BACKGROUND The pediatric Crohn’s disease activity index(PCDAI)is used as a standard tool to assess disease activity in clinical trials for pediatric Crohn’s disease.AIM To examine which items on the PCDAI drive assessment of disease activity, and how subgroups of subjective and objective items reflect change in disease state over time.METHODS Selective raw data from three prospectively collected datasets were combined, including 703 children with full PCDAI data at baseline,at 3-mo(Q1,n=670),and 1-year(Q4,n=474).Change in individual PCDAI scores from baseline to Q1 and to Q4 were examined using the non-weighted PCDAI.RESULTS Abdominal pain,well-being,weight,and stooling had the highest change scores over time. Objective indicators including albumin, abdominal exam, and height velocity followed.Change scores for well-being and abdominal exam did not explain significant variance at Q1 but were significant predictors at Q4(P<0.001 and P<0.05).Subjective and objective subgroups of items predicted less variance(18%and 22%)on total PCDAI scores at Q1 and Q4 compared to the full PCDAI,or a composite scale(both 32%)containing significant predictors.CONCLUSION Although subjective items on the PCDAI change the most over time, the full PCDAI or a smaller composite of items including a combination of subjective and objective components classifies disease activity better than a subgroup of either subjective or objective items alone.Reliance on subjective or objective items as stand-alone proxies for disease activity measurement could result in misclassification of disease state. 展开更多

AIM To investigate of pediatric ulcerative colitis activity index(PUCAI) in ulcerative colitis correlate with mucosal inflammation and endoscopic assessment of disease activity(Mayo endoscopic score).METHODS We reviewed charts from ulcerative colitis patients who had undergone both colonoscopy over 3 years. Clinical assessment of disease severity within 35 d(either before or after) the colonoscopy were included. Patients were excluded if they had significant therapeutic interventions(such as the start of corticosteroids or immunosuppressive agents) between the colonoscopy and the clinical assessment. Mayoendoscopic score of the rectum and sigmoid were done by two gastroenterologists. Inter-observer variability in Mayo score was assessed.RESULTS We identified 99 patients(53% female, 74% pancolitis) that met inclusion criteria. The indications for colonoscopy included ongoing disease activity(62%), consideration of medication change(10%), assessment of medication efficacy(14%), and cancer screening(14%). Based on PUCAI scores, 33% of patients were in remission, 39% had mild disease, 23% had moderate disease, and 4% had severe disease. There was "moderate-substantial" agreement between the two reviewers in assessing rectal Mayo scores(kappa = 0.54, 95%CI: 0.41-0.68). CONCLUSION Endoscopic disease severity(Mayo score) assessed by reviewing photographs of pediatric colonoscopy has moderate inter-rater reliability, and agreement was less robust in assessing patients with mild disease activity. Endoscopic disease severity generally correlates with clinical disease severity as measured by PUCAI score. However, children with inflamed colons can have significant variation in their reported clinical symptoms. Thus, assessment of both clinical symptoms and endoscopic disease severity may be required in future clinical studies. 展开更多

The Crohn's disease activity index(CDAI) has been commonly used to assess the effects of treatment with different agents in Crohn's disease(CD).However these studies may be compromised,if the results compared to a placebo or standard therapy group(in the absence of a placebo) substantially differ from the expected response. In addition, significan concerns have been raised regarding the reliability and validity of the CDAI.Reproducibility of the CDAI may be limited as significant inter-observer error has been recorded, even if measurements are done by experienced clinicians with expertise in the diagnosis and treatment of CD.Finally, many CDAI endpoints are open to subjective interpretation and have the potential for manipulation. This is worrisome as there is the potential for significant financial gain if the results of a clinical trial appear to provide a positive result.Physicians caring for patients should be concerned about the positive results in clinica trials that are sponsored by industry, even if the trials involve respected centers and the results appear in highly ranked medical journals. 展开更多

