When it develops into severe pancreatitis, its prognosis is poor. Therefore, early assessment of the degree of inflammatory response plays a crucial role in the treatment plan and prognosis of patients. More and more studies have shown that the levels of D-dimer (D-D), angiotensin-2 (Ang-2), phosphate, heparin-binding protein (HBP), retinol-binding protein-4 (RBP4), and osteoblastic protein (OPN) are closely related to the severity of acute pan-creatitis and can be used as effective indicators for early assessment of AP. In this paper, the research progress of the above indicators in assessing the severity of AP is summarized. 展开更多

The Influence of Integrated Chinese and Western Medicine Treatment on the Quality of Life and Laboratory Indicators of Patients with Novel Coronavirus Pneumonia

Objective: To investigate the influence of integrated Chinese and Western medicine treatment on the quality of life and laboratory indicators of patients with novel coronavirus pneumonia (COVID-19). Methods: A prospective, self-controlled study was conducted to analyze the changes in corresponding laboratory indicators and quality of life in 75 confirmed COVID-19 patients treated with integrated Chinese and Western medicine in our hospital during the early stage, middle stage, recovery period, and two weeks after discharge. The effectiveness and safety of the treatment regimen were evaluated in conjunction with the time for 2019-nCoV nucleic acid conversion, disease progression, and adverse reactions. Results: The PLT levels in the initial stage were significantly lower than those in the recovery period in 75 patients. The CRP levels in the initial stage were significantly lower than those after discharge for 2 weeks. The TBIL, IBTL, and DHIL levels in the initial stage were significantly lower than those in the middle stage. The K+ levels in the initial stage were significantly lower than those in the recovery period and after discharge for 2 weeks. The LYMGH levels in the initial stage were significantly lower than those in the recovery period and after discharge for 2 weeks. The TP and ALB levels in the initial stage were higher than those in the middle stage and the recovery period. The LDH levels, scores of daily activity limitation, scores of respiratory distress symptoms, scores of psychological emotions, CT imaging scores, and positive rate of nucleic acid were significantly lower than those in the recovery period and after discharge for 2 weeks. The AG, CK, CK-MB, and α-HBDH levels in the initial stage were significantly higher than those in the recovery period. Effects of Guizhi Shaoyao Zhimu Decoction on Laboratory Indicators and Immune Functions of Patients with Rheumatoid Arthritis

[Objectives]To explore the effects of Guizhi Shaoyao Zhimu Decoction on laboratory indicators and immune functions of patients with rheumatoid arthritis.[Methods]A total of 110 patients with rheumatoid arthritis admitted to the Second Department of Orthopedics,the Affiliated Chinese and Western Medicine Hospital of Nanjing,Nanjing University of Chinese Medicine from December 2014 to December 2016 were selected as the research objects and divided into a control group and an observation group using the random number table method,with 55 cases in each group.The control group was treated with conventional western medicine,and the observation group was treated with Guizhi Shaoyao Zhimu Decoction on the basis of the control group.Both groups were treated for one month.The clinical effects,improvement of clinical symptoms,laboratory indicators and immune function indicator levels between the two groups were compared;the incidence of adverse reactions in the two groups was counted.[Results]The total effective rate of the observation group was 92.73%,higher than that of the control group(78.18%)(P<0.05).Compared with that before treatment,after one month of treatment,the grip strength of the two groups increased,and the observation group was greater than the control group(P<0.05);the morning stiffness time and 20 m walking time of both groups were shortened,and the observation group was shorter than the control group(P<0.05);the number of joint tenderness,joint swelling,ESR,MPV,PDW,serum CRP,plasma IgA,IgG,IgM,RF levels in both groups decreased,and the observation group was lower than the control group(P<0.05);the serum C3 levels in both groups increased,and the observation group was higher than the control group(P<0.05).During the treatment,the incidence of adverse reactions in the observation group was 0.00%,which was lower than that in the control group(10.91%,P<0.05).[Conclusions]Using Guizhi Shaoyao Zhimu Decoction to treat rheumatoid arthritis can improve the laboratory indicators of patients,and enhance their immune functions,improve the curative effect,and the safety is high. 展开更多

