and biological pathway of Yiqi Huoxue decoction in the treatment of coronary heart disease with Qi deficiency and blood stasis syndrome.RESULTS Clinical treatment found that Yiqi Huoxue decoction can improve TCM syndrome score and left ventricular ejection fraction,regulate blood glucose and blood lipid levels,prolong thrombin time,and improve heart function.The results of proteomic quantitative analysis showed that there were 69 proteins with different expression levels in the disease group.Bioinformatics analysis results showed that Yiqi Huoxue decoction may regulate ApoA1,alpha-2 and other proteins to act on HDL assembly,platelet degradation,PI3K Akt signaling pathway,and then play a therapeutic role in coronary heart disease with Qi deficiency and blood stasis syndrome.CONCLUSION Yiqi Huoxue decoction can effectively improved the heart function decline caused by Qi deficiency and blood stasis syndrome of coronary heart disease.It mainly act on energy metabolism and platelet activation pathway by activating HDL assembly and platelet degradation signal pathway proteins.This study can provide reference for the follow-up treatment mechanism of Qi deficiency and blood stasis syndrome of coronary heart disease. 展开更多

Objective:To observe the efficacy of Yiqi Huoxue Decoction in the treatment of patients with diabetic nephropathy(DN)stageⅢwith qi and yin deficiency and stasis and its effects on vascular endothelial growth factor(VEGF)and transforming growth factor-β(TGF-β1).Methods:Sixty patients with stage DN of Qi-yin deficiency and stasis type DN who were treated in the Endocrinology Department of the Affiliated Hospital of Jiangxi University of Traditional Chinese Medicine from September 2018 to December 2019 were selected as the research subjects.The remainder method was used to divide them into 30 cases in the observation group and 30 cases in the control group.Patients in both groups were referred to the guidelines and expert consensus for general treatment of DN(hypogl-ycemic,antihypertensive,lipid-lowering,etc.).The control group was given pancreatic kallikrein enteric-coated tablets orally once,120U,3 times a day,and the observation group was given The traditional Chinese medicine Yiqi Huoxue Decoction was taken orally,one dose daily,twice a day in the morning and evening;the two groups intervened continuously for 8 weeks.Detect blood FPG,PBG,HbA1c,β2-MG,BUN,SCr,VEGF,TGF-β1,and urine mALB and UACR levels before and after treatment,and calculate eGFR before and after treatment in both groups;observe changes in TCM syndrome scores in the two groups,Compare its clinical efficacy.Results:After 8 weeks of treatment,the total clinical effective rate of patients in the observation group was 93.3%,which was significantly different from the control group of 76.6%(P<0.05);the TCM syndrome scores in the observation group were significantly lower than those before treatment and in the control group(P<0.05);The levels of FPG,HbA1c,PPG,mALB,β2-MG,UACR,VEGF,and TGF-β1 in the observation group were significantly lower than those in the control group,with significant differences(P<0.05).The levels of SCr and BUN in the observation group were significantly lower.Compared with before treatment,eGFR increased,but there was no signi-ficant difference(P>0.05).Conclusion:Yiqi Huoxue Decoction for the treatment of patients with DN typeⅢqi-yin deficiency and stasis type,not only helps to lower blood sugar levels,improve TCM syndromes,but also can reduce early renal damage,reduce urine albumin,and delay kidney function It may be further worsened and has better safety.The mechanism may be related to reducing the levels of VEGF and TGF-β1,thereby delaying the fibrosis of tubulointerstitial scar and inhibi-ting glomerular capillary sclerosis. 展开更多

