Background: Purpose of this study was to evaluate various parameters of meningioma after surgical treatment and analyze predictive factors for recurrence. Methods: During 1992-2007, 353 patients were operated for intracranial meningioma in our department. They were followed since, and related data were combined with parameters such as tumor histology (WHO system), tumor location and the extent of tumor resection (Simpson’s scale). The results were analyzed with respect to tumor reappearance. This study was approved by the ethics committee of G. Papanikolaou Hospital. Results: The mean follow up period was 6.45 years. The overall percentage of recurrence was 21.52%. Grade 1 meningiomas (benign) recurred at a rate of 19.1%, grade 2 tumors (atypical) showed 41.7% rate of recurrence and grade 3 meningiomas (malignant) recurred at a rate of 75%. Tumor histopathology was not significant to recurrence (p > 0.001). The location of meningiomas was not found to be significant to recurrence (p > 0.001). Complete tumor resection was accomplished in 269 patients (76.2%). Incomplete resection (Simpson grades 2-5) took place in 84 cases (23.8%). The recurrence rate after complete resection was 13.8%, while the rate for cases with incomplete resection was 46.7%. The extent of removal was significantly associated with recurrence (p < 0.001). Conclusions: Tumor recurrence was observed at 21.5% of patients in our series of intracranial meningioma surgery. The rate of recurrence was related primarily to the extent of surgical removal. Neither tumor site, nor tumor histology were predictive factors for recurrence (ma-lignant meningiomas excluded). 展开更多

BACKGROUND Cold polypectomy(CP)is a simple and safe procedure for polyps less than 10 mm in size;however, there is concern about local recurrence following CP because of unidentified margins of excised specimens and the lack of tumor suppression effect by coagulation. Some clinical trials have evaluated local persistent recurrence;their results suggest that a higher rate of local recurrence has not been documented so far. There were few reports that observed the course over long periods of time after CP in clinical practice.AIM To evaluate the presence of local recurrence following CP and hot polypectomy(HP)using propensity score matching.METHODS We analyzed 275 patients who underwent polypectomy for non-pedunculated colorectal polyps less than 10 mm(959 Lesions)between October 2016 and 2017 and underwent follow-up endoscopy subsequently. We divided them into the CP group(706 Lesions),wherein CP was performed, and the HP group(253 Lesions),wherein HP was performed.Using propensity score matching,we extracted 215 Lesions in each group and evaluated the local recurrence and content of CP in the real clinic and adverse events using medical records.RESULTS After propensity score matching, there were no significant differences in the patients’and their endoscopic background(age, use of antithrombotics, indications, size, morphology, location of polyps, and polypectomy device)between the groups. The mean duration between colorectal polypectomy and the next follow-up colonoscopy was 17.5±7.1(range,6-39)mo in the CP group and 15.7±6.0(range,6-35)mo in the HP group, which was significantly longer in the CP group(P=0.005).The local recurrence rate was 0.93%in the CP group and 0.93%in the HP group, without a significant difference(P=0.688).Additionally,no differences were observed in the macroscopic en bloc resection rate, histopathological complete resection rate, and pathological results between the groups.Adverse events did not occur in either group.CONCLUSION Local recurrence after CP was equivalent to that following HP in clinical practice.CP is useful and safe in the treatment of non-pedunculated polyps of less than 10 mm. 展开更多

Metastasis-associated processes are the predominant instigator of fatalities linked to cancer, wherein the pivotal role of circulating tumor cells lies in the resurgence of malignant growth. In recent epochs, exosomes, constituents of the extracellular vesicle cohort, have garnered attention within the field of tumor theranostics owing to their inherent attributes encompassing biocompatibility, modifiability, payload capacity, stability, and therapeutic suitability.Nonetheless, the rudimentary functionalities and limited efficacy of unmodified exosomes curtail their prospective utility. In an effort to surmount these shortcomings, intricate methodologies amalgamating nanotechnology with genetic manipulation, chemotherapy, immunotherapy, and optical intervention present themselves as enhanced avenues to surveil and intercede in tumor metastasis and relapse. This review delves into the manifold techniques currently employed to engineer exosomes, with a specific focus on elucidating the interplay between exosomes and the metastatic cascade, alongside the implementation of tailored exosomes in abating tumor metastasis and recurrence. This review not only advances comprehension of the evolving landscape within this domain but also steers the trajectory of forthcoming investigations. 展开更多