The effect of empowerment education on depression level and laboratory indicators of patients treated with hemodialysis:a meta-analysis

Objective:The objective of this study was to evaluate the effect of empowerment education on depression level and laboratory indicators of patients treated with hemodialysis.Methods:We searched databases including Cochrane Library,Web of Science,Embase,PubMed,Chinese Biomedical Literature Database(CBM),Chinese Academic Journals Full-text Database(CNKI),VIP,and Wanfang.We included all the researches about the application of empowerment education in patients treated with hemodialysis.We used RevMan 5.3 to analyze the data after assessing the quality of researches.Results:We included 10 studies eventually.The results of meta-analysis showed that the self-efficacy[MD=1.10,95%confidence interval(CI;0.71,1.48),P<0.01],depression[MD=-6.94,95%CI(-8.53,-5.34),P<0.01],serum creatinine[Scr;MD=-116.46,95%CI(-171.90,-61.03),P<0.01],albumin[Alb;MD=2.93,95%CI(2.00,3.86),P<0.01],and blood urea nitrogen[BUN;MD=-0.95,95%CI(-1.89,-0.01),P=0.05]in the empowerment education group had significantly statistical difference compared with those in the usual education group.However,there was no significant difference in hemoglobin[Hb;MD=3.03,95%CI(-0.64,6.70),P=0.11]between two groups.Conclusions:Empowerment education can improve the self-efficacy in patients treated with hemodialysis,relieve depression,and improve Scr,Alb,and BUN.However,there is no significant improvement in Hb;it needs further exploration in clinical practice. 展开更多

Background Pink bollworm,Pectinophora gossypiella(Saunders)(Lepidoptera:Gelechiidae)has become a poten-tial pest of cotton by causing substantial yield losses around the world including Pakistan.Keeping in view the facts like limited research investigations,unavailability,and high cost of artificial diet’s constituents and their premixes,the present research investigations on the dietary aspect of P.gossypiella were conducted.The larvae of P.gossypiella were reared on different diets that were prepared using indigenous elements.The standard/laboratory diet com-prised of wheat germ meal 34.5 g,casein 30.0 g,agar–agar 20.0 g,sucrose 10.0 g,brewer’s yeast 5.0 g,α-cellulose 1.0 g,potassium-sorbate1.5 g,niplagin 0.5 g,decavitamin 0.01 g,choline-chloride 0.06 g,maize-oil 3.30 g,honey 2.0 g,and water 730.0 mL.Alternatives to cotton bolls and wheat germ meal were okra seed sprouts,okra fruit,cottonseed meal,and okra seed meals,which were included in the study to introduce an efficient and economic mass-rearing system.Results The larval development completed in 19.68d±0.05 d with a weight of 20.18mg±0.20 mg at the fourth instar fed on the cottonseed meal-based diet instead of wheat germ meal based diet.On the same diet,84.00%±4.00%,17.24 mg±0.03 mg,and 7.76d±0.06 d were recorded as pupae formation,pupal weight,and pupal duration,respectively.Adult emergence,76.00%±1.00%was recorded from pupae collected from larvae raised on cottonseed meal-based diet.These male and female moths lived for 40.25d±0.10 d,and 44.34d±0.11 d,respectively.Females deposited 21.28±0.04 eggs per day with the viability of 65.78%±0.14%.The larval mortal-ity at the fourth instar was 37.20%±1.36%and malformed pupation of 12.00%±1.41%was recorded.Replacement of wheat germ meal with that of local meals(cottonseed and okra seed)in the standard laboratory diet has saved 463.80 to 467.10 PKR with 1.62 to 1.63 cost economic returns,respectively.Conclusion This research is of novel nature as it provides a concise and workable system for the economic and suc-cessful rearing of P.gossypiella under laboratory conditions. 展开更多

