Cancer patients are at high risk of malnutrition,which can lead to adverse health outcomes such as prolonged hospitalization,increased complications,and increased mortality.Accurate and timely nutritional assessment p...Cancer patients are at high risk of malnutrition,which can lead to adverse health outcomes such as prolonged hospitalization,increased complications,and increased mortality.Accurate and timely nutritional assessment plays a critical role in effectively managing malnutrition in these patients.However,while many tools exist to assess malnutrition,there is no universally accepted standard.Although different tools have their own strengths and limitations,there is a lack of narrative reviews on nutritional assessment tools for cancer patients.To address this knowledge gap,we conducted a non-systematic literature search using PubMed,Embase,Web of Science,and the Cochrane Library from their inception until May 2023.A total of 90 studies met our selection criteria and were included in our narrative review.We evaluated the applications,strengths,and limitations of 4 commonly used nutritional assessment tools for cancer patients:the Subjective Global Assessment(SGA),Patient-Generated Subjective Global Assessment(PG-SGA),Mini Nutritional Assessment(MNA),and Global Leadership Initiative on Malnutrition(GLIM).Our findings revealed that malnutrition was associated with adverse health outcomes.Each of these 4 tools has its applications,strengths,and limitations.Our findings provide medical staff with a foundation for choosing the optimal tool to rapidly and accurately assess malnutrition in cancer patients.It is essential for medical staff to be familiar with these common tools to ensure effective nutritional management of cancer patients.展开更多
BACKGROUND Few studies have simultaneously compared the predictive value of various frailty assessment tools for outcome measures in patients undergoing gastrointestinal cancer surgery.Therefore,it is difficult to det...BACKGROUND Few studies have simultaneously compared the predictive value of various frailty assessment tools for outcome measures in patients undergoing gastrointestinal cancer surgery.Therefore,it is difficult to determine which assessment tool is most relevant to the prognosis of this population.AIM To investigate the predictive value of three frailty assessment tools for patient prognosis in patients undergoing gastrointestinal cancer surgery.METHODS This single-centre,observational,prospective cohort study was conducted at the Affiliated Lianyungang Hospital of Xuzhou Medical University from August 2021 to July 2022.A total of 229 patients aged≥18 years who underwent surgery for gastrointestinal cancer were included in this study.We collected baseline data on the participants and administered three scales to assess frailty:The comprehen-sive geriatric assessment(CGA),Fried phenotype and FRAIL scale.The outcome measures were the postoperative severe complications and increased hospital RESULTS The prevalence of frailty when assessed with the CGA was 65.9%,47.6%when assessed with the Fried phenotype,and 34.9%when assessed with the FRAIL scale.Using the CGA as a reference,kappa coefficients were 0.398 for the Fried phenotype and 0.291 for the FRAIL scale(both P<0.001).Postoperative severe complications and increased hospital costs were observed in 29(12.7%)and 57(24.9%)patients,respectively.Multivariate logistic analysis confirmed that the CGA was independently associated with increased hospital costs(odds ratio=2.298,95%confidence interval:1.044-5.057;P=0.039).None of the frailty assessment tools were associated with postoperative severe complications.CONCLUSION The CGA was an independent predictor of increased hospital costs in patients undergoing surgery for gastro-intestinal cancer.展开更多

Background:To systematically evaluate the measurement performance of the maternal and child health literacy scale and the study’s methodological quality and to provide a reference for selecting and developing related...Background:To systematically evaluate the measurement performance of the maternal and child health literacy scale and the study’s methodological quality and to provide a reference for selecting and developing related health outcome measurement tools.Methods:Databases such as CNKI,PubMed,and Embase were searched,and the search time frame was established until January 2023.The literature was independently screened by two researchers.The methodological quality and measurement performance of the included scales were evaluated using the health measurement tool selection criteria,and the evaluation results were summarized and analyzed using descriptive analysis.Results:A total of six papers were included,covering six specific scales,with significant differences in the methodological quality and measurement performance of their development studies,none of which evaluated hypothesis testing,the validity of scales,cross-cultural validity,measurement error,or responsiveness.Conclusion:The methodological quality and scale measurement performance of the maternal health literacy inventory in pregnancy,the women’s reproductive health literacy in pregnancy questionnaire,and the maternal and infant health literacy scale development studies are relatively high,but the number of studies on maternal and infant health literacy specific scales is relatively insufficient,and more studies should be conducted in the future.展开更多