目的探讨益气养阴通络汤联合贝那普利治疗糖尿病肾病(diabetic nephropathy,DN)的疗效及对转化生长因子-β_(1)(transforming growth factor-β_(1),TGF-β_(1)),血管内皮生长因子(vascular endothelial growth factor,VEGF)影响。方法... 目的探讨益气养阴通络汤联合贝那普利治疗糖尿病肾病(diabetic nephropathy,DN)的疗效及对转化生长因子-β_(1)(transforming growth factor-β_(1),TGF-β_(1)),血管内皮生长因子(vascular endothelial growth factor,VEGF)影响。方法选取2019年10月—2021年10月医院收治的DN病人94例,随机分为对照组(n=48)和观察组(n=46)。入院后两组病人均实施生活方式相关的指导,包括体质量控制、运动、戒烟酒等,给予脂质的代谢紊乱相关纠正、血糖控制、血压等基础的治疗,对照组实施盐酸贝那普利片的口服治疗,观察组则在此基础上实施益气养阴通络汤治疗,两组持续治疗4周。对比两组临床疗效、尿微量白蛋白(microalbumin,mALB)、尿白蛋白肌酐比(urine albumin creatine ratio,UACR)、β_(2)微球蛋白(β_(2)-microglobulin,β_(2)-MG)水平、中医证候评分、血清同型半胱氨酸(homocysteine,Hcy)、肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)、白细胞介素-6(Interleukin-6,IL-6)、血清TGF-β_(1)和VEGF及空腹血糖(fasting plasma glucose/fasting blood glucose,FPG)、餐后2 h血糖(2 hour postprandial blood glucose,2 h PG)和糖化血红蛋白(glyeosylated hemoglobin,HbAlc)指标变化。结果治疗前两组mALB、UACR、β_(2)-MG指标间无统计学意义(P>0.05);治疗后两组mALB、UACR、β_(2)-MG指标较治疗前显著降低(P<0.05);并且观察组降低较明显(P<0.05);观察组治疗总有效率较对照组明显较高(P<0.05);治疗后两组主症评分、次症评分和中医证候总评分明显降低(P<0.05);且观察组降低较明显(P<0.05);治疗后两组血清Hcy、TNF-α、IL-6指标明显降低(P<0.05);且观察组降低较明显(P<0.05);治疗后两组血清TGF-β_(1)和VEGF指标较治疗前明显降低(P<0.05);且观察组降低较明显(P<0.05);治疗后两组FPG、2 h PG和HbAlc指标较治疗前明显降低(P<0.05);且观察组降低较明显(P<0.05)。结论采用益气养阴通络汤联合贝那普利治疗DN具有较好的临床疗效,可降低血清TGF-β_(1),VEGF指标,降低蛋白尿水平。 展开更多

目的探讨益气活血通络方对脊髓型颈椎病模型大鼠微小RNA-126a-5p(miR-126a-5p)及血管内皮生长因子(VEGF)信号通路的影响。方法30只健康雄性SD大鼠按照随机数字表法均分为假手术组、模型组和中药组。模型组、中药组均制备脊髓型颈椎病模... 目的探讨益气活血通络方对脊髓型颈椎病模型大鼠微小RNA-126a-5p(miR-126a-5p)及血管内皮生长因子(VEGF)信号通路的影响。方法30只健康雄性SD大鼠按照随机数字表法均分为假手术组、模型组和中药组。模型组、中药组均制备脊髓型颈椎病模型。中药组给予益气活血通络方灌胃,假手术组、模型组给予生理盐水灌胃,均为12周。各组干预结束后进行大鼠行为学测试,测定机械性刺激阈值和热刺激回缩时间。处死大鼠,HE染色观察各组椎间盘软骨终板结构的差异。TUNEL染色观察大鼠椎间盘纤维环细胞变化。实时荧光定量聚合酶链式反应检测大鼠椎间盘纤维环组织miR-126a-5p和VEGF mRNA。采用Western blot法检测大鼠椎间盘纤维环组织VEGF蛋白表达。结果与假手术组比较,模型组大鼠椎间盘血管芽数目减少,大鼠椎间盘纤维环细胞破坏明显、大量凋亡且细胞密度降低;模型组和中药组机械性刺激阈值降低,热刺激回缩时间缩短,miR-126a-5p水平降低,VEGF mRNA和蛋白表达水平升高(P<0.05)。与模型组比较,中药组大鼠椎间盘血管芽数目增加,大鼠椎间盘纤维环细胞破坏减轻,大鼠椎间盘纤维环细胞凋亡减少且细胞密度增加,机械性刺激阈值升高,热刺激回缩时间延长,miR-126a-5p水平增加,VEGF mRNA和蛋白表达水平降低(P<0.05)。结论益气活血通络方治疗脊髓型颈椎病的机制可能与上调miR-126a-5p、下调VEGF表达有关。 展开更多

