Alzheimer’s disease is a common neurodegenerative disorder defined by decreased reasoning abilities,memory loss,and cognitive deterioration.The presence of the blood-brain barrier presents a major obstacle to the dev...Alzheimer’s disease is a common neurodegenerative disorder defined by decreased reasoning abilities,memory loss,and cognitive deterioration.The presence of the blood-brain barrier presents a major obstacle to the development of effective drug therapies for Alzheimer’s disease.The use of ultrasound as a novel physical modulation approach has garnered widespread attention in recent years.As a safe and feasible therapeutic and drug-delivery method,ultrasound has shown promise in improving cognitive deficits.This article provides a summary of the application of ultrasound technology for treating Alzheimer’s disease over the past 5 years,including standalone ultrasound treatment,ultrasound combined with microbubbles or drug therapy,and magnetic resonance imaging-guided focused ultrasound therapy.Emphasis is placed on the benefits of introducing these treatment methods and their potential mechanisms.We found that several ultrasound methods can open the blood-brain barrier and effectively alleviate amyloid-βplaque deposition.We believe that ultrasound is an effective therapy for Alzheimer’s disease,and this review provides a theoretical basis for future ultrasound treatment methods.展开更多

BACKGROUND Congenital cytomegalovirus(CMV)infection represents a significant public health concern as the most prevalent viral infection in newborns,potentially leading to severe neurological and developmental complic...BACKGROUND Congenital cytomegalovirus(CMV)infection represents a significant public health concern as the most prevalent viral infection in newborns,potentially leading to severe neurological and developmental complications.The majority of cases are asymptomatic and remain undetected during pregnancy due to the absence of effective screening methods.CASE SUMMARY A 27-year-old primigravida presented for early pregnancy ultrasound,which revealed an atypical finding:A normal anechoic thalamus appearing hyperechoic on the mid-sagittal view of the fetal head.Subsequent ultrasound examinations during mid and late gestation demonstrated classic intracranial features sug-gestive of congenital CMV infection.Chromosomal karyotyping and microarray analysis of the fetus yielded no significant abnormalities.Following compre-hensive prenatal counseling regarding potential adverse fetal outcomes,the patient elected to continue her pregnancy.She ultimately underwent cesarean delivery at 42 weeks gestation at another facility,resulting in the birth of a female neonate.At five months of age,the infant presented with epilepsy and significant growth and developmental delays.CONCLUSION Congenital CMV infection occurs during the first trimester may manifest as hyperechoic thalamus which can be revealed by ultrasound in the mid-saggital view of the fetal head.Future research should investigate the correlation between echogenic thalamus and developmental outcomes,as well as explore early sc-reening techniques for suspected congenital CMV infection cases.展开更多

Previous studies have shown that undergoing such prenatal screening procedures could reduce the antenatal anxiety levels of expectant mothers. The present study aimed to explore the impact of first-trimester ultrasound scanning towards the antenatal anxiety and identify the predictors of antenatal anxiety among expectant mothers in the first trimester. Methods: A repeated measure design study was conducted in Maternity Clinics of University Hospital KDU, Ninewells Care Hospital and Navy General Hospital over 4 months with one hundred and fifteen (n = 115) expectant mothers. Participants completed a general information sheet first and State Trait Anxiety Inventory (STAI) (Spielberger et al., 1970) was administered before and after undergoing the USS. Results: Mean age of the participants was 28.84 ± 3.68. The Wilcoxon Signed Rank test showed that there is a significant reduction of participants’ antenatal anxiety levels following the USS z = −5.658, p Conclusions: Findings suggest that undergoing the first trimester USS significantly reduces the antenatal state anxiety and partner’s support is an important factor in reducing the antenatal anxiety experienced by expectant mothers in the first trimester. Future studies can focus on how USS can contribute to alleviating antenatal anxiety in second and third trimesters.展开更多

Activities such as dexterous sports, prolonged use of personal handheld devices, and extensive desktop keyboard usage are common contributors to wrist pain. Intersection syndrome, a form of inflammatory tenosynovitis, occurs at the intersection of the first and second dorsal compartments of the wrist. The first dorsal compartment is comprised of the tendons of abductor pollicis longus and extensor pollicis brevis, while the second dorsal compartment contains the tendons of extensor carpi radialis longus and extensor carpi radialis brevis. Intersection syndrome is diagnosed by pain localized to the dorsoradial forearm, approximately five cm proximal to the wrist joint, which worsens with resisted wrist and thumb extension. To date, the use of hydro dissection with 5% dextrose under ultrasound guidance as a treatment for Intersection syndrome has not been reported. This case report presents the first report on ultrasound-guided hydro dissection as a therapeutic approach for intersection syndrome. Methods: A case report, with informed consent, involving a 32-year-old male athlete. The patient, a hurling player, presented with chronic right wrist pain diagnosed as intersection syndrome. The condition significantly affected his work, sporting activities, and daily living activities. Previous conservative management and physiotherapy had failed to alleviate his symptoms. To confirm the diagnosis, relevant imaging was performed, supplemented by dynamic ultrasound assessment. The procedure was performed aseptically. Continuous ultrasound guidance was employed to ensure accurate needle placement. Once the needle tip position was confirmed, an initial injection of 5 mL of 0.25% chirocaine was administered. 10 mL of 5% dextrose was injected under ultrasound guidance for hydro dissection, with good visualization of the solution’s distribution. Conclusion: Ultrasound-guided hydro dissection has not previously been documented as a treatment option for intersection syndrome. In this case, it proved to be an effective pain-relieving therapy with sustained effect at three-month clinical follow-up. Further studies are required.展开更多