Objective:To determine the degree of knowledge in the usage of pulse oximeter as a home assessment tool among the community in Malaysia.Methods:A cross-sectional survey was conducted in November 2021.The questionnaire...Objective:To determine the degree of knowledge in the usage of pulse oximeter as a home assessment tool among the community in Malaysia.Methods:A cross-sectional survey was conducted in November 2021.The questionnaire assessed the knowledge in using pulse oximeters,user experience and opinions in using pulse oximeter as a home assessment tool during the pandemic.Results:A total of 504 respondents were included in the study,and the mean score in knowledge related to application of pulse oximetry was 73.00%,while the mean score in knowledge related to factors affecting pulse oximetry readings was only 38.51%.A total of 90.5% of the respondents recognised normal pulse rate and 88.5% knew the blood oxygen saturation levels of a healthy adult,while 69.4% recognised the definition of silent hypoxia.In addition,the majority of the respondents agreed that factors such as poor blood circulation(71.2%),excessive movements(69.8%),and hand position(60.7%)affected oximetry readings.However,61.7%,81.7%,77.2% and 76.8% of the respondents could not identify nail polish,skin colour,skin thickness and tattoos as factors that may affect oximetry readings respectively.Conclusions:The respondents showed a satisfactory level of knowledge related to application of pulse oximetry,but a poor level of knowledge related to factors affecting pulse oximetry readings among the community in Malaysia.Continuous efforts in educating the community on the correct use of pulse oximeters are crucial for appropriate home assessment and avoiding unnecessary stress.展开更多
BACKGROUND There are very few studies on the differential diagnosis between egocentric neglect(EN)and allocentric neglect(AN).AIM To investigate the overall trend of the previously developed assessment tools by conduc...BACKGROUND There are very few studies on the differential diagnosis between egocentric neglect(EN)and allocentric neglect(AN).AIM To investigate the overall trend of the previously developed assessment tools by conducting a descriptive review of the studies on assessment tools that can perform a differential diagnosis of EN and AN.METHODS The data were collected by using databases such as Google Scholar,PubMed,and ScienceDirect.The most commonly used search terms were“neglect”,“stroke”,“egocentric neglect”,and“allocentric neglect”.RESULTS A total of seven studies that met the inclusion criteria were selected and analyzed.We were able to confirm the research process,test method,and differential diagnosis criteria of the seven presented assessment tools from four studies on paper-based tests and three studies on computerized tests.The majority of the tests were carried out via the cancellation method using stimuli such as everyday objects or numbers.EN distinguished the left from right based on the test paper,while AN distinguished the left from right based on stimuli.In order to perform differential diagnosis,the difference in the number of left and right responses or non-responses was used based on the EN and AN criteria.CONCLUSION It was confirmed that all the seven assessment tools can effectively perform differential diagnosis of EN and AN.This study may provide important data that can be used in clinical practice for differential diagnosis and future intervention planning for neglect patients.展开更多

AIM To evaluate the National Cancer Institute(NCI)Colorectal Cancer(CRC)Risk Assessment Tool as a predictor for the presence of adenomatous polyps(AP) found during screening or surveillance colonoscopy.METHODS This is...AIM To evaluate the National Cancer Institute(NCI)Colorectal Cancer(CRC)Risk Assessment Tool as a predictor for the presence of adenomatous polyps(AP) found during screening or surveillance colonoscopy.METHODS This is a retrospective single center observational study.We collected data of adenomatous polyps in each colonoscopy and then evaluated the lifetime CRC risk.We calculated the AP prevalence across risk score quintiles,odds ratios of the prevalence of AP across risk score quintiles,area under curves(AUCs)and Youden’s indexes to assess the optimal risk score cut off value for AP prevalence status.RESULTS The prevalence of AP gradually increased throughout the five risk score quintiles:i.e.,27.63%in the first and 51.35%in the fifth quintile.The odd ratios of AP prevalence in the fifth quintile compared to the first and second quintile were 2.76[confidence interval(CI):1.71-4.47]and 2.09(CI:1.32-3.30).The AUC for all patients was 0.62(CI:0.58-0.66).Youden’s Index indicated the optimal risk score cutoff value discriminating AP prevalence status was 3.60.CONCLUSION Patients with the higher NCI risk score have higher risk of AP and subsequent CRC;therefore,measures to increase the effectiveness of CRC detection in these patients include longer withdrawal time,early surveillance colonoscopy,and choosing flexible colonoscopy over other CRC screening modalities.展开更多

Aim: The aim of this study is to identify acts of elder abuse from a literature review in order to develop items for an assessment tool for domestic elder abuse. Methods: For the literature search, ICHUSHI Web, China National Knowledge Infrastructure (CNKI), and PubMed were utilized with the keywords “elder abuse” and “elderly abuse”. A search limited to original articles over the last five years containing descriptions of elder abuse was used in this study. In addition, through hand search, books and websites concerning elder abuse were also studied. Descriptions of the acts of elder abuse from the materials were extracted and categorized by content analysis based on seven subtypes of elder abuse (physical abuse, neglect, psychological abuse, sexual abuse, economical abuse, self-neglect, and social abuse). Results & Conclusions: The final analysis included 18 original articles, two books, and eight websites on elder abuse prevention. A total of 38 acts were extracted as items for the assessment tool, which can be used to evaluate the presence and severity of elder abuse. Further research will be needed to examine the validity and reliability of the assessment tool.展开更多
