Objective:This article summarizes the experience in the prevention and control of coronavirus disease 2019(COVID-19)epidemic in non-isolated areas in a general hospital.Methods:Based on refined management theory,we pr...Objective:This article summarizes the experience in the prevention and control of coronavirus disease 2019(COVID-19)epidemic in non-isolated areas in a general hospital.Methods:Based on refined management theory,we professionally developed the standards for prevention and control of COVID-19 in non-isolated areas,systematically implemented various prevention and control measures,performed gridding audits,effectively communicated among teams and between medical staff and patients assisted by information techniques,and reported results for quality improvement.Results:There was no hospital-acquired COVID-19 infections among staff in the hospital.The rates of mask-wearing,epidemiological history screening,and the medical supplies disinfection were all 100%in the hospital.The accuracy rate of mask-wearing of patients and their families was 73.79%and the compliance rate of their hand hygiene was 40.78%.Conclusion:Refined management strategies for the prevention and control of COVID-19 infection in non-isolated areas of the general hospital are effective.The accuracy rate of mask-wearing and hand hygiene compliance of patients and their families need to be further improved.展开更多

Objective:Myanmar has been trying to improve the disease management of malaria through public health services. The involvement of private general practitioners(GPs) is recognized as one of the key contributory issues ...Objective:Myanmar has been trying to improve the disease management of malaria through public health services. The involvement of private general practitioners(GPs) is recognized as one of the key contributory issues in malaria control,but no one has yet made an attempt to study the efficiency of their role.This study aimed to assess the participation of GPs in improvement of disease management.Methods:The study was conducted with all 32 GPs practicing at three randomly selected townships with high malaria load situated in Upper Myanmar from June 2006 to March 2007 using a pretest-posttest design to assess their knowledge of the disease management prior to and after intervention.The intervention package consisted of a one-day workshop on diagnosis and treatment of malaria and the supply of facilities for microscopy.Questionnaires filled in before and after tests were compared to assess the change of knowledge after the intervention.Diagnosis and treatment practice during the study period was analysed by review of registers kept by GPs,together with a follow-up survey of their patients for the reliability of data.Results:An overall improvement of knowledge was observed and significant changes were apparent for three variables:the criteria for referral of severe malaria,the effect of incomplete treatment and recommended treatment of Plasmodium vivax.Pre-test results showed that only 65.6%of GPs perceived microscopy or Rapid Diagnostic Test kits(RDTs) for confirmation of malaria necessary,while only 15.6%and 40.6%of the GPs knew the recommended treatments of falciparum and vivax malaria,respectively. However,after intervention 92%of the patients were diagnosed as malaria by RDTs and 3%by microscopy throughout the study.The GPs prescribed artemisinin-based combination therapy(ACTs) to 95%of confirmed falciparum cases and treated 82.4%of RDT confirmed falciparum negatives with chloroquine and primaquine.Concurrent with our study,an international NGO,Population Service International,supplied GPs with RDTs and ACTs at subsidized rate which was helpful.Conclusion:The study suggests that participation of GPs may help improve the disease management of malaria and thus assist in the country’s effort to control ma- laria.展开更多

But older patients with high systolic pressure and high pulse pressure could not be cured, and will have greater cardiovascular morbidity. This study aims to evaluate the effectiveness of general practitioners (GP) management for Chinese elderly hypertension, and explore reasonable and effective blood pressure management pattern. Methods: We searched the published literature for randomized controlled trials designed to improve blood pressure with community care management delivered by general practitioners or nurses, compared with usual care. Major outcome measures were systolic and diastolic blood pressure;the percentage of patients whose blood pressures are under control with community care management and Revman 5.3 was used in this study. Results: Pooled data from all 13 researches showed a lower outcome diastolic blood pressure (SMD = −0.95, 95% CI (−1.23, −0.67)) and systolic blood pressure (SMD = −1.17, 95% CI (−1.52, −0.81)) respectively in favor of community management. Pooled data from all 11 researches showed a higher percentage of patients with blood pressure under control in favor of community management (OR = 3.85, 95% CI (1.58, 9.37)) and the difference between treatment group and control group on blood pressure control ratio had statistical significance (Z = 2.97, P Conclusions: General practitioners’ management in Chinese elderly hypertension is effective, and complies with the advanced hypertension management guidelines. Objective:In order to explore the application effect of blood glucose management mode of multiteam cooperation in perioperative period of general surgery patients.Methods:The perioperative blood glucose control of 94 ...Objective:In order to explore the application effect of blood glucose management mode of multiteam cooperation in perioperative period of general surgery patients.Methods:The perioperative blood glucose control of 94 patients undergoing surgical treatment in general surgery from January 2016 to March 2019 was reviewed by Non-synchronous.According to the perioperative blood glucose management model of multi-team cooperation,the patients were divided into intervention group and control group.From January 2016 to January 2018,64 patients who did not implemented the multi-team blood glucose management model were the control group and from February 2018 to March 2019,30 patients who implemented the multi-team blood glucose management model were the intervention group,compared with two group about the differences in perioperative blood glucose.Results:It is no statistically significant about two group in highest and lowest blood sugar levels under fasting stats;during the fluid diet the blood sugar level of the intervention group was lower than control group(P<0.05),and when the patients was in the semifluid or food-feeding period,the highest and lowest blood sugar level is that the intervention group was lower than control group(P<0.05).The time of the blood sugar reaching the standard,the coincidence of complications rate and average hospitalization days in the intervention group were particularly lower than control group(P<0.05).Conclusion:If we adopt multiteam blood glucose management model can better control the perioperative blood glucose of patients undergoing general sugar.展开更多

