Background: Calcium hydroxide has been used in dentistry since several decades. It has been used in a number of applications in the field of endodontics such as root resorption, intracanal medicament, and root canal sealers. Although this material exhibits several advantages, it also has some limitations. Objectives: To review the role of calcium hydroxide in the field of endodontics, focusing on its mechanism of action, antimicrobial effects, different applications, cytotoxicity or biocompatibility, and its removal from the root canals. Materials and Methods: An electronic search was done using different databases. Out of 2,664 articles, only 33 articles have been selected to be included in this review because they are directly related to the topic and matched the inclusion criteria of this review: “Language: English” and “Year: 2000-2016”. Results: The antimicrobial effect of calcium hydroxide is controversial. Although some studies supported the effectiveness of calcium hydroxide against some bacteria others reported its limitation against fungal infection. Calcium hydroxide can be used effectively as intracanal medicament, root canal sealer, in weeping canals, for perforation management and root resorption. Conclusions: Despite the limitation of antimicrobial activity of calcium hydroxide, it is used effectively in a number of treatment modalities in endodontics. Due to its biological and therapeutical properties, calcium hydroxide is the material of choice for all pulp therapy. However, when using calcium hydroxide as a dressing material in root canal treatment caution should be taken to prevent the overextension of the paste beyond the tooth apex and avoid the harmful side effects. 展开更多

Regenerative endodontics(RE)therapy means physiologically replacing damaged pulp tissue and regaining functional dentin–pulp complex.Current clinical RE procedures recruit endogenous stem cells from the apical papilla,periodontal tissue,bone marrow and peripheral blood,with or without application of scaffolds and growth factors in the root canal space,resulting in cementum-like and bone-like tissue formation.Without the involvement of dental pulp stem cells(DPSCs),it is unlikely that functional pulp regeneration can be achieved,even though acceptable repair can be acquired.DPSCs,due to their specific odontogenic potential,high proliferation,neurovascular property,and easy accessibility,are considered as the most eligible cell source for dentin–pulp regeneration.The regenerative potential of DPSCs has been demonstrated by recent clinical progress.DPSC transplantation following pulpectomy has successfully reconstructed neurovascularized pulp that simulates the physiological structure of natural pulp.The self-renewal,proliferation,and odontogenic differentiation of DPSCs are under the control of a cascade of transcription factors.Over recent decades,epigenetic modulations implicating histone modifications,DNA methylation,and noncoding(nc)RNAs have manifested as a new layer of gene regulation.These modulations exhibit a profound effect on the cellular activities of DPSCs.In this review,we offer an overview about epigenetic regulation of the fate of DPSCs;in particular,on the proliferation,odontogenic differentiation,angiogenesis,and neurogenesis.We emphasize recent discoveries of epigenetic molecules that can alter DPSC status and promote pulp regeneration through manipulation over epigenetic profiles. 展开更多

BACKGROUND Root canal retreatment is common after root canal therapy in clinical situations.Especially,completing the retreatment of variant root canals can be challenging.This is particularly true for the molars located at the end of the dental arch.However,advancements in digital dental diagnosis and treatment techniques can solve these problems.Here,we describe a case of a maxillary second molar with a variant distobuccal root canal treated via a novel“inlay-guided endodontics”technique based on improved computer-generated programs.CASE SUMMARY A 63-year-old man complained of a defect in the maxillary left second molar.The tooth,diagnosed with post-treatment endodontic disease,was initially treated by conventional methods,which were ineffective.Our“inlay-guided endodontics”technique was subsequently adopted,with the establishment of a precise integrated three-dimensional(3D)plate model of cone-beam computed tomography data and a digital impression of the dentition.An optimal root canal approach was generated for the“virtual file”in the 3D model.The plate data were imported into a 3D printer and printed.With the help of the guide plate,the file was accurately placed into the cervical third of the distal root canal.The root canal and prosthodontic treatments successfully proceeded subsequently.CONCLUSION Our newly developed inlay guide plates may facilitate individualized and minimally invasive root canal treatment. 展开更多

The objective of this study was to systematically review the literature about the use of the operating microscope in endodontics. It used sources of bibliographic cataloging electronically identified by MEDLINE via PubMed, LILACS, BBO and Scopus, using as descriptors: Endodontics, anatomy and microscopy. The selected articles were identified from the titles and abstracts, taking into consideration the following criteria: publications ranging from January 2001 to November 2012, case reports, published in an indexed journal, and publications in English language. The search showed 167 articles related to this topic, and these 21 articles met the inclusion criteria. These indicated that a higher magnification helps coronary access and the location of channels to identify and interpret the complexities of the anatomy of root canals, the removal of nuclei and intracoronary fractured instruments, to minimize trauma of surgeries in soft and hard tissues, and detect fractures and microfractures. It was found that its use has provided Endodontics with a significant improvement in vision of the operative field, offering better quality jobs and a higher success rate. 展开更多