A standard skill assessment(SA) tool was developed and implemented to evaluate the performance of operational forecast models in the Korea operational oceanographic system.The SA tool provided a robust way to assess m...A standard skill assessment(SA) tool was developed and implemented to evaluate the performance of operational forecast models in the Korea operational oceanographic system.The SA tool provided a robust way to assess model skill in the system by comparing predictions and observations,and involved the computation of multiple skill metrics including correlation and error skills.User-and system-based acceptance criteria of skill metrics were applied to determine whether predictions were acceptable for the system.To achieve this,the tool produced a time series comparison plot,a skill score table,and an advanced summarized diagram to effectively demonstrate the multiple skill scores.Moreover,the SA was conducted to evaluate both atmospheric and hydrodynamic forecast variables.For the atmospheric variables,acceptable error criteria were preferable to acceptable correlation criteria over short timescales,since the mean square error overwhelmed the observation variance.Conversely,for the hydrodynamic variables,acceptable root mean square percentage error(e.g.,p erms) criteria were preferable to acceptable error(e.g.,erms) criteria owing to the spatially variable tidal intensity around the Korean Peninsula.Furthermore,the SA indicated that predetermined acceptance error criteria were appropriate to satisfy a target central frequency(fc) for which errors fell within the specified limits(i.e.,the fc equals 70%).展开更多
Objective:This study aims to evaluate the reliability and validity of neuropathic pain assessment tools among Chinese patients with painful diabetic peripheral neuropathy(PDPN).Methods:One hundred patients with PDPN a...Objective:This study aims to evaluate the reliability and validity of neuropathic pain assessment tools among Chinese patients with painful diabetic peripheral neuropathy(PDPN).Methods:One hundred patients with PDPN and 70 patients with non-neuropathic pain were recruited from five grade III general hospitals in Guangzhou.Pain was assessed using the Leeds Assessment of Neuropathic Symptoms and Signs(LANSS),Douleur Neuropathique 4 questionnaire(DN4),and Brief Pain Inventory for Painful Diabetic Peripheral Neuropathy(BPI-DPN).Reliability was evaluated by internal consistency of the Cronbach's a coefficient and Guttman split-half.Construct validity was analyzed by factor analysis and Spearman correlation coefficients.Sensitivity and specificity were also assessed.Results:The Cronbach's a coefficients of the LANSS,DN4,and BPI-DPN were 0.735,0.750,and 0.898,respectively.The Guttman split-half coefficients of the LANSS,DN4,and BPIDPN were 0.660,0.726,and 0.849,respectively.The cumulative contributions of the LANSS,DN4,and BPI-DPN to the total variance were 61.945%,57.010%,and 66.056%,respectively.The items of the LANSS,DN4,and BPI-DPN presented high factorial loads,ranging from 0.387 to 0.841,0.137 to 0.948,and 0.487 to 0.953,respectively.The LANSS and DN4 exhibited sensitivities of 58.0%and 82.7%,respectively,and specificity of 97.1%.Conclusions:The LANSS or DN4 can be used to detect neuropathic pain in Chinese patients with PDPN.The BPI-DPN can be employed to monitor the effectiveness of pain intervention.展开更多

Many hospitals remain deficient in trained infection control professionals (ICPs), and now there is an acute awareness of the need to correct this shortfall. The objectives of the study were to describe the current status of ICP programs at ministry of health (MOH) and to evaluate the current status of infection prevention and control programs at health care facilities in kingdom of Saudi Arabia. A cross sectional interview and a self assessment study were conducted on infection prevention and control (IPC) program in Saudi Arabia. All the MOH hospital in all regions was randomly chosen to the study setting. The selection of facilities was done at random with a clear intention to include twenty regions. A random sample was taken from these hospitals with sample size 56 (55.4%), by Multi stage random sampling technique. Results showed that the total facility in all over the kingdom was 51.4% where central area covered the facility score 58.2%, followed by western area (54.8%), south (53.7%), east (46.3%), and lastly with (45.8%) north area. Survey revealed that the infrastructure for infection control program in Saudi Arabian hospitals remained underdeveloped. There were defects in the identified components of effective infection control programs.展开更多

Background: The effectiveness and value of ...Aims and objectives: To evaluate the effectiveness of a patient education tool (the ENAT) on self efficacy, patient knowledge, health outcomes and its usability in practice. Background: The effectiveness and value of patient education in rheumatoid arthritis have been questioned. The ENAT was developed to help the nurses to provide more focused, individualized and relevant education. The psychometric properties of the ENAT have been assessed. Design: This study is a single blind randomized controlled trial using both quantitative and qualitative methodology. The primary outcome is patient self efficacy. Secondary outcomes are physical function, psychological status, pain levels, social interaction and patient knowledge of disease. Methods: 130 RA patients will be randomized into an experimental (EG) or control group (CG). The EG patients will complete the ENAT prior to seeing a Clinical Nurse Specialist (CNS) at weeks 0, 16 and 32. The CNS will use the ENAT as a template to meet patients perceived educational needs in addition to the usual care. The CG will receive their usual care from the CNS without the aid of the ENAT. To determine the ENAT’s usability, semi-structured quailtative interviews will take place with the practitioners and a sample of patients once they have completed the quantitative arm of the study. Results: The study commenced in April 2011 and the results are expected to be available after April 2013. Conclusions: It is hypothesized that nurses’ use of the ENAT will lead to increased patient self efficacy, and improved patient knowledge and health outcomes among the patients who used it. Furthermore, it is hypothesized that the ENAT will be easy for staff and patients to use. Relevance to clinical practice: The ENAT was developed to assess the perceived educational needs of patients with rheumatoid arthritis and thus help the nurses to provide more focused, individualised and relevant education.展开更多
