In this study,we established a rat model of T9-T10 spinal cord transection injury.We found that the expression of serum response factor in injured spinal cord gray matter neurons gradually increased with time,reached its peak on the 7^(th) day,and then gradually decreased.To investigate the role of serum response factor,we used lentivirus vecto rs to ove rexpress and silence serum response factor in spinal cord tissue.We found that overexpression of serum response factor promoted motor function recovery in rats with spinal cord injury.Qualitative observation of biotinylated dextran amine anterograde tra cing showed that ove rexpression of serum response factor increased nerve fibers in the injured spinal co rd.Additionally,transmission electron microscopy showed that axon and myelin sheath morphology was restored.Silencing serum response factor had the opposite effects of ove rexpression.These findings suggest that serum response factor plays a role in the recovery of motor function after spinal cord injury.The underlying mechanism may be related to the regulation of axonal regeneration. 展开更多

To investigate the relationship between transcription factor and the change of protein expression levels in heart failure. Methods Bioinformatic method was used to analyze the data of binding-sites on the 5 ' flaking regions of four genes whose mRNA level changed in failing heart from three databases about nucleic acid-EMBL, transcriptional regulation factor-TRANSFAC and protein-SWISS-PORT.The expression level of selected transcription factor was determined by immunohischemical method.Results Nine transcription factors were inferred to influence the proteins' levels in occurrence and development of heart failure.Serum response factor (SRF) was selected from the nine factors and assayed. The results showed that there was a higher level of SRF in healthy group than in chronic heart failure (CHF), and the level was associated with the degree of CHF. It was also found that there was a relative higher level of SRF in the acute myocardial infarction (AMI) than that in CHF, but which was lower than the healthy. Conclusion It showed that SRF had a quantitative change in the development of heart failure, and suggested SRF might play an important regulative role in heart failure. The expression changes of proteins related to myocardial function might be regulated by the quantitative change of transcription factor(s). 展开更多

Early diagnosis of MSA is very difficult, and diagnostic biomarkers are limited. Growth differentiation factor 15(GDF15) is involved in the differentiation and progression of the central nervous system, and is widely distributed in peripheral blood, which may be a novel biomarker for MSA.AIM To determine serum GDF15 levels, related factors and their potential diagnostic value in MSA patients, compared with Parkinson’s disease(PD) patients and healthy controls.METHODS A case-control study was conducted, including 49 MSA patients, 50 PD patients and 50 healthy controls. Serum GDF15 levels were measured by human enzymelinked immunosorbent assay, and the differences between the MSA, PD and control groups were analyzed. Further investigations were performed in different MSA subgroups according to age of onset, sex, clinical subtypes, diagnostic criteria, and disease duration. Receiver-operating characteristic curve analysiswas used to evaluate the diagnostic value of GDF15, especially for the differential diagnosis between MSA and PD.RESULTS Serum GDF15 levels were significantly higher in MSA patients than in PD patients and healthy controls(P = 0.000). Males and those with a disease duration of more than three years showed higher serum GDF15 levels(P = 0.043 and 0.000;respectively). Serum GDF15 levels may be a potential diagnostic biomarker for MSA patients compared with healthy controls and PD patients(cutoff: 470.42 pg/m L, sensitivity: 85.7%, specificity: 88.0%;cutoff: 1075.91 pg/m L, sensitivity:51.0%, specificity: 96.0%;respectively).CONCLUSION Serum GDF15 levels are significantly higher in MSA patients and provide suggestions on the etiology of MSA. 展开更多

AIM: To detect the impact of insulin-like growth factor-1(IGF-1) and other risk factors for the early prediction of retinopathy of prematurity(ROP) and to establish a scoring system for ROP prediction by using clinical criteria and serum IGF-1 levels.METHODS: The study was conducted with 127 preterm infants. IGF-1 levels in the 1 st day of life, 1 st, 2 nd, 3 rd and 4 th week of life was analyzed. The score was established after logistic regression analysis, considering the impact of each variable on the occurrences of any stage ROP. A validation cohort containing 107 preterm infants was included in the study and the predictive ability of ROP score was calculated.RESULTS: Birth weights(BW), gestational weeks(GW) and the prevalence of breast milk consumption were lower, respiratory distress syndrome(RDS), bronchopulmonarydysplasia(BPD) and necrotizing enterocolitis(NEC) were more frequent, the duration of mechanical ventilation and oxygen supplementation was longer in patients with ROP(P<0.05). Initial serum IGF-1 levels tended to be lower in newborns who developed ROP. Logistic regression analysis revealed that low BW(<1250 g), presence of intraventricular hemorrhage(IVH) and formula feeding increased the risk of ROP. Afterwards, the scoring system was validated on 107 infants. The negative predictive values of a score less than 4 were 84.3%, 74.7% and 79.8% while positive predictive values were 76.3%, 65.5% and 71.6% respectively.CONCLUSION: In addition to BW <1250 g and IVH, formula consumption was detected as a risk factor for the development of ROP. Breastfeeding is important for prevention of ROP in preterm infants. 展开更多