The dorsal area of the anterior cingulate cortex (ACC) constructs the salience network associated with the anterior insular cortex. Conventional brain imaging studies, such as functional magnetic resonance imaging (fMRI), have demonstrated that relational memory formation occurs in the ACC. However, how such memory is encoded and retrieved remains unknown due to limited time resolution of conventional fMRI. This study aimed to investigate temporal dynamics of the dorsal ACC (dACC) during word-pair tasks based on a newly developed event-related deep brain activity (ER-DBA) method using occipital electroencephalogram (EEG) signal powers. The method assesses dACC activity at a temporal resolution of approximately 0.3 s beyond the conventional resolution limit. We found that transient deactivation of dACC during the presentation of the second word of each pair was essential for encoding success regardless of whether the words were related or unrelated. We also found that memory accuracy was not affected by the intervention of inter-trials until the recall trial. Taken together, these findings suggest that dACC deactivation for encoding success is accompanied with short-term potentiation essential for durability of memory. We further found that false memory formation associated with the presentation of word pairs was occasionally committed. In such cases, dACC exhibited a similar transient deactivation although false memory commission was independent of related or unrelated conditions. Our findings suggest that encoding and retrieval of associates are paralleled and that simultaneous production of associates seems to be an essential strategy for successful relational memory formation. The study was limited to the assessment of dACC activity and did not account for other regional brain activities or receptor regulation related to short-term potentiation. We detected fast behavior of dACC during relational memory formation using the novel ER-DBA method. Such temporal dynamics will be important for eliciting underlying mechanisms of memory dysfunctions. 展开更多

Human activities in a transborder watershed are complex under the influence of domestic policies, international relations, and global events.Understanding the forces driving human activity change is important for the development of transborder watershed. In this study,we used global historical land cover data, the hemeroby index model, and synthesized major historical events to analyze how human activity intensity changed in the Heilongjiang River(Amur River in Russia)watershed(HLRW).The results showed that there was a strong spatial heterogeneity in the variation of human activity intensity in the HLRW over the past century(1900-2016).On the Chinese side, the human activity intensity change shifted from the plain areas for agricultural reclamation to the mountainous areas for timber extraction. On the Russian side, human activity intensity changes mostly concentrated along the Trans-Siberian Railway and the Baikal-Amur Mainline.Localized variation of human activity intensity tended to respond to regional events while regionalized variation tends to reflect national policy change or broad international events. The similarities and differences between China and Russia in policies and positions in international events resulted in synchronous and asynchronous changes in human activity intensity.Meanwhile, policy shifts were often confined by the natural features of the watershed. These results reveal the historical origins and fundamental connotations of watershed development and contribute to formulating regional management policies that coordinate population,eco-nomic,social,and environmental activities. 展开更多

BACKGROUND Triglyceride-glucose(TyG)index values are a new surrogate marker for insulin resistance. This study aimed to explore the relationship between cumulative TyG index values and atrial fibrillation(AF)recurrence after radiofrequency catheter ablation(RFCA).METHODS A total of 576 patients with AF who underwent RFCA at the Second Affiliated Hospital of Xi'an Jiaotong University were included in this study. The participants were grouped based on cumulative TyG index values tertiles within 3 months after ablation.Cox regression and restricted cubic spline analyses were used to determine the relationship between cumulative TyG index values and AF recurrence. The predictive value of all risk factors was assessed by receiver operating curve analysis.RESULTS There were 375 patients completed the study(age:63.23±10.73 years,64.27%male).The risk of AF recurrence increased with increasing cumulative TyG index values tertiles. After adjusting for potential confounders, patients in the medium cumulative TyG index group[hazard ratio(HR)=4.949,95%CI:1.778–13.778,P=0.002]and the high cumulative TyG index group(HR=8.716,95%CI:3.371–22.536,P<0.001)had a higher risk of AF recurrence than those in the low cumulative TyG index group. The restricted cubic spline regression model also showed an increased risk of AF recurrence with increasing cumulative TyG index values. When considering cumulative TyG index values, left atrial diameter, and lactate dehydrogenase levels as a comprehensive factor, the model could effectively predict AF recurrence after RFCA[area under the curve(AUC)=0.847,95%CI:0.797–0.897,P<0.001].CONCLUSIONS Cumulative TyG index values were a risk factor for AF recurrence after RFCA.Monitoring longitudinal TyG index values may assist with optimized for risk stratification and outcome prediction for AF recurrence. 展开更多