BACKGROUND Hypertension(HTN)is a prevalent chronic health condition that significantly increases the risk of cardiovascular diseases-associated mortalities.Despite the use of antihypertensive medications,numerous pati...BACKGROUND Hypertension(HTN)is a prevalent chronic health condition that significantly increases the risk of cardiovascular diseases-associated mortalities.Despite the use of antihypertensive medications,numerous patients fail to achieve guidelinerecommended blood pressure(BP)targets.AIM To evaluates the efficacy of catheter-based ultrasound renal denervation(uRDN)for the treatment of HTN.METHODS Relevant studies were identified through searches in PubMed,Embase,the Cochrane Library,Web of Science,and China National Knowledge Infrastructure,with a cut-off date at April 1,2024.A random-effects model was employed in this study to mitigate potential biases.The risk of bias for included studies was assessed using the Cochrane Risk of Bias assessment tool.Statistical analyses were conducted using Review Manager version 5.3.This meta-analysis incorporated four studies encompassing a total of 627 patients.The reporting bias of this study was deemed acceptable.RESULTS Compared to the Sham group,the uRDN group demonstrated a significant reduction in daytime ambulatory systolic BP(SBP)[mean difference(MD)-3.87 mmHg,95%confidence interval(CI):-7.02 to-0.73,P=0.02],office SBP(MD-4.13 mmHg,95%CI:-7.15 to-1.12,P=0.007),and home SBP(MD-5.51 mmHg,95%CI:-8.47 to-2.55,P<0.001).However,there was no statistically significant reduction observed in either 24-hour or nighttime ambulatory SBP levels.Subgroup analysis shows that uRDN can significantly reduce the SBP in patients with non-resistant HTN(MD-6.19 mmHg,MD-6.00 mmHg,MD-7.72 mmHg,MD-5.02 mmHg,MD-3.61 mmHg).CONCLUSION The current evidence suggests that uRDN may effectively reduce home,office,and daytime SBP in patients with HTN,particularly in those with non-resistant HTN.展开更多
Segmenting a breast ultrasound image is still challenging due to the presence of speckle noise,dependency on the operator,and the variation of image quality.This paper presents the UltraSegNet architecture that addres...Segmenting a breast ultrasound image is still challenging due to the presence of speckle noise,dependency on the operator,and the variation of image quality.This paper presents the UltraSegNet architecture that addresses these challenges through three key technical innovations:This work adds three things:(1)a changed ResNet-50 backbone with sequential 3×3 convolutions to keep fine anatomical details that are needed for finding lesion boundaries;(2)a computationally efficient regional attention mechanism that works on high-resolution features without using a transformer’s extra memory;and(3)an adaptive feature fusion strategy that changes local and global featuresbasedonhowthe image isbeing used.Extensive evaluation on two distinct datasets demonstrates UltraSegNet’s superior performance:On the BUSI dataset,it obtains a precision of 0.915,a recall of 0.908,and an F1 score of 0.911.In the UDAIT dataset,it achieves robust performance across the board,with a precision of 0.901 and recall of 0.894.Importantly,these improvements are achieved at clinically feasible computation times,taking 235 ms per image on standard GPU hardware.Notably,UltraSegNet does amazingly well on difficult small lesions(less than 10 mm),achieving a detection accuracy of 0.891.This is a huge improvement over traditional methods that have a hard time with small-scale features,as standard models can only achieve 0.63–0.71 accuracy.This improvement in small lesion detection is particularly crucial for early-stage breast cancer identification.Results from this work demonstrate that UltraSegNet can be practically deployable in clinical workflows to improve breast cancer screening accuracy.展开更多

