We analyzed the source apportionment and assessed the health risk of TMs in agricultural soils by using positive matrix factorization(PMF) model and health risk assessment(HRA) model based on Monte Carlo simulation. Meanwhile, we combined PMF and HRA models to explore the health risks of TMs in agricultural soils by different pollution sources to determine the priority control factors. Results showed that the average contents of cadmium(Cd), arsenic (As), lead(Pb), chromium(Cr), copper(Cu), nickel(Ni), and zinc(Zn) in the soil were found to be 0.26, 5.93, 27.14, 61.32, 23.81, 32.45, and 78.65 mg/kg, respectively. Spatial analysis and source apportionment analysis revealed that urban and industrial sources, agricultural sources, and natural sources accounted for 33.0%, 27.7%, and 39.3% of TM accumulation in the soil, respectively. In the HRA model based on Monte Carlo simulation, noncarcinogenic risks were deemed negligible(hazard index <1), the carcinogenic risks were at acceptable level(10^(-6)<total carcinogenic risk ≤ 10^(-4)), with higher risks observed for children compared to adults. The relationship between TMs, their sources, and health risks indicated that urban and industrial sources were primarily associated with As, contributing to 75.1% of carcinogenic risks and 55.7% of non-carcinogenic risks, making them the primary control factors. Meanwhile, agricultural sources were primarily linked to Cd and Pb, contributing to 13.1% of carcinogenic risks and 21.8% of non-carcinogenic risks, designating them as secondary control factors.展开更多

The distance between the patients and the pharmacists varies depending on many factors, such as gender, posture and the patients’ mood. Only a few of these papers have actually measured and validated distance with patients. In this study, we validated our method of assessing mood and measuring distance before beginning a survey with patients. Methods: We measured comfortable interpersonal distance among men and women using an ecological scenario, in which a pharmacist approaches the subject, and the subject is asked to stop the pharmacist at the distance he/she feel comfortable with. Five pharmacists and 33 subjects participated in the study. The Japanese version of the Brief Mood Questionnaire Checklists (BMC-J) was used to quantify the subject’s mood for the day, and then the distance from the pharmacist that the subjects considered comfortable was measured at the bedside. The relationship between the mood and distance obtained was examined. Results: The comfortable distance of subjects was influenced by gender, posture, and mood. The shortest distance was 94.7 ± 11.1 cm (mean ± SD), for the male subjects versus the female pharmacists in the sitting position. The distance of male subjects shorted when they had positive emotions and lengthened when they were worried. Female subjects maintained a long distance from both male and female pharmacists when they had positive emotions and a short distance when they were worried. Conclusion: Findings show that the distance changes depending on the subjects’ mood at the time of measurement. It was found that the present measurement method can be used to determine the psychological state of the patient and measure the comfort distance at that time, and can be used as a simple method to examine these relationships. Therefore, it is also considered a practical method for the next step, which is a clinical study on patients.展开更多

Herceptin receptor over-expression is seen in 15% - 30% of breast tumours, and is associated with increased aggression, poorer prognosis, higher grade at diagnosis and increased lymphatic dissemination. This study aimed at evaluating the role of MRI in Herceptin receptor positive vs negative tumours. Methods: 193 pre-operative MRIs were performed in 2021 for staging of 162 Her-2 negative and 37 Her-2 positive tumours. Recall rates and further biopsies (ipsilateral/contralateral) were assessed in both groups, and MRI largest size was compared to pathological size of invasive cancer and DCIS. Results: 36.4% of Her-2 negative tumours were recalled;further ipsilateral malignancy was identified in 13.6%. Contralateral malignancy was identified in 1.2%. 29.7% of Her-2 positive tumours were recalled;further ipsilateral malignancy was identified in 16.2%. No contralateral malignancy was seen in Her-2 positive tumours. The OR of Her-2 positive tumours having ipsilateral foci of malignancy on MRI is 0.83 (CI 0.3, 2.2). Pathological size concordance with MRI size was seen in 70.3% of Her-2 negative, and 48.6% of Her-2 positive tumours. Discordance in both groups was due to MRI size overestimation (70.8% of Her-2 negative discordance;89.4% of Her-2 positive discordance). Conclusions: Pre-operative MRI did not detect significant increased additional foci in Her-2 positive tumours. Significant concordance with pathological size was not seen in both groups;MRI overestimation was the most frequent cause for discordance in both groups. Advances in Knowledge: This study compares MRI features of Her-2 positive and Her-2 negative tumours. It demonstrates that there is no significant increased multifocality or multicentricity of Her-2 positive tumours, but MRI over-estimates size in 30% of Her-2 negative and 51% of Her-2 positive cancers.展开更多

While any organization has a fair share of problems to address, such an overarching focus on everything that is wrong with an organization can create a harmful cycle of pessimism, depression, and low energy that can be difficult to overcome. Positive Organization Scholarship (POS) practitioners, much like the skilled therapist, help organizations shift from a negative, limiting mindset into a positive, open mindset in order to facilitate positive change. This paper explores how cognitive reappraisal strategies common in psychotherapy could be used by POS practitioners to facilitate change and growth in the organizations they serve.展开更多