RFID-based human activity recognition(HAR)attracts attention due to its convenience, noninvasiveness, and privacy protection.Existing RFID-based HAR methods use modeling,CNN,or LSTM to extract features effectively.Still, they have shortcomings:1)requiring complex hand-crafted data cleaning processes and 2)only addressing single-person activity recognition based on specific RF signals. To solve these problems, this paper proposes a novel device-free method based on Time-streaming Multiscale Transformer called TransTM.This model leverages the Transformer's powerful data fitting capabilities to take raw RFID RSSI data as input without pre-processing.Concretely,we propose a multiscale convolutional hybrid Transformer to capture behavioral features that recognizes singlehuman activities and human-to-human interactions.Compared with existing CNN-and LSTM-based methods, the Transformer-based method has more data fitting power, generalization, and scalability.Furthermore, using RF signals, our method achieves an excellent classification effect on human behaviorbased classification tasks.Experimental results on the actual RFID datasets show that this model achieves a high average recognition accuracy(99.1%).The dataset we collected for detecting RFID-based indoor human activities will be published. 展开更多

This editorial contains comments on the article“Correlation between preoperative systemic immune inflammation index, nutritional risk index, and prognosis of radical resection of liver cancer”in a recent issue of the World Journal of Gastrointestinal Surgery. It pointed out the actuality and importance of the article and focused primarily on the underlying mechanisms making the systemic immuneinflammation index(SII)and geriatric nutritional risk index(GNRI)prediction features valuable. There are few publications on both SII and GNRI together in hepatocellular carcinoma(HCC)and patient prognosis after radical surgery.Neutrophils release cytokines, chemokines, and enzymes, degrade extracellular matrix, reduce cell adhesion, and create conditions for tumor cell invasion.Neutrophils promote the adhesion of tumor cells to endothelial cells, through physical anchoring. That results in the migration of tumor cells.Pro-angiogenic factors from platelets enhance tumor angiogenesis to meet tumor cell supply needs.Platelets can form a protective film on the surface of tumor cells. This allows avoiding blood flow damage as well as immune system attack. It also induces the epithelial-mesenchymal transformation of tumor cells that is critical for invasiveness.High SII is also associated with macro-and microvascular invasion and increased numbers of circulating tumor cells.A high GNRI was associated with significantly better progression-free and overall survival.HCC patients are a very special population that requires increased attention.SII and GNRI have significant survival prediction value in both palliative treatment and radical surgery settings. The underlying mechanisms of their possible predictive properties lie in the field of essential cancer features. Those features provide tumor nutrition, growth, and distribution throughout the body, such as vascular invasion. On the other hand, they are tied to the possibility of patients to resist tumor progression and development of complications in both postoperative and cancer-related settings. The article is of considerable interest. It would be helpful to continue the study follow-up to 2 years and longer.External validation of the data is needed. 展开更多

It also aimed to identify phenolic acids present in the extracts. The 2,2-azinobis-(3-ethylbenzothiazoline-6-sulfonic acid) (ABTS), 2,2-diphenyl-1-picrylhydrazyl (DPPH), ferric reducing antioxidant power (FRAP), and ferric thiocyanate lipid peroxidation antioxidant assays were performed. High performance liquid chromatography was used to identify phenolic acids. There was no solvent effect on TPC nor on scavenging activities, and inhibition of lipid peroxidation (p > 0.05) among solvent extracts. Background: Women are thought to be more susceptible to stress than men in a stressful society, and reducing stress is crucial for women to maintain their health. Static stretching (SST) is applied in various fields to not only increase muscle flexibility but also reduce stress. Additionally, conscious slower breathing (CSB) predominates parasympathetic activity, causing a relaxing effect. These results indicate that combining SST and CSB may be more useful in reducing stress. However, to the best of our knowledge, the effect of this combination remains unclear. Objective: This study aimed to elucidate the effects of the combination of SST and CSB on autonomic activity and stress in adult women. Methods: Eleven healthy Japanese adult female participants performed SST with nonconscious natural breathing for 20 min. The same participants performed SST in combination with CSB (2 s inspiratory and 4 s expiratory) for 20 min on another day. Salivary cortisol and chromogranin A levels were measured before and after stretching as stress markers of the hypothalamic-pituitary-adrenal axis and sympathetic nervous system. The coefficient of variation of the R-R interval (CVR-R) and high-frequency component (HF), which reflect parasympathetic nerve activity, and heart rate and low-frequency component (LF)/HF ratio, which reflect sympathetic nerve activity, were measured before, during, and after stretching. Results: SST decreased cortisol levels but with no significant changes in chromogranin A, heart rate, CVR-R, HF, or LF/HF ratio. The combination of SST and CSB increased CVR-R and HF levels in addition to decreasing cortisol levels but with no significant changes in chromogranin A, heart rate, or LF/HF levels. Conclusion: These results indicate that the combination of SST and CSB may increase parasympathetic activity and reduce stress. However, future randomized controlled trials with larger sample sizes should support this conclusion. 展开更多