Objective:To investigate the effects of intravenous thrombolysis therapy with alteplase on neurological function,coagulation function and serum inflammatory factors in patients with acute cerebral infarction.Methods:A total of 96 patients with acute cerebral infarction admitted to our hospital from September 2017 to October 2019 were randomly divided into two groups,with 48 patients in each group.The control group(n=48)received routine treatment,and the observation group received intravenous thrombolysis therapy with alteplase on the basis of routine treatment.The neurological deficit score,prothrombin time(PT),activated partial thromboplastin time(APTT),tumor necrosis factor-a level(TNF-α),and high-sensitivity C-reactive protein(hs-CRP)were compared between the two groups after 15 days of treatment.Results:After treatment,NIHSS scores in both groups were lower than those before treatment;PT levels were increased,while APTT,TNF-αand hs-CRP levels were all decreased in both groups,and the changes in the observation group were greater than those in the control group,with statistically significant difference(P<0.05).Conclusions:Intravenous thrombolysis therapy with alteplase can improve the neurological function,coagulation function and serum levels of inflammatory factors in patients with acute cerebral infarction,which is worthy of clinical application. 展开更多

AIM: To investigate serum levels of soluble CD146(s CD146) and vascular endothelial growth factor receptor 2(VEGFR2) in patients with age-related macular degeneration(AMD). METHODS: Eighty-eight patients with exudative AMD and 45 sex-and age-matched healthy controls were enrolled in this study conducted in China. Serum samples was obtained from the patients with exudative AMD and from the controls. Serum sCD146 and VEGFR2 protein levels were measured using an enzyme-linked immunosorbent assay.RESULTS: We found that serum sCD146 and VEGFR2 protein levels were significantly higher in the patients with exudative AMD group than in the controls(t=3.859, P<0.001 and t=3.829, P<0.001, respectively). Serum sCD146 levels were significantly higher in patients with classic choroidal neovascularization(CNV) than in those with occult CNV(t=9.899, P<0.001). There was a significant difference in the trend for exudative AMD in the highest versus lowest quartile of circulating sCD146 levels(χ2=10.29, P=0.001). The receiver operating characteristic curve analysis showed that the area under the curve was 0.696 for s CD146(95%CI: 0.601-0.791) with an optimum diagnostic cut-off value of 157.16 ng/mL, a sensitivity of 55.7%, and a specificity of 82.2%.CONCLUSION: The serum sCD146 level increases and may be a biomarker for exudative AMD. 展开更多

Objective:To evaluate the diagnostic value of endostatin(ES),vascular endothelial growth factor (VEGF) and careinoembryonie antigen(CEA) in both serum and pleural effusion of lung cancer patients.Methods:Levels of ES,VEGF and CEA in 52 malignant pleural effusion due to lung cancer and 50 patients with non-malignant disease were measured by using sandwich enzymelinked immunosorbent assay and microparticle enzyme immunoassay.Results:The ES.VEGF and CEA levels in pleural effusion and serum,and their ratio(F/S) were higher in lung cancer group than that in benign group,and the differences were statistically significant(P【0.05).The diagnostic efficiency of ES+VEGF for lung cancer was superior to either single detection.The diagnostic efficiency of ES+VEGK+CEA was superior to either ES+VEGF or ES+CEA.Conclusions: The results suggest that ES,VEGF and CEA might be useful in the differentiation between benign and malignant pleural effusion due to lung cancer.In comparison with either single determination of concentration in serum or pleural fluid,the couiljined detection of two or three markers is of important clinical significance in the diagnosis of lung cancer. 展开更多