Methods: Forty anesthesia resident companions who received standardized residency training in our department from July 2018 to July 2020 were randomly divided into an ultrasound group and a control group, with 20 participants in each group. The ultrasound group was taught by ultrasound-guided central venipuncture, while the control group was taught by traditional anatomy. After ten training punctures, all trainees were assessed twice. Results: Both groups could master the technique of central venipuncture. The success rate of first puncture and the overall success rate of puncture in the ultrasound group were significantly higher than those in the control group (p p < 0.05). Conclusion: Compared with the traditional anatomical localization teaching, the use of ultrasound-guided technology can improve the success rate of puncture, save puncture time, reduce related complications, and have a better training effect.展开更多
BACKGROUND Severe esophagogastric varices(EGVs)significantly affect prognosis of patients with hepatitis B because of the risk of life-threatening hemorrhage.Endoscopy is the gold standard for EGV detection but it is ...BACKGROUND Severe esophagogastric varices(EGVs)significantly affect prognosis of patients with hepatitis B because of the risk of life-threatening hemorrhage.Endoscopy is the gold standard for EGV detection but it is invasive,costly and carries risks.Noninvasive predictive models using ultrasound and serological markers are essential for identifying high-risk patients and optimizing endoscopy utilization.Machine learning(ML)offers a powerful approach to analyze complex clinical data and improve predictive accuracy.This study hypothesized that ML models,utilizing noninvasive ultrasound and serological markers,can accurately predict the risk of EGVs in hepatitis B patients,thereby improving clinical decisionmaking.AIM To construct and validate a noninvasive predictive model using ML for EGVs in hepatitis B patients.METHODS We retrospectively collected ultrasound and serological data from 310 eligible cases,randomly dividing them into training(80%)and validation(20%)groups.Eleven ML algorithms were used to build predictive models.The performance of the models was evaluated using the area under the curve and decision curve analysis.The best-performing model was further analyzed using SHapley Additive exPlanation to interpret feature importance.RESULTS Among the 310 patients,124 were identified as high-risk for EGVs.The extreme gradient boosting model demonstrated the best performance,achieving an area under the curve of 0.96 in the validation set.The model also exhibited high sensitivity(78%),specificity(94%),positive predictive value(84%),negative predictive value(88%),F1 score(83%),and overall accuracy(86%).The top four predictive variables were albumin,prothrombin time,portal vein flow velocity and spleen stiffness.A web-based version of the model was developed for clinical use,providing real-time predictions for high-risk patients.CONCLUSION We identified an efficient noninvasive predictive model using extreme gradient boosting for EGVs among hepatitis B patients.The model,presented as a web application,has potential for screening high-risk EGV patients and can aid clinicians in optimizing the use of endoscopy.展开更多
BACKGROUND This review evaluated the diagnostic effectiveness of various ultrasound(US)methods compared to liver biopsy.AIM To determine the diagnostic accuracy of US techniques in assessing liver fibrosis and steatos...BACKGROUND This review evaluated the diagnostic effectiveness of various ultrasound(US)methods compared to liver biopsy.AIM To determine the diagnostic accuracy of US techniques in assessing liver fibrosis and steatosis in adults,using the area under the receiver operating characteristic curve(AUROC)as the standard measure.METHODS The review included original retrospective or prospective studies published in the last three years in peer-reviewed medical journals,that reported AUROC values.Studies were identified through PubMed searches on January 3 and April 30,2024.Quality was assessed using the QUADAS-2 tool.Results were tabulated according to the diagnostic method and the type of liver pathology.RESULTS The review included 52 studies.For liver fibrosis detection,2D-shear wave elastography(SWE)AUROCs ranged from 0.54 to 0.994,showing better accuracy for advanced stages.Modifications,including 2D-SWE with propagation map guidance and supersonic imagine achieved AUROCs of 0.84 to nearly 1.0.point SWE and classical SWE had AUROCs of 0.741-0.99,and 0.507-0.995,respectively.Transient elastography(TE),visual TE,vibration-controlled TE(VCTE),and FibroTouch reported AUROCs close to 1.0.For steatosis,VCTE with controlled attenuation parameter showed AUROCs up to 0.89(for≥S1),acoustic radiation force impulse ranged from 0.762 to 0.784,US attenuation parameter from 0.88 to 0.93,and normalized local variance measurement from 0.583 to 0.875.Most studies had a low risk of bias across all or most domains,but evidence was limited by variability in study quality and small sample sizes.Innovative SWE variants were evaluated in a single study.CONCLUSION Modern US techniques can serve as effective noninvasive diagnostic tools for liver fibrosis and steatosis,with the potential to reduce the reliance on biopsies.展开更多
Background:This study evaluates the efficacy of gabexate mesylate thermosensitive in-situ gel(GMTI) in the treatment of beagle grade Ⅲ pancreatic trauma(PT) with the assistance of contrast-enhanced ultrasound(CEUS) a...Background:This study evaluates the efficacy of gabexate mesylate thermosensitive in-situ gel(GMTI) in the treatment of beagle grade Ⅲ pancreatic trauma(PT) with the assistance of contrast-enhanced ultrasound(CEUS) and investigates its mechanism of action.Methods:A grade Ⅲ PT model consisting of 15 beagle dogs with severed main pancreatic ducts was created and treated with cephalic vein injection of gabexate mesylate(GM)(1.54mL/10kg,TID) and peripancreatic injection of GMTI(4.63 mL/10 kg,QD) guided by CEUS within 24h post-surgery.Ascites and serum levels of amylase(AMY),lipase(LPS),C-reactive protein(CRP),interleukin(IL)-6,tumor necrosis factor(TNF)-α,and urinary trypsinogen activating peptide(TAP) were detected by ELISA.Histopathological changes in the canine pancreas were observed by Hematoxylin and Eosin staining.Results:CEUS accurately displayed pancreatic lesions and guided catheterisation.Compared to the control group,the ascites was significantly reduced after treatment(p<0.01).AMY and LPS ascites significantly decreased on post-operative 1st and 2nd day(p<0.01).The levels of AMY,LPS,CRP,IL-6,and TNF-α in serum were decreased(p<0.05 or p <0.01).Urinary TAP was decreased 1 and 2 days after treatment(p<0.05or p<0.01,respectively).In the control group,pancreatic tissue necrosis was evident in the wound area.Normal glandular cell structures and fibrous tissue hyperplasia were observed in the wound area after GMTI treatment.The GMTI group performed better than the GM group in improving pancreatic histology and reducing AMY levels in the early post-operative period.Conclusion:Guided by CEUS,daily peripancreatic injections of GMTI in Beagles effectively inhibit pancreatic enzyme activity and aid in the adjuvant treatment of pancreatic trauma.展开更多
In this article,we comment on the article by Cheng et al published in recently.Posthepatectomy liver failure(PHLF)remains a leading cause of hepatectomyrelated mortality and can be evaluated according to liver reserve...In this article,we comment on the article by Cheng et al published in recently.Posthepatectomy liver failure(PHLF)remains a leading cause of hepatectomyrelated mortality and can be evaluated according to liver reserve function.Liver stiffness(LS)measured by ultrasonic elastography and spleen area demonstrate a strong correlation with hepatic proliferation,fibrosis,and portal vein congestion,thus indirectly reflecting liver reserve function.This article highlights a meticulous study aimed at investigating the potential of two-dimensional shear wave elastography in assessing LS,as well as its integration with spleen area,surgical factors,and laboratory indicators,for predicting the risk of PHLF in hepatocellular carcinoma patients.In this article,we discussed the non-invasive elastography modality by ultrasound and magnetic resonance imaging to assess LS.Based on literature data,we subsequently suggested that several risk factors may contribute to PHLF,including excessive intrahepatic neutrophil accumulation,the formation of neutrophil extracellular traps,end-stage liver disease scores,and albumin-bilirubin scores.Therefore,a more comprehensive study incor-porating detailed information requires further investigation.展开更多

