T...Colorectal cancer ranks third among the estimatedcancer cases and cancer related mortalities in United States in 2014. Early detection and efficient therapy remains a significant clinical challenge for this disease. Therefore, there is a need to identify novel tumor asso-ciated molecules to target for biomarker development and immunotherapy. In this regard, cancer testis antigens have emerged as a potential targets for developing novel clinical biomarkers and immunotherapy for various malignancies. These germ cell specific proteins exhibit aberrant expression in cancer cells and contribute in tumorigenesis. Owing to their unique expression profile and immunogenicity in cancer patients, cancer testis antigens are clinically referred as the most promising tumor associated antigens. Several cancer testis antigens have been studied in colorectal cancer but none of them could be used in clinical practice. This review is an attempt to address the promising cancer testis antigens in colorectal cancer and their possible clinical implications as biomarkers and immunotherapeutic targets with particular focus on challenges and future interventions.展开更多

Due to the lack of characteristic pathological features, it is often underdiagnosed and misdiagnosed. A variety of methods can be used to diagnose NMC, including immunohistochemistry, karyotyping, fluorescence in situ hybridization, reverse transcription-polymerase chain reaction, and next-generation sequencing. So far, there is no standard treatment plan for NMC and the prognosis is poor, related to its rapid progression, easy recurrence, and unsatisfactory treatment outcome.CASE SUMMARY A 58-year-old female came to our hospital with a complaint of eye swelling and pain for 8 d. The diagnosis of NMC was confirmed after postoperative pathology and genetic testing. The patient developed nausea and vomiting, headache, and loss of vision in both eyes to blindness after surgery. Magnetic resonance imaging(MRI) and positron emission tomography/computed tomography(PET/CT) performed after 1.5 mo postoperatively suggested tumor recurrence. The patient obtained remission after radiation therapy to some extent and after initial treatment with anti-angiogenic drugs and sonodynamic therapy(SDT), but cannot achieve long-term stability and eventually developed distant metastases, with an overall survival of only 17 mo.CONCLUSION For patients with rapidly progressing sinus tumors and poor response to initial treatment, the possibility of NMC should be considered and immunohistochemical staining with anti-NUT should be performed as soon as possible, combined with genetic testing if necessary. CT, MRI, and PET/CT imaging are essential for the staging, management, treatment response assessment and monitoring of NMC. This case is the first attempt to apply heat therapy and SDT in the treatment of NMC, unfortunately, the prognosis remained poor.展开更多

We report the demographic and clinical characteristics and the long term results in patients with epidermoid cysts. Materials and Methods: Thirteen patients with epidermoid cyst of the testis treated between June 2000 and January 2018 were retrospectively evaluated. Ten patients with available data were included in the study. Patients’ age, scrotal localization of symptoms, physical examination findings, serum tumor marker levels, pathology results and follow-up periods were recorded. Results: The current cohort consisted of 10 patients. The mean age at diagnosis was 29.7 years (range, 20 to 53 years). Eighty percent of the lesions were localized to the left hemiscrotum and 20% to the right hemiscrotum. One patient had a history of unilateral undescended testis. Serum tumor markers were normal in all patients. The mean scrotal mass lesion volume at diagnosis was 28 mm3 (range, 13 to 51 mm3). The mean follow-up period was 66.9 months (range, 12 to 216 months). Conclusions: Epidermoid cyst of the testis should be kept in mind, particularly in patients with lesions negative for tumor markers and radiological findings typical of epidermoid cysts. Annual follow-up is not required.展开更多