目的观察通腑益气活血汤配合纤支镜肺泡灌洗治疗肺炎脓毒症的疗效,并探讨其对患者T淋巴细胞亚群、线粒体损伤指数的影响。方法选取2020年1月至2023年1月河南省第二人民医院收治的92例肺炎脓毒症患者作为研究对象,按随机数表法分为观察... 目的观察通腑益气活血汤配合纤支镜肺泡灌洗治疗肺炎脓毒症的疗效,并探讨其对患者T淋巴细胞亚群、线粒体损伤指数的影响。方法选取2020年1月至2023年1月河南省第二人民医院收治的92例肺炎脓毒症患者作为研究对象,按随机数表法分为观察组和对照组各46例。对照组患者予以纤支镜肺泡灌洗治疗,观察组患者予以通腑益气活血汤配合纤支镜肺泡灌洗治疗,均治疗2周。比较两组患者治疗2周后的疗效,及治疗前、治疗1周后、治疗2周后的中医证候积分、血气指标[经皮血氧饱和度(SpO_(2))、动脉血氧分压(PaO_(2))、动脉血二氧化碳分压(PaCO_(2))]、肺功能[第1 s用力呼吸容积(FEV_(1))、FEV_(1)/用力肺活量(FVC)、第1秒用力呼气容积占预计值百分比(FEV_(1)%)]、T淋巴细胞亚群(CD3^(+)、CD4^(+)、CD8^(+))、线粒体损伤指数(CD3^(+)MDI、CD4^(+)MDI、CD8^(+)MDI)、临床肺部感染评分(CPIS)、Murray肺损伤评分(MLIS)及急性生理与慢性健康量表(APACHEⅡ)评分;随访2个月,统计两组患者的生存情况及CPIS、MLIS、APACHEⅡ评分。结果观察组患者的治疗总有效率为91.30%,明显高于对照组的76.09%,差异有统计学意义(P<0.05);治疗2周后,观察组患者的主症、次症积分及总积分明显低于对照组,差异均有统计学意义(P<0.05);治疗2周后,观察组患者的FEV_(1)、FEV_(1)%、FEV_(1)/FVC、PaO_(2)及SpO_(2)明显高于对照组,PaCO_(2)明显低于对照组,差异均有统计学意义(P<0.05);治疗2周,观察组患者的CD3^(+)、CD4^(+)明显高于对照组,CD8^(+)、CD3^(+)MDI、CD4^(+)MDI、CD8^(+)MDI明显低于对照组,差异均有统计学意义(P<0.05);治疗2周后,观察组患者的MLIS、CPIS及APACHEⅡ评分明显低于对照组,差异均有统计学意义(P<0.05);随访2个月,观察组患者的生存率为95.95%,略高于对照组的89.13%,但差异无统计学意义(P>0.05),而观察组患者的MLIS、CPIS及APACHEⅡ评分明显低于对照组,差异均有统计学意义(P<0.05)。结论通腑益气活血汤配合纤支镜肺泡灌洗应用于肺炎脓毒症,可有效调节患者的血气与免疫功能,缓解临床症状,减轻肺损伤、改善肺功能,临床应用效果显著。 展开更多

目的:在黄芪、红芪本草药用历史对比总结基础上,利用网络药理学探究黄芪、红芪在益气活血通便方治疗慢传输型便秘中的作用机制差异,利用分子对接进行验证。方法:利用TCMSP和PharmMapper数据库筛选益气活血通便方中当归、黄芪、肉苁蓉、... 目的:在黄芪、红芪本草药用历史对比总结基础上,利用网络药理学探究黄芪、红芪在益气活血通便方治疗慢传输型便秘中的作用机制差异,利用分子对接进行验证。方法:利用TCMSP和PharmMapper数据库筛选益气活血通便方中当归、黄芪、肉苁蓉、火麻仁、红芪的活性成分及靶点;通过GeneCards、DrugBank、OMIM等数据库筛选慢传输型便秘相关靶点,利用Venny 2.1.0工具作图比较益气活血通便方Ⅰ和组方Ⅱ治疗慢传输型便秘的靶点;通过STRING数据库构建活性成分和关键靶点PPI网络;利用Metascape平台进行GO和KEGG通路富集分析,比较网络药理学差异。构建益气活血通便方的成分-靶点-通路网络图,利用AutoDock软件进行分子对接。结果:得到组方Ⅰ60个活性成分,501个作用靶点;组方Ⅱ50个活性成分,622个作用靶点。得到疾病作用靶点1035个,组方Ⅰ和组方Ⅱ与慢传输型便秘交集靶点分别为161个和201个。GO富集分析得到益气活血通便方Ⅰ553条生物进程或通路,组方Ⅱ899条生物进程或通路。KEGG富集分析预测组方Ⅰ126条信号通路,组方Ⅱ144条信号通路。分子对接结果显示槲皮素、木犀草素、花生酸、山柰酚、3,9-二-O-甲基尼森香豌豆紫檀酚与EGFR、SRC、CASP3、HSP90AA1、ESR1有良好的结合活性。结论:益气活血通便方通过多点显效、协同增效治疗慢传输型便秘,红芪可发挥与黄芪相似的作用,但能否替代黄芪还有待于结合药效学实验、临床试验进一步深入研究。 展开更多