文摘Alzheimer’s disease is a common neurodegenerative disorder defined by decreased reasoning abilities,memory loss,and cognitive deterioration.The presence of the blood-brain barrier presents a major obstacle to the development of effective drug therapies for Alzheimer’s disease.The use of ultrasound as a novel physical modulation approach has garnered widespread attention in recent years.As a safe and feasible therapeutic and drug-delivery method,ultrasound has shown promise in improving cognitive deficits.This article provides a summary of the application of ultrasound technology for treating Alzheimer’s disease over the past 5 years,including standalone ultrasound treatment,ultrasound combined with microbubbles or drug therapy,and magnetic resonance imaging-guided focused ultrasound therapy.Emphasis is placed on the benefits of introducing these treatment methods and their potential mechanisms.We found that several ultrasound methods can open the blood-brain barrier and effectively alleviate amyloid-βplaque deposition.We believe that ultrasound is an effective therapy for Alzheimer’s disease,and this review provides a theoretical basis for future ultrasound treatment methods.
文摘Advancements in healthcare technology have improved mortality rates and extended lifespans,resulting in a population with multiple comorbidities that complicate patient care.Traditional assessments often fall short,underscoring the need for integrated care strategies.Among these,fluid management is particularly challenging due to the difficulty in directly assessing volume status especially in critically ill patients who frequently have peripheral oedema.Effective fluid ma-nagement is essential for optimal tissue oxygen delivery,which is crucial for cellular metabolism.Oxygen transport is dependent on arterial oxygen levels,haemoglobin concentration,and cardiac output,with the latter influenced by preload,afterload,and cardiac contractility.A delicate balance of these factors ensures that the cardiovascular system can respond adequately to varying ph-ysiological demands,thereby safeguarding tissue oxygenation and overall organ function during states of stress or illness.The Venous Excess Ultrasound(VExUS)Grading System is instrumental in evaluating fluid intolerance,providing detailed insights into venous congestion and fluid status.It was originally developed to assess the risk of acute kidney injury in postoperative cardiac patients,but its versatility has enabled broader applications in nephrology and critical care settings.This mini review explores VE×US’s application and its impact on fluid management and patient outcomes in critically ill patients.
文摘Conventional echocardiography can sometimes pose a challenge to diagnosis due to sub-optimal images.Ultrasound contrast agents(UCAs)have been shown to drastically enhance imaging quality,particularly depicting the left ventricular endocardial borders.Their use during echocardiography has become a valuable tool in non-invasive diagnostics.UCAs provide higher-quality images that may ultimately reduce the length of hospital stays and improve patient care.The higher cost associated with UCAs in many situations has been an impediment to frequent use.However,when used as an initial diagnostic test,UCA during rest echocardiogram is more cost-effective than the traditional diagnostic approach,which frequently includes multiple tests and imaging studies to make an accurate diagnosis.They can be easily performed across multiple patient settings and provide optimal images that allow clinicians to make sound medical decisions.This consequently allows for better diagnostic accuracies and improvement in patient care.