Water resources face many pressures and stresses around the world.Assessing the vulnerability of water resources is,therefore,increasingly an essential practice in water resources management.Several studies of water r...Water resources face many pressures and stresses around the world.Assessing the vulnerability of water resources is,therefore,increasingly an essential practice in water resources management.Several studies of water resources vulnerability have been carried out around the world during the last three decades.Thus,the main objective of this study was to analyze water resources vulnerability assessment tools and identify the methodological trend of water resources vulnerability assessment at the local level in existing literature.To achieve this objective,the Preferred Reporting Items for Systematic Reviews and Meta-Analyses(PRISMA)method has been applied to include or exclude articles.Articles on water vulnerability numbering 374 were preselected in the ScienceDirect and the Google Scholar databases.Only 32 out of the 374 articles met the inclusion criteria defined for the analysis.The results show that China(21.9%),U.S.A.(9.4%)and Canada(6.3%)are the most active countries publishing water resources vulnerability assessments.Some 191 vulnerability indicators from the 32 articles were classified into five categories of factors:physical factors(33.5%),socio-economic factors(28.3%),environmental or eco-environmental factors(25.1%),institutions and governance factors(7.3%)and infrastructure factors(5.8%).This study shows that almost half of the studies(47%)considered all five categories of factors,9.4%considered four categories of factors,6.3%considered three categories of factors,28%used two types of factors and 9.4%considered only one category.Researchers have used several methods to assess water resources vulnerability:overlay and index methods(59.4%),process-based models(12.5%),statistical methods(9.4%),overlay and index methods plus process-based model(9.4%),overlay and index methods plus statistical methods(6.3%)and statistical methods plus process-based models(3.1%).Geographic information system(GIS)is an important tool in assessing the vulnerability of water resources with almost 60%of the studies using it at some stage of the assessment.In recent years,there is an increasing trend toward conceptualizing the vulnerability of water resources in a holistic way.展开更多

Basically, many assessment tools have been suggested for measuring emission scenarios and achieve sustainability within university campuses. However, majority of the existing tools consider greenhouse gas emissions outside the campuses upon which universities have no control. This study identifies energy as a major driving force for Carbon Dioxide (CO2) emission and a universal variable for the assessment of CO2 emission in university campus. It proposed the Malaysia University Carbon Emission Tool (MUCET) to estimate Carbon Dioxide from the sources of energy use within the campus and assess the CO2 emission scenario within Universiti Teknologi Malaysia (UTM). The result was compared to that of similar carbon emission inventory initiated by the UTM in 2009, a total emission of 46,000 Mt CO2 was observed in 2011 using MUCET compared to 82,578 Mt CO2 given as the total CO2 emission value for the campus in 2009. Also MUCET presented 74% CO2 emission for electricity and 26% in the transportation sector in variance to 78% and 20% offered for electricity and transport respectively in 2009 using the contemporary method of assessment. The difference in values was attributed to the contribution of the indirect and off-site emissions (i.e. Scope 3 emissions) from external sources. The study showed that MUCET presents a more realistic emission value and would promote sustainability on the campus.展开更多

Therefore, the Diabetes Oral Health Assessment Tool (DiO-HAT ?) was developed with items contributed by health care professionals (diabetologists, periodontal specialists, a preventive dentist, a Certified Nurse in Diabetes Nursing, a national registered dietitian, registered nurses, a dental hygienist, and nursing researchers) who were involved in the medical care of patients with diabetes. Subsequently, a survey of 700 Diabetes Nurse Specialists (DNS) was conducted to determine their score of recognition and implementation of the DiOHAT ?, however, 304 participants (43.4%) responded. Constructive concept validation and the Cronbach’s alpha coefficient for all assessment items were 0.931, indicating high reliability: Factor 1, Patient’s oral health status (α = 0.831);Factor 2, Implementation of oral health behaviors (α = 0.890);Factor 3, Information transmission regarding dental visits (α = 0.862);and Factor 4, Perceptions and knowledge of oral health behaviors (α = 0.793). Although the mean score of recognition of DiOHAT ? was 3.5 ± 0.4 points, the mean value of the implementation score was 1.5 ± 0.5 points (obtained using a 4-grade scale). The implementation scores were significantly lower than the recognition scores for all items (p < 0.001). 文摘Cancer patients are at high risk of malnutrition,which can lead to adverse health outcomes such as prolonged hospitalization,increased complications,and increased mortality.Accurate and timely nutritional assessment plays a critical role in effectively managing malnutrition in these patients.However,while many tools exist to assess malnutrition,there is no universally accepted standard.Although different tools have their own strengths and limitations,there is a lack of narrative reviews on nutritional assessment tools for cancer patients.To address this knowledge gap,we conducted a non-systematic literature search using PubMed,Embase,Web of Science,and the Cochrane Library from their inception until May 2023.A total of 90 studies met our selection criteria and were included in our narrative review.We evaluated the applications,strengths,and limitations of 4 commonly used nutritional assessment tools for cancer patients:the Subjective Global Assessment(SGA),Patient-Generated Subjective Global Assessment(PG-SGA),Mini Nutritional Assessment(MNA),and Global Leadership Initiative on Malnutrition(GLIM).Our findings revealed that malnutrition was associated with adverse health outcomes.Each of these 4 tools has its applications,strengths,and limitations.Our findings provide medical staff with a foundation for choosing the optimal tool to rapidly and accurately assess malnutrition in cancer patients.It is essential for medical staff to be familiar with these common tools to ensure effective nutritional management of cancer patients.

