Objective:This study assessed the role of the attending medical team in the cost control of the cholecystectomy DRG components.Methods:The association between team structure,workflow,and treatment outcomes was analyzed using a mixed-methods approach combining quantitative data and qualitative interviews from 628 patients.Results:Inter-professional teamwork significantly affected length of stay,treatment costs,and recurrence rates,with experienced teams performing better in terms of emergency response and collaborative efficiency.Patient satisfaction was generally high,indicating that good teamwork enhances treatment outcomes.Significance:The study highlights the importance of optimizing team configuration to improve the quality,efficiency,and cost control of healthcare. 展开更多

With the advancement of technology, companies are increasingly dependent on technology teams to stay competitive. However, members of these teams often work in stressful and unhealthy environments, which can undermine their productivity and well-being. The humanization of the culture of technology teams is an approach that aims to create healthier and more productive work environments for team members, promoting balance between personal and professional life. Despite the importance of a healthy and productive work environment, many companies do not invest in strategies to humanize the culture of their technology teams. This can lead to high levels of stress, staff turnover and low productivity. The objective of this project is to identify effective strategies to humanize the culture of technology teams and create healthier and more productive work environments in digital companies. For this, factors such as management styles, psychological safety, human-centered development, individual beliefs and time and energy management will be analyzed. The project’s methodology will include a literature review on the subject and qualitative data analysis will be performed using a content analysis approach. This project will contribute to the advancement of knowledge about the humanization of the culture of technology teams in digital companies. The results can be applied to companies that want to create healthier and more productive work environments for their team members. 展开更多

The current advancement in cloud computing,Artificial Intelligence(AI),and the Internet of Things(IoT)transformed the traditional healthcare system into smart healthcare.Healthcare services could be enhanced by incorporating key techniques like AI and IoT.The convergence of AI and IoT provides distinct opportunities in the medical field.Fall is regarded as a primary cause of death or post-traumatic complication for the ageing population.Therefore,earlier detection of older person falls in smart homes is required to improve the survival rate of an individual or provide the necessary support.Lately,the emergence of IoT,AI,smartphones,wearables,and so on making it possible to design fall detection(FD)systems for smart home care.This article introduces a new Teamwork Optimization with Deep Learning based Fall Detection for IoT Enabled Smart Healthcare Systems(TWODLFDSHS).The TWODL-FDSHS technique’s goal is to detect fall events for a smart healthcare system.Initially,the presented TWODL-FDSHS technique exploits IoT devices for the data collection process.Next,the TWODLFDSHS technique applies the TWO with Capsule Network(CapsNet)model for feature extraction.At last,a deep random vector functional link network(DRVFLN)with an Adam optimizer is exploited for fall event detection.A wide range of simulations took place to exhibit the enhanced performance of the presentedTWODL-FDSHS technique.The experimental outcomes stated the enhancements of the TWODL-FDSHS method over other models with increased accuracy of 98.30%on the URFD dataset. 展开更多

Football is one of the most-watched sports,but analyzing players’per-formance is currently difficult and labor intensive.Performance analysis is done manually,which means that someone must watch video recordings and then log each player’s performance.This includes the number of passes and shots taken by each player,the location of the action,and whether or not the play had a successful outcome.Due to the time-consuming nature of manual analyses,interest in automatic analysis tools is high despite the many interdependent phases involved,such as pitch segmentation,player and ball detection,assigning players to their teams,identifying individual players,activity recognition,etc.This paper proposes a system for developing an automatic video analysis tool for sports.The proposed system is the first to integrate multiple phases,such as segmenting the field,detecting the players and the ball,assigning players to their teams,and iden-tifying players’jersey numbers.In team assignment,this research employed unsu-pervised learning based on convolutional autoencoders(CAEs)to learn discriminative latent representations and minimize the latent embedding distance between the players on the same team while simultaneously maximizing the dis-tance between those on opposing teams.This paper also created a highly accurate approach for the real-time detection of the ball.Furthermore,it also addressed the lack of jersey number datasets by creating a new dataset with more than 6,500 images for numbers ranging from 0 to 99.Since achieving a high perfor-mance in deep learning requires a large training set,and the collected dataset was not enough,this research utilized transfer learning(TL)to first pretrain the jersey number detection model on another large dataset and then fine-tune it on the target dataset to increase the accuracy.To test the proposed system,this paper presents a comprehensive evaluation of its individual stages as well as of the sys-tem as a whole. 展开更多

Clinical application of multidisciplinary team-and evidence-based practice project in gynecological patients with perioperative hypothermia