文摘As a non-invasive surgical procedure ultrasound cycloplasty(UCP)has gained attention among ophthalmologists in recent years.Derived from the application of highintensity focused ultrasound,it has been utilized for the treatment of various types of glaucoma,demonstrating notable efficacy and safety.This review focuses on the efficacy and safety of UCP in treating glaucoma among Asian populations.By summarizing and analyzing existing literature on indications,therapeutic outcomes,and safety profiles,this review further highlights the unique advantages of UCP in glaucoma treatment compared to traditional surgical approaches.These advantages include broader indications,non-invasive nature,quantifiable treatment,excellent intraocular pressure-lowering effects,fewer adverse reactions,and high safety.Additionally,by introducing the underlying mechanism of action,this review explores the factors influencing its therapeutic efficacy,providing theoretical insights for clinical practice and demonstrating UCP’s potential in glaucoma management.
文摘In this letter we comment on the article by Zhang et al published in the recent issue of the World Journal of Gastrointestinal Endoscopy 2024.We focus specifically on the management of gastric varices(GV),which is a significant consequence of portal hypertension,is currently advised to include beta-blocker therapy for primary prophylaxis and transjugular intrahepatic portosystemic shunt for secondary prophylaxis or active bleeding.Although it has been studied,direct endoscopic injection of cyanoacrylate glue has limitations,such as the inability to fully characterize GV endoscopically and the potential for distant glue embolism.In order to achieve this,endoscopic ultrasound has been used to support GV characterization,real-time therapy imaging,and Doppler obliteration verification.
文摘BACKGROUND Congenital cytomegalovirus(CMV)infection represents a significant public health concern as the most prevalent viral infection in newborns,potentially leading to severe neurological and developmental complications.The majority of cases are asymptomatic and remain undetected during pregnancy due to the absence of effective screening methods.CASE SUMMARY A 27-year-old primigravida presented for early pregnancy ultrasound,which revealed an atypical finding:A normal anechoic thalamus appearing hyperechoic on the mid-sagittal view of the fetal head.Subsequent ultrasound examinations during mid and late gestation demonstrated classic intracranial features sug-gestive of congenital CMV infection.Chromosomal karyotyping and microarray analysis of the fetus yielded no significant abnormalities.Following compre-hensive prenatal counseling regarding potential adverse fetal outcomes,the patient elected to continue her pregnancy.She ultimately underwent cesarean delivery at 42 weeks gestation at another facility,resulting in the birth of a female neonate.At five months of age,the infant presented with epilepsy and significant growth and developmental delays.CONCLUSION Congenital CMV infection occurs during the first trimester may manifest as hyperechoic thalamus which can be revealed by ultrasound in the mid-saggital view of the fetal head.Future research should investigate the correlation between echogenic thalamus and developmental outcomes,as well as explore early sc-reening techniques for suspected congenital CMV infection cases.
文摘BACKGROUND Adrenocortical oncocytoma is a rare,mostly benign,nonfunctional tumor that is typically detected incidentally.Its diagnosis is challenging because of the absence of distinctive imaging characteristics,necessitating pathological validation.CASE SUMMARY We present a case report of a 35-year-old woman with an adrenal mass located on the left side,where endoscopic ultrasound-guided fine-needle aspiration(EUSFNA)was performed after comprehensive diagnostic assessment.Our results are consistent with those of previously documented cases regarding tumor demographics and anatomical location.Given the limited number of reports on this condition,long-term follow-up is crucial to enhance our understanding of its prognosis.CONCLUSION For patients with adrenocortical oncocytoma,EUS-FNA can enables collection of preoperative tissue specimens leading to suitable treatment strategies.
文摘Background: Undergoing ultrasound scanning (USS) during the first trimester of pregnancy is highly imperative for expecting mothers, as it supports the early detection of any malformations, identifying the fetal number, fetal growth, fetal sex, and calculation of delivery. Previous studies have shown that undergoing such prenatal screening procedures could reduce the antenatal anxiety levels of expectant mothers. The present study aimed to explore the impact of first-trimester ultrasound scanning towards the antenatal anxiety and identify the predictors of antenatal anxiety among expectant mothers in the first trimester. Methods: A repeated measure design study was conducted in Maternity Clinics of University Hospital KDU, Ninewells Care Hospital and Navy General Hospital over 4 months with one hundred and fifteen (n = 115) expectant mothers. Participants completed a general information sheet first and State Trait Anxiety Inventory (STAI) (Spielberger et al., 1970) was administered before and after undergoing the USS. Results: Mean age of the participants was 28.84 ± 3.68. The Wilcoxon Signed Rank test showed that there is a significant reduction of participants’ antenatal anxiety levels following the USS z = −5.658, p Conclusions: Findings suggest that undergoing the first trimester USS significantly reduces the antenatal state anxiety and partner’s support is an important factor in reducing the antenatal anxiety experienced by expectant mothers in the first trimester. Future studies can focus on how USS can contribute to alleviating antenatal anxiety in second and third trimesters.
文摘Background: Wrist pain is prevalent. Activities such as dexterous sports, prolonged use of personal handheld devices, and extensive desktop keyboard usage are common contributors to wrist pain. Intersection syndrome, a form of inflammatory tenosynovitis, occurs at the intersection of the first and second dorsal compartments of the wrist. The first dorsal compartment is comprised of the tendons of abductor pollicis longus and extensor pollicis brevis, while the second dorsal compartment contains the tendons of extensor carpi radialis longus and extensor carpi radialis brevis. Intersection syndrome is diagnosed by pain localized to the dorsoradial forearm, approximately five cm proximal to the wrist joint, which worsens with resisted wrist and thumb extension. To date, the use of hydro dissection with 5% dextrose under ultrasound guidance as a treatment for Intersection syndrome has not been reported. This case report presents the first report on ultrasound-guided hydro dissection as a therapeutic approach for intersection syndrome. Methods: A case report, with informed consent, involving a 32-year-old male athlete. The patient, a hurling player, presented with chronic right wrist pain diagnosed as intersection syndrome. The condition significantly affected his work, sporting activities, and daily living activities. Previous conservative management and physiotherapy had failed to alleviate his symptoms. To confirm the diagnosis, relevant imaging was performed, supplemented by dynamic ultrasound assessment. The procedure was performed aseptically. Continuous ultrasound guidance was employed to ensure accurate needle placement. Once the needle tip position was confirmed, an initial injection of 5 mL of 0.25% chirocaine was administered. 10 mL of 5% dextrose was injected under ultrasound guidance for hydro dissection, with good visualization of the solution’s distribution. Conclusion: Ultrasound-guided hydro dissection has not previously been documented as a treatment option for intersection syndrome. In this case, it proved to be an effective pain-relieving therapy with sustained effect at three-month clinical follow-up. Further studies are required.