文摘BACKGROUND Few studies have simultaneously compared the predictive value of various frailty assessment tools for outcome measures in patients undergoing gastrointestinal cancer surgery.Therefore,it is difficult to determine which assessment tool is most relevant to the prognosis of this population.AIM To investigate the predictive value of three frailty assessment tools for patient prognosis in patients undergoing gastrointestinal cancer surgery.METHODS This single-centre,observational,prospective cohort study was conducted at the Affiliated Lianyungang Hospital of Xuzhou Medical University from August 2021 to July 2022.A total of 229 patients aged≥18 years who underwent surgery for gastrointestinal cancer were included in this study.We collected baseline data on the participants and administered three scales to assess frailty:The comprehen-sive geriatric assessment(CGA),Fried phenotype and FRAIL scale.The outcome measures were the postoperative severe complications and increased hospital RESULTS The prevalence of frailty when assessed with the CGA was 65.9%,47.6%when assessed with the Fried phenotype,and 34.9%when assessed with the FRAIL scale.Using the CGA as a reference,kappa coefficients were 0.398 for the Fried phenotype and 0.291 for the FRAIL scale(both P<0.001).Postoperative severe complications and increased hospital costs were observed in 29(12.7%)and 57(24.9%)patients,respectively.Multivariate logistic analysis confirmed that the CGA was independently associated with increased hospital costs(odds ratio=2.298,95%confidence interval:1.044-5.057;P=0.039).None of the frailty assessment tools were associated with postoperative severe complications.CONCLUSION The CGA was an independent predictor of increased hospital costs in patients undergoing surgery for gastro-intestinal cancer.

文摘With the continuous development of science and technology,artificial intelligence(AI)is coming into our lives and changing our lives.Since China entered the aging society in 2000,the degree of population aging has deepened.Comprehensive geriatric assessment(CGA)is now the accepted gold standard for the care of older people in hospitals.However,some problems limit the clinical application,such as complexity and time consuming.Therefore,by analyzing previous studies,we summarize some existing AI tools in order to find a more optimized assessment tool to complete the entire CGA process.
文摘Background:To systematically evaluate the measurement performance of the maternal and child health literacy scale and the study’s methodological quality and to provide a reference for selecting and developing related health outcome measurement tools.Methods:Databases such as CNKI,PubMed,and Embase were searched,and the search time frame was established until January 2023.The literature was independently screened by two researchers.The methodological quality and measurement performance of the included scales were evaluated using the health measurement tool selection criteria,and the evaluation results were summarized and analyzed using descriptive analysis.Results:A total of six papers were included,covering six specific scales,with significant differences in the methodological quality and measurement performance of their development studies,none of which evaluated hypothesis testing,the validity of scales,cross-cultural validity,measurement error,or responsiveness.Conclusion:The methodological quality and scale measurement performance of the maternal health literacy inventory in pregnancy,the women’s reproductive health literacy in pregnancy questionnaire,and the maternal and infant health literacy scale development studies are relatively high,but the number of studies on maternal and infant health literacy specific scales is relatively insufficient,and more studies should be conducted in the future.
文摘Objective:To determine the degree of knowledge in the usage of pulse oximeter as a home assessment tool among the community in Malaysia.Methods:A cross-sectional survey was conducted in November 2021.The questionnaire assessed the knowledge in using pulse oximeters,user experience and opinions in using pulse oximeter as a home assessment tool during the pandemic.Results:A total of 504 respondents were included in the study,and the mean score in knowledge related to application of pulse oximetry was 73.00%,while the mean score in knowledge related to factors affecting pulse oximetry readings was only 38.51%.A total of 90.5% of the respondents recognised normal pulse rate and 88.5% knew the blood oxygen saturation levels of a healthy adult,while 69.4% recognised the definition of silent hypoxia.In addition,the majority of the respondents agreed that factors such as poor blood circulation(71.2%),excessive movements(69.8%),and hand position(60.7%)affected oximetry readings.However,61.7%,81.7%,77.2% and 76.8% of the respondents could not identify nail polish,skin colour,skin thickness and tattoos as factors that may affect oximetry readings respectively.Conclusions:The respondents showed a satisfactory level of knowledge related to application of pulse oximetry,but a poor level of knowledge related to factors affecting pulse oximetry readings among the community in Malaysia.Continuous efforts in educating the community on the correct use of pulse oximeters are crucial for appropriate home assessment and avoiding unnecessary stress.