The relationships between serum pepsinogen (PG) levels and age, sex, ABO blood type, cigarette smoking and diet were studied among over 3, 000 residents selected at random in an area with high risk of stomach cancer in Shandong Province, China. Males had significantly higher median PG Ⅰ and Ⅱ levels than feamles. PG Ⅰ tended to decrease and PG Ⅱ to rise with age. Subjects with blood type A had a higher PG Ⅱ level than subjects with other blood types. Both PG Ⅰ and Ⅱ levels rose with dally consumption of cigarettes. Alcohol consumption was not related to PG levels. The PG Ⅰ/Ⅱ ratio declined with increasing consumption of sour pancakes, a fermented staple food found to contain N-nitroso compounds and to be a risk factor for stomach cancer in this population. 展开更多

Objectives Mu Lao Zu population.Methods A total of 1055 subjects of Mu Lao Zu were studied by a stratified randomized cluster sampling.Blood pressure,body height, body weight,waist circumference and serum lipid and apolipoprotein(Apo) levels were measured.The data were compared with those in 969 subjects of Han Chinese from the same region.In order to evaluate the association of hyperlipidemia with nine possible riks factors,multivariate logistic regression analysis was also performed in the combined population of Mu Lao Zu and Han,Mu Lao Zu,and Han;respectively. Results The levels of total cholesterol,triglyceride, low-density lipoprotein cholesterol and ApoB in Mu Lao Zu were significantly higher than those in Han(P【0.05-0.001); but the levels of high-density lipoprotein cholesterol,ApoA1 and ApoA1/Apo were significantly lower than those in Han (P【0.001 ).The prevalence of hyperlipidemia and Hypercholesterolemia was also significantly higher in Mu Lao Zu than in Han(53.0%vs.46.1%,P【0.001) and(44.5%vs. 37.5%,P【0.001).There was no difference in the prevalence of hypertriglyceridemia between the both ethnic groups (22.8%vs.19.4%,P】0.05).Hyperlipidemia was positively correlated with age,BMI,and systolic blood pressureMu Lao Zu(P【0.05),whereas it was positively correlated with BMI and hypertension in Han(P【0.05 for each).Conclusions The serum lipid profiles,the prevalence of hyperlipidemia and the risk factors are different between the Mu Lao Zu and Han populations.The differences in serum lipid profiles between the two ethnic groups might result from different dietary habit,life style,as well as genetic factors,in ? AIM: To investigate the effect of serum derived from rats treated with electroacupuncture at stomach meridian acupoints on the expression of epidermal growth factor receptor (EGFR) gene in gastric mucosal cells. METHODS: The stress-induced gastric mucosal injury in rat model was established by water-immersion and restrained stress methods. 52 rats were randomly divided into: normal group (n = 8), model group (n = 8), model serum group (n = 12), stomach serum group (n = 12), and gallbladder serum group (n = 12). The gastric mucosal cells were separated by pronase-EDTA digestion method and incubated with serum. The EGFR gene expression in gastric mucosal cells was detected by reverse transcription-polymerase chain reaction (RT-PCR) method. RESULTS: Compared with normal group (0.6860 ± 0.0594), the serum derived from rats of the stomach group (1.2272 ± 0.0813, P = 0.00 < 0.01) and gallbladder group (0.9640 ± 0.0387, P = 0.00 < 0.01) had a tendency to enhance the EGFR gene expression in gastric mucosal cells. Such tendency existed in the model group (0.7104 ± 0.0457) but with no signifi cant difference (P = 0.495 > 0.05) and in model serum group (0.8516 ± 0.0409) with an extremely obvious difference (P = 0.001 < 0.01). Furthermore, the EGFR gene expression in stomach serum group was significantly higher than that in gallbladder serum group (P = 0.00 < 0.01). CONCLUSION: The present study shows that serumderived from rats treated with electroacupuncture at stomach meridian acupoints can distinctly increase the EGFR gene expression of gastric mucosal cells. Therefore, there is certain meridian specificity in the serum, which could provide a proof for the TCM theory “particular relation between meridian and internal organ”. 展开更多