目的:通过网络药理学技术探究益气养阴化浊通络方治疗糖尿病肾病的分子机制。方法:通过TCMSP数据库获取益气养阴化浊通络方相关药物靶点,GeneCard、OMIM及PharmGkb数据库获取糖尿病肾病相关靶点。Venny R软件获取益气养阴化浊通络方治... 目的:通过网络药理学技术探究益气养阴化浊通络方治疗糖尿病肾病的分子机制。方法:通过TCMSP数据库获取益气养阴化浊通络方相关药物靶点,GeneCard、OMIM及PharmGkb数据库获取糖尿病肾病相关靶点。Venny R软件获取益气养阴化浊通络方治疗糖尿病肾病的潜在靶点,Cytoscape 3.8.0软件构建药物-有效成分-靶点-疾病网络图。对潜在靶点进行PPI互作网络分析、GO功能及KEGG通路富集分析。结果:共获取益气养阴化浊通络方有效成分66个,有效靶点254个,糖尿病肾病相关靶点4755个,取交集后获取潜在靶点179个。PPI分析获取核心靶点MAPK3、TP53、MAPK1、HSP90AA1、JUN、AKT1、RELA、MAPK14等;GO功能富集分析体现在对氧化应激的反应、细胞对化学压力的反应、对外来刺激的反应、对活性氧的反应等;细胞组分多在膜筏、膜微域、转录调节因子、复合囊泡腔等;分子功能多在DNA结合转录因子结合、RNA聚合酶II特异性DNA结合转录因子结合、核受体活性、配体激活的转录因子活性等;KEGG通路富集分析多在糖尿病并发症中的脂质与动脉粥样硬化、年龄-RAGE信号通路在糖尿病并发症中的作用、前列腺癌、乙肝、人巨细胞病毒感染、化学致癌-受体激活及流体切应力与动脉粥样硬化等通路。结论:通过网络药理学技术探究益气养阴化浊通络方治疗糖尿病肾病的作用机制,为后续相关研究提供理论依据和数据支撑。 展开更多

目的观察活血止痛方联合活血通络腰椎巴布膏治疗腰椎间盘突出症急性期(气滞血瘀证)临床疗效。方法选取80例腰椎间盘突出症急性期(气滞血瘀证)患者,用随机数字表法将患者分为两组各40例。在常规治疗的基础上,对照组予活血通络腰椎巴布膏... 目的观察活血止痛方联合活血通络腰椎巴布膏治疗腰椎间盘突出症急性期(气滞血瘀证)临床疗效。方法选取80例腰椎间盘突出症急性期(气滞血瘀证)患者,用随机数字表法将患者分为两组各40例。在常规治疗的基础上,对照组予活血通络腰椎巴布膏,观察组予活血止痛方联合活血通络腰椎巴布膏,均连续治疗10d。比较治疗前后视觉模拟量表(VAS)评分、Oswestry功能障碍(ODI)指数、下腰痛日本骨科协会量表(JOA)评分、中医证候积分、临床疗效及不良反应情况。结果治疗后,两组VAS、ODI、中医证候积分(腰部疼痛;下肢麻痛;腰肌僵硬,活动受限;痛有定处,痛处拒按)均较治疗前下降(P<0.05),且观察组均低于对照组(P<0.05);两组JOA评分较治疗前上升(P<0.05),观察组高于对照组(P<0.05)。观察组总有效率为95.00%,高于对照组的82.50%(P<0.05)。两组治疗期间均无不良反应。结论活血止痛方联合活血通络腰椎巴布膏能够有效减轻腰椎间盘突出症急性期(气滞血瘀证)患者的临床症状,改善腰椎功能障碍,提升患者生活质量。 展开更多

