Comparison of Ziemer FEMTO LDV "Classic" and " Crystal Line" femtosecond laser flap quality by Fourierdomain optical coherence tomography

AIM:To compare the regularity and accuracy of laser in situ keratomileusis(LASIK) flaps created by the Ziemer FEMTO LDV 'Classic'(Ziemer 'Classic') and Ziemer FEMTO LDV Crystal Line femtosecond laser(Ziemer Crystal Line). METHODS:Fourier-domain optical coherence tomography(RTVue OCT) was used to measure the morphology of 200 LASIK flaps of 100 consecutive patients created with the Ziemer Classic(100 flaps) or the Ziemer Crystal Line(100 flaps) at one week postoperatively.Flap thickness was evaluated at 36 specified measurement points on each flap.For all procedures with both lasers,the nominal flap thickness was 110μm.RESULTS:The mean flap thickness of the Ziemer Crystal Line group(102.49±2.68μm) was thinner than that of the Ziemer Classic group(107.65±5.09μm)(P【0.01).Average thickness of all flaps was uniform within 4μm at all measurement points.The flaps in the Ziemer Crystal Line group were more regular than those in the Ziemer Classic group when measured from the center to the periphery.The maximum deviation from the nominal 110μm of 36 measurements was 8μm in the Ziemer Classic group,while in the Ziemer Crystal Line group it was 9μm.Within the 3 600 measurements on the 100 eyes,differences greater than 20μm were observed 0.14% in the Ziemer Classic group,and 0.04% in the Ziemer Crystal Line group. CONCLUSION:The flaps created with the Ziemer FEMTO LDV Crystal Line femtosecond laser are more uniform and thinner than those created by the Ziemer FEMTO LDV Classic femtosecond laser. 展开更多

AIM: To assess and compare the flap morphology using anterior segment optical coherence tomography (AS-OCT) in laser in situ keratomileusis (LASIK) with Femto LDV femtosecond lasers versus Hansatome mechanical Microkeratome. METHODS: AS-OCT (Visante) was used to compare 1 month postoperatively the morphology of the flaps created with Femto LDV femtosecond lasers or Hansatome Microkeratome. The intendedfiap thickness was 110 mu m and 160 mu m respectively. The thickness of twenty-five points across each flap, which were 0mm, 1.5mm, 2.5mm, and 3.5mm to the corneal vertex on the horizontal, vertical, 45 degrees and 135 degrees meridian respectively, was evaluated. RESULTS: One month postoperative, the central flap thickness in the Femto LDV group was 107.43 +/- 4.70 mu m, while 125.90 +/- 17.50 mu m in the Hansatome group. The difference between the actual and the expectedfiap thickness was 5.61 +/- 3.84 mu m and 31.52 +/- 12.27 mu m, respectively. The Hansatome group had presented a statistically significant result (P<0.001). Flap morphology showed a more regular planar shape in the Femto LDV group and a meniscus shape in the Hansatome group. CONCLUSION: AS-OCT is a direct and fast procedure to assess the flap morphology. The morphology by AS-OCT showed that the flaps created with Femto LDV femtosecond laser were more accurate and regular than the flaps created with Hansatome microkeratome. 展开更多