There is research that supports the use of electronic records as a tool to speed documentation;and research that found that it is time consuming. The purpose of this quantitative retrospective before-after project was to measure the impact of using the laboratory value flowsheet within the EHR on documentation time. The research question was: “Does the use of a laboratory value flowsheet in the EHR impact documentation time by primary care providers (PCPs)?” The theoretical framework utilized in this project was the Donabedian Model. The population in this research was the two PCPs in a small primary care clinic in the northwest of Puerto Rico. The sample was composed of all the encounters during the months of October 2019 and December 2019. The data was obtained through data mining and analyzed using SPSS 27. The evaluative outcome of this project is that there is a decrease in documentation time after implementation of the use of the laboratory value flowsheet in the EHR. However, patients per day increase therefore having an impact on the number of patients seen per day/week/month. The implications for clinical practice include the use of templates to improve workflow and documentation as well as decreasing documentation time while also increasing the number of patients seen per day. . 展开更多

Due to the complex environment of the university laboratory,personnel flow intensive,personnel irregular behavior is easy to cause security risks.Monitoring using mainstream detection algorithms suffers from low detection accuracy and slow speed.Therefore,the current management of personnel behavior mainly relies on institutional constraints,education and training,on-site supervision,etc.,which is time-consuming and ineffective.Given the above situation,this paper proposes an improved You Only Look Once version 7(YOLOv7)to achieve the purpose of quickly detecting irregular behaviors of laboratory personnel while ensuring high detection accuracy.First,to better capture the shape features of the target,deformable convolutional networks(DCN)is used in the backbone part of the model to replace the traditional convolution to improve the detection accuracy and speed.Second,to enhance the extraction of important features and suppress useless features,this paper proposes a new convolutional block attention module_efficient channel attention(CBAM_E)for embedding the neck network to improve the model’s ability to extract features from complex scenes.Finally,to reduce the influence of angle factor and bounding box regression accuracy,this paper proposes a newα-SCYLLA intersection over union(α-SIoU)instead of the complete intersection over union(CIoU),which improves the regression accuracy while increasing the convergence speed.Comparison experiments on public and homemade datasets show that the improved algorithm outperforms the original algorithm in all evaluation indexes,with an increase of 2.92%in the precision rate,4.14%in the recall rate,0.0356 in the weighted harmonic mean,3.60%in the mAP@0.5 value,and a reduction in the number of parameters and complexity.Compared with the mainstream algorithm,the improved algorithm has higher detection accuracy,faster convergence speed,and better actual recognition effect,indicating the effectiveness of the improved algorithm in this paper and its potential for practical application in laboratory scenarios. 展开更多

BACKGROUND Although the etiology of nonalcoholic fatty liver disease(NAFLD)has not been thoroughly understood,the emerging roles of anthropometric indicators in assessing and predicting the risk of NAFLD have been highlighted by accumulating evidence.AIM To evaluate the causal relationships between five anthropometric indicators and NAFLD employing Mendelian randomization(MR)design.METHODS The Anthropometric Consortium provided genetic exposure data for five anthropometric indicators,including hip circumference(HC),waist circumference(WC),waist-to-hip ratio(WHR),body mass index(BMI),and body fat percentage(BF).Genetic outcome data for NAFLD were obtained from the United Kingdom Biobank and FinnGen Consortium.Genome-wide significant single nucleotide polymorphisms were chosen as instrumental variables.Univariable MR(UVMR)and multivariable MR(MVMR)designs with analytical approaches,including inverse variance weighted(IVW),MR-Egger,weighted median(WM),and weighted mode methods,were used to assess the causal relationships between anthropometric indicators and NAFLD.RESULTS Causal relationships were revealed by UVMR,indicating that a higher risk of NAFLD was associated with a perunit increase in WC[IVW:odds ratio(OR)=2.67,95%CI:1.42-5.02,P=2.25×10^(−3)],and BF was causally associated with an increased risk of NAFLD(WM:OR=2.23,95%CI:1.07-4.66,P=0.033).The presence of causal effects of WC on the decreased risk of NAFLD was supported by MVMR after adjusting for BMI and smoking.However,no causal association between BF and NAFLD was observed.In addition,other causal relationships of HC,WHR(BMI adjusted),and BMI with the risk of NAFLD were not retained after FDR correction.CONCLUSION This study establishes a causal relationship,indicating that an increase in WC is associated with a higher risk of NAFLD.This demonstrates that a suitable decrease in WC is advantageous for preventing NAFLD. 展开更多

