Predictive modeling for postoperative delirium in elderly patients with abdominal malignancies using synthetic minority oversampling technique

BACKGROUND Postoperative delirium,particularly prevalent in elderly patients after abdominal cancer surgery,presents significant challenges in clinical management.AIM To develop a synthetic minority oversampling technique(SMOTE)-based model for predicting postoperative delirium in elderly abdominal cancer patients.METHODS In this retrospective cohort study,we analyzed data from 611 elderly patients who underwent abdominal malignant tumor surgery at our hospital between September 2020 and October 2022.The incidence of postoperative delirium was recorded for 7 d post-surgery.Patients were divided into delirium and non-delirium groups based on the occurrence of postoperative delirium or not.A multivariate logistic regression model was used to identify risk factors and develop a predictive model for postoperative delirium.The SMOTE technique was applied to enhance the model by oversampling the delirium cases.The model’s predictive accuracy was then validated.RESULTS In our study involving 611 elderly patients with abdominal malignant tumors,multivariate logistic regression analysis identified significant risk factors for postoperative delirium.These included the Charlson comorbidity index,American Society of Anesthesiologists classification,history of cerebrovascular disease,surgical duration,perioperative blood transfusion,and postoperative pain score.The incidence rate of postoperative delirium in our study was 22.91%.The original predictive model(P1)exhibited an area under the receiver operating characteristic curve of 0.862.In comparison,the SMOTE-based logistic early warning model(P2),which utilized the SMOTE oversampling algorithm,showed a slightly lower but comparable area under the curve of 0.856,suggesting no significant difference in performance between the two predictive approaches.CONCLUSION This study confirms that the SMOTE-enhanced predictive model for postoperative delirium in elderly abdominal tumor patients shows performance equivalent to that of traditional methods,effectively addressing data imbalance. 展开更多

This paper aims to analyze and evaluate a model of forest conservation and management of ethnic minority (or indigenous) people in central Vietnam, often referred to as the spiritual forest. These forests, called sacred forest or ghost forests by the ethnic minority people in Thua Thien Hue province, have existed for a long time among forest residents. However, they have recently declined, both in quality and quantity, due to various factors, including changes in society, economy, environment, and perception, among other reasons. Based on research conducted in A Luoi district, Thua Thien Hue province with household interviews, group discussions, and field surveys, we find that spiritual forest retains religious and human significance. They are also often among the last remaining natural forests left due to deforestation by human activities. The research results indicate challenges that conservation of spiritual forest may face, while giving recommendations derived from communities for sustainable forest development and conservation in the region. 展开更多

Low fertility and population aging are important factors affecting economic and social development.From 2000 to 2020,the development of low fertility in ethnic minority areas shows three trends of accelerated declining,slow declining and first falling and then rising,which are the results of the balance between the inhibiting effect of controlling birth policy and the rapidly rising birth cost,and the promoting effect of encouraging birth policy on the fertility desire of social members.The development of aging in ethnic minority areas shows two trends of acceleration and deceleration,which are the results of the balance between the promoting effect of controlling birth policy,the outflow of young and middle-aged labor force and the rise of life expectancy,and the inhibiting effect of the encouraging birth policy,the restraint of talent return and talent introduction.Combined with the development degree of low fertility and aging,we can accurately classify and implement policies for different ethnic minority areas.The areas with serious low fertility and population aging problems need economic policies to change the population age structure as soon as possible.The areas with slight low fertility and population aging problems should carry out ideological policies in priority to maintain the benign structure of population age. 展开更多