· AIM: To compare refractive results, higher-order aberrations(HOAs), contrast sensitivity and dry eye after laser in situ keratomileusis(LASIK) performed with a femtosecond laser versus a mechanical microkeratom... · AIM: To compare refractive results, higher-order aberrations(HOAs), contrast sensitivity and dry eye after laser in situ keratomileusis(LASIK) performed with a femtosecond laser versus a mechanical microkeratome for myopia and astigmatism.·METHODS: In this prospective, non-randomized study,120 eyes with myopia received a LASIK surgery with the Visu Max femtosecond laser for flap cutting, and 120 eyes received a conventional LASIK surgery with a mechanical microkeratome. Flap thickness, visual acuity, manifest refraction, contrast sensitivity function(CSF) curves,HOAs and dry-eye were measured at 1wk; 1, 3, 6mo after surgery.·RESULTS: At 6mo postoperatively, the mean central flap thickness in femtosecond laser procedure was113.05 ±5.89 μm(attempted thickness 110 μm), and148.36 ±21.24 μm(attempted thickness 140 μm) in mechanical microkeratome procedure. An uncorrected distance visual acuity(UDVA) of 4.9 or better was obtained in more than 98% of eyes treated by both methods, a gain in log MAR lines of corrected distance visual acuity(CDVA) occurred in more than 70% of eyes treated by both methods, and no eye lost ≥1 lines of CDVA in both groups. The difference of the mean UDVA and CDVA between two groups at any time post-surgery were not statistically significant(P 】0.05). The postoperative changes of spherical equivalent occurred markedly during the first month in both groups. The total root mean square values of HOAs and spherical aberrations in the femtosecond treated eyes were markedly less than those in the microkeratome treated eyes during 6mo visit after surgery(P 【0.01). The CSF values of the femtosecond treated eyes were also higherthan those of the microkeratome treated eyes at all space frequency(P 【0.01). The mean ocular surface disease index scores in both groups were increased at 1wk, and recovered to preoperative level at 1mo after surgery. The mean tear breakup time(TBUT) of the femtosecond treated eyes were markedly longer than those of the microkeratome treated eyes at postoperative 1, 3mo(P 【0.01).·CONCLUSION: Both the femtosecond laser and the mechanical microkeratome for LASIK flap cutting are safe and effective to correct myopia, with no statistically significant difference in the UDVA, CDVA during 6mo follow-up. Refractive results remained stable after 1mo post-operation for both groups. The femtosecond laser may have advantages over the microkeratome in the flap thickness predictability, fewer induced HOAs, better CSF,and longer TBUT. 展开更多

AIM:To assess and compare the morphology of corneal flaps created by the Wavelight FS200 and Intralase FS60 femtosecond lasers in laser in situ keratomileusis(LASIK).METHODS:Four hundred eyes of 200 patients were enrolled in this study and divided into Wavelight FS200 groups(200 eyes) and Intralase FS60 groups(200 eyes).Fourier-domain optical coherence tomography(RTVue OCT) was used to measure the corneal flap thickness of 36 specified measurements on each flap one week after surgery.Results were used to analyze the regularity,uniformity and accuracy of the two types of LASIK flaps.RESULTS:The mean thickness of corneal flap and central flap was 105.71±4.72 μm and 105.39±4.50 μm in Wavelight FS200 group and 109.78±11.42 μm and 109.15±11.59 μm in Intralase FS60 group,respectively.The flaps made with the Wavelight FS200 femtosecond laser were thinner than those created by the Intralase FS60 femtosecond laser(P=0.000).Corneal flaps in the 2 groups were uniform and regular,showing an almost planar configuration.But the Wavelight FS200 group has more predictability and uniformity of flap creation.The mean deviation between achieved and attempted flap thickness was smaller in the Wavelight FS200 group than that in the Intralase FS60 group,which were 5.18±3.71 μm and 8.68±7.42 μm respectively.The deviation of more than 20 μm was 0.2% measurements in Wavelight FS200 group and 8.29% measurements in Intralase FS60 group.CONCLUSION:The morphologies of flaps created by Wavelight FS200 are more uniform and thinner than those created by Intralase FS60. 展开更多

AIM:To study the effects of different flap sizes on visual acuity,refractive outcomes,and aberrations after femtosecond laser for laser in situ keratomileusis(LASIK).METHODS:In each of the forty patients enrolled,1 eye was randomly assigned to receive treatment with a 8.1mm diameter corneal flap,defined as the small flap,while the other eye was treated with a 8.6mm diameter corneal flap,defined as the big flap.Refractive errors,visual acuity,and higher-order aberrations were compared between the two groups at week 1,month 1 and 3 postoperatively. RESULTS:The postoperative refractive errors and visual acuity all conformed to the intended goal.Postoperative higher-order aberrations were increased,especially in spherical aberration(Z12) and vertical coma(Z7).There were no statistically significant differences between the two groups in terms of postoperative refractive errors,visual acuity,root mean square of total HOAs(HO-RMS),trefoil 30°(Z6),vertical coma(Z7),horizontal coma(Z8),trefoil 0°(Z9),and spherical aberration(Z12) at any point during the postoperative follow-up.CONCLUSION:Both the small and big flaps are safe and effective procedures to correct myopia,provided the exposure stroma meets the excimer laser ablations.The personalized size corneal flap is feasible,as we can design the size of corneal flap based on the principle that the corneal flap diameter should be equal to or greater than the sum of the maximum ablation diameter and apparatus error. 展开更多