Nickel-titanium(NiTi)alloy has been extensively researched in endodontics,particularly in cleaning and shaping the root canal system.Research advances have primarily focused on the design,shape,and geometry of the NiTi files as well as metallurgy and mechanical properties.So far,extensive investigations have been made surrounding surface and thermomechanical treatments,however,limited work has been done in the realm of surface functionalization to augment its performance in endodontics.This review summarizes the unique characteristics,current use,and latest developments in thermomechanically treated NiTi endodontic files.It discusses recent improvements in nano-engineering and the possibility of customizing the NiTi file surface for added functionalization.Whilst clinical translation of this technology has yet to be fully realized,future research direction will lie in the use of nanotechnology. 展开更多

目的比较牙源性鼻窦炎(odontogenic sinusitis,ODS)患者中有鼻窦炎症状和无鼻窦炎症状群体的临床表现。方法回顾性收集2020年6月至2022年6月在首都医科大学附属北京同仁医院耳鼻咽喉头颈外科行内镜鼻窦手术的92例ODS患者的临床资料,77... 目的比较牙源性鼻窦炎(odontogenic sinusitis,ODS)患者中有鼻窦炎症状和无鼻窦炎症状群体的临床表现。方法回顾性收集2020年6月至2022年6月在首都医科大学附属北京同仁医院耳鼻咽喉头颈外科行内镜鼻窦手术的92例ODS患者的临床资料,77例有鼻窦炎症状(sinusitis symptoms,SS组),15例无鼻窦炎症状(non-SS组)。比较两组患者的病史、症状、病因、内镜检查和鼻窦电子计算机断层扫描(computed tomography,CT)等。结果non-SS组患者相比SS组患者的病程更短[(4.9±3.4)个月vs(10.4±11.0)个月,P=0.048],首诊口腔科比例更高(100%vs 24.7%,P=0.035)。两组患者报告牙痛、牙齿松动症状阳性率,差异无统计学意义(29.9%vs 53.3%,P=0.133;18.2%vs 35.3%,P=0.085)。牙周-牙髓联合病变(periodontal-endodontic combined lesions,PECL)、根尖周炎(apical periodontitis,AP)、牙周炎(periodontitis,PE)、医源性占SS组和non-SS组患者病因的比例分别为53.2%vs 20%(P=0.024)、28.6%vs 33.3%(P=0.76)、11.7%vs 26.7%(P=0.215)、6.5%vs 20%(P=0.119)。鼻窦CT显示,与non-SS组患者相比,SS组患者窦口鼻道复合体(ostiomeatal complex,OMC)、筛窦、额窦有更高比例的受累(83.1%vs 26.7%,P=0.000;80.5%vs 20%,P=0.000;41.6%vs 0%,P=0.002)。结论耳鼻喉科与口腔科的多学科合作,可以早期发现无鼻窦炎症状的ODS,及时诊治PECL等牙齿相关性疾病,减少内镜手术概率,提升患者生活质量。 展开更多

目的 Journal of Endodontics,JOE是牙髓病学科中重要的研究载体之一。在当今口腔医学研究中的影响力日渐增加。本文分析了JOE近10年来发表的相关文献的年际分布、作者空间分布、作者文献数量分布、关键词、文献引用、期刊影响因子等方... 目的 Journal of Endodontics,JOE是牙髓病学科中重要的研究载体之一。在当今口腔医学研究中的影响力日渐增加。本文分析了JOE近10年来发表的相关文献的年际分布、作者空间分布、作者文献数量分布、关键词、文献引用、期刊影响因子等方面,并探讨中国作者对该杂志文献构成的贡献,以及国内该领域内研究的组织机构构成等。方法利用ISI Web of Science(ISI WOS)数据库收集文献,将检索到的文献导入书目共现分析系统(BICOMB2.0)、Endnote X6及Excel 2007中进行分析。结果结果表明,近10年来,我国研究者在该杂志上发表的文献,数量逐年增加。作者空间分布表明相关机构具有明显的学科相关性。结论引文分析表明,中国作者发表的文献被引用次数逐年增加,说明中国研究者在牙髓病学领域的专业水平和影响力不断提高。期刊影响因子分析表明JOE是牙髓病学领域内顶尖的期刊之一,同时影响因子的变化已趋于稳定。 展开更多