Objective:To investigate the relationship between polymorphism within the 5'-untranslated region(5'-UTR)of IGF-I gene and its periparturient concentration in Iranian Holstein dairy cows.Methods:Blood samples(5 mL,n=37)were collected by caudal venipuncture from each animal into sample lubes containing the EDTA and DNA was extracted from blood.In order to measure ICF-I concentration the collection of blood samples(n=111)was also done at 14 d before calving(prepartum),25 and 45 d postpartum.Results:We found evidence for a significant effect of C to T mutation in position 512 of IGF-I gene on its serum concentration in dairy cows in Iran.Cows with CC genotype had significantly higher concentration(Mean-SD)of IGF-I at 14 d prepartum(91.8±18.1)μg/L compared to those with TT genotype(73.3±14.4)μg/L(P=0.04).A significant trend(quadratic)was found for IGF-I concentration,as higher in CC cows compared to ones with TT genotype,during the 14 d before calving to 45 d postpartum(P=0.01).Conclusions:We concluded that C/T transition in the promoter region of IGF-I gene can influence the serum concentration of ICF-I in periparturient dairy cows. 展开更多

目的:观察血清胸腺因子对大鼠妊娠晚期、分娩期、哺乳期及胚胎和胎仔出生后生长发育、学习能力以及生殖能力的影响。方法:选择交配后的雌性SD大鼠,每组20只,从妊娠第15天至哺乳期结束,分别按0.88、1.75和3.50 mg/kg连续皮下注射给予血... 目的:观察血清胸腺因子对大鼠妊娠晚期、分娩期、哺乳期及胚胎和胎仔出生后生长发育、学习能力以及生殖能力的影响。方法:选择交配后的雌性SD大鼠,每组20只,从妊娠第15天至哺乳期结束,分别按0.88、1.75和3.50 mg/kg连续皮下注射给予血清胸腺因子,同时设0.9%氯化钠注射液为溶剂对照组,定期称取F0代雌鼠体质量、摄食量;F1代仔鼠确定出生存活率、哺乳期存活率,生理发育、新生反射达标天龄,训练学习、记忆能力,观察出生第12周后的仔鼠交配后的生殖行为,测定胚胎毒性各项指标。结果:注射血清胸腺因子后,溶剂对照组和受试药物各剂量组F0代妊娠鼠和哺乳鼠各周的平均体质量增长和摄食量正常,仔鼠总量、性别比和出生存活率(96.3%~97.9%)和哺乳期存活率(97.7%~98.9%)基本一致;F1代仔鼠的哺乳期和离乳后仔鼠平均体质量增长和摄食量基本正常。溶剂对照组仔鼠的生理发育和新生反射时间点检查项目均为正常达标天龄,游泳适应时间(28.5±15.8)s,空间定位建立时间(134.9±44.1)s,暗室电击反射建立时间(4.9±3.1)s,建立记忆电击次数1.7±0.9;F1代性成熟大鼠溶剂对照组的同笼交配率100%,妊娠率90%,平均着床数10.6±2.1,平均活胎数10.2±1.9,死胎数3,畸形胎数1;以上指标受试药物各剂量组间未见明显变化,与溶剂对照组比较,差异均无统计学意义(P>0.05)。结论:在本实验条件下,溶剂对照组与血清胸腺因子3个剂量组的大鼠妊娠后期个体及F1代胚胎及胎仔出生后生长发育、神经行为以及生殖能力等均未发现明显异常,未见围产期毒性反应。 展开更多

外周血Lp-PLA2和FGF23水平变化与脑梗死后认知功能障碍的相关性

目的分析外周血脂蛋白相关磷脂酶A2(Lp-PLA2)、成纤维细胞生长因子23(FGF23)水平变化与脑梗死后患者认知功能障碍的相关性。方法选取2019-04—2022-12邯郸市中心医院收治的160例脑梗死患者为研究对象,根据患者是否发生认知功能障碍分为... 目的分析外周血脂蛋白相关磷脂酶A2(Lp-PLA2)、成纤维细胞生长因子23(FGF23)水平变化与脑梗死后患者认知功能障碍的相关性。方法选取2019-04—2022-12邯郸市中心医院收治的160例脑梗死患者为研究对象,根据患者是否发生认知功能障碍分为认知障碍组和非认知障碍组,对比2组基线资料及外周血Lp-PLA2、FGF23水平,并采用Logistic回归分析患者发生认知功能障碍的影响因素,采用Pearson相关性分析外周血Lp-PLA2、FGF23与简易智力状态评价量表(MMSE)评分的关系,采用ROC曲线评估外周血Lp-PLA2、FGF23对脑梗死后患者认知功能障碍的预测价值。结果160例脑梗死患者中,48例(30.00%)发生认知功能障碍。认知障碍组患者的平均年龄、高血压、糖尿病、吸烟、文化程度、MMSE评分及血清Lp-PLA2、FGF23水平等方面与非认知障碍组相比,差异有统计学意义(P<0.05)。Logistic回归分析显示,年龄、高血压、糖尿病、吸烟、文化程度低及血清Lp-PLA2、FGF23水平升高是影响脑梗死后认知功能障碍发生的独立危险因素(P<0.05)。Pearson相关性分析显示,脑梗死患者血清Lp-PLA2、FGF23水平与MMSE评分呈负相关(P<0.05)。ROC曲线显示,Lp-PLA2的曲线下面积为0.770,FGF23的曲线下面积为0.779,联合检测的曲线下面积为0.873(P<0.05),表示两者联合检测可作为评价脑梗死后认知功能障碍的有效指标。结论Lp-PLA2、FGF23在脑梗死后认知功能障碍患者血清中均呈高表达,二者联合检测有助于提高对脑梗死后认知功能障碍的预测价值。 展开更多