文摘Objective Multiparametric magnetic resonance imaging(MRI)has become the standard of care for the diagnosis of prostate cancer patients. This study aimed to evaluate the influence of preoperative MRI on the positive surgical margin(PSM)rates. Methods We retrospectively reviewed 1070 prostate cancer patients treated with radical prostatectomy(RP)at Siriraj Hospital between January 2013 and September 2019.PSM rates were compared between those with and without preoperative MRI.PSM locations were analyzed. Results In total,322(30.1%)patients underwent MRI before RP.PSM most frequently occurred at the apex(33.2%),followed by posterior(13.5%),bladder neck(12.7%),anterior(10.7%),posterolateral(9.9%),and lateral(2.3%)positions.In preoperative MRI,PSM was significantly lowered at the posterior surface(9.0%vs.15.4%,p=0.01)and in the subgroup of urologists with less than 100 RP experiences(32%vs.51%,odds ratio=0.51,p<0.05).Blood loss was also significantly decreased when a preoperative image was obtained(200 mL vs.250 mL,p=0.02).Multivariate analysis revealed that only preoperative MRI status was associated with overall PSM and PSM at the prostatic apex.Neither the surgical approach, the neurovascular bundle sparing technique, nor the perioperative blood loss was associated with PSM.Conclusion MRI is associated with less overall PSM,PSM at apex, and blood loss during RP.Additionally, preoperative MRI has shown promise in lowering the PSM rate among urologists who are in the early stages of performing RP.
文摘BACKGROUND Perimenopausal is the period when women's ovarian function begins to decline before and after menopause.During this period, women experience a series of mental state changes, such as decreased hormone levels, decreased libido, and even female sexual dysfunction(FSD)in severe cases, which reduces their quality of life.Factors affecting the occurrence of FSD include physiological and nonphysiological factors, among which physiological factors are uncontrollable. Therefore,it is particularly important to ascertain the related non-physiological factors that affect the occurrence of FSD for improving the quality of sexual life of perimenopausal women.AIM To investigate the mediating effect of depressive mood and body image on menopausal symptoms and sexual function in perimenopausal women.METHODS A total of 186 perimenopausal women were enrolled between January 2019 and January 2021 and divided into the FSD(134 cases)and control(52 cases)groups based on the presence and absence of FSD.Clinical data were compared between the two groups.FSD-related factors were analyzed using logistic regression analysis.Hamilton Depression Scale(HAMD),Body Image Scale(BIS),and Menopause Rating Scale(MRS)scores were compared among women with different FSD scores. The correlation of the MRS score with the BIS and HAMD scores and the mediating effect of the BIS and HAMD scores on the MRS score and female sexual function index(FSFI)were analyzed.RESULTS The HAMD and BIS scores were higher in the FSD group than in the control group, and the difference in monthly income between the two groups was statistically significant(all P<0.05).Monthly income of<2000 yuan[odds ratio(OR)=26.586,P=0.000],BIS score(OR=1.590,P=0.000),and HAMD score(OR=1.884,P=0.000)were independent risk factors for FSD.MRS scores were positively correlated with BIS and HAMD scores(r=0.358 and 0.244,P=0.000 and 0.001,respectively)and negatively correlated with FSFI scores(r=-0.433,P=0.000).Body image and depressive mood had partial mediating effects, accounting for 39.90%of the total effect.CONCLUSION Depression and body image play mediating roles between menopausal symptoms and sexual function in perimenopausal women.

文摘Objective Positive surgical margins(PSMs)after radical prostatectomy(RP)indicate failure of surgery to completely clear cancer.PSMs confer an increased risk of biochemical recurrence(BCR),but how more robust outcomes are affected is unclear. This study investigated factors associated with PSMs following RP and determined their impact on clinical outcomes(BCR,second treatment[radiotherapy and/or androgen deprivation therapy],and prostate cancer-specific mortality[PCSM]).Methods The study cohort included men diagnosed with prostate cancer(pT2-3b/N0/M0)between January 1998 and June 2016 who underwent RP from the South Australian Prostate Cancer Clinical Outcomes Collaborative database.Factors associated with risk of PSMs were identified using Poisson regression. The impact of PSMs on clinical outcomes(BCR,second treatment, and PCSM)was assessed using competing risk regression. Results Of the 2827 eligible participants,28%had PSMs—10%apical,6%bladder neck,17%posterolateral,and 5%at multiple locations.Median follow-up was 9.6 years with 81 deaths from prostate cancer recorded.Likelihood of PSM increased with higher pathological grade and pathological tumor stage, and greater tumour volume, but decreased with increasing surgeon volume(odds ratio[OR]:0.93;95%confidence interval[CI]:0.88-0.98,per 100 previous prostatectomies).PSMs were associated with increased risk of BCR(adjusted sub-distribution hazard ratio[sHR]2.5;95%CI 2.1-3.1)and second treatment(sHR 2.9;95%CI 2.4-3.5).Risk of BCR was increased similarly for each PSM location, but was higher for multiple margin sites. We found no association between PSMs and PCSM.Conclusion Our findings support previous research suggesting that PSMs are not independently associated with PCSM despite strong association with BCR.Reducing PSM rates remains an important objective, given the higher likelihood of secondary treatment with associated comorbidities.