目的比较飞秒激光小切口角膜基质透镜取出术(SMILE)和超薄瓣飞秒激光辅助准分子激光原位角膜磨镶术(FS‐LASIK)矫正中度近视的临床疗效。方法回顾性病例对照研究。本研究纳入了年龄、性别和角膜厚度匹配合适的中度近视(等效球镜度-3.00~... 目的比较飞秒激光小切口角膜基质透镜取出术(SMILE)和超薄瓣飞秒激光辅助准分子激光原位角膜磨镶术(FS‐LASIK)矫正中度近视的临床疗效。方法回顾性病例对照研究。本研究纳入了年龄、性别和角膜厚度匹配合适的中度近视(等效球镜度-3.00~-6.00 D)患者,36例接受SMILE治疗患者的72只眼和36例接受超薄瓣FS‐LASIK治疗患者的72只眼。分别于术前及术后3个月记录所有患者的视力、屈光度及角膜高阶像差等变量,最后进行统计学分析以评估手术疗效。结果在术后随访3个月时,SMILE组和超薄瓣FS‐LASIK组疗效指数和安全指数比较,差异无统计学意义(t=1.179和-0.255,P=0.242和0.800)。SMILE组和超薄瓣FS‐LASIK组术后屈光度呈正视并相对稳定,术后等效球镜度比较差异无统计学意义(t=0.206,P=0.837)。SMILE组和超薄瓣FS‐LASIK组患者的角膜后表面曲率和非球面因子Q值比较,差异无统计学意义(t=-1.852和0.576,P=0.066和0.566)。SMILE组和超薄瓣FS‐LASIK组患者术前的总高阶像差、球差及彗差与术后比较,差异均具有统计学意义(均P<0.05)。在术后3个月时,SMILE组患者的总高阶像差及球差明显低于超薄瓣FS‐LASIK组(均P<0.05)。结论SMILE和超薄瓣FS‐LASIK两种术式矫治中度近视均安全有效,但SMILE组患者的术后总高阶像差及球差增加量低于超薄瓣FS‐LASIK组。 展开更多

目的分析FEMTO LDV飞秒激光辅助制作角膜瓣的准分子激光原位角膜磨镶术(FS-LASIK)角膜瓣相关并发症的发生原因,并探讨其处理方法。方法回顾性病例研究。选取2012年7月至2022年6月于医院接受FS-LASIK的10600例(20480眼)近视散光患者,术... 目的分析FEMTO LDV飞秒激光辅助制作角膜瓣的准分子激光原位角膜磨镶术(FS-LASIK)角膜瓣相关并发症的发生原因,并探讨其处理方法。方法回顾性病例研究。选取2012年7月至2022年6月于医院接受FS-LASIK的10600例(20480眼)近视散光患者,术中均使用FEMTO LDV飞秒激光辅助制作角膜瓣,观察角膜瓣的制作情况。结果本研究共出现角膜瓣相关并发症5566眼(27.18%),其中术中并发症5516眼,占99.1%,术后并发症50眼,占0.9%。术中并发症:直径超小瓣2眼(0.10‰),游离瓣9眼(0.44‰),不全瓣、碎瓣、纽扣瓣15眼(0.73‰),角膜瓣掀开困难94眼(4.59‰),角膜基质床线状条纹96眼(4.69‰),角膜基质床颗粒状107眼(5.22‰),角膜基质微小气泡15眼(0.73‰),前房气泡13眼(0.63‰),角膜上皮脱落12眼(0.59‰),术中负压丢失2眼(0.10‰),角膜瓣蒂位置反向3眼(0.15‰),眼底出血1眼(0.05‰),角膜瓣边缘出血2078眼(10.15%),球结膜下出血1043眼(5.09%),角膜瓣偏中心2039眼(9.96%)。术后并发症:角膜瓣皱褶50眼(2.44‰)。结论FEMTO LDV飞秒激光辅助制作角膜瓣虽有并发症发生,但均不影响手术效果,全面了解设备的性能,合理设计切削参数,提高医生的操作技巧才能制作完美的角膜瓣。 展开更多