文摘Ultrasound-enabled nanomedicine leverages ultrasound to amplify the capabilities of engineered nanosystems,paving the way for innovative diagnostic and therapeutic breakthroughs in conventional nanomedicine.As a burgeoning discipline,past overviews have sometimes offered a fragmented perspective,lacking a comprehensive view.This review presents a systematic exploration of the latest advancements in ultrasound-enabled nanomedicine,with a particular emphasis on oncology.Covered topics include molecular imaging of tumors,separation of tumor markers,penetration through physiological barriers,perforation of cell membranes,targeted drug release and activation strategies,and an array of sonotherapies for oncological treatments.We delve into the research framework of each topic,the foundational design of the nanosystems,and their associated ultrasound activation mechanisms.Moreover,we highlight recent pivotal research aimed at deepening the reader’s understanding of this intricate domain.This review underscores the integration of design and foundational theories within ultrasound-enabled nanomedicine,aspiring to ignite advanced theoretical insights and introduce innovative design paradigms.In conclusion,we outline current challenges and prospective research directions.An enhanced focus on these areas will expedite the advancement of ultrasound-enabled nanomedicine.

文摘BACKGROUND Hypertension(HTN)is a prevalent chronic health condition that significantly increases the risk of cardiovascular diseases-associated mortalities.Despite the use of antihypertensive medications,numerous patients fail to achieve guidelinerecommended blood pressure(BP)targets.AIM To evaluates the efficacy of catheter-based ultrasound renal denervation(uRDN)for the treatment of HTN.METHODS Relevant studies were identified through searches in PubMed,Embase,the Cochrane Library,Web of Science,and China National Knowledge Infrastructure,with a cut-off date at April 1,2024.A random-effects model was employed in this study to mitigate potential biases.The risk of bias for included studies was assessed using the Cochrane Risk of Bias assessment tool.Statistical analyses were conducted using Review Manager version 5.3.This meta-analysis incorporated four studies encompassing a total of 627 patients.The reporting bias of this study was deemed acceptable.RESULTS Compared to the Sham group,the uRDN group demonstrated a significant reduction in daytime ambulatory systolic BP(SBP)[mean difference(MD)-3.87 mmHg,95%confidence interval(CI):-7.02 to-0.73,P=0.02],office SBP(MD-4.13 mmHg,95%CI:-7.15 to-1.12,P=0.007),and home SBP(MD-5.51 mmHg,95%CI:-8.47 to-2.55,P<0.001).However,there was no statistically significant reduction observed in either 24-hour or nighttime ambulatory SBP levels.Subgroup analysis shows that uRDN can significantly reduce the SBP in patients with non-resistant HTN(MD-6.19 mmHg,MD-6.00 mmHg,MD-7.72 mmHg,MD-5.02 mmHg,MD-3.61 mmHg).CONCLUSION The current evidence suggests that uRDN may effectively reduce home,office,and daytime SBP in patients with HTN,particularly in those with non-resistant HTN.

文摘Segmenting a breast ultrasound image is still challenging due to the presence of speckle noise,dependency on the operator,and the variation of image quality.This paper presents the UltraSegNet architecture that addresses these challenges through three key technical innovations:This work adds three things:(1)a changed ResNet-50 backbone with sequential 3×3 convolutions to keep fine anatomical details that are needed for finding lesion boundaries;(2)a computationally efficient regional attention mechanism that works on high-resolution features without using a transformer’s extra memory;and(3)an adaptive feature fusion strategy that changes local and global featuresbasedonhowthe image isbeing used.Extensive evaluation on two distinct datasets demonstrates UltraSegNet’s superior performance:On the BUSI dataset,it obtains a precision of 0.915,a recall of 0.908,and an F1 score of 0.911.In the UDAIT dataset,it achieves robust performance across the board,with a precision of 0.901 and recall of 0.894.Importantly,these improvements are achieved at clinically feasible computation times,taking 235 ms per image on standard GPU hardware.Notably,UltraSegNet does amazingly well on difficult small lesions(less than 10 mm),achieving a detection accuracy of 0.891.This is a huge improvement over traditional methods that have a hard time with small-scale features,as standard models can only achieve 0.63–0.71 accuracy.This improvement in small lesion detection is particularly crucial for early-stage breast cancer identification.Results from this work demonstrate that UltraSegNet can be practically deployable in clinical workflows to improve breast cancer screening accuracy.