It is foundation of spermatogenesis in the testis throughout adult life by balancing self-renewal and differentiation. SSCs isolation from mammalian testis is difficult because of their scarcity and the lack of well characterized cell surface markers. Thus, the isolation of SSCs is of great interest for exploration of spermatogonial physiology and therapeutic approaches for fertility preservation. CD9 is a surface marker expressed in mouse and rat male germline stem cells. In this study, CD9 positive SSCs were successfully isolated from the goat testis using enzymatic digestion followed by three step purification: Differential plating, Percoll discontinuous density gradient followed by Magnetic activated cell sorting (MACS). Percoll discontinuous density gradient showed significant differences in the percentage of CD9+ SSCs across individual fraction. The fraction 36% and 40% gave the highest percentage of CD9+ SSCs i.e. 82% ± 1.2 and 9.2% ± 1.3 respectively. Magnetic activated cell sorting of CD9+ cells in the magnetic fraction of goat testes was in the range of 15% - 18% which is upto threefolds. CD9+ SSCs were further recovered with appreciable efficiency after immunomagnetic isolation by using various bead: cells ratio in which 4:1 ratio gave the highest yield of 69.06 × 105 with 18% of CD9+ SSCs. Magnetic activated cell sorting using anti-CD9 antibodies provides an efficient and fast approach as compared to conventional approaches such as differential plating and percoll discontinuous density gradient for enrichment strategy for spermatogonial stem cells from goat testes for undertaking research on basic and applied reproductive biology.展开更多

Male mice were orally administered with the two doses 0.1 and 1 mg/kg b.w /d. of aroclor 1254 for 7, 14, 21 and 28 days. Results:Results showed the degenerative changes in the testis of mice, namely, atrophied seminiferous tubules, expanded space in interstitial and necrosis in the germinal epithelial cells of seminiferous tubules, of the seminiferous tubules and deceleration of spermatogenesis. Conclusions: Therefore, the results of the present study suggested that the sub-acute exposure of very low doses of aroclor 1254, can subsequently mediate the cytoskeleton dysfunction in the testis of mice.展开更多

The results indicate that the fine structure of the two kinds of cells in different development stages presents an obvious difference. The smooth endoplasmic reticular and tubular mitochondria of Leydig cell and Sertoli cell are well developed in the testis of gonadal precocity, but poorly developed in the testis of immaturation. We suggest that the reason for gonadal precocity in the large yellow croaker may be related to the earlier development and maturation of Leydig cell and Sertoli cell.展开更多

文摘The small yellow croaker Larimichthys polyactis is an economically important marine fish in Northeast Asia.Currently, its natural resources are threatened by overfishing and environmental pollution. Therefore, research on the reproductive system of the fish is crucial. Here,we studied the testis development and ultrastructural features of spermatogenesis in cultured L.polyactis using anatomical, histological, and ultrastructural techniques.A pair of testes, consisting of a central sperm duct and radial seminiferous lobules, were observed. The reproduction cycle of testes can be divided into stages I–VI.March to May was confirmed as the breeding season for male L.polyactis, while April is the ideal period for artificial breeding. The male L.polyactis can attain sexual maturity within 1 year. The spermatogenesis of L.polyactis comprised spermatogonium, spermatocyte, spermatid, and mature spermatozoon. The morphology of spermatogenic cells changed obviously during spermiogenesis, including nuclear shaping, midpiece and flagellum formation. The mature sperms consist of an ellipsoidal head,a short midpiece, and a long flagellum. The anterior of the head with a kidney-shaped nucleus can be distinguished. The midpiece is located posterior to the head and includes four to six spherical mitochondria. The flagellum has irregular lateral fins. The testis of L.polyactis is an unrestricted lobular type, with cystic spermatogenesis, type II spermiogenesis, and type II spermatozoa. These features are highly similar to those of other Sciaenid species. Our findings provide useful insights into the mechanism underlying testis development and spermatogenesis of L.polyactis, which can facilitate the artificial breeding of this species.
文摘Objective:To evaluate the management and outcomes of patients who presented with torsion of an undescended testis and review the reported series in the literature. Methods:The case records of 13 patients operated for testicular torsion involving undescended testis were retrospectively reviewed. The medical records included age at presentation, medical history, physical examination, operative findings and the results of follow-up.The diagnosis of torsion of undescended testis was made clinically and confirmed by inguinal exploration. Results:In six cases the testis was preserved and orchiopexy was performed, while in seven cases orchidectomy was performed due to testicular gangrene in six patients and testicular tumor discovered peroperatively in one case.Mean duration of symptoms at time of surgery in the orchiopexy group was 6.5 h and in the orchidectomy group was 21.2 h.From six patients treated by orchiopexy, two patients suffered from testicular atrophy at a mean of 24 months. Conclusion:Testicular torsion in undescended testis is still diagnosed with delay which may affect testicular salvage. The importance of examination of external genital organs is highlighted which should be routinely included by emergency physicians in physical examination for abdominal or groin pain.

