However, in certain cases these infections may invade head and neck tissues, resulting in orofacial abscesses, cellulitis and sepsis, with resultant high morbidity and even mortality. In the present studies, we developed a novel model of spreading dentoalveolar infections in mice by treatment with neutralizing antibodies against both interleukin-la (IL-1a) and IL-1β. Surprisingly male but not female mice given anti-lL-1 antibodies developed orofacial abscesses, weight loss, splenomegaly and sepsis. Female mice developed abscesses and sepsis comparable to males following ovariectomy (OVX), which was reversed by estrogen supplementation. Anti-lL-1 blockade inhibited IL-12, interferon y (IFNy) and IL-6 but not IL-IO expression in infrabony lesions, suggestive of a local anti-inflammatory response. There was greater infiltration of neutrophils and other inflammatory ceils into lesions in anti-lL-l-treated animals; however, blood leukocytes had reduced bacterial phagocytic and killing activity ex vivo. Estrogen directly stimulated IL-1 production by macrophages, suggesting that the resistance of females to disseminating dentoalveolar infections may be due to their heightened pro-inflammatory responses following bacterial challenge, leading to enhanced localization of these infections.展开更多

A total of 30 floristic plots of 40 m × 40 m (1600 m2) were delineated in the different ecosystems (forest, forest-savanna contact zone and the savanna). Within each plot, all individuals with a diameter at breast height ≥ 10 cm at 1.30 m were taken into account. ANOVA was used to compare the means of the diversity indices of the different ecosystems, and the DUNCAN test at 5% significance level (XLSTAT 2016) to separate these means. A total of 85 species belonging to 67 genera and 33 families were recorded. That is, 68 species in semi-caducifolia forest, 45 in forest-savanna contact zone and 18 in savanna. Urticaceae, Combretaceae, Apocynaceae, Myristicaceae and Euphorbiaceae are the most diverse families in the forest. On the other hand, in contact zones, the Euphorbiaceae, Fabaceae, Annonaceae and Moraceae families are the most diversified, while in the savannas, the Combretaceae, Fabaceae and Annonaceae families are the most diversified. Shannon’s diversity index values (1.7 to 2.37 bits) reflect low specific diversity;Piélou’s equitability (0.86 to 0.9) shows good distribution of individuals within species. The diametric structure of woody species shows an inverted “J” shape in all plant communities. This indicates a high regeneration potential characterized by a greater number of small trees than large trees. Mesophanerophytes and megaphanerophytes are dominant in forests and forest-savanna contact zones, and microphanerophytes in the savannas. Endozoochory is the main mode of diaspore dissemination. As regards the distribution of phytogeographical types, it shows the overall dominance of Guineo-Congolese species in all plant communities. Our study provides detailed data on forest-savanna floristic and plant life traits in the mosaic and recommends further studies on the impact of environmental parameter variations on the forest encroachment into savanna.展开更多

The overall reward obtained by the CP depends on the user's degree of interest in the content and the user's role in disseminating the content copies. Thus, to maximize the reward, the content provider is motivated to disseminate the authorized content to the most interested users. In this paper, we study how to effectively disseminate the authorized content in Interest-centric Opportunistic Social Networks(IOSNs) such that the reward is maximized. We first derive Social Connection Pattern(SCP) data to handle the challenging opportunistic connections in IOSNs and statistically analyze the interest distribution of the users contacted or connected. The SCP is used to predict the interests of possible contactors and connectors. Then, we propose our SCP-based Dissemination(SCPD)algorithm to calculate the optimum number of content copies to disseminate when two users meet. Our dataset based simulation shows that our SCPD algorithm is effective and efficient to disseminate the authorized content in IOSNs.展开更多

Here,we report a case of AITL induced hemophagocytic lymphohistiocytosis(HLH)and disseminated intravascular coagulopathy(DIC).CASE SUMMARY An 83-year-old man presented with fever and purpura of both lower limbs for one month. Groin lymph node puncture and flow cytometry indicated a diagnosis of AITL. Bone marrow examination and other laboratory related indexes indicated DIC and HLH. The patient rapidly succumbed to gastrointestinal bleeding and septic shock.CONCLUSION This is the first reported case of AITL induced HLH and DIC. AITL is more aggressive in older adults. In addition to male gender, mediastinal lymphadenopathy, anaemia, and sustained high level of neutrophil-to-lymphocyte ratio may indicate a greater risk of death. Early diagnosis, early detection of severe complications, and prompt and effective treatment are vital.展开更多

