'Harmony between the heart and kidney' refers to the physiological relationship between these two zang organs in Chinese medicine, while 'disharmony between the heart and kidney' (also called disharmony between fire and water) refers to the pathological state. The pattern of disharmony between the heart and kidney is widely observed in patients with insomnia, anxiety disorder and menopausal syndrome,etc..In order to gain a full and systematical understanding of this pattern, from the perspective of ancient Chinese philosophy and zang-fu theory in Chinese medicine,we systematically reviewed and discussed the functions and physiological characteristics of the heart and kidney, the origin and development of theories relating to heart-kidney relationship, the pathogenesis and identification of the pattern,as well as the commonly used classical formulas for its treatment, including Coptis and DonkeyHide Gelatin Decoction (Huánglián (E)jiāo Tāng) and Grand Communication Pill (Jiāotài Wán).Two examples of clinical modifications of these formulas in the treatment of insomnia and menopausal syndrome are provided in this article. It should be noted that in clinical practice, these formulas should be used flexibly, and modified in accordance to the condition of the patient. 展开更多

Cyclic 3’,5’-adenosine monophosphate(cAMP)and cyclic 3’,5’-guanosine monophosphate(cGMP)are considered as potential biomarkers for Yin-Yang disharmony in traditional Chinese medicine.However,phosphodiesterase-mediated ex vivo degradation of these molecules in biological samples may result in their underestimation. In the present study,a ultra-high performance liquid chromatography-tandem mass spectrometry(UHPLC-MS/MS)method was developed for determination of cAMP and cGMP in rat plasma, with special consideration of their stability ex vivo.Following precipitation of proteins from plasma samples with 0.4 M perchloric acid, the analytes were chromatographed on a Shimadzu Shimpack-XR-ODS II column with 2.5 mM ammonium acetate and methanol in gradient mode. The MS/MS detection was performed using multiple reaction monitoring in the positive electrospray ionization mode. The lower limit of quantification was 0.27 ng/mL for cAMP and 0.37 ng/mL for cGMP.The method was used to determine the plasma cAMP and cGMP levels in normal and Yin deficiency diabetic rats treated with or without Rehmannia glutinosa. The developed method may be useful for evaluating the regulatory effects of Chinese herbal medicine on the levels of cAMP and cGMP in the body. 展开更多

Previous studies have demonstrated that disharmony among voltage-source-controlled units(VSCUs)may occur on an alternating current(AC)transmission or distribution line under steady-state operating conditions(SSOCs)or quasistatic operating conditions(QSSOCs).As the studies on frequency disharmony have been expanded to multiple disharmonized vS-CUs in the local power grid, its adverse effects on AC lines and equivalent load(EL)at the bus without active voltage control ability(non-active bus)need to be investigated further.Considering the locality of disharmony and common topological con-nections among VSCUs,this paper adopts a Y-type three-termi-nal local power grid(LPG)as the research object. The disharmony among the three VSCUs is discussed.Firstly, for the load at non-active bus,the formulas for single-phase instantaneous voltage, load current, load power,as well as average power un-der disharmony operating conditions(DOCs)are derived. The characteristic indicators of the above electrical quantities are defined, which can measure the amplification and reduction de-grees of the above electrical quantities before and after disharmony.Secondly, for the line directly connected to VSCUs,the formulas for single-phase instantaneous line current and power and the average power under DOCs are derived. The characteristic indicators of power flow are defined, which can be used to quantify the peak amplification impact of oscillation before and after disharmony. Finally, the case study on the Y-type three-terminal LPG under the single-disharmony and the multi-dis-harmony switching scenarios indicates that the long-period pow-er oscillation caused by disharmony may occur in the load flow at the non-active bus and the line flow. The oscillation causes a serious decrease in load capability and a significant amplification of the peak of line power oscillation. 展开更多

OBJECTIVE: To systematically assess the clinical ef- fectiveness of traditional Chinese medicine (TCM) in the treatment of functional dyspepsia (FD) of liv- er-stomach disharmony syndrome by meta-analysis. METHODS: Random controlled trials (RCTs) were retrieved from databases, including Pubmed, China national knowledge infrastructure, Wanfang Data, VlP Information, and the Cochrane Library. Trials were selected according to inclusion criteria. The ef- fects of traditional Chinese medicine (TCM) versus prokinetic agents in the treatment of functional dyspepsia (FD) of liver-stomach disharmony syn- drome were compared by meta-analysis. RevMan 5.0.24 was used for data analysis. The effective rate was assessed by odds ratios (ORs), and 95% confi- dence intervals (C'/s) were calculated. The cure rate was analyzed by the Peto OR. Simple statistica~ anal- ysis was chosen to assess the frequency of pre- scribed Chinese herbs in treating this syndrome.RESULTS: Thirteen trials were included, involving 1153 patients, and these were of poor methodolog- ical quality. Twelve studies mentioned the effective rate and cure rate. TCM therapy showed a better clinical effect rate compared with that with proki- netic agents [OR: 3.2, 95% CI (2.27, 4.51)]. The TCM group also had a better cure rate than that in the group of prokinetic agents [Peto OR: 2.26, 95% CI (1.61, 3.18)]. With regard to the frequency of Chi- nese herbs used in these 13 trials, Baishao (Radix Paeoniae), Chaihu (Radix Bupleuri), and Gancao (Ra- dix Glycyrrhizae) were mostly prescribed, followed by Xiangfu (Rhizoma Cyperi), Zhishi (Fructus Auran- tii Immaturus), Zhiqiao (Fructus Aurantii), Foshou (Citrus medica var.sarcodactylis), and Chenpi (Peri- carpium Citri Reticulatae). No serious adverse ef- fects were reported. CONCLUSION: TCM therapy shows a superior effec- tive rate and cure rate compared with those in pro- kinetic agents in the treatment of FD of liver-stom- ach disharmony syndrome. However, further strict- ly designed RCTs are required because of the poor quality of included trials. 展开更多