文摘BACKGROUND As one of the major microvascular complications of diabetes, diabetic retinopathy(DR)is the leading cause of blindness in the working age population.Because the extremely complex pathogenesis of DR has not been fully clarified, the occurrence and development of DR is closely related to tissue ischemia and hypoxia and neovascularization The formation of retinal neovascularization(RNV)has great harm to the visual acuity of patients.AIM To investigate the expression of P-element-induced wimpy testis-interacting RNA(piRNA)in proliferative DR mice and select piRNA related to RNV.METHODS One hundred healthy C57BL/6J mice were randomly divided into a normal group as control group(CG)and proliferative DR(PDR)group as experimental group(EG),with 50 mice in each group.Samples were collected from both groups at the same time, and the lesions of mice were evaluated by hematoxylin and eosin staining and retinal blood vessel staining. The retinal tissues were collected for second-generation high-throughput sequencing, and the differentially expressed piRNA between the CG and EG was detected, and polymerase chain reaction(PCR)was conducted for verification. The differentially obtained piRNA target genes and expression profiles were enrichment analysis based on gene annotation(Gene Ontology)and Kyoto Encyclopedia of Genes and Genomes.RESULTS In the CG there was no perfusion area, neovascularization and endothelial nucleus broke through the inner boundary membrane of retinap. In the EG,there were a lot of nonperfused areas, new blood vessels and endothelial nuclei breaking through the inner boundary membrane of the retina. There was a statistically significant difference in the number of vascular endothelial nuclei breaking through the inner retinal membrane between the two groups.High-throughput sequencing analysis showed that compared with the CG,a total of 79 piRNAs were differentially expressed in EG,among which 43 piRNAs were up-regulated and 36 piRNAs were down-regulated.Bioinformatics analysis showed that the differentially expressed piRNAs were mainly concentrated in the signaling pathways of angiogenesis and cell proliferation.Ten piRNAs were selected for PCR,and the results showed that the expression of piR-MMU-40373735,piRMMU-61121420,piR-MMU-55687822,piR-MMU-1373887 were high, and the expression of piR-MMU-7401535,piR-MMU-4773779,piR-MMU-1304999,and piR-MMU-5160126 were low, which were consistent with the sequencing results.CONCLUSION In the EG,the abnormal expression of piRNA is involved in the pathway of angiogenesis and cell proliferation, suggesting that piRNAs have some regulatory function in proliferative diabetic-retinopathy.

文摘Colorectal cancer ranks third among the estimatedcancer cases and cancer related mortalities in United States in 2014. This review is an attempt to address the promising cancer testis antigens in colorectal cancer and their possible clinical implications as biomarkers and immunotherapeutic targets with particular focus on challenges and future interventions.