应用眼球自旋控制功能的FS-LASIK与SMILE术后角膜高阶像差的比较 被引量:3

目的比较应用眼球自旋控制功能的飞秒激光辅助准分子激光原位角膜磨镶术(femtosecond laser-assisted laser in situ keratomileusisis,FS-LASIK)与飞秒激光小切口角膜基质透镜取出术(small incision lenticule extraction,SMILE)术后... 目的比较应用眼球自旋控制功能的飞秒激光辅助准分子激光原位角膜磨镶术(femtosecond laser-assisted laser in situ keratomileusisis,FS-LASIK)与飞秒激光小切口角膜基质透镜取出术(small incision lenticule extraction,SMILE)术后角膜前表面高阶像差的变化。方法采用前瞻性非随机对照研究方法,选取2021年4~12月于徐州医科大学附属徐州市立医院行角膜屈光手术的近视患者70例(70眼),所有患者均只纳入右眼,根据患者选择的术式分为FS-LASIK组与SMILE组,每组各35例。分别于术前及术后1、3个月应用Pentacam AXL系统测量角膜前表面6mm区域的高阶像差。结果术后1天裸眼视力(uncorrected visual acuity,UCVA)≥5.0的占比FS-LASIK组为94.30%(33眼),SMILE组为60.00%(21眼),差异有统计学意义(χ^(2)=11.67,P<0.01);两组术后1、3个月UCVA≥5.0的占比比较,差异无统计学意义(P>0.05)。FS-LASIK组术后1天及术后1、3个月等效球镜度数均高于SMILE组,差异均有统计学意义(P均<0.05)。两组术后1、3个月,除SMILE组水平彗差与术前无明显变化,余均较同组术前明显提高,差异有统计学意义(P均<0.05);术后1、3个月,两组垂直彗差及球差组间比较,差异均有统计学意义(P值<0.01)。结论FS-LASIK和SMILE术后垂直彗差、球差及总高阶像差均较术前增加,但应用眼球自旋控制功能的FS-LASIK垂直彗差的绝对值低于SMILE,而球差高于SMILE。 展开更多

Background Thin-flap laser in situ keratomileusis (LASIK) is the new trend of refractive error correction surgery, the formation of corneal flap is crucial for a success of LASIK surgery. This study aimed to assess and compare the variations of LASIK flap created by the IntraLase femtosecond laser, Moria One Use-Plus SBK and Moria M2 Single-Use 90 pm- head microkeratome using Anterior segment optical coherence tomography (Visante OCT). Methods One hundred and sixty-one eyes of 81 consecutive patients were enrolled in this prospective study and randomly divided into three groups depending on the flap creation method: flap creation with the the IntraLase femtosecond laser (IntraLase group, 59 eyes), flap creation with the Moria One Use-Plus SBK (SBK group, 44 eyes), and flap creation with the Moria M2 Single-Use 90 pm-head microkeratome (M2SU90 group, 58 eyes). The nominal flap thickness was 110 m for all patients and for the three devices. One month after surgery, Visante OCT was used to measure the flap thickness at 20 locations on each cornea and the results were assessed for uniformity, regularity, and accuracy. Results At 1 month after surgery, the mean central flap thickness was (111±3) pm in the IntraLase group, (114±8) pm in the SBK group, and (118±13) pm in the M2SU90 group respectively. The flaps in the IntraLase group and the SBK group were more regular, showing an almost planar configuration, than the meniscus-shaped flaps in the M2SU90 group. The maximum deviation from the intended flap thickness (110 pm) was 6 pm in the IntraLase group, 10 pm in the SBK group, and 20 pm in the M2SU90 group respectively. A difference greater than 20 pm was observed in 0.42% of measurements in the IntraLase group; 2.95% of the measurements in the SBK group and 21.12% of measurements in the M2SU90 group. Conclusions The flaps created by the IntraLase femtosecond laser and Moria One Use-Plus SBK are more uniform; more regular, and more accurate than those created by the Moria M2 Single-Use 90 pm-head microkeratome. The first two methods can make precise flaps for Sub-Bowman Keratomileusis. 展开更多