BACKGROUND Perioperative hypothermia(PH)negatively affects the physical and mental health of patients to varying degrees.Currently,there is no effective multidisciplinary team(MDT)intervention for gynecological patients with PH.AIM To apply the best evidence on the prevention and management of PH in gynecological patients,improve the quality of perioperative evidence-based care based on treatment by an MDT for gynecological patients and analyze the effect of MDT-and evidence-based practice(EBP)projects on the psychological status and cognitive function of gynecological patients with PH.METHODS Under the guidance of knowledge translation and combined with the opinions of involved stakeholders and clinical experts,the best evidence for PH prevention and management in gynecological patients was selected and adjusted to suit the practice setting.Based on the evidence,the practice plan was developed,and the MDT intervention was carried out in the preoperative ward,the preoperative preparation room,the intraoperative operating room,the postanesthesia care unit,and the 24-hour postoperative gynecological ward through the EBP program.The incidence of hypothermia,the nurses’awareness,the implementation rate of examination indicators,and the thermal comfort level,psychological status and cognitive function of patients were compared before and after the implementation of the program.RESULTS The incidence of PH in gynecological patients decreased from 43.33%to 13.33%after the implementation of the scheme.The implementation rate of examination indicators 6-10,12,14,16-18,21,and 22 reached 100%,and that of other indicators was above 90%,except for examination indicators 5 and 13,which was 66.67%;the indices were significantly improved compared with the baseline(before evidence application)(P<0.05).The score of nurses'awareness of PH prevention and management in gynecological patients increased from 60.96±9.70 to 88.08±8.96,and the difference was statistically significant(P<0.001).The total score of the perioperative thermal comfort level of patients undergoing gynecological surgery was 27.97±2.04,which was significantly increased compared with the score of 21.27±1.57 observed by researchers at baseline(P<0.001).The perioperative Hamilton Depression Scale and Hamilton Anxiety Scale scores of patients undergoing gynecological surgery decreased from 15.03±3.16 and 13.93±2.64 to 4.30±1.15 and 3.53±0.78,respectively,with statistically significant differences(P<0.001).The perioperative Montreal Cognitive Assessment Scale score of the gynecological surgery patients increased from 23.17±1.68 to 26.93±1.11,also with statistical significance(P<0.001).CONCLUSION MDT-based EBP for PH prevention and management in gynecological patients during the perioperative period can standardize nursing operations,improve nurses'awareness and behavioral compliance with gynecological hypothermia management,and reduce the occurrence of PH in gynecological patients while playing a positive role in reducing patients’negative emotions and enhancing their cognitive function. 展开更多

Entry-Level Forward Surgical Team Training Is Associated with Increased Confidence of Primary Combat Surgeons

Background: In recent years, we have established an entry-level Forward Surgical Team (FST) training program in a Chinese military medical university for the 5th grade undergraduates, who would be deployed to different military medical services as primary combat surgeons. This study aimed to assess the role of this pre-service training in improving their confidence with combat medical skills, after several years since they received the training. Methods: We conducted a nationwide survey of 239 primary combat surgeons who have ever participated in an entry-level FST training program before deployment between June 2016 and June 2020, which was for evaluating on a 5-point Likert scale the benefits of entry-level FST training and conventional surgery training in improving their confidence with combat medical skills. The difference in scores was compared using the student t-test. Significance was considered as P Results: The total score was significantly higher for entry-level FST training than that for conventional surgery training (30.76 ± 4.33 vs. 28.95 ± 4.80, P There was no significant difference between the training for surgical skills confidence scores (18.03 ± 8.04 vs. 17.51 ± 8.30, P = 0.098), but for non-technical skills, the score of entry-level FST training was significantly higher than that of conventional surgery training (12.73 ± 5.39 vs. 11.44 ± 5.62, P The distributions of confidence scores were different under various subgroups by demographics. There were no significant differences in scores between the two training in all specific surgical skill sets except “life-saving surgery” (P = 0.011). Background:Teamwork is essential to provide the highest quality of care for patients.Feeling supported within a nursing unit is a significant factor in nursing satisfaction,intention to remain in the job,and the capacity to provide safe patient care by Aiken et al[1].Purpose:This study examined mutual support among a nursing team to measure the influence of an educational intervention focusing on Mutual Support from the Team STEPPS curriculum by Agency for Healthcare Research and Quality,AHRQ[2].Methods:The study design used a Likert scale survey,the Nursing Teamwork Survey,before and following an education intervention adapted from the Team STEPPS curriculum on Mutual Support.Demographic data from the 41 participants were analyzed for impact on educational background,roles and responsibilities,age,and other factors.Results:Pre-Post education intervention results varied among the survey items,although scores demonstrated heightened awareness of teamwork following the educational intervention.The subscale of Backup illustrated the strongest improvement.Conclusion:The study demonstrates that education can have an impact on perceptions and awareness of mutual support among nursing team members.The survey instrument can be used effectively to inform leadership areas for improvement and staff development in the effort to improve team coordination and mutual support. 展开更多

目的:评价基于提高医疗质量和患者安全的团队策略和工具包(Team Strategies and Tools to Enhance Performance and Patient Safety,Team STEPPS)构建的"ICU-病房"肺部延伸护理模式的应用效果。方法:回顾性选取2016年1-12月... 目的:评价基于提高医疗质量和患者安全的团队策略和工具包(Team Strategies and Tools to Enhance Performance and Patient Safety,Team STEPPS)构建的"ICU-病房"肺部延伸护理模式的应用效果。方法:回顾性选取2016年1-12月符合纳入标准的术后患者为对照组(98例),给予常规护理。纳入2017年1-12月符合纳入标准的术后患者为实验组(135例),运用基于Team STEPPS模型构建的"ICU-病房"肺部延伸护理模式。结果:实验组患者非计划重返ICU率、因肺部原因重返ICU率均低于对照组,患者对护理工作满意度评分高于对照组,差异均有统计学意义(P<0.05)。结论:运用基于Team STEPPS模型构建的"ICU-病房"肺部延伸护理模式可降低患者重返ICU率,提高患者满意度。 展开更多