文摘BACKGROUND There are very few studies on the differential diagnosis between egocentric neglect(EN)and allocentric neglect(AN).AIM To investigate the overall trend of the previously developed assessment tools by conducting a descriptive review of the studies on assessment tools that can perform a differential diagnosis of EN and AN.METHODS The data were collected by using databases such as Google Scholar,PubMed,and ScienceDirect.The most commonly used search terms were“neglect”,“stroke”,“egocentric neglect”,and“allocentric neglect”.RESULTS A total of seven studies that met the inclusion criteria were selected and analyzed.We were able to confirm the research process,test method,and differential diagnosis criteria of the seven presented assessment tools from four studies on paper-based tests and three studies on computerized tests.The majority of the tests were carried out via the cancellation method using stimuli such as everyday objects or numbers.EN distinguished the left from right based on the test paper,while AN distinguished the left from right based on stimuli.In order to perform differential diagnosis,the difference in the number of left and right responses or non-responses was used based on the EN and AN criteria.CONCLUSION It was confirmed that all the seven assessment tools can effectively perform differential diagnosis of EN and AN.This study may provide important data that can be used in clinical practice for differential diagnosis and future intervention planning for neglect patients.

文摘Methodological quality(risk of bias)assessment is an important step before study initiation usage.Therefore,accurately judging study type is the first priority,and the choosing proper tool is also important.In this review,we introduced methodological quality assessment tools for randomized controlled trial(including individual and cluster),animal study,non-randomized interventional studies(including follow-up study,controlled before-and-after study,before-after/pre-post study,uncontrolled longitudinal study,interrupted time series study),cohort study,case-control study,cross-sectional study(including analytical and descriptive),observational case series and case reports,comparative effectiveness research,diagnostic study,health economic evaluation,prediction study(including predictor finding study,prediction model impact study,prognostic prediction model study),qualitative study,outcome measurement instruments(including patient-reported outcome measure development,content validity,structural validity,internal consistency,cross-cultural validity/measurement invariance,reliability,measurement error,criterion validity,hypotheses testing for construct validity,and responsiveness),systematic review and meta-analysis,and clinical practice guideline.The readers of our review can distinguish the types of medical studies and choose appropriate tools.In one word,comprehensively mastering relevant knowledge and implementing more practices are basic requirements for correctly assessing the methodological quality.
文摘AIM To evaluate the National Cancer Institute(NCI)Colorectal Cancer(CRC)Risk Assessment Tool as a predictor for the presence of adenomatous polyps(AP) found during screening or surveillance colonoscopy.METHODS This is a retrospective single center observational study.We collected data of adenomatous polyps in each colonoscopy and then evaluated the lifetime CRC risk.We calculated the AP prevalence across risk score quintiles,odds ratios of the prevalence of AP across risk score quintiles,area under curves(AUCs)and Youden’s indexes to assess the optimal risk score cut off value for AP prevalence status.RESULTS The prevalence of AP gradually increased throughout the five risk score quintiles:i.e.,27.63%in the first and 51.35%in the fifth quintile.The odd ratios of AP prevalence in the fifth quintile compared to the first and second quintile were 2.76[confidence interval(CI):1.71-4.47]and 2.09(CI:1.32-3.30).The AUC for all patients was 0.62(CI:0.58-0.66).Youden’s Index indicated the optimal risk score cutoff value discriminating AP prevalence status was 3.60.CONCLUSION Patients with the higher NCI risk score have higher risk of AP and subsequent CRC;therefore,measures to increase the effectiveness of CRC detection in these patients include longer withdrawal time,early surveillance colonoscopy,and choosing flexible colonoscopy over other CRC screening modalities.

文摘There is a discrepancy between the information from clotting tests which have routinely been used in clinical practice and evidence regarding thrombotic and bleeding events in patients with liver disease. This discrepancy leads us to rely on other variables which have been shown to be involved in haemostasis in these patients and/or to extrapolate the behaviour of these patients to other settings in order to decide the best clinical approach. The aims of the present review are as follows:(1) to present the information provided by clotting tests in cirrhotic patients;(2) to present the factors that may influence clotting in these patients;(3) to review the clinical evidence; and(4) to put forward a clinical approach based on the first 3 points.
文摘Background: With the “super-aging” of society, the prevalence of elder abuse can be expected to come under greater scrutiny. It will be important, therefore, to evaluate the presence and severity of elder abuse so as to prevent abuse and provide greater support for families. Aim: The aim of this study is to identify acts of elder abuse from a literature review in order to develop items for an assessment tool for domestic elder abuse. Methods: For the literature search, ICHUSHI Web, China National Knowledge Infrastructure (CNKI), and PubMed were utilized with the keywords “elder abuse” and “elderly abuse”. A search limited to original articles over the last five years containing descriptions of elder abuse was used in this study. In addition, through hand search, books and websites concerning elder abuse were also studied. Descriptions of the acts of elder abuse from the materials were extracted and categorized by content analysis based on seven subtypes of elder abuse (physical abuse, neglect, psychological abuse, sexual abuse, economical abuse, self-neglect, and social abuse). Results & Conclusions: The final analysis included 18 original articles, two books, and eight websites on elder abuse prevention. A total of 38 acts were extracted as items for the assessment tool, which can be used to evaluate the presence and severity of elder abuse. Further research will be needed to examine the validity and reliability of the assessment tool.