Thermal ablation(TA),including radiofrequency ablation(RFA)and microwave ablation(MWA),has become the main treatment for early-stage hepatocellular carcinoma(HCC)due to advantages such as safety and minimal invasiveness. However,HCC is prone to local recurrence, with more aggressive malignancies after TA closely related to TA-induced changes in epithelial-mesenchymal transition(EMT)and remodeling of the tumor microenvironment(TME).According to many studies, various components of the TME undergo complex changes after TA,such as the recruitment of innate and adaptive immune cells, the release of tumor-associated antigens(TAAs)and various cytokines, the formation of a hypoxic microenvironment, and tumor angiogenesis.Changes in the TME after TA can partly enhance the anti-tumor immune response;however, this response is weak to kill the tumor completely.Certain components of the TME can induce an immunosuppressive microenvironment through complex interactions, leading to tumor recurrence and progression. How the TME is remodeled after TA and the mechanism by which the TME promotes HCC recurrence and progression are unclear. Thus,in this review,we focused on these issues to highlight potentially effective strategies for reducing and preventing the recurrence and progression of HCC after TA. 展开更多

BACKGROUND Endoscopic retrograde cholangiopancreatography(ERCP)is an accurate diagnostic method for choledocholithiasis and treatment option for stone removal.Additionally, ursodeoxycholic acid(UDCA)can dissolve cholesterol stones and prevent their development and reappearance by lowering the cholesterol concen-tration in bile.Despite these treatment options, there are still patients who experience stone recurrence. The clinical data of 100 patients with choledochal stones who were hospitalized at the Yixing People’s Hospital and underwent ERCP for successful stone extraction between June 2020 and December 2022 were retrospectively collected.According to the post-ERCP treatment plan,100 patients were classified into UDCA(n=47)and control(n=53)groups.We aimed to assess the clinical efficacy and rate of relapse in the two patient populations. We then collected information(basic demographic data, clinical characteristics, and serum biochemical indicators)and determined the factors contributing to relapse using logistic regression analysis. Our secondary goal was to determine the effects of UDCA on liver function after ERCP.Compared to the control group, the UDCA group demonstrated a higher clinical effectiveness rate of 92.45%vs 78.72%(P<0.05).No significant differences were observed in liver function indices, including total bilirubin, direct bilirubin,gamma-glutamyl transpeptidase, alanine aminotransferase, alkaline phosphatase, and aspartate aminotransferase, between the two groups before treatment. After treatment, all liver function indices were significantly reduced.Comparing the control vs UDCA groups, the UDCA group exhibited significantly lower levels of all indices(55.39±6.53 vs 77.31±8.52,32.10±4.62 vs 45.39±5.69,142.32±14.21 vs 189.63±16.87,112.52±14.25 vs 149.36±15.36,122.61±16.00 vs 171.33±22.09,96.98±10.44 vs 121.35±11.57,respectively,all P<0.05).The stone recurrence rate was lower in the UDCA group(13.21%)in contrast with the control group(44.68%).Periampullary diverticula(OR:6.00,95%CI:1.69-21.30),maximum stone diameter(OR:1.69,95%CI:1.01-2.85),stone quantity>3(OR:4.23,95%CI:1.17-15.26),and positive bile culture(OR:7.61,95%CI:2.07-27.91)were independent factors that influenced the relapse of common bile duct stones after ERCP(P<0.05).Furthermore,postoperative UDCA was identified as a preventive factor(OR:0.07;95%CI:0.08-0.09).CONCLUSION The intervention effect of UDCA after ERCP for common bile duct stones is adequate, providing new research directions and references for the prevention and treatment of stone recurrence. 展开更多

We show that the nonlinear stage of the dual-wavelength pumped modulation instability(MI)in nonlinear Schrödinger equation(NLSE)can be effectively analyzed by mode truncation methods. The resulting complicated heteroclinic structure of instability unveils all possible dynamic trajectories of nonlinear waves.Significantly, the latticed-Fermi-Pasta-Ulam recurrences on the modulated-wave background in NLSE are also investigated and their dynamic trajectories run along the Hamiltonian contours of the heteroclinic structure. It is demonstrated that there has much richer dynamic behavior,in contrast to the nonlinear waves reported before. This novel nonlinear wave promises to inject new vitality into the study of MI. 展开更多