Landslides are highly dangerous phenomena that occur in different parts of the world and pose significant threats to human populations. Intense rainfall events are the main triggering process for landslides in urbanized slope regions, especially those considered high-risk areas. Various other factors contribute to the process;thus, it is essential to analyze the causes of such incidents in all possible ways. Soil moisture plays a critical role in the Earth’s surface-atmosphere interaction systems;hence, measurements and their estimations are crucial for understanding all processes involved in the water balance, especially those related to landslides. Soil moisture can be estimated from in-situ measurements using different sensors and techniques, satellite remote sensing, hydrological modeling, and indicators to index moisture conditions. Antecedent soil moisture can significantly impact runoff for the same rainfall event in a watershed. The Antecedent Precipitation Index (API) or “retained rainfall,” along with the antecedent moisture condition from the Natural Resources Conservation Service, is generally applied to estimate runoff in watersheds where data is limited or unavailable. This work aims to explore API in estimating soil moisture and establish thresholds based on landslide occurrences. The estimated soil moisture will be compared and calibrated using measurements obtained through multisensor capacitance probes installed in a high-risk area located in the mountainous region of Campos do Jordão municipality, São Paulo, Brazil. The API used in the calculation has been modified, where the recession coefficient depends on air temperature variability as well as the climatological mean temperature, which can be considered as losses in the water balance due to evapotranspiration. Once the API is calibrated, it will be used to extrapolate to the entire watershed and consequently estimate soil moisture. By utilizing recorded mass movements and comparing them with API and soil moisture, it will be possible to determine thresholds, thus enabling anticipation of landslide occurrences. 展开更多

Background:Excessive heat exposure can lead to hyperthermia in humans, which impairs physical performance and disrupts cognitive function. While heat is a known physiological stressor,it is unclear how severe heat stress affects brain physiology and function. Methods:Eleven healthy participants were subjected to heat stress from prolonged exercise or warm water immersion until their rectal temperatures(T_(re))attained 39.5℃,inducing exertional or passive hyperthermia, respectively. In a separate trial, blended ice was ingested before and during exercise as a cooling strategy.Data were compared to a control condition with seated rest(normothermic).Brain temperature(T_(br)),cerebral perfusion, and task-based brain activity were assessed using magnetic resonance imaging techniques.Results:T_(br)in motor cortex was found to be tightly regulated at rest(37.3℃±0.4℃(mean±SD))despite fluctuations in T_(re).With the development of hyperthermia,T_(br)increases and dovetails with the rising T_(re).Bilateral motor cortical activity was suppressed during high-intensity plantarflexion tasks, implying a reduced central motor drive in hyperthermic participants(T_(re)=38.5℃±0.1℃).Global gray matter perfusion and regional perfusion in sensorimotor cortex were reduced with passive hyperthermia.Executive function was poorer under a passive hyperthermic state, and this could relate to compromised visual processing as indicated by the reduced activation of left lateral-occipital cortex.Conversely, ingestion of blended ice before and during exercise alleviated the rise in both T_(re)and T_(bc)and mitigated heat-related neural perturbations. Conclusion:Severe heat exposure elevates T_(br),disrupts motor cortical activity and executive function, and this can lead to impairment of physical and cognitive performance. 展开更多