基于神经功能、中医证候评分评价活血通络汤在急性缺血性脑卒中治疗中的效果

目的探讨活血通络汤在急性缺血性脑卒中(acute ischemic stroke,AIS)患者治疗中的效果及对其对中医证候、神经功能的影响。方法选取82例六安市中医院2021年1月—2022年1月收治的AIS患者作为研究对象,将其分为观察组(41例)和对照组(41例)... 目的探讨活血通络汤在急性缺血性脑卒中(acute ischemic stroke,AIS)患者治疗中的效果及对其对中医证候、神经功能的影响。方法选取82例六安市中医院2021年1月—2022年1月收治的AIS患者作为研究对象,将其分为观察组(41例)和对照组(41例)(随机数字表法)。对照组接受常规西医治疗,观察组在对照组基础上接受活血通络汤治疗。两组治疗时间均为1个月。对两组治疗前后的美国国立卫生研究院卒中量表(National Institute of Health stroke scale,NIHSS)、蒙特利尔认知评估(Montreal Cognitive Assessment,MoCA)评分、中医证候积分、生活质量评分、全血高切黏度、全血低切黏度、血浆纤维蛋白原(fibrinogen,FIB)及D-二聚体(D-dimer,D-D)水平、血管内皮功能及治疗后的临床疗效进行比较。结果与观察组比较,对照组治疗后临床总有效率较低;治疗后两组中医证候各项评分、NIHSS评分、全血高切黏度、全血低切黏度、血浆FIB、D-D水平、血清内皮素-1(endothelin-1,ET-1)、血管性假血友病因子(von Willebrand factor,vWF)水平与治疗前比均降低,其中观察组低于对照组;治疗后两组生活质量评分、MoCA评分、血清一氧化氮(nitric oxide,NO)、血管舒张功能(flow-mediated dilation,FMD)水平与治疗前比均升高,组间进行比较,对照组较低(P<0.05)。结论活血通络汤应用于AIS患者可有效改善中医证候,利于神经功能和认知功能恢复,提高生活质量,调节血液状态和血管内皮功能,疗效显著。 展开更多

目的利用网络药理学探讨益气通络汤治疗糖尿病周围神经病变(DPN)的作用机制。方法通过中药系统药理学数据库和分析平台、中药综合数据库和中医药证候关联数据库,对益气通络汤所含核心成分进行筛选并预测作用靶标蛋白,利用GeneCards、OMI... 目的利用网络药理学探讨益气通络汤治疗糖尿病周围神经病变(DPN)的作用机制。方法通过中药系统药理学数据库和分析平台、中药综合数据库和中医药证候关联数据库,对益气通络汤所含核心成分进行筛选并预测作用靶标蛋白,利用GeneCards、OMIM和TTD数据库检索DPN相关基因,利用维恩图筛选益气通络汤中核心成分治疗DPN的作用靶标蛋白。利用Cytoscape构建核心成分-作用靶标蛋白网络图。利用String数据库绘制益气通络汤蛋白互作网络。利用Metascape数据库对靶标蛋白进行GO及KEGG富集分析。结果益气通络汤所含10味中药共筛选出191种核心成分,涉及263个靶标蛋白,其中有189个基因与DPN相关。网络分析结果表明,益气通络汤治疗DPN涉及的生物学过程主要富集在细胞群体增殖的负调控、DNA代谢过程、蛋白质水解的调节等;涉及的分子功能主要富集在脂质结合、激酶结合、蛋白激酶结合等;细胞组分主要富集在溶酶体、轴突、突触后。其作用机制可能与PI3K-Akt、MAPK、钙、Ras、cAMP、Rap1、趋化因子、NOD样受体、Wnt、cGMP-PKG、JAK-STAT、Hippo、mTOR、催产素、磷脂酶D、调节干细胞多能性、Apelin、雌激素、胰岛素和细胞凋亡通路有关。结论本研究初步揭示了益气通络汤多靶点整体调节DPN的作用特点,为临床上进一步开展本方治疗DPN作用机制的研究提供了新的思路和方法。 展开更多