背景飞秒激光制作准分子激光角膜原位磨镶术(LASIK)角膜瓣技术日渐成熟,术后角膜创伤愈合及神经纤维的再生成为关注热点。目的对比分析FEMTOLDV飞秒激光及Hansatome机械角膜刀制瓣对LASIK后角膜细胞的影响。方法采用前瞻性病例对照... 背景飞秒激光制作准分子激光角膜原位磨镶术(LASIK)角膜瓣技术日渐成熟,术后角膜创伤愈合及神经纤维的再生成为关注热点。目的对比分析FEMTOLDV飞秒激光及Hansatome机械角膜刀制瓣对LASIK后角膜细胞的影响。方法采用前瞻性病例对照研究设计。纳人2010年3—7月在武警河南总队医院眼科连续收治的行LASIK的近视患者38例38眼,分为FEMTOLDV组20例20眼(采用FEMTOLDV飞秒激光制瓣)及Hansatome组18例18眼(采用Hansatome微型角膜刀制瓣),两组患者性别、年龄、术前等效球镜度等资料相匹配。分别于术前及术后1周、1个月、3个月应用共焦显微镜检测角膜中央及角膜瓣边缘的形态学变化,并比较两种制瓣方式术后角膜细胞形态学的异同。观察两组患者的术后视力、屈光度变化和裂隙灯下表现。结果本研究中两组术眼术后1个月最佳矫正视力均≥1.0。FEMTOLDV组平均等效球镜度为(+0.21±0.48)D,Hansatome组为(-0.04±0.54)D,两组屈光度结果接近。FEMTOLDV组术后1周角膜上皮有增厚表现,但与术前比较差异无统计学意义(t=1.63,P〉0.05)。FEMTOLDV组术后1周、1个月和3个月浅基质细胞密度较术前均明显减少,差异均有统计学意义(t=-27.99、-25.49、-28.87,P〈0.01)。Hansatome组术后1周角膜上皮厚度为(56.73±2.47)μm,较术前值(51.16±1.11)μm明显增加,差异有统计学意义(t=9.29,P〈0.05),3个月时接近术前厚度;Hansatome组术后1周、1个月和3个月浅基质细胞密度较术前明显减少,差异均有统计学意义(t=-17.57、-14.13、-19.63,P=0.00)。所有眼均可在切削面见到高反光颗粒,FEMTOLDV组术后1周、1个月和3个月切削面高反光颗粒密度均明显少于Hansatome组,差异均有统计学意义(t=-13.505、-11.900、-14.084,P〈0.01)。术后1周时两组均观察到角膜细胞形态改变,切削界面后见激活的基质细胞,3个月时激活细胞减少。FEMTOLDV组术后3个月可见完整的神经纤维,而Hansatome组神经纤维短小。术后3个月,FEMTOLDV组周边角膜基质高反光,并有不规则纤维化,而Hansatome组角膜瓣边缘基质反光较弱。结论与Hansatome机械角膜刀制瓣LASIK比较,FEMTOLDV飞秒激光制瓣LASIK术后神经纤维再生较早,角膜瓣边缘瘢痕化改变较明显,显示出较强的愈合过程。 展开更多

目的:探究飞秒激光与机械板层刀制瓣准分子激光原位角膜磨镶(LASIK)术对角膜瓣厚度稳定性和干眼症的影响。方法:回顾性分析于本院眼科行LASIK术的近视患者80例160眼,根据制瓣方式不同分为飞秒激光组和机械板层刀组。测量术后两组患者不... 目的:探究飞秒激光与机械板层刀制瓣准分子激光原位角膜磨镶(LASIK)术对角膜瓣厚度稳定性和干眼症的影响。方法:回顾性分析于本院眼科行LASIK术的近视患者80例160眼,根据制瓣方式不同分为飞秒激光组和机械板层刀组。测量术后两组患者不同时间点(1h,1d,1wk,1、3mo)的角膜瓣厚度,并比较不同方向上距离角膜顶点1、2、3mm及角膜顶点角膜瓣厚度的差异。比较患者术后1、3mo,1、2aOSDI评分和干眼症的发生率。结果:飞秒激光组角膜瓣顶点厚度为99.62±4.50μm,机械板层刀组角膜瓣顶点厚度为125.25±12.81μm。飞秒激光制瓣不同离心点位置的角膜瓣厚度较为均匀,而机械板层刀制瓣角膜厚度变化较大。术后1、3mo,1、2a机械板层刀组OSDI评分高于飞秒激光组(P<0.01)。随访2a时间,飞秒激光组干眼发病率低于机械板层刀组(χ2=4.692,P=0.030)。术后1、2a,飞秒激光组轻度干眼所占比例高于机械板层刀组(P=0.044、0.001)。结论:相对于机械板层刀制瓣,飞秒激光制瓣LASIK术患者的角膜瓣厚度稳定性更好,更为均匀,误差更小,且患者术后干眼的发生率更低,以轻度干眼为主。 展开更多