In this article,a series of high refractive indices(1.50-1.53)thiol phenyl polysiloxane(TPS)were synthesized via hydrolytic sol-gel reaction. The Fourier transform infrared spectra(FT-IR)and nuclear magnetic resonance spectra(NMR)results showed that TPS conformed to the predicted structures.Natural terpene linalool was exploited as photocrosslinker to fabricate UV-curing linalool-polysiloxane hybrid films(LPH)with TPS via photoinitiated thiol-ene reaction.LPH rapidly cured under UV irradiation at the intensity of 80 mW/cm2 in 30 s,exhibiting good UV-curing properties. The optical transmittance of LPH in the wavelength of 300-800 nm was over 90%,exhibiting good optical transparency. The water contact angle and water vapor permeability results showed that the introduction of phenyl groups enhance the hydrophobicity and water vapor barrier properties of LPH.The results indicated the potential of LPHs in the applications of optical functional coatings. 展开更多

Background:Prolonged sitting and reduced physical activity lead to low energy expenditures. However, little is known about the joint impact of daily sitting time and physical activity on body fat distribution. We investigated the independent and joint associations of daily sitting time and physical activity with body fat among adults. Methods:This was a cross-sectional analysis of U.S.nationally representative data from the National Health and Nutrition Examination Survey2011-2018 among adults aged 20 years or older.Daily sitting time and leisure-time physical activity(LTPA)were self-reported using the Global Physical Activity Questionnaire.Body fat(total and trunk fat percentage)was determined via dual X-ray absorptiometry. Results:Among 10,808 adults, about 54.6%spent 6 h/day or more sitting;more than one-half reported no LTPA(inactive)or less than 150 min/week LTPA(insufficiently active)with only 43.3%reported 150 min/week or more LTPA(active)in the past week. After fully adjusting for sociodemographic data, lifestyle behaviors, and chronic conditions, prolonged sitting time and low levels of LTPA were associated with higher total and trunk fat percentages in both sexes. When stratifying by LTPA,the association between daily sitting time and body fat appeared to be stronger in those who were inactive/insuufficiently active. In the joint analyses,inactive/insuufficiently active adults who reported sitting more than 8 h/day had the highest total(female:3.99%(95%confidence interval(95%CI):3.09%-4.88%);male:3.79%(95%CI:2.75%-4.82%))and trunk body fat percentages(female:4.21%(95%CI:3.09%-5.32%);male:4.07%(95%CI:2.95%-5.19%))when compared with those who were active and sitting less than 4 h/day.Conclusion:Prolonged daily sitting time was associated with increased body fat among U.S.adults.The higher body fat associated with 6 h/day sitting may not be offset by achieving recommended levels of physical activity. 展开更多

Inpatient falls from beds in hospitals are a common problem.Such falls may result in severe injuries. This problem can be addressed by continuous monitoring of patients using cameras.Recent advancements in deep learning-based video analytics have made this task of fall detection more effective and efficient.Along with fall detection, monitoring of different activities of the patients is also of significant concern to assess the improvement in their health.High computation-intensive models are required to monitor every action of the patient precisely. This requirement limits the applicability of such networks.Hence,to keep the model lightweight, the already designed fall detection networks can be extended to monitor the general activities of the patients along with the fall detection.Motivated by the same notion,we propose a novel, lightweight, and efficient patient activity monitoring system that broadly classifies the patients’activities into fall, activity, and rest classes based on their poses. The whole network comprises three sub-networks,namely a Convolutional Neural Networks(CNN)based video compression network,a Lightweight Pose Network(LPN)and a Residual Network(ResNet)Mixer block-based activity recognition network. The compression network compresses the video streams using deep learning networks for efficient storage and retrieval;after that,LPN estimates human poses. Finally, the activity recognition network classifies the patients’activities based on their poses. The proposed system shows an overall accuracy of approx.99.7% over a standard dataset with 99.63% fall detection accuracy and efficiently monitors different events, which may help monitor the falls and improve the inpatients’health. 展开更多