文摘A standard skill assessment(SA) tool was developed and implemented to evaluate the performance of operational forecast models in the Korea operational oceanographic system.The SA tool provided a robust way to assess model skill in the system by comparing predictions and observations,and involved the computation of multiple skill metrics including correlation and error skills.User-and system-based acceptance criteria of skill metrics were applied to determine whether predictions were acceptable for the system.To achieve this,the tool produced a time series comparison plot,a skill score table,and an advanced summarized diagram to effectively demonstrate the multiple skill scores.Moreover,the SA was conducted to evaluate both atmospheric and hydrodynamic forecast variables.For the atmospheric variables,acceptable error criteria were preferable to acceptable correlation criteria over short timescales,since the mean square error overwhelmed the observation variance.Conversely,for the hydrodynamic variables,acceptable root mean square percentage error(e.g.,p erms) criteria were preferable to acceptable error(e.g.,erms) criteria owing to the spatially variable tidal intensity around the Korean Peninsula.Furthermore,the SA indicated that predetermined acceptance error criteria were appropriate to satisfy a target central frequency(fc) for which errors fell within the specified limits(i.e.,the fc equals 70%).
文摘Objective:This study aims to evaluate the reliability and validity of neuropathic pain assessment tools among Chinese patients with painful diabetic peripheral neuropathy(PDPN).Methods:One hundred patients with PDPN and 70 patients with non-neuropathic pain were recruited from five grade III general hospitals in Guangzhou.Pain was assessed using the Leeds Assessment of Neuropathic Symptoms and Signs(LANSS),Douleur Neuropathique 4 questionnaire(DN4),and Brief Pain Inventory for Painful Diabetic Peripheral Neuropathy(BPI-DPN).Reliability was evaluated by internal consistency of the Cronbach's a coefficient and Guttman split-half.Construct validity was analyzed by factor analysis and Spearman correlation coefficients.Sensitivity and specificity were also assessed.Results:The Cronbach's a coefficients of the LANSS,DN4,and BPI-DPN were 0.735,0.750,and 0.898,respectively.The Guttman split-half coefficients of the LANSS,DN4,and BPIDPN were 0.660,0.726,and 0.849,respectively.The cumulative contributions of the LANSS,DN4,and BPI-DPN to the total variance were 61.945%,57.010%,and 66.056%,respectively.The items of the LANSS,DN4,and BPI-DPN presented high factorial loads,ranging from 0.387 to 0.841,0.137 to 0.948,and 0.487 to 0.953,respectively.The LANSS and DN4 exhibited sensitivities of 58.0%and 82.7%,respectively,and specificity of 97.1%.Conclusions:The LANSS or DN4 can be used to detect neuropathic pain in Chinese patients with PDPN.The BPI-DPN can be employed to monitor the effectiveness of pain intervention.

文摘Infection Prevention and Control is a Quality Standard and is crucial in all health care facilities. Many hospitals remain deficient in trained infection control professionals (ICPs), and now there is an acute awareness of the need to correct this shortfall. The objectives of the study were to describe the current status of ICP programs at ministry of health (MOH) and to evaluate the current status of infection prevention and control programs at health care facilities in kingdom of Saudi Arabia. A cross sectional interview and a self assessment study were conducted on infection prevention and control (IPC) program in Saudi Arabia. All the MOH hospital in all regions was randomly chosen to the study setting. The selection of facilities was done at random with a clear intention to include twenty regions. A random sample was taken from these hospitals with sample size 56 (55.4%), by Multi stage random sampling technique. Results showed that the total facility in all over the kingdom was 51.4% where central area covered the facility score 58.2%, followed by western area (54.8%), south (53.7%), east (46.3%), and lastly with (45.8%) north area. Survey revealed that the infrastructure for infection control program in Saudi Arabian hospitals remained underdeveloped. There were defects in the identified components of effective infection control programs.
文摘Aims and objectives: To evaluate the effectiveness of a patient education tool (the ENAT) on self efficacy, patient knowledge, health outcomes and its usability in practice. Background: The effectiveness and value of patient education in rheumatoid arthritis have been questioned. The ENAT was developed to help the nurses to provide more focused, individualized and relevant education. The psychometric properties of the ENAT have been assessed. Design: This study is a single blind randomized controlled trial using both quantitative and qualitative methodology. The primary outcome is patient self efficacy. Secondary outcomes are physical function, psychological status, pain levels, social interaction and patient knowledge of disease. Methods: 130 RA patients will be randomized into an experimental (EG) or control group (CG). The EG patients will complete the ENAT prior to seeing a Clinical Nurse Specialist (CNS) at weeks 0, 16 and 32. The CNS will use the ENAT as a template to meet patients perceived educational needs in addition to the usual care. The CG will receive their usual care from the CNS without the aid of the ENAT. To determine the ENAT’s usability, semi-structured quailtative interviews will take place with the practitioners and a sample of patients once they have completed the quantitative arm of the study. Results: The study commenced in April 2011 and the results are expected to be available after April 2013. Conclusions: It is hypothesized that nurses’ use of the ENAT will lead to increased patient self efficacy, and improved patient knowledge and health outcomes among the patients who used it. Furthermore, it is hypothesized that the ENAT will be easy for staff and patients to use. Relevance to clinical practice: The ENAT was developed to assess the perceived educational needs of patients with rheumatoid arthritis and thus help the nurses to provide more focused, individualised and relevant education.