Objective: To observe the clinical effect of acupoint sticking of Chinese medicine at Shenque (CV 8) for preventing spleenstomach disharmony caused by venous analgesia pump. Methods: A total of 120 post-surgery patients using venous analgesia pump and also conforming to the inclusion criteria were randomized into 2 groups by the random number table. There were 58 patients in the control group who received conventional post-surgery nursing care, and there were 62 patients in the treatment group who received acupoint sticking at Shenque (CV 8) on the basis of conventional nursing care. After treatment, the incidences of postoperative nausea and vomiting, and abdominal bloating were measured. Results: The total incidence of nausea, vomiting and abdominal bloating in the treatment group was 11.3%, versus 39.7% in the control group, and the difference of the incidence between the two groups showed a statistical significance (P〈0.05). In comparison of the severity of spleen-stomach dishormony which happened during 72 h after surgery, cases in the treatment group showed lower severity than those in the control group (P〈0.05). Conclusion: Acupoint sticking at Shenque (CV 8) can effectively prevent spleen-stomach disharmony caused by venous analgesia pump, which will alleviate sufferings of the patients. 展开更多

BACKGROUND Tong Xie Yao Fang is a representative traditional Chinese prescription for the treatment of liver and spleen deficiency, abdominal pain and diarrhea. It has a unique function in the treatment of gastrointestinal dysfunction including irritable bowel syndrome(IBS),is a common functional bowel disease. Its main symptoms are recurrent abdominal pain, diarrhea, constipation or alternations between diarrhea and constipation. There are obvious differences in metabolites between TCM syndromes.By comparing the body fluid metabolism maps of model animals, metabolomics can discover disease biomarkers, analyze the differences in metabolic pathways and understand the pathological process and the metabolic pathways of substances in the body. Thus, the evaluation of animal models tends to be comprehensive and objective. This may provide further understanding between the interaction between Tong Xie Yao Fang and the IBS model.AIM To evaluate the effect of Tong Xie Yao Fang on IBS rats by using metabolomics method.METHODS Wistar rats were used to establish IBS models, and then randomly divided into four groups:A model control group and three Tong Xie Yao Fang treatment groups(high, medium and low doses).A normal,non-IBS group was established. The rats were treated for 2 wk.On days 0 and 14 of the experimental model, urine was collected for 12 h and was analyzed by ultra-performance liquid chromatography quadrupole time-of-flight mass spectrometry.Nine potential biomarkers were identified, and six major metabolic pathways were found to be related to IBS.RESULTS In the study of metabonomics, nine potential biomarkers including L-serine,4-methylgallic acid,L-threonine,succinylacetone,prolyl-hydroxyproline,valylserine,acetyl citrate, marmesin rutinoside and 5-hydroxy-L-tryptophan were identified in urine, which were assigned to amino acids, organic acids, succinyl and glycosides.Furthermore, the metabolic pathway of L-serine,L-threonine and 5-hydroxy-L-tryptophan was found in the Kyoto Encyclopedia of Genes and Genomes, which mainly involved the metabolism of cysteine and methionine, vitamin B6 metabolism, serotonin synapse, tryptophan metabolism, sphingolipid metabolism, digestion, absorption of protein and amino acid metabolism. These pathways are related to intestinal dysfunction, inflammatory syndrome, nervous system dysfunction and other diseases.CONCLUSION Tong Xie Yao Fang has pharmacological effects on IBS,and its mechanism may be related to the metabolism of the nine potential biomarkers identified above in urine. 展开更多

In the last issue, two case reports separately present examples of the extremely rare and complex congenital heart diseases that show concordant atrioventricular connections to the L-looped ventricles in the presence of situs solitus. Both cases highlight that the relationship between the two ventricles within the ventricular mass is not always harmonious with the given atrioventricular connection.Such disharmony between the connections and relationships requires careful assessment of the three basic facets of cardiac building blocks, namely their morphology, the relationship of their component parts, and their connections with the adjacent segments.3D imaging and printing can now facilitate an otherwise difficult diagnosis in such complex situations.Rotation of either the 3D images or the models permit accurate assessment of the ventricular topologic pattern by creating the right ventricular en-face septal view, thus facilitating placement of the observer’s hands. As we now emphasize,an alternative approach, which might prove more attractive to imagers,is to rotate the ventricular mass to provide the ventricular apical view, thus permitting determination of the ventricular relationship without using the hands. 展开更多