HIV and AIDS has continued to be a major public health concern, and hence one of the epidemics that the world resolved to end by 2030 as highlighted in sustainable development goals (SDGs). A colossal amount of effort has been taken to reduce new HIV infections, but there are still a significant number of new infections reported. HIV prevalence is more skewed towards the key population who include female sex workers (FSW), men who have sex with men (MSM), and people who inject drugs (PWID). The study design was retrospective and focused on key population enrolled in a comprehensive HIV and AIDS programme by the Kenya Red Cross Society from July 2019 to June 2021. Individuals who were either lost to follow up, defaulted (dropped out, transferred out, or relocated) or died were classified as attrition;while those who were active and alive by the end of the study were classified as retention. The study used density analysis to determine the spatial differences of key population attrition in the 19 targeted counties, and used Kilifi county as an example to map attrition cases in smaller administrative areas (sub-county level). The study used synthetic minority oversampling technique-nominal continuous (SMOTE-NC) to balance the datasets since the cases of attrition were much less than retention. The random survival forests model was then fitted to the balanced dataset. The model correctly identified attrition cases using the predicted ensemble mortality and their survival time using the estimated Kaplan-Meier survival function. The predictive performance of the model was strong and way better than random chance with concordance indices greater than 0.75. 展开更多

Gaokao has been considered as the gatekeeper of intergenerational class reproduction in today’s slowing social mobility in China.In order to bolster the fairness of Gaokao,the Policy of Bonus Scores for Ethnic Minority Candidates(PBSEMC)has been issued,whose,however,effectiveness in promoting upward social class has not been adequately empirically explored.The difference-in-differences(DID)analysis based on cross-sectional data,which is distinct from the traditional DID analysis paradigm,was implemented to assess the Reform of PBSEMC in the Guangxi Zhuang Autonomous Region in 1988.The results showed that PBSEMC had a significant and positive impact on the social classes of ethnic minorities,and gender,residence type and occupation type were also relevant to social class,aligning with the previous literature.The PBSEMC is effective in improving the social classes of ethnic minorities.However,the PBSEMC policy should be made more precise,in order to benefit the candidates who are in genuine need of assistance. 展开更多

Among these are various ethnic folk chess games. They are time-honored with many types such as the “Fang” chess of the Hui ethnic group, the “Jiu” chess of the Tibetans, the “King and Ministers” chess, the Mongolian chess, the Mongolian military chess, etc. These games embody distinctive ethnic and regional characteristics, as well as the cultural elements of a particular ethnic minority group. It is of great significance, therefore, to study the ethnic chess by collecting, documenting and comparing different types of related chess. The paper explores ways to standardize, protect and promote them scientifically in joint efforts, and draws a conclusion that examining their significance in the present era and the ethnic characteristics is the key to understanding the profound culture behind the chess. 展开更多

China has made great efforts to alleviate poverty in rural ethnic minority areas and targeted achieving the poverty-alleviation task by the end of 2020.Aba,Ganzi,and Liangshan,three of the poorest ethnic prefectures in Sichuan Province,Southwest China,have all implemented“Internet+”tactics since 2013,which have had the positive effect of increasing family revenues by improving communication infrastructure and encouraging the large-scale use of e-commerce.This paper aims to comprehensively investigate whether“Internet+”tactics play a key role in poverty alleviation in Sichuan’s rural ethnic minority areas and to propose further measures to enhance the efficiency of e-commerce practice.To this end,we conduct an analysis using the framework of classic growth theory and use panel data from 2000 to 2018 to examine the relationship between Communication Infrastructure Investment(CII)and a set of poverty-alleviation indicators,including local GDP growth rate(LGGR),local government revenue(LGR),and per-capita income of residents(PCIR).The results indicate that strengthening CII improves the PCIR and local economic growth,playing a key role in poverty alleviation.However,the stimulation of CII on LGGR and LGR wanes as time passes.More financial and technical actions will be needed to improve the efficiency and quality of current strategies for sustainable development in those areas. 展开更多