活血益气通经汤联合美洛昔康对腰椎间盘突出症患者炎症反应及下腰痛的影响

目的:探讨活血益气通经汤联合美洛昔康在腰椎间盘突出症(LDH)患者中的应用价值。方法:选择2020年1月—2023年1月吉安市第一人民医院收治的LDH患者60例,按随机数字表法分为两组。对照组(n=30)采用美洛昔康治疗,观察组(n=30)采用活血益气... 目的:探讨活血益气通经汤联合美洛昔康在腰椎间盘突出症(LDH)患者中的应用价值。方法:选择2020年1月—2023年1月吉安市第一人民医院收治的LDH患者60例,按随机数字表法分为两组。对照组(n=30)采用美洛昔康治疗,观察组(n=30)采用活血益气通经汤联合美洛昔康治疗,两组均治疗14 d。对比分析两组炎症反应、下腰痛程度、腰椎功能、临床疗效、不良反应。结果:治疗后,观察组C反应蛋白(CRP)[(7.53±0.86)mg/L]、白介素(IL)-6[(92.43±10.07)μg/L]、IL-1β[(5.17±0.94)ng/L]、肿瘤坏死因子-α(TNF-α)[(1.14±0.21)ng/L]及视觉模拟评分法(VAS)评分[(2.75±0.69)分]均低于对照组[(9.03±1.04)mg/L、(107.03±14.25)μg/L、(6.04±1.13)ng/L、(1.87±0.29)ng/L、(4.30±1.18)分],日本骨科协会评估治疗分数(JOA)评分中主观症状评分[(7.59±0.67)分]、客观体征评分[(5.20±0.41)分]、日常生活受限度评分[(12.07±0.84)分]及总分[(24.86±2.01)分]均高于对照组[(6.63±1.04)、(4.31±0.54)、(10.23±1.26)、(21.17±2.86)分],差异均有统计学意义(P<0.05);观察组治疗总有效率为93.33%,高于对照组的73.33%,差异有统计学意义(P<0.05);两组治疗期间均未出现不良反应。结论:活血益气通经汤联合美洛昔康可下调LDH患者CRP、IL-6、IL-1β、TNF-α水平,减轻下腰痛,改善腰椎功能,且安全可靠。 展开更多

目的:观察益气活血通络汤联合丁苯酞治疗脑梗死恢复期患者的效果。方法:选取2019年2月至2022年2月该院收治的75例脑梗死恢复期患者进行前瞻性研究,按照随机数字表法将其分为研究组38例与对照组37例。对照组采用丁苯酞软胶囊治疗,研究组... 目的:观察益气活血通络汤联合丁苯酞治疗脑梗死恢复期患者的效果。方法:选取2019年2月至2022年2月该院收治的75例脑梗死恢复期患者进行前瞻性研究,按照随机数字表法将其分为研究组38例与对照组37例。对照组采用丁苯酞软胶囊治疗,研究组在对照组基础上联合益气活血通络汤治疗。比较两组临床疗效、治疗前后日常生活能力[Barthel指数(BI)]评分、肢体运动功能[Fugl-Meyer运动功能量表(FMA)]评分、神经功能相关指标[改良Rankin量表(MRS)、中枢神经特异性蛋白(S100β)]水平、脑血流动力学指标[大脑前动脉(ACA)、基底动脉(BA)血流速度]水平及不良反应发生率。结果:研究组治疗总有效率为94.74%(36/38),高于对照组的75.68%(28/37),差异有统计学意义(P<0.05);治疗后,两组BI、FMA评分均高于治疗前,且研究组高于对照组,差异有统计学意义(P<0.05);两组MRS评分和S100β水平均低于治疗前,且研究组低于对照组,差异有统计学意义(P<0.05);两组ACA、BA血流速度均高于治疗前,且研究组高于对照组,差异有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:益气活血通络汤联合丁苯酞治疗脑梗死恢复期患者可提高治疗总有效率、BI评分、FMA评分和脑血流动力学指标水平,降低神经功能相关指标水平,其效果优于单纯丁苯酞软胶囊治疗。 展开更多