文摘BACKGROUND The ABCD stratification[combination of serum pepsinogen(PG)levels and titers of antibody(immunoglobulin G,IgG)against Helicobacter pylori(H.pylori)]is effective for the classification of individuals at risk of developing gastric cancer(GC).The Kita–Kyushu lung cancer antigen-1(KK-LC-1)is a Cancer/Testis antigen frequently expressed in GC.AIM To evaluate the effectiveness of KK-LC-1 and ABCD stratification in the diagnosis of GC.METHODS We analyzed the gene expression of KK-LC-1 in surgical specimens obtained from GC tumors. The levels of serum PG I/PG II and IgG against H.pylori were measured.According to their serological status, the patients were classified into the four groups of the ABCD stratification.RESULTS Of the 77 examined patients,63(81.8%)expressed KK-LC-1.The IgG titers of H.pylori and PG II were significantly higher in patients expressing KK-LC-1 than those measured in patients not expressing KK-LC-1(P=0.0289 and P=0.0041,respectively).The expression of KK-LC-1 in group C[PG method(+)/H.pylori infection(+)]was as high as 93.9%high.KK-LC-1 was also detected in group A[-/-].CONCLUSION The KK-LC-1 expression in GC was associated with H.pylori infection and atrophic status,so that,KK-LC-1 may be a useful marker for the diagnosis of GC.
文摘Testicular torsion(TT) is a medical emergency that primary affects newborns and young adolescents. It causes testicular injury due to the torsion of the spermatic cord and its components, initially in the venous blood flow and finally in the arterial blood flow.Prompt diagnosis and early surgical management are necessary in managing this urgent situation. The process of the pathophysiological events in ischemia-reperfusion is multifactorial and deals with the perception of the oxidative stress responsible for the consequences of ischemia/reperfusion(I/R) stress following TT.Duration and severity of torsion also play a significant role in the oxidative stress.A detrimental result of the defense system of the testes takes place resulting finally in testicular atrophy and impaired function.Antioxidant factors have been experimentally studied in an effort to front this state. They have been classified as endogenous or exogenous antioxidants.Endogenous antioxidants comprise a structure of enzymic enzymatic and nonenzymic enzymatic particles presented within cytoplasm and numerous other subunits in the cells.Exogenous antioxidants include a variety of natural and pharmaceutical agents that may prevent or ameliorate the harmful effects of I/R injury. In this study we review those factors and their ability to enhance the oxidative status of the testis.A feature insight into where we are heading is attempted.

文摘Summary:Cancer testis(CT)antigens have received particular attention in cancer immunotherapy.OY-TES-1 is a member of CT antigens. This study was to evaluate OY-TES-1 expression and immunogenicity in hepatocelluar carcinoma(HCC).OY-TES-1 mRNA expression was detected in 56 HCC tissues and 5 normal liver tissues by reverse transcriptase PCR(RT-PCR).Of the 56 cases of HCC tissues tested,37 cases had tumor and matched adjacent non-cancer tissues and were subjected to both RT-PCR and quantitative real-time PCR.OY-TES-1 protein was subsequently observed on a