目的:探讨光活化消毒技术(PAD)治疗牙髓-牙周综合征疗效及对患者龈沟液炎症因子水平的影响。方法:选取牙髓-牙周综合征患者56例,随机分为两组各28例,对照组接受常规一次性根管治疗术与牙周基础治疗,观察组在对照组的基础上,同时应用PAD... 目的:探讨光活化消毒技术(PAD)治疗牙髓-牙周综合征疗效及对患者龈沟液炎症因子水平的影响。方法:选取牙髓-牙周综合征患者56例,随机分为两组各28例,对照组接受常规一次性根管治疗术与牙周基础治疗,观察组在对照组的基础上,同时应用PAD治疗。比较两组治疗前后牙周相关指标、龈下细菌相对丰度、龈沟液炎症指标以及临床有效率。结果:两组治疗前牙周相关指标、龈下细菌相对丰度、龈沟液炎症指标水平比较差异均无统计学意义(均P>0.05);治疗后观察组牙龈指数、牙周袋探诊深度、附着丧失水平、探诊出血指标水平低于对照组(均P<0.05);治疗后观察组卟啉单胞菌、中间普雷沃菌、福赛坦纳菌、齿垢密螺旋体相对丰度低于对照组(均P<0.05);治疗后观察组龈沟液肿瘤坏死因子-α、白细胞介素-18、白细胞介素-6、白细胞介素-1β水平低于对照组(均P<0.05);观察组临床治疗有效率(100.00%)高于对照组(71.43%)(P<0.05)。结论:对于牙髓-牙周综合征患者,在常规治疗基础上实施PAD辅助治疗,可有效改善牙周状况,抑制口腔细菌,改善炎症,提高临床疗效。 展开更多

BACKGROUND The authors describe a technique for building an alternative jawbone phantom using dental gypsum and rice for research and dental radiology instruction.AIM To investigate the potential of an alternative phantom to simulate the trabecular bone aspect of the human maxilla in periapical radiographs.METHODS Half-maxillary phantoms built from gypsum-ground rice were exposed to X-rays,and the resulting images(experimental group)were compared to standardized radiographic images produced from dry human maxillary bone(control group)(n=7).The images were blindly assessed according to strict criteria by three examiners for the usual trabecular aspects of the surrounding bone,and significant differences between groups and in assessment reliability were compared using Fisher’s exact and kappa tests(α=0.05).RESULTS The differences in the trabecular aspects between groups were not statistically significant.In addition,interobserver agreement among observers was 0.43 and 0.51 for the control and experimental groups,respectively,whereas intraobserver agreement was 0.71 and 0.73,respectively.CONCLUSION The tested phantom seemed to demonstrate potential for trabecular bone image Munhoz EA et al.Model of hard tissue simulation for dental radiography WJM https://www.wjgnet.com 2 March 20,2024 Volume 14 Issue 1 simulation on maxillary periapical radiographs. 展开更多

不同冲洗消毒的药物在根管治疗中对牙周牙髓病变的远期效果对比

目的 分析不同冲洗消毒药物对根管治疗牙周牙髓联合病变的远期效果。方法 110例牙周牙髓联合病变患者,按照随机数字表法分为参照组与研究组,每组55例。所有患者均进行根管治疗,参照组辅以次氯酸钠溶液超声冲洗+氢氧化钙消毒,研究组辅以... 目的 分析不同冲洗消毒药物对根管治疗牙周牙髓联合病变的远期效果。方法 110例牙周牙髓联合病变患者,按照随机数字表法分为参照组与研究组,每组55例。所有患者均进行根管治疗,参照组辅以次氯酸钠溶液超声冲洗+氢氧化钙消毒,研究组辅以替硝唑溶液超声冲洗+盐酸米诺环素消毒。比较两组的牙周菌斑指数(PLI)、牙周袋深度(PD)、探诊出血指数(BI)、牙龈指数(GI)、临床附着水平(CAL)、疗效、复发率及牙齿松动、牙周炎、叩痛发生率。结果 研究组的治疗总有效率为96.36%,高于参照组的85.45%,差异具有统计学意义(P<0.05)。治疗后1、3个月,研究组的BI、PD、PLI、GI、CAL分别为(1.96±0.54)、(4.48±0.46)mm、(1.29±0.56)、(1.47±0.33)、(3.40±0.47)mm与(1.62±0.40)、(4.22±0.40)mm、(1.05±0.43)、(0.80±0.25)、(3.18±0.44)mm,均低于参照组的(2.30±0.52)、(4.90±0.53)mm、(1.68±0.49)、(1.69±0.31)、(3.83±0.52)mm与(1.93±0.47)、(4.87±0.47)mm、(1.33±0.40)、(1.23±0.27)、(3.65±0.46)mm,差异具有统计学意义(P<0.05)。治疗后6个月,研究组的PD(4.05±0.33)mm、CAL(3.10±0.38)mm低于参照组的(4.24±0.35)、(3.42±0.37)mm,差异具有统计学意义(P<0.05)。治疗后6个月,两组的BI、GI和PLI没有显著差异(P>0.05)。治疗后6个月,研究组的复发率3.64%、牙齿松动发生率1.82%、牙周炎发生率5.45%低于参照组的14.55%、12.73%、18.18%,差异具有统计学意义(P<0.05);两组的叩痛发生率没有显著差异(P>0.05)。结论 替硝唑溶液+米诺环素是合适的冲洗消毒药物,该方案具有良好的远期效果,能够改善牙周牙髓联合病变患者的牙周情况,预防疾病的复发。 展开更多