Pulse rate is one of the important characteristics of traditional Chinese medicine pulse diagnosis, and it is of great significance for determining the nature of cold and heat in diseases. The prediction of pulse rate based on facial video is an exciting research field for getting palpation information by observation diagnosis. However, most studies focus on optimizing the algorithm based on a small sample of participants without systematically investigating multiple influencing factors.A total of 209 participants and 2,435 facial videos, based on our self-constructed Multi-Scene Sign Dataset and the public datasets, were used to perform a multi-level and multi-factor comprehensive comparison. The effects of different datasets, blood volume pulse signal extraction algorithms, region of interests, time windows, color spaces, pulse rate calculation methods, and video recording scenes were analyzed.Furthermore,we proposed a blood volume pulse signal quality optimization strategy based on the inverse Fourier transform and an improvement strategy for pulse rate estimation based on signal-to-noise ratio threshold sliding. We found that the effects of video estimation of pulse rate in the Multi-Scene Sign Dataset and Pulse Rate Detection Dataset were better than in other datasets.Compared with Fast independent component analysis and Single Channel algorithms,chrominance-based method and plane-orthogonal-to-skin algorithms have a more vital anti-interference ability and higher robustness. The performances of the five-organs fusion area and the full-face area were better than that of single sub-regions,and the fewer motion artifacts and better lighting can improve the precision of pulse rate estimation. 展开更多

In this paper,we study the one-dimensional motion of viscous gas near a vacuum, with the gas connecting to a vacuum state with a jump in density. The interface behavior, the pointwise decay rates of the density function and the expanding rates of the interface are obtained with the viscosity coefficientμ(ρ)=ρ^(α)for any 0<α<1;this includes the timeweighted boundedness from below and above. The smoothness of the solution is discussed. Moreover,we construct a class of self-similar classical solutions which exhibit some interesting properties, such as optimal estimates. The present paper extends the results in[Luo T,Xin Z P,Yang T.SIAM J Math Anal,2000,31(6):1175-1191]to the jump boundary conditions case with density-dependent viscosity. 展开更多

This study evaluated the genetic and agronomic parameter estimates of maize under different nitrogen rates. The trial was established at the Njala Agricultural Research Centre experimental site during 2021 and 2022 in a split block design with three maize varieties (IWCD2, 2009EVDT, and DMR-ESR-Yellow) and seven nitrogen (0, 30, 60, 90, 120, 150 and 180 kg∙N∙ha−1) rates. Findings showed that cob diameter and anthesis silking time (ASI) had intermediate heritability, ASI had high genetic advance, ASI and grain yield had high genotypic coefficient of variation (GCV), while traits with high phenotypic coefficient of variation (PCV) were plant height, ASI, grain yield, number of kernel per cob, number of kernel rows, ear length, and ear height. The PCV values were higher than GCV, indicating the influence of the environment in the studied traits. Nitrogen rates and variety significantly (p < 0.05) influenced grain yield production. Mean grain yields and economic parameter estimates increased with increasing nitrogen rates, with the 30 and 180 kg∙N∙ha−1 plots exhibiting the lowest and highest grain yields of 1238 kg∙ha−1 and 2098 kg∙ha−1, respectively. Variety and nitrogen effects on partial factor productivity (PFPN), agronomic efficiency (AEN), net returns (NR), value cost ratio (VCR) and marginal return (MR) indicated that these parameters were significantly affected (p < 0.05) by these factors. The highest PFPN (41.3 kg grain kg−1∙N) and AEN (29.4 kg grain kg−1∙N) were obtained in the 30 kg∙N∙ha−1 plots, while the highest VCR (2.8) and MR (SLL 1.8 SLL−1 spent on N) were obtained in the 180 kg∙N∙ha−1. The significant influence of variety and nitrogen on traits suggests that increasing yields and maximizing profits require use of appropriate nitrogen fertilization and improved farming practices that could be exploited for increased productivity of maize. 展开更多

The high rate of early recurrence in hepatocellular carcinoma(HCC)post curative surgical intervention poses a substantial clinical hurdle, impacting patient outcomes and complicating postoperative management. The advent of machine learning provides a unique opportunity to harness vast datasets, identifying subtle patterns and factors that elude conventional prognostic methods.Machine learning models, equipped with the ability to analyse intricate relationships within datasets, have shown promise in predicting outcomes in various medical disciplines. In the context of HCC,the application of machine learning to predict early recurrence holds potential for personalized postoperative care strategies. This editorial comments on the study carried out exploring the merits and efficacy of random survival forests(RSF)in identifying significant risk factors for recurrence, stratifying patients at low and high risk of HCC recurrence and comparing this to traditional COX proportional hazard models(CPH).In doing so,the study demonstrated that the RSF models are superior to traditional CPH models in predicting recurrence of HCC and represent a giant leap towards precision medicine. 展开更多