panel of tissue microarrays.Sera from patients were tested for OY-TES-1 antibody by ELISA.To identify OY-TES-1 capable of inducing cellular immune response,OY-TES-1 protein was used to sensitize dentritic cells and the cytotoxicity effect was measured in vitro. The results showed that OY-TES-1 mRNA was highly expressed in 41 of the 56(73.21%)HCC tissues, whereas none in 5 normal liver tissues.OY-TES-1 mRNA was frequently expressed not only in HCC tissues(72.97%,27/37),but also in paired adjacent non-cancer tissues(64.86%,24/37).But the mean expression level of OY-TES-1 mRNA in HCC tissues was significantly higher than that in adjacent non-cancer tissues(0.76854 vs.0.09834,P=0.021).Immunohistochemistry showed that OY-TES-1 protein expression was detected in 6 of the 49 cases of HCC tissues, and absent in 9 cases of normal liver and 6 cases of cirrhosis tissues.Seropositivity was detected in 10 of the 45 HCC patients, but not detected in 17 cirrhosis patients and 76 healthy donors. The specific cytotoxic T cells elicited by OY-TES-1 could kill HLA-A2^+HCC cell line which expressed OY-TES-1.The target lysis was mainly HLA class I-dependent and could be blocked by antibodies against monomorphic HLA class I but not HLA class II molecule. In summary,OY-TES-1 expression is upregulated in HCC tissues and can be recognized by humoral and cellular responses, which suggests that OY-TES-1 is an attractive target for tumor immunotherapy in HCC.
文摘The objective was to evaluate the toxicity effect of gossypol on ultrastructure of mouse testis and the expression of Bax mRNA and Bcl-2 mRNA of sperm cells in mice.Forty-eight male mice were randomly divided into four groups:control group,L-group(30 mg·kg^(-1)·d),M-group(60 mg·kg^(-1)·d)and H-group(120 mg·kg^(-1)·d)and were orally administrated with gossypol diluted by sodium carboxymethyl cellulose(SCC)or SCC(control group)for 20 days.On the 21st day,all the mice were killed and ultrastructure changes of testis were observed by TEM.mRNA expression of Bax and Bcl-2 in testis was measured by semiquantitative RT-PCR.The results showed that the testicular ultrastructure in three treated groups was gradually damaged, according to the dosage of gossypol and cellular structure disordered and organelle degenerated, manifesting vacuolation of mitochondria, expansion of endoplasmic reticulum.mRNA expression of Bcl-2 in testis significantly increased(p<0.05)in L-group and then significantly decreased(p<0.05,p<0.01)in M-group and H-group compared with that in the control group;mRNA expression of Bcl-2 in M-group and H-group significantly decreased(p<0.05,p<0.01)than that in L-group and Bcl-2 mRNA expression in H-group showed a significant decrease(p<0.05)compared with that in M-group.On the other hand,mRNA expression of Bax significant increased(p<0.05,p<0.01)in M-group and H-group than that in the control group. The ratio of Bcl-2/Bax significantly reduced(p<0.05,p<0.01)in the treated group than that in the control group and was found to be an obvious dose-dependent.It demonstrated that the gossypol could induce the changes on ultrastructure of mice testis,down-regulate mRNA expression of Bcl-2 and up-regulate mRNA expression of Bax,which indicated that sperm cells were induced apoptosis.