背景采用准分子激光角膜屈光手术矫正屈光不正可减少患者对框架眼镜的依赖,其术后抗炎药物使用过程中不良反应的防治一直是临床关注的问题。氯替泼诺混悬滴眼液是糖皮质激素类抗炎药,已广泛用于准分子激光角膜屈光术后,以缓解和减轻... 背景采用准分子激光角膜屈光手术矫正屈光不正可减少患者对框架眼镜的依赖,其术后抗炎药物使用过程中不良反应的防治一直是临床关注的问题。氯替泼诺混悬滴眼液是糖皮质激素类抗炎药,已广泛用于准分子激光角膜屈光术后,以缓解和减轻术眼术后的炎症反应,但其长期应用后对眼压影响的报道较少。目的探讨准分子激光角膜屈光手术后使用质量分数0.5%氯替泼诺混悬滴眼液对眼压的影响。方法采用回顾性病例分析研究设计,收集2011年1月至2012年1月在唐山市眼科医院行准分子激光角膜屈光手术并有完整随访资料的近视患者1552例1552眼,其中行准分子激光角膜原位磨镶术(LASIK)者321眼,行准分子激光前弹力层下角膜原位磨镶术(SBK)者608眼,行飞秒激光制瓣准分子激光角膜原位磨镶术(FS—LASIK)者623眼。所有患者术后均应用0.5%氯替泼诺混悬滴眼液点眼,每日4次,每周递减1次,共1个月。术前及术后1、2、3、4周分别用非接触眼压计测量眼压,并根据眼压升高的幅度分为〈5mmHg(1mmHg=0.133kPa)、5~15mmHg和〉15mmHg三种糖皮质激素反应。术后发现眼压升高及眼压恢复正常1个月时采用海德堡视网膜断层扫描仪进行视盘参数的检测和观察,并进行比较。结果纳入的1552眼中,术后1个月内眼压升高者47眼,其中行LASIK者用药后眼压升高者占3.12%(10/321),行SBK者占3.29%(20/608),行FS—LAStK者占2.73%(17/623),三者间比较差异无统计学意义(x2=1.95,P〉0.05)。停用0.5%氯替泼诺混悬滴眼液或加用降眼压药物治疗后,术眼眼压降至正常水平,仅1眼出现轻度反弹,视力无下降。术后眼压升高与眼压恢复后1个月时,受检眼的盘沿面积分别为(1.65±0.44)mm2和(1.66±0.44)mm2,盘沿容积分别为(0.40±0.09)mm2和(0.39±0.08)mm2,平均神经纤维层厚度分别为(0.28±0.08)mm和(0.29±0.07)mm,差异均无统计学意义(t=0.34、0.81、0.48,P〉0.05)。结论准分子激光角膜屈光手术后使用0.5%氯替泼诺混悬滴眼液进行抗炎治疗导致的眼压升高为可逆性,用药期间应定期监测眼压变化。 展开更多