文摘The influence maximization(IM)problem aims to find a set of seed nodes that maximizes the spread of their influence in a social network. The positive influence maximization(PIM)problem is an extension of the IM problem, which consider the polar relation of nodes in signed social networks so that the positive influence of seeds can be the most widely spread. To solve the PIM problem, this paper proposes the polar and decay related independent cascade(IC-PD)model to simulate the influence propagation of nodes and the decay of information during the influence propagation in signed social networks. To overcome the low efficiency of the greedy based algorithm, this paper defines the polar reverse reachable(PRR)set and devises a signed reverse influence sampling(SRIS)algorithm. The algorithm utilizes the ICPD model as well as the PRR set to select seeds. There are two phases in SRIS.One is the sampling phase, which utilizes the IC-PD model to generate the PRR set and a binary search algorithm to calculate the number of needed PRR sets. The other is the node selection phase, which uses a greedy coverage algorithm to select optimal seeds. Finally,Experiments on three real-world polar social network datasets demonstrate that SRIS outperforms the baseline algorithms in effectiveness.Especially on the Slashdot dataset,SRIS achieves 24.7% higher performance than the best-performing compared algorithm under the weighted cascade model when the seed set size is 25.
文摘Objectives: MRI is the most sensitive modality for local staging of breast cancer. It demonstrates that there is no significant increased multifocality or multicentricity of Her-2 positive tumours, but MRI over-estimates size in 30% of Her-2 negative and 51% of Her-2 positive cancers.

文摘At present, with the development of technology, the detection of cryptococcal antigen(CRAG)plays an increasingly important role in the diagnosis of cryptococcosis. However, the three major CRAG detection technologies, latex agglutination test(LA),lateral flow assay(LFA)and Enzyme-linked Immunosorbent Assay, have certain limitations.Although these techniques do not often lead to false-positive results, once this result occurs in a particular group of patients(such as human immunodeficiency virus patients),it might lead to severe consequences.

文摘Chromium atom photolithography gratings are a promising technology for the development of nanoscale length standard substances due to their high accuracy, uniformity, and consistency. However, the inherent difference between the interaction of positive and negative frequency detuning standing wave field and the atoms can cause a difference in the adjacent peak-to-valley heights of the grating in positive and negative frequency detuning chromium atom lithography, which greatly reduces its accuracy. In this study,we performed a controlled variable growth simulation using the semi-classical theoretical model and Monte Carlo method with trajectory tracking and ballistic deposition methods to investigate the influence of key experimental parameters on the surface growth process of positive and negative frequency detuning atomic lithography gratings. We established a theoretical model based on simulation results and summarized empirical equations to guide the selection of experimental parameters. Our simulations achieved uniform positive and negative frequency detuning atomic lithography gratings with a period of 1/4 of the wavelength corresponding to the atomic transition frequency, and adjacent peak-to-valley heights differing by no more than 2 nm,providing an important theoretical reference for the controllable fabrication of these gratings.
文摘Many organizational development perspectives and discussions adopt a medical or diagnostic viewpoint of organizational dysfunction, focusing heavily on the numerous deficiencies, mistakes, and other negative aspects of struggling organizations. This paper explores how cognitive reappraisal strategies common in psychotherapy could be used by POS practitioners to facilitate change and growth in the organizations they serve.

文摘In this paper, we study the Schrodinger equations (-△)^(s)u + V(x)u = a(x)|u|^(p-2)u + b(x)|u|^(q-2)u, x∈R^(N),where 0 < s < 1, 2 < q < p < 2_(s)^(*), 2_(s)^(*) is the fractional Sobolev critical exponent. Under suitable assumptions on V, a and b for which there may be no ground state solution, the existence of positive solutions are obtained via variational methods.

文摘As the essence of traditional Chinese educational thought, heuristic teaching has gradually enriched and devel-oped its ideas through the continuous efforts of educational researchers of the past dynasties. In front-line teach-ing,heuristic teaching,as a teaching principle that can help teachers and students to interact and learn, undoubtedly plays an important role in students’acquisition of knowledge and scientific thinking activities. But teachers’understanding of heuristic teaching is not the same.In actual teaching, there are obvious gaps in language ability among different teachers. This research aims to enrich the heuristic teaching theory based on the perspective of psychology, and provide a certain theoretical basis for teacher training.Through the research and analysis of the generation path of the heuristic teaching language, this paper helps more teachers realize the advantages of the heuristic teaching language, improve their own heuristic teaching language level, and provide reference for the scientific teaching evaluation standard. We apply the heuristic instruction language approach proposed in this study and apply it to real classrooms. In the application,it can be clearly found that the students’thinking and participation have been strengthened, and the teaching effect has been significantly improved com-pared with the past, which indicates that the heuristic teaching language generation path in this study has certain practical application value.