BACKGROUND The R-on-T phenomenon is a malignant arrhythmia associated with potentially catastrophic consequences.It may initiate ventricular tachycardia or ventricular fibrillation,which can result in syncope or sudde...BACKGROUND The R-on-T phenomenon is a malignant arrhythmia associated with potentially catastrophic consequences.It may initiate ventricular tachycardia or ventricular fibrillation,which can result in syncope or sudden cardiac death.This manifestation poses a great challenge for anesthesiologists.However,it is rarely encountered in the perioperative setting.CASE SUMMARY We herein present a case in which the R-on-T phenomenon was incidentally revealed by 24-h Holter monitoring in a patient diagnosed with sigmoid colon cancer.Careful evaluation and treatment with mexiletine were carried out preoperatively under consultation with a cardiovascular specialist,and surgery was uneventfully performed under general anesthesia after thorough preparation.CONCLUSION Physicians should be vigilant about this infrequent but potentially fatal arrhythmia.Our experience suggests that the anesthetic process can be greatly optimized with careful preparation.展开更多

文摘The realty(real estate)management of ownership of buildings includes the owner(homeowner)entrust-management and the owner self-management,the former through the way of realty service contracts,the latter through the resolution of the owners’assembly and the owners’committee.The legal disputes of realty service contracts involve balancing interests among the realty service enterprise,individual owner,and all the owners.We should use dynamic balancing of the interests of the three parties to implement the interests of the win-win cooperation rather than the interests of the trade-off of“all or nothing.”The realty service contract is similar to the entrustment contract,both of which are based on the owners’entrustment.The duty to provide realty service is not simply equivalent to fiduciary duty.The realty service contract has the essence of both behavior debt and result debt.For the former,the general provisions of the entrustment contract can be referred to,and for the latter,the general provisions of the contract for work can be referred to.In view of the group characteristics of the realty service contract,owners have no right to terminate the realty service contract at any time but should obey the principle of due process and take the majority mechanism to make a dismissal resolution resolution.The resolutions and management stipulations worked out by the owners’assembly in a majority mechanism,both of which belong to resolution behavior and are legal tools of the owners’autonomy,shall be binding on all the owners.The relationship between the owner,the owner’s assembly,and the owner’s committee is similar to the fiduciary relationship.If the decision made by the owners’assembly or the owners’committee and the management stipulation infringes upon the legal rights and interests of the owners,the injured owner has the right to revocation litigation.At present,residential commodity owners have a relatively low degree of autonomy in China,and legal disputes over realty services are frequent.In the future,we need to perfect the legal incentive and constraint measures,and stimulate the vitality of owner autonomy to promote better implementation of the owner self-management and owner entrust-management.

文摘Objective:In order to explore the application effect of blood glucose management mode of multiteam cooperation in perioperative period of general surgery patients.Methods:The perioperative blood glucose control of 94 patients undergoing surgical treatment in general surgery from January 2016 to March 2019 was reviewed by Non-synchronous.According to the perioperative blood glucose management model of multi-team cooperation,the patients were divided into intervention group and control group.From January 2016 to January 2018,64 patients who did not implemented the multi-team blood glucose management model were the control group and from February 2018 to March 2019,30 patients who implemented the multi-team blood glucose management model were the intervention group,compared with two group about the differences in perioperative blood glucose.Results:It is no statistically significant about two group in highest and lowest blood sugar levels under fasting stats;during the fluid diet the blood sugar level of the intervention group was lower than control group(P<0.05),and when the patients was in the semifluid or food-feeding period,the highest and lowest blood sugar level is that the intervention group was lower than control group(P<0.05).The time of the blood sugar reaching the standard,the coincidence of complications rate and average hospitalization days in the intervention group were particularly lower than control group(P<0.05).Conclusion:If we adopt multiteam blood glucose management model can better control the perioperative blood glucose of patients undergoing general sugar.

In order to do the technical management in the project for export contract, it is necessary to do the work based on the Contract, to know the related laws and regulations, and national conditions, to coordinate the relationship among various parties well, and to carry out the management ahead of time.
文摘BACKGROUND The R-on-T phenomenon is a malignant arrhythmia associated with potentially catastrophic consequences.It may initiate ventricular tachycardia or ventricular fibrillation,which can result in syncope or sudden cardiac death.This manifestation poses a great challenge for anesthesiologists.However,it is rarely encountered in the perioperative setting.CASE SUMMARY We herein present a case in which the R-on-T phenomenon was incidentally revealed by 24-h Holter monitoring in a patient diagnosed with sigmoid colon cancer.Careful evaluation and treatment with mexiletine were carried out preoperatively under consultation with a cardiovascular specialist,and surgery was uneventfully performed under general anesthesia after thorough preparation.CONCLUSION Physicians should be vigilant about this infrequent but potentially fatal arrhythmia.Our experience suggests that the anesthetic process can be greatly optimized with careful preparation.