目的 探究老年急性脑梗死(ACI)患者血清内皮发育调节基因-1(Del-1)、白细胞介素-17(IL-17)水平与脑梗死面积、病情严重程度及预后的关系。方法 回顾性收集2019-01-2021-12厦门大学附属东南医院收治的126例老年ACI患者(病例组)及105名健... 目的 探究老年急性脑梗死(ACI)患者血清内皮发育调节基因-1(Del-1)、白细胞介素-17(IL-17)水平与脑梗死面积、病情严重程度及预后的关系。方法 回顾性收集2019-01-2021-12厦门大学附属东南医院收治的126例老年ACI患者(病例组)及105名健康体检者(对照组)的临床资料,根据脑梗死面积将病例组患者分为大面积梗死组(梗死最大直径>5 cm,n=14)、中面积梗死组(梗死最大直径3~5 cm,n=53)和小面积梗死组(梗死最大直径<3 cm,n=59);根据NIHSS评分分为重症组(NIHSS评分≥16分,n=13)、中症组(5分<NIHSS评分<15分,n=58)和轻症组(NIHSS评分≤5分,n=55);根据m RS评分分为预后良好组(mRS评分≤2分,n=81)和预后不良组(mRS评分>2分,n=45)。采用荧光免疫法检测血清Del-1、IL-17水平,受试者工作特征(ROC)曲线评估血清Del-1、IL-17水平预测老年ACI患者预后的价值。结果 病例组吸烟史比例、收缩压、舒张压、糖化血红蛋白、血清IL-17水平均高于对照组,血清Del-1水平低于对照组(P<0.05)。不同脑梗死面积患者血清Del-1水平比较,小面积梗死组>中面积梗死组>大面积梗死组;血清IL-17水平比较,小面积梗死组<中面积梗死组<大面积梗死组(P<0.05)。轻症组血清Del-1水平高于中症组、重症组(P<0.05),不同病情严重程度患者血清IL-17水平比较,重症组>中症组>轻症组(P<0.05)。预后良好组患者血清Del-1水平高于预后不良组,血清IL-17水平低于预后不良组(P<0.05)。ROC曲线分析显示,血清Del-1、IL-17预测ACI患者预后的AUC值分别为0.763、0.747(P<0.05)。结论 老年ACI患者血清Del-1水平较低,IL-17水平较高,二者与患者脑梗死面积、病情严重程度和预后关系密切,可作为预测老年ACI患者预后的可靠指标。 展开更多