文摘BACKGROUND Nuclear protein in testis(NUT) carcinoma is a rare aggressive malignant epithelial cell tumor, previously known as NUT midline carcinoma(NMC), characterized by an acquired rearrangement of the gene encoding NUT on chromosome 15q14. This case is the first attempt to apply heat therapy and SDT in the treatment of NMC, unfortunately, the prognosis remained poor.
文摘BACKGROUND Pleomorphic rhabdomyosarcoma(RMS)of the spermatic cord is a group of rare neoplasms, and a secondary hydrocele testis occasionally occurs. The misdiagnosis of paratesticular mass may lead to a therapeutic delay.CASE SUMMARY A 79-year-old man presented to our clinic complaining of a 1-mo history of painless scrotal swelling.Physical examination revealed approximately a 15 cm×10 cm×5 cm inguinal mass with limited mobility.Contrast-enhanced magnetic resonance imaging showed a hydrocele testis, several enlarged inguinal lymph nodes, and a heterogeneously enhanced lesion with a relatively well-defined margin in the left inguinal region.Due to the imaging findings,he was diagnosed with pleomorphic RMS and received a wide resection of the mass,an inguinal incision with a high section of the left spermatic cord, and a left radical orchiectomy. He experienced local relapse 1 mo postoperatively and received radiotherapy and anlotinib hydrochloride-based immunotherapy as adjuvant therapy. The patient died 3 mo after the surgery.CONCLUSION The optimal interventions for advanced-stage pleomorphic RMS patients should be investigated by more preclinical studies and clinical trials.Physicians need to be aware of the occurrence of pleomorphic RMS in unusual locations, especially when accompanied by a hydrocele testis.
文摘Background: In this study, we aimed to share our experiences in patients with epidermoid cyst of the testis. Annual follow-up is not required.
文摘The well documented source for adult multipotent stem cells is spermatogonial stem cells (SSCs) of mammalian testis. Magnetic activated cell sorting using anti-CD9 antibodies provides an efficient and fast approach as compared to conventional approaches such as differential plating and percoll discontinuous density gradient for enrichment strategy for spermatogonial stem cells from goat testes for undertaking research on basic and applied reproductive biology.
文摘Objective:To evaluate the testis and the cauda epididymis after treating both healthy and diabetic rats with rooibos(Aspalathus linearis),honeybush(Cyclopia intermedia),and sutherlandia(Lessertia frutescens),respectively. Methods:Ninety male Wistar rats were randomly divided into nine groups(n=10 per group).Fifty rats were randomized to receive tap water,0.1 M citrate buffer,2.0%fermented rooibos,4.0%fermented honeybush,or 0.2%unfermented sutherlandia.Diabetes was induced via a single intraperitoneal injection of streptozotocin(STZ)at 45 mg/kg in 40 rats. The diabetic rats were randomized to receive tap water,2.0%fermented rooibos,4.0%fermented honeybush,or 0.2%unfermented sutherlandia.Rats were sacrificed after seven weeks of diabetic induction.Blood was collected for hormonal assay, while the testes and caudal epididymis were retrieved for histomorphological evaluations. Results:Diabetic rats treated with 2.0%fermented rooibos exhibited a significant increase in fasting plasma glucose(FPG)versus the vehicle group(P=0.002),and a 3.6%decrease compared to the diabetic control group(P>0.05).Diabetic rats treated with honeybush or sutherlandia showed 23.6%and 15.4%decrease in FPG when compared to the diabetic control group, respectively(both P>0.05).The diabetic control rats had a significant decrease in the percentage of histologically normal seminiferous tubules compared to the control group(P=0.049).There was 21.0%,14.0%,and 5.4%rise in the percentage of normal seminiferous tubules in diabetic rats receiving rooibos, honeybush or sutherlandia, respectively, compared to the diabetic control group. The infusion control groups(rooibos, honeybush and sutherlandia)showed normal seminiferous tubules, presence of spermatozoa in the epididymal lumen, and had normal overall architecture. Both testicular and epididymal morphology were altered in the diabetic control group, but these disruptions were mildly ameliorated by rooibos, honeybush, and sutherlandia. Conclusions:The detrimental effects of diabetes on the histomorphological architecture of the testis and epididymis are mildly ameliorated by the infusions(2.0%fermented rooibos,4.0%fermented honeybush, and 0.2%unfermented sutherlandia).
文摘MicroRNAs(miRNAs)are small noncoding RNAs involved in the regulation of mRNA transcription and translation, and possess all desirable features of an ideal tumor marker.Of almost 31 different miRNA clusters identified in germ cell tumors(GCTs),miR-371a-3p has shown exceptionally high sensitivity and specificity for both seminomatous and nonseminomatous GCTs.It is easily obtainable and correlates well with tumor burden.Recent multiinstitutional prospective studies have shown promising test characteristics for miR-371a-3p as a diagnostic blood-based biomarker for GCT prior to orchiectomy including 80%e100%sensitivity and 90%e100%specificity.This accuracy may address other unmet needs in the management of patients with GCT.Early studies have suggested the utility of miR-371a-3p in detecting occult nodal metastasis in high-risk clinical stage I and early stage II disease.Ongoing clinical trials including SWOG 1823 and AGCT1531 are specifically designed to confirm the utility of miR-371a-3p in clinical stage I GCT.Despite its strong association with viable GCT after treatment with chemotherapy,miR-371a-3p does not seem to accurately predict the presence of teratoma in residual lesions. Also, standardization of extraction and interpretation methods is a necessary step to assure uniform results across different institutions.
文摘Objective:To evaluate the dose and duration dependent cytotoxicity of aroclor 1254 on mice testis. Methods: The study tests the hypothesis that in vivo exposure of very low dose aroclor 1254, comparable to that of possible human exposure from different environmental sources, will provoke dose and duration dependent histological damage in the mice testis. Conclusions: Therefore, the results of the present study suggested that the sub-acute exposure of very low doses of aroclor 1254, can subsequently mediate the cytoskeleton dysfunction in the testis of mice.