In China, meteorological forecasting relies on meteorological data obtained from regional and national stations. However, there were discrepancies between the data collected from the meteorological station at the passion fruit growing base and the data from regional and national stations. Consequently, the high and low temperature disaster indicators determined by the meteorological station at the passion fruit growing base cannot be applied to meteorological forecasting. To address this issue and facilitate the monitoring and early warning of high and low temperature disasters in passion fruit cultivation in Fujian, China, we used multi-source hourly temperature data (including the data from meteorological observation stations in passion fruit growing bases, the nearest regional stations, and national surface conventional meteorological observation stations) in three cities in southwestern Fujian (Longyan, Sanming, and Zhangzhou) spanning the years 2020 to 2022. By employing comprehensive statistical analysis methods (0.5 interval division and Cumulative frequency), we identified that passion fruit in southwestern Fujian was susceptible to high temperature disasters during the blooming-fruiting period, as well as low temperature disasters during the sprouting period. Consequently, we developed high and low temperature disaster indicators based on data from regional and national stations for different phenological periods of passion fruit in this region. 展开更多

This paper proposes an intelligent management system (IMS) to help managers in their delicate and tedious task of exploiting the plethora of data (indicators) contained in management dashboards. This system is based on intelligent agents, ontologies and data mining. It is implemented by PASSI (Process for Agent Societies Specification and Implementation) methods for agent design and implementation, the Methodology for Knowledge Modeling and Hot-Winters for data prediction. Intelligent agents not only track indicators but also store the knowledge of managers within the company. Ontologies are used to manage the representation and presentation aspects of knowledge. Data mining makes it possible to: make the most of all available data;model the industrial process of data selection, exploration and modeling;and transform behaviors into predictive indicators. An instance of the IMS named SYGISS, currently in operation within a large brewery organization, allows us to observe very interesting results: the extraction of indicators is done in less than 5 minutes whereas manual extraction used to take 14 days;the generation of dashboards is instantaneous whereas it used to take 12 hours;the interpretation of indicators is instantaneous whereas it used to take a day;forecasts are possible and are done in less than 5 minutes whereas they did not exist with the old management. Objective:The objective of the study is to explore the traditional Chinese medicine (TCM) syndrome distribution in colorectal cancer (CRC) and its correlation with treatment methods and clinical laboratory indicators.Materials and Methods: Using the CRC cases report form of TCM,760 CRC patients with TCM four diagnosis information,western medicine treatment information and clinical laboratory indicators were collected,and TCM syndromes distribution in CRC were summarized.The correlation between TCM syndrome type and western medicine treatments,clinical laboratory indicators such as liver and kidney function,immune function,and tumor biomarkers was analyzed. Results:In 760 cases of CRC, Spleen deficiency syndrome (SDS, 25%),liver and kidney Yin deficiency syndrome (LKYDS, 13%), LKYDS-SDS,12%,spleen deficient Qi stagnation syndrome (SDQSS, 10%),and damp heat syndrome (DHS, 9%) were more common TCM syndrome types.LKYDS, SDS,LKYDS-SDS,and SDQSS were significantly distributed under different treatment methods (P < 0.001).There was no statistically significant difference in the distribution of immune fUnction and cytokine among the five TCM syndromes (P > 0.05),but there was statistically significant difference in the distribution of blood routine,liver and kidney function, and tumor biomarkers (P < 0.05). Conclusion: LKYDS,SDS,LKYDS-SDS, SDQSS,and DHS were the first five TCM syndromes in CRC.There were the significant correlations between the distribution of TCM syndrome and the clinical laboratory indicators,and the distribution of TCM syndromes was affected by surgery, radiotherapy,and chemotherapy. 展开更多