阿司匹林联合氯吡格雷治疗脑梗死的效果分析

目的探究对于脑梗死患者采用阿司匹林联合氯吡格雷的临床效果。方法选取2021年7月—2022年12月在靖江市人民医院接受治疗的76例脑梗死患者为研究对象,并将其按照随机数表法分为观察组以及参照组,每组38例,参照组给予阿司匹林治疗,观察... 目的探究对于脑梗死患者采用阿司匹林联合氯吡格雷的临床效果。方法选取2021年7月—2022年12月在靖江市人民医院接受治疗的76例脑梗死患者为研究对象,并将其按照随机数表法分为观察组以及参照组,每组38例,参照组给予阿司匹林治疗,观察组在阿司匹林治疗基础上联合氯吡格雷治疗,比较两组的血清炎性因子、血液流动学指标和氧化酶活性指标情况。结果两组患者经过治疗之后,症状均有所改善,其中观察组C-反应蛋白、白介素-6、白介素-8水平分别为(4.53±0.97)mg/L、(5.22±1.98)pg/mL、(60.28±2.78)ng/mL,均明显优于参照组,差异有统计学意义(t=15.180、7.789、22.697,P均<0.05);同时血浆黏度、血小板聚集率、全血黏度分别为(1.12±0.02)mPa·s、(58.42±5.13)%、(3.01±0.56)mPa·s,均明显优于参照组,差异有统计学意义(t=22.695、7.239、4.484,P均<0.05);除此之外,观察组的超氧化物歧化酶、丙二醛、氧化氢酶指标分别为(74.56±2.95)kU/L、(3.24±1.11)nmol/mL、(26.28±5.71)IU/mL,均优于参照组,差异有统计学意义(t=18.287、3.131、5.746,P均<0.05)。结论阿司匹林联合氯吡格雷治疗脑梗死,能够显著改善患者血液流变学指标,降低血清炎性因子水平,促进氧化酶活性指标恢复到正常值,具有显著的临床疗效。 展开更多

目的观察苏筋通络方结合“行气活血”针刺法治疗急性踝关节扭伤临床效果。方法将医院创伤骨科门急诊就诊的102例急性踝关节扭伤患者(2020年5月—2022年7月)纳入研究,以随机数字表法将患者分为两组,每组51例,对照组采取西医常规治疗,观... 目的观察苏筋通络方结合“行气活血”针刺法治疗急性踝关节扭伤临床效果。方法将医院创伤骨科门急诊就诊的102例急性踝关节扭伤患者(2020年5月—2022年7月)纳入研究,以随机数字表法将患者分为两组,每组51例,对照组采取西医常规治疗,观察组采取西医常规治疗同时结合苏筋通络方及“行气活血”针刺法治疗。数据比较:临床治疗效果、中医症状(踝关节疼痛肿胀、皮下瘀斑、关节活动受限等)积分变化、踝关节跖屈活动度与背伸活动度变化、视觉模拟评分(Visual Analogue Scale,VAS)变化、美国足踝外科协会(American Orthopaedic Foot and Ankle Society,AOFAS)踝-后足评分变化、血清疼痛物质(神经生长因子、神经肽、P物质)水平变化、白细胞介素-6(Interleukin-6,IL-6)、白细胞介素-1β(Interleukin-1β,IL-1β)、超敏C反应蛋白(Hypersensitive C-reactive protein,hs-CRP)变化。结果观察组患者与对照组患者治疗总有效率相比[98.04%(50/51)vs86.27%(44/51)],P<0.05;治疗后各组患者中医症状(踝关节疼痛肿胀、皮下瘀斑、关节活动受限等)积分、踝关节跖屈活动度与背伸活动度、VAS评分、AOFAS踝-后足评分、神经生长因子、神经肽、P物质、IL-6、IL-1β、hs-CRP等指标均改善,观察组患者治疗后中医症状(踝关节疼痛肿胀、皮下瘀斑、关节活动受限等)积分、踝关节跖屈活动度与背伸活动度、VAS评分、AOFAS踝-后足评分、神经生长因子、神经肽、P物质、IL-6、IL-1β、hs-CRP等指标均明显优于对照组患者,P均<0.05。结论苏筋通络方结合“行气活血”针刺法治疗急性踝关节扭伤临床效果显著,患者经治疗后临床症状显著改善,疼痛缓解,踝关节功能改善,值得应用。 展开更多