文摘Microstructure and submicrostructure of Leydig cell and Sertoli cell in the testis of gonadal precocity and immaturation in cultured large yellow croaker, Pseudosciaena crocea, are studied using histology and electron microscopic technique. We suggest that the reason for gonadal precocity in the large yellow croaker may be related to the earlier development and maturation of Leydig cell and Sertoli cell.

文摘BACKGROUND Although gastric cancer is one of the most prevalent cancers worldwide, cases of gastric cancer metastasis to the male reproductive system are rare. Here,we report a case involving testicular and epididymal gastric cancer metastases.CASE SUMMARY A 75-year-old Chinese man complained of experiencing a palpable painful mass in the right scrotum for 6 mo.He had undergone distal gastrectomy with chemotherapy for pT3N3aMx poorly differentiated gastric adenocarcinoma 9 mo before.Physical examination revealed a moderate right hydrocele and a painful mass in the right testis and epididymis.Serum tumor biomarkers were all normal except for elevated beta-human chorionic gonadotropin levels.Computed tomography urography and B-ultrasound imaging revealed a moderate right hydrocele and a mixed solid-cystic mass in the right testicular and epididymal area. Thus, the patient underwent right radical orchiectomy.Immunohistochemical analysis revealed that the tumor cells were positive for pancytokeratins and caudal related homeodomain transcription 2.Metastatic,poorly differentiated gastric adenocarcinoma of the testis and epididymis was confirmed by pathology. He continued to undergo chemotherapy at the department of oncology of our hospital.Mesenteric lymph node metastases were found at the postoperative 1-mo follow-up.CONCLUSION Palpable, painful scrotal mass, history of gastric cancer, and imaging features may indicate testicular and epididymal metastatic gastric cancer.

文摘BACKGROUND Metastases from pancreas or ampullary malignancies are common, but the spread to testicle and paratesticular tissue is exceedingly rare.To the best of our knowledge, fewer than 30 cases have been reported in the literature.More rarely, metastasis to tunica vaginalis testis occurs without involvement of the testes and epididymis.CASE SUMMARY A 65-year-old male who complained of painless swelling of the left scrotum for over 1 wk was referred to the Department of Urology.Scrotal ultrasound showed left testicular hydrocele with paratesticular masses.Chest computed tomography revealed lung metastasis and enlarged left supraclavicular lymph node. The blood tumor markersalpha-fetoprotein,human chorionic gonadotropin, and serum lactate dehydrogenase were withinnormal limits. The preoperative diagnosis was left testicular tumor with lung metastasis. Then radical orchidectomy of the left testicle and high ligation of the spermatic cord were performed, and postoperative histopathology suggested metastatic tumors that was confirmed by an abdominal computed tomographic scan. The positive computed tomography findings,in conjunction with the expression of cytokeratin 7(CK7),CK20,CK5/6,and absence of expression of Wilms’tumor suppressor gene 1,calretinin,melanocyte,prostate-specific antigen, thyroid transcription factor-1,GATA binding protein 3,caudal type homeobox 2,and napsinA supported the diagnosis of pancreatic adenocarcinoma. The outcome of this patient was unsatisfactory, and he died 3 mo later.CONCLUSION This case suggests that pancreatic metastatic carcinoma must be considered in the differential diagnosis of scrotal enlargement. The advanced age of the patient wassuggestive of a secondary testicular tumor. In addition, careful physical examination and ultrasonography as well as radiological examination have become a standard modality.
文摘We describe a 16-year-old male with ultrasound evidence of a 1.3 cm right paratesticular nodule, which was managed by intraoperative frozen section and excisional biopsy. The pathologic findings were consistent with benign fibrous pseudotumor of the tunica vaginalis testis, which is a very rare lesion in the pediatric population.Consideration of fibrous pseudotumor in the differential diagnosis of pediatric paratesticular masses may help prevent unnecessarily aggressive therapy.