文摘BACKGROUND Tuberculosis is a chronic infectious disease and an important public health pro-blem.Despite progress in controlling tuberculosis, the incidence of tuberculosis in China is still very high, with 895000 new cases annually. This case report des-cribes the investigation of a case of severe disseminated tuberculosis in a young adult with normal immune function, conducted to ascertain why a Mycobacterium tuberculosis(M.tuberculosis)strain caused such severe disease.CASE SUMMARY A previously healthy 28-year-old woman presented to our hospital with a 1-mo-nth history of fever and fatigue. She was diagnosed with severe disseminated pulmonary tuberculosis, spinal tuberculosis with paravertebral abscesses, and tuberculous meningitis.M.tuberculosis was isolated from bronchoal-veolar lavage fluid. She was treated with standard antituberculous therapy and underwent debridement, bone graft, and internal fixation surgery for spinal tuberculosis. She responded to therapy and regained her ability to walk following the surgery. We analysed the whole-genome sequence of the strain and designated it BLM-A21.Additional M.tuberculosis genomes were selected from the Virulence Factor Database(http://www.mgc.ac.cn/cgi-bin/VFs/genus.cgi?Genus=Mycobacterium)for comparison. An evolutionary tree of the BLM-A21 strain was built using PhyML maximum likelihood software.Further gene analysis revealed that, except for the pks1 gene,BLM-A21 had similar virulence genes to the CDC 1551 and H37Rv strains, which have lower dissemination.CONCLUSION We speculate that the pks1 virulence gene in BLM-A21 may be the key virulence gene responsible for the wide-spread dissemination of M.tuberculosis infection in this previously healthy adult with normal immune function.
文摘With the aim of assessing floristic diversity, stand structure and vegetation ecology, the present work was carried out in the savanna mosaic forest of the locality of Ndjole (Centre Cameroon). Our study provides detailed data on forest-savanna floristic and plant life traits in the mosaic and recommends further studies on the impact of environmental parameter variations on the forest encroachment into savanna.
文摘As one of the Four Great Masterpieces of China,The Romance of the Three Kingdoms has become a landmark of ancient Chinese literature with vivid and profound character images, ups and downs of politics and war descriptions, and appealing historical plots. After being introduced to Japan in the second half of the 17th century,it was spread in Japanese society and was accepted for literary value, social needs and other reasons, then a series of evolutions were produced by combining with local culture, eventually becoming a national culture in Japan. This paper selects the most popular text of The Romance of the Three Kingdoms with 24 chapters and 120 verses in the Wanli period of the Ming Dynasty, and the Japanese translation of The Popular Romance of the Three Kingdoms published by Hunan Wenshan in 1689 as the objects, guided by Ralph Linton’s“Manifestation,Selection,Integration”cultural communication research method,to analyze the dissemination, acceptance and evolution of The Romance of the Three Kingdoms in 17-20 centuries’Japan, which started from the field of literature, then combined with various cultural forms and spread to the civic class, and finally entered politics area and produced a series of evolutions under the influence of Japanese values to become the representative culture of Japan. This paper suggests that in today’s world of frequent cultural exchanges,China should, based on strengthening cultural self-confidence,actively and appropriately select and accept foreign cultures with the attitude of prioritizing our own culture and utilizing foreign culture for our own purposes. The research outcome can provide experience for the current Chinese culture to go to the world with high quality.

文摘Authorized content is a type of content that can be generated only by a certain Content Provider(CP).The content copies delivered to a user may bring rewards to the CP if the content is adopted by the user. Our dataset based simulation shows that our SCPD algorithm is effective and efficient to disseminate the authorized content in IOSNs.
文摘Rationale:Disseminated tuberculosis involves the central nervous system in up to a third of cases. However, meningitis and spondylodiscitis due to miliary tuberculosis rarely occur together, particularly in the immuno-competent population.Patient concerns:A 37-year-old immunocompetent male presented with altered level of consciousness for one week and lower back pain with evening pyrexia for one month.Examination revealed spastic paraplegia and left hemiparesis.Diagnosis:Disseminated tuberculosis presenting with meningitis and spondylodiscitis.Interventions:Category I anti-tuberculous therapy with a tapering regimen of intravenous dexamethasone was administered.Outcomes:There was clinical improvement after nine months of treatment.Lessons:Tuberculosis may present with atypical clinical manifestations.Contrast enhanced computed tomography scan or magnetic resonance imaging combined with histopathological features,a high index of suspicion and clinical improvement with anti-tuberculous treatment can confirm the diagnosis in the absence of microbiological evidence, especially in extrapulmonary tuberculosis.