A well-recognized fact is that addressing the impacts of climate change on vulnerable communities and minority groups remains a central focus toward achieving the Sustainable Development Goals,specifically Goals 11 and 13.Approaches for effective adaptation to climate change through national and local efforts fundamentally aim to create environmentally sustainable,socially inclusive,and economically vibrant communities.This paper associates the impacts of climate change to the preservation of threatened minority languages in semi-arid areas in Northern Ghana.This review relies on primary and secondary sources on climate-induced migration,minority languages,and threats of language loss through a keyword search followed by rigorous content analysis.The study confirms that forced displacement due to harsh climatic and other environmental conditions is currently occurring in the upper regions(Upper East and Upper West Regions)of Ghana with minority linguistic groups being forced to migrate to the southern part of the country,where their culture and language are threatened due to large linguistic groups.The literature well establishes the north-south mobility with various debates on its root causes.However,the phenomenon is understudied along with the lack of specific national strategies for addressing it and the associated language loss.Therefore,the need emerges for further studies to enhance the current understanding of the phenomenon to inform policy interventions and protect minority languages threatened by climate-induced migration.The focus on an understudied subject and geographic scope makes the findings extremely relevant for the expansion of knowledge on internal migration in the context of climate change in Northern Ghana. 展开更多

Traditional medicine systems practiced by various ethnic minorities represent an important part of traditional Chinese medicine.The past 12 months have witnessed extensive research pertaining to different Chinese minority traditional medicine(CMTM).The annual CMTM review evaluates research published during 2019 in different CMTM including Tibetan medicine,Uyghur medicine,Mongolian medicine,Korean medicine and Zhuang medicine.Research in the field of Tibetan medicine focused on pharmacology,pharmacy,plant sciences,medicinal chemistry and integrated complementary medicine and the top three countries were China,USA and India.Research in Uyghur medicine mainly pertained to chemistry,pharmacology,pharmacy,and food science technology and the publications were mainly from China.Research in Mongolian medicine mainly pertained to pharmacology,pharmacy,analytical chemistry,biochemistry,molecular biology and experimental research;the publications were mainly from China and Mongolia.In short,research related to traditional medicine of various ethnic minorities was mainly conducted in China and the neighboring countries.The research focus for each minority medicine is essentially on the effects and mechanisms of action of the active ingredients of the ethnodrugs and the special prescriptions or therapies.The generated evidence will facilitate further developments in this field. 展开更多

This paper, by the case studies of minority languages in New Zealand, Netherlands, and China, illustrates the effects of these countries' general language policies on their minority languages. Minority languages are seen as a part of cultural identity of minority groups. To protect their own culture and identity, they devote themselves to seek the equal right for their languages and cultures. China boasts 56 nationalities who are mostly inhabited in the sparsely populated mountain regions in West China. Like many other places in the Han nationality distributed regions, minority nationality inhabited regions are also facing the challenge of sustainable development in terms of both natural and human factors. Taking the Tibetan inhabited regions (TIR) in Sichuan Province as a case, this paper points out some major problems in the process of economic development in perspective of sustainability and leaves some hints for development strategies. While natural conditions such as geological instability are playing a major role to the environmental degradation, human activities and improper use of resources might constitute an important agent in the environmental deterioration. In order to promote local economic development and improve people’s living standard, sustainable planning is expected to be made and people’s awareness of sustainability should be strengthened. 展开更多

ABSTRACT: China is a country that has a large number of minorities, who live in specific regions which are mainly located in central and west China. These regions are abundant in natural resources, which are potential advantages and also an important basis for economic development in these regions. They are of general importance for China in strategic and ecological terms. They will not only affect the environment for survival of the minority regions but also affect the sustainable development of the eastern and central parts of China and even the survival and development of the Chinese nation as a whole. Minorities live in relatively compact communities, among the 55 minorities, 35 of them were highly compact minorities, with a compact level of more than 90% . Here the development in infrastructure as well as social and economic development (e. g. export-oriented economy) has been and is still slow, regional poverty are most concentrated. The economic structures in minority areas are still in the low stage, which have resulted in underdevelopment in these regions. 展开更多