基于炎症微环境调控组织工程再生性牙髓治疗

背景:近年来,再生性牙髓治疗概念随着组织工程的发展逐渐形成,炎症微环境在调控牙髓再生方面起着关键作用。目的:文章从炎症牙髓微环境变化出发,着眼于炎症与再生平衡,重点介绍炎症微环境中组织工程再生性牙髓治疗的研究进展,为未来再... 背景:近年来,再生性牙髓治疗概念随着组织工程的发展逐渐形成,炎症微环境在调控牙髓再生方面起着关键作用。目的:文章从炎症牙髓微环境变化出发,着眼于炎症与再生平衡,重点介绍炎症微环境中组织工程再生性牙髓治疗的研究进展,为未来再生性牙髓治疗的研究提供参考。方法:在PubMed和中国知网数据库中进行检索,中文检索词为“牙髓再生,炎症,再生性牙髓治疗,组织工程”;英文检索词为“pulp regeneration,inflammation,regenerative endodontic therapy,tissue engineering”,分别检索2013-2023年发表的相关文献,最终纳入61篇文献进行综述分析。结果与结论:①牙髓炎症微环境的变化涉及一系列细胞与分子反应,随炎症进展,微环境对组织修复的作用弊大于利。②炎症通过干细胞募集及激活补体系统等方式促进牙髓再生,也能通过免疫抑制、纤维化破坏再生过程。③组织工程三要素(干细胞、生长因子及支架材料)协同介导炎症反应以恢复炎症-再生平衡,如利用白细胞介素6调节牙髓干细胞功能介导炎症微环境以促进牙髓再生。④当前研究缺乏对感染、炎症问题的关注,牙髓炎症微环境的变化机制尚不清晰,通过有机结合组织工程再生性牙髓治疗的三要素调控炎症微环境,构建再生微环境,精准地调控炎症-再生平衡或是提高临床适用性的突破口之一,但该部分研究仍存在较大空缺,期望未来的研究就如何实现炎症环境下的牙髓再生展开更多的探索,也还需更多动物实验、随机临床试验为组织工程再生性牙髓治疗的临床实践奠定基础。 展开更多

目的探讨盐酸米诺环素辅助显微根管联合牙周组织再生术治疗牙周牙髓联合病变伴畸形根面沟的效果。方法选取2020年4月—2022年1月常州市中医医院收治的102例(共102颗患牙)牙周牙髓联合病变伴畸形根面沟患者,以随机数字表法分为对照组和... 目的探讨盐酸米诺环素辅助显微根管联合牙周组织再生术治疗牙周牙髓联合病变伴畸形根面沟的效果。方法选取2020年4月—2022年1月常州市中医医院收治的102例(共102颗患牙)牙周牙髓联合病变伴畸形根面沟患者,以随机数字表法分为对照组和研究组,每组51例。对照组接受显微根管联合牙周组织再生术治疗,研究组在对照组基础上给予盐酸米诺环素治疗。对比两组手术成功率、牙周相关指标、炎症因子、咀嚼功能评分、疼痛情况及并发症。结果研究组术后1年手术成功率(90.20%)高于对照组(72.55%)(P<0.05)。研究组治疗前后龈沟出血指数、探诊深度、牙石指数、菌斑指数、C反应蛋白、肿瘤坏死因子、白细胞介素-17、咀嚼功能评分、视觉模拟评分的差值均高于对照组(P<0.05)。两组并发症总发生率比较,差异无统计学意义(P>0.05)。结论盐酸米诺环素辅助显微根管联合牙周组织再生术治疗牙周牙髓联合病变伴畸形根面沟患者疗效确切,可改善牙周组织状态,减轻炎症反应、疼痛。 展开更多