血清PLGF/sFlt-1联合胎盘3D-PDI对子痫前期的预测价值

目的探讨血清胎盘生长因子(PLGF)/可溶性血管内皮生长因子受体-1(sFlt-1)联合胎盘三维能量多普勒指数(3D-PDI)对子痫前期(PE)的预测价值。方法选取2021年1月至2022年12月于我院就诊且有PE高危因素的孕妇120例,随访至分娩后1周。于孕14~2... 目的探讨血清胎盘生长因子(PLGF)/可溶性血管内皮生长因子受体-1(sFlt-1)联合胎盘三维能量多普勒指数(3D-PDI)对子痫前期(PE)的预测价值。方法选取2021年1月至2022年12月于我院就诊且有PE高危因素的孕妇120例,随访至分娩后1周。于孕14~20周常规产检时检测血清PLGF和sFlt-1水平,并计算PLGF/sFlt-1比值,同时超声检测胎盘3D-PDI,包括血管化指数(VI)、血流指数(FI)、血管化-血流指数(VFI)。根据孕20周后是否发生PE,分为PE组(55例)和对照组(65例),根据病情轻重将PE组进一步分为非重度PE组(35例)和重度PE组(20例)。比较各组PLGF/sFlt-1及3D-PDI,并分析PLGF、sFlt-1与3D-PDI的相关性,绘制受试者操作特征(ROC)曲线,分析各指标单独或联合预测PE的价值。结果2组孕妇年龄、检测孕周、妊娠高血压史及生育史等均无差异(P>0.05)。PE组收缩压(SBP)、舒张压(DBP)、24 h尿蛋白水平、早产率、新生儿重症监护室入住率及孕前体质量指数均高于对照组(P<0.05),新生儿体质量低于对照组(P<0.05)。PE组血清PLGF/sFlt-1、3D-PDI均低于对照组,重度PE组血清PLGF/sFlt-1、3D-PDI均低于非重度PE组(均P<0.05)。PLGF与VFI呈显著正相关(P<0.01),sFlt-1与VFI呈显著负相关(P<0.01)。ROC曲线分析显示,PLGF/sFlt-1、VI、FI、VFI对PE均有预测价值,VI、FI、VFI联合预测PE的价值高于各参数指标[曲线下面积(AUC)=0.951],血清PLGF/sFlt-1、VI、FI、VFI四者联合预测价值最高(AUC=0.987)。结论PE患者孕早期血清PLGF、sFlt-1与胎盘VFI均具有显著相关性,且孕早期血清PLGF/sFlt-1及胎盘VI、FI、VFI均减低,4种指标联合应用预测PE的效能最高,可为临床早期筛查或预测PE提供参考。 展开更多

解剖性肝切除术后腹腔感染的影响因素及早期血清指标预测价值

目的 分析解剖性肝切除术后腹腔感染的影响因素及早期血清指标的预测价值。方法 解剖性肝切除术患者100例,依据术后是否发生腹腔感染分为感染组(n=25)、非感染组(n=75),分析腹腔感染病原学特点、对比感染组与非感染组性别、年龄、术前... 目的 分析解剖性肝切除术后腹腔感染的影响因素及早期血清指标的预测价值。方法 解剖性肝切除术患者100例,依据术后是否发生腹腔感染分为感染组(n=25)、非感染组(n=75),分析腹腔感染病原学特点、对比感染组与非感染组性别、年龄、术前白蛋白(Alb)、手术相关指标等临床资料;分析影响腹腔感染的因素,比较感染组与非感染组的预后转归情况及肝功能[总胆红素(TBIL)、直接胆红素(DBIL)、丙氨酸氨基转移酶(ALT)、门冬氨酸氨基转移酶(AST)、凝血酶原时间(PT)]变化;检测两组早期感染性血清指标[C反应蛋白(CRP)、降钙素原(PCT)、白介素-6(IL-6)及肿瘤坏死因子-α(TNF-α)],并分析术后第3天内平均CRP、PCT、IL-6、TNF-α对腹腔感染的预测价值。结果 25例出现腹腔感染(25.00%),共检出病原菌42株,革兰阴性菌20株,革兰阳性菌22株;年龄、合并糖尿病、手术时间、术中胶体使用量、行胆管切开引流或胆肠吻合为术后腹腔感染独立危险因素,血清Alb为保护因素(P<0.05);感染组住院时间较非感染组延长,感染组住院费用较非感染组更高(P<0.05),感染组术前DBIL、ALT及术后第1天DBIL高于非感染组(P<0.05),两组28 d死亡率比较,差异无统计学意义(P>0.05);感染组术前、术后第1天、第3天CRP、PCT、IL-6、TNF-α均高于非感染组(P<0.05);术后第3天内平均CRP、PCT、IL-6、TNF-α预测腹腔感染的ROC曲线下面积(AUC)分别为0.821、0.817、0.813、0.772。结论 年龄、合并糖尿病、手术时间等是解剖性肝切除术后腹腔感染的影响因素,检测CRP、PCT、IL-6、TNF-α对早期腹腔感染有一定的预测价值。 展开更多