文摘Vision restoration presents a considerable challenge in the realm of regenerative medicine,while recent progress in ultrasound stimulation has displayed potential as a non-invasive therapeutic approach.This narrative review offers a comprehensive overview of current research on ultrasound-stimulated neuromodulation,emphasizing its potential as a treatment modality for various nerve injuries.By examining of the efficacy of different types of ultrasound stimulation in modulating peripheral and optic nerves,we can delve into their underlying molecular mechanisms.Furthermore,the review underscores the potential of sonogenetics in vision restoration,which involves leveraging pharmacological and genetic manipulations to inhibit or enhance the expression of related mechanosensitive channels,thereby modulating the strength of the ultrasound response.We also address how methods such as viral transcription can be utilized to render specific neurons or organs highly responsive to ultrasound,leading to significantly improved therapeutic outcomes.

文摘Low-intensity ultrasound was applied to the pressureless consolidation of AlSi10Mg powders in a broad temperature range from 600 to 860℃.Under static conditions,the consolidation of AlSi10Mg powders can only be achieved at 860℃,but still with the presence of some residual unconsolidated regions.The introduction of low-intensity ultrasound at this temperature eliminates the unconsolidated regions and transforms the columnar grains observed in original directional solidification into equiaxed or globular grains.Remarkably,the application of low-intensity ultrasound significantly reduces the consolidation temperature to 620℃,without compromising the microhardness of the resulting samples when compared to static conditions.Furthermore,by lowering the temperature to 600℃,a well-sintered porous material is obtained through the assistance of the low-intensity ultrasound.
文摘Objective: To evaluate the application effect of ultrasound-guided central venous catheterization in the teaching of anesthesia residents. Methods: Forty anesthesia resident companions who received standardized residency training in our department from July 2018 to July 2020 were randomly divided into an ultrasound group and a control group, with 20 participants in each group. The ultrasound group was taught by ultrasound-guided central venipuncture, while the control group was taught by traditional anatomy. After ten training punctures, all trainees were assessed twice. Results: Both groups could master the technique of central venipuncture. The success rate of first puncture and the overall success rate of puncture in the ultrasound group were significantly higher than those in the control group (p p < 0.05). Conclusion: Compared with the traditional anatomical localization teaching, the use of ultrasound-guided technology can improve the success rate of puncture, save puncture time, reduce related complications, and have a better training effect.

文摘BACKGROUND Severe esophagogastric varices(EGVs)significantly affect prognosis of patients with hepatitis B because of the risk of life-threatening hemorrhage.Endoscopy is the gold standard for EGV detection but it is invasive,costly and carries risks.Noninvasive predictive models using ultrasound and serological markers are essential for identifying high-risk patients and optimizing endoscopy utilization.Machine learning(ML)offers a powerful approach to analyze complex clinical data and improve predictive accuracy.This study hypothesized that ML models,utilizing noninvasive ultrasound and serological markers,can accurately predict the risk of EGVs in hepatitis B patients,thereby improving clinical decisionmaking.AIM To construct and validate a noninvasive predictive model using ML for EGVs in hepatitis B patients.METHODS We retrospectively collected ultrasound and serological data from 310 eligible cases,randomly dividing them into training(80%)and validation(20%)groups.Eleven ML algorithms were used to build predictive models.The performance of the models was evaluated using the area under the curve and decision curve analysis.The best-performing model was further analyzed using SHapley Additive exPlanation to interpret feature importance.RESULTS Among the 310 patients,124 were identified as high-risk for EGVs.The extreme gradient boosting model demonstrated the best performance,achieving an area under the curve of 0.96 in the validation set.The model also exhibited high sensitivity(78%),specificity(94%),positive predictive value(84%),negative predictive value(88%),F1 score(83%),and overall accuracy(86%).The top four predictive variables were albumin,prothrombin time,portal vein flow velocity and spleen stiffness.A web-based version of the model was developed for clinical use,providing real-time predictions for high-risk patients.CONCLUSION We identified an efficient noninvasive predictive model using extreme gradient boosting for EGVs among hepatitis B patients.The model,presented as a web application,has potential for screening high-risk EGV patients and can aid clinicians in optimizing the use of endoscopy.