目的 探讨益气养阴消癥通络汤治疗早期糖尿病肾病(DN)的临床效果。方法 64例早期糖尿病肾病患者,随机分为对照组和观察组,每组32例。对照组使用常规西药治疗,在此基础上,观察组使用益气养阴消癥通络汤治疗。比较两组临床疗效,血糖及肾... 目的 探讨益气养阴消癥通络汤治疗早期糖尿病肾病(DN)的临床效果。方法 64例早期糖尿病肾病患者,随机分为对照组和观察组,每组32例。对照组使用常规西药治疗,在此基础上,观察组使用益气养阴消癥通络汤治疗。比较两组临床疗效,血糖及肾功能指标[空腹血糖(FBG)、餐后2 h血糖(2 h PG)、糖化血红蛋白(HbA1c)、血肌酐(SCr)、尿素氮(BUN)、尿白蛋白排泄率(UAER)],中医证候积分,血液流变学指标[血浆粘度、全血高切粘度、全血低切粘度、纤维蛋白原(FIB)、D-二聚体(D-D)]。结果 观察组治疗总有效率为93.75%,明显高于对照组的75.00%(P<0.05)。观察组治疗后FBG、2 h PG、HbA1c、BUN、Scr、UAER分别为(6.35±0.90)mmol/L、(8.97±1.25)mmol/L、(6.71±0.86)%、(5.95±1.04)mmol/L、(120.73±17.68)μmol/L、(75.70±10.67)μg/min,均低于对照组的(7.84±1.21)mmol/L、(10.96±1.78)mmol/L、(8.10±0.93)%、(8.67±1.39)mmol/L、(135.06±19.28)μmol/L、(109.31±12.75)μg/min(P<0.05)。观察组治疗后咽干口渴、手足心热、神疲乏力、腰痛膝软、肢体水肿等中医证候积分分别为(0.98±0.16)、(0.92±0.15)、(1.01±0.21)、(0.96±0.18)、(0.95±0.20)分,均明显低于对照组的(1.53±0.21)、(1.49±0.20)、(1.75±0.24)、(1.50±0.23)、(1.48±0.23)分(P<0.05)。观察组治疗后血浆粘度、全血高切粘度、全血低切粘度、FIB、D-D分别为(1.23±0.21)mPa·s、(3.67±0.68)mPa·s、(8.74±1.36)mPa·s、(2.62±0.37)g/L、(1.90±0.35)μg/ml,均低于对照组的(1.88±0.25)mPa·s、(5.14±0.79)mPa·s、(11.08±1.93)mPa·s、(3.29±0.45)g/L、(2.68±0.40)μg/ml(P<0.05)。结论 益气养阴消癥通络汤治疗早期糖尿病肾病的临床效果显著,能提高血糖控制效果,改善肾功能,降低血液粘稠度,缓解证候。 展开更多

目的探讨中药内服联合针刺及正骨推拿治疗腰椎间盘突出症(LDH)的效果。方法选取2019年11月—2021年11月于宜春市第二人民医院就诊的LDH患者70例,按随机数字表法分为对照组和观察组,各35例。所有患者均予以常规西医治疗,对照组于此基础... 目的探讨中药内服联合针刺及正骨推拿治疗腰椎间盘突出症(LDH)的效果。方法选取2019年11月—2021年11月于宜春市第二人民医院就诊的LDH患者70例,按随机数字表法分为对照组和观察组,各35例。所有患者均予以常规西医治疗,对照组于此基础上辅以正骨推拿,观察组于对照组基础上加用中药活血通络汤内服及针刺治疗,连续治疗4周。对比2组临床疗效、下肢感觉障碍改善情况、自由基代谢与不良反应。结果观察组治疗总有效率为94.29%(33/35),高于对照组的77.14%(27/35);下肢感觉障碍改善总有效率为91.43%(32/35),高于对照组的71.43%(25/35),差异均有统计学意义(P<0.05)。治疗前2组超氧化物歧化酶(SOD)、丙二醛(MDA)比较,差异无统计学意义(P>0.05);治疗后,观察组MDA低于对照组,SOD高于对照组,差异均有统计学意义(P<0.05)。2组患者治疗期间均无不良反应发生。结论中药内服联合针刺及正骨推拿治疗LDH患者效果确切,可有效改善其下肢感觉障碍情况、调节自由基代谢,且安全性高。 展开更多