目的比较飞秒激光制瓣的准分子激光原位角膜磨镶术(FS-LASIK)与全飞秒激光小切口角膜基质内透镜取出术(SMILE)对屈光不正患者眼表的影响。方法选取2018年1月至2019年6月于郑州大学第一附属医院行角膜屈光手术的63例(126眼)屈光不正患者... 目的比较飞秒激光制瓣的准分子激光原位角膜磨镶术(FS-LASIK)与全飞秒激光小切口角膜基质内透镜取出术(SMILE)对屈光不正患者眼表的影响。方法选取2018年1月至2019年6月于郑州大学第一附属医院行角膜屈光手术的63例(126眼)屈光不正患者进行观察。根据患者接受的手术方式不同分为两组,FS-LASIK组31例(62眼),SMILE组32例(64眼),术后随访6个月,观察并收集数据。观察指标包括眼表疾病指数(OSDI)量表评分、角膜荧光素染色评分、泪河高度(TMH)、非侵入式泪膜破裂时间(NIKBUT)、脂质层厚度、睑板腺功能评估得分、眼红指数(HI)。对两组患者各项指标进行统计学分析。结果FS-LASIK组患者术后1周、1个月、6个月OSDI评分较术前均增加,差异均有统计学意义(均为P<0.05);在术后1周时OSDI评分增加最明显。SMILE组患者术后1周、1个月OSDI评分较术前增加,差异均有统计学意义(均为P<0.05)。两组患者术后1周、1个月、6个月OSDI评分相比,差异均有统计学意义(均为P<0.05),FS-LASIK组均高于SMILE组。两组患者术后1周角膜荧光素染色评分均较术前提高,差异均有统计学意义(均为P<0.05);术后1个月、6个月与术前相比,差异均无统计学意义(均为P>0.05)。FS-LASIK组患者术后1周、1个月、6个月TMH与术前相比均减小,差异均有统计学意义(均为P<0.05),且在术后1周减小最为显著。SMILE组患者术后1周、1个月TMH亦均较术前减小(均为P<0.05)。FS-LASIK组患者术后1周、1个月、6个月NIKBUT与术前相比均减小,差异均有统计学意义(均为P<0.05),且在术后1周减小最为显著。SMILE组患者术后1周、1个月、6个月NIKBUT与术前相比均减小,差异均有统计学意义(均为P<0.05)。两组患者术后1周、1个月、6个月NIKBUT相比,差异均有统计学意义(均为P<0.05),FS-LASIK组均小于SMILE组。FS-LASIK组患者术后1周、1个月、6个月脂质层厚度均较术前减小,差异均有统计学意义(均为P<0.05)。SMILE组患者术后1周、1个月脂质层厚度亦均较术前减小,差异均有统计学意义(均为P<0.05)。两组患者术后1周、1个月、6个月睑板腺功能评估得分与术前相比,差异均无统计学意义(均为P>0.05)。FS-LASIK组和SMILE组患者术后1周HI与术前相比均升高,差异均有统计学意义(均为P<0.05)。两组患者术后1周HI相比,差异有统计学意义(P<0.05),FS-LASIK组高于SMILE组。结论FS-LASIK与SMILE均会对患者眼表产生不同程度的影响,但FS-LASIK对术后眼表的影响较SMILE明显,且这种影响作用的时间更长。 展开更多

目的:探讨VisuMax飞秒激光仪制作角膜瓣的预测性及其影响因素。方法:回顾性病例系列研究。将150例300眼近视眼按预设角膜瓣厚度分为100μm组(204眼)和110μm组(96眼),角膜瓣直径7.9和8.3mm。将术前设定的角膜瓣直径和厚度与术中、术后... 目的:探讨VisuMax飞秒激光仪制作角膜瓣的预测性及其影响因素。方法:回顾性病例系列研究。将150例300眼近视眼按预设角膜瓣厚度分为100μm组(204眼)和110μm组(96眼),角膜瓣直径7.9和8.3mm。将术前设定的角膜瓣直径和厚度与术中、术后测量的实际结果进行差异性分析,对相关影响因素进行多元逐步回归分析。结果:100μm组和110μm组的角膜瓣厚度测量值分别为103.11±4.07,113.35±5.71μm,与目标值比较,差异无统计学意义(t=0.396,1.284,P>0.05)。左右眼差异无统计学意义(t100μm=-0.901,t110μm=-0.490;P>0.05)。100μm组和110μm组患者的角膜厚度测量值分别为533.52±20.89,530.24±25.43μm,年龄分别为23.80±6.71,21.56±5.08岁,角膜中央最大K值分别为43.46±1.39,43.44±1.38D,角膜瓣直径分别为8.46±0.47,8.72±0.43mm,眼压分别为16.02±2.51,16.66±2.21mmHg(1mmHg=0.133kPa)。两组角膜瓣厚度与角膜瓣直径(r=0.003,0.018)、术前年龄(r=0.022,0.050)、角膜厚度(r=0.051,0.101)、角膜中央最大K值(r=-0.048,-0.136)及眼压(r=-0.113,0.047)均无相关性(P>0.05)。预设角膜瓣直径为7.9,8.3mm的角膜厚度分别为536.88±27.99,529.83±33.52μm,角膜中央最大K值分别为43.48±1.19,41.88±1.25D。相关性分析显示:术前角膜中央最大K值与角膜瓣直径呈正相关(r=0.359,0.532;P=0.01,0.007<0.05)。结论:VisuMax飞秒激光仪制作角膜瓣厚度预测性好,不受角膜瓣直径、角膜厚度、角膜中央最大K值、年龄、眼压等因素的影响。 展开更多