Quality improvement projects were instituted in 2011 in line with ISO 15189 requirements for accreditation of medical laboratory. In this study we evaluated the performance of the laboratory through some set of quality indicators (QI). Methodology: This was a retrospective study to evaluate laboratory QIs monitored from January 2011 to December 2013. The QIs were specimen rejection rate (SSR), turnaround time (TAT), proficiency testing performance (PTP) and client satisfaction survey (CSS). Data was collected into an excel file for analysis and percentage performance compared among years. SSR & TAT were evaluated with the Sigma scale. Results: A total of 7920 (2194 in 2011, 2715 in 2012, 3011 in 2013) specimens were received for testing. 22 (0.28%) specimens were rejected and 81 (1.02%) specimens’ results were reported after the acceptable TAT, giving a Sigma level of 4.27 and 3.82 for SSR and TAT respectively. There was steady improvement in PTP: CD4+ from 67% in 2011 to 90% in 2013, hematology from 81% in 2012 to 83% in 2013, blood film reading 79% in 2011 to 83% in 2013 and chemistry from 90% in 2011 to 93% in 2013. HIV serology recorded 100% throughout. CSS increased from 59% in 2012 to 78% in 2013. However, there was no statistically significance difference reported for PTP and CSS over the years (P > 0.05). Conclusion: The study highlights the need to continuously evaluate QIs and calls for more effort to improve on PTP and focuses on understanding and improving on clients concerns. 展开更多

BACKGROUND Isotretinoin is a widely prescribed drug for various dermatological conditions,especially acne.Blood tests are routinely performed to monitor its side effects.However,the optimal testing schedule remains controversial.AIM To evaluate the laboratory monitoring tests and schedules of patients with acne taking isotretinoin to determine the most efficient testing regimen.METHODS We retrospectively reviewed electronic medical records from the Dermatology Department of Dongguk University Ilsan Hospital from 2005 to 2020 for patients prescribed isotretinoin for acne who underwent monthly blood tests.RESULTS Aspartate aminotransferase(AST),alanine aminotransferase(ALT),and trigly-ceride(TG)levels significantly changed between 5 and 6 mo when the total prescription period and dose variables were considered altogether.The high-density lipoprotein level also significantly changed between 5 and 6 mo.However,low-density lipoprotein(LDL)and total cholesterol levels significantly changed between 1 and 2 mo.CONCLUSION We recommend testing AST,ALT,and TG levels once every 5 to 6 mo.We also suggest testing LDL and total cholesterol levels during the first and second months. 展开更多

Objective: To evaluate the interest of the simulation in the learning of the obstetric examination of parturient by medical students. Materials and methods: It was an analytical cross-sectional study that took place from 1 to 28 February 2020 at the Yalgado Ouedraogo Teaching Hospital in Ouagadougou, Burkina Faso. The study population was made up of medical students at the Doctorate II level. The previous experience of the students in the obstetric examination of the parturient, the contribution of the simulation on their feelings and on their skills towards this examination were studied. The Mc Nemar Chi<sup>2</sup> test was used to compare percentages on paired samples. Results: The level of difficulty in performing the obstetrical examination in the parturient evolved downwards after the practice of the simulation. The stress level to perform the obstetric exam has been significantly reduced. On the other hand, confidence has increased. The students’ ability to appreciate each of the elements of the obstetric examination studied had improved after the practice of simulation. Conclusion: The simulation in the skills laboratory was a great contribution and the acquisition of models of better fidelity would help to improve the contribution to the confidence building and the acquisition of skills by learners. 展开更多

A novel expandable conductor was designed and applied in deep-water drilling to improve the vertical and lateral bearing capacity with a significant reduction of conductor jetting depth and soaking time. The vertical and lateral bearing capability of expandable conductors was depicted based on the ultimate subgrade reaction method and pile foundation bearing theory. The load-bearing characteristics of a laboratory-scale expandable conductor were analyzed through laboratory experiments. The serial simulation experiments are accomplished to study the bearing characteristics(vertical ultimate bearing capacity, lateral soil pressure, and lateral displacement) during the conductor soaking process. The laboratory experimental results show that the larger the length and thickness of expandable materials are,the higher the bearing capacity of the wellhead will be. During the conductor soaking process, the soil pressure around the three expandable conductors increases faster, strings representing a stronger squeezing effect and resulting in higher vertical bearing capacity. Furthermore, the lateral displacement of novel expandable conductor is smaller than that of the conventional conductor. All the advantages mentioned above contributed to the reduction of conductor’s jetting depth and soaking time. Lastly, the application workflow of a novel expandable deep-water drilling conductor was established and the autonomous expandable conductor was successfully applied in the South China Sea with a significant reduction of conductor’s jetting depth and soaking time. According to the soil properties and designed installation depth of the surface conductor, the arrangement of expandable materials should be designed reasonably to meet the safety condition and reduce the construction cost of the subsea wellhead. 展开更多