Quantum error correction is a crucial technology for realizing quantum computers. These computers achieve faulttolerant quantum computing by detecting and correcting errors using decoding algorithms.Quantum error correction using neural network-based machine learning methods is a promising approach that is adapted to physical systems without the need to build noise models. In this paper,we use a distributed decoding strategy, which effectively alleviates the problem of exponential growth of the training set required for neural networks as the code distance of quantum error-correcting codes increases. Our decoding algorithm is based on renormalization group decoding and recurrent neural network decoder. The recurrent neural network is trained through the ResNet architecture to improve its decoding accuracy. Then we test the decoding performance of our distributed strategy decoder, recurrent neural network decoder, and the classic minimum weight perfect matching(MWPM)decoder for rotated surface codes with different code distances under the circuit noise model, the thresholds of these three decoders are about 0.0052,0.0051,and 0.0049,respectively.Our results demonstrate that the distributed strategy decoder outperforms the other two decoders, achieving approximately a 5%improvement in decoding efficiency compared to the MWPM decoder and approximately a 2%improvement compared to the recurrent neural network decoder. 展开更多

This editorial comments on the article“Efficacy of multi-slice spiral computed tomography in evaluating gastric cancer recurrence after endoscopic submucosal dissection”.We focus on the importance of paying more attention to postendoscopic submucosal dissection(ESD)gastric cancer recurrence in patients with early gastric cancer(EGC)and how to manage it effectively.ESD has been a wellknown treatment and the mainstay for EGC,with the advantages of less invasion and fewer complications when compared with traditional surgical procedures.Despite a lower local recurrence rate after ESD,the problem of postoperative recurrence in patients with EGC has become increasingly non-ignorable with the global popularization of ESD technology and the increasing number of post-ESD patients. 展开更多

The Earth’s surface kinematics and deformation are fundamental to understanding crustal evolution. An effective research approach is to estimate regional motion field and deformation fields based on modern geodetic networks. If the discrete observed velocity field is obtained, the velocity related fields, such as dilatation rate and maximum shear strain rate, can be estimated by applying varied mathematical approaches. This study applied Akaike's Bayesian Information Criterion(ABIC)method to calculate strain rate fields constrained by GPS observations in the southeast Tibetan Plateau.Comparison with results derived from other three methods revealed that our ABIC-derived strain rate fields were more precise. The maximum shear strain rate highlighted the Xianshuihe–Xiaojiang fault system as the main boundary for the outward migration of material in southeastern Tibet, indicating rotation of eastern Tibet material around the eastern Himalaya rather than whole extrusion along a fixed channel.Additionally, distinct dilatation rate patterns in the northeast and southwest regions of the fault system were observed. The northeast region, represented by the Longmenshan area, exhibited negative dilatational anomalies;while the southwest region, represented by the Jinsha River area north of 29°N,displayed positive dilatational anomalies. This indicates compression in the former and extension in the latter.Combined with deep geophysical observations,we believe that the upper and lower crusts of the Jinsha River area north of 29°N are in an entire expanding state, probably caused by the escape-drag effect of material. The presence of a large,low-viscosity region south of 29°N may not enable the entire escape of the crust, but instead result in a differential escape of the lower crust faster than the upper crust. 展开更多

Background:Recurrent acute cholecystitis(RAC)can occur after non-surgical treatment for acute cholecystitis(AC),and can be more severe in comparison to the first episode of AC.Low skeletal muscle mass or adiposity have various effects in several diseases. We aimed to clarify the relationship between RAC and body parameters. Methods:Patients with AC who were treated at our hospital between January 2011 and March 2022 were enrolled. The psoas muscle mass and adipose tissue area at the third lumbar level were measured using computed tomography at the first episode of AC.The areas were divided by height to obtain the psoas muscle mass index(PMI)and subcutaneous/visceral adipose tissue index(SATI/VATI).According to median VATI,SATI and PMI values by sex, patients were divided into the high and low PMI groups. We performed propensity score matching to eliminate the baseline differences between the high PMI and low PMI groups and analyzed the cumulative incidence and predictors of RAC.Results:The entire cohort was divided into the high PMI(n=81)and low PMI(n=80)groups.In the propensity score-matched cohort there were 57 patients in each group. In Kaplan-Meier analysis, the low PMI group and the high VATI group had a significantly higher cumulative incidence of RAC than their counterparts(log-rank P=0.001 and 0.015,respectively).In a multivariate Cox regression analysis, the hazard ratios of low PMI and low VATI for RAC were 5.250(95%confidence interval 1.083-25.450,P=0.039)and 0.158(95%confidence interval:0.026-0.937,P=0.042),respectively.Conclusions:Low skeletal muscle mass and high visceral adiposity were independent risk factors for RAC. 展开更多