While the interaction between information and disease in static networks has been extensively investigated, many studies have ignored the characteristics of network evolution. In this study,we construct a new two-layer coupling model to explore the interactions between information and disease. The upper layer describes the diffusion of disease-related information, and the lower layer represents the disease transmission. We then use power-law distributions to examine the influence of asymmetric activity levels on dynamic propagation, revealing a mapping relationship characterizing the interconnected propagation of information and diseases among partial nodes within the network.Subsequently,we derive the disease outbreak threshold by using the microscopic Markov-chain approach(MMCA).Finally,we perform extensive Monte Carlo(MC)numerical simulations to verify the accuracy of our theoretical results. Our findings indicate that the activity levels of individuals in the disease transmission layer have a more significant influence on disease transmission compared with the individual activity levels in the information diffusion layer. Moreover, reducing the damping factor can delay disease outbreaks and suppress disease transmission, while improving individual quarantine measures can contribute positively to disease control. This study provides valuable insights into policymakers for developing outbreak prevention and control strategies. 展开更多

Introduction: Carbai tunnel syndrome [CTS] is compression of the median nerve at the wrist , this causes tingling in the hands, pain, numbness, tingling in the fingers particularly the thumb, index and middle fingers, loss of sensation in the hands and fingers, also weakness in the hands. The aim of the present study was to study a possible association which could be found between electrophysiological data in CTS, BMI, hyperlipidemia, and vitamin D [Vit D] levels. Methods: We used a sample of 40 females of the same age group, who were divided into Group 1 as a control consists of 18 healthy females and Group II consisted of 22 age matched females with clinical and electrophysiological evidence of CTS. We measured atherogenic index [AI] as a marker of hyperlipidemia, body mass index [BMI], Vit D status and electrophysiological tests of CTS. Results: Subjects with CTS had deficient Vit D status, they had significantly high atherogenic index (AI), and significant high BMI all compared to control Group I. Median sensory conduction velocity was significantly correlated negatively with BMI and atherogenic index, and positively correlated significantly with Vit D status. But median sensory and motor action potential latency were significantly correlated positively with BMI and atherogenic index, and negatively correlated significantly with Vit D status The analysis revealed BMI, atherogenic index and Vit D status as predictors of median nerve sensory and motor action potential latency and sensory nerve conduction velocity in CTS. Conclusion: The results of this study suggest that obesity and hyperlipidemia are potent CTS risk factors and declared the direct association between Vit D status and CTS occurrence. Our study supports the notion of the compensatory neuroprotective role of Vit D which could have a direct impact on the nerves integrity as it has an anti-inflammatory property which acts in relieving nervous insults and stress. . 展开更多

Traditional selection of combustion catalysis is time-consuming and labor-intensive.Theoretical calculation is expected to resolve this problem. The adsorption energy of HMX and O atoms on 13 metal oxides was calculated using DMol3,since HMX and O are key substances in decomposition process. And the relationship between the adsorption energy of HMX,O on metal oxides(TiO_(2),Al_(2)O_(3),PbO,CuO,Fe_(2)O_(3),Co_(3)O_(4),Bi_(2)O_(3),NiO)and experimental T30 values(time required for the decomposition depth of HMX to reach 30%)was depicted as volcano plot. Thus, the T30 values of other metal oxides was predicted based on their adsorption energy on volcano plot and validated by previous experimental data.Further, the adsorption energy of HMX on ZrO_(2)and MnO_(2)was predicted based on the linear relationship between surface energy and adsorption energy, and T30 values were estimated based on volcano plot. The apparent activation energy data of HMX/MgO,HMX/SnO_(2),HMX/ZrO_(2),and HMX/MnO_(2)obtained from DSC experiments are basically consistent with our predicted T30 values, indicating that it is feasible to predict the catalytic activity based on the adsorption calculation, and it is expected that these simple structural properties can predict adsorption energy to reduce the large quantities of computation and experiment cost. 展开更多

Older people are generally considered dependent on others as they grow older. The purpose of this article is to explore the entrepreneurship activities of Nepalese older adults. Data for this study were collected from the project Help Age International (HAI) implemented in Nepal. Qualitative data observations and interviews were used to collect data. The findings of this study show the formation of the Older People’s Association (OPA) has supported many older people to participate outside the home in various social activities. Moreover, regular deposits through OPAs offer little help. OPAs support older people in their need of financial support to implement minor entrepreneurship. Older people who received support were pleased and were actively involved in their activities and also regularly deposited money in them. Subsequently, older people’s participation in social activities has increased and also helped to lower elderly abuse, loneliness, and depression. Local governments should promote such activities which will help with healthy aging. 展开更多