文摘BACKGROUND This review evaluated the diagnostic effectiveness of various ultrasound(US)methods compared to liver biopsy.AIM To determine the diagnostic accuracy of US techniques in assessing liver fibrosis and steatosis in adults,using the area under the receiver operating characteristic curve(AUROC)as the standard measure.METHODS The review included original retrospective or prospective studies published in the last three years in peer-reviewed medical journals,that reported AUROC values.Studies were identified through PubMed searches on January 3 and April 30,2024.Quality was assessed using the QUADAS-2 tool.Results were tabulated according to the diagnostic method and the type of liver pathology.RESULTS The review included 52 studies.For liver fibrosis detection,2D-shear wave elastography(SWE)AUROCs ranged from 0.54 to 0.994,showing better accuracy for advanced stages.Modifications,including 2D-SWE with propagation map guidance and supersonic imagine achieved AUROCs of 0.84 to nearly 1.0.point SWE and classical SWE had AUROCs of 0.741-0.99,and 0.507-0.995,respectively.Transient elastography(TE),visual TE,vibration-controlled TE(VCTE),and FibroTouch reported AUROCs close to 1.0.For steatosis,VCTE with controlled attenuation parameter showed AUROCs up to 0.89(for≥S1),acoustic radiation force impulse ranged from 0.762 to 0.784,US attenuation parameter from 0.88 to 0.93,and normalized local variance measurement from 0.583 to 0.875.Most studies had a low risk of bias across all or most domains,but evidence was limited by variability in study quality and small sample sizes.Innovative SWE variants were evaluated in a single study.CONCLUSION Modern US techniques can serve as effective noninvasive diagnostic tools for liver fibrosis and steatosis,with the potential to reduce the reliance on biopsies.
文摘Background:This study evaluates the efficacy of gabexate mesylate thermosensitive in-situ gel(GMTI) in the treatment of beagle grade Ⅲ pancreatic trauma(PT) with the assistance of contrast-enhanced ultrasound(CEUS) and investigates its mechanism of action.Methods:A grade Ⅲ PT model consisting of 15 beagle dogs with severed main pancreatic ducts was created and treated with cephalic vein injection of gabexate mesylate(GM)(1.54mL/10kg,TID) and peripancreatic injection of GMTI(4.63 mL/10 kg,QD) guided by CEUS within 24h post-surgery.Ascites and serum levels of amylase(AMY),lipase(LPS),C-reactive protein(CRP),interleukin(IL)-6,tumor necrosis factor(TNF)-α,and urinary trypsinogen activating peptide(TAP) were detected by ELISA.Histopathological changes in the canine pancreas were observed by Hematoxylin and Eosin staining.Results:CEUS accurately displayed pancreatic lesions and guided catheterisation.Compared to the control group,the ascites was significantly reduced after treatment(p<0.01).AMY and LPS ascites significantly decreased on post-operative 1st and 2nd day(p<0.01).The levels of AMY,LPS,CRP,IL-6,and TNF-α in serum were decreased(p<0.05 or p <0.01).Urinary TAP was decreased 1 and 2 days after treatment(p<0.05or p<0.01,respectively).In the control group,pancreatic tissue necrosis was evident in the wound area.Normal glandular cell structures and fibrous tissue hyperplasia were observed in the wound area after GMTI treatment.The GMTI group performed better than the GM group in improving pancreatic histology and reducing AMY levels in the early post-operative period.Conclusion:Guided by CEUS,daily peripancreatic injections of GMTI in Beagles effectively inhibit pancreatic enzyme activity and aid in the adjuvant treatment of pancreatic trauma.

文摘In this article,we comment on the article by Cheng et al published in recently.Posthepatectomy liver failure(PHLF)remains a leading cause of hepatectomyrelated mortality and can be evaluated according to liver reserve function.Liver stiffness(LS)measured by ultrasonic elastography and spleen area demonstrate a strong correlation with hepatic proliferation,fibrosis,and portal vein congestion,thus indirectly reflecting liver reserve function.This article highlights a meticulous study aimed at investigating the potential of two-dimensional shear wave elastography in assessing LS,as well as its integration with spleen area,surgical factors,and laboratory indicators,for predicting the risk of PHLF in hepatocellular carcinoma patients.In this article,we discussed the non-invasive elastography modality by ultrasound and magnetic resonance imaging to assess LS.Based on literature data,we subsequently suggested that several risk factors may contribute to PHLF,including excessive intrahepatic neutrophil accumulation,the formation of neutrophil extracellular traps,end-stage liver disease scores,and albumin-bilirubin scores.Therefore,a more comprehensive study incor-porating detailed information requires further investigation.

文摘In this article,we discuss the study by Cheng et al,published in the World Journal of Gastroenterology,focusing on predictive methods for post-hepatectomy liver failure(PHLF).PHLF is a common and serious complication,and accurate prediction is critical for clinical management.The study examines the potential of ultrasound elastography and splenic size in predicting PHLF.Ultrasound elastography reflects liver functional reserve,while splenic size provides additional predictive value.By integrating these factors with serological markers,we developed a comprehensive prediction model that effectively stratifies patient risk and supports personalized clinical decisions.This approach offers new insights into predicting PHLF.These methods not only assist clinicians in identifying high-risk patients earlier but also provide scientific support for personalized treatment strategies.Future research will aim to validate the model's accuracy with larger sample sizes,further enhancing the clinical application of these non-invasive indicators.