This paper examines the performance of Full-Duplex Cooperative Rate Splitting(FD-CRS)with Simultaneous Wireless Information and Power Transfer(SWIPT)support in Multiple Input Single Output(MISO)networks. In a Rate Splitting Multiple Access(RSMA)multicast system with two local users and one remote user, the common data stream contains the needs of all users, and all users can decode the common data stream. Therefore, each user can receive some information that other users need, and local users with better channel conditions can use this information to further enhance the reception reliability and data rate of users with poor channel quality.Even using Cell-Center-Users(CCUs)as a cooperative relay to assist the transmission of common data can improve the average system speed. To maximize the minimum achievable rate,we optimize the beamforming vector of Base Station(BS),the common streamsplitting vector, the cooperative distributed beamvector and the strong user transmission power under the power budget constraints of BS and relay devices and the service quality requirements constraints of users.Since the whole problem is not convex,we cannot solve it directly. Therefore,we propose a low complexity algorithm based on Successive Convex Approximation(SCA)technology to find the optimal solution to the problemunder consideration. The simulation results show that FD C-RSMA has better gain andmore powerful than FD C-NOMA,HD C-RSMA,RSMA and NOMA. 展开更多

Cyanea nozakii,a common jellyfish distributed in offshore China, has a complex trophic relationship with other zooplankton groups. However, few studies have reported the predation rates and prey selection patterns of C.nozakii medusae on different prey items.Research is also lacking on the intraguild predation of Aurelia coerulea(another common bloom jellyfish in offshore China)by C.nozakii.To address the knowledge gaps, the clearance rates of C.nozakii for different prey items, including copepods(small<1000μm and large>1000μm),fish larvae, and gelatinous prey(hydromedusae,A.coerulea ephyrae, and chaetognaths),were measured. The influence of predator size on the clearance rate was also determined.Additionally,we examined the intraguild predation of C.nozakii on A.coerulea medusae. The clearance rates of C.nozakii varied widely with prey organisms, being independent of prey concentrations.Gelatinous organisms, except for chaetognaths, were captured with considerably high efficiency, followed by fish larvae and copepods, indicating the preferential prey selection of gelatinous organisms by C.nozakii.The clearance rate increased linearly with the cross-sectional area of C.nozakii.Body size in medusae may,to some extents, underpin their capacity to capture more prey by increasing the encounter rate and capture success through ontogeny.C.nozakii preyed voraciously on A.coerulea in high feeding efficiency, but the clearance rate decreased with increasing A.coerulea(as prey)size. This phenomenon of intraguild predation suggests a speculative hypothesis of potential population regulation of A.coerulea by C.nozakii.The information regarding the feeding ecology of C.nozakii reported in this study is important for understanding plankton dynamics in marine ecosystems with extensive occurrences of this jellyfish. 展开更多

BACKGROUND Indentifying predictive factors for postoperative recurrence of hepatocellular carcinoma(HCC)has great significance for patient prognosis.AIM To explore the value of gadolinium ethoxybenzyl diethylenetriamine pentaacetic acid(Gd-EOB-DTPA)enhanced magnetic resonance imaging(MRI)combined with clinical features in predicting early recurrence of HCC after resection.METHODS A total of 161 patients with pathologically confirmed HCC were enrolled. The patients were divided into early recurrence and non-early recurrence group based on the follow-up results. The clinical, laboratory, pathological results and Gd-EOB-DTPA enhanced MRI imaging features were analyzed.RESULTS Of 161 patients,73 had early recurrence and 88 were had non-early recurrence.Univariate analysis showed that patient age, gender, serum alpha-fetoprotein level, the Barcelona Clinic Liver Cancer stage,China liver cancer(CNLC)stage, microvascular invasion(MVI),pathological satellite focus, tumor size, tumor number, tumor boundary, tumor capsule, intratumoral necrosis, portal vein tumor thrombus, large vessel invasion, nonperipheral washout, peritumoral enhancement, hepatobiliary phase(HBP)/tumor signal intensity(SI)/peritumoral SI,HBP peritumoral low signal and peritumoral delay enhancement were significantly associated with early recurrence of HCC after operation.Multivariate logistic regression analysis showed that patient age,MVI,CNLC stage, tumor boundary and large vessel invasion were independent predictive factors.External data validation indicated that the area under the curve of the combined predictors was 0.861,suggesting that multivariate logistic regression was a reasonable predictive model for early recurrence of HCC.CONCLUSION Gd-EOB-DTPA enhanced MRI combined with clinical features would help predicting the early recurrence of HCC after operation. 展开更多