文摘Water resources face many pressures and stresses around the world.Assessing the vulnerability of water resources is,therefore,increasingly an essential practice in water resources management.Several studies of water resources vulnerability have been carried out around the world during the last three decades.Thus,the main objective of this study was to analyze water resources vulnerability assessment tools and identify the methodological trend of water resources vulnerability assessment at the local level in existing literature.To achieve this objective,the Preferred Reporting Items for Systematic Reviews and Meta-Analyses(PRISMA)method has been applied to include or exclude articles.Articles on water vulnerability numbering 374 were preselected in the ScienceDirect and the Google Scholar databases.Only 32 out of the 374 articles met the inclusion criteria defined for the analysis.The results show that China(21.9%),U.S.A.(9.4%)and Canada(6.3%)are the most active countries publishing water resources vulnerability assessments.Some 191 vulnerability indicators from the 32 articles were classified into five categories of factors:physical factors(33.5%),socio-economic factors(28.3%),environmental or eco-environmental factors(25.1%),institutions and governance factors(7.3%)and infrastructure factors(5.8%).This study shows that almost half of the studies(47%)considered all five categories of factors,9.4%considered four categories of factors,6.3%considered three categories of factors,28%used two types of factors and 9.4%considered only one category.Researchers have used several methods to assess water resources vulnerability:overlay and index methods(59.4%),process-based models(12.5%),statistical methods(9.4%),overlay and index methods plus process-based model(9.4%),overlay and index methods plus statistical methods(6.3%)and statistical methods plus process-based models(3.1%).Geographic information system(GIS)is an important tool in assessing the vulnerability of water resources with almost 60%of the studies using it at some stage of the assessment.In recent years,there is an increasing trend toward conceptualizing the vulnerability of water resources in a holistic way.
文摘Universities are challenged to play a leadership role in addressing unsustainable practices that are exacerbating global warming. Basically, many assessment tools have been suggested for measuring emission scenarios and achieve sustainability within university campuses. However, majority of the existing tools consider greenhouse gas emissions outside the campuses upon which universities have no control. This study identifies energy as a major driving force for Carbon Dioxide (CO2) emission and a universal variable for the assessment of CO2 emission in university campus. It proposed the Malaysia University Carbon Emission Tool (MUCET) to estimate Carbon Dioxide from the sources of energy use within the campus and assess the CO2 emission scenario within Universiti Teknologi Malaysia (UTM). The result was compared to that of similar carbon emission inventory initiated by the UTM in 2009, a total emission of 46,000 Mt CO2 was observed in 2011 using MUCET compared to 82,578 Mt CO2 given as the total CO2 emission value for the campus in 2009. Also MUCET presented 74% CO2 emission for electricity and 26% in the transportation sector in variance to 78% and 20% offered for electricity and transport respectively in 2009 using the contemporary method of assessment. The difference in values was attributed to the contribution of the indirect and off-site emissions (i.e. Scope 3 emissions) from external sources. The study showed that MUCET presents a more realistic emission value and would promote sustainability on the campus.
文摘The performance on prediction by mathematical models which represent the conceived image of a system such as hydrology is oftentimes represented through calibration and verification processes. Oftentimes a best fit between observed and predicted flows is obtained through correlation coefficient (R2) and the Nash Sutcliffe model efficiency (NSE) by minimizing the average Root Mean Square Error (RMSE) of the observed versus simulated flows. However, these days, a new paradigm is emerging wherein accounting for the flow variability for the protection of freshwater biodiversity and maintenance of goods and services that rivers provide is paramount. Therefore, from an ecohydrology perspective, it is not clear if the existing method of model calibration meets the needs of the riverine ecosystem at its best. Thus, this study investigates and proposes a methodology using entropy theory to gage the calibration of Soil and Water Assessment Tool (SWAT) from an ecohydrology perspective characterized by the natural flow-regime paradigm: Indicators of Hydrologic Alteration.
文摘Although some studies had suggested a bidirectional relationship between diabetes and periodontal disease, there were no appropriate tools for nurses to evaluate oral status and oral health behaviors in patients with diabetes. Therefore, the Diabetes Oral Health Assessment Tool (DiO-HAT ?) was developed with items contributed by health care professionals (diabetologists, periodontal specialists, a preventive dentist, a Certified Nurse in Diabetes Nursing, a national registered dietitian, registered nurses, a dental hygienist, and nursing researchers) who were involved in the medical care of patients with diabetes. Subsequently, a survey of 700 Diabetes Nurse Specialists (DNS) was conducted to determine their score of recognition and implementation of the DiOHAT ?, however, 304 participants (43.4%) responded. Constructive concept validation and the Cronbach’s alpha coefficient for all assessment items were 0.931, indicating high reliability: Factor 1, Patient’s oral health status (α = 0.831);Factor 2, Implementation of oral health behaviors (α = 0.890);Factor 3, Information transmission regarding dental visits (α = 0.862);and Factor 4, Perceptions and knowledge of oral health behaviors (α = 0.793).