文摘Vehicular Adhoc Networks(VANETs)enable vehicles to act as mobile nodes that can fetch, share, and disseminate information about vehicle safety, emergency events, warning messages, and passenger infotainment. However, the continuous dissemination of information fromvehicles and their one-hop neighbor nodes,Road Side Units(RSUs),and VANET infrastructures can lead to performance degradation of VANETs in the existing hostcentric IP-based network. Therefore,Information Centric Networks(ICN)are being explored as an alternative architecture for vehicular communication to achieve robust content distribution in highly mobile, dynamic, and errorprone domains. In ICN-based Vehicular-IoT networks, consumer mobility is implicitly supported, but producer mobility may result in redundant data transmission and caching inefficiency at intermediate vehicular nodes. This paper proposes an efficient redundant transmission control algorithm based on network coding to reduce data redundancy and accelerate the efficiency of information dissemination. The proposed protocol, called Network Cording Multiple Solutions Scheduling(NCMSS),is receiver-driven collaborative scheduling between requesters and information sources that uses a global parameter expectation deadline to effectively manage the transmission of encoded data packets and control the selection of information sources.Experimental results for the proposed NCMSS protocol is demonstrated to analyze the performance of ICN-vehicular-IoT networks in terms of caching, data retrieval delay, and end-to-end application throughput. The end-to-end throughput in proposed NCMSS is 22%higher(for 1024 byte data)than existing solutions whereas delay in NCMSS is reduced by 5%in comparison with existing solutions.

文摘The content-centric networking(CCN)architecture allows access to the content through name, instead of the physical location where the content is stored, which makes it a more robust and flexible content-based architecture.Nevertheless,in CCN,the broadcast nature of vehicles on the Internet of Vehicles(IoV)results in latency and network congestion. The IoVbased content distribution is an emerging concept in which all the vehicles are connected via the internet.Due to the high mobility of vehicles, however,IoV applications have different network requirements that differ from those of many other networks, posing new challenges.Considering this,a novel strategy mediator framework is presented in this paper for managing the network resources efficiently.Software-defined network(SDN)controller is deployed for improving the routing flexibility and facilitating in the interinteroperability of heterogeneous devices within the network.Due to the limited memory of edge devices, the delectable bloom filters are used for caching and storage. Finally, the proposed scheme is compared with the existing variants for validating its effectiveness.

文摘BACKGROUND Angioimmunoblastic T-cell lymphoma(AITL) is a subtype of peripheral T-cell lymphoma, with heterogenous clinical manifestations and poor prognosis. Early diagnosis, early detection of severe complications, and prompt and effective treatment are vital.
文摘BACKGROUND Forehead osteoma is a commonly encountered benign facial bone tumor.Endoscopic excision of benign forehead masses is widely performed. Here,we report a rare case of recurrent forehead osteoma that disseminated after a previous osteoma excision.CASE SUMMARY A 54-year-old female patient had previously undergone endoscopic removal of a single forehead osteoma at 30 years of age. However, she had a recurrent osteoma around the same site and underwent another endoscopic resection at 40 years of age.During her first visit to our outpatient clinic, she presented with a cobblestone-like irregular surface on the forehead and a 3D facial bone computed tomography scan revealed a widely ragged surface of the inoculated osteoma on the outer table of the frontal bone.Under general anesthesia,we performed a radical complete excision of the disseminated osteoma through a bicoronal incision using an osteotome, chisel, mallet, and rasping. We hypothesized that the recurrence may have been caused by the inoculation of residual osteoma remnants from the previous procedure.Craniofacial surgeons should be cautious when removing osteoma particles, particularly when using an endoscopic approach.CONCLUSION To prevent recurrence,it is essential to conduct additional meticulous burring and a thorough inspection of the surface after copious irrigation.
文摘BACKGROUND Case reports of symmetric digital gangrene resulting from high-dose vasopressors use in patients with alcoholic ketoacidosis, leading to cardiac arrest, are rare.To date,no specific treatment method for autolysis or surgical amputation or guidelines for determining the level of amputation have been established.CASE SUMMARY In this case report,we describe a treatment method that effectively preserved the function of fingers by surgical treatment under local anesthesia with a minimum operative time, while also preserving finger length to the maximum possible extent.CONCLUSION Our approach may contribute to improved postoperative quality of life by preserving finger length.