Background: Poor sleep, fatigue, and pain are major health problems in patients with systemic lupus erythematosus (SLE). However, only cross-sectional surveys on these health outcomes have been conducted, and the association between day-to-day fluctuations remains unknown. Objectives: We aimed to characterize daily fluctuations in sleep quality, physical activity, pain, and fatigue in patients with SLE. Method: Exploratory study with a cross-sectional design. Two rheumatology centers (a university hospital and a prefectural hospital) in Japan between September 2017 and May 2019. The sample size was set to 20. Demographic and clinical data were collected. Sleep and physical activity were measured with monitoring devices;pain and fatigue levels were recorded daily during the 4-week period. The Pittsburgh Sleep Quality Index, Short Form Health Survey-12, the Japanese version of the Lupus Patient Outcome, and SLE Disease Activity Index 2000 were collected at the start and end of the study. Descriptive statistics and coefficients of variation (CV) were tabulated to examine daily fluctuations. Pearson correlation coefficients were obtained for monitored variables. Results: The mean age was 43.7 ± 8.5 years, and the mean SLE duration was 16.0 ± 7.2 years. The mean moderate-to-vigorous physical activity (MVPA) duration was 7.8 ± 5.8 min/day, and the mean total sleep duration was 391.8 ± 65.3 min, with a mean sleep efficiency of 88.6% ± 6.1%. Daily fluctuations were high for leaving the bed frequency, MVPA duration, pain, and waking after sleep onset. Seventeen participants showed correlations between some of the variables, such as fatigue or longer MVPA duration and poorer sleep outcomes;longer sleep latency and increased frequency of leaving the bed;and higher physical activity and increased pain and fatigue. Conclusion: The quality of sleep and fatigue fluctuated daily, and correlations existed between these variables, as well as for pain and physical activity. The impact of MVPA duration on pain and fatigue is of concern as increased physical activity may worsen the quality of life patients with SLE. The monitoring of sleep and physical activity using the device seems feasible for SLE symptom management. 展开更多

Background: Descriptive statistics have been used to document the high prevalence of sleep disorders, fatigue, and pain in patients with systemic lupus erythematosus (SLE). Objectives: To describe day-to-day variations in sleep indicators, physical activity level, pain, and fatigue measured using objective and subjective indicators in lupus patients with sleep disorders. Methods: We selected three patients with sleep disorders, measured by the Pittsburgh Sleep Quality Index score (≥9), out of 20 participants followed up for 4 weeks. We compared the self-rated quality of life (QoL) measured using the Pittsburgh Sleep Quality Index (PSQI) with that of objectively measured sleep indicators monitored using the Nemuri scan. The detailed data collection methods have been previously published. Results: The sleep status generated by the Nemuri scan revealed that Case A with a flare and Case B without a flare required frequent daytime rest. The PSQI sleep duration and sleep latency were generally in agreement with the monitored data, although the PSQI failed to capture the complexity of sleep disorders, particularly the fluctuations in the quality of sleep indicators. Patient C approximately 4 hours on weekdays and 10 hours on Saturday nights. All three cases had high fatigue levels, and their moderate-to-vigorous physical activity duration was less than one-half of duration that recommended by the World Health Association. Conclusion: Visual display for objective monitoring of sleep quality is an excellent tool for understanding patients who require frequent resting and irregular sleep indicators. Objective monitoring of sleep quality, along with self-rated pain and fatigue, promotes an understanding of how patients with SLE cope with severe symptoms. 展开更多
